BackgroundO’Donnell-Luria-Rodan syndrome (ODLURO) is an autosomal-dominant neurodevelopmental disorder caused by pathogenic, mostly truncating variants in KMT2E. It was first described by O’Donnell-Luria et al in 2019 in a cohort of 38 patients. Clinical features encompass macrocephaly, mild intellectual disability (ID), autism spectrum disorder (ASD) susceptibility and seizure susceptibility.MethodsAffected individuals were ascertained at paediatric and genetic centres in various countries by diagnostic chromosome microarray or exome/genome sequencing. Patients were collected into a case cohort and were systematically phenotyped where possible.ResultsWe report 18 additional patients from 17 families with genetically confirmed ODLURO. We identified 15 different heterozygous likely pathogenic or pathogenic sequence variants (14 novel) and two partial microdeletions of KMT2E. We confirm and refine the phenotypic spectrum of the KMT2E-related neurodevelopmental disorder, especially concerning cognitive development, with rather mild ID and macrocephaly with subtle facial features in most patients. We observe a high prevalence of ASD in our cohort (41%), while seizures are present in only two patients. We extend the phenotypic spectrum by sleep disturbances.ConclusionOur study, bringing the total of known patients with ODLURO to more than 60 within 2 years of the first publication, suggests an unexpectedly high relative frequency of this syndrome worldwide. It seems likely that ODLURO, although just recently described, is among the more common single-gene aetiologies of neurodevelopmental delay and ASD. We present the second systematic case series of patients with ODLURO, further refining the mutational and phenotypic spectrum of this not-so-rare syndrome.

This article describes the advantages of teaching Agnès Varda’s early short film L’Opéra-Mouffe, known in English as Diary of a Pregnant Woman, in an introductory course on film form. This award-winning, sixteen-minute film offers a compelling demonstration of the core characteristics of the essay film, along with readily teachable examples of visual metaphor, cinematography, sound, and editing. Its cultural and auteurist contexts can also complement a variety of curricular topics, including the French New Wave and Left Bank creators, post-war France, and feminist filmmaking. Finally, its focus on Varda’s personal experience of pregnancy also makes L’Opéra-Mouffe an ideal vehicle to introduce radical feminist pedagogies that recognize and value personal experience as a valid way of knowing about the world.

This introduction to the dossier on Teaching Women’s Filmmaking contextualizes the contributions among recent work that examines classroom praxis and relevant innovations in film and media studies scholarship, such as videographic criticism and online publication. It considers three key components of building a course—what to study, how to run a classroom, and how to implement assessments—and discusses each area according to critical feminist pedagogies.

Bariatric surgery continues to be the most reliable treatment for the disease of obesity. Despite excellent results, some patients experience weight recurrence with or without concomitant recurrence of co-morbidities. There is currently no standard definition for clinically significant weight recurrence after bariatric surgery so that patients and clinicians have a platform from which to plan treatment. The Post-Operative Weight Recurrence (POWER) Task Force was formed by the American Society for Metabolic and Bariatric Surgery to address this aspect of the disease of obesity. This article reviews the literature of existing definitions for weight recurrence and their limitations. Furthermore, the term weight recurrence is introduced to replace weight regain or recidivism, and the term nonresponder is introduced to replace inadequate weight loss after surgery.

This essay highlights the shared critical terrain of adaptation and nostalgia: how they critically juxtapose the past with the present, and how they underscore the impossibility of return while also relying on prior experience. It also explores nostalgia’s effect on personal responses to adaptations and its interaction with textual form. Drawing from various areas of literary, media, and performance studies, including film adaptations of children’s literature, Watchmen and its screen adaptations, and Disney’s live-action remakes, this essay underscores how both nostalgia and adaptation are inherently multivalent concepts, and how they each rely on perspective to generate critical meaning.

Cem Yılmaz has become nearly synonymous with Turkish comedy through comedies like Arif V 216 (2018). The impact of the SARS-CoV-2 virus on delivery of hospital care during the ongoing COVID-19 pandemic has required effective nursing leadership of large teams of Advanced Practice Registered Nurses (APRNs). At a large academic medical center in the mid-Atlantic United States, nurse leaders combined knowledge of the psychological phases of disaster and principles of the Serving Leadership framework to mobilize and lead APRN teams to deliver high-quality care to all patients and provide a safe working environment for healthcare teams. In this article, we discuss exemplars that demonstrate the flexibility and strengths of APRNs in all roles who addressed rapidly evolving needs such as resource allocation and education. The conclusion highlights the value and relevance of APRN skillsets to clinical practice and the opportunity to further capitalize on these strengths during the continuing journey through this COVID-19 pandemic and beyond.

Objectives Sepsis is a life-threatening medical emergency. There is a paucity of information on whether quality improvement approaches reduce the in-hospital sepsis caseload or save lives and decrease the healthcare system and society's cost at the provincial/national levels. This study aimed to assess the outcomes and economic impact of a province-wide quality improvement initiative in Canada. Design Retrospective population-based study with interrupted time series and return on investment analyses. Setting The sepsis cases and deaths averted over time for British Columbia were calculated and compared with the rest of Canada (excluding Quebec and three territories). Patients Aggregate data were obtained from the Canadian Institute for Health Information on risk-adjusted in-hospital sepsis rates and sepsis mortality in acute care sites across Canada. Interventions In 2012, the British Columbia Sepsis Network was formed to reduce sepsis occurrence and mortality through education, knowledge translation, and quality improvement. Measurements and main results A return on investment analysis compared the financial investment for the British Columbia Sepsis Network with the savings from averted sepsis occurrence and mortality. An estimated 981 sepsis cases and 172 deaths were averted in the post-British Columbia Sepsis Network period (2014-2018). The total cost, including the development and implementation of British Columbia Sepsis Network, was $449,962. Net savings due to cases averted after program costs were considered were $50.6 million in 2018. This translates into a return of $112.5 for every dollar invested. Conclusions British Columbia Sepsis Network appears to have averted a greater number of sepsis cases and deaths in British Columbia than the national average and yielded a positive return on investment. Our findings strengthen the policy argument for targeted quality improvement initiatives for sepsis care and provide a model of care for other provinces in Canada and elsewhere globally.

This article presents the story of a symposium dedicated to the life and legacy of Agnès Varda and its move online due to COVID-19, reflecting on this process and its implications for future scholarship. Based on this experience, and using Varda's own late-career digital transition as a model, the author interrogates the potential for online platforms to serve as a kind of scholarly postcard that offers accessible and more sustainable practices for sharing research and building more resilient and inclusive academic communities.

Adaptation is a creative process that crosses and blurs boundaries: from page to stage, from small screen to big screen – and then, sometimes, back again. Beyond questions of form and medium, many adaptations also cross national borders and language barriers, making them important tools for intercultural communication and identity formation. This paper calls for a more intensive, transnational study of adaptation across print, stage, and screens in EU member and affiliate countries. For the highest possible effectiveness, interdisciplinarity is key; as a cultural phenomenon, adaptation benefits from perspectives rooted in a variety of fields and research methods. Its influence over transnational media flows, with patterns in production and reception across European culture industries, offers scholars a better understanding of how narratives are transformed into cultural exports and how these exchanges affect transnational relationships. The following questions are proposed to shape this avenue for research: (1) How do adaptations track narrative and media flows within and across national, linguistic, and regional boundaries? (2) To what extent do adapted narratives reflect transnational relationships, and how might they help construct Europeanness? (3) How do audiences in the EU respond to transnational adaptation, and how are European adaptations circulated and received outside Europe? (4) What impact does adaptation have in the culture industries, and what industrial practices might facilitate adaptation across media platforms and/or national boundaries? The future of adaptation studies and of adaptation as a cultural practice in Europe depends on the development of innovative, comparative, and interdisciplinary approaches to adaptation. The outcomes of future research can hold significant value for European media industries seeking to expand their market reach, as well as for scholars of adaptation, theater, literature, translation, and screen media.

Background: Quality improvement (QI) has received much wider attention in public policy, given the ever-increasing aging population and growing demand of long-term care (LTC). In the policy context, health system planners and administrators need to know the evidence on cost-effectiveness for resource allocation decisions. The objectives of this study were to: identify costs and benefits of the QI strategies in the economic literature; and synthesize the methodological approaches used in the economic studies and reported evidence around the cost-effectiveness of QI strategies in LTC. Methods: A scoping review of three online databases, including PubMed, EconLit, and Google Scholar, was conducted to identify economic studies focusing on elderly population published from September 2010 to August 2020. A combination of search expressions was used to identify the original research articles. Study titles and abstracts were screened, and only full-text articles meeting the inclusion criteria were retained in the review. A standard data extraction template was used capturing patient population, intervention, comparator, the outcome of interest, and study design (PICOS). Results: The search identified 657 records, of which 16 were included in the review. The financial costs were categorized into three groups: (i) developmental, (ii) operational, and (iii) health resource utilization. The terms ‘Quality of life,’ ‘Health-related Quality of Life’, and ‘Quality of Care’ were interchangeably used to express the health outcomes. This study showed mixed results suggesting a substantial variability around cost-effectiveness of QI interventions targeting elderly population in LTC. Almost all studies reported a public payer perspective and a discount rate ranged from 0 to 7%. A poorly defined baseline, the complex nature of the intervention, a lack of appropriate comparator, and varying duration of implementation were key methodological limitations. Conclusions: This review highlights important knowledge gaps regarding societal costs and inconsistencies in reporting of the outcome measures. More research is needed to analyze long-term costs and consequences to better inform decisions on resource allocation in LTC.

After her passing in 2019, French director Agnes Varda left a pioneering, deeply personal oeuvre. Her connections and collaborations had always influenced her work: her marriage to Jacques Demy, her status as “mother” of the French New Wave, her co-directorship with artist JR. The personal reflections in her final documentaries, particularly The Beaches of Agnes (2008) and her final film Varda par Agnes (2019) invite new consideration of these sometimes friendly, sometimes fraught relationships between Varda and the men who made films around and alongside her. In undertaking this reconsideration, this essay poses broader questions about Varda’s legacy and examines what both film studies and popular cinephilia can learn from her singular, feminine approach to screen art.

Commissioned research was undertaken to explore the role of networks in supporting large-scale change and improvement. Participatory action research and social network analysis were used to study the BC Sepsis Network. Findings of this research include insights into distributed leadership, enablers and barriers within a network approach; the importance of relationships and trust; and the need for meaningful and timely data. Recommendations are made for health leaders who are considering utilizing networks for improving patient quality and safety.

Health care in general, and surgical care specifically, is a team sport. To provide optimal patient care and enjoyment at work, teamwork and communication within surgical services benefit from deliberate development. This chapter describes the characteristics of highly effective health care teams and what is required of team members. Overall team performance is affected by the skills of members, the clarity of the team's goals, and the effectiveness of the interlaced activities of members. Teams practice and learn. Effective teams provide safer care. In addition to the capabilities of teams, the system(s) in which the teams work and the culture of the workplace directly affect team performance and their patient care outcomes. This chapter provides an overview of the systems and culture as they affect teams and describes an approach to measurement of team communication and safety culture that goes beyond the assessment of the collective technical competence of the team. The authors describe their experience in using the Safety Attitudes Questionnaire and the NOTECHS nontechnical skills framework to assess the culture of the surgical environment and surgical teams.

Marjane Satrapi's Poulet aux prunes offers an intriguing example of self-adaptation from comics to live-action film. This essay will consider how the Franco-Iranian Satrapi, within her dual role as self-adapter and transnational filmmaker, uses intertextuality and remediation beyond her own source text in ways that pointedly expand the transnational resonance of her film. These narrative and aesthetic strategies also extend to the film's paratextual discourses, namely, the extras available on the French DVD release of the film. The book, film, and DVD paratexts related to Poulet aux prunes thus form the core of this discussion of self-adaptation and transnationality. © The Author 2014.

Background: The U.S. nursing workforce is facing a severe shortage of professionals due to an anticipated retirement of more than 500,000 RNs in the next 5 years. Method: An Escape to Nursing event was created and delivered in a rural medical center college of nursing in the spring of 2017. This faculty team developed four distinct rooms that were designed with key nursing concepts and patient health conditions. Recruits worked in teams to solve the patient health care puzzle, which then enabled them to proceed to the next room. Results: Feedback was positive from the recruits, including comments about expanding the game to more health care scenarios and reaching out to more recruits. Additional recruitment may lead to better turnout of recruits. Conclusion: Linear, sequential formatting for the escape room game design is recommended for use with recruits into nursing. Chapter 10 Known for making movies about movies, writer-director Quentin Tarantino reached new heights of Oscar success with the intertextual war film Inglourious Basterds (2009) and the allusive representation of slavery in Django Unchained (2012). This chapter examines both films through the lens of industry recognition by looking at how the history of writing awards at the Academy indicates a shifting relationship to the kind of multi-sourced, cinema-focused adaptation that Tarantino undertakes. The categorization of Tarantino’s writing under ‘Original Screenplay’ shows how contemporary awards culture defines both adaptation and originality, while his success with awards voters has started new cycles of prestige and pushed industry focus away from conventional representations of war and white-centred representation of race relations.

Wild Fragaria supercore accessions from the U.S. Department of Agriculture (USDA) National Plant Germplasm System collection have been evaluated in temperate climates; however, there have been no characterizations of supercore accessions in non-temperate climates or in annualized production systems. Because Florida can serve as a model system for annualized winter and spring production worldwide, the objective of this study was to characterize an elite group of wild strawberry accessions under field and high tunnel production systems for mortality and the phenological responses of flowering and runner production. The wild accessions along with cultivars were planted in open-field and high tunnel production environments in a randomized complete block design within each environment with raised beds serving as blocks. Four replications of five-plant plots were planted in each of two environments in 2 separate years. With the exception of Darrow 72, F. chiloensis accessions did not perform well in the minimum-chill annualized winter production system. The accessions of this species generally did not flower and were prolific instead in runner production. The F. virginiana accessions performed better with F. virginiana subsps. grayana and virginiana accessions appearing more adapted for minimum-chill winter production because they flowered well and had few runners. We conclude that NC 96-48-1 (PI 612324), NC 95-21-1 (PI 612569), Darrow 72 (PI 236579), and RH 30 (PI 612499) would be attractive for inclusion in germplasm development in a minimal-chill, winter annual production system. In addition to producing many flowers and few runners, these accessions had low mortality.

Powdery mildew (PM) of strawberry (Fragaria sp.) is a ubiquitous, wind-spread disease caused by the obligate parasite Podosphaera aphanis. To control PM, multiple fungicide applications are necessary each season, and none of the major cultivars in Florida have high levels of resistance. Therefore, the objectives of this study were to observe the response to selection and to estimate genetic parameters for PM and related traits in the University of Florida breeding population. In 2010, clonally replicated individuals from seven biparental crosses arising from 11 parents were included in a field trial in which clonally replicated seedlings were evaluated visually for percent coverage of PM mycelium using a modified Horsfall-Barratt scale of 0 to 6. Broad- (H2) and narrow-sense (h2) heritabilities for PM score were (mean ± se) 0.50 ± 0.08 and 0.40 ± 0.39, respectively, for the base population. After the second round of selection in the resistant population, no additive variance was detected, indicating that alleles for PM resistance had become fixed. In contrast, after two rounds of divergent selection in the susceptible population, there remained considerable additive variance (h2 = 0.42 ± 0.65). Moderate to high heritability estimates and a clear response to selection indicate that resistance to PM is genetically controlled through mostly additive effects. Selection of parents based on field trials with natural inoculum should result in good progress toward more resistant cultivars. The consistently moderate to strong genotypic and genetic correlations among PM and canopy density (CD) indicate that selection for PM resistance will result in reduced CD. Therefore, CD must be monitored over successive rounds of selection for low levels of PM to prevent CD falling below the commercially acceptable range.

Many breeders have turned to wild relatives in search of beneficial traits such as disease resistance. In strawberry, the wild octoploid species Fragaria virginiana and F. chiloensis are fully interfertile with the cultivated species, F. ×ananassa, and are therefore potential sources of resistance. Powdery mildew may increase in economic importance in Florida in the near future as a result of the use of high tunnels and rowcovers for freeze protection, which limit free water and provide a favorable environment for disease development. The objective of this study was to screen an elite group of wild strawberry accessions for resistance to powdery mildew under two production systems. In 2010–11 and 2012–13, wild accessions, commercial standard cultivars, and susceptible controls were planted in open-field and high tunnel environments at the Gulf Coast Research and Education Center in Balm, FL. Although there was a significant year × genotype effect, some taxa showed high levels of resistance that were consistent across years. There was a high correlation for ratings of powdery mildew between the high tunnel and the open field for all genotypes (r = 0.89, P < 0.001). This information may be useful for breeders, because sources of resistance to powdery mildew are available within the tested genotypes. However, some accessions are highly susceptible to powdery mildew, and this must be considered when using these genotypes in breeding programs.

Laparoscopic Roux-en-Y gastric bypass (RYGBP) has been used increasingly more often in the past 10 years. The authors summarize their experience and safety/complications data based on 849 laparoscopic RYGBP procedures. They also evaluate the use of the Endopath trocar in terms of trocar-site hernias, bowel obstruction, and elimination of time-consuming fascial closure. From July 2000 to December 2003, 849 laparoscopic RYGBP procedures were performed using a bladeless, 12-mm, visual entry trocar. The patients’ average body mass index (BMI) was 53.2 kg/m2. The trocar ports (n = 3,744) were not closed. Perioperative and postoperative assessments were performed. In this study, 74% of the patients were retained for follow-up evaluation (mean, 10 months). Among these patients, no intraoperative bowel or vascular injuries, no mortality, and two trocar-site hernias (0.2%) were found. At 1 year, the mean excess weight loss was 73.4%. The Endopath trocar system shows a trend toward reducing trocar-site hernias, decreasing bowel obstruction, and eliminating the need for time-consuming fascial closure, although further studies are needed to confirm these findings.

There is experimental evidence but very few human studies that suggest a role for obesity in the formation and progression of some glomerular lesions. We report the case of a morbidly obese male with hematuria and proteinuria that was subsequently diagnosed with renal failure which required dialysis. Histological findings of the renal biopsy performed during a laparoscopic gastric bypass are presented. His renal failure resolved with the weight loss.
