- Abstract
The upper lip frenum is a crease-shaped mucous membrane located in the oral region in the sagittal plane, often can present changes in its structure and shape, causing an interincisive diastema, problems in hygiene, speech and gingival recession, favoring vicious habits, and can generate compromised lip movement. This study aims to report a clinical case of upper lip frenectomy in a child patient highlighting the diagnosis, surgical treatment and prognosis. The clinical case refers to a seven-year-old female patient who, after the eruption of the upper central and lateral incisors, clinically presented the frenum with low insertion, causing interincisal diastema. After clinical and radiographic examination, upper labial frenectomy was planned, using the modified Archer technique. Therefore, we conclude that this technique is less traumatic to the infant patient, due to its simplicity and speed, with a positive result and good prognosis. [ABSTRACT FROM AUTHOR]

Periapical cyst is considered one of the most common non-odontogenic cysts affecting oral and maxillofacial tissues. It is characterized by having a pathological cavity lined with epithelium, and is most often asymptomatic, originating from a tooth without vitality, derived from a long-term inflammatory process, and for this reason, it ends up being considered an imaging finding when diagnosed through imaging exams. The aim of this study is to report a clinical case of a root cyst in the mandibular body region, emphasizing the large volume and extent of the lesion presented, relating endodontic treatment with the use of biomaterial to fill the tooth, and the association of the surgical technique of cystic enucleation. A 63-year-old male patient presented with a periapical cyst located at the apex of tooth 34, a radiographic finding discovered through a complementary examination (panoramic radiography) requested during a routine consultation. Endodontic treatment was chosen for tooth 34, associated with surgical treatment of the cystic lesion in the mandible, using the enucleation technique. The surgical intervention associated with the endodontic treatment proved to be effective based on the imaging exams, where it was possible to see the beginning of bone neoformation and maintenance of the tooth without altering its masticatory function. The objective of this study is to present a surgical approach to reactive lesions for rehabilitation purposes in an elderly patient. A 63-year-old male patient presented to the dental clinic of a higher education institution reporting swelling in the inner/upper part of his mouth, as well as aesthetic dissatisfaction when smiling. During the medical history, he reported that he had used the prosthesis for approximately 5 months but stopped using it continuously due to severe pain in the areas corresponding to the "edges" of the prosthesis, but he did use it while chewing. On intraoral examination, lesions with a hyperplastic appearance could be highlighted in the fundus of the vestibule of the upper arch bilaterally. Based on the clinical characteristics presented, it was decided with the patient to remove the lesions surgically, followed by referral for prosthetic rehabilitation. The fragments were removed and sent for confirmation of the condition, confirming the hypothesis of reactive fibroepithelial hyperplasia. The patient was then referred for prosthetic rehabilitation. Surgical approach for conditioning of gingival tissue prior to rehabilitation treatment is, therefore, a viable option with a good prognosis when it comes to a reactive lesion, making the environment suitable for the installation of removable prostheses. - Abstract
Extraction of third molars can cause some postoperative complications for patients, among the main ones are pain, trismus, infections and alveolitis. Currently, there are some materials that reduce such sequelae, enhancing faster healing of the region. The Platelet- and Leukocyte-Rich Fibrin (LPRF) membrane emerged more than 20 years ago and has since been associated with dental procedures, such as tooth extractions. L-FRP is defined as a material an autogenous material, capable of promoting the regeneration of hard and soft tissues. This material has the advantages of biocompatibility, low cost and easy protocol over synthetics. The objective of this study is to report a clinical case of third molar extraction in which the patient underwent a blinded study assessment, with the placement of L-PRF after tooth extraction on one side and the procedure being performed conventionally on the other. After 30 days postoperatively, better tissue healing was observed in the region where the fibrin membrane was placed. [ABSTRACT FROM AUTHOR]

- Abstract
Marsupialization, or Partsch 1 technique, is a very common treatment technique used in more extensive cyst lesions. It is considered a more conservative and easy-to-perform treatment, although cystectomy or Partsch 2 is the technique of choice for treating odontogenic cysts. Marsupialization promotes a reduction in the existing intracystic pressure, leading to a reduction in the cyst and subsequently bone neoformation. The aim of this article was to present a brief literature review on marsupialization and to report a clinical case. Case report: Male, melanodermic, 54 years old. The treatment of choice was marsupialization, mainly because the patient was a user of antiresorptive drugs as an adjuvant therapy for prostate cancer, minimizing the risk of bone exposure. The result of the anatomopathological examination was compatible with a periapical cyst. Conclusion: After one year's follow-up, we were able to see total regression of the facial deformity previously seen intraorally and extraoral. [ABSTRACT FROM AUTHOR]

RESUMO Buscou-se investigar e conhecer a frequência e a distribuição de frenotomias e fenectomias realizadas no Sistema Único de Saúde (SUS) após os dispositivos legais que instituíram o teste da linguinha, bem como entender quais profissionais estão realizando o procedimento cirúrgico. Estudo descritivo analítico, retrospectivo, com dados secundários, disponíveis no Sistema de Informações Ambulatoriais do DataSUS, extraídos por meio do TabWin. Observou-se um aumento considerável no número de procedimentos, porém, com flutuações nos anos seguintes. Das frenectomias registradas no SUS, no período, 33% foram realizadas por dentistas na atenção primária. Os resultados apontaram que houve um aumento substancial no número de procedimentos cirúrgicos em dois momentos, em 2014 e em 2017, possivelmente em decorrência da publicação da Lei nº 13.002/2014 e da Nota Técnica (NT) nº 09/2016. A NT nº 35/2018 deslocou o diagnóstico e o tratamento para a média complexidade, possivelmente resultando em filas, principalmente em se tratando de cirurgias que poderiam ser realizadas na atenção primária. Apesar de não haver unanimidade quanto à correlação anquiloglossia e desmame, não parece haver um monitoramento para evitar cirurgias desnecessárias, uma vez que a ordenha pode não ser afetada e o acompanhamento ser realizado para confirmar ou não essa necessidade.

15. Lipoma intramuscular em língua: relato de caso e atualização da literatura

Objetivo: Descrever um caso de paciente com lipoma oral intramuscular e compilar os principais aspectos dessa rara lesão. Relato do caso: Mulher caucasiana de 78 anos queixou-se de inchaço na língua. O aumento de volume apresentava cerca de 5 anos de evolução. O exame físico revelou uma lesão nodular na borda lateral direita da língua com diâmetro de aproximadamente 2,5 cm, macia ao toque, superfície lisa sem indícios de trauma, coloração semelhante à da mucosa lingual e com limites nítidos. Realizou-se biópsia excisional e o exame histopatológico revelou tecido adiposo maduro, com fibras musculares aprisionadas no interior e ao redor da lesão, ausência de atipias citológicas e mitoses, estabelecendo o diagnóstico definitivo de lipoma intramuscular. Não houve recorrência durante um período de 4 anos. Discussão: Lipoma é uma neoplasia benigna composta por adipócitos maduros, incomum na cavidade oral. Seu padrão intramuscular é uma forma rara de lipoma com comportamento infiltrativo e taxa de recorrência maior que o lipoma comum. Sua etiopatogenia não é totalmente compreendida e seu tratamento padrão é a excisão cirúrgica. Essa lesão faz diagnóstico diferencial com outras patologias importantes, inclusive o lipossarcoma. Existem poucos casos documentados de lipomas intramusculares em língua. Conclusão: O lipoma oral intramuscular é raro e o cirurgião-dentista precisa atentar ao seu diagnóstico diferencial com lipossarcoma e ao seu maior potencial de recidiva em relação ao lipoma comum.

Pyogenic Granuloma is considered a non-neoplastic proliferative process, with multifactorial causes, including the frequent use of some medications, such as phenytoin, the drug of first choice for the treatment of epilepsy, which has a prevalence of 50% in terms of the development of gingival hyperplasia. The lesion usually presents with a smooth or lobulated surface, and may be pedunculated or sessile, often painless, but easily bleeding. The size varies from a few millimeters to large lesions. The gingiva is the most affected site, but it can also be found on the tongue, hard palate, lip, and buccal mucosa, less frequently. The treatment of choice is excisional biopsy with a small safety margin, accompanied by prophylactic oral hygiene measures. The objective of this study is to report a case of pyogenic granuloma on the dorsum of the tongue, in a 47-year-old female patient, with epilepsy as a comorbidity, user of phenytoin, as well as to carry out a brief review of the literature on the induction of proliferative growths non-neoplastic disorders associated with the use of this medication. 17. Exérese de tórus mandibular bilateral com finalidade protética: relato de caso

O tórus é uma protuberância óssea benigna, que acomete a maxila e a mandíbula, de etiologia multifatorial, estando associado a fatores genéticos e ambientais. O objetivo deste estudo é relatar um caso clínico com foco no manejo cirúrgico de tórus mandibular bilateral que necessitou de remoção cirúrgica com finalidade protética. Paciente, do sexo masculino, de 64 anos, compareceu à Clínica Odontológica da Faculdade Adventista da Bahia (FADBA), buscando tratamento reabilitador protético. Ao exame físico intraoral, presença de edentulismo parcial em ambas as arcadas dentárias, bem como presença de massa endurecida e assintomática em rebordo alveolar lingual mandibular bilateralmente foram constatadas. Optou-se, assim, por realizar a cirurgia para exérese do tórus mandibular. Foi realizado bloqueio do nervo alveolar inferior e lingual bilateralmente, confecção do retalho tipo envelope, descolamento mucoperiosteal, osteotomia com sulcos de orientação e clivagem dos blocos ósseos, regularização do rebordo com broca maxicut, irrigação copiosa com soro fisiológico, seguida de sutura interpapilar. Por tratar-se de patologia assintomática na maior parte dos casos, dispensa tratamento. Em casos de maior extensão, no entanto, quando ocorrem lesões traumáticas de repetição relacionadas à alimentação, às alterações fonéticas ou algum empecilho para o tratamento protético reabilitador, indica-se remoção cirúrgica. Após a avaliação clínica, foi definido o diagnóstico de tórus mandibular, esclareceu-se o paciente sobre a necessidade de remoção cirúrgica, uma vez que a presença das exostoses impossibilitaria o tratamento protético. Após a cirurgia, o paciente foi encaminhado para dar seguimento ao seu tratamento reabilitador.

Periapical cysts of endodontic origin originate from an infection that affects the apical region, causing pulpal necrosis that stimulates an inflammatory response. Among many pathologies found on radiographs, one of them is the root cyst, considered to be a lesion frequently found in the maxilla and mandible, associated with the apex of a tooth with pulp necrosis. They present slow growth and are discovered in routine examinations because they are asymptomatic. Due to the chronic aggression, the lesion does not present painful symptoms in most cases, and grows slowly, thus being able to reach large extensions, with the presence of swelling and sensitivity, as well as slight mobility in the affected tooth and adjacent ones. The objective of this study was to report a clinical case of swelling in the anterior region of the mandible, which was diagnosed as a periapical cyst, in this way the treatments performed and the final result after 18 months of follow-up will be reported. However, to achieve good results during periapical cyst treatment, it is necessary to make a correct diagnosis, followed by adequate planning, always performing clinical and radiographic follow-up, in addition to the patient's cooperation, so that good results can be achieved during the treatment. It can be observed that a well-performed endodontic treatment, associated with a elaborated surgical technique, in addition to anatomopathological evidence and radiographic follow-up, make the therapeutic success to be obtained in cases of large periapical cysts. Aim: To evaluate, tomographically, in two clinical cases, the effect of the new MTA formulation of the Angelus, the MTA Repair HP, in the contribution to remission of periapical lesions in paraendodontic surgery, as well its physical properties and manipulation. Case report: Patient R.C.R, 57, attended at the Faculty of Dentistry of the Federal University of Juiz de Fora for evaluation of periapical lesion in the element 11. It was observed, at the clinical and radiographic exams, endodontic treatment and a circumscribed, unilocular and radiolucid area at the apex of the tooth. It was requested a CBCT exam (I-Cat®, Imaging Sciences International, Hatfield, Pennsylvania, USA), which confirmed the presence of the lesion and rupture of the buccal cortical plate. Thus, it was performed curettage, apicectomy, retropreparation and retrofilling with the described cement. It was solicited two additional CBCT exams at the period of three and five months post-operative. Patient R.J.G.M, 46, with mean complain of pain, increased intraoral volume and purulent discharged in the upper left arch. The clinical and radiographic exams revealed fistula and refractory lesion, despite the endodontic treatment of the tooth 25. A CBCT exam was done and it was observed a circumscribed hypodense area, around the apex of the 25, with rupture of the buccal cortical plate. It was suggested the same surgical treatment described in the case above. It was performed two more CBCT exam, at three and five months after the procedure. Discussion: The MTA Repair HP was developed from the necessity of a more biocompatible material with the dental structures in the retrograde filling process. This product helps in the remission of periapicals lesions and in the new formation of the periodontum components; also it has the advantages of longer setting time, not causing the darkening of the coronary portion and easier manipulation. Conclusion: Regarding the remission of the lesions, it was noticed the generation of calcified tissue across the periods of three and five months. Regarding the manipulation and application, it was not observed any improvement related to the addition of the plasticizer. However, it was noticed a shorter setting time, making it difficult to insert the material.

Introdução: O câncer está atualmente entre as cinco causas mais frequentes de mortes no planeta, tudo isso devido a hábitos deletérios ou predisposição genética. O câncer de boca ocupa uma posição de destaque nesse panorama e necessita de uma atenção odontológica especial, devido à associação da perda dentária com o tratamento oncológico. Objetivo: O objetivo deste trabalho foi fazer uma análise biológica dos processos desencadeados pelo tratamento radioterápico e apresentar um método alternativo de exodontia seguro para os pacientes oncológicos. Revisão da literatura: O tratamento oncológico radioterápico desencadeia uma série de complicações bucais que eventualmente exigirão a exodontia, entre eles: osteorradionecrose, hipossalivação, cáries, doença periodontal, entre outros. Discussão: A exodontia com elásticos ortodônticos consiste no envolvimento dentário na altura cervical com os elásticos causando uma esfoliação lenta, gradual e mais atraumática possível da raiz, esse processo leva em torno de 5 a 8 semanas. Esta técnica associada a um bom pós-operatório garantirá uma cicatrização adequada aumentando significativamente as chances de um prognóstico positivo. Conclusão: Os resultados dessa associação de técnicas são seguros e diminuem os riscos de complicações pós-operatória, garantindo assim, uma recuperação sem maiores problemas ao paciente.

Objetivo: Relatar dois casos clínicos de alteração de frênulo lingual em gemelares, com 10 anos de idade, ambas do sexo feminino, avaliadas nos aspectos anatômicos e funcionais da língua antes e 15 dias após a frenectomia. Relato de casos: Inicialmente, o odontopediatra realizou a anamnese com a mãe. Em seguida, as participantes foram submetidas à avaliação clínica odontológica e, posteriormente, foi realizada a avaliação fonoaudiológica do frênulo lingual. A avaliação fonoaudiológica foi realizada por meio do Protocolo de Avaliação do Frênulo da Língua, que foi aplicado antes e 15 dias após a frenectomia. As duas participantes apresentaram melhor desempenho em provas anatômicas após o procedimento cirúrgico. Entretanto, algumas alterações, principalmente ao elevar a língua, continuaram sendo observadas em uma das pacientes 15 dias após a frenectomia. Ademais, as duas participantes passaram a executar a maioria dos movimentos de língua com maior precisão. No que se refere à fala, somente a participante que apresentava um maior número de alterações fonéticas anteriores, permaneceu com algumas alterações na fala decorridos 15 dias da cirurgia. Discussão: Algumas dificuldades envolvendo mobilidade da língua e alterações na fala podem persistir após a realização da cirurgia, indicando a necessidade de intervenção fonoaudiológica específica. Conclusão: Ambos os casos apresentaram melhora no desempenho tanto nas provas anatômicas quanto funcionais após a frenectomia.

Aim: The objectives of this study were to evaluate the prevalence of the mandibular third molars position in according to the Winter and Pell & Gregory classification, and to determine the levels of difficulty for exodontia through the Pederson index. Materials and Methods: Radiographic, cross-sectional and retrospective study evaluated 100 panoramic radiographs of patients between 15 and 61 years old, from dental records seen at the Oral Surgery department in the period from 2015 to 2017. The panoramic radiographs were evaluated using ImageJ software regarding the positioning of the lower third molars by the Winter and Pell & Gregory classification, while the level of difficulty was evaluated by the Pederson index. The results obtained were submitted to descriptive statistical analysis. Results: The highest prevalence of included third molars was observed in women, with an age average of 26 years. Regarding the position of the included teeth, the vertical position (44.0%), level A (74.7%), and class III (69.9%) were the most prevalent. In the Pederson scale, the “very difficult” surgical difficulty was the most found. Discussion: results suggest establishing better surgical planning and prognosis for patients, minimizing the possibility of complications during and after extractions of mandibular third molars. Conclusion: Both classifications help the dental surgeon to establish better planning and prognosis for patients. The applicability of panoramic radiographs, associated with clinical evaluation, justifies a fewer possibility of complications in the lower third molar post-extraction.

Sialolithiasis is a pathology characterized by obstruction of salivary flow due to the deposition of calcium salts in the gland ducts. Among the main salivary glands most involved are the parotid, submandibular and sublingual glands, the submandibular being the most affected by the pathology, representing 90% of cases, and the parotid glands from 5 to 20%. The literature discusses several hypotheses for its etiology, and the association of clinical and radiographic exams are essential for the diagnosis. The present study aims to report a clinical case of sialolithiasis of the parotid duct gland in a 64- year-old Caucasian male patient, seen at the clinic school of the Recife School of Dentistry, who presented with a clinical complaint of facial pain and swelling for about 30 days. After the conclusive diagnosis, he underwent surgical treatment through an intraoral approach to remove sialoliths and place an intraductal catheter to prevent ductal stenosis, under local anesthesia. The patient responded well to the postoperative period, presenting no clinical complaints, returning to the school clinic after 8 days for removal of the synthesis and clinical follow-up. Numerous complications can occur during dental surgical procedures, and oroantral communication is one of the most discussed in literature. That condition results mainly from extractions of posterior superior dental elements, given its proximity to the maxillary sinus. Its diagnosis is basically made through the Valsalva maneuver and analysis of fragment, however complementary exams can demonstrate that condition. The literature also considers the treatment approach can be quite diversified. Objective: To present, through a narrative review of literature, technical possibilities of clinical approaches for cases of oroantral communication. Methodology: The bibliographic research was online, including articles in Portuguese, English and Spanish, published in the last 10 years, except for those fundamental on the topic, with longer publication time. Letters to editor and editorials were also excluded. A descriptive analysis of articles was then carried out and the data were organized in order to support dentistry professionals to intervene safely in face of that complication. Final Considerations: Oroantral communication is a reality that can occur in the dental office. Its consequences are quite varied, and it is up to the dental surgeon, in addition to avoiding and diagnosing, to know how to intervene effectively when necessary, given that its complications may limit the patient's quality of life. The anticholinergic drugs with a variety of pharmacological activities produce xerostomia as one of the main side effects. The aim of this study was to identify the prevalence of the use of anticholinergic drugs by patients from a dental surgery clinic and evaluate the risk of potential drug interactions. A cross-sectional pilot study was carried out, including 24 patients treated at the Surgery Clinic of the Dentistry Faculty of Federal University of Juiz de Fora. Data was collected by exploratory questionnaire. The drugs were classified according to the Brazilian anticholinergic activity drug scale. Potential drug interactions were analyzed by Drugs® through the medication used by each patient alongside simulations of commonly used drugs in Odontology. A 29% prevalence in the use of at least 1 medication with anticholinergic activity was observed. Twelve drugs with anticholinergic activity were identified, especially with activity in the Central Nervous System. Furthermore, 17 potential drug interactions of serious or moderate clinical importance were identified. An important use of anticholinergic drugs was observed, with a theoretical risk of clinically relevant drug interactions in patients who need oral surgery. [ABSTRACT FROM AUTHOR]

36. Tratamento conservador em ceratocístico odontogênico utilizando um dispositivo intra-ósseo: relato de caso

O ceratocisto odontogênico (CO) classificado pela Organização Mundial da Saúde (OMS) como uma lesão cística de desenvolvimento benigna, se destaca entre as demais lesões odontogênicas em virtude do comportamento biológico potencialmente agressivo, com tendência a recorrências. O presente trabalho tem como objetivo, relatar um caso CO em mandíbula, bem como discutir aspectos relacionados aos achados radiográficos e tipos de tratamentos dessa lesão. Paciente do sexo masculino, leucoderma, exibia uma lesão radiolúcida circunscrita, e consistência endurecida, com duração de seis meses, medindo aproximadamente 1cm de extensão em região posterior de mandíbula esquerda. Sob a hipótese clínica de cisto odontogênico de desenvolvimento, foi realizada biópsia incisional. Com base no diagnóstico definitivo de CO após realização do exame histopalógico, foi realizada a remoção das unidades dentárias acometidas e colocado um dispositivo intraósseo para que houvesse a descompressão do CO. Após seis meses foi observada a regressão parcial da lesão, e o dispositivo ósseo foi retirado e o remanescente da lesão removida realizando osteotomia periférica. Após um ano de proservação não foi observada recidiva do CO até o presente momento. Os achados do presente caso ressaltam a importância do diagnóstico clínico e radiográfico, assim como o correto tratamento cirúrgico, enaltecendo a importância de um exame clínico minucioso da cavidade oral.

Tem sido associado a alguns casos em consequência de tratamentos dentários, como extração de terceiros molares e tratamento endodôntico. O diagnóstico precoce e o manejo correto são fundamentais para a prevenção da progressão da lesão e futuras complicações, pois pode se espalhar pelos profundos planos faciais do pescoço, ocasionando enfisema retrofaríngeo e, ainda, pode causar pneumotórax e pneumomediastino. Objetivo: Avaliar a ocorrência de enfisema subcutâneo no estado do Rio Grande do Sul. Metodologia: Um questionário foi enviado por e-mail aos profissionais especialistas em Cirurgia e Traumatologia Bucomaxilofacial (CTBMF), Periodontia e Endodontia registrados no Conselho Regional de Odontologia (CRO-RS), contendo 8 perguntas: especialidade, conhecimento em relação ao enfisema subcutâneo e causa, número de cirurgias realizadas mensalmente, instrumento mais utilizado para procedimento cirúrgico, instrumento envolvido nos casos conhecidos, instrumento utilizado para secagem dos canais radiculares em endodontia, se já possuiu paciente com enfisema subcutâneo e se ouviu falar sobre enfisema durante a prática clínica. Resultados: Foram enviados 1954 questionários, obtendo 260 respostas dentro dos critérios da pesquisa (13,3%). A grande maioria da amostra (89,1%) relatou ter conhecimento sobre enfisema subcutâneo e suas complicações e 28 (10,7%) profissionais já tiveram casos relacionados a procedimentos odontológicos. Conclusões: O conhecimento por parte dos profissionais da área da odontologia sobre enfisema subcutâneo é importante, pois a partir desse estudo observamos que, apesar de raro, 10,7% dos entrevistados já tiveram casos de enfisema subcutâneo relacionados a procedimentos odontológicos.

Partial or total rehabilitation in edentulous patients with implants has become part of the clinical routine with long-term results. However, regions without adequate bone volume make it impossible to install implants. Some procedures increase bone thickness. A Split Crest technique shows the clinical advantages over other surgical procedures with the same practice. The objective of this work is to relate the Split Crest technique followed by the installation of implants. Female patient, 44 years old, without systemic involvement, underwent the installation of two implants in the region of teeth 25 and 26 associated with tissue regeneration guided with biomaterial. Radicular cysts are the most common odontogenic cysts. Mainly because it is a pathology of common occurrence, silent and asymptomatic, the request for routine orthopantomographic examination allows the diagnosis in the incipient stage of the lesion, which minimizes the degree of operative trauma, which occurs when it is diagnosed late. This study aimed to report a case of radicular cyst in the mandible treated by total enucleation and filling the operating room with fibrin sponge, which allowed the primary closure of the lesion in a 51-year-old male patient, culminating in the injury healing process. Exodontics is one of the techniques most used by dentists in dental clinics. In any surgery, it is essential that the operator request the complementary radiographic examination. For the same, it indicates structures not visible present in the operative field. In cases of third molar extractions, it is essential to check the orthopantomography. Since, if there is proximity to the roots, with the inferior alveolar nerve, it can originate a state of temporary or permanent paresthesia in the patient. Introdução: Os dentes supranumerários podem apresentar-se em todas as regiões da mandíbula ou maxila, mas existe uma predisposição para localizarem-se na região anterior da maxila. Objetivo: Relatar um caso de localização incomum de dente supranumerário fusionado na região posterior da maxila. Relato de Caso: A radiografia panorâmica sugeriu uma alteração da forma do terceiro molar superior, porém não foi possível obter o diagnóstico final. Assim, a tomografia computadorizada de feixe cônico foi utilizada para determinar o diagnóstico de fusão e também auxiliar no planejamento cirúrgico.Foi realizada uma intervenção cirúrgica com o objetivo de remover o terceiro molar fusionado ao supranumerário.Conclusão:Neste relato, observamos que a tomografia computadorizada de feixe cônico mostrou detalhes da ocorrência rara, permitiu o estabelecimento do diagnóstico e possibilitou a correta abordagem do caso, evitando intercorrências e complicações cirúrgicas.

Introdução: O adenoma pleomórfico, uma neoplasia benigna, é uma das patologias mais comuns de glândula salivar, cujo tratamento é sua excisão cirúrgica. O objetivo deste estudo é relatar um caso de adenoma pleomórfico de grande proporção em glândula salivar acessória na região do palato duro. Relato de caso: Paciente do sexo feminino, 22 anos, encaminhada de sua unidade básica de saúde devido a uma lesão em região de palato, assintomática, com evolução de 1 ano. No exame clínico intra-bucal observou-se um nódulo extenso em palato do lado direito. A biópsia incisional revelou o diagnóstico de adenoma pleomórfico. A paciente foi submetida a excisão cirúrgica da lesão sob anestesia geral. Apresentou boa evolução pós-operatória, sem recidivas. Discussão: O Adenoma Pleomórfico é uma neoplasia de diagnóstico clínico, com confirmação a partir de exame anatomopatológico. Embora seja uma patologia de crescimento lento, quando o tratamento não é realizado de maneira precoce, ele pode atingir grandes dimensões, o que leva a um aumento da morbidade do procedimento cirúrgico e pode levar a maiores taxas de recidiva e/ou transformação maligna. Conclusão: Salientamos a importância de um diagnóstico preciso e um tratamento efetivo com a maior brevidade. Quando possível, as próteses obturadoras/aparelhos intraorais acrílicos podem e devem ser utilizados de forma associada aos tratamentos cirúrgicos de tumores; assim, restabelecendo função e dando conforto no pós-operatório.

Foreign body is an object or structure included accidentally or intentionality in the intimacy of the organic tissues of individuals. They can trigger important inflammatory/infectious processes, depending on the nature of the same, requiring its surgical removal in most cases, in order to prevent damage to the patient. This study aimed to report a case of a foreign body (fragment of a dental crown) accidentally included by dento-alveolar trauma in lower lip in a twelve-year-old female patient, which was neglected when attending the patient in the emergency room, having surgical removal was necessary 06 months after the accident. The authors emphasize the need for a thorough clinical examination of the patient when facial trauma occurs, regardless of the magnitude, especially when soft tissue wounds occur and with loss of dental substance. Squamous papilloma is a slow-growing lesion, its etiology being associated with the human papilloma virus. They are mostly painless neoplastic lesions, being the most frequent papillary lesion of the oral cavity. The treatment for this type of injury is surgical with a small safety margin. The aim of the present study is to report a clinical case of recurrent squamous papilloma affecting the mucosa of the maxillary alveolar process in a 78-year-old female patient. - Abstract
The Odontogenic Keratocyst (OK) is an odontogenic diagnosis of development that mainly occurs in posterior region of the mandible. It's characterized for your aggressive behavior, infiltrative development and high relapse rates (5 to 62%). The OK was recently reallocated to the cyst category and the conservative treatment was highly defended. The present study reports a clinical case of a 25-year-old non-syndromic male patient, who was referred to HUGOL after a routine radiographic examination showing an image suggestive of an injury. Aspiration puncture and incisional biopsy of the cystic lesion were performed. Histopathologically, a cystic cavity covered by stratified parakeratinized squamous epithelium was observed, confirming the clinical suspicion of OQ. The proposed treatment was enucleation of the lesion, together with extraction of the 48. After 12 months of proservation, the patient evolved with a good healing aspect of the surgical wound and imaging control showing significant and satisfactory regression of the lesion. - Abstract
The biopsy is a surgical intervention which is removing part or the entire injury, examining after such procedure as histopathological features of the specimen. Gingival recession is defined as the dislocation of the gingival margin apically to the cementoenamel junction. Among the surgical techniques used for root coverage, the conjunctive tissue graft stands out. The objective of this study is to report the clinical case of a patient treated at the dental specialization center of stomatology of the Asces-Unita school of dentistry, in which she presented a defect in the gingival region inserted after excisional biopsy. The patient reported gum lesion for four months, without symptoms, she was submitted to excisional biopsy and after the procedure reported sensitivity in the region, where through oral examination the presence of root exposure was seen. In the face of this condition, conjunctive tissue graft was performed to correct the gingival defect. The use of subepithelial conjunctive tissue graft to reduce root exposure is a viable alternative, with predictable results and short- and long-term stability. [ABSTRACT FROM AUTHOR]
