New surgical procedures can expose patients to harm and should be carefully evaluated before widespread use. The InSpace balloon (Stryker, USA) is an innovative surgical device used to treat people with rotator cuff tears that cannot be repaired. We aimed to determine the effectiveness of the InSpace balloon for people with irreparable rotator cuff tears.We conducted a double-blind, group-sequential, adaptive randomised controlled trial in 24 hospitals in the UK, comparing arthroscopic debridement of the subacromial space with biceps tenotomy (debridement only group) with the same procedure but including insertion of the InSpace balloon (debridement with device group). Participants had an irreparable rotator cuff tear, which had not resolved with conservative treatment, and they had symptoms warranting surgery. Eligibility was confirmed intraoperatively before randomly assigning (1:1) participants to a treatment group using a remote computer system. Participants and assessors were masked to group assignment. Masking was achieved by using identical incisions for both procedures, blinding the operation note, and a consistent rehabilitation programme was offered regardless of group allocation. The primary outcome was the Oxford Shoulder Score at 12 months. Pre-trial simulations using data from early and late timepoints informed stopping boundaries for two interim analyses. The primary analysis was on a modified intention-to-treat basis, adjusted for the planned interim analysis. The trial was registered with ISRCTN, ISRCTN17825590.Between June 1, 2018, and July 30, 2020, we assessed 385 people for eligibility, of which 317 were eligible. 249 (79%) people consented for inclusion in the study. 117 participants were randomly allocated to a treatment group, 61 participants to the debridement only group and 56 to the debridement with device group. A predefined stopping boundary was met at the first interim analysis and recruitment stopped with 117 participants randomised. 43% of participants were female, 57% were male. We obtained primary outcome data for 114 (97%) participants. The mean Oxford Shoulder Score at 12 months was 34·3 (SD 11·1) in the debridement only group and 30·3 (10·9) in the debridement with device group (mean difference adjusted for adaptive design -4·2 [95% CI -8·2 to -0·26];p=0·037) favouring control. There was no difference in adverse events between the two groups.In an efficient, adaptive trial design, our results favoured the debridement only group. We do not recommend the InSpace balloon for the treatment of irreparable rotator cuff tears.Efficacy and Mechanism Evaluation Programme, a Medical Research Council and National Institute for Health and Care Research partnership.

As the American eldercare industry prepares to attract and receive consumers from the “baby boomer” generation, facility designers and administrators are increasingly concerned with catering to the lifestyles and taste preferences of aging adults perceived to be more “active,” affluent, and accustomed to “choice” than previous generations. This article considers these trends in terms of their material, aesthetic, and discursive impacts on the socio-material construction of space in residential eldercare facilities. The study draws on discourse and visual analysis of winning entries in published design competitions sponsored by the architecture, interior design, and eldercare industries. Through this analysis, eldercare spaces emerge as sites of consumption where designers privilege both “household/home-like” and “commercial/hospitality” aesthetics and atmospheres. In recent years, placemaking strategies to create home-like environments increasingly overlap with social spaces inspired by the hospitality industry. Our discussion demonstrates how these strategies materialize in structures and interiors increasingly open to the non-resident public and integrated into their surrounding communities. As such, we argue that the negotiation of degrees and kinds of “publicness” and “privateness” in spaces of care reflect shifting views of the roles and characteristics of acts of consumption and consumers in these facilities and in the broader healthcare industry.

Emerging Voices on New Architectural Ecologies is a selection of essays from worldwide researchers who respond to the Lisbon Triennale Millennium bcp Universities Competition open call expanding perspectives on the topics of the four main exhibitions of the Triennale 2022, Terra. The 2022 Triennale looks into communities throughout the world to embrace our common home, planet Earth. How do resource depletion, socio-economic inequalities, and climate alter-actions intertwine at different scales? Terra is a call to action inspired by shared local insights from around the globe with the potential to transform the current fragmented linear system of cities-as-machines into a circular holistic model of cities-as-organisms. In the imagination of prominent architects and architectural theorists, the detail figures as both a promising and perilous element of built form. This paper explores the tension of the “make or break” qualities of the architectural detail(s) in the learning and doing of design, through an ethnomethodologically-informed ethnography of North American design-build architecture education. The discussion parses three episodes which highlight the generative qualities of the detail, in terms of its pedagogical value for architecture, and as a heuristic for the study of design practice. As both the material process of joining disparate elements, and a locus for complex social and professional relations and meaning-making activities, the assembly of the detail is inherently messy, though productively so.

This paper reflects on the outcomes and lessons learned from a wearable circuit class project that brought together students in two upper-division undergraduate and graduate level courses: Textiles, Technology and Culture and Experimental Apparel Design. The assignment included both individual and small group work. Each individual produced a machine-embroidered design motif and wearable LED circuit component inspired by a motif or symbol from a cultural region or historical era previously covered in Textiles, Technology and Culture. This component was then coordinated with those of peers, as well as integrated with a garment design created by a student in Experimental Apparel Design. The learning objectives for both courses included technical, design, and humanistic skill building, along with social and communication skills. Our objective was to combine design and humanities approaches and research methods in a project pushing the boundaries of the technical skills of the students.

Fashion curation is an integral form of public scholarship within our field and yet has historically been overlooked as such. Unlike creative design scholarship, we have not collectively developed nor agreed upon methods for peer review. As a result, curatorial work is often not considered in tenure-review processes, despite fashion exhibitions meeting the core principles of disseminating scholarship beyond the academy. A recent article in the Clothing and Textiles Research Journal, “Fashion Exhibitions as Scholarship,” challenged members of our field to implement peer review of fashion curation as a way to encourage and elevate this important form of scholarship (Green, et al., 2019). In this experimental salon session, we built upon Green et al.’s (2019) work by using the innovative salon session format as a juried venue for presenting fashion exhibitions rooted in rigor and research. Five fashion exhibitions were presented and discussed during the session.

State attempts to rationalize Moroccan craft education reflect the ambivalent status of traditional knowledge in a modern economy. Female artisans, recently organized as a cooperative, navigate this ambivalence in a weaving “theory” class and in their “occupation” of the cooperative structure itself. During performances of learning and cooperation, weavers fulfill the terms of the state's policies and their roles as “economic citizens,” while accomplishing local modes of belonging and the management of social relations.

This article explores multimodal communication and social interaction in university-level architecture education. Drawing on ethnography of North American programs of ‘design-build’ architecture, we consider how the judgment of a ‘good’ (or ‘bad’) design is as much a result of how it is communicated as what is communicated. In settings like the design ‘review’, students endeavor to persuade an audience of the merits of their proposed design. This is ideally accomplished through the ‘convergence’ of multiple design media on the same ‘idea’ or design gestalt. ‘Convergence’ involves not just technical competency; it is also a social achievement: an effect of composing and coordinating multimodal semiotic media according to shared representational and communicative conventions. Failure to recognize convergence is often an effect of intersemiotic dissonance. This is also the risk of a design’s failure in the eyes of the faculty jury, who often direct their critiques toward communicative inconsistencies.

- Abstract
This book engages readers via the international contributions from “home” field sites around the world and international authors. Importantly, the various chapters address a wide spectrum of educational contexts – ranging from higher education, to K-12 public and private schools, to prison schools. The realistic accounts portrayed in each of the chapters address how local collaborations are instantiated through the research process, from access and data collection to the write-up phases. The major themes that emerge across the chapters highlight 1) positionality and negotiation of multiple roles, i.e., researcher, educator, colleague, friend, community member; 2) reconciling multiple, hybrid, and intersectional identities with varying insider/outsider statuses vis-à-vis research participants; 3) resulting power dynamics in connection to relational identities – sometimes conflicting, consolidating, equalizing, and/or elevating; 4) innovative methodological responses to these dilemmas; and 5) integrated research designs and research ethics, offering possibilities for participation and insights on the social impact of research findings. The book's chapters thus individually and collectively treat and resolve local ways of doing home (field) work and highlight the creation and sharing of knowledge among researchers and research participants.

Background: cancellation of planned surgery impacts substantially on patients and health systems. This study describes the incidence and reasons for cancellation of inpatient surgery in the UK NHS. Methods: we conducted a prospective observational cohort study over 7 consecutive days in March 2017 in 245 NHS hospitals. Occurrences and reasons for previous surgical cancellations were recorded. Using multilevel logistic regression, we identified patient- and hospital-level factors associated with cancellation due to inadequate bed capacity. Results: we analysed data from 14 936 patients undergoing planned surgery. A total of 1499 patients (10.0%) reported previous cancellation for the same procedure; contemporaneous hospital census data indicated that 13.9% patients attending inpatient operations were cancelled on the day of surgery. Non-clinical reasons, predominantly inadequate bed capacity, accounted for a large proportion of previous cancellations. Independent risk factors for cancellation due to inadequate bed capacity included requirement for postoperative critical care [odds ratio (OR)=2.92; 95% confidence interval (CI), 2.12–4.02; PConclusions: a significant proportion of patients presenting for surgery have experienced a previous cancellation for the same procedure. Cancer surgery is relatively protected, but bed capacity, including postoperative critical care requirements, are significant risk factors for previous cancellations.

This paper looks at the relationship between shifting moral codes and changing undergarments for American women in the 1920s. While existing work in dress and social history focuses on the length of hemlines, bobbed hair, contraception, and women gaining the right to vote, little attention has been given to women's lingerie and the messages about sexuality women received throughout their day. Drawing on historical and gender studies perspectives and methods, this study examines representations of women's undergarments in popular magazines and other media from the period. The discussion explores the symbolism of early lingerie in discourse surrounding women's sexuality in the 20s, and how messages about lingerie and sexuality were crucial to the shaping of the 1920s woman.

This paper explores what it means to be a "traditional" textile artisan in a modern, globalizing economy. The discussion examines three discourses that converge on artisanal textile production in colonial and post-colonial Morocco: fair trade and development-motivated representations of "craft"; colonial administrative and scholarly analyses of the Moroccan craft sector; and the words and biographies of contemporary artisans. The argument draws on over two years of ethnographic fieldwork in Morocco, research in the archives of the French Protectorate (1912-1956), and analysis of published colonial scholarship, post-colonial handicrafts policy, and representations of "traditional" crafts and their makers in today's global marketplace. While these discourses partially align in their mutual ambivalence towards capitalism, the ways they cast the relationship between artisans and their labor differ substantially. This research explores the concept of alienation within these various discourses, and troubles its relevance to scholarship on contemporary craft in non-Western contexts.

Conducting ethnographic research on ‘traditional’ craftsmanship and its objects in contemporary Morocco, one cannot help but acknowledge an uncomfortable proximity to French colonial scholarship. In no small part current constructions of ‘traditional’ handicrafts reflect the discursive residues of colonial knowledge production. The massive Protectorate project of ethnographic documentation and description focused in large part on mapping out craft guild structures, handicraft techniques, types of objects and regional styles of dress. These earlier French models treat handicrafts as expressions of collective identities, categorised in terms of gender, ethnicity, religious beliefs, residency, and tribe. Present day representations of artisans and handicraft objects recapitulate these early definitions and standardisations in multiple ways. Since 2005, University College London Hospital (UCLH) has operated a hotel-based Ambulatory Care Unit (ACU) for hematology and oncology patients requiring intensive chemotherapy regimens and hematopoietic stem cell transplants. Between January 2005 and 2011 there were 1443 patient episodes, totaling 9126 patient days, with increasing use over the 6-year period. These were predominantly for hematological malignancy (82%) and sarcoma (17%). Median length of stay was 5 days (range 1-42), varying according to treatment. Clinical review and treatment was provided in the ACU, with patients staying in a local hotel at the hospital's expense. Admission to the inpatient ward was arranged as required, and there was close liaison with the inpatient team to preempt emergency admissions. Of the 523 unscheduled admissions, 87% occurred during working hours.
