Urease-powered nanobots significantly enhanced the efficacy of radionuclide therapy in bladder cancer treatment, showing superior tumor accumulation and penetration. This novel approach promises transformative advancements in managing non-muscle-invasive bladder cancer, as detailed in a recent study.

Sexually transmitted infections and urogenital-perinatal infections are significant health challenges owing to their asymptomatic nature, multidrug-resistant pathogens, and lack of effective vaccines. Surfactants are under investigation as potential antimicrobial agents and alternatives to traditional antibiotics. Here, we discovered that N-dodecylpyridinium bromide (C12PB), a cationic quaternary ammonium surfactant, has very low potential to induce antimicrobial resistance with no antibiotic cross-resistance or inflammation in vitro. Therefore, we developed a preclinical antibiotic-free cationic surfactant-based cellulose hydrogel for treating sexually transmitted infections. The C12PB-hydrogels provided sustained surfactant release, enhancing their biocompatibility and antibacterial activity without inflammation or epithelial disruption of the vaginal tract. In a preclinical model of Neisseria gonorrhoeaeinfection, a single application of the C12PB-hydrogel showed a 2- to 3-fold reduction in infection. This lays the foundation for the future development of C12PB-hydrogels for sexually transmitted infections, demonstrating potent antibacterial activity and minimal risk of antimicrobial resistance or inflammation.

Photodynamic therapy (PDT), an emergent noninvasive cancer treatment, is largely dependent on the presence of efficient photosensitizers (PSs) and a sufficient oxygen supply. However, the therapeutic efficacy of PSs is greatly compromised by poor solubility, aggregation tendency, and oxygen depletion within solid tumors during PDT in hypoxic microenvironments. Despite the potential of PS-based metal–organic frameworks (MOFs), addressing hypoxia remains challenging. Boron dipyrromethene (BODIPY) chromophores, with excellent photostability, have exhibited great potential in PDT and bioimaging. However, their practical application suffers from limited chemical stability under harsh MOF synthesis conditions. Herein, we report the synthesis of the first example of a Zr-based MOF, namely, 69-L2, exclusively constructed from the BODIPY-derived ligands via a single-crystal to single-crystal post-synthetic exchange, where a direct solvothermal method is not applicable. To increase the PDT performance in hypoxia, we modify 69-L2with fluorinated phosphate-functionalized methoxy poly(ethylene glycol). The resulting 69-L2@F is an oxygen carrier, enabling tumor oxygenation and simultaneously acting as a PS for reactive oxygen species (ROS) generation under LED irradiation. We demonstrate that 69-L2@F has an enhanced PDT effect in triple-negative breast cancer MDA-MB-231 cells under both normoxia and hypoxia. Following positive results, we evaluated the in vivoactivity of 69-L2@F with a hydrogel, enabling local therapy in a triple-negative breast cancer mice model and achieving exceptional antitumor efficacy in only 2 days. We envision BODIPY-based Zr-MOFs to provide a solution for hypoxia relief and maximize efficacy during in vivoPDT, offering new insights into the design of promising MOF-based PSs for hypoxic tumors.

Engineered liposomal nanoparticles have unique characteristics as cargo carriers in cancer care and therapeutics. Liposomal theranostics have shown significant progress in preclinical and clinical cancer models in the past few years. Liposomal hybrid systems have not only been approved by the FDA but have also reached the market level. Nanosized liposomes are clinically proven systems for delivering multiple therapeutic as well as imaging agents to the target sites in (i) cancer theranostics of solid tumors, (ii) image-guided therapeutics, and (iii) combination therapeutic applications. The choice of diagnostics and therapeutics can intervene in the theranostics property of the engineered system. However, integrating imaging and therapeutics probes within lipid self-assembly “liposome” may compromise their overall theranostics performance. On the other hand, liposomal systems suffer from their fragile nature, site-selective tumor targeting, specific biodistribution and premature leakage of loaded cargo molecules before reaching the target site. Various engineering approaches, viz., grafting, conjugation, encapsulations, etc., have been investigated to overcome the aforementioned issues. It has been studied that surface-engineered liposomes demonstrate better tumor selectivity and improved therapeutic activity and retention in cells/or solid tumors. It should be noted that several other parameters like reproducibility, stability, smooth circulation, toxicity of vital organs, patient compliance, etc. must be addressed before using liposomal theranostics agents in solid tumors or clinical models. Herein, we have reviewed the importance and challenges of liposomal medicines in targeted cancer theranostics with their preclinical and clinical progress and a translational overview.

Since the early 1990s, the intersection of genetics and nanomedicine has found a home in the clinic as one of the game changers of the past decade, holding great promise in fighting diseases by rapidly developing much-needed therapeutic platforms, from cancer to infectious or genetic diseases. And this revolution was just triggered by the amazing evolving world of messenger RNA and its ‘cues’.

Bio-mimicking principles have recently been proposed for the surface functionalization of nanoparticles (NPs). Such a strategy is based on camouflaging the NP surface with functional biomembranes to render superior biocompatibility, interfacial features, immune evasion, and active targeting properties to nanomaterials. In this area of research, cell membranes derived from a plethora of highly optimized cells, such as red blood cells, immune cells, platelets, stem cells, cancer cells, and others, have been the pioneers as coating materials. This biomimetic concept has then been applied to subcellular structures, namely extracellular vesicles and intracellular organelles. Exosomes are a nanosized extracellular vesicle subtype secreted by most cells. These phospholipid bilayer nanovesicles are surface enriched with proteins accounting for their dynamic and prominent roles in immune escape, cell-cell communication, and specific cell uptake. Their intrinsic stability, biocompatibility, reduced immunogenicity and toxicity, and specific cell-targeting features denote an optimal biological nanocarrier for biomedical applications. This review highlights the current clinical applications of exosome membrane-coated nanosystems in cancer diagnosis and therapy. These biomimetic nanosystems have emerged as a promising avenue to provide effective, highly specific, and safer cancer-targeted applications. Finally, challenges hindering their clinical application will be mentioned.
