Abstract

Introduction: Ivacaftor is a cystic fibrosis transmembrane conductance regulator (CFTR) potentiator that has demonstrated clinical benefits in phase 3 trials. We report results from a real-world study (BRIO) to assess the effectiveness of ivacaftor in people with cystic fibrosis (pwCF) in France. Methods: BRIO was an observational study conducted at 35 centers in France. Both pwCF initiating ivacaftor treatment and those already taking ivacaftor were included and prospectively followed for 24 months. The primary objective was to evaluate the effect of ivacaftor on percent predicted forced expiratory volume in 1 s (ppFEV1); secondary objectives were evaluating the effect of ivacaftor on clinical effectiveness, healthcare resource utilization (HCRU), and safety. Results: A total of 129 pwCF were enrolled; 58.9% were aged < 18 years; 64.3% had a G551D-CFTR allele. Mean age at ivacaftor initiation was 19.1 years (range, 2–64 years); ppFEV1 increased by a least squares mean of 8.49 percentage points in the first 6 months and was sustained through 36 months of ivacaftor use. Growth metrics increased during the first 12 months post-ivacaftor and remained stable. The rate of pulmonary exacerbations (PEx) decreased during the 12 months post-ivacaftor compared with the 12 months pre-ivacaftor; estimated rate ratios (95% CI) were 0.57 (0.43–0.75) for PEx events and 0.25 (0.13–0.48) for PEx requiring hospitalization. No new safety concerns were identified; no deaths occurred. Conclusions: The results from this real-world study of ivacaftor usage in France were consistent with prior clinical trial outcomes, confirming the clinical effectiveness of ivacaftor, as well as an associated reduction in HCRU. Abstract

International audience; Rationale: Lumacaftor-ivacaftor is a CFTR (cystic fibrosis transmembrane conductance regulator) modulator combination recently approved for patients with cystic fibrosis (CF) homozygous for the Phe508del mutation.Objectives: To evaluate the safety and effectiveness of lumacaftor-ivacaftor in adolescents (≥12 yr) and adults (≥18 yr) in a real-life postapproval setting.Methods: The study was conducted in the 47 CF reference centers in France. All patients who initiated lumacaftor-ivacaftor from January 1 to December 31, 2016, were eligible. Patients were evaluated for lumacaftor-ivacaftor safety and effectiveness over the first year of treatment following the French CF Learning Society's recommendations.Measurements and Main Results: Among the 845 patients (292 adolescents and 553 adults) who initiated lumacaftor-ivacaftor, 18.2% (154 patients) discontinued treatment, often owing to respiratory (48.1%, 74 patients) or nonrespiratory (27.9%, 43 patients) adverse events. In multivariable logistic regression, factors associated with increased rates of discontinuation included adult age group, percent predicted FEV1 (ppFEV1) less than 40%, and numbers of intravenous antibiotic courses during the year before lumacaftor-ivacaftor initiation. Patients with continuous exposure to lumacaftor-ivacaftor showed an absolute increase in ppFEV1 (+3.67%), an increase in body mass index (+0.73 kg/m2), and a decrease in intravenous antibiotic courses by 35%. Patients who discontinued treatment had significant decrease in ppFEV1, without improvement in body mass index or decrease in intravenous antibiotic courses.Conclusions: Lumacaftor-ivacaftor was associated with improvement in lung disease and nutritional status in patients who tolerated treatment. Adults who discontinued lumacaftor-ivacaftor, often owing to adverse events, were found at high risk of clinical deterioration.

5. Change in phase 3 slope of exhaled CO2 in response to β2 agonist inhalation is associated with asthma control in children

Abstract

Background: Airway hyper-reactivity, inflammation and remodelling contribute to inhomogeneity of ventilation-perfusion ratio (V/Q) in asthma. Short-term variations in V/Q can cause changes in expired capnographic indices. Objectives: to measure acute changes in the phase 3 slope of the exhaled volumetric capnogram after β2-agonist inhalation (ΔSv3), for comparison with airway response based on FEV1 (ΔFEV1), and asthma control. Subjects and Methods: After ethical approval and informed consent, 72 children aged 6.0 – 17.7 y, followed up for asthma underwent spirometry and exhaled capnography before and after β2-agonist inhalation through a spacer, using a side-stream rapid infrared analyser. Asthma control was assessed using the GINA questionnaire. Results: Children with positive reversibility tests (defined as ΔFEV1>12%) had a significantly higher ΔSv3 (m±SE: 87.4±41.4) vs. negative tests (31.3±14.0%, p=0.001). Uncontrolled asthma was associated with a significantly larger ΔSv3 (103.4±64.0%, n=7) compared to partly controlled (52.0±26.1, n=24; p=0.009) and controlled asthma (30.8±16.3, n=41; p=0.003). Neither Bohr dead space nor ΔFEV1 were different between asthma control groups. Conclusions: mean ΔSv3 was significantly larger in children with positive response to β2-agonist, and in uncontrolled asthmatics, albeit with some variability. To our knowledge these are the first data on exhaled CO2 phase 3 volumetric slope change in asthma control. The positive β2-agonist-induced change in ΔSv3 could be due to increased ventilation of inhomogeneous peripheral lung units, and merits further evaluation as a potential phenotypic biomarker in asthma.

Body: Rationale: Upper airway and nasal airflow resistance are significant in infancy, due to the fact that infants are nasal breathers in early life, and that obstructive nasal diseases are frequent at this age. Nasal flow resistance is closely determined by anatomy, which rapidly changes with growth. The goal of this study was to assess standardized measurements of the nasal cavity dimensions in the first 2 years of life. Methods: 34 head CT scans of infants aged 4.13±4.08 months (range: 0.07 to 19.4, 17 male, 17 female) available in the Amiens University Hospital Dept. of Radiology database were retrospectively analyzed. Infants with craniofacial deformities were excluded. Images were 3D reconstructed, axially oriented and the nasal cavity was segmented by density thresholding from nares to vocal cords. Surface areas of the piriform and choanal apertures were automatically computed in the coronal plane, in standardized positions based on bony reference points. 7. La tuberculose pédiatrique dans la Somme‎ : 10 ans après l'arrêt de la vaccination

Abstract

La tuberculose est un problème de santé publique en France. L'identification du contaminateur et des sujets susceptibles d'avoir été contaminés, en particulier les enfants, est déterminante dans la prise en charge de la tuberculose. L'objectif de cette étude est l'évaluation du profil des patients pédiatriques atteints de tuberculose dans la Somme, concernant l'ITL et la TM, 10 ans après l'arrêt de la vaccination. Une étude observationnelle, rétrospective a été menée chez les patients de moins de 18 ans, présentant une TM ou une ITL, du 1er janvier 2008 au 31 octobre 2018 dans la Somme. La population avait été hospitalisée au sein de CHU d'Amiens, ou été suivie en consultation par un pédiatre hospitalier ou un médecin du CLAT. 67 patients ont été inclus dans l'étude. 38 étaient atteints de TM et 29 d'ITL. L'âge moyen était de 9,3 ans. 33% des enfants étaient vacciné, mais 56% auraient dû vaccinés car définis comme à risque de tuberculose. 34,3% vivaient en situation de précarité. 46% de patient étaient immigrés et pour 74% d'entre eux, originaires d'Afrique subsaharienne. Parmi les enfants dont le contage tuberculeux était connu, 49% étaient contaminés par leurs parents. Parmi les enfants atteints de TM, 87% avaient une atteinte intra-thoracique, et 24% n'avaient pas été déclarés au CLAT. Les enfants les plus jeunes sont les plus à risque d'évoluer vers une tuberculose maladie. Les facteurs de vulnérabilité sont retrouvés dans notre étude. Une proportion importante d'enfants vivait en situation de précarité, et séjournait en France depuis moins d'un an. L'identification des populations à risque reste indispensable pour une vaccination ciblée par le BCG.
