Item does not contain fulltext, BACKGROUND: Several reports have demonstrated the impact of the COVID-19 pandemic on the management and outcome of patients with ST-segment elevation myocardial infarction (STEMI). The aim of the current analysis is to investigate the potential gender difference in the effects of the COVID-19 pandemic on mechanical reperfusion and 30-day mortality for STEMI patients within the ISACS-STEMI COVID-19 Registry. METHODS: This retrospective multicenter registry was performed in high-volume primary percutaneous coronary intervention (PPCI) centers on four continents and included STEMI patients undergoing PPCIs in March-June 2019 and 2020. Patients were divided according to gender. The main outcomes were the incidence and timing of the PPCI, (ischemia time ≥ 12 h and door-to-balloon ≥ 30 min) and in-hospital or 30-day mortality. RESULTS: We included 16683 STEMI patients undergoing PPCIs in 109 centers. In 2020 during the pandemic, there was a significant reduction in PPCIs compared to 2019 (IRR 0.843 (95% CI: 0.825-0.861, p < 0.0001). We did not find a significant gender difference in the effects of the COVID-19 pandemic on the numbers of STEMI patients, which were similarly reduced from 2019 to 2020 in both groups, or in the mortality rates. Compared to prepandemia, 30-day mortality was significantly higher during the pandemic period among female (12.1% vs. 8.7%; adjusted HR [95% CI] = 1.66 [1.31-2.11], p < 0.001) but not male patients (5.8% vs. 6.7%; adjusted HR [95% CI] = 1.14 [0.96-1.34], p = 0.12). CONCLUSIONS: The COVID-19 pandemic had a significant impact on the treatment of patients with STEMI, with a 16% reduction in PPCI procedures similarly observed in both genders. Furthermore, we observed significantly increased in-hospital and 30-day mortality rates during the pandemic only among females. Trial registration number: NCT 04412655.

The aim of the current analysis is to evaluate the age-related effects of the COVID-19 pandemic on mechanical reperfusion and 30-day mortality for STEMI within the registry ISACS-STEMI COVID-19. METHODS: This retrospective multicenter registry was performed in high-volume PPCI centers on four continents and included STEMI patients undergoing PPCI in March-June 2019 and 2020. Patients were divided according to age (< or ≥75 years). The main outcomes were the incidence and timing of PPCI, (ischemia time longer than 12 h and door-to-balloon longer than 30 min), and in-hospital or 30-day mortality. RESULTS: We included 16,683 patients undergoing PPCI in 109 centers. In 2020, during the pandemic, there was a significant reduction in PPCI as compared to 2019 (IRR 0.843 (95%-CI: 0.825-0.861, p < 0.0001). We found a significant age-related reduction (7%, p = 0.015), with a larger effect on elderly than on younger patients. Furthermore, we observed significantly higher 30-day mortality during the pandemic period, especially among the elderly (13.6% vs. 17.9%, adjusted HR (95% CI) = 1.55 [1.24-1.93], p < 0.001) as compared to younger patients (4.8% vs. 5.7%; adjusted HR (95% CI) = 1.25 [1.05-1.49], p = 0.013), as a potential consequence of the significantly longer ischemia time observed during the pandemic. CONCLUSIONS: The COVID-19 pandemic had a significant impact on the treatment of patients with STEMI, with a 16% reduction in PPCI procedures, with a larger reduction and a longer delay to treatment among elderly patients, which may have contributed to increase in-hospital and 30-day mortality during the pandemic.

Abstract

The chapter titled “Eight-membered Rings with two Heteroatoms 1,2” deals with heterocine rings with two heteroatoms in a 1,2 relationship, namely 1,2-diazocine, 2H-1,2-oxazocine, 2H-1,2-thiazocine, 1,2-dioxocin, 1,2-oxathiocin and 1,2-dithiocin. This article covers the literature from 2007 to 2019 and reports the chemistry of uncondensed derivatives, heterocines fused to carbocycles and heterocycles, as well as bridged heterocines. As usual, in the case that a particular section is not mentioned, it means that no chemistry has been reported. In this article, the “Biosynthesis” paragraph was additionally introduced, since in the latest years the contribution of Nature to the synthesis of diheterocines became important, especially for biologically active compounds. Following this approach, the natural sources will be analyzed, as well as the biosynthetic pathway whenever available. In this edition, as happened in the previous one, the interest in 1,2-heterocines has been driven by the pharmacological activity shown by several derivatives or some potentially important industrial applications.

Abstract

ObjectiveThe initial data of the International Study on Acute Coronary Syndromes - ST Elevation Myocardial Infarction COVID-19 showed in Europe a remarkable reduction in primary percutaneous coronary intervention procedures and higher in-hospital mortality during the initial phase of the pandemic as compared with the prepandemic period. The aim of the current study was to provide the final results of the registry, subsequently extended outside Europe with a larger inclusion period (up to June 2020) and longer follow-up (up to 30 days).MethodsThis is a retrospective multicentre registry in 109 high-volume primary percutaneous coronary intervention (PPCI) centres from Europe, Latin America, South-East Asia and North Africa, enrolling 16 674 patients with ST segment elevation myocardial infarction (STEMI) undergoing PPPCI in March/June 2019 and 2020. The main study outcomes were the incidence of PPCI, delayed treatment (ischaemia time >12 hours and door-to-balloon >30 min), in-hospital and 30-day mortality.ResultsIn 2020, during the pandemic, there was a significant reduction in PPCI as compared with 2019 (incidence rate ratio 0.843, 95% CI 0.825 to 0.861, p75 years) (p=0.015), and was not related to the peak of cases or deaths due to COVID-19. Abstract

Innovative, automated, and non-invasive techniques have been developed by scientific community to indirectly assess structural conditions and support the decision-making process for a worthwhile maintenance schedule. Nowadays, machine learning tools are in the spotlight because of their outstanding capabilities to deal with data coming from even heterogeneous sources and their ability to extract information from the structural systems, providing highly effective, reliable, and efficient damage classification tools. In the current study, a supervised multi-level damage classification strategy has been developed regarding Ground Penetrating Radar (GPR) profiles for the assessment of tunnel lining conditions. In previous research, the authors firstly considered a convolutional neural network (CNN), adopting the quite popular ResNet-50, initialized through transfer learning. In the present work, further enhancements have been attempted by adopting two configurations of the newest state-of-art advanced neural architectures: the neural transformers. The foremost is the original Vision Transformer (ViT), whose core is an encoder entirely based on the innovative self-attention mechanism and does not rely on convolution at all. The second is an improvement of ViT which merges convolution and self-attention, the Compact Convolution Transformer (CCT). In conclusion, a critical discussion of the different pros and cons of adopting the above-mentioned different architectures is finally provided, highlighting the actual powerfulness of these technologies in the future civil engineering paradigm nevertheless.

Item does not contain fulltext, OBJECTIVE: The initial data of the International Study on Acute Coronary Syndromes - ST Elevation Myocardial Infarction COVID-19 showed in Europe a remarkable reduction in primary percutaneous coronary intervention procedures and higher in-hospital mortality during the initial phase of the pandemic as compared with the prepandemic period. The aim of the current study was to provide the final results of the registry, subsequently extended outside Europe with a larger inclusion period (up to June 2020) and longer follow-up (up to 30 days). METHODS: This is a retrospective multicentre registry in 109 high-volume primary percutaneous coronary intervention (PPCI) centres from Europe, Latin America, South-East Asia and North Africa, enrolling 16 674 patients with ST segment elevation myocardial infarction (STEMI) undergoing PPPCI in March/June 2019 and 2020. The main study outcomes were the incidence of PPCI, delayed treatment (ischaemia time >12 hours and door-to-balloon >30 min), in-hospital and 30-day mortality. RESULTS: In 2020, during the pandemic, there was a significant reduction in PPCI as compared with 2019 (incidence rate ratio 0.843, 95% CI 0.825 to 0.861, p<0.0001). This reduction was significantly associated with age, being higher in older adults (>75 years) (p=0.015), and was not related to the peak of cases or deaths due to COVID-19. The heterogeneity among centres was high (p<0.001). Furthermore, the pandemic was associated with a significant increase in door-to-balloon time (40 (25-70) min vs 40 (25-64) min, p=0.01) and total ischaemia time (225 (135-410) min vs 196 (120-355) min, p<0.001), which may have contributed to the higher in-hospital (6.5% vs 5.3%, p<0.001) and 30-day (8% vs 6.5%, p=0.001) mortality observed during the pandemic. Abstract

Limited therapeutic options are available for advanced colorectal cancer (CRC). Herein, we report that exposure to a neo-synthetic bis(indolyl)thiazole alkaloid analog, nortopsentin 234 (NORA234), leads to an initial reduction of proliferative and clonogenic potential of CRC sphere cells (CR-CSphCs), followed by an adaptive response selecting the CR-CSphC-resistant compartment. Cells spared by the treatment with NORA234 express high levels of CD44v6, associated with a constitutive activation of Wnt pathway. In CR-CSphC-based organoids, NORA234 causes a genotoxic stress paralleled by G2-M cell cycle arrest and activation of CHK1, driving the DNA damage repair of CR-CSphCs, regardless of the mutational background, microsatellite stability, and consensus molecular subtype. Synergistic combination of NORA234 and CHK1 (rabusertib) targeting is synthetic lethal inducing death of both CD44v6-negative and CD44v6-positive CRC stem cell fractions, aside from Wnt pathway activity. These data could provide a rational basis to develop an effective strategy for the treatment of patients with CRC.

25. AN UNCOMMON CASE OF MINOCA

Abstract

We report the case of a 51 year old male, smoker of about 20 cigarettes/day and with history of uninvestigated headache. In March 2023 he arrived in the Emergency Department for oppressive retrosternal chest pain radiating to the left arm; the electrocardiogram (ECG) showed sinus rhythm, no acute ST segment changes; HS troponin T was elevated. Transthoracic echocardiogram showed 55% EF, no kinesis defects, no major valvulopathy, neither pericardial effusion. The patient was admitted to the ICU with a diagnosis of NSTE myocardial infarction. Despite on admission HS troponin T HS was further increased and ECG evolved with inferior and lateral subepicardial ischemia, coronary angiogram showed normal coronary arteries, with an area of haziness in the right coronary artery. To investigate the etiology of myocardial damage the patient underwent cardiac MRI, that showed a left ventricle with normal size and mildly reduced systolic function; inferior apical and septal segment were akinetic; right ventricle was normal with preserved systolic function; edema and fibrosis with ischemic pattern was seen in the inferior apical and septal segment. Then we looked for embolic sources: continuous ECG monitoring excluded supraventricular arrhythmias. We performed echocardiogram that raised suspicion of a patent foramen ovale (PFO) confirmed on transesophageal contrast echocardiogram. Brain MRI showed no obvious changes compatible with recent ischemic lesions; few millimeters focal hyperintense areas in T2 and FLAIR, of gliotic nonspecific significance were reported in the frontal subcortical white matter bilaterally, left parietal and in the right corona radiata. We concluded for diagnosis of paradoxical coronary embolism and the patient was treated with percutaneous closure of PFO: by right femoral vein route and under intracardiac ultrasound guidance, closure of PFO was performed with Amplatzer No. 25 with excellent fluoroscopic and final ultrasound result. The broad spectrum of the possible cause of myocardial injury in MINOCA makes defining the extact underlying etiology challenging, but correctly knowing the culprit mechanism improves therapies and outcome; paradoxical coronary embolism due to a patent foramen ovale is rarely diagnosed; in our clinical case integrated multimodality imaging diagnostic work up, proved highly effective in identifying the cause of the myocardial damage and allowed the appropriate treatment.

Abstract

Per le loro finalità generali, le azioni di orientamento possono senza dubbio considerarsi interventi in ambito psicosociale, e come tali per esse può considerarsi imprescindibile il tema della valutazione (Fraccaroli e Vergani, 2004). Attraverso la verifica della qualità delle attività offerte e degli effetti che esse hanno sull’utenza, si può comprendere se tali interventi abbiano funzionato o meno, in che misura, in che modo e, con adeguate riflessioni a margine, perché. La valutazione di una azione di orientamento dovrebbe assumere come oggetto di analisi diverse dimensioni, non necessariamente convergenti (Ferrieux e Carayon, 1998; Tronti, 2002). Un serio problema nasce dal fatto che non risulta ancora ben chiaro quali siano le variabili da prendere in considerazione nella valutazione degli effetti e dei risultati di una pratica orientativa. La difficoltà principale sta nella definizione stessa di tali risultati e nell’individuazione dei parametri (Gaudron, Bernaud e Lemoine, 2001). A tal proposito possono essere utili gli studi longitudinali, che permettono di comprendere se gli effetti positivi dell’orientamento tendono a rimanere stabili nel tempo, o se invece tendono a scomparire e, inoltre, se l’intervento può avere ricadute a medio e lungo termine, anche diverse da quelle immediate (Pombeni, 2004, Masdonati e Dauwalder, 2010). Il presente lavoro prende in esame gli effetti a lungo termine di un intervento di orientamento realizzato presso il Centro di Orientamento e Tutorato (COT) dell'Università degli Studi di Palermo denominato "Scelta e progetto di carriera", ed ispirato al modello di Bilancio delle Competenze (Pace, Ciaccio, Di Bernardo, Governale, Messana e Pupillo, 2007). La finalità è stata quella di verificare se, a distanza di anni, gli studenti che hanno partecipato alle attività abbiano avuto risultati positivi in termini di successo formativo e professionale, rispetto a coloro che non hanno intrapreso tale percorso, e quali siano le implicazioni in termini di soddisfazione rispetto al percorso universitario ed alla eventuale professione intrapresa. La ricerca, a disegno longitudinale, ha preso in esame complessivamente 211 studenti (suddivisi tra gruppo sperimentale e di controllo) ad 8 anni dall'intervento. I risultati mostrano, in linea con la letteratura, che l’effetto di un intervento di orientamento ha marcati effetti sugli aspetti motivazionali e di soddisfazione formativa e professionale, mentre dal punto di vista della performance formativa esso non mostra una incidenza specifica.
