3. Association between electroencephalogram alpha-band oscillations and executive and processing functions in patients with cerebral small vessel diseases.

Abstract

Electroencephalogram (EEG) alpha-band oscillations may reflect executive and processing function in patients with cerebral small vessel disease (CSVD). We aimed to assess such association and its relationship with CSVD severity, and to identify specific alpha-band parameters and the cut-off values for cognitive screening. We analysed the dispersion of amplitude-frequency characteristics of EEG alpha-band and different alpha-band parameters (PFα, ΔPFα, PPα, NCL) in different brain locations. We also assessed patients' executive and processing functions using verbal fluency test (VFT) and color trails test (CTT), and CSVD severity using total burden and Fazekas scores. 129 patients were recruited in the study. After adjusting for age, gender and education, PFα(F3), PFα(F4) and NCL were significantly associated with VFT-composite performance (p < 0.05). CTT-1 time and error were associated with PFα(F3), PFα(F4), ΔPFα(O1;F3) and CSVD severity (p < 0.05), whereas CTT-2 time was only associated with CSVD severity. Moreover, the correlations between alpha-band oscillations and cognitive function were higher in low than in high disease-severity group (ρ: −0.58 vs. −0.38, p < 0.05). The AUC of selected alpha-band parameters were higher than 0.8 for VFT and CTT. Specific alpha-band parameters in the frontal lobe were identified to correspond to executive and processing function. Abstract

Health care workers (HCWs) are susceptible to cumulative stressors and their detrimental effects. Hence, it is important to identify the factors in HCWs’ stress management behaviors to help them better cope with daily stress. Inspired by the health belief model (HBM), we decided to focus on examining the psychosocial pathway between stressors and stress outcomes among HCWs. A sequential explanatory mixed-methods study was conducted in two general hospitals in Wuhan, China. A constructive questionnaire was applied to assess the psychological measures of stress, and the Holter devices were used to evaluate the physical effects of stress. Subsequently, a multiple case study was conducted to better understand factors related to stress management attitudes and behaviors among HCWs. A total of 292 HCWs were included in this study. The results showed that 67.1% of participants were overstressed, with 57.5%, 69.2%, and 78.4% of participants suffering from depression, anxiety, and stress symptoms, respectively. Only half of the participants adopted stress management behaviors, and stress management attitudes and behaviors played a mediating role between stressors and stress outcomes for both physical indicators (β = 0.014, p = .006) and psychological measures (β = −.001, p = .052). Analyses of the results of this qualitative study consistently found that knowledge, skills, and positive feedback were critical elements for stress management behaviors. Promoting positive health beliefs around stress management, and emphasizing, in particular, the positive role of stress management in HCWs’ personal lives, was critical to help HCWs build stress management behaviors. 20. The Effect of Massage on the Cardiac Autonomic Nervous System and Markers of Inflammation in Night Shift Workers: a Pilot Randomized Crossover Trial

Abstract

Background Shift work is a necessary part of many industries; however, it can have detrimental effects on health over time. Purpose This study investigated the effect of a massage intervention on the cardiac autonomic activity and blood inflammatory markers of healthy medical residents working night shifts. Setting This trial was conducted at British Columbia Children’s and Women’s Hospital between February 2014 and June 2016. Participants Included participants were generally healthy medical residents and were working rotating night shifts on a regular basis. Research Design This was a randomized, controlled, crossover, open-label trial (NCT02247089). Interventions Participants received either a 30-min massage intervention or reading control after consecutive periods of night shift. Main Outcome Measures The primary outcome was high frequency, a proxy for the cardiac parasympathetic activity, measured via heart rate variability. Secondary outcomes included other heart rate variability measures, blood markers of inflammation, and blood pressure. Results Twelve participants were recruited (nine female) with median age of 28 years. There was no significant difference between the massage intervention and the reading control for the primary outcome, (median relative change between pre- and postmassage [interquartile range]: 62% [−1 to 150], pre- and postreading: 14% [−10 to 51], p = .16). Similarly, there was no difference with respect to blood inflammatory markers and blood pressure. Median high frequency significantly increased between pre- and postmassage (185 vs. 358 ms2, p = .04). Conclusion This pilot study found no statistically significant difference between the massage intervention and the reading control; however, we did observe a significant increase in median high frequency from before massage to after massage, indicative of increased parasympathetic activity. This study may help inform planning of larger trials evaluating massage interventions on the activity of the autonomic nervous system and managing shift work stress.

Abstract

A noninferiority trial was conducted on 637 stable, severely ill children with hemoglobin concentration less than 9.5g per deciliter after being admitted in an intensive care unit for seven days to determine that a restrictive transfusion strategy of using packed red cells that were leukocyte-reduced before storage is as safe as a liberal transfusion strategy. It was inferred that the transfusion requirements were reduced without increasing adverse results in children with a hemoglobin threshold of 7 g per deciliter for red-cell transfusion.

Available evidence suggests that prolonged and exclusive breastfeeding is associated with lower infant weight and length by 6 to 12 months of age. This evidence, however, is based on observational studies, which are unable to separate the effects of feeding mode per se from selection bias, reverse causality, and the confounding effects of maternal attitudinal factors. Design/Methods. A cluster-randomized trial in the Republic of Belarus of a breastfeeding promotion intervention modeled on the World Health Organization (WHO)/UNICEF Baby-Friendly Hospital Initiative versus control (then current) infant feeding practices. Healthy, full-term, singleton breastfed infants (n = 17 046) weighing ≥ 2500 g were enrolled soon after birth and followed up at 1, 2, 3, 6, 9, and 12 months old for measurements of weight, length, and head circumference. Data were analyzed according to intention-to-treat, while accounting for within-cluster correlation. To assess the potential for bias in observational studies of breastfeeding, we also analyzed our data as if we had conducted an observational study by ignoring treatment, combining the 2 randomized groups, and comparing 1378 infants weaned in the first month and those breastfed for the full 12 months of follow-up with either ≥ 3 months (n = 1271) or ≥ 6 months (n = 251) of exclusive breastfeeding. Results. Infants from the experimental sites were significantly more likely to be breastfed (to any degree) at 3, 6, 9, and 12 months and were far more likely to be exclusively breastfed at 3 months (43.3% vs 6.4%). Mean birth weight was nearly identical in the 2 groups (3448 g, experimental; 3446 g, control). Mean weight was significantly higher in the experimental group by 1 month of age (4341 vs 4280 g). The difference increased through 3 months (6153 g vs 6047 g), declined slowly thereafter, and disappeared by 12 months (10564 g vs 10571 g). Analysis by z scores confirmed that infants in both groups gained more weight than the WHO/Centers for Disease Control and Prevention reference, with no evidence of undernutrition in the control group. Length followed a similar pattern. In the observational analyses, infants weaned in the first month were slightly lighter and shorter at birth and their weight-for-age and length-for-age z scores declined by 1 month, but they caught up to both experimental and the other observational groups by 6 months and were heavier and longer by 12 months. Among infants in the 2 prolonged and exclusive breastfeeding groups, weight-for-age z scores fell slightly between 3 and 12 months; length-for-age fell below the reference by 6 months with catch-up to the reference by 12 months. Head circumference showed no significant differences at any age between the 2 trial groups or among the observational groups. Conclusions. Our data, the first in humans based on a randomized experiment, suggest that prolonged and exclusive breastfeeding may actually accelerate weight and length gain in the first few months, with no detectable deficit by 12 months old. These results add support to current WHO and UNICEF feeding recommendations. Our observational analysis showing faster weight and length gains with early weaning and slower gains with prolonged and exclusive breastfeeding may reflect unmeasured confounding differences or a true biological effect of formula feeding.
