Adolescent growth depends on impulse control, which impacts wellbeing, social interactions and academic achievement. The purpose of this narrative review is to assess the efficacy of therapeutic and educational initiatives aimed at enhancing impulse control during this crucial developmental phase. The review highlights various successful interventions that can be used to improve impulse control in adolescents. These interventions include self-regulation strategies, serious games, mindfulness practices, cognitive-behavioural approaches and biopsychosocial workshops. To support comprehensive adolescent development, future research should focus on long-term outcomes and the integration of these interventions into broader educational and therapeutic frameworks.

Autoimmune diseases are now well reported with COVID vaccination. Rheumatological, neurological, cardiac, and hematological adverse events are observed globally. Dermatological adverse events are reported in a few cases but dermatological with rheumatological manifestation with reversible autoimmune phenomenon after COVID vaccination is not reported commonly in literature. “Mechanic’s hands” is blackish discoloration of palmar aspects of both hands and knuckles commonly reported in rheumatological autoimmune condition such as dermatomyositis. In this case report, a 28-year-old male presented with polyarthralgia with myalgia and fatigability after 1 month of COVID vaccination. His joint pain symptoms worsened over 4 weeks and developed blackish pigmentation of both hands predominantly involving palms to present as mechanic’s hands. Vasculitis workup was inconclusive and rheumatologic workup documented raised antinuclear antibody titers and rheumatoid factor. We have started him on steroid and hydroxychloroquine and observed satisfactory clinical response with near complete resolution of “mechanic’s hands” in 24 weeks. Mechanic’s hands with rheumatological syndrome which is a rare vaccine-related adverse event, reversible, and easily treatable with routinely available medicines, and importantly it is having excellent prognosis. Mechanic’s hand is first time reported in medical literature after COVID vaccination. Minimal systemic adverse events are known to occur with all viral vector vaccines, but its occurrence is rare and it should not impact on routine vaccinations, as vaccination is a key step in this pandemic to protect humankind.

Nutritional policies center around supporting breastfeeding and ensuring access to nutritious food, laying the foundation for a healthy start in life. Early childhood education programs and parental education initiatives are crucial components, emphasizing cognitive, social, and emotional development from the earliest stages. The review underscores the importance of a holistic perspective, urging the integration of services across sectors to provide a comprehensive framework for ECD. Research-based policies, continuous monitoring, and evaluation mechanisms ensure evidence-driven decision-making and ongoing refinement of strategies. Additionally, the role of legislation and advocacy is highlighted in protecting the rights and well-being of children and advocating for ECD at various levels. In the end, this review highlights the importance of ECD guidelines and health policies as investments that not only help individual children but also enhance societal productivity and general well-being. By prioritizing the early years, policymakers can lay a strong foundation for future health, learning, and success.

Fibrous histiocytoma is a benign soft-tissue neoplasm that commonly involves the dermis. It is rare in the oral cavity. This lesion creates a diagnostic dilemma due to its nonspecific clinical appearance and microscopic similarities with other benign fibrous tissue lesions. Microscopically, it shows the proliferation of both spindle-shaped fibroblasts and histiocytes. Surgical excision is the treatment of choice with a good prognosis. The purpose of this paper is to report a rare case of gingival benign fibrous histiocytoma occurring in an 11-year-old female patient. The diagnosis of this lesion is based on combined clinical, histopathological, and immunohistochemical features.

BACKGROUND: Competency-based medical education (CBME) is being implemented across India in medical colleges from the 2019 batch. Several new components are introduced which require focused faculty training and handholding at times. This study was conducted to explore the challenges faced by faculty and students of Phase 1 and Phase 2 MBBS during the implementation of a competency-based undergraduate curriculum with regard to curriculum planning and delivery, teaching–learning, and assessment; to determine the perceived/proposed solutions by the faculty and students to overcome the said challenges. METHODS: An exploratory qualitative study was done at the Institute of Postgraduate Medical Education and Research, Kolkata among the faculty members and students of first and second phase of the MBBS CBME batch. This study was done over 10 months. Nonprobability purposive sampling was employed in the study. Free listing was done initially to elicit the views of faculty members to meet the intended objectives. Visual Anthropac software was used to identify the salient variables using Smith’s Salience Score. RESULTS: Twenty faculty members participated in the free listing and pile sorting. Moreover, total of 18 students also participated in the focus group discussion. A total of 20 responses were obtained about the key aspects of CBME, of which 15 were identified as the salient variables depending on the cutoff value of 0.15 (Smith’s Salience Score) and subjected to pile sorting. Similarly, 13 challenges were identified in the implementation of the program during the free listing, and all were included in the second stage of pile sorting. CONCLUSION: The majority of the responses by faculty and students being positive indicate that CBME is the need of the decade. Organizing orientation/sensitization sessions, following timetables, daily feedback, preparing the universal assessment tool, and emphasizing attitudes, ethics, and communication through a case-based module was the proposed solution.

Objective: To compare the efficacy and safety of a fixed-dose combination of aspirin and pantoprazole with that of aspirin alone for the prevention of gastro duodenal mucosal damage in patients taking aspirin for secondary prevention of cardiovascular disease or cerebrovascular disease. Methods: This was a comparative, double-blind, double-dummy, randomized, multicenter, phase III study conducted in patients taking aspirin ≤150 mg daily for ≥3 to ≤6 months and expected to require daily aspirin therapy for at least 6 months for the secondary prevention of cardiovascular disease or cerebrovascular disease. Results: A total of 240 patients were randomized to receive either a fixed-dose combination of aspirin 150 mg and pantoprazole 20 mg or aspirin 150 mg alone in a 2:1 ratio. The proportion of non-responders (patients experiencing gastroduodenal events) was 9.7 % in the test group (fixed-dose combination of aspirin 150 mg and pantoprazole 20 mg) compared to 19.7 % in the comparator group (aspirin 150 mg) at week 12, while the proportions were 11.0 % in the test group and 22.4 % in the comparator group at the end of 24 weeks of treatment (p-value was

The shock wave boundary layer interaction (SWBLI) due to curved shock is studied prominently, as most military aircrafts and missiles undergo strong SWTBLI due to its curved or blunt shape. These interactions effect the performance of vehicles at supersonic flow regime. The present numerical study investigates, interaction between a shock wave and turbulent boundary layer (SWTBLI) for cylindrical shock generators at freestream Mach number 3. The diameter (D) of cylindrical shock generator is varied to determine the effect of strength of shock on boundary layer and effect on impinging location from leading edge of flat plate, which depends upon the position of cylindrical shock generator. Two dimensional numerical simulations are carried out on mentioned model using commercially available CFD solver that employs k-omega SST turbulence model. Computational results show a good agreement qualitatively in terms of separation location and separation bubble length and quantitatively predicts the surface pressure, accurately as compared to the experiments conducted by literature.

Introduction: Dental caries results from an ecologic shift within the dental biofilm from a balanced population of microorganisms to an acidogenic, aciduric, and cariogenic microbiological population developed and maintained by frequent consumption of fermentable dietary carbohydrates. Total caries removal (TCR) of deep lesion may result in pulpal exposure requiring more invasive treatment. Hence, current pediatric dentistry has shifted to minimally invasive treatment that avoids more complex, time-consuming procedure, and the child’s discomfort. Aim: The aim of this study is to evaluate and compare clinical performance and radiographic changes after complete and incomplete caries removal procedures. Materials and Methods: The study was conducted on 60 primary molars in children aged 6–9 years. Selected 60 primary molars were randomly divided into two groups. Group 1 (PCR): infected dentin was removed, while the affected dentin was maintained on the pulpal wall. Group 2 (TCR): both infected and affected dentin were removed through low-speed carbide bur and hand excavator. Teeth were evaluated at 4 and 6 months clinically and radiographically. Results: The proportion was compared using Fisher’s exact test. The Statistical Package for the Social Sciences version 21 was used for analysis. The level of significance was kept at 5%. Conclusion: The clinical and radiographic success rates of ICR and CCR in primary teeth with deep carious lesions were high and did not differ significantly, indicating that the retention of carious dentin does not interfere with pulp vitality. Thus, ICR is a reliable minimally invasive approach that might replace the CCR in primary teeth when correctly indicated.

Context: Medical colleges regularly examine mass drug administration (MDA) programs to determine their effectiveness and pinpoint areas for development. These analyses frequently show a discrepancy between the program’s coverage and actual drug use. This study was conducted in the Chamorshi and Armori blocks of the Gadchiroli District in Maharashtra. Aim: This study aimed to find out the coverage and compliance of MDA in the Chamorshi and Armori blocks of Gadchiroli, Maharashtra. Settings and Design: This study was a cross-sectional study. Materials and Methods: Thirty houses from each of the three randomly chosen villages and a ward were surveyed in each implementing unit. Thus, a total of 120 households’ worth of data were gathered and examined. Information was gathered from each individual in the chosen home at the individual level. Statistical Analysis Used: Data were collected and duly filled out on questionnaire forms, which were then entered in Microsoft (MS) Excel. Statistical Package for the Social Sciences (SPSS) version 16.0 (Chicago) was used as a statistical tool in the analysis. Results: Consumption rates in rural areas were much higher than in urban areas. A minimum consumption rate was found in the 2- to 5-year-old age range. Conclusions: In this study, the percentage of coverage and compliance rates was 89.5% and 99.4% and 90.81% and 99.2% for the Armori and Chamorshi blocks, respectively. In comparison, coverage was better in rural areas than in urban areas. The MDA program must therefore be significantly strengthened in urban areas, particularly by guaranteeing improved compliance through monitored drug consumption.

Abstract This cross-sectional study investigated differences in the plasma metabolome in two groups of adults that were of similar age but varied markedly in body composition and dietary and physical activity patterns. Study participants included 52 adults in the lifestyle group (LIFE) (28 males, 24 females) and 52 in the control group (CON) (27 males, 25 females). The results using an extensive untargeted ultra high-performance liquid chromatography-high resolution mass spectrometry (UHPLC-HRMS) metabolomics analysis with 10,535 metabolite peaks identified 486 important metabolites (variable influence on projections scores of VIP ≥ 1) and 16 significantly enriched metabolic pathways that differentiated LIFE and CON groups. A novel metabolite signature of positive lifestyle habits emerged from this analysis highlighted by lower plasma levels of numerous bile acids, an amino acid profile characterized by higher histidine and lower glutamic acid, glutamine, β-alanine, phenylalanine, tyrosine, and proline, an elevated vitamin D status, higher levels of beneficial fatty acids and gut microbiome catabolism metabolites from plant substrates, and reduced levels of N-glycan degradation metabolites and environmental contaminants. This study established that the plasma metabolome is strongly associated with body composition and lifestyle habits. The robust lifestyle metabolite signature identified in this study is consistent with an improved life expectancy and a reduced risk for chronic disease.

Introduction: In the rapidly evolving landscape of health care, artificial intelligence (AI) has emerged as a promising tool to enhance diagnostic accuracy across various medical disciplines. Within the realm of dentistry, one critical area of focus is endodontics, which involves the diagnosis and treatment of dental pulp diseases. This systematic review investigates the diagnostic precision of AI in contrast to dental experts when predicting endodontic outcomes. Materials and Methods: The review was conducted based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. The review was registered on the Open Science Framework database. A systematic search was performed of the literature on the application of AI versus dental experts in endodontics. The search was conducted on PubMed, Scopus, Web of Science, and OVID until December 2022. The QUADAS-2 tool was used to evaluate the risk of bias. Results: The initial search retrieved 972 records, of which eight articles were included in the study. The studies reported application of AI for working length determination, periapical lesions, pulp and tooth segmentation, apical periodontitis, vertical root fracture, and C-shaped canals. QUADAS-2 tool revealed a low risk of bias in five out of the eight included studies. Conclusion: AI models demonstrated a notable increase in accuracy and effectiveness in endodontic diagnosis and treatment planning. These results suggest that the integration of AI technology in endodontic diagnosis and treatment planning has immense potential to serve as a promising aid.

This study explores the complex dynamics of magnetohydrodynamic (MHD) Casson fluid in a non-uniform rough channel, focusing on the effects of temperature-dependent viscosity and variable thermal conductivity under no-slip boundary conditions. The study employs an innovative approach by utilising a rough surface with irregular textures to analyse flow patterns and assess drag forces on channel objects. A novel mathematical model, governed by continuity, momentum, and heat transfer equations, is developed and transformed into dimensionless, nonlinear Ordinary Differential Equations (ODEs) using non-dimensional quantities and fundamental assumptions. The Optimal Homotopy Analysis Method (OHAM) is applied to solve these equations to enhance convergence speed and accuracy. The research explores the impact of surface roughness on velocity profiles and temperature distributions under various physical constraints. Numerical simulations are conducted to determine skin friction coefficients and Nusselt numbers. Furthermore, the study examines the influence of confined boluses on fluid flow in diverse physiological conditions. A comprehensive analysis is performed to elucidate the combined effects of surface roughness on fluid passage, including flow separation, pattern alterations, pressure distribution and drop, heat transfer characteristics, and flow resistance. The intricate interplay between temperature-dependent viscosity, varying thermal conductivity, and surface roughness is thoroughly investigated to explain the complex dynamics of MHD Casson fluid movement in non-uniform channels. Implementing a magnetic field over the rough, non-uniform channel is found to provide stability and prevent fluid overflow. This research has significant real-world applications, including soil erosion prevention, blood flow regulation in arteries, and optimisation of hydropower channels and penstocks. By enhancing our understanding of flow dynamics through rough and non-uniform channels, this study contributes valuable insights into both theoretical fluid mechanics and practical engineering applications.

The current diagnostic agronomy study of the Bankura region of West Bengal, India, examined the variations in crop yields through a socio-ecological analysis of multiple production system components. Envirotyping for root cause analysis was employed to delve into the variables that affect the performance of rainfed production systems. Mother Earth, man, machine, management, and materials (5Ms concept) were the five indicators under which the variables were grouped. Findings demonstrated the fragility of the region’s soils due to its undulating terrain, unpredictable rainfall patterns, and frequent drought scenarios. The LULC’s NDVI showed that the agricultural area is about 60% and 43% of the total geographical area in the Hirbandh and Ranibandh blocks, respectively. Soils are acidic and diagnosed with deficiency of both macro and micronutrients (phosphorous, sulfur, and boron) having poor water holding capacity (35 to 55 mm for a 50 cm soil depth). The sand and soil organic carbon contents ranged between 43.04%–82.32% and 0.17%–1.01%, respectively with a low bacterial population. These factors are the root cause for low cropping intensity (106%) and low paddy productivity (3,021 kg/ha). Overall, the study contributes to designing and scaling-up of sustainable landscape management practices that could ensure higher cropping intensity and system productivity in similar agro-ecologies with limited evidence.

Magnetized non-Newtonian models nowadays have attracted many researchers because it is an exciting field in the collaboration of material science, fluid dynamics, and applied physics. Their unique properties and adaptability render them invaluable across various technological and industrial applications, promising further innovations as research advances. This research unveils the intricate rheological and thermal behavior of magnetized non-Newtonian nanofluids undergoing peristaltic motion. The study aims to enhance engineering design techniques for optimal biophysiological performance by incorporating second-order slip, convective conditions, and temperature-dependent thermal conductivity. The Buongiorno nanofluid model is adopted to investigate heat and mass transfer phenomena, while the Prandtl non-Newtonian fluid model is employed to comprehend the complex rheological characteristics of the fluid. A long-wavelength approximation with a low Reynolds number was employed to simplify the governing equations. Analytical solutions have been obtained by solving the nonlinear transformed equations using the Homotopy perturbation technique. The findings are validated with previous literature and indicate that magnetic fields play a key role in controlling peristaltic flow behavior and nanofluid pumping rates. Moreover, the interplay between non-Newtonian rheology and nanofluid parameters significantly affects temperature distribution patterns. An increase in the species Biot number and thermophoresis parameter leads to improved concentration behavior. Conversely, a reversible trend is noted with the augmentation of the Prandtl number, Eckart number, variable thermal conductivity, and Brownian motion parameters. This research provides new insights into magnetohydrodynamic transport mechanisms in peristaltic systems. The modeling approach, coupled with analysis, lays the background for improved fluid circulation, oxygen delivery, waste removal, and nutrient transport in biomedical applications. Specifically, the findings are important for advancing the design of peristaltic pumps tailored for targeted drug delivery and optimizing fluid flow within gastrointestinal tracts.

Separation and quantification of highly polar metformin (MTF) alone or in combination with dipeptidyl peptidase-4 inhibitors, such as evogliptin (EVG), sitagliptin, saxagliptin, vildagliptin, linagliptin and teneligliptin, specifically from human plasma and pharmaceutical products is still difficult in mostly preferred ODS based RP-HPLC techniques. Since, owing to weak retention of MTF in ODS, it elutes together with the biological fluid components and drug excipients. Therefore, in this study, alternative to ODS based RP-HPLC, comparatively less known analytical techniques like strong cation exchange chromatography (SCX-3), hydrophilic interaction liquid chromatography (HILIC) and mix-mode chromatography (MMC) were comprehensively evaluated for their potentials and limitations in simultaneous quantification of newly approved EVG and MTF combination (Valera-M). In results, prolonged application of SCX-3, exhibits irreproducible and irreversible retention of MTF and EVG. While in HILIC with Cyano column, excessive acetonitrile as eluent developed the precipitation of MTF and column back pressure. Comparatively, the Acclaimed® mix-mode HILIC-1, demonstrated much promising and reproducible results for MTF and EVG separation and importantly, it can be used either reverse phase or HILIC mode. Considering the overall benefits of Acclaimed® mix-mode HILIC-1, it was used in estimation of MTF and EVG from human plasma and pharmaceutical formulations.
