En effet cette pathologie peut revêtir de nombreux aspects, tant au niveau dentaire, squelettique, fonctionnel qu’articulaire, qui pourront de plus se compliquer par un trouble temporo-mandibulaire associé. C’est le cas d’Emilie, 12 ans et 8 mois lors de la première consultation.

Radiofrequency energy is a common treatment modality for chronic pain. While there are different forms of radiofrequency-based therapeutics, the common concept is the generation of an electromagnetic field in the applied area, that can result in neuromodulation (pulsed radiofrequency—PRF) or ablation. Our specific focus relates to PRF due to the possibility of modulation that is in accordance with the mechanisms of action of orthobiologics. The proposed mechanism of action of PRF pertaining to pain relief relies on a decrease in pro-inflammatory cytokines, an increase in cytosolic calcium concentration, a general effect on the immune system, and a reduction in the formation of free radical molecules. The primary known properties of orthobiologics constitute the release of growth factors, a stimulus for endogenous repair, analgesia, and improvement of the function of the injured area. In this review, we described the mechanism of action of both treatments and pertinent scientific references to the use of the combination of PRF and orthobiologics. Our hypothesis is a synergic effect with the combination of both techniques which could benefit patients and improve the life quality.

This research conducted at PT x aims to find out how much influence Motivation has on performance which is partially and simultaneously moderated by employee commitment. This research uses two methods, which is Descriptive Method and Verification Method. The sample for this research are 100 employees whom are chosen randomly. Penelitian ini dilakukan di PT x untuk mengetahui besaran pengaruh motivasi karyawan terhadap kinerja karyawan yang dimoderasi dengan komitmen karyawan secara parsial dan simultan. Ada dua model penelitian yang digunakan dalam penelitian ini, yakni metode deskriptif dan metode verifikatif. Sampel dari penelitian ini adalah karyawan PT x yang berjumlah 100 jiwa yang dipilih secara acak. Penelitian menyimpulkan bahwa kinerja dan motivasi di PT x dikatakan baik, terdapat pengaruh yang signifikan antara motivasi kerja terhadap kinerja karyawanyang dimoderasi komitmen kerja terhadap performansi baik secara parsial maupun simultan.

Osseous manifestations of neurofibromatosis 1 (NF-1) occur in a minority of the affected subjects but may be because of significant clinical impairment. Typically, they involve the long bones, commonly the tibia and the fibula, the vertebrae, and the sphenoid wing. The pathogenesis of NF-1 focal osseous lesions and its possible relationships with other osseous NF-1 anomalies leading to short stature are still unknown, though it is likely that they depend on a common mechanism acting in a specific subgroup of NF-1 patients. Indeed, NF-1 gene product, neurofibromin, is expressed in all the cells that participate to bone growth: osteoblasts, osteoclasts, chondrocytes, fibroblasts, and vascular endothelial cells. Absent or low content of neurofibromin may be responsible for the osseous manifestations associated to NF-1. Among the focal NF-1 osseous anomalies, the agenesis of the sphenoid wing is of a particular interest to the neurosurgeon because of its progressive course that can be counteracted only by a surgical intervention. The sphenoid wing agenesis is regarded as a dysplasia, which is a primary bone pathology. However, its clinical progression is related to a variety of causes, commonly the development of an intraorbital plexiform neurofibroma or the extracranial protrusion of temporal lobe parenchyma and its coverings. Thus, the cranial bone defect resulting by the primary bone dysplasia is progressively accentuated by the orbit remodeling caused by the necessity of accommodating the mass effect exerted by the growing tumor or the progression of the herniated intracranial content. The aim of this paper is to review the neurosurgical and craniofacial surgical modalities to prevent the further progression of the disease by "reconstructing" the normal relationship of the orbit and the skull.

During the SARS-CoV-2 pandemic, higher education worldwide had to switch to digital formats. The purpose of this study was to evaluate CoRad-19, a digital teaching tool created by the German Radiological Society for medical students during the COVID-19 pandemic.A total of 13 German-speaking universities implemented CoRad-19 in their curriculum and partially or completely replaced their classes with the online courses. Previous experience and contact with radiology and the participants' opinions regarding the medium of e-learning were surveyed using a custom questionnaire. The subjective level of knowledge regarding the individual modules was also surveyed before and after participation to measure learning effects. The data of 994 medical students from the participating sites were analyzed and compared intraindividually using the Friedman test.From 4/1/2020-10/1/2020, 451 complete data sets from a total of 994 surveys were included. E-learning was rated "very useful" both before and after course participation (4 [IQR 3-4], p = 0.527, r = 0.16). E-learning as a method was also rated as a "very good" medium both before and after participation (4 [IQR 3-4], p = 0.414, r = 0.17). After participation, participants rated radiology as particularly suitable for digital teaching (before: 3 [IQR 3-4] vs. after 4 [IQR 3-4], p = 0.005, r = 0.6). Significant learning gains were measurable in all course modules (p ≤ 0.009). Post-hoc analysis showed interest in radiology to increase significantly after course participation (p = 0.02).In the representative survey, significant learning effects were observed in all course modules. In addition, it should be particularly emphasized that the students' interest in radiology was increased by course participation. In dieser Studie sollte CoRad-19, ein im Rahmen der COVID-19-Pandemie erstelltes digitales Lehrangebot der Deutschen Röntgengesellschaft (DRG) für Medizinstudierende, evaluiert werden.Insgesamt 13 deutschsprachige Hochschulstandorte haben CoRad-19 in ihr Curriculum implementiert sowie ihre Lehrangebote teilweise oder sogar vollständig durch die Online-Kurse ersetzt. Mithilfe eines eigens entworfenen Fragebogens wurden Vorerfahrungen in und bisherige Kontakte zur Radiologie erfragt sowie die Meinung der Teilnehmenden gegenüber dem Medium E-Learning erhoben. Als Maßstab für den Lernerfolg wurde der subjektive Kenntnisstand bezüglich der einzelnen Module jeweils vor und nach der Teilnahme erhoben. However, for the healing process to occur, an inflammatory phase is also deemed necessary. Leukocytes present in the inflammatory phase release both pro- and anti-inflammatory molecules. The latter may play an important role in the process of "inflammatory regeneration". Thus, we propose that in the context of healing, both platelets and leukocytes play an important role, specifically due to the macrophage's plasticity to switch from the M1 to M2 fraction. Therefore, we propose that PRP products derived from the buffy coat may be more beneficial than detrimental from a standpoint of the regenerative potential of PRP.

Introduction: Oronasal fistula have several etiologies. Most of the time, the origin is congenital associated with cleft lip and patate patients, or it can be acquired after trauma, infection, neoplasm, postsurgical complications or radio and chemonecrosis. Excepted for palatal obturators, their treatment is surgical and may use different intraoral flaps. Observation: A 9-year-old girl was sent to consult presenting an oronasal fistula as a sequela of a cleft lip and palate, initially treated in Kosovo (Albania). This was an anterior midpalate fistula, large around 2 cm, functionnaly responsible of a nasal speech and food reflux in the nasal airways. The closure has been done with a tongue flap. Commentary: Most of the oronasal fistulae secondary to a cleft palate are managed with palatal rotational flaps. Nevertheless, they cannot be used in some cases, especially when the fistula size is too large, the localization too anterior and with already scarred palatal tissues around. Then, tongue flap may represent a reliable alternative. Conclusion: The tongue flap allowed a complete closure of the oronasal fistula, with a very good tolerance from the child. It shows its interest into the list of the numerous existing oral flaps. However, considering the discomfort of this technique, it must be done in selected cases, with compliant patients.

Introduction : La dysharmonie dents-arcades autrefois appelée « dysharmonie dento-maxillaire » ou « macrodontie/microdontie relative » correspond à une disproportion de volume entre les dents et les bases osseuses. En présence d’un volume dentaire trop important par rapport à la longueur d’arcade, un traitement avec extraction peut être proposé. Les techniques de « distraction osseuse » mises au point par Ilizarov dans les années 1950 permettent actuellement d’augmenter la longueur d’arcade au maxillaire et à la mandibule. Matériels et méthodes : Les auteurs décriront la mise en œuvre orthodontico-chirurgicale de cette augmentation de longueur d’arcade depuis l’établissement du plan thérapeutique jusqu’à l’obtention d’une longueur d’arcade suffisante ainsi que l’intérêt thérapeutique devant l’encombrement dentaire, la vestibuloversion et les troubles des fonctions orofaciales. Discussion : La distraction est un moyen thérapeutique intéressant à envisager dans la prise en charge de la dysharmonie dents-arcades, qui permet de gagner la longueur d’arcade nécessaire à l’objectif thérapeutique fixé.

Penelitian ini bertujuan untuk mengetahui seberapa besar pengaruh layanan sumber daya manusia dan fasilitas fisik terhadap kepuasan masyarakat Desa Batujajar Barat di sektor pemerintahan, baik secara parsial maupun simultan dimana metode penelitian yang digunakan adalah metode deskriptif dan verifikatif. Populasi penelitian ini adalah komunitas Desa Batujajar Barat, yang berjumlah sekitar 15.000 orang dengan sampel 100 responden dimana analisis teknik data yang digunakan adalah teknik regresi linier berganda korelasi dan koefisien determinasi. Hasil penelitian menunjukkan bahwa sumber daya manusia perangkat desa dapat dikatakan tidak baik dalam hal kompetensi, sikap dan pengetahuan, serta proses pelayanan, dapat dikatakan tidak baik dalam hal kecepatan, kemudahan prosedur dan waktu kepastian. The short osteotomy was first reported by Blair in 1907 and was later described and characterized by Kater and Paulus in 2013. The technique involves an oblique supra-lingular osteotomy from internal to external, from top to bottom and from back to front. It is designed to divide the ramus into two segments, the joint and the mandibular body, and to ensure minimal valve equivalents. Both osseous valves were maintained in contact by obtaining surface contact proportional to the angle of the osteotomy. We present a series of 82 patients to demonstrate the benefits of this technique in terms of stability and conservation of sensitivity. 82 patients were included, of which 74 (90%) described normal sensitivity and 8 (10%) reported modified sensitivity two years post-surgery. On account of the nature and height of the short osteotomy cut, it is possible to avoid direct manipulation of the lower alveolar nerve. The postoperative sensitivity level is identical to the pre-operative level in more than 90% of cases. In good indications, this technique could be used preferentially to optimize postoperative nerve recovery. We also discuss the current limitations of this orthognathic surgery procedure, as well as the perspectives linked to the development of 3D printing and preoperative planning.

L’ostéotomie sagittale de la mandibule est devenue un standard dans la chirurgie mandibulaire des dysmorphoses dento-faciales. Nous la proposons même pour les dysmorphoses de faible importance. Ceci doit nous forcer à en réduire la morbidité. Nous décrivons une technique opératoire à surface de clivage réduit. La section osseuse suit une ligne oblique depuis l’épine de Spix en bas et en dehors vers la région supra-angulaire. Cette section est complétée par une ostéotomie du bord postérieur de la branche montante. Ce clivage n’atteint jamais le canal dentaire. La protection du nerf alvéolaire est accrue, ce qui diminue considérablement le risque de séquelles nerveuses de cette intervention. La majorité des déplacements mandibulaires sont possibles par cette technique à l’exclusion des propulsions importantes et des augmentations de la hauteur du ramus.

Platelet-rich plasma (PRP) has emerged as a significant therapy used in medical conditions with heterogeneous results. There are some important classifications to try to standardize the PRP procedure. The aim of this report is to describe PRP contents studying celular and molecular components, and also propose a new classification for PRP. The main focus is on mononuclear cells, which comprise progenitor cells and monocytes. In addition, there are important variables related to PRP application incorporated in this study, which are the harvest method, activation, red blood cells, number of spins, image guidance, leukocytes number and light activation. The other focus is the discussion about progenitor cells presence on peripherial blood which are interesting due to neovasculogenesis and proliferation. The function of monocytes (in tissue-macrophages) are discussed here and also its plasticity, a potential property for regenerative medicine treatments.

Tooth-arch discrepancy is a disproportion between dental volume and bone base. Extraction therapy can be a solution in case of excessive tooth volume and insufficient basal bone length. Techniques including bone distraction popularized by Ilizarov in the fifties allow the increase of the basal arch length in the maxilla as well as in the mandible.We will describe the procedure of this dental arch length augmentation since the reflection about the therapeutic plan until the sufficient arch length is obtained and describe the indications of this orthodontic and surgical treatment in case of dental crowding, buccal and labial inclination and functional problems.Distraction is an interesting technique to be considered for the management of macrodontia that allows to get enough basal bone lenght to reach the therapeutic goal.
