Introduction The importance of our study lies in the fact that we have demonstrated the occurrence of mechanical dysfunction within polypoid tissues, which promotes the development of polyps in the nasal cavity. Objective To change the paradigm of nasal polyposis (NP). In this new conception, the chronic nasal inflammatory process that occurs in response to allergies, to pollution, to changes in the epithelial barrier, or to other factors is merely the trigger of the development of the disease in individuals with a genetic predisposition to an abnormal tissue remodeling process, which leads to a derangement of the mechanical properties of the nasal mucosa and, consequently, allows it to grow unchecked. Data Synthesis We propose a fundamentally new approach to intervening in the pathological process of NP, addressing biomechanical properties, fluid dynamics, and the concept of surface tension. Conclusion The incorporation of biomechanical knowledge into our understanding of NP provides a new perspective to help elucidate the physiology and the pathology of nasal polyps, and new avenues for the treatment and cure of NP.

OBJETIVO: O pós-operatório da tonsilectomia é, em geral, bastante doloroso e os pacientes necessitam de analgésicos. Este estudo visou avaliar a eficácia da aplicação do laser terapêutico no controle da dor no pós-operatório de tonsilectomia. MÉTODOS: 18 crianças de cinco a 15 anos de idade foram submetidas à adenotonsilectomia, no período de junho de 2005 a outubro de 2006, sendo randomizadas para receber aplicações de laser terapêutico na área cirúrgica imediatamente após o procedimento e 24 horas após a cirurgia (n=9) ou seguir a rotina, com analgesia farmacológica, se necessário. A avaliação da dor foi realizada por escala analógica de dor, pela necessidade de analgésicos e pela aceitação da dieta no pós-operatório. RESULTADOS: Os pacientes submetidos à aplicação do laser apresentaram medianas das notas da escala de avaliação da dor menores e utilizaram menos analgésicos no pós-operatório em comparação aos pacientes controles. A aceitação da dieta nos dois grupos não foi diferente. CONCLUSÕES: Os resultados preliminares mostraram que o laser terapêutico foi eficaz na diminuição da dor e na redução de uso de analgésicos no pós-operatório de tonsilectomias em crianças e adolescentes.OBJECTIVE: The postoperative period of a tonsillectomy is usually very painful, requiring the use of pain-relieving drugs. The aim of this study was to evaluate the efficacy of low-level laser therapy in post-tonsillectomy pain control. METHODS: 18 children aged 5 to 15 years undergoing adenotonsillectomy between June 2005 and October 2006 were randomized to receive either local application of therapeutic laser immediately after surgery and 24 hours postoperatively (n=9) or routine analgesic drug therapy, if necessary. Pain was assessed by visual analog scale scores, need for analgesics, and acceptance of diet during the postoperative period. RESULTS: Patients undergoing laser applications had lower median pain scores and required less analgesic medication postoperatively than the control group. Acceptance of diet was similar in both groups. CONCLUSIONS: Preliminary results showed that low-level laser therapy is effective in the reduction of post-tonsillectomy pain, minimizing the need of analgesic medication in children and adolescents.

This paper is a review of the most recent literature concerning the role of inflammatory genes in nasal polyposis. The data provide evidence of a pivotal role for some inflammatory mediators in the development and maintenance of nasal polyps.Nasal polyposis is a common disease of the nasal mucosa; the exact mechanisms leading its development are still unknown. The role of the immune system in the pathogenesis of this disease is poorly understood. This lack of understanding makes definitive treatment very difficult. Activated eosinophils are a prominent feature of nasal polyps. Their presence in tissue results from a complex series of events that regulates their influx from the vasculature, as well as their movement, activation and survival within the tissue. Several studies have demonstrated that there are many potent chemoattractants that can activate eosinophils and trigger the inflammatory response.Cytokines, chemokines and growth factors play an important role in the persistence of mucosal inflammation associated with nasal polyps. Metalloproteinases seem to be crucial in nasal tissue remodeling in these patients. Arachidonic acid metabolites seem to be particularly important in the pathogenesis of nasal polyps in patients with aspirin hypersensitivity. We discuss the contribution of each one for the polyp formation.

Background The objective of this study was to investigate the spectrum of inflammatory gene expression in patients with nasal polyposis. Methods The cDNA microarray technique was used to identify gene expression in tissue samples from nasal polyps and adjacent inflammatory nasal mucosa of 21 patients with nonallergic nasal polyposis. To validate the microarray analysis, we compared the expression of five genes by reverse transcription-polymerase chain reaction (RT-PCR): tumor necrosis factor, IL-5, IL-9, fibroblast growth factor, and transforming growth factor (TGF)-β1. Results We tested 96 different inflammatory genes in our samples. Thirty-six genes exhibited differences in expression between the two tissue types. In all 36 genes the level of expression was greater in the inflammatory mucosa than the polyps. The RT-PCR confirmed the cDNA results. Conclusion We believe that the high expression of TGF-β1 in inflammatory mucosa compared with the low expression in polyps may reflect an important role in the inhibitory mechanisms of nasal polyposis.

Trinta por cento das faringotonsilites agudas são de etiologia estreptocócica com potencial de complicações como a glomerulonefrite difusa aguda e febre reumática. Crianças de creches apresentam maior incidência destas infecções. OBJETIVO: Identificar e comparar a prevalência do Streptococcus pyogenes na orofaringe entre crianças que freqüentam creches e crianças não-institucionalizadas, em duas regiões do Brasil. CASUÍSTICA E MÉTODO: Estudo prospectivo com 200 crianças, provenientes da cidade de Porto Velho/RO e São Paulo/SP, em quatro grupos, freqüentadoras ou não de creches. Realizou-se swab de orofaringe e cultura para identificação do Streptococcus pyogenes. RESULTADOS: Prevalência de 8% e 2% entre as crianças de São Paulo que atendem a creches e do grupo controle, respectivamente, apresentando valor estatístico (p=0,02). Prevalência de 24% e 16% nos grupos de Porto Velho/RO que freqüentam creche e controle respectivamente, não caracterizando diferença estatisticamente significante (p=0,18). Observou-se diferença estatisticamente significante entre os grupos creche e controle de São Paulo/SP aos seus correspondentes de Porto Velho/RO (p < 0,01). CONCLUSÃO: Os resultados deste estudo nos permitem sugerir que freqüência em creches representa um fator de risco para a colonização de orofaringe pelo Streptococcus pyogenes, fato observado em populações distintas, porém com significância estatística em apenas uma das duas amostras. Rabdomiosarcoma é uma neoplasia maligna originária de células mesenquimais primitivas, podendo ocorrer em qualquer lugar do corpo. É o sarcoma de partes moles mais comum na infância, e localiza-se mais freqüentemente na cabeça e pescoço. OBJETIVO: Estudar a ocorrência de RMS na cabeça e pescoço na infância, correlacionando aspectos clínicos e histopatológicos. FORMA DE ESTUDO: Clínico retrospectivo. MÉTODO: Oitenta e dois pacientes com diagnóstico de sarcomas de partes moles, atendidos no Instituto de Oncopediatria da UNIFESP-EPM de 1988 a 2002 foram incluídos neste estudo. Foram estudados os seguintes parâmetros: incidência de RMS na infância e na cabeça e pescoço, distribuição segundo sexo, faixa etária, tipo histológico, localização primária, óbito X localização, causa mortis. RESULTADOS: Neste estudo 65% dos casos de sarcomas de partes moles corresponderam à RMS; 33% dos casos de RMS localizavam-se na cabeça e pescoço; 77% dos casos de sarcoma de partes moles de cabeça e pescoço corresponderam ao RMS. A média de idade no diagnóstico foi de 7,62 anos, predominando na faixa etária dos 5 aos 9 anos (41%). Em relação ao sexo, encontramos 47% do sexo feminino e 53% do sexo masculino. Quanto ao tipo histológico, o mais comum foi o RMS embrionário correspondendo a 64,6% do total. O sítio primário mais comum foi o orbital (52,8%). Cem por cento, 50% e 33,3% dos pacientes com RMS parameníngeo, não parameníngeo e orbital, respectivamente, evoluíram para óbito. CONCLUSÃO: O RMS é o sarcoma de partes moles mais comum na infância, localizando-se preferencialmente na cabeça e pescoço. Houve predominância do sexo masculino neste estudo; idade média de 7,62 anos predominando a faixa etária dos 5 aos 9 anos. O tipo histológico predominante foi o embrionário e a localização orbital foi mais freqüente. O maior índice de óbito pertenceu aos RMS parameníngeo e o menor ao orbital.

The bacteriological and clinical efficacy and the safety of gatifloxacin for the treatment of non-complicated acute rhinosinusitis was evaluated in 49 adult patients in an open-label multicenter study in Brazil. Patients under age 18, or with associated systemic diseases, were excluded. Diagnosis was based on symptoms, otorhinolaryngological examination, and X-rays of the sinus. At the first visit, all patients were treated with a single daily dose of 400 mg gatifloxacin for 10 days. Middle nasal meatus secretion was collected and sent for culture before and after treatment. Patients were all reevaluated at days 3 to 5; days + 1 to + 5 and 18 to 25 days + 7 to + 14 . Ninety three percent of the patients were considered clinically cured at the end of the treatment. The most frequent bacteria isolated were Haemophilus influenzae, Streptococcus pneumoniae and Moraxella catarrhalis, and at the end of the treatment, presumed bacteriological eradication was observed in almost all patients. Adverse effects were observed in 19 of the cases, mostly mild and self limiting, including diarrhea, abdominal pain, nausea and vomiting. Treatment had to be interrupted in two cases. Gatifloxacin was found to be efficacious and safe for the treatment of acute rhinosinusitis in adults.

OBJETIVO: Avaliar a eficácia e segurança do tratamento homeopático em crianças com adenóide obstrutiva, com indicação cirúrgica. FORMA DE ESTUDO: Clínico prospectivo. Material e método: Estudo prospectivo, duplo-cego, randomizado, em que foram incluídas 40 crianças com idade variando de 3 a 7 anos, 20 crianças foram tratadas com medicação homeopática individualizada (Simillimum), baseada no princípio da similitude e 20 crianças receberam placebo. Todas as crianças do grupo medicação homeopática foram medicadas diariamente com Agraphis nutans 6 CH, Thuya 6 CH e Adenóide 21CH; e as do grupo placebo receberam diariamente medicamentos sem o princípio ativo. A duração do estudo de cada paciente foi de 4 meses. A avaliação dos resultados foi clínica, por meio de questionário padrão, de exame otorrinolaringológico e nasofaringoscopia direta com fibroscópio flexível, no primeiro e no último dia de tratamento. Utilizou-se como critério de inclusão a adenóide que ocupou mais do que 70% da luz coanal. RESULTADOS: Das 20 crianças tratadas com medicamento homeopático, 13 não apresentaram alteração no tamanho da adenóide nos exames nasofaringoscópicos e 7 tiveram diminuição da adenóide; das 20 crianças que receberam placebo por 4 meses, 11 não apresentaram alterações no tamanho da adenóide, 4 tiveram diminuição da adenóide e 5 crianças tiveram aumento. Não houve diferença estatística significante entre os grupos (P= 0,069). Na avaliação clínica da evolução dos pacientes, dos 20 pacientes tratados com medicamento homeopático, 17 se mantiveram inalterados, com respiração oral e ronco, um paciente melhorou, ficando sem ronco e dois foram curados, isto é, a respiração alterou-se de oral para nasal e sem ronco. Dos 20 pacientes tratados com placebo, 17 pacientes se mantiveram inalterados, um paciente melhorou do ronco e dois foram curados, não tendo havido diferença estatística significante entre os grupos (P>0,999). CONCLUSÕES: O tratamento homeopático não foi eficaz nas crianças com adenóide obstrutiva, mantendo-se a indicação cirúrgica em 85% dos pacientes. O medicamento homeopático não provocou eventos adversos nas crianças. AIM: the efficacy and security of homeopathic treatment was investigated on children with obstructive adenoid justifying an operation. STUDY DESIGN: Clinical prospective. MATERIAL AND METHOD: In a prospective, randomized, double-blind clinical trial included 40 children between the ages of 3 to 7 years old, 20 children were treated with homeopathic medication, based in the principle of similarity (Simillimum), and 20 children with placebo. All the children of the homeopathic group/ adenoid, were treated daily with Agraphis nutans 6 CH, Thuya 6 CH and Adenoid 21CH, and the patients of the placebo group received daily placebo medication. The duration of the study of each children was 4 months. The evaluation of the results was clinical, and it was made by questionnaire standard, clinical examination and direct flexible fiberoptic nasopharyngoscopy, in the first and last day of treatment. The criterion of selection was the adenoid that occuped more than 70% of the coanal space. RESULTS: From the group of 20 children treated with homeopathic treatment, 13 did not show any change on the size of adenoid after nasopharyngoscopy, and 7 children had their adenoid decreased; from another group of 20 children that have treated with placebo for 4 months, 11 did not show any change on the size of their adenoid, 4 had their adenoid decreased and 5 had their adenoid increased. The statistical analysis showed a not significant difference (P= 0,069). The clinical evaluation of the patients showed that from the group of 20 patients treated with homeopathy, 17 kept unchanging, with oral breathing and snoring, one patient got better, eliminating the snoring and two were cured, which mean that their oral breathing turned to nasal breathing without snoring. From the group of 20 patients treated with placebo, 17 kept unchanging, one eliminated the snoring and two were cured; and these differences were not statistically significant (P> 0,999). CONCLUSIONS: the homeopathic treatment was not efficient in the patients with obstructive adenoid, remaining it surgical indication in 85% of the children. The homeopathic remedies did not provoke adverse events in the children. Introdução: O lagoftalmo decorrente da paralisia facial periférica deve ser corrigido a fim de se evitar complicações oculares. O uso de implantes de ouro como uma alternativa à tarsorrafia vem sendo estudado, mostrando bons resultados. Objetivo: O objetivo deste trabalho foi estudar os efeitos da implantação de peso de ouro em pálpebra superior para correção do lagoftalmo. Forma de estudo: clínico retrospectivo não randomizado. Material e método: Foram estudados retrospectivamente durante seis anos 59 pacientes com lagoftalmo secundário à paralisia facial de diversas etiologias e que foram submetidos à colocação de peso de ouro palpebral. Resultados: Em todos os pacientes foi consegui-do o fechamento ocular completo. As complicações do procedimento foram raras e incluíram a extrusão do peso em 8,5% dos pacientes e infecção em 5%. O tempo de permanência do peso variou de 6 meses a 6 anos, tendo sido removido em 20,3% por melhora da paralisia facial periférica. Discussão: Vários métodos têm sido utilizados para promover a proteção ocular nos casos de paralisia facial com graus variáveis de sucesso e aceitação pelo paciente. O implante de peso de ouro em pálpebra superior é um método simples, de fácil reversão e com bom resultado estético. O tempo de permanência do peso palpebral é variável na literatura e as complicações do procedimento são raras. Conclusão: O uso do peso de ouro palpebral em pacientes com paralisia facial traz bons resultados na correção do lagoftalmo e prevenção de complicações oculares. Introduction: Lagophthalmus in facial palsy must be treated in order to avoid ocular complications. The gold weight eyelid implant as an alternative to tarsorrhaphy has been studied with good results. Aim: The purpose of this report is to study the effects of the gold weight eyelid implants for treatment of lagophthalmus. Study design: clinical retrospective not randomized. Material and method: 59 patients with Lagophthalmus associated with facial paralysis of many causes underwent gold weight implantation and were studied retrospectively. Results: All patients achieved satisfactory eyelid closure and improvement of ocular symptoms. 8.5% of patients had implant extrusion and 5% had infection. The average time with the implant varied from 6 months to 6 years, being removed in 20.3% of patients with recover of facial palsy. Discussion: Many methods have been described for enhance corneal protection in patients with facial palsy with different degrees of success and acceptance by the patient. The gold weight eyelid loading is a simple easy technique with good cosmetic result. The average time with the implant is inconstant and the complications are rare. Conclusion: Gold weight eyelid implants in patients with facial palsy have good results for the treatment of lagophthalmus and prevention of ocular complications.
