Abstract

Acquired brain injury (ABI) is a leading cause of lifelong disability, but access to treatment in the chronic stages has significant barriers. Group-based, remotely delivered neurorehabilitation reduces costs, travel barriers, and infection risk; however, its feasibility for patients with ABI is not well-established. To investigate the feasibility of remotely group-based cognitive and mood therapies for persons with chronic ABI. Three hundred and eighty-eight adults with chronic ABI participated in group tele-neurorehabilitation modules comprising Cognitive Behavioral Therapy, Goal Management Training®, Relaxation and Mindfulness Skills Training, and/or a novel Concussion Education & Symptom Management program. Assessments comprised quantitative metrics, surveys, as well as qualitative semi-structured interviews in a subset of participants. High retention, adherence, and satisfaction were observed. Facilitators of treatment included accessibility, cost-effectiveness, and convenience. Adoption of technology was high, but other people's technological interruptions were a barrier. Self-reported benefits specific to group-based format included improved mood, stress management, coping, interpersonal relationships, cognitive functioning, and present-mindedness. The present study examined chronic ABI patients' perceptions of telerehabilitation. Patients found remotely delivered, group-based mood, and cognitive interventions feasible with easy technology adoption. Group format was considered a benefit. Recommendations are provided to inform design of remotely delivered ABI programs. Group-based mood and cognitive telerehabilitation is feasible for persons with chronic acquired brain injury, with high reported satisfaction. Screening for technical proficiency and providing ongoing technical support improves therapy adherence and retention. Abstract

More than a quarter of adults with concussion endure prolonged symptoms of >3 months. We developed the Concussion Education Self-Management program to help people manage persisting symptoms. Here, we assess feasibility, preliminary efficacy, and correlates of response. N = 80 adults participated in the program; ages ranged from 18 to 65 years and time post-injury ranged from 6 months to 18 years. Weekly sessions, delivered remotely and in groups, comprised education and strategies for management of cognitive, emotional, and physical symptoms. Primary outcome: Confidence to self-manage symptoms. Secondary outcomes: Quality of life; mood/anxiety/stress. Predictors of response: Self-reported cognitive, emotional and physical symptoms at intake. Pre- to post-program improvements were observed in confidence to self-manage, p < 0.03; quality of life, p < 0.001; depression, p < 0.001; anxiety, p < 0.001; and stress, p < 0.001. Considering confidence to self-manage, those with fewer cognitive and physical symptoms benefitted more (p's < 0.0005 and p < 0.01, respectively). This program shows promise for improving self-management of prolonged symptoms. Those with high symptom burden may need extra sessions to benefit. This is a cost-effective and scalable program that can reach people regardless of geographic location or impediments to travel. [ABSTRACT FROM AUTHOR]

Abstract

Progressive cortical volumetric loss following moderate–severe traumatic brain injury (TBI) has been observed; however, regionally specific changes in the structural determinants of cortical volume, namely, cortical thickness (CT) and cortical surface area (CSA), are unknown and may inform the patterns and neural substrates of neurodegeneration and plasticity following injury. We aimed to (a) assess differences in CT and CSA between TBI participants and controls in the early chronic stage post‐injury, (b) describe longitudinal changes in cortical morphometry following TBI, and (c) examine how regional changes in CT and CSA are associated. We acquired magnetic resonance images for 67 participants with TBI at up to 4 time‐points spanning 5 months to 7 years post‐injury, and 18 controls at 2 time‐points. In the early chronic stage, TBI participants displayed thinner cortices than controls, predominantly in frontal regions, but no CSA differences. Throughout the chronic period, TBI participants showed widespread CT reductions in posterior cingulate/precuneus regions and moderate CT increase in frontal regions. Additionally, CSA showed a significant decrease in the orbitofrontal cortex and circumscribed increase in posterior regions. No changes were identified in controls. Relationships between regional cortical changes in the same morphological measure revealed coordinated patterns within participants, whereas correlations between regions with CT and CSA change yielded bi‐directional relationships. This suggests that these measures may be differentially affected by neurodegenerative mechanisms such as transneuronal degeneration following TBI and that degeneration may be localized to the depths of cortical sulci. Abstract

Objective: To design and preliminarily test a questionnaire intended to measure patient treatment burden resulting from participation in cognitive assessments and interventions. Methods: An expert consensus process was used to develop the concept of patient treatment burden and to determine the first set of questionnaire items and administration protocol. The pilot questionnaire was administered to 20 patients with mild to severe acquired brain injuries on completion of a 2-h or longer neuropsychological assessment. Following preliminary testing, the questionnaire was revised and re-evaluated by a second expert panel and content validity was assessed. Results: Burden was defined as psychologically and/or physically aversive symptoms in response to cognitive assessment or intervention. The first questionnaire contained 21 items assigned to 3 categories: physical, cognitive, and emotional. Eightyfive percent of patients endorsed symptom level increases, with "tired/fatigued" the most frequently endorsed item (80% of patients). Instructions and test items were easily understood, and the questionnaire was quick to administer. Content validity ratio (CVR) of the revised questionnaire yielded 23 acceptable items and a subset met the highest CVR threshold (>0.78). Conclusion: This patient-reported outcome will ultimately help patients give voice to aversive experiences, and help clinicians and researchers to monitor and adapt assessments/treatments appropriately. Future steps in development are described.

Abstract

Background: Long term effects of concussions and sub‐concussions in athletes can include delayed neurodegeneration as seen in Alzheimer's disease (AD) and chronic traumatic encephalopathy (CTE). The PET tracer [18F]AV1451 has high affinity and selectivity for paired helical filament‐tau in vivo, the pathological form of tau in CTE and AD. The aim of this study was to assess the effect of tau‐PET binding on brain atrophy in former contact sports athletes and evaluate differences between subjects with and without tau‐PET binding. Method: 31 retired athletes (age:53±15; sex: 29M, 2F; concussions/athlete: 7±6) negative for AD (CSF: p‐tau <68 pg/mL and ATI >0.8 and/or normal range of plasma p‐tau181) and 54 normal controls (age: 50±13) were included. All athletes completed MRI, [18F]AV1451 tau‐PET, the Sport Concussion Assessment tool 2 (SCAT2) and the Personality Assessment Inventory (PAI) depression and anxiety scales. Tau‐PET standardized uptake value ratios (SUVR) images were created using cerebellar gray matter (GM); tau‐PET positivity was set to ≥1.30 SUVR. Normal control MRI scans were used to create age/intracranial volume corrected GM volume maps. Result: GM volumes were significantly lower in tau‐PET positive voxels in retired athletes ≥40 y.o. (N = 25; ‐0.48±0.40 vs. ‐0.42±0.41; p =.02) and there was a trend for lower GM volumes in tau‐PET positive voxels across athletes of all ages (N = 30; ‐0.40±0.42 vs. ‐0.35±0.41; p =.09), compared to tau‐PET negative voxels. Mean tau‐PET SUVR values in the cortical GM were used to divide retired athletes into tau‐PET positive (N = 20; SUVR ≥1.30) and negative (N = 11; SUVR <1.30) groups. The tau‐PET positive group did not differ in age (52±16 vs. 56±12; p =.4) or concussion number (9±7 vs. 7±5; p = 1.0) from the tau‐PET negative group; the tau‐PET positive group had significantly higher SCAT2 symptom severity (28.26±25.15 vs. 8.89±12.67; p =.01), PAI anxiety t‐scores (49.90±11.25 vs. 42.55±4.13; p =.01), and a trend for higher PAI depression t‐scores (54.75±16.58 vs. 47.09±8.08; p =.095), compared to the tau‐PET negative group. Conclusion: Tau‐PET may be useful for identifying those at risk for delayed neurodegeneration. Older athletes showed relatively greater brain atrophy in tau‐PET positive regions, suggesting that detection of tau‐PET at a younger age may enable interventions to prevent future degeneration.
