19. Attainment of therapeutic fluoride levels in serum without major side effects using a slow-release preparation of sodium fluoride in postmenopausal osteoporosis

Abstract

The bioavailability, biochemical effects, and safety of a slow-release preparation of sodium fluoride were examined. In 8 normal volunteers, a single administration of slow-release sodium fluoride (25 mg) caused a slow rise and gradual decline in serum fluoride concentration, thus avoiding sharp peaks produced by a rapid-release preparation. In 37 patients with postmenopausal osteoporosis, serum fluoride concentration was kept within the "therapeutic window" (95-100 ng/ml) during long-term intermittent sodium fluoride (slow-release) therapy (25 mg twice/day, given for 3 months in each 5-month cycle over five cycles). Serum fluoride was also kept within the therapeutic window in 64 patients who took sodium fluoride (slow release) continuously over 12 months. Serum osteocalcin concentration increased progressively during fluoride treatment (correlation coefficient of 0.88, p less than .001 for the relationship between serum osteocalcin and duration of therapy). Side effects to slow-release sodium fluoride therapy, assessed in 101 patients at two study sites, were minor and included diarrhea in 2 patients, nausea in 2 patients, abdominal pain and cramping in 2 patients, foot pain in 2 patients, and joint pain in 6 patients. Thus, slow-release sodium fluoride confers desired level of fluoride in serum, while providing safety of usage.

Abstract

Abstract: The antiplane analysis is made for a bimaterial BaTiO3–CoFe2O4 composite wedge containing an interface crack. The coupled magneto-electro-elastic field is induced by the piezoelectric/piezomagnetic BaTiO3–CoFe2O4 composite materials. For the crack problems, the intensity factors of stress, strain, electric displacement, electric field, magnetic induction and magnetic field at crack tips are derived analytically. Also, the energy density criterion is applied to predict the fracture behavior of the interface crack. The numerical results also show that the energy release rate for a crack in a single wedge is negative. [Copyright &y& Elsevier]

Abstract

Chronic kidney disease (CKD) is associated with accelerated cardiovascular disease and heart failure. Endothelial nitric oxide synthase (eNOS) Glu298Asp single nucleotide polymorphism (SNP) genotype has been associated with a worse phenotype amongst patients with established heart failure and in patients with progression of their renal disease. The association of a cardiac functional difference in non-dialysis CKD patients with no known previous heart failure, and eNOS gene variant is investigated. Methods 140 non-dialysis CKD patients, who had cardiac magnetic resonance (CMR) imaging and tissue doppler echocardiography as part of two clinical trials, were genotyped for eNOS Glu298Asp SNP retrospectively. Results The median estimated glomerular filtration rate (eGFR) was 50mls/min and left ventricular ejection fraction (LVEF) was 74 with no overt diastolic dysfunction in this cohort. There were significant differences in LVEF across eNOS genotypes with GG genotype being associated with a worse LVEF compared to other genotypes (LVEF: GG 71, TG 76, TT 73, p = 0.006). After multivariate analysis, (adjusting for age, eGFR, baseline mean arterial pressure, contemporary CMR heart rate, total cholesterol, high sensitive C-reactive protein, body mass index and gender) GG genotype was associated with a worse LVEF, and increased LV end-diastolic and systolic index (p = 0.004, 0.049 and 0.009 respectively). Conclusions eNOS Glu298Asp rs1799983 polymorphism in CKD patients is associated with relevant sub-clinical cardiac remodelling as detected by CMR. This gene variant may therefore represent an important genetic biomarker, and possibly highlight pathways for intervention, in these patients who are at particular risk of worsening cardiac disease as their renal dysfunction progresses. © 2015 Chand et al.

Abstract

This paper presents the general solutions of antiplane electro-mechanical field solutions for a piezoelectric finite wedge subjected to a pair of concentrated forces and free charges as shown in Fig.1. The boundary conditions on the circular segment are considered as fixed and grounded. Employing the finite Mellin transform method, the stress and electrical displacement at all fields of piezoelectric finite wedge are derived analytically. In addition, the singularity orders and intensity factors of stress and electrical displacement can also be obtained too. These parameters can be applied to examine the fracture behavior of the wedge structure. After being reduced to the problem of an antiplane edge crack or an infinite wedge in a piezoelectric medium, the results compare well with those of previous studies. [ABSTRACT FROM AUTHOR]

Abstract

The problem for an internal crack located within a functionally graded piezoelectric/piezomagnetic half plane is studied. It is subjected to the mechanical, electrical and magnetic loads. The crack is normal to the free boundary of a functionally graded piezoelectric/piezomagnetic materials. The elastic modulus, piezoelectric constant, dielectric constant, piezomagnetic coefficient, magnetoelectric coefficient and magnetic permeability of the functionally graded piezoelectric/piezomagnetic materials are assumed in exponential forms and vary along the crack direction. By using the Fourier Transform theorem, the problem can be reduced to a system of singular integral equations and solved by using the Gauss-Chebyshev integration technique.
