Patients with hypertension who have high blood pressure readings during 24-hour monitoring have an increased risk of heart attack, according to a study of 1,963 patients. All of the patients were taking medications to treat their high blood pressure. This indicates that even patients under treatment should have 24-hour blood pressure monitoring periodically.

The prognostic relevance of masked uncontrolled hypertension (MUCH) is incompletely clear, and its global impact on cardiovascular outcomes and mortality has not been assessed. The aim of this study was to perform a meta-analysis on the prognostic value of MUCH. We searched for articles assessing outcome in patients with MUCH compared with those with controlled hypertension (CH) and reporting adjusted hazard ratio and 95% CI. We identified 6 studies using ambulatory blood pressure monitoring (12 610 patients with 933 events) and 5 using home blood pressure measurement (17 742 patients with 394 events). The global population included 30 352 patients who experienced 1327 events. Selected studies had cardiovascular outcomes and all-cause mortality as primary outcome, and the main result is a composite of these events. The overall adjusted hazard ratio was 1.80 (95% CI, 1.57-2.06) for MUCH versus CH. Subgroup meta-analysis showed that adjusted hazard ratio was 1.83 (95% CI, 1.52-2.21) in studies using ambulatory blood pressure monitoring and 1.75 (95% CI, 1.38-2.20) in those using home blood pressure measurement. Risk was significantly higher in MUCH than in CH independently of follow-up length and types of studied events. MUCH was at significantly higher risk than CH in all ethnic groups, but the highest hazard ratio was found in studies, including black patients. Risk of cardiovascular events and all-cause mortality is significantly higher in patients with MUCH than in those with CH. MUCH detected by ambulatory or home blood pressure measurement seems to convey similar prognostic information.

- Abstract
Guidelines summarize and evaluate available evidence with the aim of assisting health professionals in selecting the best management strategies for an individual patient with a given condition. Guidelines and their recommendations should facilitate decision making of health professionals in their daily practice. However, the final decisions concerning an individual patient must be made by the responsible health professional(s) in consultation with the patient and caregiver as appropriate. A great number of guidelines have been issued in recent years by the European Society of Cardiology (ESC) and by the European Society of Hypertension (ESH), as well as by other societies and organisations. Because of the impact on clinical practice, quality criteria for the development of guidelines have been established in order to make all decisions transparent to the user. The recommendations for formulating and issuing ESC Guidelines can be found on the ESC website (http://www.escardio.org/Guidelines-&-Education/Clinical- Practice-Guidelines/Guidelines-development/Writing-ESC-Guidelines). ESC Guidelines represent the official position of the ESC on a given topic and are regularly updated.

35. Comparison of athletes with life-threatening ventricular arrhythmias with two groups of healthy athletes and a group of normal control subjects

Sudden cardiac death in well-trained athletes is most often superimposed on the presence of structural heart disease. However, some athletes die suddenly in the absence of overt heart disease. To improve identification of athletes at high risk for ventricular tachycardia (VT), ventricular repolarization, the signal-averaged electro-cardiogram (ECG), and the echocardiogram from 13 male athletes with symptomatic VT and without evidence of manifest cardiac disease were compared with data obtained in 3 matched control groups (15 apparently healthy professional road cyclists, 10 professional basketball players, and 15 normal control subjects without any sports activity). All patients had apparently normal QRS duration on the routine ECG, and none were taking antiarrhythmic drugs. Echocardiography and signal-averaged electrocardiography were useful in distinguishing the group of athletes with tachyarrhythmias from the group of normal nonsporting controls, but not from both groups of normal athletes. The QT interval ([V.sup.4]) and the QT interval corrected with the cubic root were shorter for the nonsporting controls. 36. Influence of arterial blood pressure and aldosterone on left ventricular hypertrophy in moderate essential hypertension

In a group of 36 untreated patients with mild-to-moderate essantial hypertension (office systolic blood pressure [SBP] 160 [plus or minus] 3.4 mm Hg, office diastolic blood pressure [DBP], L02 [plus or minus]1.5 mm Hg), 24-hour ambulatory blood pressure monotoring, and determination of left ventricular (LV) mass index according to the formula of Devereux were performed. After an overnight fast, blood samples were taken far the determination of serum aldosterone levels and plasma renin activity. Urinary catechelamine concentrations were assayed from 24-hour urine collections. Left ventricular mans index (143.7[plus or minus] [8 g/m.sup.2) did not correlate significantly with either office SBP or office DBP. The correlation of LV mass index with mean 24-hour SBP (145[plus or minus] 3 mm Hg) was statistically significant: r = 0.395, p = 0.026. However, the best correlation was obtained with mean 24-hour DBP (50 [plus or minus]3 mm Hg) with r = 0.499 (p= 0.004). Urinary catecholamine levels did not correlate with LV mass index, In addition, LV mass Index correlated significantly with plasma renin activity (r= 0.346, p = 0.050) and serum aldonterone levels (r= 0.559, p = O.0009). There was a strongly significant correlation between LV mass index and serum aidosterone levels even after adjustment for mean 24-hour SBP (r= 0.496, p = 0.005) and DBP (r= 0.5:14, p = 0.004). These results demonstrate that ambulatory blood pressure determinations but not office blood pressure parameters correlate well with left ventricular hypertrophy In essential hypertension. Nonhemo. dynamic factors are important determinants of left ventricular mass as well. Moreover, aldostetone seems to play an important role in cardiac hypertrophy independent of its relation with arterial blood pressure. (Am J Cardiol 1993;71:17A-20A)

37. Delayed restoration of atrial function after conversion of atrial flutter by pacing or electrical cardioversion

It is often suggested but never proven that atrial function is not affected during atrial flutter, nor after its conversion to normal sinus rhythm. To evaluate this hypothesis, a prospective study was performed in 22 patients (age range 20 to 88 years) with atrial flutter. Diastolic transmitral flow was analyzed with echo-Doppler before and after conversion. After randomization, conversion was attempted with overdrive pacing or up to two 50 J shocks. If the initial method was unsuccessful, a 250 J shock was administered. All patients were converted to sinus rhythm with this protocol. Shortly after conversion (at 1 and 6 hours), atrial contribution to ventricolar filling was absent in 4 of 22 patients. In the remaining 18 patients, atiral contribution to ventricolar filling was small. Atrial contribution to transmitral flow improved from 20 to 27% within 24 hours (p 1 week. The findings suggest that stasis in the atria con remain temporarily present after successful conversion of atrial flutter to sinus rhythm. (Am J Cardiol 1993;71:63-67)

The hemodynamic and renal effects of anaritide (human atrial natriuretic peptide 102-126), a synthetic analog of atrial natriuretic peptide, were evaluated in 35 patients with chronic New York Heart Association class II to IV heart failure. There were 32 men and 3 women, aged 33 to 75 (mean standard error of the mean 56 [+ or -] 2) years. in the first phase of the study, right-sided heart catheterization was performed and anaritide was administered as 1-hour infusions. The rate of the infusion varied among patients from 0.03 to 0.3 [mu]g/kg/min. in response to anaritide, there were decreases in mean systemic arterial (94 [+ or -] 2 to 87 [+ or -] 2 mm Hg), right atrial (10 [+ or -] 1 to 8 1 mm Hg), mean pulmonary arterial (33 2 to 28 [+ or -] 2 mm Hg) and pulmonary artery wedge (22 [+ or -] 2 to 15 [+ or -] 2 mm Hg) pressures (all p [is less than]O.OS). Cardiac index increased (2.39 [+ or -] 0.15 to 2.62 [+ or -] 0.15 liters/min/[m.sup.2], p [is less than]O.OS) and heart rate was unchanged. Systemic vascular resistance decreased significantly, but pulmonary vascular resistance was unchanged. There were increases in urine volume (1.6 [+ or -] 0.2 to 2.3 [+ or -] 0.4 mi/ min), sodium excretion (47 [+ or -] 13 to 74 [+ or -] 20 [mu]Eq/ min) and fractional excretion of sodium (0.41 [+ or -] 0.11 to 0.59 [+ or -] 0.14%, all p [is less than] 0.05), while potassium excretion and creatinine clearance did not change. In the second phase of the study, patients received 2-hour infusions of anaritide (0.03 to 0.6 [mu]g/kg/min) and placebo with noninvasive monitoring. Mean systemic arterial pressure and plasma norepinephrine concentration decreased, while urine volume, sodium excretion and fractional excretion of sodium increased (all p [is less than] 0.05) during anaritide infusion, but were unchanged on placebo. Adverse effects, usually related to excessive reduction of systemic arterial pressure, were observed in 6 patients. Short-term infusion of atrial natriuretic peptide produces improvement in hemodynamics and renal function in patients with heart failure. (Am J Cardiel 1990;65:211-215), Congestive heart failure is a condition marked by circulatory congestion caused by some heart disorders. It is associated with the retention of water and salt by the kidneys. Atrial natriuretic peptide (ANP) is a hormone produced by specialized cells in the heart. When ANP is given to healthy patients, it increases the loss of water and acts to dilate blood vessels. Although levels of ANP are generally high in patients with heart failure, patients with congestive heart failure may still have constricted blood vessels and excessive urine retention. Synthetic ANP was administered to 35 patients with congestive heart failure. After one hour of intravenous ANP, blood pressure was reduced, the output of the heart increased and the heart rate remained the same. Urine and salt excretion was increased. Side effects observed in six patients were the result of lowered blood pressure. ANP given over a short period of time improved the force and flow of blood and kidney functioning in patients with congestive heart failure.
