Despite the substantial literature and human factors guidance, evaluators report challenges in selecting cognitive workload measures for the evaluation of complex human–technology systems. A review of 32 articles found that self-report measures and secondary tasks were systematically sensitive to human–system interface conditions and correlated with physiological measures. Therefore, including a self-report measure of cognitive workload is recommended when evaluating human–system interfaces. Physiological measures were mainly used in method studies, and future research must demonstrate the utility of these measures for human–system evaluation in complex work settings. However, indexes of physiological measures showed promise for cognitive workload assessment. The review revealed a limited focus on the measurement of excessive cognitive workload, although this is a key topic in nuclear process control. To support human–system evaluation of adequate cognitive workload, future research on behavioural measures may be useful in the identification and analysis of underload and overload. This review provides background for the selection of cognitive workload measures for the evaluation of complex human–technology systems and identifies future research needs for applied cognitive workload assessment. This paper explores the complex and time-critical work practices within operational train traffic in Sweden by reporting on an incident causing an infrastructure breakdown and large traffic disruptions. Based on a workplace study approach, we report on how the control room workers – train traffic controllers and information officers – grasp, make sense of, and handle the consequences of the incident as it unfolds in real-time. We portray how the workers develop and acquire a sense of place in relation to the incident's severity which is essential for successfully handling the situation. By introducing the 'sense of place' concept originally derived from the field of natural resources to the domain of operational train traffic, we provide a deepened understanding of the challenges characterising remote control work from a safety-critical socio-technical systems perspective. In safety-critical automatic systems, safety can be compromised if operators lack engagement. Effective detection of undesirable engagement states can inform the design of interventions for enhancing engagement. However, the existing engagement measurement methods suffer from several limitations which damage their effectiveness in the work environment. A novel engagement evaluation methodology, which adopts Artificial Intelligence (AI) technologies, has been proposed. It was developed using motorway control room operators as subjects. Openpose and Open Source Computer Vision Library (OpenCV) were used to estimate the body postures of operators, then a Support Vector Machine (SVM) was utilised to build the engagement evaluation model based on discrete states of operator engagement. The average accuracy of the evaluation results reached 0.89 and the weighted average precision, recall, and F1-score were all above 0.84. This study emphasises the importance of specific data labelling when measuring typical engagement states, forming the basis for potential control room improvements. Practitioner summary: This study demonstrates an automatic, real-time, objective, and relatively unobtrusive method for measuring dynamic operator engagement states. Computer vision technologies were used to estimate body posture, then machine learning (ML) was utilised to build the engagement evaluation model. Process control room operators (PCRO) perform a range of complex cognitive safety-critical tasks. The aim of this exploratory sequential mixed methods study was to develop an occupation specific tool to measure the task load of PCRO using NASA Task Load Index (TLX) methodology. Participants were 30 human factors experts and 146 PCRO at two refinery complexes in Iran. Dimensions were developed via a cognitive task analysis, a research review, and three expert panels. Six dimensions were identified: perceptual demand, performance, mental demand, time pressure, effort, and stress. Data from 120 PCRO confirmed that the developed PCRO-TLX has acceptable psychometric properties, and a comparison with the NASA-TLX confirmed that perceptual, not physical, demand was relevant for measuring workload in PCRO. There was a positive convergence of scores of the Subjective Workload Assessment Technique and the PCRO-TLX. This reliable tool (α = 0.83) is recommended for risk assessing the task load of PCRO. Practitioner summary: There are benefits of having a specific tool to measure task load in safety critical roles. Thus, we developed and validated an easy-to-use targeted tool, the PCRO-TLX, for process control room operatives. The analysis for estimating operator's activity and quantity of information in control rooms of underground coal mines is presented in this paper. Monitoring of operators in control rooms was conducted in mines Rembas, Soko, Ibar, Jasenovac and Stavalj. The network models are used during the analysis of operator's activity in control rooms. The result of the analysis is proposed critical path network model. The application of network models improves the quality and level of research, making it more efficient. Most of the analysed information are related to normal and alarm state data received from the mine shaft and operator's response to this data. Results are given according to actual scope of work depending on the work dynamics in underground coal mines, via histogram of changes in average number of activities per 1 h for 24-h period, then number of operator's activities per work hour and insight into the global view of technological process in the coal mine with focus on the work scope of mine workers. Analysis of operator's activities presented in this paper determines the relations between devices in control rooms and operators' capabilities in order to get better ergonomic design of control rooms. A new methodology using a free turbulent flow model to evaluate control room habitability is developed, and the theoretical model can be applied to the postulated event of rupture or line break of the on-site hazardous gas pressurized tank/system. Based on the conservation of mass law and momentum equations, correlations of the control room ventilation hazardous gas intake concentration and the control room buildup toxic concentration were established and can be used to evaluate control room habitability. Compared with current methodology widely used in the industry (introduced by NUREG-0570), the developed theoretical analysis methodology is applicable to events occurring without any constraint on the distance between the site of toxic gas release and the inlet of the control room fresh air intake or the control room. With a given amount of hazardous gas release source, the analysis results indicate that maximum control room toxic gas concentration will depend on the mass release rate or its break size, the density of the hazardous gas, and the distance between the site of the toxic release and the control room fresh air intake. The limiting case of the control room habitability analysis will occur at the break size resulting in the highest control room toxic gas concentration. The control room toxic gas transient concentration at the limiting break size can be predicted by the model and compared with its acceptance criteria of short-term exposure limit and time-weighted average to evaluate the control room habitability whether protection actions of the control room operators are required to prevent incapacitation or death due to the postulated events of toxic gas release. Context: Though many arrangements had been made during the first wave, multiple complaints were reported regarding the arrangements made for COVID patients. The control room was set up as a 24 × 7 administrative coordination center to attend to issues after the initial setup of COVID units. Aim: To identify the major problems reported to the control room during the second wave of COVID-19 and find the related cause for the same. Methods and Materials: Issues reported to the control room were recorded from April to June (2021). The control room residents were asked to allocate each problem into six categories. Next, the residents allotted a frequency score to each of the problems depending on how regularly it was reported. Secondary data from various reports were also considered. Statistical Analysis Used: Data were analyzed using MS Excel software. Results: Various administrative issues were identified to be due to issues in communication and coordination (23%), policy and procedures (22%), human resource and training (16%), administration and implementation (15%), infrastructure (14%), and HMIS and IT (10%). Conclusion: Most frequent problems reported to the control room were related to infrastructure, policy implementation, and manpower training. After the initial implementation of new processes, there was a requirement for regular follow-up and coordination. This model of setting up an administrative control room and evidence-based approach will serve as a template for public hospitals to ensure coordination of hospital operations. As a complex socio-technical system, the Control Room (CR) is central to many industries, such as process, aviation, transportation, and mining. CRs' complexity impacts the safety, operational, engineering, regulatory, and financial performance of the system. In this study, a search strategy was defined and applied to three reputable databases: Scopus, Web of Science, and PubMed. The search results underwent a two-stage screening process based on inclusion and exclusion criteria. The inclusion criteria were investigating the safety in the CR, full-text availability, and writing in English. The exclusion criteria were unrelated to the safety in the CR, non-English Language, and non-original studies. A total of 59 studies were included in the analysis. The included articles were systematically reviewed from the Human Reliability Assessment (HRA) and Human Factors Engineering (HFE) perspectives. Since these records covered various subjects related to human reliability and human factor aspects, a categorization step was added to the study process. In this step, all included studies were categorized based on their subjects. A total of 7 categories were identified, including reliability (17 documents), safety performance (13 documents), decision-support systems (7 documents), fire safety (2 documents), communication/teamwork (11 documents), situation awareness (4 documents), and others (5 documents). Finally, the included studies in each category were analyzed and discussed. The results of this study help identify different aspects of safety in the control room and plan to improve their safety weaknesses, which ultimately leads to an increase in the efficiency of employees and various industries.

The Advanced Reactor Control and Operations (ARCO) facility was constructed in January 2018 to serve as a test bed for advanced reactor control rooms and operator support systems. Since then, it has supported human-machine interface user experience research, fault detection and mitigation technology development, control room concept of operations development, and remote operations research. ARCO serves as the control room for the Compact Integral Effects Test (CIET) facility, which replicates the primary-side flow paths and thermal-hydraulic behavior of a fluoride-salt-cooled high-temperature reactor (FHR) using simulant fluids and scaling principles. New reactor designs feature different operating conditions and scenarios than those in existing reactors. ARCO supports the research and development of digital tools for operator communications, intuitive real-time data analysis, online health monitoring and prognostics, and control room cybersecurity. By integrating these different technologies, ARCO acts as a prototypical control system to iteratively develop methods and tools of operation in advanced small modular nuclear reactors. Mitsubishi Heavy Industries, Ltd. has developed a computer-aided decision-making supporting panel system called DMP to support decision making of management staff in nuclear power plants by presenting associated information (e.g., plant situation and on-site/off-site conditions) in an organized and timely manner during emergency conditions. DMP will be located on-site as well as at an office off-site and will present associated information for staff and managers to share information and promote communications. DMP has been developed considering lessons learned from the Fukushima Daiichi accident including situation awareness distraction and workload issues under severe accidents. It was recognized that it is difficult to collect, organize, and analyze plant data and associated information under high-stress conditions and a severely degraded work environment. DMP will collect plant status information using plant chronological data logs and associated information, such as site situations (e.g., various kinds of information showing damaged equipment and hazard area, etc.), verbal/written status reports, and communication logs among staffs who perform corresponding actions at the site. DMP has been developed using a human-centered design approach based on International and Japanese human factors engineering design guidance (i.e., IEC 60964, "Nuclear Power Plants -- Control Rooms -- Design," and JEAG 4617, "Guideline for Development and Design of Computerized Human-Machine Interface in the Central Control Room") and verified with human factors verification (task support verification) to ensure the DMP support decision-making process. The Systems Usability Case (SUC) approach enables a requirement-based human factors (HF) evaluation of complex technical systems that may cover the entire verification and validation process. SUC is based on the Safety Case approach and on the Systems Usability (SU) construct. One of the main aims of establishing a Safety Case is to bring the arguments and evidence for safety to the front in such a way that the reasoning supports the work of a regulator or licensing organization. In the end, the approach enables evaluating the SU of a system and making a reasonable solid argument about the acceptance of the system for use. The question is how the conclusions are reached through a reasoning process in which the arguments are made about the evidence [i.e., identified human engineering discrepancies (HEDs)] to approve or reject the claim concerning the quality of the system. The paper presents an application of SUC to real data from an integrated system validation of the modernized control room (CR) of the Loviisa nuclear power plant. The results of the validation are discussed from the point of view of how the SUC approach enables forming a statement about the acceptance of the CR. Moreover, practical examples are given to demonstrate the identified HF issues and how they were handled in the validation process. The paper provides a general framework for handling of HEDs and for their resolution that can be used in the consolidation of validation test results in a real-world validation project. Nuclear power plant (NPP) control room operators must make ongoing computations and decisions that maximize production and ensure safety, which places a high cognitive burden on the operators. How cognitions such as attention, visuospatial ability, and working memory interact with socio-technical systems to achieve optimal operations is well studied. However, there is an absence of research that examines how cognitive functioning within the NPP control room environment is moderated by developmental aging processes. This is of critical importance because different types of cognitive actions are known to develop and peak at different times across the adult life span, and it is becoming increasingly clear that there is no age at which all cognitive faculties operate at maximum capacity. Thus, given that NPPs are experiencing an aging workforce, it is vital to identify how mission-critical cognitions change with age. This paper reviews implications of aging on reactor operators in the current and new fleet. We highlight lessons that can be learned from state-of-the-art human factors research that considers aging, lessons from the large cognitive aging literature, and lessons from aging workers in other industries that use sophisticated socio-technical systems, such as aviation. We also consider the important subject of aging effects versus expertise and present preliminary data that support the premise that age of operator is linked to effective and efficient operations but that this relationship may be moderated by level of operations expertise. Alarm floods pose a challenge to the successful operation of transmission and distribution systems because the alarm rate is often higher than what operators can effectively manage. Such a high volume of alarms can distract operators from unrelated but relevant alarms that occurred during the flood. To mitigate this situation, we propose a method to group alarm floods into clusters and identify additional alarms that might be lost in the flood and ignored by the operator. To do so, three different clustering approaches are introduced and tested using historical alarm data from a transmission system operator (TSO). Importantly, the results are validated by the end user highlighting the applicability of the proposed alarm flood management tool in real world scenarios. • Identify and analyze alarm floods from a real alarm dataset from a control room. • Propose and compare three different approaches for clustering alarm floods in power systems. • Introduce an outlier detection method to identify alarms not belonging to floods. • Present a user study to evaluate the impact of the proposed alarm flood management tool. Introduction: In many workplaces today, the incidence of human error can lead to catastrophic accidents in which human error is the main cause of accidents. Due to the vital role of the control room in guiding and controlling various sites of the pipe industry, especially the outer coating sector, the incidence of any error can lead to human accidents, damage to machinery, and interruption in production. This study aimed to identify and evaluate human error by Human Error Calculator (HEC) method in the epoxy control room of a pipe mill company. Materials and Methods: In the present descriptive cross-sectional study, the HEC method was used to identify and evaluate human errors. The HEC technique is provided by Risk Map Company, in which the probability of human error is based on five factors affecting the occurrence of human error, including a degree of urgency, complexity, importance, degree of individual skill, and task repetition, using a disk-shaped tool called Risk Disk is determined through direct observation, available instructions and interview with the head of the mentioned unit. Results: According to the results of this study, out of 11 identified tasks, five job tasks with a risk number of 70% have a high probability of human error, four job tasks with a risk number of 50%, and one job task with a number There is a 40% risk of moderate human error, And a job task with a 20% risk number has an increased chance of human error. Conclusion: The results of the present study showed that the HEC method is easy to use and is a simple and useful tool for professionals to calculate the probability of human error. In addition, HEC is a practical, effective and beneficial method for managers to reduce human error.

This paper suggests a methodology (conception and principles) for building two-mode monitoring systems (SMs) for industrial facilities and their adjacent territories based on the application of unmanned aerial vehicle (UAV), Internet of Things (IoT), and digital twin (DT) technologies, and a set of SM reliability models considering the parameters of the channels and components. The concept of building a reliable and resilient SM is proposed. For this purpose, the von Neumann paradigm for the synthesis of reliable systems from unreliable components is developed. For complex SMs of industrial facilities, the concept covers the application of various types of redundancy (structural, version, time, and space) for basic components—sensors, means of communication, processing, and presentation—in the form of DTs for decision support systems. The research results include: the methodology for the building and general structures of UAV-, IoT-, and DT-based SMs in industrial facilities as multi-level systems; reliability models for SMs considering the applied technologies and operation modes (normal and emergency); and industrial cases of SMs for manufacture and nuclear power plants. The results obtained are the basis for further development of the theory and for practical applications of SMs in industrial facilities within the framework of the implementation and improvement of Industry 4.0 principles. The German manufacturing industry has been carrying out new developments towards the next industrial revolution, focusing on smart manufacturing environments. Our work emphasizes human-centered control rooms in the context of production plants. Increased automation does not have to come with less human control. Therefore, we report on multimodal collaborative interaction techniques to augment industrial control rooms. In particular, we include mobile workers who use the control room while being in the production hall using tablets or specifically mixed reality glasses. Collaborative annotation dashboards support discussions and a shared understanding among analysts. Manufacturing-related data can be integrated into business analytics environments so that holistic analyses can be performed. Multimodal interaction techniques can support effective interaction with the control room based on the users' preferences. Immersive experience through mixed reality-based three-dimensional visualizations and interaction possibilities support users in obtaining a clear understanding of the underlying data. Due to the increasing size of electric power systems and its monitoring and operation needs, people at control rooms face complex situations, analyzing great amounts of information, protocols, operation states, alarms, and control parameters. Additionally, new renewable energy sources data is taking its place nowadays, hindering the process of quickly reading and interpreting information. This is known as the loss of Situational Awareness, SA, of system operators. This work presents proposals for displaying useful information in control rooms and visualization techniques to support accurate SA. Grid data displays, maps, trending graphics, lists, bars graphics, table plots, hierarchical levels, proximity regions, and connections are also proposed and analyzed, presenting an application case in a microgrid control room.

Fire protection is one of important issues to ensure safety and reduce risks of nuclear power plants (NPPs). While robust programs to shut down commercial reactors in any fires have been successfully maintained, the concept and associated regulatory requirements are constantly changing or strengthening by lessons learned from operating experiences and information all over the world. As part of this context, it is necessary not only to establish specific fire hazard assessment methods reflecting the characteristics of research reactors and educational reactors but also to make decisions based on advancement encompassing numerical analyses and experiments. The objectives of this study are to address fire simulation in the control room of an educational reactor and to discuss integrity of digital console in charge of main operation as well as analysis results through comparison. Three electrical fire scenarios were postulated and twenty-four thermal analyses were carried out taking into account two turbulence models, two cable materials and two ventilation conditions. Twelve supplementary thermal analyses and six subsequent structural analyses were also conducted for further examination on the temperature and heat flux of cable and von Mises stress of digital console, respectively. As consequences, effects of each parameter were quantified in detail and future applicability was briefly discussed. On the whole, higher profiles were obtained when Deardorff turbulence model was employed or polyvinyl chloride material and larger ventilation condition were considered. All the maximum values considered in this study met the allowable criteria so that safety action seems available by sustained integrity of the cable linked to digital console within operators’ reaction time of 300 s.

This study analyzed and explored the cognitive load of Australian energy market operators managing one of the longest inter-connected electrical networks in the world. Each operator uses a workstation with seven screens in an active control room environment, with a large coordination screen to show information and enable collaboration between different control centers. Cognitive load was assessed during both training scenarios and regular control room operations via the integration of subjective and physiological measures. Eye-tracking glasses were also used to analyze the operators gaze behavior. Our results indicate that different events (normal or unexpected), different participants for the same session, and different periods of one session all have varying degrees of cognitive load. The system design was observed to be inefficient in some situations and to have an adverse affect on cognitive load. In critical situations for instance, operator collaboration was high and the coordination screen was used heavily when collaborating between two control centers, yet integration with the system could be improved. Eye tracking data analysis showed that the layout of applications across the seven screens was not optimal for many tasks. Improved layout strategies, potential combination of applications, redesigning of certain applications, and linked views are all recommended for further exploration in addition to improved integration of procedures and linking alarms to visual cues. Following the occurrence of human errors in industry control rooms, catastrophic events with a high socio-economic burden may occur. The present study was aimed to investigate the probability of human error in Iranian industrial control rooms. In this review study, related articles were thoroughly reviewed in the Google Scholar, Scopus, Embase, PubMed, Web of Science, Magiran, SID, and IranDoc databases from 2010 to 2021. The used search terms were "human error", "Human failure", "control room", "industry", "human error assessment", "safety management", "Error analysis” and "human error probability". In order to extract the required data, all parts of the articles have been reviewed. To evaluate the quality of the reviewed articles, the JBI checklist was used. Out of 412 studies identified through systematic search, 22 articles were qualitative for analysis of which 14 articles (63.6%) are related to process industries and 8 articles (36.4%) are related to other industries. Among the 10 techniques used in the articles, CREAM and SHERPA techniques were the most used. In the process industries, using the CREAM method, execution failure (31.72%-55%), and interpretation failure (18.57%-29.20%), and using the SHERPA method, action errors (48.62%-67.64%) and checking errors (11.61%-31.97%) were the main types of errors in the control rooms. As well as, in other industries, using the SHERPA method, action errors (38.08%-58.80%) and checking errors (29.40%-39.04%), and using the Human HAZOP method, delete errors and performance errors were the main types of errors in the control rooms. The results of studies show that human error has a significant share in the occurrence of accidents in control rooms, but in Iran in some industries such as rail and nuclear has been less attention. Submarine control room layouts have remained similar across decades of operation, despite the introduction of new technologies that allow for the co-location of the sound and control room. Operation of an inwards, rather than outwards, facing ring control room configuration was examined in three scenarios: Return to Periscope Depth, Inshore Operations, and Dived Tracking. A case study design employed a serving team of qualified submariners participated in all three scenarios with high and low demand. Communications and activities in the control room were recorded and analysed using the Event Analysis of Systemic Teamwork (EAST) method. EAST models collaborative teamwork through social, information, and task networks. The results from the ring configuration were compared to an outward facing baseline of the contemporary control room layout with a separate sound and control room. The ring control room configuration increased communications between operators, leading to a more information exchanged, and more tasks completed. Practitioner summary: Control room design on submarines, and other domains, has traditionally been outward facing with supervisory staff looking over the shoulders of their subordinates.
