1. Spatial dynamics of tertiary lymphoid aggregates in head and neck cancer: insights into immunotherapy response

Abstract

Abstract Background Recurrent/metastatic head and neck squamous cell carcinoma (R/M HNSCC) generally has a poor prognosis for patients with limited treatment options. While incorporating immune checkpoint inhibitors (ICIs) has now become the standard of care, the efficacy is variable, with only a subset of patients responding. The complexity of the tumor microenvironment (TME) and the role of tertiary lymphoid structures (TLS) have emerged as critical determinants for immunotherapeutic response. Methods In this study, we analyzed two independently collected R/M HNSCC patient tissue cohorts to better understand the role of TLS in response to ICIs. Utilizing a multi-omics approach, we first performed targeted proteomic profiling using the Nanostring GeoMx Digital Spatial Profiler to quantify immune-related protein expression with spatial resolution. This was further characterized by spatially resolved whole transcriptome profiling of TLSs and germinal centers (GCs). Deeper single-cell resolved proteomic profiling of the TLSs was performed using the Akoya Biosciences Phenocycler Fusion platform. Results Our proteomic analysis revealed the presence of T lymphocyte markers, including CD3, CD45, and CD8, expressing cells and upregulation of immune checkpoint marker PD-L1 within tumor compartments of patients responsive to ICIs, indicative of ‘hot tumor’ phenotypes. We also observed the presence of antigen-presenting cells marked by expression of CD40, CD68, CD11c, and CD163 with upregulation of antigen-presentation marker HLA-DR, in patients responding to ICIs. Transcriptome analysis of TLS and GCs uncovered a marked elevation in the expression of genes related to immune modulation, diverse immune cell recruitment, and a potent interferon response within the TLS structure. Notably, the distribution of TLS-tumor distance was found to be significantly different across response groups (H = 9.28, p = 0.026). The proximity of TLSs to tumor cells was found to be a critical indicator of ICI response, implying that patients with TLSs located further from tumor cells have worse outcomes. Conclusion The study underscores the multifaceted role of TLSs in modulating the immunogenic landscape of the TME in R/M HNSCC, likely influencing the efficacy of ICIs. Spatially resolved multi-omics approaches offer valuable insights into potential biomarkers for ICI response and highlight the importance of profiling the TME complexity when developing therapeutic strategies and patient stratification.

2. Timing of tertiary trauma surveys during a time of increased trauma presentations – The Alice Springs Hospital Finke Desert Race experience

Abstract

Purpose: The Finke Desert Race is an offroad motorbike and buggy race held annually in central Australia. Owing to the treacherous conditions, this race sees a significant influx of trauma presentations to Alice Springs Hospital, the closest rural hospital. Completion of a tertiary trauma survey (TTS) within 24 hours of a patient's admission is part of standard trauma management. Method: A retrospective analysis was undertaken of trauma presentations managed by general surgery over a 5-day period of the Finke Desert Race weekend, compared to a 3-month control period from February to April of the same year. To be included, patients met the criteria for completion of a TTS. Results: The total number of trauma presentations over the 5-day period of the race weekend was 18 (an incidence rate of 3.6 cases/day), compared to a total of 31 in the 3-month control period (an incidence rate of 0.36 cases/day). The daily rate of major trauma presentations during the Finke race weekend was 9.9 times greater than during the control period. Completion of TTS was missed in only 5.6 % of patients over the Finke weekend, compared to 14.3 % of patients in the control period. The median time from presentation to the emergency department to completion of TTS during the Finke weekend was 20 h 19 min, compared to 20 h 36 min during the control period. Conclusion: Despite the substantial influx of trauma during the race weekend, fewer patients missed having a TTS completed compared to the control period. The median time taken to completion of TTS was similar between the two time periods. These findings suggest that the general surgery department was able to maintain standard trauma management principles.

3. Evaluating the generalizability of deep learning image classification algorithms to detect middle ear disease using otoscopy

Abstract To evaluate the generalizability of artificial intelligence (AI) algorithms that use deep learning methods to identify middle ear disease from otoscopic images, between internal to external performance. 1842 otoscopic images were collected from three independent sources: (a) Van, Turkey, (b) Santiago, Chile, and (c) Ohio, USA. Diagnostic categories consisted of (i) normal or (ii) abnormal. Deep learning methods were used to develop models to evaluate internal and external performance, using area under the curve (AUC) estimates. A pooled assessment was performed by combining all cohorts together with fivefold cross validation. AI-otoscopy algorithms achieved high internal performance (mean AUC: 0.95, 95%CI: 0.80–1.00). However, performance was reduced when tested on external otoscopic images not used for training (mean AUC: 0.76, 95%CI: 0.61–0.91). Overall, external performance was significantly lower than internal performance (mean difference in AUC: −0.19, p ≤ 0.04). Combining cohorts achieved a substantial pooled performance (AUC: 0.96, standard error: 0.01). Internally applied algorithms for otoscopy performed well to identify middle ear disease from otoscopy images. However, external performance was reduced when applied to new test cohorts. Further efforts are required to explore data augmentation and pre-processing techniques that might improve external performance and develop a robust, generalizable algorithm for real-world clinical applications.

Abstract

Sustainable agricultural water systems are critical to ensure prosperous agricultural production, secure water resources, and support healthy ecosystems that sustain livelihoods and well-being. Many growing regions are using water unsustainably, leading to groundwater and streamflow depletion and polluted water bodies. Often, this is driven by global consumer demands, with environmental and social impacts occurring in regions far from where the crop is ultimately consumed. This letter defines sustainable agricultural water limits, both for quantity and quality, tying them to the impacts of agricultural water use, such as impacts on ecosystems, economies, human health, and other farmers. Imposing these limits will have a range of both positive and negative impacts on agricultural production, food prices, ecosystems, and health. Pathways forward exist and are proposed based on existing studies, showing the gains that can be made from the farm to global scale to ensure sustainable water systems while sustaining agricultural production.

Abstract Head and neck squamous cell carcinoma (HNSCC) represents a heterogeneous group of tumors. While significant progress has been made using multimodal treatment, the 5‐year survival remains at 50%. Developing effective therapies, such as immunotherapy, will likely lead to better treatment of primary and metastatic disease. However, not all HNSCC tumors respond to immune checkpoint blockade therapy. Understanding the complex cellular composition and interactions of the tumor microenvironment is likely to lead to new knowledge for effective therapies and treatment resistance. In this review, we discuss HNSCC characteristics, predictive biomarkers, factors influencing immunotherapy response, with a focus on the tumor microenvironment.

Abstract

Tropical coral reef-lined coasts are exposed to storm wave-driven flooding. In the future, flood events during storms are expected to occur more frequently and to be more severe due to sea-level rise, changes in wind and weather patterns, and the deterioration of coral reefs. Hence, disaster managers and coastal planners are in urgent need of decision-support tools. In the short-term, these tools can be applied in Early Warning Systems (EWS) that can help to prepare for and respond to impending storm-driven flood events. In the long-term, future scenarios of flooding events enable coastal communities and managers to plan and implement adequate risk-reduction strategies. Modeling tools that are used in currently available coastal flood EWS and future scenarios have been developed for open-coast sandy shorelines, which have only limited applicability for coral reef-lined shorelines. The tools need to be able to predict local sea levels, offshore waves, as well as their nearshore transformation over the reefs, and translate this information to onshore flood levels. In addition, future scenarios require long-term projections of coral reef growth, reef composition, and shoreline change. To address these challenges, we have formed the UFORiC (Understanding Flooding of Reef-lined Coasts) working group that outlines its perspectives on data and model requirements to develop EWS for storms and scenarios specific to coral reef-lined coastlines. It reviews the state-of-the-art methods that can currently be incorporated in such systems and provides an outlook on future improvements as new data sources and enhanced methods become available.

Abstract

Functional integrity on coral reefs is strongly dependent upon coral cover and coral carbonate production rate being sufficient to maintain three-dimensional reef structures. Increasing environmental and anthropogenic pressures in recent decades have reduced the cover of key reef-building species, producing a shift towards the relative dominance of more stress-tolerant taxa and leading to a reduction in the physical functional integrity. Understanding how changes in coral community composition influence the potential of reefs to maintain their physical reef functioning is a priority for their conservation and management. Here, we evaluate how coral communities have changed in the northern sector of the Mexican Caribbean between 1985 and 2016, and the implications for the maintenance of physical reef functions in the back- and fore-reef zones. We used the cover of coral species to explore changes in four morpho-functional groups, coral community composition, coral community calcification, the reef functional index and the reef carbonate budget. Over a period of 31 years, ecological homogenization occurred between the two reef zones mostly due to a reduction in the cover of framework-building branching (Acropora spp.) and foliose-digitiform (Porites porites and Agaricia tenuifolia) coral species in the back-reef, and a relative increase in non-framework species in the fore-reef (Agaricia agaricites and Porites astreoides). This resulted in a significant decrease in the physical functionality of the back-reef zone. At present, both reef zones have negative carbonate budgets, and thus limited capacity to sustain reef accretion, compromising the existing reef structure and its future capacity to provide habitat and environmental services.

Abstract Background Blockade of the PD-1/PD-L1 immune checkpoint pathway is emerging as a promising immunotherapeutic approach for the management and treatment of head and neck cancer patients who do not respond to 1st/2nd line therapy. However, as checkpoint inhibitors are cost intensive, identifying patients who would most likely benefit from anti PD-L1 therapy is required. Developing a non-invasive technique would be of major benefit to the patient and to the health care system. Case presentation We report the case of a 56 year old man affected by a supraglottic squamous cell carcinoma (SCC). A CT scan showed a 20 mm right jugulodigastric node and suspicious lung lesions. The lung lesion was biopsied and confirmed to be consistent with SCC. The patient was offered palliative chemotherapy. At the time of presentation, a blood sample was taken for circulating tumour cell (CTC) analysis. The dissemination of cancer was confirmed by the detection of CTCs in the peripheral blood of the patient, measured by the CellSearch System (Janssen Diagnostics). Using marker-independent, low-shear spiral microfluidic technology combined with immunocytochemistry, CTC clusters were found in this patient at the same time point, expressing PD-L1. Conclusion This report highlights the potential use of CTCs to identify patients which might respond to anti PD-L1 therapy.

Abstract

What can one do when the things and processes used to sustain oneself – physically, economically, emotionally – are executed at a scale and intensity whereby they become poisonous? Such a scenario describes certain addictions. An illustration of such an addiction on a planetary scale is exemplified by the neologism agrilogistics. This planned approach to the environment seeks to organise, divide, and manage the earth in order to increase the likelihood of human survival. From parasite-vulnerable monocrops to systemic biopolitical violence upon indigenous peoples, these conditions expose the dark and unwanted externalities to the logistical algorithm that we have only recently begun to understand as deeply intertwined with each other and within all life on the planet. What can art and design do to intervene in this positive feedback loop of economic growth and environmental catastrophe? Perhaps one thing we can do is to use visual, material, and spatial media to think the world differently by questioning the premise of anthropocentrism, establishing instead new conditions of relation between human and nonhuman and with it, new forms of kinship with other species. The exhibition Ambiguous Territory: Architecture, Landscape and the Postnatural assembles a shifting tide of practices, objects, and images by architects, landscape architects, and artists that points to ways of operating within this new paradigm.

A zombie idea is one that has been repeatedly refuted by analysis and evidence, and should have died, but clings to life for reasons that are difficult to understand without further investigation. The perception that investments in modern irrigation systems automatically save water constitutes a zombie idea. On face value, most would accept that modernizing irrigation systems makes sense: agriculture represents 70% of global water withdrawals while physical irrigation efficiencies range between 25% and 50% worldwide—that is, most of the water entering the irrigation system never makes it to the targeted crop. However, the impacts of modern irrigation systems are complex, and as we show, usually have the opposite effect to that intended through altered cropping and water application decisions by farmers, that aggravate water scarcity. This paper investigates how this zombie idea forms; why it persists, even when proven wrong by scientific evidence; and how to overcome it.

Abstract

Research was conducted involving a series of lateral impact tests on a horizontal sled facility by scientists at the Air Force Research Laboratory (AFRL). The purpose of the research was to conduct an assessment of the biodynamic response of an anthropomorphic test device (ATD) to support the development of AFRL neck injury criteria. Impacts were completed using a 50th male Hybrid III aerospace ATD due to this ATD being used by the USAF to qualify and evaluate ejection systems. A test matrix was developed to assess ATD response as a function of various seat configurations which were an approximation of the seat configurations used by the Medical College of Wisconsin (MCW) for previously conducted lateral impact tests of PMHS subjects (post-mortem human subjects). The specially fabricated seat configurations were a rigid seat fixture with a 5-point harness and a padded rigid seat with a 3-point harness. The input acceleration pulses were trapezoidal in shape and varied in peak magnitude from 8.5 to 17 G. The rigid and padded seat configurations both generated fairly linear ATD responses across the input acceleration range. The ATD’s response with the padded seat and the 3-point restraint was greater than the ATD’s response with the rigid seat and the 5-point restraint with the upper neck. The My torque showed the greatest increase from the rigid seat configuration to the padded seat configuration. This highlights the importance of a proper restraint and the importance of controlling the motion of the torso since it could reduce the loads and torques of the unrestrained head and neck, resulting in a lower probability of injury. The lateral impact program with the ATD provided critical impact data to fill data gaps that support the development of the ATD-to-human transfer functions for AFRL’s Multi-Axial Neck Injury Criteria (MANIC) for lateral impact or MANICy calculation. The program also highlighted gaps in human and PMHS head response data in identical lateral impact configurations that would not only improve the current MANICy transfer function but would allow the investigation of the efficacy of using the 6F-MANICy to replace the current MANICy.

13. Machine Learning and Conflict Prediction: A Use Case

Abstract

For at least the last two decades, the international community in general and the United Nations specifically have attempted to develop robust, accurate and effective conflict early warning system for conflict prevention. One potential and promising component of integrated early warning systems lies in the field of machine learning. This paper aims at giving conflict analysis a basic understanding of machine learning methodology as well as to test the feasibility and added value of such an approach. The paper finds that the selection of appropriate machine learning methodologies can offer substantial improvements in accuracy and performance. It also finds that even at this early stage in testing machine learning on conflict prediction, full models offer more predictive power than simply using a prior outbreak of violence as the leading indicator of current violence. This suggests that a refined data selection methodology combined with strategic use of machine learning algorithms could indeed offer a significant addition to the early warning toolkit. Finally, the paper suggests a number of steps moving forward to improve upon this initial test methodology.

Abstract

Although ecosystem services are increasingly recognized as benefits people obtain from nature, we still have a poor understanding of how they actually enhance multidimensional human well-being, and how well-being is affected by ecosystem change. We develop a concept of "ecosystem service elasticity" (ES elasticity) that describes the sensitivity of human well-being to changes in ecosystems. ES Elasticity is a result of complex social and ecological dynamics and is context dependent, individually variable, and likely to demonstrate nonlinear dynamics such as thresholds and hysteresis. We present a conceptual framework that unpacks the chain of causality from ecosystem stocks through flows, goods, value, and shares to contribute to the well-being of different people. This framework builds on previous conceptualizations, but places multidimensional well-being of different people as the final element. This ultimately disaggregated approach emphasizes how different people access benefits and how benefits match their needs or aspirations. Applying this framework to case studies of individual coastal ecosystem services in East Africa illustrates a wide range of social and ecological factors that can affect ES elasticity. For example, food web and habitat dynamics affect the sensitivity of different fisheries ecosystem services to ecological change. Meanwhile high cultural significance, or lack of alternatives enhance ES elasticity, while social mechanisms that prevent access can reduce elasticity. Mapping out how chains are interlinked illustrates how different types of value and the well-being of different people are linked to each other and to common ecological stocks. We suggest that examining chains for individual ecosystem services can suggest potential interventions aimed at poverty alleviation and sustainable ecosystems while mapping out of interlinkages between chains can help to identify possible ecosystem service trade-offs and winners and losers. We discuss conceptual and practical challenges of applying such a framework and conclude on its utility as a heuristic for structuring interdisciplinary analysis of ecosystem services and human well-being.

This paper reports on a study of the differences in the dominant cognitive processes preferred by groups of engineering and education students and examines the implications of these differences for the assessment of student engagement with university courses. Concern is expressed that the items commonly used to capture student engagement data do not adequately cover the full range of the dominant cognitive processes preferred by tertiary students. The paper sets out a brief overview of student engagement along with the theory of dominant and auxiliary cognitive processes, as developed by Jung and later by Myers. Evidence is presented of the differing frequencies of the eight cognitive processes, as assessed by the Myers-Briggs Type Indicator, that are preferred by cohorts of students undertaking courses in engineering and education. The implications of these differences are discussed in the context of subject disciplines in university environments.

16. NRF2 activation restores disease related metabolic deficiencies in olfactory neurosphere-derived cells from patients with sporadic Parkinson's disease.

Abstract

Without appropriate cellular models the etiology of idiopathic Parkinson's disease remains unknown. We recently reported a novel patient-derived cellular model generated from biopsies of the olfactory mucosa (termed olfactory neurosphere-derived (hONS) cells) which express functional and genetic differences in a disease-specific manner. Transcriptomic analysis of Patient and Control hONS cells identified the NRF2 transcription factor signalling pathway as the most differentially expressed in Parkinson's disease.We tested the robustness of our initial findings by including additional cell lines and confirmed that hONS cells from Patients had 20% reductions in reduced glutathione levels and MTS [3-(4,5-dimethylthiazol-2-yl)-5-(3-carboxymethoxyphenyl)-2-(4-sulfophenyl)-2H-tetrazolium, inner salt] metabolism compared to cultures from healthy Control donors. We also confirmed that Patient hONS cells are in a state of oxidative stress due to higher production of H(2)O(2) than Control cultures. siRNA-mediated ablation of NRF2 in Control donor cells decreased both total glutathione content and MTS metabolism to levels detected in cells from Parkinson's Disease patients. Conversely, and more importantly, we showed that activation of the NRF2 pathway in Parkinson's disease hONS cultures restored glutathione levels and MTS metabolism to Control levels. Paradoxically, transcriptomic analysis after NRF2 pathway activation revealed an increased number of differentially expressed mRNAs within the NRF2 pathway in L-SUL treated Patient-derived hONS cells compared to L-SUL treated Controls, even though their metabolism was restored to normal. We also identified differential expression of the PI3K/AKT signalling pathway, but only post-treatment.Our results confirmed NRF2 as a potential therapeutic target for Parkinson's disease and provided the first demonstration that NRF2 function was inducible in Patient-derived cells from donors with uniquely varied genetic backgrounds. However, our results also demonstrated that the response of PD patient-derived cells was not co-ordinated in the same way as in Control cells. This may be an important factor when developing new therapeutics.

Abstract

Background: Hypertrophic pyloric stenosis in otherwise healthy neonates frequently requires urgent surgical procedure but anaesthesia care may result in respiratory complications, such as hypoxaemia, pulmonary aspiration of gastric contents, and postoperative apnoea. The primary aim was to study whether or not the incidence of difficult airway management and of hypoxaemia in neonates undergoing pyloric stenosis repair was higher than that in neonates undergoing other surgeries. Methods: Data on neonates and infants undergoing anaesthesia and surgery for pyloric stenosis were extracted from the NEonate and Children audiT of Anesthesia pRactice In Europe (NECTARINE) database, for secondary analysis. Results: We identified 310 infants who had anaesthesia for surgery for pyloric stenosis. Difficult airway management (more than two attempts at laryngoscopy) was higher in children with pyloric stenosis when compared with the entire NECTARINE cohort (7.9% [95% confidence interval {CI}, 5.22–11.53] vs 4.4% [95% CI, 1.99–6.58]; relative risk [RR]=1.81 [95% CI, 1.21–2.69]; P=0.004), whereas transient hypoxaemia with oxygen saturation

24. Evaluating the generalizability of deep learning image classification algorithms to detect middle ear disease using otoscopy

Abstract

Purpose To evaluate the generalizability of artificial intelligence (AI)-otoscopy algorithms to identify middle ear disease using otoscopic images. Methods 1842 otoscopic images were collected from 3 independent sources: a) Van, Turkey, b) Santiago, Chile, and c) Ohio, USA. Diagnostic categories consisted of (i) normal or (ii) abnormal. Deep and transfer learning-based methods were used to develop models to evaluate internal and external performance, using area under the curve (AUC) estimates. A pooled assessment was performed by combining all cohorts together with 5-fold cross validation. Results AI-otoscopy algorithms achieved high internal performance (mean AUC: 0.95, 95%CI: 0.80–1.00). However, performance was reduced when tested on external otoscopic images not used for training (mean AUC: 0.76, 95%CI: 0.61–0.91). Overall, external performance was significantly lower than internal performance (mean difference in AUC: -0.19, mean standard error: 0.02, p≤0.04). Combining cohorts achieved a substantial pooled performance (AUC: 0.96, standard error: 0.01). Conclusion Internally applied AI-otoscopy algorithms performed well in identifying middle ear disease from otoscopy images. However, external performance was reduced when applied to new test cohorts. Further efforts are required to explore data augmentation and pre-processing techniques that might improve external performance and develop a robust, generalizable algorithm for real-world clinical applications.

Abstract

Background: Neonates and infants are susceptible to hypoxaemia in the perioperative period. The aim of this study was to analyse interventions related to anaesthesia tracheal intubations in this European cohort and identify their clinical consequences. Methods: We performed a secondary analysis of tracheal intubations of the European multicentre observational trial (NEonate and Children audiT of Anaesthesia pRactice IN Europe [NECTARINE]) in neonates and small infants with difficult tracheal intubation. The primary endpoint was the incidence of difficult intubation and the related complications. The secondary endpoints were the risk factors for severe hypoxaemia attributed to difficult airway management, and 30 and 90 day outcomes. Results: Tracheal intubation was planned in 4683 procedures. Difficult tracheal intubation, defined as two failed attempts of direct laryngoscopy, occurred in 266 children (271 procedures) with an incidence (95% confidence interval [CI]) of 5.8% (95% CI, 5.1e6.5). Abstract

BACKGROUND Little is known about current clinical practice concerning peri-operative red blood cell transfusion in neonates and small infants. Guidelines suggest transfusions based on haemoglobin thresholds ranging from 8.5 to 12���g���dl-1, distinguishing between children from birth to day 7 (week 1), from day 8 to day 14 (week 2) or from day 15 (���week 3) onwards. OBJECTIVE To observe peri-operative red blood cell transfusion practice according to guidelines in relation to patient outcome. DESIGN A multicentre observational study. SETTING The NEonate-Children sTudy of Anaesthesia pRactice IN Europe (NECTARINE) trial recruited patients up to 60 weeks' postmenstrual age undergoing anaesthesia for surgical or diagnostic procedures from 165 centres in 31 European countries between March 2016 and January 2017. PATIENTS The data included 5609 patients undergoing 6542 procedures. Inclusion criteria was a peri-operative red blood cell transfusion. MAIN OUTCOME MEASURES The primary endpoint was the haemoglobin level triggering a transfusion for neonates in week 1, week 2 and week 3. Secondary endpoints were transfusion volumes, 'delta haemoglobin' (preprocedure - transfusion-triggering) and 30-day and 90-day morbidity and mortality. RESULTS Peri-operative red blood cell transfusions were recorded during 447 procedures (6.9%). The median haemoglobin levels triggering a transfusion were 9.6 [IQR 8.7 to 10.9] g���dl-1 for neonates in week 1, 9.6 [7.7 to 10.4] g���dl-1 in week 2 and 8.0 [7.3 to 9.0] g���dl-1 in week 3. The median transfusion volume was 17.1 [11.1 to 26.4] ml���kg-1 with a median delta haemoglobin of 1.8 [0.0 to 3.6] g���dl-1. Thirty-day morbidity was 47.8% with an overall mortality of 11.3%. CONCLUSIONS Results indicate lower transfusion-triggering haemoglobin thresholds in clinical practice than suggested by current guidelines. The high morbidity and mortality of this NECTARINE sub-cohort calls for investigative action and evidence-based guidelines addressing peri-operative red blood cell transfusions strategies. Abstract

OBJECTIVE: Little is known about provider perspectives on programmatic responses to structural disadvantage and cultural differences within early intervention in psychosis (EIP) services, programs, and models. The primary objective of this study was to investigate providers’ perspectives on the impacts of disadvantage and minority race, ethnicity, and culture and to describe current practices and perceived gaps and concerns. METHODS: An online survey of specialized EIP providers was disseminated in the United Kingdom, United States, Canada, Australia, and Chile. A total of 164 providers, representing 110 unique sites, completed the survey. Closed-ended questions gathered demographic and program data, including information on formal assessment of trauma or adversity, integration of trauma-informed care, integration of formal cultural assessment tools, training focused on culture, programmatic changes to address culture-related issues, and consultation with cultural insiders. Open-ended questions addressed the demographic mix of the program’s client population; the perceived role and influence of trauma, structural disadvantage, and cultural differences; and concerns and needs related to these topics. Frequencies were examined for closed-ended items; open-ended responses were systematically coded. RESULTS: Overall, survey findings suggested low levels of implementation of a variety of assessment and support practices related to cultural diversity in EIP programs. Coding of open-ended responses revealed numerous concerns regarding the impacts of disadvantage and cultural difference on clients and perceived gaps in policy and implementation. CONCLUSIONS: An expansion of research and service development aimed at better meeting the disadvantage- and culture-related needs of young people with early psychosis and their families should be a priority for the field.

Abstract

The transition to fatherhood is a complex journey scarcely researched in Australian First Nations populations. Historical and political legacy, along with cultural traditions must be taken into consideration before exploring the experiences of Australia's First Nations expectant fathers, especially when the experience is related to the millennia honored “women's business.” This article shares the challenges, opportunities, and rewards the authors experienced while researching with Australia's First Nations men who supported their partners during childbirth in a hospital setting. Ethical insights and input from all members of the research team ensured that culturally safe strategies were used to address all encountered difficulties. Building a trusting relationship with the local community was fundamental to the legitimacy, richness, and success of this research project. Lessons learned from this experience can provide an insight to non-Indigenous researchers choosing to conduct or support research with Australia's First Nations people in a respectful and meaningful manner.

Abstract

The complexity of water, food and energy security is analysed from the perspectives of (i) water and food and (ii) water and energy and their interconnectivity and focuses ultimately on water as a primary input into processes, the entry point for participants of the Third World Irrigation Forum. The paper provides an overview of trends in water, food and energy security, highlights the interconnectivity between the various elements and introduces the water–food–energy nexus as a tool for improving productivity and sector policies, avoiding unintended consequences on other sectors. Invariably, there will be trade‐offs and the challenge is to find combinations of measures that have a net positive outcome. In order to quantify security in the three elements and the trade‐offs between them, emerging modelling approaches for the nexus are discussed. Sub‐theme 3 of the forum focuses on productivity and technology interventions and sub‐theme 2 on stakeholder interaction. The combination of modelling, technology innovations and stakeholder participation in a water–food–energy nexus approach leads to better understanding of linkages and more robust policies and is used to derive recommendations for an enabling policy environment.

Observations of Energetic Electron Substorm Injection Signatures by Cluster and BepiColumbo During an Earth FlybyWe present an analysis of the energetic electron signatures observed by BepiColumbo and Cluster during the Bepi flyby of Earth on 10 April 2020, as well as other spacecraft. After closest approach, the SIXS instrument on Bepi observed two separate substorm injection fronts, while Cluster RAPID/IES also observed a sequence of energetic electron signatures. Bepi and Cluster were in a particularly favourable configuration during this event, with Bepi moving rapidly radially outward near the nightside equatorial plane while the four Cluster spacecraft cut the same region in a north/south direction in a string of pearls configuration. The coincidence of this favourable geometry with the substorm activity is highly fortuitous and appears to show a complicated sequence of spatially and temporally separated injections and drift echoes.

Abstract

To develop an artificial intelligence image classification algorithm to triage otoscopic images from rural and remote Australian Aboriginal and Torres Strait Islander children.Retrospective observational study.Tertiary referral center.Rural and remote Aboriginal and Torres Strait Islander children who underwent tele-otology ear health screening in the Northern Territory, Australia between 2010 and 2018.Otoscopic images were labeled by otolaryngologists to classify the ground truth. Deep and transfer learning methods were used to develop an image classification algorithm.Accuracy, sensitivity, specificity, positive predictive value, negative predictive value, area under the curve (AUC) of the resultant algorithm compared with the ground truth.Six thousand five hundred twenty seven images were used (5927 images for training and 600 for testing). The algorithm achieved an accuracy of 99.3% for acute otitis media, 96.3% for chronic otitis media, 77.8% for otitis media with effusion (OME), and 98.2% to classify wax/obstructed canal. To differentiate between multiple diagnoses, the algorithm achieved 74.4 to 92.8% accuracy and an AUC of 0.963 to 0.997. The most common incorrect classification pattern was OME misclassified as normal tympanic membranes.The paucity of access to tertiary otolaryngology care for rural and remote Aboriginal and Torres Strait Islander communities may contribute to an under-identification of ear disease. Computer vision image classification algorithms can accurately classify ear disease from otoscopic images of Indigenous Australian children. In the future, a validated algorithm may integrate with existing telemedicine initiatives to support effective triage and facilitate early treatment and referral.

The authors regret that errors were present in the above article. Abstract

Background: Neonates and infants requiring anaesthesia are at risk of physiological instability and complications, but triggers for peri-anaesthetic interventions and associations with subsequent outcome are unknown. Methods: This prospective, observational study recruited patients up to 60 weeks' postmenstrual age undergoing anaesthesia for surgical or diagnostic procedures from 165 centres in 31 European countries between March 2016 and January 2017. The primary aim was to identify thresholds of pre-determined physiological variables that triggered a medical intervention. The secondary aims were to evaluate morbidities, mortality at 30 and 90 days, or both, and associations with critical events. Results: Infants (n=5609) born at mean (standard deviation [SD]) 36.2 (4.4) weeks postmenstrual age (35.7% preterm) underwent 6542 procedures within 63 (48) days of birth. Abstract

The Indus basin of Pakistan occupies about 16 million ha (Mha) of land. The Indus River and its tributaries are the primary sources of surface water. An estimated 122 km3 of surface water is diverted annually through an extensive canal system to irrigate this land. These surface water supplies are insufficient to meet the crop water requirements for the intensive cropping system practiced in the Indus basin. The shortfall in surface water is met by exploiting groundwater. Currently, about 62 km3 of groundwater is pumped annually by 1.36 million private and public tube wells. About 1.0 million tubewells are working only in the Punjab province. Small private tubewells account for about 80% of the pumped volume. Inadequate water allocation along the irrigation canals allows excessive water use by head-end farmers, resulting in waterlogging. In contrast, the less productive use of erratic supplies by tail-end farmers often results in soil salinity. The major issues faced by irrigated agriculture in Pakistan are low crop yields and water use efficiency, increasing soil salinization, water quality deterioration, and inefficient drainage effluent disposal. Currently, 4.5 Mha (about 30% of the total irrigated area) suffers from adverse salinity levels. Critical governance issues include inequitable water distribution, minimizing the extent to which salt is mobilized, controlling excessive groundwater pumping, and immediate repair and maintenance of the infrastructure. This paper suggests several options to improve governance, water and salt management to support sustainable irrigated agriculture in Pakistan. In saline groundwater areas, the rotational priorities should be reorganized to match the delivery schedules as closely as possible to crop demand, while emphasizing the reliability of irrigation schedules. Wherever possible, public tubewells should pump fresh groundwater into distributaries to increase water availability at the tail ends. Any substantial reform to make water delivery more flexible and responsive would require an amendment to the existing law and reconfiguration of the entire infrastructure, including thousands of kilometers of channels and almost 60,000 outlets to farmer groups. Within the existing political economy of Pakistan, changing the current water allocation and distribution laws without modernizing the infrastructure would be complicated. A realistic reform program should prioritize interventions that do not require amendment of the Acts or reconstruction of the entire system and are relatively inexpensive. If successful, such interventions may provide the basis for further, more substantial reforms. The present rotational water supply system should continue, with investments focusing on lining channels to ensure equitable water distribution and reduce waterlogging at the head ends. Besides that, the reuse of drainage water should be encouraged to minimize disposal volumes. The timely availability of farm inputs can improve individual farmers’ productivity. Farmers will need to have access to new information on improved irrigation management and soil reclamation approaches. Simultaneously, the government should focus more on the management of drainage and salinity.

Abstract

The ‘footprint’ concept is widely used as an indicator to assess CO2 emissions and the water embodied in crop production. A comparison of key features reveals that CO2 footprints are a global concern no matter their location or source; water footprints only have local relevance, being locally generated and impacting only at local levels. As such, addressing excessive water use is a local concern. Where excessive use is not managed, a process of ‘chaotic disallocation’ from irrigated agriculture ensues, resulting in reduced local production and, through market mechanisms, increased demand elsewhere. Those areas where water scarcity is managed sustainably will see more profitable opportunities for irrigated production, though the impact on prices will be of little concern to consumers in the developed world.
