Abstract

BackgroundInternet search engines are increasingly being utilized as the first port of call for medical information by the public. The prevalence of allergies in developed countries has risen steadily over time. There exists significant variability in the quality of health-related information available on the web. Inaccurately diagnosed and mismanaged allergic disease has major downstream effects on patients, general practitioners, and regional allergy services. ObjectiveThis study aimed to verify whether Ireland has a relatively high rate of web-based allergy-related searches, to establish the proportion of medically accurate web pages encountered by the public, and to compare current search results localized to Dublin, Ireland with urban centers elsewhere. MethodsGoogle Trends was used to evaluate regional interest of allergy-related search terms over a 10-year period using terms “allergy,” “allergy test,” “food allergy,” and “food intolerance.” These terms were then inputted into Google search, localizing them to cities in Ireland, the United Kingdom, and the United States. Output for each search was reviewed by two independent clinicians and deemed rational or nonevidence based, as per current best practice guidelines. Searches localized to Dublin were initially completed in 2015 and repeated in 2019 to assess for changes in the quality of search results over time. ResultsIreland has a persistently high demand for web-based information relating to allergy and ranks first worldwide for “allergy test,” second for “food allergy” and “food intolerance,” and seventh for “allergy” over the specified 10-year timeframe. Results for each of the four subsearches in Dublin (2015) showed that over 60% of websites promoted nonevidence-based diagnostics. A marginal improvement in scientifically robust information was seen in 2019, but results for “allergy test” and “food intolerance” continued to promote alternative testing 57% (8/14) of the time. This strongly contrasted with results localized to Southampton and Rochester, where academic and hospital-affiliated web pages predominantly featured. Government-funded Department of Health websites did not feature in the top five results for Dublin searches “allergy testing,” “food allergy,” or “food intolerance” in either 2015 or 2019. ConclusionsThe Irish public demonstrates a keen interest in seeking allergy-related information on the web. The proportion of evidence-based websites encountered by the Irish public is considerably lower than that encountered by patients in other urban centers. Factors contributing to this are the lack of a specialist register for allergy in Ireland, inadequate funding for allergy centers currently in operation, and insufficient promotion by the health service of their web-based health database, which contains useful patient-oriented information on allergy. Increased funding of clinical allergology services will more meaningfully impact the health of patients if there is a parallel investment by the health service in information and communication technology consultancy to amplify their presence on the web.

11. Positive Behaviour Support : development and evaluation of an online staff training protocol

Abstract

Positive behaviour support (PBS) is recognised as an evidenced based approach to addressing challenging behaviours exhibited by people with intellectual disabilities (IDD). The quality of staff performance is a crucial determinants of client outcomes but the comparative effectiveness of PBS training methods with regard to staff behaviour is rarely studied. The purpose of the current programme of research was to examine the effectiveness of behavioural training protocols in providing frontline care staff with the knowledge and skills required to deliver PBS. Firstly, a systematic review was conducted to expand on previous reviews of PBS training by including recent studies of PBS training and refining inclusion criteria to be more relevant to social care settings. Results indicated that training duration, skills practice and supervision were important determinants of positive training outcomes. The need for more comparision and control trials was also identified. Study 1 investigated the comparative effectiveness of behavioural skills training (BST) and workshop training on staff knowledge and competence, finding that there were no differences between groups in knowledge or competence at post-test. An analysis of interview data indicated that participants preferred BST and had a positive view of PBS. Participants believed that organisational factors, training individualisation and skills practice were important determinants of training success. In Study 2, a three-arm randomised controlled trial evaluated the comparative effectiveness of programmed instruction (PI), standard online instruction (SOI) and workshop training on participants attitudes, knowledge and application of knowledge related to PBS. It found that knowledge and application score increases were highest for those in the PI condition and lowest within the workshop condition. No changes to attitudes to challenging behaviour were observed following training. Study 3 utilised a within-subjects, pre-post-test design to evaluate the impact of PI training on the participants' attitudes to challenging behaviour, knowledge of PBS, burnout and intervention design abilities. Results indicated that PI based PBS training was effective in increasing staff knowledge. While burnout was observed to have decreased following training, no changes in attitudes to challenging behaviour or intervention design ability were observed. The findings of the thesis indicate that in addition to previously identified features of PBS training, training methodology impacts significantly on training outcomes. They also suggest that online PI-based PBS training is an effective training methodology. Evidence from the thesis' studies isuggest that PBS training can have positive impacts on outcomes with regard to PBS knowledge, performing behaviour analytic procedures, applying knowledge and staff burnout. Implications for future PBS training research and practice are discussed.

Abstract

Background: Chronic spontaneous urticaria (CSU) is a common, debilitating skin disorder characterized by recurring episodes of raised, itchy and sometimes painful wheals lasting longer than 6 weeks. CSU is mediated by mast cells which are absent from peripheral blood. However, lineage−CD34hiCD117int/hiFcεRI+ cells in blood have previously been shown to represent a mast cell precursor. Methods: We enumerated FcεRI−, FcεRI+ and FcεRIhi lineage−CD34+CD117+ cells using flow cytometry in blood of patients with CSU (n = 55), including 12 patients receiving omalizumab and 43 not receiving omalizumab (n = 43). Twenty‐two control samples were studied. Disease control and patient response to omalizumab was evaluated using the urticaria control test. We performed single‐cell RNA sequencing (scRNA‐Seq) on lineage−CD34hiCD117hi blood cells from a subset of patients with CSU (n = 8) and healthy controls (n = 4). Results: CSU patients had more lineage−CD34+CD117+FcεRI+ blood cells than controls. Lineage−CD34+CD117+FcεRI+ cells were significantly higher in patients with CSU who had an objective clinical response to omalizumab when compared to patients who had poor disease control 90 days after initiation of omalizumab. scRNA‐Seq revealed that lineage−CD34+CD117+FcεRI+ cells contained both lymphoid and myeloid progenitor lineages, with omalizumab responsive patients having proportionally more myeloid progenitors. The myeloid progenitor lineage contained small numbers of true mast cell precursors along with more immature FcεRI− and FcεRI+ myeloid progenitors. Conclusion: Increased blood CD34+CD117+FcεRI+ cells may reflect enhanced bone marrow egress in the setting of CSU. Abstract

Background: Concern about disease exacerbations and fear of reactions after coronavirus disease 2019 (COVID-19) vaccinations are common in chronic urticaria (CU) patients and may lead to vaccine hesitancy. Objective: We assessed the frequency and risk factors of CU exacerbation and adverse reactions in CU patients after COVID-19 vaccination. Methods: COVAC-CU is an international multicenter study of Urticaria Centers of Reference and Excellence (UCAREs) that retrospectively evaluated the effects of COVID-19 vaccination in CU patients aged ≥18 years and vaccinated with ≥1 dose of any COVID-19 vaccine. We evaluated CU exacerbations and severe allergic reactions as well as other adverse events associated with COVID-19 vaccinations and their association with various CU parameters. Results: Across 2769 COVID-19–vaccinated CU patients, most (90%) received at least 2 COVID-19 vaccine doses, and most patients received CU treatment and had well-controlled disease. The rate of COVID-19 vaccination–induced CU exacerbation was 9%. Of 223 patients with CU exacerbation after the first dose, 53.4% experienced recurrence of CU exacerbation after the second dose. CU exacerbation most often started <48 hours after vaccination (59.2%), lasted for a few weeks or less (70%), and was treated mainly with antihistamines (70.3%). Factors that increased the risk for COVID-19 vaccination–induced CU exacerbation included female sex, disease duration shorter than 24 months, having chronic spontaneous versus inducible urticaria, receipt of adenovirus viral vector vaccine, having nonsteroidal anti-inflammatory drug/aspirin intolerance, and having concerns about getting vaccinated; receiving omalizumab treatment and Latino/Hispanic ethnicity lowered the risk. First-dose vaccine–related adverse effects, most commonly local reactions, fever, fatigue, and muscle pain, were reported by 43.5% of CU patients. Seven patients reported severe allergic reactions.
