In einer früheren Arbeit konnte gezeigt werden, dass Seren von Patienten mit AMA positiver/Anti-M2 negativer PBC mit bisher nicht weiter identifizierten mitochondrialen Proteinen mit einem Molekulargewicht von 60 und 80 kD reagieren. In der 60 kD Antigenfraktion konnte mittels Massenspektrometrie – neben anderen Enzymen - das Core I Protein der Cytochrom C-Reduktase identifiziert werden. Ziel der vorliegenden Arbeit war daher, zu überprüfen, ob es sich bei diesem Protein tatsächlich um ein Targetantigen von Autoantikörpern bei der PBC handelt. Ferner sollte das 80 kD Protein weiter analysiert werden. Seren von 88 AMA positiven/Anti-M2 negativen sowie 74 AMA positiven/Anti-M2 positiven PBC-Patienten wurden im WesternBlot und teilweise im ELISA analysiert. Als Kontrollen dienten Seren von Patienten mit anderen Lebererkrankungen und gesunden Blutspendern. Als Antigene dienten eine mittels Sucrosedichtegradientenzentrifugation aus Rinderherzmitochondrien isolierte 1.16/1.18 Fraktion sowie eine kommerziell erhältliche Cytochrom C-Reduktase. Ferner wurde das Core I Protein der Cytochrom C-Reduktase rekombinant in E. coli exprimiert. Zweiundzwanzig der 88 AMA positiven/Anti-M2 negativen PBC-Patienten (25 %) reagierten mit der 60 kD Bande in der 1.16/1.18 Fraktion im Westernblot, ebenfalls 25 % mit der 80 kD-Determinante. AMA positiv/Anti-M2 positive Patienten zeigten diese Reaktionen in 27 % bzw. 8 % der Fälle. Die 80 kD Determinante wurde mittels Elektroelution weiter gereinigt und einer massenspektrometrischen Analyse unterworfen, die aber keine eindeutigen Ergebnisse erbrachte. Um zu überprüfen, ob die Anti-60 kD Antikörper gegen das Core I Protein der Cytochrom C-Reduktase gerichtet sind, wurden Seren der Patienten mit AMA positiver/Anti-M2 negativer PBC gegen das kommerziell erhältliche Enzym sowie das rekombinant hergestellte Core I Protein der Cytochrom C-Reduktase getestet. Es ergaben sich aber keine eindeutigen Reaktionen und insbesondere keine Korrelationen mit den Ergebnissen, die unter Verwendung der 1.16/1.18 Dichtegradientenfraktion erhalten worden waren. In der vorliegenden Arbeit konnte die Existenz weiterer bisher nicht identifizierter AMA-Spezifitäten bei Patienten insbesondere mit Anti-M2 negativer aber auch Anti-M2 positiver PBC bestätigt werden. Als Targetantigene der Anti-60 kD Antikörper waren bisher das Core I Protein der Cytochrom C-Reduktase sowie die F1-ATPase diskutiert worden. Das Core I Protein kann jetzt nach den hier präsentierten Befunden mit großer Wahrscheinlichkeit ausgeschlossen werden, eine parallel durchgeführte Arbeit macht aber die Existenz von Antikörpern gegen die F1-ATPase wahrscheinlich. Wie aber der Nachweis von Antikörpern gegen ein 80 kD Protein belegt, muss es noch weitere AMA-Spezifitäten geben, deren Targetantigene noch nicht identifiziert werden konnten. Nachdem die Anti-M2 Antikörper bisher keine eindeutigen Hinweise zur Ätiopathogenese der PBC geliefert haben, eröffnet möglicherweise die Entdeckung weiterer, eventuell auch funktionell aktiver Autoantikörper diesbezüglich neue Aspekte. Previously it was shown, that sera from patients with antimitochondrial antibody (AMA) positive/anti-M2 negative primary biliary cirrhosis (PBC) react with yet undefined mitochondrial proteins with a molecular weight of 60 and 80 kDa (kilo Dalton). Using mass-spectrometry, the Core I Protein of the Cytochrome C-Reductase could be identified in the 60 kDa antigen fraction. The aim of this study was to see, whether this protein is a target antigen for antibodies in PBC. Sera from 88 AMA positive/Anti-M2 negative and 74 AMA positive/Anti-M2 positive PBC patients were analysed, using Western blotting and enzyme linked immunosorbent assay (ELISA). Sera from blood donors and patients with different liver diseases served as controls. An antigen fraction, obtained from bovine heart mitochondria via sucrose density centrifugation (1.16/1.18) and a commercial Cytochrome C-Reductase antigen were used. In addition, the Core 1 protein of the Cytochrome C-Reductase was recombinantly expressed in E. coli. The presence of Cytochrome C-Reductase was verified by incubating the antigen 1.16/1.18 with a commercial anti-Core I protein antibody. Twenty-two of the 88 AMA positive/anti-M2 negative PBC patients (25%) showed a positive reaction with the 60 kDa and the 80 kDa-determinant of the 1.16/1.18 fraction in the Western blot. Testing the 74 AMA positive/anti-M2 negative patients, 27% were positive with the 60 kDa and 8% with the 80 kDa determinant. The 80 kDa determinant was purified by electro elution and analyzed by mass-spectrometry but no significant results were obtained.. In order to see, whether the anti-60 kDa antibodies were directed against the Core I Protein of the Cytochrome C-Reductase, the sera from patients with AMA positive/anti-M2 negative PBC were tested against the commercially available enzyme and the recombinantly produced Core I protein of the Cytochrome C-Reductase. However, there was no significant reaction with this enzyme, and there was also no correlation between the presence of anti-Cytochrome C-Reductase and anti-60kDa antibodies. In the present study it could be shown that in sera from PBC-patients further AMA-specificities besides anti-M2 can be detected. In previous studies it was suspected, that they may react with the core I protein of Cytochrome C-Reductase, but this enzyme could now be excluded with high probability as relevant target antigen of anti-60kDa antibodies. Its identity and the clinical relevance of these antibodies have, therefore, to be evaluated in further studies. Moreover, it has to be seen whether those antibodies may give more insights into the pathogenesis of PBC than the anti-M2 antibodies.
