Lipschütz ulcer is a non-sexually transmitted genital lesion of unknown etiology, which presents as a painful vulvar ulcer. Lipschütz ulcers have been described in most continents. This is the first case reported in Peru and South America. We present the case of a 33-year-old female patient with a Lipschütz ulcer after being vaccinated with the second dose of the AstraZeneca COVID-19 vaccine. She reported having had only one sexual partner in her lifetime. Laboratory results were negative for herpes simplex 2, Cytomegalovirus, Toxoplasma gondii, Epstein-Barr virus, and syphilis. The patient received symptomatic treatment. Ten days after the onset, the patient was significantly better during follow-up. This case report displays a potential adverse effect of the AstraZeneca COVID-19 vaccine as a Lipschütz ulcer triggered by the host humoral immune response. However, further research is needed to establish the causal relationship between these two.

Objetive: To describe a pediatric case with initial diagnosis of lymph node tuberculosis (TB) that became multidrug resistant miliar and meningeal tuberculosis (TB-MDR) due to treatment dropout twice. Case report: a 4-year-old boy with initial diagnosis of lymph node tuberculosis who had two episodes of dropout from the sensitive scheme treatment. Three months later, there was evidence of bilateral involvement suggestive of miliary TB. During hospitalization, he developed meningeal tuberculosis and hydrocephalus. Due to the history of treatment dropout, a sensitivity of gastric aspirate study was requested, identifying it as multidrug-resistant TB. He started the EZLfxKmEtoCs treatment scheme and complications management, achieving improvement. Conclusions: Treatment dropout is one of the main causes of drug resistance in tuberculosis and its complications. It is necessary to reinforce the early detection and effective treatment of this infection in children, focusing on the follow-up of cases to avoid treatment dropout and the consequent complications. Objetivo: Presentar el caso de un paciente pediátrico con cuadro inicial de tuberculosis (TB) ganglionar con abandono de tratamiento en 2 ocasiones que evolucionó a la forma miliar y meningitis tuberculosa multidrogorresistente. Reporte del caso: Varón de 4 años con diagnóstico inicial de tuberculosis ganglionar, que abandonó el esquema sensible de tratamiento en dos ocasiones. Tres meses después, se evidenció compromiso bilateral sugestivo de TB miliar y durante la hospitalización desarrolló tuberculosis meníngea e hidrocefalia. Por el antecedente de abandono de tratamiento, se solicitó un estudio de sensibilidad en aspirado gástrico, identificándose como multidrogorresistente; pasando al esquema EZLfxKmEtoCs y manejo de complicaciones, consiguiendo mejoría. Conclusiones: El abandono de tratamiento es una de las principales causas de resistencia a fármacos antituberculosos y de complicaciones. Es necesario reforzar la detección temprana y tratamiento efectivo de esta infección en niños, poniéndose énfasis el seguimiento de casos para evitar abandonos de tratamiento y las complicaciones consecuentes.

Background: Obesity in children and adolescents has increased exponentially around the world. Furthermore, the COVID-19 pandemic has led to a higher pediatric obesity rate. The excess adipose tissue generates a dysregulation of adiponectin, ghrelin, and leptin, among others. Metabolic alterations can develop cardiovascular disease, dyslipidemias, arterial hypertension, type 2 diabetes mellitus, nonalcoholic fatty liver disease, sleep disorders, and higher risk of COVID-19 severity. Obesity has different therapeutic approaches such as behavioral weight loss programs, pharmacologic treatments, and surgical procedures. Therefore, timely diagnosis and treatment are important to decrease the mortality in obesity among pediatric population. Introducción: La obesidad en niños y adolescentes ha aumentado exponencialmente en todo el mundo. Asimismo, la pandemia del coronavirus (COVID-19) ha conducido a una mayor tasa de obesidad pediátrica. El exceso de tejido adiposo genera una desregulación de hormonas como adiponectina, ghrelina y leptina, entre otras. Las alteraciones metabólicas pueden ocasionar enfermedad cardiovascular, dislipidemias, hipertensión arterial, diabetes mellitus tipo 2, enfermedad del hígado graso no alcohólico, trastornos del sueño y mayor riesgo de COVID-19 severo. La obesidad tiene diferentes enfoques terapéuticos, como programas conductuales de pérdida de peso, tratamientos farmacológicos y procedimientos quirúrgicos. De esta manera, el diagnóstico y el tratamiento oportuno es importante para disminuir la mortalidad asociada a la obesidad en la población pediátrica.

Summary Anaplastic thyroid cancer (ATC) is the type of thyroid cancer that has the worst prognosis. It usually presents as a rapidly growing cervical mass that generates compressive symptoms. Its association with thyrotoxicosis is rare. A 76-year-old woman, with no contributory history, presented with a 3-month course of fast-growing cervical tumor, associated with tenderness, cough, and weight loss. Physical examination revealed goiter, localized erythema, and a painful and stone tumor dependent on the right thyroid lobe. Due to the malignant findings of the thyroid ultrasound, the patient underwent a thyroid core needle biopsy, which indicated ATC. Laboratory tests revealed leukocytosis, decreased thyroid-stimulating hormone, elevated free thyroxine (fT4), and increased thyroperoxidase (TPO) antibodies. At the beginning, we considered that the etiology of thyrotoxicosis was secondary to subacute thyroiditis (SAT) after SARS-CoV-2 infection, due to the immunochromatography result and chest tomography findings. The result of markedly elevated TPO antibodies left this etiology more remote. Therefore, we suspected Graves’ disease as an etiology; however, thyroid histopathology and ultrasound did not show compatible findings. Therefore, we suspect that the main etiology of thyrotoxicosis in the patient was the destruction of the thyroid follicles caused by a rapid invasion of malignant cells, which is responsible for the consequent release of preformed thyroid hormone. ATC is a rare endocrine neoplasm with high mortality; it may be associated with thyrotoxicosis, whose etiology can be varied; therefore, differential diagnosis is important for proper management. Learning points Anaplastic thyroid cancer is the thyroid cancer with the worst prognosis and the highest mortality. The association of anaplastic thyroid cancer with thyrotoxicosis is rare, and a differential diagnosis is necessary to provide adequate treatment. Due to the current pandemic, in patients with thyrotoxicosis, it is important to rule out SARS-CoV-2 as an etiology. Anaplastic thyroid cancer, due to its aggressive behavior and rapid growth, can destroy thyroid follicular cells, generating preformed thyroid hormone release, being responsible for thyrotoxicosis.

Objetivos: Determinar si la edad, el sexo, el tiempo de enfermedad, la leucocitosis, el hematocrito, la glicemia, la creatitina, la bilirrubina total y la amilasa sérica son factores asociados a la pertinencia de la indicación tomográfica axial computarizada en pacientes con sospecha de pancreatitis aguda. Materiales y Métodos: Se realizó un estudio de corte transversal en el Hospital Regional Docente de Trujillo, en el que se evaluó 623 pacientes con sospecha de pancreatitis aguda, luego que se excluyeran a aquellos que no cumplieron los criterios de inclusión, la muestra final fue 573 pacientes. Resultados: La prevalencia de pertinencia de la indicación tomográfica fue 7,68%. El análisis bivariado identificó al hematocrito, la glicemia sérica, la creatinina sérica y la amilasa sérica como factores asociados a la pertinencia tomográfica. El análisis multivariado identificó a las variables independientemente asociadas a la pertinencia tomográfica en pacientes con sospecha de pancreatitis aguda al tiempo de enfermedad (ORa = 1,167 IC 95% [1,012-1,345], amilasa sérica (ORa = 1,000 IC 95% [1,000-1,001] y glicemia (ORa = 1,010 IC 95% [1,002-1,018]. Conclusiones: El tiempo de enfermedad, la amilasa sérica y glicemia estuvieron asociados independientemente a la pertinencia tomográfica en pacientes con sospecha de pancreatitis aguda. El artículo remarca que es fundamental brindar una adecuada capacitación y utilizar materiales de protección, así como adoptar medidas políticas efectivas para salvaguardar la vida de todos los médicos que forman parte de la primera línea de defensa frente a la pandemia del COVID-19.

Background Preliminary data from different cohorts of small sample size or with short follow‐up indicate poorer prognosis in people with obesity compared to other patients. This study aims to precisely describe the strength of association between obesity in patients hospitalised with Covid‐19 and mortality and clarify the risk according to usual cardiometabolic risk factors in a large cohort. Methods This is a prospective cohort study including 5795 patients aged 18‐79 years hospitalized from 1st February 2020 to 30 April 2020 in Paris area, with confirmed infection by SARS‐CoV‐2. Adjusted regression models were used to estimate the odds ratios (OR) and 95% confidence intervals (95% CI) for mortality rate at 30 days across BMI classes, without and with imputation for missing BMI. Results 891 deaths occurred at 30 days. Mortality was significantly raised in people with obesity with the following OR in BMI 30‐35, 35‐40 and >40 kg/m2: 1.89 (95%CI 1.45‐2.47), 2.79 (1.95‐3.97) and 2.55 (1.62‐3.95), respectively (18.5‐25 kg/m2, as the reference class). This increase holds for all age classes. Conclusion Obesity doubles mortality in patients hospitalized with Covid‐19.

Objetivo: Establecer la asociación entre la edad, sexo, Índice de masa corporal, consumo de bebidas alcohólicas y consumo de cigarrillos con la respuesta inadecuada a la vacuna contra la Hepatitis B, en los pacientes con infección por VIH que fueron atendidos en la Unidad Clínica de Enfermedades Infecciosas y Tropicales del Hospital Regional Docente de Trujillo - Perú, entre 2014-2016. Materiales y métodos: Se realizó un estudio observacional, retrospectivo, de corte transversal, tipo casos y controles. La proteinosis alveolar pulmonar es una enfermedad intersticial difusa poco frecuente, en la cual se produce obstrucción alveolar, debido al acúmulo de surfactante pulmonar. Reporte del caso: Varón de 30 años de edad, presentó disnea progresiva y tos seca de un año de evolución. Antecedente personal: estibador de caña de azúcar. Presentó infecciones respiratorias recurrentes. Al examen físico se encontró cianosis, crepitantes difusos bilaterales y subcrepitantes en bases pulmonares. En la tomografía torácica con contraste se encontró un patrón de "empedrado loco". Se realizó videobroncoscopia con lavado broncoalveolar, aspirando material lechoso, espumoso y mucoso. Por citología se encontró linfocitos y material eosinofílico proteináceo acelular. El estudio anatomopatológico de la biopsia transbronquial reveló proteinosis alveolar pulmonar. El paciente reunió los criterios para tratamiento con lavado broncoalveolar total. Luego de este procedimiento, evolucionó favorablemente. Cómo conclusión la proteinosis alveolar pulmonar constituye una enfermedad importante a considerar, por el desafío diagnóstico y terapéutico que representa.

Se realizó un estudio descriptivo transversal para evaluar la utilidad de la videobroncoscopía en el diagnóstico de las enfermedades neumológicas prevalentes en pacientes del Hospital Regional Docente de Trujillo entre Junio del 2011 y Diciembre del 2012. Se sistematizaron los datos en la hoja de cálculo Excel 2010; se procedió al análisis estadístico de frecuencias mediante el paquete de análisis estadístico para ciencias sociales (SPSS) 20. De los 151 pacientes que ingresaron al estudio 87 (57.6%) fueron de sexo masculino y 64 (42.4%) del sexo femenino. La edad promedio fue de 50 años, con un rango de 16 a 92 años. Procedieron de consultorios externos 60 pacientes (39.7%), 79 pacientes (52.3%) de hospitalización del servicio de neumología, 12 pacientes (7.9%) de otros servicios. Los hallazgos broncoscópicos anormales se evidenciaron en los bronquios y parénquima pulmonar de 150 pacientes (99.3 %) y en la laringe de 01 paciente (0.7 %). Se informaron hallazgos broncoscópicos los cuales correspondieron 99 (65.6%) a signos inflamatorios agudos (SIA); 34 (22.5%) a signos indirectos de neoplasia (ST); 24 (15.9%) a signos inflamatorios crónicos (SIC); 17 (11.3%) a ectasia bronquial (BQ); 12 (7.9 %) a signos de sangrado (SGD); 10 (6.6%) a broncorrea (B); 6 (4.0%) a antracosis 1 (0.7%)y a granuloma laríngeo 1 (0.7%). Se identificaron gérmenes comunes en 11 pacientes (7.3%). A 104 pacientes se les solicitó baciloscopías y cultivos de BK en los cuales se identificó al Mycobacterium tuberculosis en 40 pacientes (26,5%). De los 151 pacientes a 104 se les solicitó estudio citológico de Papanicolaou (PAP), en 11 (7.3%) se evidenciaron células sospechosas de neoplasia (CSN). A 29 pacientes se realizaron toma de biopsias para estudio de anatomía patológica; los resultados anatomopatológicos más frecuentes fueron: bronquitis más inflamación crónica 11 (7.3%), bronquitis más inflamación aguda 05 (3.3%), adenocarcinoma bronquial 5 (3.3%). Los resultados corroboran la utilidad de la videobroncoscopía flexible en el Hospital Regional Docente de Trujillo, siendo su mayor aporte el diagnóstico temprano de enfermedades infecciosas como la tuberculosis posibilitando su tratamiento también precoz con los importantes beneficios epidemiológicos.Palabras clave: videobroncoscopía, bronquitis, signos inflamatoriosABSTRACTA cross-sectional study was performed to evaluate the usefulness of videobronchoscopy in diagnosing pneumological diseases on patients of the “Hospital Regional Docente de Trujillo” from June 2011 to December 2012. Data was organized in Excel 2010 spreadsheets. Frequency statistical analysis was performed using the Statistical Package for the Social Sciences ver. 20 (SPSS 20). Out of 151 patients, 87 (57.6%) were male and 64 (42.4%) were females. Average age of the patients was 50 years, ranging from 16 to 92 years. There were 60 (39.7%) outpatients, 79 (52.3%) in-patients from the Pulmonology Department and 12 (7.9%) patients from other services. Abnormal bronchoscopic findings were demonstrated in the bronchial and pulmonary parenchyma of 150 patients (99.3%) and in one (0.7%) patient's larynx. Bronchoscopic findings were attributed to the following causes: 99 (65.6%) of them to acute inflammatory signs (SIA), 34 (22.5%) to indirect signs of neoplasia (ST), 24 (15.9%) to chronic inflammatory signs (SIC), 17 (11.3%) to bronchial ectasia (BQ), 12 (7.9%) to bleeding (SGD) 10 (6.6%) to bronchorrhea (B), 6 (4.0%) to anthracosis 01 (0.7%) to laryngeal granuloma 01 (0.7%). Common germs were identified in 11 patients (7.3%). 104 patients were requested BK smears and cultures in which mycobacterium tuberculosis was identified in 40 patients (26.5%). Out of 151 admitted patients, 104 were requested a cytology Papanicolaou (PAP) and 11 (7.3%) showed suspicious cells of neoplasia (CSN). Biopsies were taken in 29 patients for pathology study, the most frequent pathological findings were: bronchitis with chronic inflammation 11 (7.3%), bronchitis with acute inflammation 05 (3.3%), bronchial adenocarcinoma 5 (3.3%).
