SUMMARYAims– To highlight the major discrepancies that emerged between evidence and routine practice in the framework of the SIEP-DIRECT's Project (DIscrepancy betweenRoutine practice andEvidence in psychiatricCommunityTreatments onSchizophrenia). The Project was conducted in 19 Italian mental health services (MHS), with the aims of: a) evaluating the appropriateness of the NICE Guidelines for Schizophrenia in the Italian context, b) developing and testing a set of 103 indicators that operationalised preferred clinical practice requirements according to the NICE Guidelines, and c) evaluating their actual application in Italian MHSs.Methods– The indicators investigated five different areas: common elements in all phases of schizophrenia; first episode treatment; crisis treatment; promoting recovery; the aggressive behaviour management.Results– The NICE recommendations examined were judged in most instances to be appropriate to the Italian MHS context, and the indicators fairly easy to use. The more severe and frequently encountered evidence-practice discrepancies were: lack of written material, guidelines, and information to be systematically provided to users; lack of intervention monitoring and evaluation; difficulty in implementingspecific and structured forms of intervention; difficulty in considering patients' family members as figures requiring targeted support themselves and who should also be regularly involved in the patient care process.Conclusions– The key actions to be undertaken to favour implementation of evidence-based routine practices are: focussing on mental illness onset and family support/involvement in care; planning training activities aimed at achieving specific treatment goals; encouraging MHS participation in evaluation activities; identifying thresholds for guideline application and promoting specific guideline implementation actions; and activating decision making and resource allocationprocesses that rely more strictly on evidence and epidemiological assessment. These considerations are of value for rethinking the model of community psychiatry in Italy as well as in other countries.Declaration of Interest: None.

Classical dynamins (DNMs) are GTPase proteins engaged in endocytosis, a fundamental process for cargo internalization from the plasma membrane. In mammals, three DNM genes are present with different expression patterns. DNM1 is expressed at high levels in neurons, where it takes place in the recycling of synaptic vesicles; DNM2 is ubiquitously expressed, while DNM3 is found in the brain and in the testis. Due to the conservation of genes in comparison to mammals, we took advantage of a zebrafish model for functional characterization of dnm1a, ortholog of mammalian DNM1. Our data strongly demonstrated that dnm1a has a nervous tissue-specific expression pattern and plays a role in the formation of both axon and synapse. This is the first in vivo study that collects evidence about the effects of dnm1a loss of function in zebrafish, thus providing a new excellent model to be used in different scientific fields.

Summary Cancer-initiating cells (CICs) have been implicated in tumor development and aggressiveness. In ovarian carcinoma (OC), CICs drive tumor formation, dissemination, and recurrence, as well as drug resistance, thus accounting for the high death-to-incidence ratio of this neoplasm. However, the molecular mechanisms that underlie such a pathogenic role of ovarian CICs (OCICs) remain elusive. Here, we have capitalized on primary cells either from OC or from its tissues of origin to obtain the transcriptomic profile associated with OCICs. Among the genes differentially expressed in OCICs, we focused on CD73, which encodes the membrane-associated 5′-ectonucleotidase. The genetic inactivation of CD73 in OC cells revealed that this molecule is causally involved in sphere formation and tumor initiation, thus emerging as a driver of OCIC function. Furthermore, functional inhibition of CD73 via either a chemical compound or a neutralizing antibody reduced sphere formation and tumorigenesis, highlighting the druggability of CD73 in the context of OCIC-directed therapies. The biological function of CD73 in OCICs required its enzymatic activity and involved adenosine signaling. Mechanistically, CD73 promotes the expression of stemness and epithelial-mesenchymal transition-associated genes, implying a regulation of OCIC function at the transcriptional level. CD73, therefore, is involved in OCIC biology and may represent a therapeutic target for innovative treatments aimed at OC eradication., Graphical Abstract, Highlights • CD73 is enriched in ovarian cancer-initiating cells (OCICs) • CD73 orchestrates OCIC stemness and EMT • OC initiation and growth require CD73 activity • OCIC-associated CD73 is a therapeutic target useful for OC eradication, Cavallaro et al. characterized the transcriptome of OCIC-enriched primary cultures and found CD73 as an upregulated gene. CD73 was then shown to regulate the expression of stemness and EMT-associated genes. The expression and function of CD73 in OCICs is required for tumor initiation, and CD73-targeted drugs decrease the rate of tumor take and inhibit cancer growth.

Summary Adaptor protein 2 (AP2) is a major constituent of clathrin-coated pits (CCPs). Whether it is essential for all forms of clathrin-mediated endocytosis (CME) in mammalian cells is an open issue. Here, we demonstrate, by live TIRF microscopy, the existence of a subclass of relatively short-lived CCPs lacking AP2 under physiological, unperturbed conditions. This subclass is retained in AP2-knockout cells and is able to support the internalization of epidermal growth factor receptor (EGFR) but not of transferrin receptor (TfR). The AP2-independent internalization mechanism relies on the endocytic adaptors eps15, eps15L1, and epsin1. The absence of AP2 impairs the recycling of the EGFR to the cell surface, thereby augmenting its degradation. Accordingly, under conditions of AP2 ablation, we detected dampening of EGFR-dependent AKT signaling and cell migration, arguing that distinct classes of CCPs could provide specialized functions in regulating EGFR recycling and signaling., Graphical Abstract, Highlights • Distinct classes of CCPs exist, molecularly defined by the presence or lack of AP2 • The AP2-negative CCPs support the internalization of EGFR but not of TfR • The AP2-negative CCPs rely on the endocytic adaptors eps15/eps15L1 and epsin1 • The two classes of CCPs determine distinct EGFR fates and signaling outputs, EGFR signaling controls different cell physiological processes, including proliferation and migration. Pascolutti et al. describe an additional layer of regulation of EGFR signaling, relying on the sequestration of receptors into molecularly distinct clathrin-coated vesicles that regulate receptor fate toward recycling versus degradation, with impact on the final cellular output.

The docking protein p140Cap negatively regulates tumour cell features. Its relevance on breast cancer patient survival, as well as its ability to counteract relevant cancer signalling pathways, are not fully understood. Here we report that in patients with ERBB2-amplified breast cancer, a p140Cap-positive status associates with a significantly lower probability of developing a distant event, and a clear difference in survival. p140Cap dampens ERBB2-positive tumour cell progression, impairing tumour onset and growth in the NeuT mouse model, and counteracting epithelial mesenchymal transition, resulting in decreased metastasis formation. One major mechanism is the ability of p140Cap to interfere with ERBB2-dependent activation of Rac GTPase-controlled circuitries. Our findings point to a specific role of p140Cap in curbing the aggressiveness of ERBB2-amplified breast cancers and suggest that, due to its ability to impinge on specific molecular pathways, p140Cap may represent a predictive biomarker of response to targeted anti-ERBB2 therapies., p140Cap adaptor proteins interfere with adhesion and growth factor-dependent signalling in cancer cells but the mechanisms are unclear. Here the authors show that p140Cap interferes with ERBB2-dependent activation of Rac GTPase-controlled circuitries reducing metastasis and cancer progression.

In ogni classe numerosa c'è un certo numero di cancres (scaldabanchi, asini, zucconi) che sono invariabilmente gli ultimi in ogni compito scolastico e che non profittano quasi mai di ciò che viene insegnato in classe. Essi dipendono supinamente da ciò che dice il maestro, come i mendicanti che d'inverno vanno a riscaldarsi al Louvre e rimangono insensibili alla bellezza dei quadri di Rembrandt. Nulla di più interessante è il conoscere la psicologia di questi cancres; bisogna esaminarli ad uno ad uno, sapere per quale ragione essi occupano una posizione tanto infima; se è per difetto d'intelligenza oppure di carattere e se la loro posizione può venir migliorata. Si tratta di un problema di grande importanza sociale perché bisogna preoccuparsi costantemente di diminuire il numero di questi casi, in modo che essi non diventino fatali. E ci si può chiedere già fin d'ora quanti siano gli insegnanti che abbiano fatto a tutt'oggi uno studio attento di questi cancres, che abbiano cercato di aiutarli e che si siano chiesti: se questi allievi non riescono negli studi forse l'errore è anche mio” (Alfred Binet: Les idées modernes sur les enfants, 1911).

INQUADRAMENTO TEORICO Nella teoria banduriana della condotta aggressiva assumono un ruolo centrale l’adesione a determinati principi morali contrari all’aggressività e il costrutto del moral disengagement, che ipotizza l’utilizzo di meccanismi di disimpegno. Tali meccanismi consentono di operare una scissione tra i valori professati e le azioni compiute (Bandura, 2002). La discussione in gruppo di dilemmi morali favorisce la consapevolezza dell’utilizzo di queste condotte autoassolutorie ed eleva le abilità di ragionamento morale: a un basso livello di moralità corrisponde un comportamento fortemente antisociale, mentre stadi superiori di pensiero morale sono una condizione necessaria, sebbene non sufficiente, per un comportamento morale coerente e prosociale (Rest, 1974; Lind, 2006). OBIETTIVI Un obiettivo della ricerca è stato quello di valutare le differenze nel livello di disimpegno morale in studenti iscritti a tre tipologie di scuola superiore (liceo scientifico, istituto tecnico e istituto professionale). E’ stata poi misurata l’efficacia di un intervento di educazione morale nel contrastare l’utilizzo dei meccanismi di disimpegno. METODO La presente ricerca è stata realizzata con la collaborazione di 6 scuole superiori statali: 2 licei, 2 istituti tecnici e 2 professionali (498 soggetti, 294 M e 204 F). Ogni scuola ha fornito 4 classi: 2 prime e 2 seconde, suddivise omogeneamente e casualmente tra gruppo sperimentale e di controllo. Delle 24 classi coinvolte, 12 (6 prime e 6 seconde) hanno funto da gruppo di controllo e altre 12 (6 prime e 6 seconde) da gruppo sperimentale. Gli strumenti utilizzati sono stati un questionario sui meccanismi di disimpegno morale e alcuni dilemmi morali. Il questionario di disimpegno morale (Caprara, Pastorelli e Bandura, 1995), autovalutativo e anonimo, è composto da item con risposta su scala Likert, e presenta buona attendibilità e coerenza interna (α di Cronbach = .805). Gli item misurano i meccanismi banduriani di disimpegno: giustificazione morale, etichettamento eufemistico, confronto vantaggioso, diffusione di responsabilità, dislocamento di responsabilità, distorsione delle conseguenze, deumanizzazione della vittima, attribuzione di colpa alla vittima. La prima somministrazione del questionario è avvenuta contemporaneamente nei due gruppi; a distanza di tre mesi, periodo durante il quale si è attuato il training di educazione morale con il gruppo sperimentale, è stata effettuata la seconda somministrazione, secondo le stesse modalità. I dilemmi utilizzati per l’intervento sono stati selezionati tra una serie di stimoli codificati in letteratura (Goldstein e Glick, 1990). Sono stati scelti dilemmi in grado di sollevare riflessioni, e di provocare dissenso e conflitto cognitivo, alla portata delle capacità intellettuali dei ragazzi, che implicassero diverse possibilità di scelta per soggetti potenzialmente a livelli diversi di sviluppo morale. RISULTATI Il pre-test indica che il gruppo di controllo e quello sperimentale sono omogenei per quanto riguarda il livello di disimpegno morale. Per gli studenti del liceo è stato rilevato il livello di disimpegno morale più basso; l’utilizzo dei meccanismi di disimpegno aumenta progressivamente negli istituti tecnici e professionali. Le differenze in base alla tipologia di scuola sono statisticamente significative per sette meccanismi su otto. Confrontando i dati del pre-test con quelli del post-test si è evidenziato un miglioramento statisticamente significativo per tre meccanismi: giustificazione morale (p
