Hordeolum and chalazion are the most frequent inflammatory eyelid tumors. They can occur in association with underlying diseases causing Meibomian gland dysfunction and/or chronic blepharitis. Due to the typical morphological features and clinical course, the diagnosis can mostly be clinically established. The majority of these lesions resolve spontaneously over time. In some instances, surgical intervention is unavoidable. In persistent, recurrent or clinically atypical cases malignant tumors must be excluded as a differential diagnosis by excisional biopsy and histopathological assessment.

Silicone oil is an established intraocular surgical aid, which enables the treatment of the most complex starting situations but no other alternative has been found; however, the available data indicate that an unclear loss of visual acuity during or after an intraocular silicone oil tamponade possibly occurs more frequently than assumed from the clinical routine. Various pathological mechanisms are under discussion as causes, but the exact causes are actually unclear. In addition to atrophic alterations in the optical coherence tomography (OCT) examination, there are a clear reduction in visual acuity and mostly a central scotoma with otherwise inconspicuous findings. Unclear loss of visual acuity can also occur after removal of the silicone oil. Whether this is caused by the same pathological mechanism is unclear. Furthermore, there are no reproducible risk factors that appear a priori to possibly cause an unclear loss of vision under silicone oil; however, oil removal as soon as possible and a good adjustment of the intraocular pressure are recommended by the authors. Overall, a silicone oil tamponade should be carefully weighed up even when using modern highly purified silicone oils and it should therefore continue to be reserved particularly for unfavorable initial situations or complicated courses with the necessity for a silicone oil tamponade. Against this background, a study for systematic recording and processing of cases of unclear loss of visual acuity after silicone oil tamponade seems to be meaningful.Silikonöl ist ein etabliertes und bis heute durch keine Alternative zu ersetzendes intraokular chirurgisches Hilfsmittel, das die Versorgung von komplexesten Ausgangssituationen ermöglicht. Verfügbare Daten weisen jedoch darauf hin, dass eine unklare Visusminderung während oder nach einer intraokularen Silikonöltamponade möglicherweise häufiger auftritt, als dies der klinische Alltag vermuten lässt. Unterschiedliche Pathomechanismen werden hierfür ursächlich diskutiert, tatsächlich sind die genauen Ursachen aber unklar. Neben atrophischen Veränderungen in der OCT(optische Kohärenztomographie)-Untersuchung finden sich ein deutlich reduzierter Visus sowie meist ein Zentralskotom bei sonst unauffälligen Befunden. Unklare Visusminderungen können auch nach einer Silikonölentfernung auftreten. Ob hier die gleichen Pathomechanismen ursächlich sind, ist unklar. Ferner gibt es keine reproduzierbaren Risikofaktoren, die eine unklare Visusminderung unter Silikonöl a priori wahrscheinlich erscheinen lassen, jedoch werden möglichst frühzeitige Ölentfernung und gute Einstellung des intraokularen Druckes von den Autoren empfohlen. Insgesamt sollte eine Silikonöltamponade auch unter Verwendung von modernen hoch-aufgereinigten Silikonölen sorgfältig abgewogen werden und damit weiterhin v. a. ungünstigen Ausgangssituationen oder komplizierten Verläufen mit Notwendigkeit einer Silikonöltamponade vorbehalten bleiben. Vor diesem Hintergrund scheint eine Studie zur systematischen Erfassung und Aufarbeitung der Fälle von unklarer Visusminderung nach Silikonöltamponade sinnvoll.

The visual system is highly complex. Monocular or binocular disorders can develop, for example during tractive alterations of the macula, such as in epimacular membranes, and result in symptoms, such as double vision, aniseikonia or metamorphopsia. These symptoms are also observed following vitreomacular or vitreoretinal surgery, especially after treatment of large retinal detachment involving the macular area using pars plana vitrectomy or scleral buckle surgery but also as a result of vitreomacular interventions, such as membrane peeling in epimacular gliosis. Some patients have preoperatively existing latent conditions, which are not explicitly addressed during routine preoperative diagnostics. For patients and surgeons it is worth noting that these troublesome symptoms are often manifested in patients with good restoration of best corrected visual acuity following vitreomacular surgery. In such cases, patients tend to be dissatisfied with the surgical result despite good visual acuity. Treatment of postoperative metamorphopsia, aniseikonia and double vision is often difficult. In clinical studies, the maximum depth of tractive retinal folds has been described as a reliable morphological biomarker for the occurrence of metamorphopsia in epimacular gliosis. As metamorphopsia tends to postoperatively resolve slowly and even persist in ca. 30% of cases, this biomarker can have an impact on preoperative consulting and postoperative monitoring. Persistent binocular discomfort is often treatable by exact refraction correction and by applying a vertical and, if necessary, a horizontal prism. This often requires patience on the part of both the patient and the physician but is mostly successful due to fusion and adaptability.Das visuelle System ist sehr komplex. Monokulare oder binokulare Störungen können sich z. B. im Rahmen traktiver Veränderungen der Makula wie bei epimakulärer Membran entwickeln und sich symptomatisch als Metamorphopsie, Aniseikonie oder auch Doppelbild bemerkbar machen. Diese Symptome finden sich auch nach vitreomakulärer bzw. vitreoretinaler Chirurgie, besonders nach der Behandlung großflächiger Ablationes mit Makulabeteiligung mittels Vitrektomie oder Buckelchirurgie, aber auch in der Folge vitreomakulärer Eingriffe wie Membranpeeling bei epimakulärer Gliose. Bei manchen Patienten bestehen präoperative latente Befunde, die in der präoperativen Routinediagnostik nicht explizit eruiert werden. Für Patient und Operateur besonders bemerkenswert ist die Tatsache, dass diese störenden Symptome besonders dann zutage treten, wenn die postoperative bestkorrigierte Sehschärfe gut ansteigt, also ein quoad visum gutes Ergebnis erzielt wurde. Eine Behandlung postoperativ auftretender Doppelbilder, Metamorphopsien oder Aniseikonie gestaltet sich oft schwierig. Als reliabler morphologischer Biomarker für das Auftreten von Metamorphopsien im Rahmen der epimakulären Gliose wurde in klinischen Studien die maximale Tiefe der traktiven Netzhautfalten beschrieben. Da Metamorphopsien sich postoperativ tendenziell langsam zurückbilden und in ca. 30 % der Fälle sogar persistieren, kann dieser Biomarker bei der Beratung vor einer Revisionsoperation und im Rahmen der postoperativen Verlaufsbeobachtung hilfreich sein. Persistierende Binokularbeschwerden sind im Intervall mit exakter Refraktionskorrektion und einem Vertikal- und ggf. Horizontalprisma häufig behandelbar. Dies erfordert zwar oft eine gewisse Geduld aufseiten des Patienten und Arztes, ist aber dank Fusion und Adaptationsfähigkeit meist erfolgreich.

The incidence of retinal folds following surgical repair of retinal detachment is underestimated. The extent of retinal folds is variable and can include the complete retina with all layers and an apposition of the photoreceptor outer segments or only partially affect the outer or inner retinal layers. While complete folds can be relatively easily detected by clinical examination, discrete partial folds are sometimes difficult to assess biomicroscopically. In these cases, high-resolution optical coherence tomography (OCT) plays an important role as a tool for differential diagnosis. If macular translocation occurs during the formation of folds, mostly in associated with inferior retinal folds, patients often complain of binocular double vision. A significant reduction of visual acuity and metamorphopsia occur in cases where the folding involves the fovea. In general, retinal folds tend to resolve spontaneously over a prolonged period of follow-up of several months; however, in cases of foveal involvement and corresponding symptoms, a surgical revision can be indicated, although the surgical procedure is not standardized.Netzhautfalten nach chirurgischer Versorgung einer Netzhautablösung treten häufiger auf als erwartet. Das Ausmaß der Falten ist sehr variabel und kann die gesamte Netzhaut mit allen Schichten und einer Apposition der Photorezeptoraußensegmente oder nur partiell die äußere oder innere Netzhaut betreffen. Während komplette Falten relativ einfach im Rahmen einer klinischen Untersuchung detektiert werden können, sind diskrete, partielle Falten biomikroskopisch teils schwer zu beurteilen. Hier besitzt die hochauflösende optische Kohärenztomographie (OCT) zur differenzialdiagnostischen Beurteilung einen besonderen Stellenwert. Kommt es im Rahmen der Faltenbildung zu einer Translokation der Makula, meist verbunden mit einer Faltenbildung inferior, treten oft binokulare Doppelbilder auf. Eine deutliche Visuseinschränkung und Metamorphopsien bestehen in Fällen, in denen die Faltenbildung die Fovea mit einbezieht. Grundsätzlich zeigen Netzhautfalten über einen Beobachtungszeitraum von mehreren Monaten einen positiven Spontanverlauf. In Fällen mit fovealer Beteiligung und entsprechender Symptomatik kann eine chirurgische Revisionsoperation erwogen werden. Das chirurgische Vorgehen ist hierbei nicht standardisiert.

Advances in imaging techniques of the retina have substantially enhanced our current understanding of the pathogenesis, morphology and prognosis of vitreomacular retinal diseases. Optical coherence tomography-based criteria and classification systems were recently proposed for uniform diagnoses and treatment recommendations for patients with vitreomacular traction, epiretinal gliosis and the various forms of macular holes. This article provides an overview of the different retinal imaging modalities as well as the currently recommended classification for vitreomacular traction pathologies.

Persistierende, grose oder chronische Makulaforamina sind nach wie vor eine chirurgische Herausforderung, die mit der hier dargestellten Technik angegangen werden kann. Im Rahmen einer transkonjunktivalen, nahtlosen Pars-plana-Vitrektomie wird mittels subretinaler BSS(„balanced salt solution“)-Injektion am hinteren Pol die Makularegion eleviert und anschliesend das Foramen wahrend des Flussigkeit-Luft-Austausches verschlossen, gefolgt von einer Gas- oder Oltamponade. Damit lassen sich in vielen Fallen anatomische Verschlusse bzw. deutliche Verkleinerungen des Loches mit anliegendem Rand erreichen. Die genaue Aufklarung der Patienten uber das zu erwartende chirurgische Ergebnis erscheint besonders wichtig.

Persisting macular holes (PMH) after surgical release of any epiretinal traction of the vitreous and adjacent membrane may rely on secondary firm adhesions between the retracted retina and adjacent retinal pigment epithelium. Secondary application of subretinal (SR)-fluid may release these adhesions followed by an anatomical closure. Twelve surgeons applied in a consecutive case series SR-fluid in 41 eyes with PMH and reported retrospectively their initial surgical, anatomical and functional experience with this approach. The mean duration of the MH prior to SR-fluid application was 17 months (6–96 months). The mean age of the patients at the time of surgery was 72 years (54–88). The mean preoperative aperture diameter of the opening was 1212 μm (239–4344 μm), base diameter 649 μm (SD 320 μm). The mean preoperative BCVA prior to surgery was 0.1 (0.01–0.3). All patients (41/41) complained about reduced BCVA and a significant central scotoma (negative scotoma) in their central field of vision. The secondary closure rate for our PMH was 85.36% (35 out of 41 eyes) at 6 weeks after surgery. The postoperative BCVA improved to 0.22 (0.02–0.5). The application of SR-fluid was not associated with major intraoperative adverse effects. Remaining SR-adhesions may inhibit PMH closure. Their release by application of SR-fluid will lead to a fast and immediate anatomical closure in many cases without serious adverse events.

Peripheral exudative hemorrhagic chorioretinopathy (PEHCR) is a rare degenerative chorioretinal disease with subretinal or subepithelial hemorrhage and exudation in the fundus periphery in older patients. Even though it is the second most common entity among pseudomelanomas and its characteristic features simplify diagnosis, PEHCR is often not recognized and therefore misdiagnosed. In most cases stabilization or regression spontaneously occur. Therefore, treatment for PEHCR should be cautiously selected when function is good and surgical intervention should be preferably considered when visual loss is imminent.Die periphere exsudative hämorrhagische Chorioretinopathie (PEHCR) ist eine seltene degenerative Netzhaut-Aderhaut-Erkrankung mit subretinaler oder subepithelialer Blutung und Exsudation in der Fundusperipherie bei älteren Patienten. Obwohl es sich hierbei um die zweithäufigste Entität unter den Pseudomelanomen handelt und ihre charakteristischen Merkmale die Diagnosestellung vereinfachen, wird sie häufig nicht erkannt und daher fehldiagnostiziert. In den meisten Fällen kommt es zu einer Stabilisierung oder spontanen Regression. Aus diesem Grund sollte die Therapie der PEHCR bei guter Funktion zurückhaltend und eine chirurgische Intervention bevorzugt bei drohendem Visusverlust in Betracht gezogen werden.

BackgroundA consensus on an optical coherence tomography definition of lamellar macular hole (LMH) and similar conditions is needed.MethodsThe panel reviewed relevant peer-reviewed literature to reach an accord on LMH definition and to differentiate LMH from other similar conditions.ResultsThe panel reached a consensus on the definition of three clinical entities: LMH, epiretinal membrane (ERM) foveoschisis and macular pseudohole (MPH). LMH definition is based on three mandatory criteria and three optional anatomical features. The three mandatory criteria are the presence of irregular foveal contour, the presence of a foveal cavity with undermined edges and the apparent loss of foveal tissue. Optional anatomical features include the presence of epiretinal proliferation, the presence of a central foveal bump and the disruption of the ellipsoid zone. ERM foveoschisis definition is based on two mandatory criteria: the presence of ERM and the presence of schisis at the level of Henle’s fibre layer. Three optional anatomical features can also be present: the presence of microcystoid spaces in the inner nuclear layer (INL), an increase of retinal thickness and the presence of retinal wrinkling. MPH definition is based on three mandatory criteria and two optional anatomical features. Mandatory criteria include the presence of a foveal sparing ERM, the presence of a steepened foveal profile and an increased central retinal thickness. Optional anatomical features are the presence of microcystoid spaces in the INL and a normal retinal thickness.ConclusionsThe use of the proposed definitions may provide uniform language for clinicians and future research.

Zusammenfassung Hintergrund In Deutschland sind 2 intravitreal anzuwendende Kortikosteroide (Dexamethason und Fluocinolon) für die Behandlung des diabetischen Makulaödems (DMÖ) zugelassen. Der Einsatz von DEX-Implantat bei DMÖ ist in der täglichen Praxis bisher nicht im Detail definiert. Mithilfe eines Delphi-Panels und einer Runde von Retinaspezialisten sollte ein Konsens für die Anwendung von DEX-Implantat erarbeitet werden. Material und Methoden Aus der Literatur wurden internationale und nationale Behandlungsempfehlungen identifiziert. Eine Steuerungsgruppe erarbeitete einen Katalog von 72 Aussagen zur Ätiologie und Pathogenese des DMÖ, Therapie mit dem DEX-Implantat, Einsatz bei mit VEGF-Hemmern vorbehandelten Patienten, Verwendung von DEX-Implantat in der Kombination mit anderen Therapien, Sicherheit von DMÖ-Therapien sowie Therapiebelastung des Patienten. 22 niedergelassene Ophthalmologen und 6 Ophthalmologen aus der Klinik gaben über Survey Monkey ihre Bewertung zu den Aussagen ab. Die Definition des Konsenses zu einer Aussage galt als erfüllt, wenn mehr als 75% der Befragten einer Aussage zustimmten bzw. nicht zustimmten. Über nicht konsensfähige Aussagen wurde nach erneuter Diskussion in der Konsensrunde nochmals abgestimmt. In der Folge sollte auch ein Behandlungsschema für DMÖ mit fovealer Beteiligung vorgeschlagen werden. Ergebnisse Sofern ein Patient unter VEGF-Hemmern unzureichend anspricht (Visusgewinn Schlussfolgerungen Der hier vorgestellte Algorithmus entspricht den Ergebnissen des Delphi-Prozesses und weicht u. U. von den Empfehlungen der Fachgesellschaft ab. Die im Rahmen der Befragung und Expertenrunde erarbeiteten Konsensempfehlungen für die Therapie von DMÖ können für den Einsatz des DEX-Implantats in der täglichen Praxis dienen.

Die Entscheidung, ob es sich um eine Bagatelle oder um einen ernstzunehmenden Notfall handelt, kann in einem hausarztlichen Setting sehr schwierig sein. Dieser Artikel dient zur Ubersicht und als Leitfaden im Management der wichtigsten Augennotfalle.

Die moderne Makulaforamenchirurgie fuhrt zu hohen Verschlussraten von uber 90 % und guten funktionellen Ergebnissen, besonders bei Makulaforamina bis 400 µm Durchmesser. Fur die meisten dieser Falle haben sich als chirurgische Technik die transkonjunktivale nahtlose Pars-plana-Vitrektomie mit ILM(innere Grenzmembran)-Peeling um das Foramen und Gastamponade mit anschliesender Lagerung des Patienten etabliert. Da bei Makulaforamina ab ca. 400 µm mit zunehmender Lochgrose besonders die Verschlussraten sinken, aber auch das funktionelle Ergebnis schlechter ist, wurden alternative Techniken entwickelt, um das anatomische und damit auch das funktionelle Ergebnis zu verbessern. Diese Techniken umfassen das Einbringen von Gewebe in das Makulaforamen, um so einen verbesserten Verschluss des Foramens zu erreichen. Dies gelingt mit ILM-Gewebe im Rahmen der ILM-Flap- oder Free-Flap-Technik sowie der Transplantation von autologer Netzhaut oder Linsenkapsel bzw. homologem Amniongewebe in den bzw. unter den Defekt. Ein weiterer vielversprechender Ansatz ist die Annaherung der Lochrander durch die Mobilisierung der Netzhaut am hinteren Pol um das Foramen durch Induktion einer umschriebenen Netzhautablosung durch Injektion von BSS („balanced salt solution“) in den subretinalen Raum mittels einer 41-Gauge-Kanule. Abgeschlossen wird die Operation jeweils durch eine Endotamponade mit Gas oder Silikonol.

Die Diagnose wird aufgrund des morphologischen Befundes und der Anamnese meist klinisch gestellt. Die Mehrzahl zeigt eine gute Spontanheilung. In einigen Fällen ist eine chirurgische Intervention unumgänglich. Bei persistierenden, rezidivierenden oder atypischen Verläufen sollte immer ein Malignom als Differenzialdiagnose in Erwägung gezogen werden und eine Biopsie mit histopathologischer Aufarbeitung erfolgen.

Dieser Beitrag bietet sowohl einen Überblick über die verschiedene Modalitäten retinaler Bildgebung als auch über aktuell empfohlene Einteilungen bei traktiven Vitreomakulopathien.

Large, persistent or chronic macular holes still represent a surgical challenge, which can be addressed using the presented surgical technique. A subretinal injection of balanced salt solution (BSS) at the posterior pole of the macular region is performed during transconjunctival pars plana vitrectomy in order to induce an elevation of the neurosensory retina in the macular region. The hole is then attenuated during fluid-air exchange, followed by a gas or silicone oil tamponade. The presented technique appears to be useful for large and persistent macular holes, which are not suitable for conventional macular hole surgery. We aimed to estimate the incidence and prevalence of sVMA in Germany.Materials and methods: A systematic literature review was conducted to identify the incidence and prevalence of sVMA based on international epidemiologic studies, weighted for study size and then averaged across eligible studies. A second systematic review aimed to estimate the proportion of vitrectomy undertaken for sVMA in Germany. This was combined with the reported number of vitrectomies in Germany in 2016 to estimate the number of patients undergoing vitrectomy for sVMA.Results: The prevalence of sVMA is 1,365 per 100,000 population, with an incidence of 6.96 per 100,000 per year. For Germany, this translates to 1,119,300 cases, with 5,700 new cases reported annually. Analyzing the national hospital statistics, ~2,300 patients undergo vitrectomy due to sVMA in Germany each year, of which about 1,700 patients have VMT.Conclusion: Incidence figures, driven by patients presenting to clinic, are much lower than expected based on population-based studies, suggesting that many patients with sVMA exist outside of the clinic system. Zusammenfassung Hintergrund Bei der Behandlung eines diabetischen Makulaödems mit fovealer Beteiligung kann es trotz intensiver und konsequenter initialer Therapie mit einem Anti-VEGF-Medikament zu einem unzureichenden Ansprechen kommen. Für eine weitere Therapie stehen derzeit ein Wechsel des Anti-VEGF-Präparates oder die intravitreale Gabe der Steroidimplantate Fluocinolonacetonid oder Dexamethason zur Verfügung. Es soll zum einen systematisch untersucht werden, welche Studiendaten evidenzbasiert zur Wirksamkeit einer zugelassenen Therapie nach vorangegangener Anti-VEGF-Therapie vorliegen, und zum anderen, welche Ausgaben für die Krankenversicherungen mit einer weiteren intravitrealen Therapie verbunden sind. Material und Methoden Es erfolgte eine systematische Literatursuche (SLR) zu randomisierten, kontrollierten Studien (RCT) in Medline und Embase. Ein Kurzzeitkostenmodell in MS Excel mit 3 Jahren Zeithorizont wurde erstellt, um die zugelassenen Optionen Ranibizumab (Lucentis®), Aflibercept (Eylea®), Fluocinolonacetonid-Implantat (Iluvien®) und Dexamethason-Implantat (Ozurdex®) zu vergleichen. Kostenkomponenten sind Arzneimittel- und Injektionskosten, optische Kohärenztomografie (OCT) und unerwünschte Ereignisse wie Endophthalmitis, den Augeninnendruck senkende Medikation bzw. Eingriffe oder Kataraktoperationen. Ergebnisse Es wurden 42 Publikationen zu 20 RCTs identifiziert, wobei keine eine Population nach vorangegangener Anti-VEGF-Therapie hatte, sodass kein direkter Vergleich der Wirksamkeit möglich war. Im Kostenmodell ergaben sich Kosten von 17 542 € für Ranibizumab, 15 896 € für Aflibercept, 10 826 € für das Fluocinolonacetonid-Implantat und 12 365 € für das Dexamethason-Implantat. Bei allen Therapien sind die Medikamentenkosten die bestimmende Kostenkomponente. In der uni- und multivariaten Sensitivitätsanalyse zeigten sich die Ergebnisse als robust gegen Veränderungen des Inputs. Schlussfolgerung Zusammenfassend konnte in einem Kostenvergleichsmodell über 3 Jahre gezeigt werden, dass die Steroidimplantate in der Therapie des diabetischen Makulaödems nach vorangegangener Anti-VEGF-Gabe erhebliche Kosteneinsparungen im Vergleich zu einer weiteren In-Label-Anti-VEGF-Therapie liefern können. Dabei stellt eine einmalige Gabe des Steroidimplantats Fluocinolonacetonid die kostengünstigste zulassungskonforme Alternative dar.

ZusammenfassungDas vitreomakuläre Traktionssyndrom (VMTS) wird definiert als partielle hintere Glaskörperabhebung mit persistierender vitreomakulärer Anhaftung. Bisher wurden traktive Veränderungen im Rahmen der Anwendung von VEGF-Hemmern im Wesentlichen im Sinne vitreoretinaler Traktionen im Rahmen der Therapie proliferativer ischämischer Netzhauterkrankungen wie der proliferativen diabetischen Retinopathie (PDR) beschrieben. In dieser Kasuistik werden 2 Fälle vorgestellt, bei denen Patienten mit einer altersbedingten neovaskulären Makuladegeneration und einem diabetischen Makulaödem als Folge einer Therapie mit VEGF-Hemmern ein ausgeprägtes VMTS entwickelten. Ein VMTS im Rahmen einer Anti-VEGF-Behandlung bei einer exsudativen Makulaerkrankung ist selten. Der genaue Pathomechanismus ist bisher unklar. Es wird diskutiert, dass die Anwendung von VEGF-Hemmern und die damit verbundene Fibrosierung bei noch anliegendem Glaskörper zu ausgeprägten Traktionen im vitreoretinalen Interface führen.

Das Auge und insbesondere die Sehkraft haben in unserem Alltag und Berufsleben einen immens hohen Stellenwert. Daher sollten Sie bei Patienten mit Augenproblemen — insbesondere bei visusbedrohenden Erkrankungen — die Diagnose moglichst fruh stellen und die Erkrankung zusammen mit dem Ophthalmologen behandeln. In seltenen Fallen konnen sich auch systemische Grundleiden zunachst am Auge manifestieren. Diese Patienten profitieren besonders von einer fruhen Diagnose und einer entsprechendenTherapie.

Purpose: To investigate the impact of spectral domain optical coherence tomography (SD-OCT) morphological predictive markers on visual acuity and outcome using ocriplasmin for macular hole and vitreomacular traction syndrome. Methods: A series of 40 patients in a retrospective study received intravitreal ocriplasmin. The primary endpoint was defined as morphological resolution of vitreomacular traction or closure of a macular hole. We analyzed the impact of pre- and postinjection SD-OCT findings on the outcome and visual acuity. Results: Thirteen of the 40 patients benefited from treatment. Statistical correlation between baseline characteristics and outcome revealed that higher foveal thickness (p = 0.018) and nontractional epiretinal membranes (p = 0.05) resulted in a worse outcome. In treatment success best corrected visual acuity gained was 9 (SD 12) letters and in failure 1 (SD 9) letter. We could not observe an influence of preinjection SD-OCT findings and other factors on visual outcome. Conclusion: We could confirm the therapeutic effect of ocriplasmin injections. SD-OCT morphological factors that influence treatment success and visual acuity were determined.

Introduction: Diabetic macular edema (DME) is a sight threatening disease and a major cause for blindness for people in working age. The pathogenesis is multifactorial and complex. The pharmacotherapy of DME addresses both the inhibition of vascular endothelial growth factor (VEGF) by the intravitreal injection of VEGF inhibitors and inflammatory processes by the intravitreal application of steroids. Several trials have been published reporting on the efficacy and safety of these treatments.Areas covered: This review discusses original research articles including basic science and clinical studies as well as review articles focusing on the role of inflammation and VEGF expression in DME. It discusses newly published clinical trials on intravitreal pharmacotherapy for DME. The literature was searched using Medline/PubMed and was selected given its relevance for the topic to be discussed.Expert opinion: Our knowledge regarding the pathophysiology of diabetic macular edema has significantly increased. Some of these insights have been successfully transferred into current treatment strategies already including VEGF suppression or anti-inflammatory treatments using steroids. The identification of additional pathophysiological aspects and their relevance as potential treatment targets will be a future challenge in the treatment of DME. A better knowledge on the complex pathophysiology will also help to establish combination strategies.

Currently two intravitreally applied corticosteroids (dexamethasone and fluocinolone) are licensed in Germany for treatment of diabetic macular oedema (DME). The use of DEX implant for DME in daily clinical practice has not been defined in detail. Following a Delphi panel survey, a group of retina experts set out to come up with a consensus for use of the DEX implant in DME.International and national treatment recommendations were identified from the literature. A steering group generated a catalogue of 72 statements on the aetiology and pathogenesis of DME, therapy with DEX implant, use of DEX implant in patients previously treated with VEGF-inhibitors, use of DEX implant in combination therapy, safety of DME therapies as well as patients' burden of treatment. Twenty-two ophthalmologists from private practice and 6 hospital ophthalmologists participated in the Delphi panel via Survey Monkey. Consensus was reached if at least 75% of participants agreed or disagreed with a statement. Statements for which consensus was not reached were discussed once more during the expert consensus meeting and a vote was taken. Based on these results a treatment algorithm for foveal DME was proposed.If a patient does not show sufficient response after 3 - 6 months of anti-VEGF treatment (visual acuity gain of 5 ETDRS letters or reduction of central retinal thickness ≤ 20%), a switch to DEX implant should take place. DEX implant is also suitable in eyes with longer presentation of DME, showing e.g. massive lipid exudates. DEX implant is suitable as first-line therapy especially in pseudophakic patients, patients unwilling or able to comply with tight anti-VEGF injection intervals or patients with known vascular diseases. With fixed control visits every 4 - 8 weeks, use of DEX implant is flexible and individual. Decision parameters for repeated use should be visual acuity, retinal thickness and intraocular pressure. Treatment of both eyes on the same day should not take place.The algorithm presented reflects survey as well as expert discussion results and may differ from recommendations issued by the German professional society. The consensus recommendations for the treatment of DME generated during the survey and meeting of retina experts are intended to guide use of DEX implant in daily practice.In Deutschland sind 2 intravitreal anzuwendende Kortikosteroide (Dexamethason und Fluocinolon) für die Behandlung des diabetischen Makulaödems (DMÖ) zugelassen. Mithilfe eines Delphi-Panels und einer Runde von Retinaspezialisten sollte ein Konsens für die Anwendung von DEX-Implantat erarbeitet werden.Aus der Literatur wurden internationale und nationale Behandlungsempfehlungen identifiziert. Eine Steuerungsgruppe erarbeitete einen Katalog von 72 Aussagen zur Ätiologie und Pathogenese des DMÖ, Therapie mit dem DEX-Implantat, Einsatz bei mit VEGF-Hemmern vorbehandelten Patienten, Verwendung von DEX-Implantat in der Kombination mit anderen Therapien, Sicherheit von DMÖ-Therapien sowie Therapiebelastung des Patienten. 22 niedergelassene Ophthalmologen und 6 Ophthalmologen aus der Klinik gaben über Survey Monkey ihre Bewertung zu den Aussagen ab. Die Definition des Konsenses zu einer Aussage galt als erfüllt, wenn mehr als 75% der Befragten einer Aussage zustimmten bzw. nicht zustimmten. Über nicht konsensfähige Aussagen wurde nach erneuter Diskussion in der Konsensrunde nochmals abgestimmt. In der Folge sollte auch ein Behandlungsschema für DMÖ mit fovealer Beteiligung vorgeschlagen werden.Sofern ein Patient unter VEGF-Hemmern unzureichend anspricht (Visusgewinn 5 ETDRS-Buchstaben oder Reduktion der zentralen Netzhautdicke ≤ 20%), sollte nach 3 – 6 Monaten auf das DEX-Implantat umgestellt werden. DEX-Implantat ist ebenfalls geeignet für Augen mit länger bestehendem DMÖ, bei denen z. B. massive Lipidexsudate vorhanden sind. DEX-Implantat eignet sich als Ersttherapie insbesondere bei Pseudophaken, Patienten, die nicht willens oder in der Lage sind, enge Injektionsintervalle unter Anti-VEGF-Therapie einzuhalten oder für Patienten mit vaskulären Vorerkrankungen. Bei festgelegten Kontrollintervallen von 4 – 8 Wochen kann die Anwendung des DEX-Implantats flexibel und individuell erfolgen. Als Entscheidungsparameter gelten hierbei neben Visus und Netzhautdicke auch der Augeninnendruck. Eine Behandlung von beiden Augen am selben Tag sollte nicht stattfinden.Der hier vorgestellte Algorithmus entspricht den Ergebnissen des Delphi-Prozesses und weicht u. U. von den Empfehlungen der Fachgesellschaft ab. Die im Rahmen der Befragung und Expertenrunde erarbeiteten Konsensempfehlungen für die Therapie von DMÖ können für den Einsatz des DEX-Implantats in der täglichen Praxis dienen.

Modern macular hole surgery results in high closure rates of over 90% and good functional results especially in macular holes up to 400 µm in diameter. The standard of care in most of these cases consists of transconjunctival sutureless pars plana vitrectomy, peeling of the inner limiting membrane (ILM) around the hole, followed by gas tamponade and positioning of the patient. As closure rates and functional results decrease with larger macular hole diameters over approximately 400 µm, alternative surgical techniques have been introduced to improve anatomical and functional results in these cases. These techniques include the positioning of tissue within the macular hole to improve hole closure. This can be performed using an ILM flap or free flap technique and the transplantation of autologous retinal tissue, lens capsule or homologous amniotic tissue in or under the defect. An alternative promising approach is the attenuation of the rim of the hole by induction of a localized retinal detachment at the posterior pole which is achieved by subretinal injection of balanced salt solution (BSS) using a 41 gauge needle. The operation is completed by an endotamponade using gas or silicone oil.

Methods: Bovine RPE cells were incubated with AV-17 (0.0625–0.5 mg/mL) for 30 seconds or 5 minutes. Viability was determined by live/dead staining, cleaved CASP3 immunostainings, and MTT test. Actin cytoskeleton was visualized by Alexa 488-phalloidin. Immunocytochemistry was performed to determine the levels of ZO-1, CTNNB1, and KRT19. Results: Exposure to AV-17 at the concentrations of 0.25–0.5 mg/mL resulted in a dose-dependent decrease in viability, the loss of ZO-1 from tight junctions, translocation of CTNNB1 into the cytoplasm and nucleus, disarrangement of the actin cytoskeleton, and a slight increase in KRT19. Ein 31-jahriger Patient stellt sich aufgrund einer seit zwei Wochen bestehenden Schwellung am linken Oberlid in unserer Sprechstunde vor. Er berichtet, dass zunachst nur eine diffuse Oberlidschwellung bestand, die sich dann innerhalb einer Woche auf den medialen Bereich lokalisierte. Im Verlauf seien die Schmerzen deutlich zuruckgegangen, die Lidrotung sei jedoch geblieben.

Purpose To correlate spectral-domain optical coherence tomography (SDOCT) criteria and clinical data with pathology of the vitreomacular interface (VMI) in eyes with diabetic macular edema (DME). Design Retrospective cross-sectional study and laboratory investigation. Methods We included specimens of 27 eyes of 26 patients with center-involved DME that underwent vitrectomy with peeling of the internal limiting membrane (ILM). Selection of specimens was consecutive and in retrospect using our register of the Vitreoretinal Pathology Unit. Clinical data and SDOCT examinations were correlated to immunocytochemistry and transmission electron microscopy. Classification of DME comprised sponge-like diffuse retinal thickening, cystoid macular edema, and serous retinal detachment. VMI was evaluated for presence of epiretinal membrane (ERM) and thickened vitreous cortex (tVC). Results ERMs and tVC were found in all DME types. Diffuse DME showed tVC more often than cystoid DME. Hyalocytes, contractile myofibroblasts, glial cells, matrix metalloproteinases-2 and -9, and collagen type I, II, and III were positive tested irrespective of DME type. There were no significant cell fragments at the retinal side of the ILM. Visual acuity improved in the majority of cases and macular thickness decreased significantly during mean follow-up of 17 ± 10 months. Conclusions All eyes presented pathologic VMI changes irrespective of the OCT classification of DME type or presence of ERM. Composition of fibrocellular membranes at the VMI indicated remodeling of vitreous cortex and transdifferentiation of hyalocytes into myofibroblasts. Our findings might argue for an early surgical intervention in eyes with DME irrespective of the presence of traction formation imaged by SDOCT.
