2. Developing and Implementing a Comprehensive AIDS Education Program in a Public School District.

Abstract

The Pennsylvania Department of Education mandated that all school districts in the state include K-12 instruction about Acquired Immune Deficiency Syndrome (AIDS) and related issues beginning in September 1987. This paper describes one school district's comprehensive AIDS education program. A survey administered in the fall of 1991 produced information about a lack of knowledge of AIDS policies among high school staff and in the community. According to data analysis, the curriculum was not being taught at all mandated levels, and teachers and principals felt a lack of knowledge about AIDS education and district policies and procedures. As a result, the school district became actively engaged in enhancing the AIDS education of administration, staff, students, parents, and community groups; an AIDS curriculum Committee and a Teen AIDS Peer Group were established; and topics such as policy issues and community resources were major topics for AIDS inservice programs. Appendixes include the Pennsylvania State AIDS Mandate, the school district's AIDS policy, an elementary school AIDS instruction summary, parent letters for AIDS instruction, grades 8-12 pre/post AIDS instruction test; staff and parent surveys; a school district AIDS survey, and curriculum revisions. Abstract

Single ventricle patients, including those with hypoplastic left heart syndrome (HLHS), typically undergo three palliative heart surgeries culminating in the Fontan procedure. HLHS is associated with high rates of morbidity and mortality, and many patients develop arrhythmias, electrical dyssynchrony, and eventually ventricular failure. However, the correlation between ventricular enlargement and electrical dysfunction in HLHS physiology remains poorly understood. Here we characterize the relationship between growth and electrophysiology in HLHS using computational modeling. We integrate a personalized finite element model, a volumetric growth model, and a personalized electrophysiology model to perform controlled in silico experiments. We show that right ventricle enlargement negatively affects QRS duration and interventricular dyssynchrony. Conversely, left ventricle enlargement can partially compensate for this dyssynchrony. These findings have potential implications on our understanding of the origins of electrical dyssynchrony and, ultimately, the treatment of HLHS patients. [ABSTRACT FROM AUTHOR]

Abstract

Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia, characterised by a rapid and irregular electrical activation of the atria. Treatments for AF are often ineffective and few atrial biomarkers exist to automatically characterise atrial function and aid in treatment selection for AF. Clinical metrics of left atrial (LA) function, such as ejection fraction (EF) and active atrial contraction ejection fraction (aEF), are promising, but have until now typically relied on volume estimations extrapolated from single-slice images. In this work, we study volumetric functional biomarkers of the LA using a fully automatic SEGmentation of the left Atrium based on a convolutional neural Network (SEGANet). SEGANet was trained using a dedicated data augmentation scheme to segment the LA, across all cardiac phases, in short axis dynamic (CINE) Magnetic Resonance Images (MRI) acquired with full cardiac coverage. Using the automatic segmentations, we plotted volumetric time curves for the LA and estimated LA EF and aEF automatically. The proposed method yields high quality segmentations that compare well with manual segmentations (Dice scores [$0.93 \pm 0.04$], median contour [$0.75 \pm 0.31$] mm and Hausdorff distances [$4.59 \pm 2.06$] mm). LA EF and aEF are also in agreement with literature values and are significantly higher in AF patients than in healthy volunteers. Our work opens up the possibility of automatically estimating LA volumes and functional biomarkers from multi-slice CINE MRI, bypassing the limitations of current single-slice methods and improving the characterisation of atrial function in AF patients.

Abstract

Background: The impact of major trauma on individuals with pre-existing and enduring mental health difficulties is poorly understood.Method: Twenty individuals with pre-existing and enduring mental health difficulties were assessed with the Clinician Administered Post Traumatic Stress Disorder Scale, the 28-item version of the GHO, the Hospital Anxiety and Depression Scale, and the impact of Event Scale between four and eight weeks after their involvement in a major coach accident.Results: Ten (50%) individuals satisfied the full DSM-IV criteria for a diagnosis of PTSD. The questionnaire scores indicated a high level of psychological suffering among the group. On dividing the sample into those individuals with previous diagnosis of anxiety or depressive disorders and those with a previous diagnosis of chronic schizophrenia, the schizophrenia group displayed less marked psychological sequelae on all measures.Conclusion: There was a high incidence of early psychological sequelae among the group as a whole. Some diagnoses (e.g. depression and anxiety) may predispose to psychological sequelae while others (e.g. chronic schizophrenia) may not. [ABSTRACT FROM AUTHOR]

Abstract

Background: There is a need for a valid self-rating questionnaire to screen for psychiatric morbidity in patients with chronic fatigue syndrome (CFS). This study had the aim of assessing the utility and validity of two commonly used measures.Method: Scores obtained on the General Health Questionnaire (GHQ) and the Beck Depression Inventory (BDI) were compared with various diagnostic and severity ratings obtained via a validating clinical interview, the Schedules for the Clinical Assessment of Neuropsychiatry (SCAN) in 95 consecutively referred subjects at a medical out-patient clinic who fulfilled standard criteria for CFS, and 48 healthy controls. Outcome measures were validating coefficients and receiver operating characteristics (ROC) for different thresholds and scoring on GHQ and BDI and index of definition (ID) as measured by SCAN; and Pearson and point by serial correlation coefficients for different diagnostic groups derived via SCAN and defined according to ICD-10 and DSM-III-R.Results: GHQ and BDI perform poorly as screeners of psychiatric morbidity in CFS subjects when compared with various SCAN derived ratings although results for controls are comparable with other studies.Conclusions: Neither the GHQ nor BDI alone can be recommended as screeners for psychiatric morbidity in CFS subjects.
