2. Epigenetic regulation of SST2 expression in small intestinal neuroendocrine tumors

BackgroundSomatostatin receptor type 2 (SST2) expression is critical for the diagnosis and treatment of neuroendocrine tumors and is associated with improved patient survival. Recent data suggest that epigenetic changes such as DNA methylation and histone modifications play an important role in regulating SST2 expression and tumorigenesis of NETs. However, there are limited data on the association between epigenetic marks and SST2 expression in small intestinal neuroendocrine tumors (SI-NETs).MethodsTissue samples from 16 patients diagnosed with SI-NETs and undergoing surgical resection of the primary tumor at Erasmus MC Rotterdam were analysed for SST2 expression levels and epigenetic marks surrounding the SST2 promoter region, i.e. DNA methylation and histone modifications H3K27me3 and H3K9ac. As a control, 13 normal SI-tissue samples were included.ResultsThe SI-NET samples had high SST2 protein and mRNA expression levels; a median (IQR) of 80% (70-95) SST2-positive cells and 8.2 times elevated SST2 mRNA expression level compared to normal SI-tissue (p=0.0042). In comparison to normal SI-tissue, DNA methylation levels and H3K27me3 levels were significantly lower at five out of the eight targeted CpG positions and at two out of the three examined locations within the SST2 gene promoter region of the SI-NET samples, respectively. No differences in the level of activating histone mark H3K9ac were observed between matched samples. While no correlation was found between histone modification marks and SST2 expression, SST2 mRNA expression levels correlated negatively with DNA methylation within the SST2 promoter region in both normal SI-tissue and SI-NETs (p=0.006 and p=0.04, respectively).ConclusionSI-NETs have lower SST2 promoter methylation levels and lower H3K27me3 methylation levels compared to normal SI-tissue. Moreover, in contrast to the absence of a correlation with SST2 protein expression levels, significant negative correlations were found between SST2 mRNA expression level and the mean level of DNA methylation within the SST2 promoter region in both normal SI-tissue and SI-NET tissue. These results indicate that DNA methylation might be involved in regulating SST2 expression. However, the role of histone modifications in SI-NETs remains elusive.

Pollen extract represents an innovative approach for the management of the clinical symptoms related to prostatitis and pelvic inflammatory disease (PID). In this context, the aims of the present work were to analyze the phenolic composition of a hydroalcoholic extract of PollenAid Plus soft gel capsules, and to evaluate the extract’s cytotoxic effects, in human prostate cancer PC3 cells and human ovary cancer OVCAR-3 cells. Additionally, protective effects were investigated in isolated prostate and ovary specimens exposed to lipopolysaccharide (LPS). The phytochemical investigation identified catechin, chlorogenic acid, gentisic acid, and 3-hydroxytyrosol as the prominent phenolics. The extract did not exert a relevant cytotoxic effect on PC3 and OVCAR-3 cells. However, the extract showed a dose-dependent inhibition of pro-inflammatory IL-6 and TNF-α gene expression in prostate and ovary specimens, and the extract was effective in preventing the LPS-induced upregulation of CAT and SOD gene expression, which are deeply involved in tissue antioxidant defense systems. Finally, a docking approach suggested the capability of catechin and chlorogenic acid to interact with the TRPV1 receptor, playing a master role in prostate inflammation. Overall, the present findings demonstrated anti-inflammatory and antioxidant effects of this formulation; thus, suggesting its capability in the management of the clinical symptoms related to prostatitis and PID.

BackgroundFirst-generation somatostatin receptor ligands (fg-SRLs) represent the first-line medical treatment for acromegaly, recommended in patients with persistent disease after neurosurgery, or when surgical approach is not feasible. Despite the lack of strong recommendations from guidelines and consensus statements, data from national Registries report an increasing use of medical therapy as first-line treatment in acromegaly.ObjectiveWe retrospectively evaluated the potential role of a large number of clinical and radiological parameters in predicting the biochemical response to 6-month treatment with fg-SRLs, in a cohort of naïve acromegaly patients referred to a single tertiary center for pituitary diseases.MethodsUnivariable and multivariable logistic regression and linear regression analyses were performed. Biochemical response was defined based on IGF-1 levels, represented as both categorical (tight control, control, >50% reduction) and continuous (linear % reduction) variables.ResultsFifty-one patients (33 females, median age 57 years) were included in the study. At univariable logistic regression analysis, we found that younger age (≤ 40 years; OR 0.04, p=0.045) and higher BMI (OR 0.866, p=0.034) were associated with a lower chance of achieving >50% IGF-1 reduction. On the contrary, higher IGF-1 xULN values at diagnosis (OR 2.304, p=0.007) and a T2-hypointense tumor (OR 18, p=0.017) were associated with a significantly higher likelihood of achieving >50% IGF-1 reduction after SRL therapy. Of note, dichotomized age, IGF1 xULN at diagnosis, and T2-hypointense signal of the tumor were retained as significant predictors by our multivariable logistic regression model. Furthermore, investigating the presence of predictors to the linear % IGF-1 reduction, we found a negative association with younger age (≤ 40 years; β -0.533, p50% IGF-1 reduction and linear % IGF-1 reduction after 6 month fg-SRL treatment in naïve acromegaly patients. These parameters should be considered in the light of an individualized treatment for acromegaly patients.

Objective The aim of this study was to develop an open-source and reproducible digital quantitative analysis (DIA) of somatostatin receptor subtype 2a (SST2) staining in formalin-fixed paraffin-embedded tissues of pancreatic neuroendocrine tumors (panNETs) and growth hormone (GH)-secreting pituitary adenomas (GHomas). Design SST2 immunostaining of 18 panNETs and 39 GHomas was assessed using a novel DIA protocol and compared with a widely used semi-quantitative immunoreactivity score (IRS). Methods The DIA software calculates the staining intensity/area and the percentage of positive cells (%PC). Four representative images were selected for each sample by two independent selectors (S1 and S2), with the analysis performed by two independent analyzers (A1 and A2). Agreement between observers was calculated using the concordance correlation coefficient (CCC). Results In panNETs, the CCC ranged 0.935–0.977 for intensity/area and 0.942–0.983 for %PC. In GHomas, the CCC ranged 0.963–0.997 for intensity/area and 0.979–0.990 for %PC. In both panNETs and GHomas, the DIA staining intensity was strongly correlated with the IRS (Spearman rho: 0.916–0.969, P P 2 expression, evaluated both as DIA intensity/area (Spearman rho: −0.448 to −0.527, P = 0.007–0.004) and DIA %PC (Spearman rho: −0.558 to −0.644, P ≤ 0.001). Conclusions The DIA has an excellent inter-observer agreement and showed a strong correlation with the widely used semi-quantitative IRS. The DIA protocol is an open-source, highly reproducible tool and provides a reliable quantitative evaluation of SST2 immunohistochemistry.

The Mar Piccolo is a transitional water system located in Taranto city (Southern Italy); it is a semi-enclosed basin affected by severe pollution issues due to the presence of various industrial, agricultural and other anthropic activities that require careful monitoring and management. The pollution levels reached over time have harmed marine biodiversity and human health, repeatedly requiring timely actions for its mitigation. Characterization methodologies and techniques today play a fundamental role in supporting the decision-making phase, processing large quantities of data and identifying complex patterns and correlations. An approach focused on gaining detailed knowledge of complex environmental contexts through clustering map techniques enables highly precise results, capturing even the smallest variations in the features of the study object and strongly correlating them with possible sources of pollution. The use of these techniques improves the precision of the analyses and can significantly contribute to improving the understanding of the environmental state in the Mar Piccolo area. This study addresses the issue of pollution in Mar Piccolo due to marine litter, which has led to the formation of synanthropic habitats on the seabed. It also highlights the value of clustering maps and other characterization techniques for achieving detailed insights at various levels of analysis. Data processing through the proposed methodology can generate very detailed mapping useful for planning precision reclamation interventions that also include species conservation actions, as well as a better understanding of how synanthropic habitats are distributed and evolve. In summary, this study demonstrates how it is possible to improve the precision of data processing, providing crucial details for the management and conservation of highly threatened marine ecosystems.

7. Trabecular Bone Score as a Reliable Measure of Lumbar Spine Bone Microarchitecture in Acromegalic Patients

Although GH and IGF-1 excess has a controversial impact on bone mineral density (BMD), acromegalic patients display variable degrees of bone structure impairment. In this study, we aim to investigate the usefulness of trabecular bone score (TBS), compared to BMD, in identifying acromegalic patients with impaired lumbar spine trabecular microarchitecture. Forty-four acromegalic patients were investigated for disease control, metabolic and gonadal status, bone metabolism parameters, and the presence of vertebral fractures (VFs). Patients and matched healthy controls underwent BMD and TBS examination. Mean TBS values were lower in patients than in controls (p < 0.001), without significant differences in mean lumbar and femoral BMD. TBS values were significantly higher in controlled patients compared to the uncontrolled ones (p = 0.012). No significant differences were found in bone markers with respect to disease control. Mean TBS or lumbar BMD did not significantly differ in patients with or without VFs (prevalence 11.4%). TBS and BMD levels were lower in hypogonadal patients compared to the eugonadal ones (p = 0.030 and p < 0.001, respectively). In conclusion, TBS values are significantly lower in patients than in controls, confirming the presence of impaired lumbar spine trabecular bone in acromegaly. Both uncontrolled disease and hypogonadism contribute to TBS deterioration in acromegaly.

Acromegaly is a disease characterized by elevated growth hormone (GH) and insulin-like growth factor 1 (IGF-1) levels. Surgery is the only curative treatment, while medical therapies are administered life-long. To date, almost 30% of patients treated with the currently available medical therapies do not achieve biochemical control.This review focuses on new drugs in development for acromegaly. In detail, we provide an overview of the new molecules designed to improve disease control rate (such as novel somotostatin receptor ligands and antisense oligonucleotides), as well as the new formulations of existing medications aiming to improve patients' compliance (e.g. oral or long-acting subcutaneous octreotide).The constant progresses in the medical treatment of acromegaly could lead to an individualized therapy based on tumor, as well as patient's characteristics. Besides disease control, patient's need represents a major target of medical treatment in chronic diseases such as acromegaly, in order to improve compliance to therapy and patients' quality of life.

Soil seed banks serve as reservoirs of taxonomic and genetic diversity and can buffer the effect of perturbations on plant communities, and thus are critical for community resilience. We studied the relationship of seed bank abundance and richness with different hydrological attributes, evaluated beta diversity patterns among wetlands in the Middle Parana River floodplain, and experimentally explored soil seed bank germination after a drought period. Seed abundance was positively related to drought intensity and, along with richness, negatively related to drought and flood frequency. Turnover and balanced variation in abundance greatly contributed to the total beta diversity probably associated with the environmental heterogeneity of the floodplain; nestedness had a lower contribution probably associated with different dispersal abilities of species. Germination behavior varied between rooted and free-floating plants. In conclusion, the soil seed banks studied are highly variable among habitats; drought and flood frequency, and drought intensity are related with the seed banks abundance and richness; and the germination responses after a drought phase differ among broad ecological groups of wetland plants.

Acromegaly is a rare and severe disease caused by an increased and autonomous secretion of growth hormone (GH), thus resulting in high circulating levels of insulin-like growth factor 1 (IGF-1). Comorbidities and mortality rate are closely related to the disease duration. However, in most cases achieving biochemical control means reducing or even normalizing mortality and restoring normal life expectancy. Current treatment for acromegaly includes neurosurgery, radiotherapy and medical therapy. Transsphenoidal surgery often represents the recommended first-line treatment. First-generation somatostatin receptor ligands (SRLs) are the drug of choice in patients with persistent disease after surgery and are suggested as first-line treatment for those ineligible for surgery. However, only about half of patients treated with octreotide (or lanreotide) achieve biochemical control. Other available drugs approved for clinical use are the second-generation SRL pasireotide, the dopamine agonist cabergoline, and the GH-receptor antagonist pegvisomant. In the present paper, we revised the current literature about the management of acromegaly, aiming to highlight the most relevant and recent therapeutic strategies proposed for patients resistant to first-line medical therapy. Furthermore, we discussed the potential molecular mechanisms involved in the variable response to first-generation SRLs. Due to the availability of different medical therapies, the choice for the most appropriate drug can be currently based also on the peculiar clinical characteristics of each patient.

Cannabis sativa L. belongs to the Cannabaceae family and includes annual, robust, fast-growing and generally dioecious plants. Industrial hemp, and particularly the inflorescences, has been recently recognized as important source of bioactive extracts with antioxidant and antimicrobial effects. The goal of the present study was to explore botanical, phytochemical, and biological properties of water and hydroalcoholic hemp pollen extracts from male inflorescences. The extracts from hemp pollen were found rich in phenolic compounds, such as hydroxytyrosol, coumaric acid, and hesperitin. The phenolic profile was also consistent with the observed scavenging/reducing, enzyme inhibitory, and antimicrobial properties of the extracts. Regarding the antimicrobial effects, Escherichia coli, Trycophyton rubrum, and T. tonsurans were the most sensitive to growth inhibitory effects (MIC values: 9.92-79.37 µg/mL) of the extracts. Whereas, null effects on prostate PC3 and myocyte C2C12 cell viability, in the range 1-1000 µg/mL, are consistent with MIC values and suggest extracts' biocompatibility. The experimental data obtained, which are not reflected in the literature as the topic of hemp pollen is almost completely unexplored, confirm the innovativeness of a product obtained directly from bees, which in the face of greater variability and complexity can reserve promising applications in food, pharmaceutical, and cosmetic sectors.

PURPOSE Acromegaly is a severe chronic endocrine disease. Achieving biochemical control often needs a multimodal treatment approach, including prolonged medical treatment. Aim of the study is to evaluate the burden of treatment direct costs with respect to the different therapeutic strategies, disease control, and follow-up length. METHODS Single center retrospective study on 73 acromegaly patients. Costs of acromegaly treatments were computed based on a detailed revision of patients' clinical charts. RESULTS Median total treatment cost/patient was €47,343 during the entire follow-up (8 years), while median treatment cost/patient/year was €6811. The majority of patients received medical therapy (71/73, 97.3%). First-generation somatostatin receptor ligands (fg-SRLs), such as octreotide (OCT), represent the first-line medical therapy in acromegaly. Fg-SRLs show a preferential binding affinity for somatostatin receptor subtype-2 (SST2), while the second-generation ligand, pasireotide (PAS), has high affinity for multiple SSTs (SST5 >, SST2 >, SST3 >, SST1). Whether PAS acts via SST2 in somatotroph tumors, or through other SSTs (e.g., SST5), is a matter of debate. In this light, the combined treatment OCT+PAS could result in additive/synergistic effects. We evaluated the efficacy of OCT and PAS (alone and in combination) on growth hormone (GH) secretion in primary cultures from human somatotroph tumors, as well as on cell proliferation, intracellular signaling and receptor trafficking in the rat GH4C1 cell line. The results confirmed the superimposable efficacy of OCT and PAS in reducing GH secretion (primary cultures), cell proliferation, cAMP accumulation and intracellular [Ca2+] increase (GH4C1 cells), without any additive effect observed for OCT+PAS. In GH4C1 cells, co-incubation with a SST2-selective antagonist reversed the inhibitory effect of OCT and PAS on cell proliferation and cAMP accumulation, while both compounds resulted in a robust internalization of SST2 (but not SST5). In conclusion, OCT and PAS seem to act mainly through SST2 in somatotroph tumor cells in vitro, without inducing any additive/synergistic effect when tested in combination.

Context Discordant growth hormone (GH) and insulin-like growth factor-1 (IGF-1) values are frequent in acromegaly. Objective To evaluate the impact of different GH cutoffs on discordance rate. To investigate whether the mean of consecutive GH measurements impacts discordance rate when matched to the last available IGF-1 value. Design Retrospective study. Setting Referral center for pituitary diseases. Patients Ninety acromegaly patients with at least 3 consecutive evaluations for GH and IGF-1 using the same assay in the same laboratory (median follow-up 13 years). Interventions Multimodal treatment of acromegaly. Main Outcome Measures Single fasting GH (GHf) and IGF-1 (IGF-1f). Mean of 3 GH measurements (GHm), collected during consecutive routine patients’ evaluations. Results At last evaluation GHf values were 1.99 ± 2.79 µg/L and age-adjusted IGF-1f was 0.86 ± 0.44 × upper limit of normality (mean ± SD). The discordance rate using GHf was 52.2% (cutoff 1 µg/L) and 35.6% (cutoff 2.5 µg/L) (P = 0.025). “High GH” discordance was more common for GHf Discussion GH/IGF-1 discordance strongly depends on GH cutoffs. The use of GHm lessen the impact of GH cutoffs. Measurement of fasting GH levels (both GHf and GHm) is a poor predictor of IGF-1f normalization in our cohort.

The mixture of water extracts from Tanacetum parthenium and Salix alba was studied in an ex vivo assessment of neurotoxicity constituted by isolated mouse cortex specimens challenged with K+ 60 mM Krebs–Ringer buffer (neurotoxicity stimulus). The effects of the mixture on lactate dehydrogenase (LDH), nitrite and serotonin levels were investigated. The phytochemical profile of the mixture was also evaluated. A docking approach was conducted to predict, albeit partially, the putative mechanism underlying the observed effects. The extracts displayed a good profile of polyphenolic compounds (22 chromatographic peaks detected), with caftaric acid and epicatechin being the prominent phenols. In isolated cortex, the association of T. parthenium and S. alba extracts was effective in reducing the K+ 60 mM-induced levels of LDH and nitrites, whereas the neurotoxicity stimulus-induced serotonin depletion was prevented by the treatment. Regarding the inhibition of serotonin catabolism, epicatechin (44.65 µg/mg) and caftaric acid (10.51 µg/mg) were putatively the main compounds involved in the inhibition of monoamineoxidase-A, which is known to play a master role in serotonin turnover. Collectively, the results of the present study point to the efficacy of the present extract mixture as an innovative pharmacological tool to prevent the onset of migraine.

Acromegaly is a rare and severe disease caused by an increased and autonomous secretion of growth hormone (GH), thus resulting in high circulating levels of insulin-like growth factor 1 (IGF-1). Comorbidities and mortality rate are closely related to the disease duration. However, in most cases achieving biochemical control means reducing or even normalizing mortality and restoring normal life expectancy. Current treatment for acromegaly includes neurosurgery, radiotherapy and medical therapy. Transsphenoidal surgery often represents the recommended first-line treatment. First-generation somatostatin receptor ligands (SRLs) are the drug of choice in patients with persistent disease after surgery and are suggested as first-line treatment for those ineligible for surgery. However, only about half of patients treated with octreotide (or lanreotide) achieve biochemical control. Other available drugs approved for clinical use are the second-generation SRL pasireotide, the dopamine agonist cabergoline, and the GH-receptor antagonist pegvisomant. In the present paper, we revised the current literature about the management of acromegaly, aiming to highlight the most relevant and recent therapeutic strategies proposed for patients resistant to first-line medical therapy. Furthermore, we discussed the potential molecular mechanisms involved in the variable response to first-generation SRLs. Due to the availability of different medical therapies, the choice for the most appropriate drug can be currently based also on the peculiar clinical characteristics of each patient.

Sobre todo, permite profundizar en sus filiaciones visuales; en este caso puntual, su interés por óleos tales como Jeremías lamentando la destrucción de Jerusalén (1630) de Rembrandt van Rijn; una de las cuatro versiones de El muchacho del chaleco rojo (c.1889-90) de Paul Cézanne, y las naturalezas muertas del italiano Giorgio Morandi, a partir de los cuales probablemente Couve construyó el cuadro hasta ahora no documentado que aquí se somete a escrutinio.

Pasireotide is a second-generation somatostatin (SRIF) receptor ligand (SRL), approved for medical treatment of acromegaly and Cushing’s disease (CD). The molecule is a stable cyclohexapeptide synthetized based on SRIF structure. Differently from first-generation SRLs (e.g. octreotide), preferentially binding somatostatin receptor (SST) subtype 2 (SST2), pasireotide has high affinity for multiple SSTs (SST5 > SST2 > SST3 > SST1). Interestingly, early preclinical studies demonstrated that pasireotide shows distinct functional properties compared to SRIF and first-generation SRLs when binding SSTs. We aimed to highlight the differential receptor-targeted action of pasireotide in the treatment of somatotroph and corticotroph adenomas, throughout the critical revision of preclinical studies carried out on acromegaly and CD models. Different authors demonstrated that the antisecretory effect of pasireotide in somatotroph adenoma cell cultures is comparable to that of the SST2-preferential agonist octreotide. Some reports even show a direct correlation between SST2 mRNA expression and GH reduction after pasireotide treatment, thus laying for a predominant role of SST2 in driving pasireotide efficacy in somatotropinomas in vitro. On the other hand, the inhibitory effect of pasireotide on ACTH secretion in corticotropinoma cells seems to be mainly mediated by SST5. Indeed, most reports show a higher potency and efficacy of pasireotide compared to SST2 preferential agonists, while functional studies confirm the pivotal role of SST5 targeting in corticotroph cells. The analysis of preclinical studies carried out in somatotroph and corticoph adenomas points out that pasireotide shows a cell-specific activity, exerting its biological effects via different SSTs in the different adenoma histotypes.

A low-sodium diet is an essential part of the treatment of hypertension. However, some concerns have been raised with regard to the possible reduction of iodine intake during salt restriction. We obtained 24-h urine collections for the evaluation of iodine (UIE) and sodium excretion (UNaV) from 136 hypertensive patients, before and after 9 ±, 1 weeks of a simple low-sodium diet. Body mass index (BMI), blood pressure (BP), and drug consumption (DDD) were recorded. Data are average ±, SEM. Age was 63.6 ±, 1.09 year. BMI was 25.86 ±, 0.40 kg/m2 before the diet and 25.38 ±, 0.37 kg/m2 after the diet (p <, 0.05). UNaV decreased from 150.3 ±, 4.01 mEq/24-h to 122.8 ±, 3.92 mEq/24-h (p <, 0.001), UIE decreased from 186.1 ±, 7.95 µ, g/24-h to 175.0 ±, 7.74 µ, g/24-h (p = NS), both systolic and diastolic BP values decreased (by 6.15 ±, 1.32 mmHg and by 3.75 ±, 0.84 mmHg, respectively, p <, DDD decreased (&Delta, DDD 0.29 ±, 0.06, p <, 0.05). UNaV and UIE were related both before (r = 0.246, p = 0.0040) and after the diet (r = 0.238, p = 0.0050). UNaV and UIE were significantly associated both before and after the diet (p <, 0.0001 for both). After salt restriction UIE showed a non-significant decrease remaining in an adequate range. Our dietary suggestions were aimed at avoiding preserved foods, whereas the cautious use of table salt was permitted, an approach which seems safe in terms of iodine intake.

In recent years, anti-Mullerian hormone (AMH) has been considered a reliable index of ovarian reserve. There are few data on AMH values in thyroid cancer. The aim of this study was to evaluate AMH levels in pre-menopausal women with a history of low-risk thyroid cancer. Thirty-four women (aged 40.7 ± 6.7 years) were studied 7.1 ± 0.9 years after surgery and at least one RAI treatment. A group of 23 thyroid cancer women (41.6 ± 7.4 years) who had undergone only thyroidectomy served as controls. AMH, follicle-stimulating hormone (FSH) and estradiol were assayed on days 2–3, and prolactin and progesterone levels on days 21–24 of the menstrual cycle. Pregnancy (RAI group 62%; control group 61%) and miscarriage rates (18% and 26%) were similar. AMH levels were similar in the RAI (10.7 ± 1.7 pmol/l) and control (17.5 ± 4.7 pmol/l) groups. Regular menses were reported in 41% and 52% of RAI and control subjects, respectively. Non-ovulatory cycles were noted in 26% and 35% of RAI and control women, respectively. AMH levels were found to be negatively correlated with age (RAI group P = 0.0003; control group P = 0.0001) and FSH, and positively correlated with progesterone, but not with the other hormonal parameters. AMH should replace FSH in the evaluation of gonadal reserve in pre-menopausal thyroid cancer women. At present, age is the only predictor of AMH levels. About one out of two women with a history of thyroid cancer suffers from menstrual dysregulation, but infertility must be considered a low risk.

Purpose: Epidemiological data are pivotal for the estimation of disease burden in populations. Aim: Of the study was to estimate the incidence and prevalence of acromegaly in Italy along with the impact of comorbidities and hospitalization rates as compared to the general population. Methods: Retrospective epidemiological study (from 2000 to 2014) and case control-study. Data were extracted from the Health Search Database (HSD). HSD contains patient records from about 1000 general practitioners (GPs) throughout Italy, covering a population of more than 1 million patients. It includes information about patient demographics and medical data including clinical diagnoses and diagnostic tests. Results: At the end of the study period, 74 acromegaly patients (out of 1,066,871 people) were identified, resulting in a prevalence of 6.9 per 100,000 inhabitants [95% CI 5.4–8.5]. Prevalence was higher in females than men (p = 0.004), and showed a statistically significant trend of increase over time (p < 0.0001). Overall, incidence during the study period was 0.31 per 100,000 person-years. Hypertension and type II diabetes mellitus were the comorbidities more frequently associated with acromegaly (31.3 and 14.6%, respectively) and patients were more likely to undergo a high frequency of yearly hospitalization (≥3 accesses/year, p < 0.001) compared to sex-age matched controls. Conclusions: This epidemiological study on acromegaly carried out using a large GP-based database, documented a disease prevalence of about 7 cases per 100,000 inhabitants. As expected, acromegaly was associated with a number of comorbidities (mainly hypertension and type II diabetes mellitus) and a high rate of patients’ hospitalization.

In 2011, the European Council stressed the significance of water quality for sustainable development in Europe and emphasized the need for better integration of the water policy objectives into the Common Agriculture Policy reform in rural areas. Since 2000, the Water Framework Directive (WFD) has required the EU Member States to target good ecological status for their water bodies. However, the implementation of the Directive for the numerous Mediterranean temporary streams has been delayed indicating the need for elaboration of the tools and methods that address the special characteristics of such water bodies. This requirement has been addressed by the recently completed MIRAGE project. In the context of the recent publication of the European Commission's Blueprint to Safeguard Europe's Waters, the MIRAGE-proposed framework for the characterization of the eco-hydrological dynamics and the systematic description of the measured impact for temporary rivers could bring considerable added value to the EU revision of all relevant water policies. The project recommends additions to WFD articles including an explicit definition of temporary rivers, adaptation of environmental objectives to their peculiarities and establishment of a proper method to determine the initial status and specific actions in River Basin Management Plans.

Toxicants on fish may induce genetic alterations that can be used as genotoxic markers. All animals face the possibility of limitations in food resources that could ultimately lead to mortality caused by starvation. The primary goal of this study was to characterize the various physiological strategies that allow fish to survive starvation. A multiparametric approach, including morphological biomarkers, blood plasma metabolites, oxidative stress and energy reserves, was used to assess starvation effects on the fish Hoplosternum littorale. Adult specimens were maintained at four experimental groups: control (fed ad libitum), and starved (not fed) fish for 7 and 28 days. Significant changes were observed not only after 28 days, but also after 7 days of starvation. In the shorter period, the hepatosomatic index as well as plasma triglycerides and glucose were significantly lower in starved fish than in the control ones. These results were accompanied by reduced lipid, glycogen and protein reserves in liver and diminished glycogen content in muscle, suggesting the need of these macromolecules as fuel sources. In addition, increased antioxidant enzyme activities were observed in gills, without evidence of oxidative stress in any of the evaluated tissues. Most significant differences were found in 28-days starved fish: total body weight together with the hepatosomatic index was lower when compared to control fish. The plasmatic metabolites tested (glucose, triglyceride, cholesterol and protein), all energy reserves in liver and glycogen content in muscle decreased in 28-days starved fish. Lipid oxidative damage was reported in liver, kidney and brain, and antioxidant enzymes (GST, GR, GPx and CAT) were activated in gills. According to the multivariate analysis, oxidative stress markers and metabolic parameters were key biomarkers that contributed in separating starved from fed fish. 29. Reactive-transport modelling of gypsum dissolution in a coastal karst aquifer in Puglia, southern Italy

The gypsum coastal aquifer of Lesina Marina (Puglia, southern Italy) has been affected by sinkhole formation in recent decades. Previous studies based on geomorphologic and hydrogeological data ascribed the onset of collapse phenomena to the erosion of material that fills palaeo-cavities (suffosion sinkholes). The change in the hydrodynamic conditions of groundwater induced by the excavation of a canal within the evaporite formation nearly 100 years ago was identified as the major factor in triggering the erosion, while the contribution of gypsum dissolution was considered negligible. A combined reactive-transport/density-dependent flow model was applied to the gypsum aquifer to evaluate whether gypsum dissolution rate is a dominant or insignificant factor in recent sinkhole formation under current hydrodynamic conditions. The conceptual model was first defined with a set of assumptions based on field and laboratory data along a two-dimensional transect of the aquifer, and then a density-dependent, tide-influenced flow model was set up and solved using the numerical code SEAWAT. Finally, the resulting transient flow field was used by the reactive multicomponent transport model PHT3D to estimate the gypsum dissolution rate. The validation tests show that the model accurately represents the real system, and the multi-disciplinary approach provides consistent information about the causes and evolution time of dissolution processes. The modelled porosity development rate is too low to represent a significant contribution to the recent sinkhole formation in the Lesina Marina area, although it justifies cavity formation and cavity position over geological time.

The climate in recent decades has aroused interest in the scientific community, prompting us to analyse the mechanisms that regulate it, to understand the climate change responsible for an increase in extreme phenomena. Consequently, the increase in hydrogeological instability in the Italian territory has led to an in-depth study of atmospheric parameters to understand the variations of the atmospheric system. One tool capable of detecting such variations is the weather radar. The weather radar data available in the area provided by the National Radar Network of the Department of Civil Protection allow the evaluation of variations on a national scale for hydro-meteorological-climatic monitoring as well as the disasters that have occurred. Using open-source programming software, the servers can be queried and data retrieved from a source to perform processing for specific purposes through data extraction techniques.

The combined effects of two synthetic insecticides on a wide array of biomarkers in the freshwaterfish Piaractus mesopotamicus were studied. Fish were exposed to sublethal concentrations of endosul-fan (ED), lambda-cyhalothrin (LC), and the combination of both pesticides for 96 h. The set of analyzedbiomarkers included morphometric and hematological parameters, transaminases and alkaline phos-phatase activities, antioxidant enzymes activities and oxidative damage biomarkers measured in gills,liver, kidney, brain and muscle. According to the principal component analysis, the most significant effectswere produced by the ED?LC combination. So, the mixture of both insecticides produced an increase inthe liver-somatic index, hematological changes related to immunological biomarkers (increased whiteblood cells count, and alterations in the differential leukocytes count), decreased liver transaminasesactivity, antioxidant enzymes induction in almost every tissue, and lipid peroxidation levels increases inliver, kidney and brain of exposed fish. Our results suggest deleterious effects of ED and LC insecticidesin combination, and support the usefulness of the multi-biomarker approach for the characterization oftoxicological mechanisms induced by pesticides. El cambio de hábitos en el tipo de colación de los escolares está relacionado, principalmente, con las promociones de los kioscos escolares. Los objetivos del trabajo fueron conocer los productos preferidos comprados en los kioscos escolares para la colación y las razones de su elección (en dos escuelas primarias públicas de la ciudad de Santa Fe) y determinar si las restricciones y exigencias de ventas influyen en la selección de una colación saludable. Los datos fueron recolectados mediante dos encuestas: áulica y en boca de kiosco. La mitad de los niños compra la colación en el kiosco escolar, un 35 % aproximadamente trae colación del hogar, siendo ésta, mayoritariamente, recomendable. Existen diferencias significativas entre las proporciones que eligen un alimento ?recomendable? en el kiosco escolar en ambas escuelas. El principal motivo de selección es el gusto. Es importante regular la venta de productos en los kioscos escolares y educar a los niños en la elección de los alimentos para que realicen una colación saludable. Thirty stomach–content samples (n = 30) were analyzed belonging to the Yellow– billed Tern from the floodplain of the Middle Paraná River, Argentina. The trophic spectrum was composed of 12 taxa (all animal fractions). IRI values were fish= 18 000, crustaceans = 550 and insects = 300. The Hk was 1,52. Regarding the circadian rhythm of feeding activity, an increased activity was observed during midday hours. Food efficiency was 98 % in spring, 99 % in summer, 96,5 % in autumn, and 95,9 % in winter. Prey size ranged from 20–60 mm, being more frequent in the range of 20–40 mm (corresponding to H. pequira). The habitat use index yielded a value of 0.5 for the beach or shore unit. S. supercilliaris presents a carnivorous diet, basically ichthyophagous, whose optimal foraging is linked to the abundant supply of aquatic resources. Fil: Olguin, Pamela Fernanda. Prochilodus lineatus, a fish, was exposed to sublethal concentrations of cypermethrin: 0.075, 0.150, and 0.300 μg L−1 and a control group (without cypermethrin) for 96 h. Five specimens were exposed in each concentration for triplicate (n = 60). Hepatic biochemical values and behavioral changes were studied. The results revealed a significantly higher level of aspartate aminotransferase, alanine aminotransferase, and alkaline phosphatase in hepatic tissue at different concentrations of cypermethrin tested compared to control (p < 0.05). A decrease in total protein was observed in exposed groups but not significantly (p > 0.05). Dithiocarbamates (DTCs) are a group of chemicals used primarily as fungicides, although they are exploited for various other applications. DTCs represent one of the oldest classes of broad-spectrum fungicides employed worldwide to control fungal diseases on many crops. Due to their ease of synthesis, low production costs (cheap and readily available starting materials) and a fungicidal activity with a multi-site mode of action, they are still among modern agriculture’s most extensively used pesticides. Although the environmental degradation in air, water, and soil is relatively rapid due to photolysis and/or hydrolysis, they are among the most frequently detected pesticides in the European Union (EU), also with a high frequency of maximum residue level (MRL) exceedances. The current review aims to comprehensively survey all aspects of DTCs, including the environmental fate, toxicity and analytical methods for determining parental compounds and degradation products in environmental and food samples. Furthermore, the accumulation of carbamate and dithiocarbamate pesticides in vegetables, fruits, bioindicator organisms and human biological samples, as well as their health effects on humans, are also considered in this study.

Karst aquifers are essential drinking water sources, representing about 25% of the total available sources globally. Groundwater ecosystems consist of fissured carbonate rocks commonly covered with canopy collapse sinkholes. The open nature of karst aquifers makes them susceptible to rapidly transporting contaminants from the surface in dissolved and particulate forms. The principal aim of this review is to contribute to filling the gap in knowledge regarding major concerns affecting karst aquifers and understanding their vulnerabilities and dynamics. The principal groundwater pollutants of relevance are detailed in the present work, including well-known issues, such as the input of agriculture and its role in water quality. Emerging pollutants such as microplastics, still poorly studied in the groundwater systems, were also considered. Case studies for each typology of pollutant were highlighted, as their relative concerns for karst environments. Final considerations underlined an approach for studying karst environments more focused on understanding dynamics and links among different pollutants inputs and their drivers than on individual sources and impacts.

38. Characterization of the Belowground Microbial Community in a Poplar-Phytoremediation Strategy of a Multi-Contaminated Soil

Due to their widespread use in industrial applications in recent decades, Polychlorobiphenyls (PCBs) and heavy metals (HMs) are the most common soil contaminants worldwide, posing a risk for both ecosystems and human health. In this study, a poplar-assisted bioremediation strategy has been applied for more than 4 years to a historically contaminated area (PCBs and HMs) in Southern Italy using the Monviso poplar clone. This clone was effective in promoting a decrease in all contaminants and an increase in soil quality in terms of organic carbon and microbial abundance. Moreover, a significant shift in the structure and predicted function of the belowground microbial community was also observed when analyzing both DNA and cDNA sequencing data. In fact, an increase in bacterial genera belonging to Proteobacteria able to degrade PCBs and resist HMs was observed. Moreover, the functional profiling of the microbial community predicted by PICRUSt2 made it possible to identify several genes associated with PCB transformation (e.g., bphAa, bphAb, bphB, bphC), response to HM oxidative stress (e.g., catalase, superoxide reductase, peroxidase) and HM uptake and expulsion (e.g., ABC transporters). This work demonstrated the effectiveness of the poplar clone Monviso in stimulating the natural belowground microbial community to remove contaminants and improve the overall soil quality. It is a practical example of a nature based solution involving synergic interactions between plants and the belowground microbial community.

This research activity, conducted in collaboration with the Aero-Naval Operations Department of the Guardia di Finanza of Bari as part of the Special Commissioner for urgent measures of reclamation, environmental improvements and redevelopment of Taranto’s measurement, is based on the use of a high-resolution airborne sensor, mounted on board a helicopter to identify and map all in operation and abandoned mussel farming in the first and second inlet of Mar Piccolo. In addition, factors able to compromise the environmental status of the Mar Piccolo ecosystem were also evaluated. The methodological workflow developed lets extract significant individual frames from the captured video tracks, improves images by applying five image processing algorithms, georeferences the individual frames based on flight data, and implements the processed data in a thematic Geographical Information System. All mussel farms, in operation and derelict, all partially submerged and/or water-coated invisible to navigation poles and other elements such as illegal fishing nets and marine litter on the seabed up to about 2 m deep, have been identified and mapped. The creation of an instant, high-precision cartographic representation made it possible to identify the anthropogenic pressures on the Mar Piccolo of Taranto and the necessary actions for better management of the area.

A Monviso clone has been applied to promote PCB degradation in a soil historically contaminated by polychlorinated biphenyls (PCBs) and heavy metals (HMs). The multi-contaminated area is located in Southern Italy. PCBs, HMs, and the soil microbial community (abundance, viability, and structure) were analysed in selected plots of the poplar-treated area. At 900 days after poplar planting, chemical analyses showed that PCBs and most of HMs diminished under the Italian legal limits. The overall results suggest that the poplar clone was effective in promoting PCB rhizodegradation and HM phytostabilization. Organic carbon content increased strongly in the rhizosphere of the planted plots. Microbiological results highlighted an overall increase in microbial abundance, cell viability, and the presence of bacterial groups involved in PCB degradation. The poplar-based bioremediation technology is a nature-based solution able to promote the recovery of soil quality in terms of contaminant removal, increase in organic carbon, and stimulation of autochthonous bacterial groups able to transform PCBs.

41. A Relevant Screening of Organic Contaminants Present on Freshwater and Pre-Production Microplastics

Microplastics (MPs) have recently been discovered as considerable pollutants of all environmental matrices. They can contain a blend of chemicals, some of them added during the manufacture of plastic to improve their quality (additives) and others adsorbed from the surrounding environment. In light of this, a detailed study about the identification and quantification of target organic pollutants and qualitative screening of non-target compounds present on MPs was carried out in different types of samples: environmental MPs, collected from an Italian river, and pre-production MPs, taken from the plastic industry. Polychlorobiphenyls (PCBs), organochlorine pesticides (OCPs), and polycyclic aromatic hydrocarbons (PAHs) were chosen as target compounds to be quantified by Gas Chromatography-Mass Spectrometry (GC–MS), while the non-target screening was carried out by High Resolution Gas Chromatography-Mass Spectrometry (HRGC–MS). The target analysis revealed concentrations of 16 priority Polycyclic Aromatic Hydrocarbons by Environmental Protection Agency (EPA-PAHs) in the range of 29.9–269.1 ng/g; the quantification of 31 PCBs showed values from 0.54 to 15.3 ng/g, identifying CB-138, 153, 180, 52, and 101 primarily; and the detected OCPs (p,p’-DDT and its metabolites) ranged between 14.5 and 63.7 ng/g. The non-target screening tentatively identified 246 compounds (e.g., phthalates, antioxidants, UV-stabilizers), including endocrine disruptors, toxic and reprotoxic substances, as well as chemicals subjected to risk assessment and authorisation. The large assortment of plastic chemicals associated with MPs showed their role as a presumable source of pollutants, some of which might have high bioaccumulation potential, persistence, and toxicity.
