Background Prolonged air leak (PAL) (>5 days) after robotic-assisted pulmonary lobectomy is a significant complication. This study aimed to determine patient- and surgeon-related factors that can predict PAL after robotic lobectomy for lung cancer. Methods This study was a retrospective review of a single-center experience of robotic-assisted lobectomy for lung cancer. Perioperative variables, including surgeon case experience, patient demographics, diffusion capacity of lung for carbon monoxide, forced expiratory volume in 1 second, body mass index, and smoking status were evaluated. Results A total of 305 robotic-assisted lobectomies performed by 4 surgeons met inclusion criteria from June 2016 to February 2019. The 30-day postoperative mortality was 1.2%. PAL developed in 27 of 305 (8.8%) patients. Surgeons’ robotic experience was grouped by 10-case increments. When adjusted for age and sex, the odds for PAL decreased by 15% for every 10 robotic lobectomies the surgeons performed (odds ratio [OR], 0.85; 95% CI, 0.74-0.99; P = .0384). Logistic regression models showed a linear transition curve at the 50th case. Female sex (OR, 2.62; 95% CI, 1.03-6.69; P = .0314) and younger age (OR, 0.61; 95% CI, 0.41-0.91; P = .0184) were statistically significant risk factors for PAL. Cumulative sum analysis similarly showed a strong association between experience and PAL. Preoperative diffusing capacity of lung for carbon monoxide, forced expiratory volume in 1 second, body mass index, and smoking status were not statistically significant predictive factors. Conclusions These results show that surgeon robotic case experience is associated with the rate of postoperative PAL: as the number of robotic lobectomies increases, the rate of PAL significantly decreases. It is imperative to emphasize that a learning curve exists for this approach that directly affects patient outcomes.

Nucleation of ultrafine aerosols near the tropical tropopause, followed by transport throughout the stratosphere by the Brewer-Dobson circulation, is thought to be the primary source of stratospheric aerosol number concentration. However, depending on how rapidly the aerosols grow by condensation, many of the ultrafine particles generated by new particle formation (NPF) may be lost due to coagulation with accumulation-mode aerosols. In this study, we use recent high-altitude aircraft measurements of aerosol size distribution, along with microphysical calculations, to investigate this issue. Initial ultrafine and accumulation-mode size distributions are specified based on the aircraft measurements, and a bin microphysics model is used to simulate the evolution of the aerosol size distributions. Coagulation and condensation of gases such as sulfuric acid, ammonia, and nitric acid are included. Preliminary results indicate that for typical conditions, most of the ultrafine aerosols generated by NPF are removed by coagulation, resulting in a relatively small contribution to the total aerosol number concentration. We have used the model to investigate the sensitivities of total number concentration evolution to aerosol size distributions and condensation rates.

The Asian monsoon anticyclone transports aerosol and gas phase pollutants from the boundary layer to the upper troposphere and lower stratosphere, from whence they are transported out over the Western Pacific by eddy shedding. This significantly increases aerosol loading in the upper troposphere and maintains a layer of aerosol in the lowermost stratosphere with important implications for climate and stratospheric chemistry. Models show large spread in the spatial distribution and microphysical properties of aerosols transported by the Asian monsoon, and the chemical and radiative effects remain uncertain.In August 2022 we measured aerosol size distributions in Asian summer monsoon outflow from two aircraft, the NCAR GV and NASA WB57, as part of the Asian Summer Monsoon Chemical Climate Impacts Project (ACCLIP). On both aircraft we operated a Nucleation Mode Aerosol Size Spectrometer (NMASS, a custom battery of 5 condensation particle counters) and a modified Ultra-High Sensitivity Aerosol Spectrometer (an optical particle counter from Droplet Measurement Technologies) to measure size distributions from 3 to 1500 nm at 1 Hz time resolution.Here we use these data together with concurrently measured trace gases and reactive gases, and cloud properties, to quantify the transport of primary aerosol by the monsoon system, and the formation of secondary aerosol in the monsoon outflow. We show that new particle formation occurs in the upper troposphere in monsoon outflow and investigate its relation to lofting of condensable vapours and wet scavenging of larger aerosols by deep convection. We use data taken in the upper troposphere and lower stratosphere of the NASA Atmospheric Tomography Mission (ATom) to compare aerosol microphysical properties in the summer monsoon outflow with those in less anthropogenically influence air.

Aerosols in the climate system have a direct link to the Earth’s energy balance. Aerosols interact directly with the solar radiation through scattering and absorption; and indirectly by changing cloud properties. The effect aerosols have on climate is one of the major causes of radiative forcing (RF) uncertainty in global climate model simulations. Thus, reducing aerosol RF uncertainty is key to improving climate prediction. The objective of this work is to understand the magnitude and causes of aerosol uncertainty in the chemical transport model TM5.Perturbed Parameter Ensembles (PPEs) are a set of model runs created by perturbing an ensemble of parameters. Parameters are model inputs, for this study we focus on parameters describing aerosol emissions, properties and processes, such as dry deposition, aging rate, emissions to aerosols microphysics. PPEs vary theses parameters over their uncertainty range all at once to study their combine effect on TM5.Varying these parameters along with others through their value range, will reflect on TM5 outputs. The TM5 outputs parameters we are using in our sensitivity study are the cloud droplet number concentration and the ambient aerosol absorption optical thickness at 550nm.Here we discuss the design of the PPE, and one-at-a-time sensitivity studies used in this process. The PPE samples the parameter space to enable us to use emulation. Emulating is a machine learning technique that uses a statistical surrogate model to replace the chemical transport model. The aim is to provide output data with more dense sampling throughout the parameter space. We will be using a Gaussian process emulator, which has been shown to be an efficient technique for quantifying parameter sensitivity in complex global atmospheric models.We also describe plans to extend this work to emulate an aerosol PPE for EC-Earth. The PPE for EC-Earth will also contain cloud parameters that will vary over their uncertainty range together with the aerosol parameters to examine the influence of aerosol parametric uncertainty on RF.

This article provides an overview of the NASA Atmospheric Tomography (ATom) mission and a summary of selected scientific findings to date. ATom was an airborne measurements and modeling campaign aimed at characterizing the composition and chemistry of the troposphere over the most remote regions of the Pacific, Southern, Atlantic, and Arctic Oceans, and examining the impact of anthropogenic and natural emissions on a global scale. These remote regions dominate global chemical reactivity and are exceptionally important for global air quality and climate. ATom data provide the in situ measurements needed to understand the range of chemical species and their reactions, and to test satellite remote sensing observations and global models over large regions of the remote atmosphere. Lack of data in these regions, particularly over the oceans, has limited our understanding of how atmospheric composition is changing in response to shifting anthropogenic emissions and physical climate change. ATom was designed as a global-scale tomographic sampling mission with extensive geographic and seasonal coverage, tropospheric vertical profiling, and detailed speciation of reactive compounds and pollution tracers. ATom flew the NASA DC-8 research aircraft over four seasons to collect a comprehensive suite of measurements of gases, aerosols, and radical species from the remote troposphere and lower stratosphere on four global circuits from 2016 to 2018. Flights maintained near-continuous vertical profiling of 0.15–13-km altitudes on long meridional transects of the Pacific and Atlantic Ocean basins. Analysis and modeling of ATom data have led to the significant early findings highlighted here.

Biomass burning emits ~34–41 Tg yr−1 of smoke aerosol to the atmosphere. Biomass burning aerosol directly influences the Earth’s climate by attenuation of solar and terrestrial radiation; however, its abundance and distribution on a global scale are poorly constrained, particularly after plumes dilute into the background remote troposphere and are subject to removal by clouds and precipitation. Here we report global-scale, airborne measurements of biomass burning aerosol in the remote troposphere. Measurements were taken during four series of seasonal flights over the Pacific and Atlantic Ocean basins, each with near pole-to-pole latitude coverage. We find that biomass burning particles in the remote troposphere are dilute but ubiquitous, accounting for one-quarter of the accumulation-mode aerosol number and one-fifth of the aerosol mass. Comparing our observations with a high-resolution global aerosol model, we find that the model overestimates biomass burning aerosol mass in the remote troposphere with a mean bias of >400%, largely due to insufficient wet removal by in-cloud precipitation. After updating the model’s aerosol removal scheme we find that, on a global scale, dilute smoke contributes as much as denser plumes to biomass burning’s scattering and absorption effects on the Earth’s radiation field. Aerosol particles produced by biomass burning are ubiquitous in the remote troposphere, according to global airborne measurements over remote ocean regions.

In Urban Rituals in Sacred Landscapes in Hellenistic Asia Minor, Christina G. Williamson examines the phenomenon of monumental sanctuaries in the countryside of Asia Minor that accompanied the second rise of the Greek city-state in the Hellenistic period. Moving beyond monolithic categories, Williamson provides a transdisciplinary frame of analysis that takes into account the complex local histories, landscapes, material culture, and social and political dynamics of such shrines in their transition towards becoming prestigious civic sanctuaries. This frame of analysis is applied to four case studies: the sanctuaries of Zeus Labraundos, Sinuri, Hekate at Lagina, and Zeus Panamaros. All in Karia, these well-documented shrines offer valuable insights for understanding religious strategies adopted by emerging cities as they sought to establish their position in the expanding world.

From 2016 to 2018 a DC-8 aircraft operated by the US National Aeronautics and Space Administration (NASA) made four series of flights, profiling the atmosphere from 180 m to ∼12 km above sea level (km a.s.l.) from the Arctic to the Antarctic over both the Pacific and Atlantic oceans. This program, the Atmospheric Tomography Mission (ATom), sought to sample the troposphere in a representative manner, making measurements of atmospheric composition in each season. This paper describes the aerosol microphysical measurements and derived quantities obtained during this mission. Dry size distributions from 2.7 nm to 4.8 µm in diameter were measured in situ at 1 Hz using a battery of instruments: 10 condensation particle counters with different nucleation diameters, two ultra-high-sensitivity aerosol size spectrometers (UHSASs), one of which measured particles surviving heating to 300 ∘C, and a laser aerosol spectrometer (LAS). The dry aerosol measurements were complemented by size distribution measurements from 0.5 to 930 µm diameter at near-ambient conditions using a cloud, aerosol, and precipitation spectrometer (CAPS) mounted under the wing of the DC-8. Dry aerosol number, surface area, and volume, and optical scattering and asymmetry parameters at several wavelengths from the near-UV to the near-IR ranges were calculated from the measured dry size distributions (2.7 nm to 4.8 µm). Dry aerosol mass was estimated by combining the size distribution data with particle density estimated from independent measurements of aerosol composition with a high-resolution aerosol mass spectrometer and a single-particle soot photometer. We describe the instrumentation and fully document the aircraft inlet and flow distribution system, the derivation of uncertainties, and the calculation of data products from combined size distributions. Comparisons between the instruments and direct measurements of some aerosol properties confirm that in-flight performance was consistent with calibrations and within stated uncertainties for the two deployments analyzed. The unique ATom dataset contains accurate, precise, high-resolution in situ measurements of dry aerosol size distributions, and integral parameters, and estimates and measurements of optical properties, for particles µm in diameter that can be used to evaluate aerosol abundance and processes in global models.

The inorganic fraction of fine particles affects numerous physicochemical processes in the atmosphere. However, there is large uncertainty in its burden and composition due to limited global measurements. Here, we present observations from eleven different aircraft campaigns from around the globe and investigate how aerosol pH and ammonium balance change from polluted to remote regions, such as over the oceans. Both parameters show increasing acidity with remoteness, at all altitudes, with pH decreasing from about 3 to about −1 and ammonium balance decreasing from almost 1 to nearly 0. On the NASA Atmospheric Tomography Mission (ATom), we observed a sharp hemispheric contrast in the concentration of ultrafine aerosols (3-12 nm diameter) in the lowermost stratosphere that persisted through all four seasons. Exploring possible causes, we show that this is likely caused by aircraft, which emit both ultrafine aerosol and precursor gases for new particle formation (NPF) in quantities that agree well with our observations. While aircraft may emit a range of NPF precursors, we focus here on sulphur dioxide (a major source of atmospheric sulphuric acid), of which we have observations from the same mission. We observe the same hemispheric contrast in sulphur dioxide as ultrafine aerosol, and find that the observed concentrations are in alignment with inventoried aircraft emissions. We present box modeling and thermodynamic calculations that support the plausibility of NPF under the conditions and sulphur dioxide concentrations observed on ATom.While the direct climate impact of ultrafine aerosol in the lowermost stratosphere (LMS) may currently be small, our observations show a definitive size distribution shift of the background aerosol distribution in the northern hemisphere. This is important for assessing aviation impacts, and the expected impacts of increased air-traffic. Furthermore, climate intervention via injection of sulphate or aerosols into the stratosphere is a current subject of research. Our study shows that NPF is possible and likely already happening in the LMS, which must be accounted for in models for stratospheric modification, and points out that we must consider that any intentional stratospheric modification will be applied to two very different hemispheres: a largely pristine southern hemisphere; and an already anthropogenically modified northern hemisphere.

There are distinct types of aerosol particles in the lower stratosphere. Stratospheric sulfuric acid particles with and without meteoric metals coexist with mixed organic-sulfate particles that originated in the troposphere. That these particles remain distinct has important implications for aerosol chemistry and the concentrations of several gas-phase species. Neither semi-volatile organics nor ammonia can be in equilibrium with the gas phase. The gas-phase concentrations of semi-volatile organics and ammonia must be very low, or else the sulfuric acid particles would not stay so pure. The upper concentration limits are around a pptv. Yet the sulfuric acid particles in the Northern Hemisphere show a very small but measurable uptake of organics and ammonia, indicating non-zero gas-phase concentrations of those species. Finally, the organic-sulfate particles must be resistant to photochemical loss, or else they would no longer retain their organic content.

The size of aerosol particles has fundamental effects on their chemistry and radiative effects. We explore those effects using aerosol size and composition data in the lowermost stratosphere along with calculations of light scattering. In the size range between about 0.1 and 1.0 µm diameter (accumulation mode), there are at least two modes of particles in the lowermost stratosphere. The larger mode consists mostly of particles produced in the stratosphere, and the smaller mode consists mostly of particles transported from the troposphere. The stratospheric mode is similar in the Northern and Southern Hemisphere, whereas the tropospheric mode is much more abundant in the Northern Hemisphere. The purity of sulfuric acid particles in the stratospheric mode shows that there is limited production of secondary organic aerosol in the stratosphere, especially in the Southern Hemisphere. Out of eight sets of flights sampling the lowermost stratosphere (four seasons and two hemispheres) there were three with large injections of specific materials: volcanic, biomass burning, or dust. The stratospheric and tropospheric modes have very different roles for radiative effects on climate and for heterogeneous chemistry. Because the larger particles are more efficient at scattering light, most of the radiative effect in the lowermost stratosphere is due to stratospheric particles. In contrast, the tropospheric particles can have more surface area, at least in the Northern Hemisphere. The surface area of tropospheric particles could have significant implications for heterogeneous chemistry because these particles, which are partially neutralized and contain organics, do not correspond to the substances used for laboratory studies of stratospheric heterogeneous chemistry. We then extend the analysis of size-dependent properties to particles injected into the stratosphere, either intentionally or from volcanoes. There is no single size that will simultaneously maximize the climate impact relative to the injected mass, infrared heating, potential for heterogeneous chemistry, and undesired changes in direct sunlight. In addition, light absorption in the far ultraviolet is identified as an issue requiring more study for both the existing and potentially modified stratosphere.

Lung cancer screening has improved mortality among high-risk smokers but has coincidentally detected a fraction of nonprogressive adenocarcinoma historically classified as bronchoalveolar carcinoma (BAC). In the National Lung Screening Trial (NLST) the majority of BAC-comprising 29% of computed tomography-detected stage I lung adenocarcinoma-were considered overdiagnosis after extended follow-up comparison with the control arm. In the current classification, adenocarcinoma in situ and minimally invasive adenocarcinoma have replaced BAC but together comprise only ∼5% of stage I lung adenocarcinoma. Lepidic and subsets of papillary and acinar adenocarcinoma also infrequently recur. We, therefore, propose criteria for low malignant potential (LMP) adenocarcinoma among nonmucinous adenocarcinoma measuring ≤3 cm in total, exhibiting ≥15% lepidic growth, and lacking nonpredominant high-grade patterns (≥10% cribriform, ≥5% micropapillary, ≥5% solid), >1 mitosis per 2 mm2, angiolymphatic or visceral pleural invasion, spread through air spaces or necrosis. We tested these criteria in a multi-institutional cohort of 328 invasive stage I (eighth edition) and in situ adenocarcinomas and observed 16% LMP and 7% adenocarcinoma in situ/minimally invasive adenocarcinoma which together (23%) approximated the frequency of overdiagnosed stage I BAC in the NLST. The LMP group had 100% disease-specific survival. The proposed LMP criteria, incorporating multiple histologic parameters, may be a clinically useful "low-grade" prognostic group. Validation of these criteria in additional retrospective cohorts and prospective screen-detected cohorts should be considered.

Understanding the vertical distribution of aerosol helps to reduce the uncertainty in the aerosol lifecycle and therefore in the estimation of the direct and indirect aerosol forcing. To improve our understanding, we use measurements from four deployments of the Atmospheric Tomography (ATom) field campaign (ATom1-4) which systematically sampled data over the Pacific and Atlantic Oceans with near pole-to-pole coverage. We evaluate the UK Earth system model (UKESM1) against ATom observations in terms of joint biases in the vertical profile of three variables related to new particle formation: total particle number concentration (NTotal), sulphur dioxide (SO2) mixing ratio and the condensation sink. The NTotal, SO2 and condensation sink are interdependent quantities and have a controlling influence on the vertical profile of each other. Improving only one of these quantities in comparison with observations can lead to a misleading impression that overall model performance has improved. Analysing NTotal, SO2 and condensation sink simultaneously helps reduce the probability of getting the right answer for the wrong reasons. The model's condensation sink is within a factor of 2 of observations, but the NTotal and SO2 shows larger biases mainly in the tropics and high latitudes. Algorithmic improvements to the model and perturbations to key atmospheric processes help reduce tropospheric model biases consistently. We performed a series of model sensitivity tests to identify atmospheric processes that have the strongest influence on overall model performance (NTotal, SO2 and condensation sink simultaneously). In the boundary layer (which we define in this study as below 1 km altitude) and lower troposphere (1–4 km) inclusion of a boundary layer nucleation scheme (Metzger et al., 2010), which is switched off in the default version of UKESM, is critical to obtaining better agreement with observations. However, in the mid (4–8 km) and upper troposphere (> 8 km), sub-3 nm particle growth, pH of cloud droplets, DMS emissions, upper tropospheric nucleation rate, SO2 gas scavenging rate and cloud erosion rate are found to play a more dominant role. Analysing the data with altitude, we find that perturbations to boundary layer nucleation, sub 3 nm growth, cloud droplet pH and DMS emissions reduces the boundary layer and upper tropospheric model bias. We performed a combined simulation with all 4 perturbations included and found that the model's NTotal, SO2 and condensation sink biases were reduced in most cases (up to a 50 % reduction) in both the boundary layer and upper troposphere simultaneously. These perturbations are well-motivated in that they improve the physical basis of the model and are suitable for implementation in future versions of UKESM.

Brown carbon (BrC) is an organic aerosol material that preferentially absorbs light of shorter wavelengths. Global‐scale radiative impacts of BrC have been difficult to assess due to the lack of BrC observational data. To address this, aerosol filters were continuously collected with near pole‐to‐pole latitudinal coverage over the Pacific and Atlantic basins in three seasons as part of the Atmospheric Tomography Mission. BrC chromophores in filter extracts were measured. We find that globally, BrC was highly spatially heterogeneous, mostly detected in air masses that had been transported from regions of extensive biomass burning. We calculate the average direct radiative effect due to BrC absorption accounted for approximately 7% to 48% of the top of the atmosphere clear‐sky instantaneous forcing by all absorbing carbonaceous aerosols in the remote atmosphere, indicating that BrC from biomass burning is an important component of the global radiative balance., Key Points Globally, biomass burning is a large source of light‐absorbing carbonaceous aerosol that directly affect the planetary radiation balanceTransported over long distances, brown carbon is a significant component of these aerosols, but its contribution was highly variableBrown carbon contributed up to 48% of average clear‐sky instantaneous forcing by light absorption by carbonaceous aerosols

The Aerodyne Aerosol Mass Spectrometer (AMS) is a widely used instrument to quantify the composition of non-refractory submicron aerosol, in particular, organic aerosol (OA). Past comparisons, particularly of aircraft data in continental areas, have shown good overall agreement with other chemical and optical sensors. Recently, theoretically-based concerns have been raised regarding the overall AMS calibration uncertainties (particularly for OA), although there is no evidence that those apply to aircraft datasets.The ATom mission sampled the remote marine troposphere from 87S to 82N and from 0 to 12.5 km over the course of four aircraft deployments over the space of 2 years, carrying an advanced aerosol payload that included particle sizing instruments operated by NOAA ESRL, as well as several chemical sensors: UNH Mist Chamber and Filters for inorganic aerosol, NOAA SP2 for black carbon measurements, NOAA PALMS instrument for single particle composition and the CU aircraft high-resolution AMS for non-refractory submicron mass. This provides a unique opportunity to explore the agreement of the different instruments over a very large range of conditions and calibration regimes, and improve our understanding of the various instrumental uncertainties in field data.Special attention was paid to characterize the AMS size-dependent transmission with in-field calibrations; this provided crucial context when comparing with instruments with very different size cuts. Excellent agreement was found between the AMS calculated volume (including black carbon from the SP2) and the PM1 volume derived from the NOAA particle sizing measurements over three orders of magnitude (slope 0.94). The comparisons for sulfate, OA, and seasalt (the three main components of the remote PM1 aerosol) measured by AMS with the PALMS instrument showed similar consistency once differences in particle detection at different sizes were accounted for. Similarly, comparisons with sulfate from filters showed good consistency once episodes with large supermicron mass were filtered out. Comparisons of the AMS with the mist chamber sulfate were affected by the variable time response of the latter instrument but were overall consistent. Overall, no evidence for AMS calibration artifacts or unknown sources of error was found for these datasets. A comprehensive evaluation of the different sources of uncertainty and their impact on the comparisons was performed, and factors to be considered for performing such intercomparisons and improving the reliability of submicron mass quantification in the future are discussed.

Current estimates suggest that globally, about one third of low-level cloud condensation nuclei (CCN) originate from new particle formation (NPF) in the free troposphere. However, the exact mechanisms of how these new particles form and grow to CCN sizes are not yet well quantified. We investigate the formation of new particles and their initial growth in the remote marine atmosphere over the Pacific and Atlantic basins (~80 °N to ~86 °S using (1) gas-phase and size distribution measurements (0.003-4.8 µm) from the airborne-based NASA Atmospheric Tomography global survey (ATom; 2016-2018), (2) back trajectory data, and (3) two aerosol microphysics box models.In the ATom observations, newly formed particles were ubiquitous at high altitudes throughout broad regions of the tropics and subtropics under low condensation sink conditions and were associated with upwelling in convective clouds. This pattern was observed over four seasons and both ocean basins.In this study, we explore processes that govern NPF and growth in the tropical and subtropical free troposphere, discuss similarities and differences in NPF over both ocean basins, use box models to examine which nucleation schemes (e.g. binary, ternary, or charged) best explain the observations, and evaluate whether sulfuric acid precursors alone can explain the NPF and the initial particle growth. Comparing aerosol size distribution measurements with box model simulations shows that none of the NPF schemes commonly used in global models are consistent with observations, regardless of precursor concentrations. Newer schemes that incorporate organic compounds as nucleating or growth agents can plausibly replicate the observed size distributions. We conclude that organic precursor species may be particularly important in NPF in the tropical upper troposphere, even above marine regions.

6 pags., 5 figs., Dimethyl sulfide (DMS), emitted from the oceans, is the most abundant biological source of sulfur to the marine atmosphere. Atmospheric DMS is oxidized to condensable products that form secondary aerosols that affect Earth’s radiative balance by scattering solar radiation and serving as cloud condensation nuclei. We report the atmospheric discovery of a previously unquantified DMS oxidation product, hydroperoxymethyl thioformate (HPMTF, HOOCH2SCHO), identified through global-scale airborne observations that demonstrate it to be a major reservoir of marine sulfur. Observationally constrained model results show that more than 30% of oceanic DMS emitted to the atmosphere forms HPMTF. Coincident particle measurements suggest a strong link between HPMTF concentration and new particle formation and growth. Earth's radiation budget is affected by new particle formation (NPF) and the growth of these nanometre-scale particles to larger sizes where they can directly scatter light or act as cloud condensation nuclei (CCN). Large uncertainties remain in the magnitude and spatiotemporal distribution of nucleation (less than 10 nm diameter) and Aitken (10–60 nm diameter) mode particles. Acquiring size-distribution measurements of these particles over large regions of the free troposphere is most easily accomplished with research aircraft. We report on the design and performance of an airborne instrument, the nucleation mode aerosol size spectrometer (NMASS), which provides size-selected aerosol concentration measurements that can be differenced to identify aerosol properties and processes or inverted to obtain a full size distribution between 3 and 60 nm. By maintaining constant downstream pressure the instrument operates reliably over a large range of ambient pressures and during rapid changes in altitude, making it ideal for aircraft measurements from the boundary layer to the stratosphere. We describe the modifications, operating principles, extensive calibrations, and laboratory and in-flight performance of two NMASS instruments operated in parallel as a 10-channel battery of condensation particle counters (CPCs) in the NASA Atmospheric Tomography Mission (ATom) to investigate NPF and growth to cloud-active sizes in the remote free troposphere. An inversion technique to obtain size distributions from the discrete concentrations of each NMASS channel is described and evaluated. Concentrations measured by the two NMASS instruments flying in parallel are self-consistent and also consistent with measurements made with an optical particle counter. Extensive laboratory calibrations with a range of particle sizes and compositions show repeatability of the response function of the instrument to within 5–8 % and no sensitivity in sizing performance to particle composition. Particle number, surface area, and volume concentrations from the data inversion are determined to better than 20 % for typical particle size distributions. The excellent performance of the NMASS systems provides a strong analytical foundation to explore NPF around the globe in the ATom dataset.

Atmospheric aerosol is a key component of the chemistry and climate of the Earth's atmosphere. Accurate measurement of the concentration of atmospheric particles as a function of their size is fundamental to investigations of particle microphysics, optical characteristics, and chemical processes. We describe the modification, calibration, and performance of two commercially available, Ultra-High Sensitivity Aerosol Spectrometers (UHSASs) as used on the NASA DC-8 aircraft during the Atmospheric Tomography Mission (ATom). To avoid sample flow issues related to pressure variations during aircraft altitude changes, we installed a laminar flow meter on each instrument to measure sample flow directly at the inlet as well as flow controllers to maintain constant volumetric sheath flows. In addition, we added a compact thermodenuder operating at 300 °C to the inlet line of one of the instruments. With these modifications, the instruments are capable of making accurate (ranging from 7 % for Dp Dp > 0.13 µm), precise ( 1000 to 225 hPa, while simultaneously providing information on particle volatility.We assessed the effect of uncertainty in the refractive index (n) of ambient particles that are sized by the UHSAS assuming the refractive index of ammonium sulfate (n = 1.52). For calibration particles with n between 1.44 and 1.58, the UHSAS diameter varies by +4/−10 % relative to ammonium sulfate. This diameter uncertainty associated with the range of refractive indices (i.e., particle composition) translates to aerosol surface area and volume uncertainties of +8.4/−17.8 and +12.4/−27.5 %, respectively. In addition to sizing uncertainty, low counting statistics can lead to uncertainties of 1000 cm−3.Examples of thermodenuded and non-thermodenuded aerosol number and volume size distributions as well as propagated uncertainties are shown for several cases encountered during the ATom project. Uncertainties in particle number concentration were limited by counting statistics, especially in the tropical upper troposphere where accumulation-mode concentrations were sometimes −3 (counting rates ∼ 5 Hz) at standard temperature and pressure.

The CLOUD (Cosmics Leaving OUtdoor Droplets) experiment at CERN (European Council for Nuclear Research) investigates the nucleation and growth of aerosol particles under atmospheric conditions and their activation into cloud droplets. A key feature of the CLOUD experiment is precise control of the experimental parameters. Temperature uniformity and stability in the chamber are important since many of the processes under study are sensitive to temperature and also to contaminants that can be released from the stainless steel walls by upward temperature fluctuations. The air enclosed within the 26 m3 CLOUD chamber is equipped with several arrays (strings) of high precision, fast-response thermometers to measure its temperature. Here we present a study of the air temperature uniformity inside the CLOUD chamber under various experimental conditions. Measurements were performed under calibration conditions and run conditions, which are distinguished by the flow rate of fresh air and trace gases entering the chamber at 20 and up to 210 L min−1, respectively. During steady-state calibration runs between −70 and +20 °C, the air temperature uniformity is better than ±0.06 °C in the radial direction and ±0.1 °C in the vertical direction. Larger non-uniformities are present during experimental runs, depending on the temperature control of the make-up air and trace gases (since some trace gases require elevated temperatures until injection into the chamber). The temperature stability is ±0.04 °C over periods of several hours during either calibration or steady-state run conditions. During rapid adiabatic expansions to activate cloud droplets and ice particles, the chamber walls are up to 10 °C warmer than the enclosed air. This results in temperature differences of ±1.5 °C in the vertical direction and ±1 °C in the horizontal direction, while the air returns to its equilibrium temperature with a time constant of about 200 s.

Single-particle mass spectrometry (SPMS) instruments characterize the composition of individual aerosol particles in real time. Their fundamental ability to differentiate the externally mixed particle types that constitute the atmospheric aerosol population enables a unique perspective into sources and transformation. However, quantitative measurements by SPMS systems are inherently problematic. We introduce a new technique that combines collocated measurements of aerosol composition by SPMS and size-resolved absolute particle concentrations on aircraft platforms. Quantitative number, surface area, volume, and mass concentrations are derived for climate-relevant particle types such as mineral dust, sea salt, and biomass burning smoke. Additionally, relative ion signals are calibrated to derive mass concentrations of internally mixed sulfate and organic material that are distributed across multiple particle types. The NOAA Particle Analysis by Laser Mass Spectrometry (PALMS) instrument measures size-resolved aerosol chemical composition from aircraft. We describe the identification and quantification of nine major atmospheric particle classes, including sulfate–organic–nitrate mixtures, biomass burning, elemental carbon, sea salt, mineral dust, meteoric material, alkali salts, heavy fuel oil combustion, and a remainder class. Classes can be sub-divided as necessary based on chemical heterogeneity, accumulated secondary material during aging, or other atmospheric processing. Concentrations are derived for sizes that encompass the accumulation and coarse size modes. A statistical error analysis indicates that particle class concentrations can be determined within a few minutes for abundances above ∼10 ng m−3. Rare particle types require longer sampling times. We explore the instrumentation requirements and the limitations of the method for airborne measurements. Reducing the size resolution of the particle data increases time resolution with only a modest increase in uncertainty. The principal limiting factor to fast time response concentration measurements is statistically relevant sampling across the size range of interest, in particular, sizes D µm for accumulation-mode studies and D > 2 µm for coarse-mode analysis. Performance is compared to other airborne and ground-based composition measurements, and examples of atmospheric mineral dust concentrations are given. The wealth of information afforded by composition-resolved size distributions for all major aerosol types represents a new and powerful tool to characterize atmospheric aerosol properties in a quantitative fashion.

From 2016–2018 a DC-8 aircraft operated by the U.S. National Aeronautics and Space Administration (NASA) made four series of flights, profiling the atmosphere from 150 m to ~ 12 km above sea level from the Arctic to the Antarctic over both the Pacific and Atlantic Oceans. This program, the Atmospheric Tomography (ATom) mission, sought to sample the troposphere in a representative manner, making measurements of atmospheric composition in each season. This paper describes the aerosol microphysical measurements and derived quantities obtained during this mission. Dry size distributions from 2.7 nm to 4.8 µm in diameter were measured in-situ at 1 Hz using a battery of instruments: 10 condensation particle counters with different nucleation diameters, two ultra-high sensitivity aerosol size spectrometers (UHSAS), one of which measured particles surviving heating to 300 °C, and a laser aerosol spectrometer (LAS). The dry aerosol measurements were complemented by size distribution measurements from 0.5–930 µm diameter at near-ambient conditions using a cloud, aerosol, and precipitation spectrometer (CAPS) mounted under the wing of the DC-8. Dry aerosol number, surface area, and volume, and optical scattering and asymmetry parameter at several wavelengths from the near-UV to the near-IR were calculated from the measured dry size distributions (2.7 nm to 4.8 µm). Dry aerosol mass was estimated by combining the size distribution data with particle density estimated from independent measurements of aerosol composition with a high-resolution aerosol mass spectrometer and a single particle soot photometer. This paper briefly describes the instrumentation and fully documents the aircraft inlet and flow distribution system, the derivation of uncertainties, and the calculation of data products from combined size distributions. Comparisons between the instruments and direct measurements of some aerosol properties confirm that in-flight performance was consistent with calibrations and within stated uncertainties for the two deployments analyzed. Cloud condensation nuclei (CCN) can affect cloud properties and therefore Earth’s radiative balance1–3. New particle formation (NPF) from condensable vapours in the free troposphere has been suggested to contribute to CCN, especially in remote, pristine atmospheric regions4, but direct evidence is sparse, and the magnitude of this contribution is uncertain5–7. Here we use in situ aircraft measurements of vertical profiles of aerosol size distributions to present a global-scale survey of NPF occurrence. We observe intense NPF at high altitudes in tropical convective regions over both Pacific and Atlantic oceans. Together with the results of chemical-transport models, our findings indicate that NPF persists at all longitudes as a global-scale band in the tropical upper troposphere, covering about 40 per cent of Earth’s surface. Furthermore, we find that this NPF in the tropical upper troposphere is a globally important source of CCN in the lower troposphere, where CCN can affect cloud properties. Our findings suggest that the production of CCN as new particles descend towards the surface is not adequately captured in global models, which tend to underestimate both the magnitude of tropical upper tropospheric NPF and the subsequent growth of new particles to CCN sizes. Widespread formation of new particles from condensable vapours observed in the tropical upper troposphere is an important source of cloud condensation nuclei in the lower troposphere, affecting cloud properties.

We present the first data on the concentration of sea-salt aerosol throughout most of the depth of the troposphere and over a wide range of latitudes, which were obtained during the Atmospheric Tomography (ATom) mission. Sea-salt concentrations in the upper troposphere are very small, usually less than 10 ng per standard m3 (about 10 parts per trillion by mass) and often less than 1 ng m−3. This puts stringent limits on the contribution of sea-salt aerosol to halogen and nitric acid chemistry in the upper troposphere. Within broad regions the concentration of sea-salt aerosol is roughly proportional to water vapor, supporting a dominant role for wet scavenging in removing sea-salt aerosol from the atmosphere. Concentrations of sea-salt aerosol in the winter upper troposphere are not as low as in the summer and the tropics. This is mostly a consequence of less wet scavenging in the drier, colder winter atmosphere. There is also a source of sea-salt aerosol over pack ice that is distinct from that over open water. With a well-studied and widely distributed source, sea-salt aerosol provides an excellent test of wet scavenging and vertical transport of aerosols in chemical transport models.

Objective Lung cancer screening with low-dose chest computed tomography improves survival. However, concerns about overdiagnosis and unnecessary interventions persist. We reviewed our lung cancer screening program to determine the rate of surgery and invasive procedures for nonmalignant disease. Methods We reviewed all patients undergoing lung cancer screening from January 2012 to June 2017 with follow-up through January 2019. Patients with suspicious findings (Lung CT Screening Reporting and Data System 4) were referred for further evaluation. Results Of 3280 patients screened, 345 (10.5%) had Lung CT Screening Reporting and Data System 4 findings. A total of 311 patients had complete follow-up, of whom 93 (29.9%) were diagnosed with lung cancer. Eighty-three patients underwent lung surgery (2.5% of screened patients). Forty patients underwent lobectomy (48.2%), 3 patients (3.6%) underwent bilobectomy, and 40 patients (48.2%) underwent sublobar resection. Fourteen patients underwent surgery for benign disease (0.43% of screened patients). Fifty-four patients, 5 with benign disease, had at least 1 invasive diagnostic procedure but never underwent surgery. The incidence of any invasive intervention for nonmalignant disease was 0.95% (31/3280 patients). There were no postprocedural deaths within 60 days. Twenty-five patients (0.76%) underwent stereotactic body radiation therapy; 19 patients (76%) had presumed lung cancer without pretreatment pathologic confirmation. Conclusions Surgical resection for benign disease occurred in 0.43% of patients undergoing lung cancer screening. The combined incidence of any invasive diagnostic or therapeutic intervention, including surgical resection, for benign disease was only 0.95%. Periprocedural complications were rare. These results indicate that concern over unnecessary interventions is overstated and should not hinder adoption of lung cancer screening. A multidisciplinary team approach, including thoracic surgeons, is critical to maintain an appropriate rate of interventions in lung cancer screening.

Many of the particles in the troposphere are formed in situ but what fraction of all tropospheric particles do they constitute and how exactly are they made? Bianchi et al. report results from a high altitude research station. Roughly half of the particles were newly formed by the condensation of highly oxygenated multifunctional compounds. A combination of laboratory results field measurements and model calculations revealed that neutral nucleation is more than 10 times faster than ion induced nucleation that particle growth rates are size dependent and that new particle formation occurs during a limited time window.Science this issue p. 1109New particle formation (NPF) is the source of over half of the atmosphere’s cloud condensation nuclei thus influencing cloud properties and Earth’s energy balance. Unlike in the planetary boundary layer few observations of NPF in the free troposphere exist. We provide observational evidence that at high altitudes NPF occurs mainly through condensation of highly oxygenated molecules (HOMs) in addition to taking place through sulfuric acid–ammonia nucleation. Neutral nucleation is more than 10 times faster than ion induced nucleation and growth rates are size dependent. NPF is restricted to a time window of 1 to 2 days after contact of the air masses with the planetary boundary layer; this is related to the time needed for oxidation of organic compounds to form HOMs. These findings require improved NPF parameterization in atmospheric models.

Convective systems dominate the vertical transport of aerosols and trace gases. The most recent in situ aerosol measurements presented here show that the concentrations of primary aerosols including sea salt and black carbon drop by factors of 10 to 10,000 from the surface to the upper troposphere. In this study we show that the default convective transport scheme in the National Science Foundation/Department of Energy Community Earth System Model results in a high bias of 10–1,000 times the measured aerosol mass for black carbon and sea salt in the middle and upper troposphere. A modified transport scheme, which considers aerosol activation from entrained air above the cloud base and aerosol‐cloud interaction associated with convection, dramatically improves model agreement with in situ measurements suggesting that deep convection can efficiently remove primary aerosols. In the ancient Greek world contact with the divine was primarily regulated through the ritual of sacrifice and the exchange it represented. Temples were not essential to places of cult, but those that had them were clearly dominated by them and they lent prestige to their local communities. Once studied primarily for their exterior architecture and perfection of design, temples are increasingly considered an integral part of the ritual context of the Greek sanctuary. This extends to their interiors and the rituals that took place there that revolved around the cult image, such as prayer. All of this makes the question of the role of their doorways especially pertinent. This chapter briefly examines the function of Greek temples and the issue of how accessible they were to the public at large, as well as the general attraction of the entrance in relation to the cult image. Although the cult images were considered as works of art, they were at the same time imbued with the energeia of the divine. Doorways did much more than provide access -- they illumined and framed the image, preparing the visitor for the epiphany. Concepts such as the 'doorway effect' and 'guided perception' are used to explore how this worked; two exceptional temples, the Knidian sanctuary of Aphrodite and the temple of Apollo at Didyma, further serve to highlight the pivotal role of doorways in the conception of the divine.

Importance Clinicians rely heavily on fluorodeoxyglucose F18–labeled positron emission tomography (FDG-PET) imaging to evaluate lung nodules suspicious for cancer. We evaluated the performance of FDG-PET for the diagnosis of malignancy in differing populations with varying cancer prevalence. Objective To determine the performance of FDG-PET/computed tomography (CT) in diagnosing lung malignancy across different populations with varying cancer prevalence. Design, Setting, and Participants Multicenter retrospective cohort study at 6 academic medical centers and 1 Veterans Affairs facility that comprised a total of 1188 patients with known or suspected lung cancer from 7 different cohorts from 2005 to 2015. Exposures 18F fluorodeoxyglucose PET/CT imaging. Main Outcome and Measures Final diagnosis of cancer or benign disease was determined by pathological tissue diagnosis or at least 18 months of stable radiographic follow-up. Results Most patients were male smokers older than 60 years. Overall cancer prevalence was 81% (range by cohort, 50%-95%). The median nodule size was 22 mm (interquartile range, 15-33 mm). Positron emission tomography/CT sensitivity and specificity were 90.1% (95% CI, 88.1%-91.9%) and 39.8% (95% CI, 33.4%-46.5%), respectively. False-positive PET scans occurred in 136 of 1188 patients. Positive predictive value and negative predictive value were 86.4% (95% CI, 84.2%-88.5%) and 48.7% (95% CI, 41.3%-56.1%), respectively. On logistic regression, larger nodule size and higher population cancer prevalence were both significantly associated with PET accuracy (odds ratio, 1.027; 95% CI, 1.015-1.040 and odds ratio, 1.030; 95% CI, 1.021-1.040, respectively). As the Mayo Clinic model–predicted probability of cancer increased, the sensitivity and positive predictive value of PET/CT imaging increased, whereas the specificity and negative predictive value dropped. Conclusions and Relevance High false-positive rates were observed across a range of cancer prevalence. Normal PET/CT scans were not found to be reliable indicators of the absence of disease in patients with a high probability of lung cancer. In this population, aggressive tissue acquisition should be prioritized using a comprehensive lung nodule program that emphasizes advanced tissue acquisition techniques such as CT-guided fine-needle aspiration, navigational bronchoscopy, and endobronchial ultrasonography.

Evaluation of indeterminate pulmonary nodules is a complex challenge. Most are benign but frequently undergo invasive and costly procedures to rule out malignancy. A plasma protein classifier was developed that identifies likely benign nodules that can be triaged to CT surveillance to avoid unnecessary invasive procedures. The clinical utility of this classifier was assessed in a prospective–retrospective analysis of a study enrolling 475 patients with nodules 8–30 mm in diameter who had an invasive procedure to confirm diagnosis at 12 sites. Using this classifier, 32.0 % (CI 19.5–46.7) of surgeries and 31.8 % (CI 20.9–44.4) of invasive procedures (biopsy and/or surgery) on benign nodules could have been avoided. Patients with malignancy triaged to CT surveillance by the classifier would have been 24.0 % (CI 19.2–29.4). This rate is similar to that described in clinical practices (24.5 % CI 16.2–34.4). This study demonstrates the clinical utility of a non-invasive blood test for pulmonary nodules. This study presents the difference between oxidised organic compounds formed by α-pinene oxidation under various conditions in the CLOUD environmental chamber: (1) pure ozonolysis (in the presence of hydrogen as hydroxyl radical (OH) scavenger) and (2) OH oxidation (initiated by nitrous acid (HONO) photolysis by ultraviolet light) in the absence of ozone. We discuss results from three Atmospheric Pressure interface Time-of-Flight (APi-TOF) mass spectrometers measuring simultaneously the composition of naturally charged as well as neutral species (via chemical ionisation with nitrate). Natural chemical ionisation takes place in the CLOUD chamber and organic oxidised compounds form clusters with nitrate, bisulfate, bisulfate/sulfuric acid clusters, ammonium, and dimethylaminium, or get protonated. The results from this study show that this process is selective for various oxidised organic compounds with low molar mass and ions, so that in order to obtain a comprehensive picture of the elemental composition of oxidation products and their clustering behaviour, several instruments must be used. We compare oxidation products containing 10 and 20 carbon atoms and show that highly oxidised organic compounds are formed in the early stages of the oxidation.

Background and objectives There is a lack of consensus on the optimal repair technique and the definition of good outcomes in paraesophageal hernia (PEH) repair. We reviewed long-term patient-reported outcomes of open and laparoscopic PEH repair to assist with our future surgical consent process. Methods This was a retrospective case-control study including all patients with PEH repair performed from 2000 through 2012 at a single center without the use of mesh. We mailed questionnaires to patients to assess reoperation, symptom control, and satisfaction. Results Chart review identified 217 patients who underwent PEH repair. Nineteen died during the follow-up period. Of the 106 returning the questionnaire, 87 underwent laparoscopic repair, and 19 had open repair, with follow-up of 6.6 (SD 3.9) years and 7.0 (SD 4.1) years, respectively. Reoperation rates were 9.9% and 5.3%, respectively (P = .720). Dysphagia, heartburn, and regurgitation improved in 95.4% of patients after laparoscopic repair and 89.5% after open repair (P = .318). Medication for symptom control was necessary in 54.0% of patients after laparoscopic repair and 26.3% after open repair (P = .029). In each group, 90% stated that they would still choose to have the operation (P = .713). Conclusions Long-term patient-specific outcomes showed comparable, encouraging results between open and laparoscopic repair of PEH without mesh reinforcement. However, half of those undergoing laparoscopic repair required the use of medication for symptom control. This study adds to the literature describing long-term patient-specific outcomes and can be useful when counseling patients about PEH repair.

However, the concentration and molecular composition of the neutral (uncharged) clusters have not been reported so far due to the lack of suitable instrumentation. Here we report on experiments from the Cosmics Leaving Outdoor Droplets chamber at the European Organization for Nuclear Research revealing the formation of neutral particles containing up to 14 SA and 16 DMA molecules, corresponding to a mobility diameter of about 2 nm, under atmospherically relevant conditions. These measurements bridge the gap between the molecular and particle perspectives of nucleation, revealing the fundamental processes involved in particle formation and growth. The neutral clusters are found to form at or close to the kinetic limit where particle formation is limited only by the collision rate of SA molecules. Even though the neutral particles are stable against evaporation from the SA dimer onward, the formation rates of particles at 1.7-nm size, which contain about 10 SA molecules, are up to 4 orders of magnitude smaller comparedwith those of the dimer due to coagulation and wall loss of particles before they reach 1.7 nm in diameter. This demonstrates that neither the atmospheric particle formation rate nor its dependence on SA can simply be interpreted in terms of cluster evaporation or the molecular composition of a critical nucleus., Main text plus SI

Binary nucleation of sulfuric acid and water as well as ternary nucleation involving ammonia are thought to be the dominant processes responsible for new particle formation (NPF) in the cold temperatures of the middle and upper troposphere. Ions are also thought to be important for particle nucleation in these regions. However, global models presently lack experimentally measured NPF rates under controlled laboratory conditions and so at present must rely on theoretical or empirical parameterizations. Here with data obtained in the European Organization for Nuclear Research CLOUD (Cosmics Leaving OUtdoor Droplets) chamber, we present the first experimental survey of NPF rates spanning free tropospheric conditions. The conditions during nucleation cover a temperature range from 208 to 298 K, sulfuric acid concentrations between 5 × 105 and 1 × 109cm-3, and ammonia mixing ratios from zero added ammonia, i.e., nominally pure binary, to a maximumof ~1400 parts per trillion by volume (pptv).We performed nucleation studies under pure neutral conditions with zero ions being present in the chamber and at ionization rates of up to 75 ion pairs cm-3 s-1 to study neutral and ion-induced nucleation. We found that the contribution from ion-induced nucleation is small at temperatures between 208 and 248 K when ammonia is present at several pptv or higher. However, the presence of charges significantly enhances the nucleation rates, especially at 248 K with zero added ammonia, and for higher temperatures independent of NH3 levels.We compare these experimental data with calculated cluster formation rates from the Atmospheric Cluster Dynamics Code with cluster evaporation rates obtained from quantum chemistry.

International audience; The magnitude of aerosol radiative forcing caused by anthropogenic emissions depends on the baseline state of the atmosphere under Q:11 pristine preindustrial conditions. Measurements in the European Organisation for Nuclear Research (CERN) CLOUD chamber show that particle formation in atmospheric conditions can occur solely from Q:12 biogenic vapors. Here, we evaluate the potential effect of this source of particles on preindustrial cloud condensation nucleus (CCN) concentrations and aerosol cloud radiative forcing over the industrial period.Model simulations show that the pure biogenic particle formation mechanism has a much larger relative effect on CCN concentrations in the preindustrial atmosphere than in the present atmosphere because of the lower aerosol concentrations. Consequently, preindustrial cloud albedo is increased more than under present day conditions, and therefore, the cooling forcing of anthropogenic aerosols is reduced. Q:13 The mechanism increases CCN concentrations by 20–100% over a large fraction of the preindustrial lower atmosphere, and the magnitude of annual global mean radiative forcing caused by changes of cloud albedo since 1750 is reduced by 0.22 W m−² (27%) to −0.60 W m−².Model uncertainties, relatively slow formation rates, and limited available ambient measurements make it difficult to establish the significance of a mechanism that has its dominant effect under preindustrial conditions. Our simulations predict more particle formation in the Amazon than is observed. However, the first observation of pure organic nucleation has now been reported for the free troposphere. Given the potentially significant effect on anthropogenic forcing, effort should be made to better understand such naturally driven aerosol processes.
