Background: Retroclival hematomas and clival fractures are not common, and retroclival hematoma is one of the types of posterior fossa extra-axial hematomas. The exact incidence is not known. They can be categorized into epidural or subdural hematomas based on their relationship to the tectorial membrane. In the literature, most cases occur in the pediatric population, and a few cases have been reported in the adult population as well. The etiology is related to accidental trauma in most cases. Others occur generally spontaneously due to coagulopathy, pituitary apoplexy, and ruptured aneurysms. Still, some remain idiopathic without an identifiable cause. Case presentation: This is a 28-year-old male patient who presented after sustaining a motor vehicle accident. He was a pedestrian walking by the side of a road when he was suddenly hit by a minibus. He lost consciousness immediately and was non-communicating after the trauma with right ear bleeding. GCS is 9/15, right pupil has cataract, and left pupil is midsized and reactive. A brain CT scan demonstrated retroclival hematoma extending from midclivus to the lower level of C2, right mastoid fracture, multiple post-traumatic subarachnoid hemorrhage, pneumocephalus, and transverse upper third clival fracture. This patient's GCS on the second day improved to 14/15. All cranial nerves are intact. Discharged on the 7th day with GCS 15/15. Conclusion: Retroclival hematomas and clival fractures are very rare in adults, most often reported in pediatric age groups, and mostly occur due to accidental trauma. Associated cranial nerve palsy is common and the management is non-surgical in most of the cases.

An adolescent male presented with orthostatic headaches following head trauma. MRI showed cerebellar tonsil displacement and a bony defect in the clival skull base. Digital subtraction myelography (DSM) confirmed a cerebrospinal fluid‐venous fistula (CVF). This was repaired endoscopically. CVFs cause uncontrolled flow of CSF into the venous system resulting in symptoms of intracranial hypotension. They're often difficult to identify on initial imaging. This is the first reported CVF originating in the central skull base, and the first treated via endoscopic trans‐nasal approach. Isolated bilateral sixth nerve palsies are rare, particularly in the setting of trauma. Most post-head injury cases with bilateral abducens palsy involve either basal skull fractures, particularly clival fractures. We present a case of bilateral abducens palsy after closed head injury in a young male who presented to the emergency department and a comprehensive literature review based on our clinical case. A Medline search for bilateral abducens palsy in closed head injury showed 89 results. Articles were excluded if crush head injury, non-traumatic bilateral abducens nerve palsy, associated vascular malformations were reported. After thorough search and filtering of those articles, fifty-one publications were found which reported and discussed about traumatic bilateral abducens palsy with closed head injury. In these 51 articles, a total of 139 cases were recorded. Several theories have been postulated to explain mechanisms of abducens nerve injury in trauma both in immediate and delayed settings. In our case, patient presented with immediate onset of bilateral abducens palsy. On imaging, clival fracture was seen in CT brain, which can be attributed for the nerve injury. Cases with retroclival extradural haematoma had higher chances of multiple cranial nerve injuries. Cases with multiple basal skull fracture involving petrous temporal bone fracture had higher chances of facial nerve injury. Along with bilateral involvement, the poorer outcome for recovery can be related with the severity of the adduction deficit. Our case showed no improvement in bilateral abduction during follow-up at 6 months. Clinical presentation of traumatic bilateral abducens nerve palsy is rare following closed head trauma and is usually associated with other injuries which are incompatible with life. It can be associated with other nerve injuries depending on basal skull fractures.

Background: Clival fractures are a rare traumatic finding and are often the result of high-impact craniofacial trauma. Rarely, longitudinal clival fractures can be associated with incarceration of the basilar artery within the fracture and/or the sphenoid sinus. Of the 12 reported cases of basilar incarceration, 11 of these injuries have proved to be fatal due to pontine infarction. We present a patient with basilar artery incarceration without any neurologic deficits. Case Description: The case reported is a 17-year-old male who presented after a motor vehicle collision with a linear and longitudinal clival fracture with entrapment of the basilar artery within the sphenoid sinus. Diagnostic subtraction angiography showed a small intimal tear with possible intraluminal thrombus. The patient was started on aspirin and at 3-month post injury had no neurologic deficits. Conclusion: Basilar artery incarceration is an injury often associated with pontine infarction secondary to basilar artery dissection and/or thrombus developing at the site of entrapment. Our case illustrates a favorable outcome after this injury. Based on these results, antiplatelet therapy may be a viable option for prevention of brainstem infarcts in patients with this injury; however, further prospective studies must be done to assess the overall efficacy and validity of this treatment. There are no established treatment guidelines for this condition. Objectives/Hypothesis: Examine a protective crumple zone effect of paranasal sinuses and nasal cavity on skull base fractures. Study Design: Randomized-control, cadaveric study. Methods: In the experimental group (n = 4), the nasal cavity and bilateral sinuses of cadavers were obliterated with bone cement, whereas the control group (n = 4) had native sinus architecture. Increasing frontal, glabellar impacts were introduced. Each impact event was examined with a high-speed video camera and sphenoid sinus pressure sensor. After each impact, computed tomography scans were performed and fracture sites were analyzed. Results: The control group with intact sinuses showed statistically longer time duration, during which kinetic energy transfer occurred, and longer sphenoid wall pressure equilibrium time after an impact (P<0.05). In the experimental group, there were statistically higher fracture incidences of clivus, petrous portion of internal carotid, occipital bone, and foramen magnum (P<0.05). The type A pattern (n = 6) had anterior skull base failure occurring before posterior skull base failure. Type B pattern (n = 2), seen only in two experimental specimens, is marked by premature posterior skull base collapse occurring before anterior skull base failure with grossly disrupted posterior cranial fossa structures. Conclusion: The presence of nasal cavity and paranasal sinuses behaves as a crumple zone to protect the cranial structures, preferentially posterior cranial fossa. Obliteration of the nasal cavity and paranasal sinuses with bone cement significantly increased structural tolerance of the anterior cranial vault to frontal, glabellar impacts at the cost of premature, posterior cranial fossa failure. Infarction of the posterior cerebral artery circulation arising from entrapment of the basilar artery in a fracture of the clivus has been reported in the medical literature, predominantly in the radiology and emergency medicine journals. Review of the medical literature on the topic revealed 14 published cases of entrapment of the basilar and/or vertebral artery within a longitudinal fracture of the clivus. These were all reported between 1964 and 2016 and postmortem examination had been conducted on seven cases. Cases of cerebrospinal fluid (CSF) rhinorrhea due to clival fracture are rare. We present a case of bacterial meningitis with CSF rhinorrhea after a clival fracture. Heavily T2-weighted images showed a bone flap in the thinned clivus and fluid collection in the sphenoid sinus. CSF rhinorrhea developed at 1 month after mild trauma. The fracture may have been caused by the trauma and/or by the pressure gradient between the intracranial CSF space and the sphenoid sinus. A detailed history to identify trauma and an examination to detect bone defects in the skull base are necessary when patients present with bacterial meningitis and persistent rhinorrhea.
