1. RURAL-URBAN HEALTHCARE ACCESS INEQUALITY CHALLENGE: TRANSFORMATIVE ROLES OF INFORMATION TECHNOLOGY.

Abstract

Rural-urban healthcare access inequality refers to a disparity between rural and urban people with severe medical ailments in gaining access to the high-quality healthcare services they need. Although much hope has been pinned on the use of health information technology (HIT) to alleviate this critical and enduring societal challenge, the realized societal impact of HIT is unclear. Anchoring on both social transformation theory and affordance actualization theory, we conducted an in-depth qualitative study with two rounds of data collection in China. In addition to investigating how the societal challenge has triggered transformative HIT interventions, our analysis contributes to a theory on an HIT solution for the rural-urban healthcare access inequality challenge by establishing a link between HIT affordances and HIT interventions. This is done by examining how microlevel HIT effects escalate to macrolevel HIT effects through societal-level affordance actualization, which can affect this healthcare access inequality challenge. Along with providing policy implications on introducing HIT solutions to address intricate and complex societal challenges, this study extends existing theories by revealing the adaptation of the HIT intervention and differentiating the effects of collective and shared affordances. 2. FOLLOWERSHIP IN AN OPEN-SOURCE SOFTWARE PROJECT AND ITS SIGNIFICANCE IN CODE REUSE.

Abstract

Code reuse is fundamental to the development of open-source software (OSS). Therefore, understanding how and why it occurs is important. To date, researchers have examined code reuse in OSS largely from the perspective of leaders. We show why followers must be considered as well. “Followers” are people who have had previous contacts with an individual from another project and who continue to associate with him or her. We consider two types of followers: developers (those directly involved in software development) and observers (those indirectly involved in it). We conduct a series of empirical investigations by using a longitudinal dataset of OSS projects hosted in GitHub, along with a survey and qualitative data. We find that followership can affect code reuse, but the effect depends on the nature of the follower (developer or observer). Overall, our study suggests that followership is important for code reuse in OSS because it enables participants to learn, and learning promotes code reuse. [ABSTRACT FROM AUTHOR]

Abstract

The reduction of operation and maintenance costs as well as the improvement of reliability of wind energy conversion systems are of top priorities in the renewable energy industry as the demand for wind energy continuously grow tremendously. With that, the condition monitoring system (CMS) plays a crucial role in maximizing the potential of wind energy in generating electricity. Therefore, this review paper vividly captures the fact that the recent advancements in embedded control in CMS in wind energy conversion systems are able deliver tangible benefits to the renewable energy technology (RET) industry as a whole. It is proven that CMS based maintenance is deemed to be more suitable in multiple aspects with fewer drawbacks, as compared to scheduled and corrective maintenance. In this review paper, an attempt to evaluate the viability of CMS is being made. The main purpose is to shed some light on the significant contributions provided by CMS in shortening the downtime and increasing the production rate of the wind energy conversion system. Forthcoming promising applications with good potential to surpass the recent dilemmas were also explored and analyzed at the end of this paper.

Abstract

Background & Aims Continuing recalcitrance to therapy cements pancreatic cancer (PC) as the most lethal malignancy, which is set to become the second leading cause of cancer death in our society. The study aim was to investigate the association between DNA damage response (DDR), replication stress, and novel therapeutic response in PC to develop a biomarker-driven therapeutic strategy targeting DDR and replication stress in PC. Methods We interrogated the transcriptome, genome, proteome, and functional characteristics of 61 novel PC patient–derived cell lines to define novel therapeutic strategies targeting DDR and replication stress. Validation was done in patient-derived xenografts and human PC organoids. Results Patient-derived cell lines faithfully recapitulate the epithelial component of pancreatic tumors, including previously described molecular subtypes. Biomarkers of DDR deficiency, including a novel signature of homologous recombination deficiency, cosegregates with response to platinum (P < .001) and PARP inhibitor therapy (P < .001) in vitro and in vivo. We generated a novel signature of replication stress that predicts response to ATR (P < .018) and WEE1 inhibitor (P < .029) treatment in both cell lines and human PC organoids. Replication stress was enriched in the squamous subtype of PC (P < .001) but was not associated with DDR deficiency. 5. SEQUENTIALITY OF PRODUCT REVIEW INFORMATION PROVISION: AN INFORMATION FORAGING PERSPECTIVE.

Abstract

The literature highlights the importance of product review information to a consumer, but limited knowledge exists on the provision of such information in a shopping website. This research uses information foraging theory (IFT) to determine how to provide product reviews. Product reviews differ according to whether the content is mainly on product attributes (i.e., attribute-oriented product reviews) or usage experience (i.e., usage-oriented product reviews). Two empirical studies were conducted. Study 1 examined consumer information diet for product reviews using the think-aloud approach. Results showed that consumers use two different genres of product review information sequentially (1) to follow product attribute-oriented review information during the screening phase and (2) to forage for product usage-oriented review information during the evaluation phase. The findings were extended to Study 2 through a field experiment, in which different patches of product reviews were purposefully and sequentially given in accordance with the consumer information diet for product reviews. The results revealed that an online shopping website could offer varying genres of product reviews at different junctures to enhance consumers’ decision-making performance. This research presents empirical evidence on the effectiveness of embedding product review information on a shopping website. This work further contributes to IFT, which is traditionally descriptive and conceptual in nature, by theorizing information provision and information-foraging behaviors of online consumers. Abstract

Pancreatic ductal adenocarcinoma (PDAC) can be divided into transcriptomic subtypes with two broad lineages referred to as classical (pancreatic) and squamous. We find that these two subtypes are driven by distinct metabolic phenotypes. Loss of genes that drive endodermal lineage specification, HNF4A and GATA6, switch metabolic profiles from classical (pancreatic) to predominantly squamous, with glycogen synthase kinase 3 beta (GSK3β) a key regulator of glycolysis. Pharmacological inhibition of GSK3β results in selective sensitivity in the squamous subtype; however, a subset of these squamous patient-derived cell lines (PDCLs) acquires rapid drug tolerance. Using chromatin accessibility maps, we demonstrate that the squamous subtype can be further classified using chromatin accessibility to predict responsiveness and tolerance to GSK3β inhibitors. Our findings demonstrate that distinct patterns of chromatin accessibility can be used to identify patient subgroups that are indistinguishable by gene expression profiles, highlighting the utility of chromatin-based biomarkers for patient selection in the treatment of PDAC.

Abstract

Background Pancreatic cancer (PC) has the worst survival of all periampullary cancers. This may relate to histopathological differences between pancreatic cancers and other periampullary cancers. Our aim was to examine the distribution and histopathologic features of pancreatic, ampullary, biliary and duodenal cancers resected with a pancreaticoduodenectomy (PD) and to examine local trends of periampullary cancers resected with a PD. Methods A retrospective review of PD between January 2000 and December 2012 at a public metropolitan database was performed. The institutional ethics committee approved this study. Results There were 142 PDs during the study period, of which 70 cases were pre-2010 and 72 post-2010, corresponding to a recent increase in the number of cases. Of the 142 cases, 116 were for periampullary cancers. There were also proportionately more PD for PC (26/60, 43% pre-2010 vs 39/56, 70% post-2010, P = 0.005). There were 65/116 (56%) pancreatic, 29/116 (25%), ampullary, 17/116 (15%) biliary and 5/116 (4%) duodenal cancers. Nodal involvement occurred more frequently in PC (78%) compared to ampullary (59%), biliary (47%) and duodenal cancers (20%), P = 0.002. Perineural invasion was also more frequent in PC (74%) compared to ampullary (34%), biliary (59%) and duodenal cancers (20%), P = 0.002. Microvascular invasion was seen in 57% pancreatic, 38% ampullary, 41% biliary and 20% duodenal cancers, P = 0.222. Overall, clear margins (R0) were achieved in fewer PC 41/65 (63%) compared to ampullary 27/29 (93%; P = 0.003) and biliary cancers 16/17 (94%; P = 0.014). Conclusions This study highlights that almost half of PD was performed for cancers other than PC, mainly ampullary and biliary cancers. The volume of PD has increased in recent years with an increased proportion being for PC. PC had higher rates of nodal and perineural invasion compared to ampullary, biliary and duodenal cancers.

13. Priority assessment of patients for elective general surgery: game on?

Abstract

Background: Clinical priority assessment criteria (CPAC) are used to generate a score by which patients are prioritized and rationed for elective surgery. It is widely believed that surgeons elevate scores to ensure their patients’ acceptance for elective surgery, colloquially called gaming. The purpose of the present paper was therefore to investigate whether there was a temporal trend to an increase in the assigned priority score from the inception of CPAC to the present. Methods: Priority and weighted inlier equivalent separations (WIES) scores between 23 April 1999 and 23 July 2002 were collected for elective general surgical cases at Auckland Hospital. A total of 5440 cases was retrospectively analysed using multiple regression techniques. Priority score was included as the dependent variable and time as an independent variable. Any change in case complexity over that period was accounted for by including the WIES score as a covariate. Multiple regression was undertaken for the combined surgeons and for individuals. Results: The combined model was statistically significant but accounted for only 17% of the priority score variance. An increase of one WIES unit leads to an increase of 2.7 in priority score (P = 0.0001). The relationship of priority score with time was dependent on the surgeon performing the prioritization. However, only half the surgeons had individual models that indicated gaming. Conclusions: The results show that gaming is occurring but that not all surgeons participate in this. The difference between surgeons’ participation in gaming is a potential source of practice variation in the prioritization process.

Abstract

Metal to metal bonding, particularly Cu-Cu bonding, is an important process step of three-dimensional integrated circuits (3DICs) that utilizes wafer bonding. The use of Cu to Cu bonding in 3DICs is advantageous as it functions as both the glue layer and electrical interconnection. It has been observed that the contact resistance of bonded Cu interface could be decreased under direct current stressing. In this article, the mentioned phenomenon is modeled and simulated. Electromigration-induced contact resistance reduction of bonded interconnects may provide a method for postbonding bond property improvement for 3DICs. [ABSTRACT FROM AUTHOR]

Abstract

The presence of Comparison Shopping (CS) websites not only allows consumers to gain quick access to multiple merchants? product offers, but also permits consumers to perform extensive comparison of products and prices prior to purchase. Given the significant reduction in search cost, it has been touted that CS websites can put merchants under increased price competition, resulting in commoditized markets, limited value of branding, and ultimately, convergence of prices to the competitive equilibrium. However, some studies suggest that lower search cost could make any price movement apparent to all participating merchants and hence promote price collusion. This research seeks to explicate the conditions under which CS websites are more likely or less likely to intensify market competition. Following the principles of experimental economics, we modeled and examined the impact of CS websites in many simulated markets featuring merchant characteristics (such as absence and presence of market power) and product type (such as commodity products and differentiated products). Through two series of experiments, we find that the lowering of search cost by CS websites could have opposite effects on market performance, depending on the underlying market structure. The article offers updates on two India-based manufacturing companies, Pegasus Sewing Machine Pte Ltd. and Best Corp. Ltd. (BCL). The sales of Pegasus in 2011 is estimated to be at the same level with 2010 due to dyeing issue in Tirupur though it is forecasted to increase by 15 to 20% in 2012. Meanwhile, BCL is considered as one of the pioneers in the manufacturing of knitted garments in Tirupur and an integrated textile company having operations from spinning to garmenting.

18. Chronic Triangular Fibrocartilage Complex Tears with Distal Radioulna Joint Instability: A New Method of Triangular Fibrocartilage Complex Reconstruction

Abstract

From September 1996 to September 1997, 27 adult patients were diagnosed with chronic triangular fibrocartilage complex (TFCC) tears with distal radioulna joint (DRUJ) instability in our clinic. They all received the procedure of TFCC reconstruction with partial extensor carpi ulnaris (ECU) tendon combined with or without ulnar shortening. There were 26 males and one female in the study with a mean age of 22.4 years. The follow-up period ranged from 22 to 28 months with a mean of 26.2 months. 24 patients who had positive or zero ulnar variance received the procedure of ulnar shortening. Three patients who had minus ulnar variance only received TFCC reconstruction. After the operation, the DRUJ was fixed by a 2.0 mm K-wire for 4 weeks. All patients underwent a rehabilitation program and they were reexamined at our outpatient department (OPD). The results were graded according to the Mayo Modified Wrist Score. Five of the 27 patients rated their wrists ‘excellent’, 18 rated ‘good’, and 4 rated ‘fair’. Overall, a total of 23 patients (85%) rated satisfactorily and returned to work or sport activities. In the 4 patients rated ‘fair’, mild pain at work and/or exercise, and mildly limited supination were found; however their grip strength was improved at least by 65% compared to the opposite hand. Therefore, as suggested by this study, TFCC reconstruction with partial ECU tendon combined with the ulnar shortening procedure is an alternative method for chronic TFCC tears with DRUJ instability.
