The COVID-19 pandemic brought to the surface the critical qualities of air – airflow, ventilation, particulates etc. – in relation to the well-being of people living in high-rise. Engineering and architectural research has burgeoned in response. However, in focusing on models of airflow, ventilation and particulates as discrete variables, engineering and architectural discourse fails to capture the diverse ways that air enters into and shapes the everyday lived experience of high-rise dwelling. Drawing on research in Sydney's Southwestern suburbs, we reveal high-rise as an assemblage that links apartment dwelling with air, via car-dependent suburbanisation, pollution and climate-change induced temperature extremes in the Anthropocene. In addition to viruses, air teems with carbon, insects, noise and pollutants, while viscerally mediating human encounters with fluctuating temperatures. Multiple relations between people, buildings and air unfold simultaneously, confounding attempts to account for air's elemental milieu through single variables like greenhouse gas or COVID-19. While embedded in relations of power, high-rise assemblages unleash vernacular adaptation that through low-tech and low-cost technologies work to connect suburban high-rise with evolving ecological systems. Recognising the pharmacological quality of air, as both 'poison' and 'cure', we contrast models of high-rise as encapsulated environments with resident (and other) experiments that orient high-rise to the elements, and the interlinked challenges of urban living in the Anthropocene. OBJECTIVES: Limited research has assessed how virtual care (VC) affects cardiovascular disease (CVD) risk management, especially in community clinic settings. This study assessed change in community clinic patients' CVD risk management during the COVID-19 pandemic and CVD risk factor control among patients who had primarily in-person or primarily VC visits. STUDY DESIGN: Retrospective interrupted time-series analysis. METHODS: Data came from an electronic health record shared by 52 community clinics for index (March 1, 2019, to February 29, 2020) and follow-up (July 1, 2020, to February 28, 2022) periods. Analyses compared follow-up period changes in slope and level of population monthly means of 10-year reversible CVD risk score, blood pressure (BP), and hemoglobin A1c (HbA1c) among patients whose completed follow-up period visits were primarily in person vs primarily VC. Propensity score weighting minimized confounding. RESULTS: There were 10,028 in-person and 6593 VC patients in CVD risk analyses, 9874 in-person and 5390 VC patients in BP analyses, and 8221 in-person and 4937 VC patients in HbA1c analyses. The VC group was more commonly younger, female, White, and urban. Mean reversible CVD risk, mean systolic BP, and percentage of BP measurements that were 140/90 mm Hg or higher increased significantly from index to follow-up periods in both groups. Rate of change between these periods was the same for all outcomes in both groups, regardless of care modality. CONCLUSIONS: Among community clinic patients with CVD risk, receiving a majority of care in person vs a majority of care via VC was not significantly associated with longitudinal trends in reversible CVD risk score or key CVD risk factors. Age-Friendly Health Systems (AFHS) commit to providing evidence-based, low-risk, coordinated care centered on what matters most to older adults, their families, and caregivers. Nova Southeastern University's South Florida Geriatric Workforce Enhancement Program (NSU SFGWEP) has partnered with primary care clinics to provide AFHS training and support to promote AFHS transformation in Broward and Miami-Dade Counties. NSU SFGWEP provides face-to-face and virtual training for AFHS and Electronic Health Record (EHR) documentation as part of the initiative. This project focuses on a group of primary care clinics in Broward County, Florida. In this paper, we evaluate the progress of AFHS transformation through six e-clinical measures that collectively provide indicators of the 4 M framework of AFHS (What Matters, Medication, Mentation, and Mobility). We used provider feedback and e-clinical measures aligned with the Center for Medicare & Medicaid Services (CMS) Merit-based Incentive Payment System (MIPS) to measure clinic outcomes. From Jan 1– Dec 31, 2019, to Jan 1–Dec 31, 2020, the clinics improved high-risk medication management (0–3.71%), advanced care planning (6.79%–20.74%), and fall risk assessment (no data- 46.72%). In response to the demand for more STEM-certified teachers, identity has emerged as a theoretical lens for examining how candidates can be recruited into and retained in the teaching profession. This study explores the intersection between teaching identity and disciplinary identity as they relate to the development of a disciplinary teaching identity. Survey responses were collected from students in a teaching recruitment class for STEM majors, and regression models were developed to predict disciplinary teaching identity and program persistence. Findings from both models indicated that teaching identity is a strong predictor of both disciplinary teaching identity and program persistence, but disciplinary identity plays less of a role. These findings align with previous qualitative research on teacher identity development. Additionally, these findings highlight a need to engage STEM faculty in efforts to recruit students who strongly identify with STEM into the teaching profession. [ABSTRACT FROM AUTHOR]

This article focuses on the predicament of owner-occupiers and small investors presently liable for the removal of combustible cladding on build-to-sell residential high-rise. We argue that while the proliferation of combustible materials has been shaped by cost-cutting and risk-shifting by construction firms, these practices did not on their own transfer the responsibility of remediation to consumers. Examining the attempts of one densifying nation, Australia, to locate responsibility for combustible cladding through two parliamentary inquiries, we analyse witness testimonies to show how public policies encouraged materials substitution, removed on-site inspection and protected corporations from litigation. Moving beyond neoliberalism, these policy reforms leveraged information asymmetries and the material complexity of residential high-rise to create a climate of "imperceptibility" towards unsafe materials. Together, material, financial and policy dimensions intersected to enable capital accumulation through the expansion of consumer harm in high-rise housing markets. The financialisation of housing is seen to undermine tenants' rights, affordable housing and planning controls in order to make housing and homes more amenable to profit extraction. However, the extent to which owner-occupiers themselves seek to influence urban development and planning processes to protect their housing assets has been less well-considered. Through an online survey of 1122 owner-occupiers in Australia, this article redresses this gap. By identifying the financial values participants attach to their home, and their inclinations to join resident action groups, we reveal that those with the strongest investment values are also most inclined to join resident action groups. Expanding conceptualisations of investors beyond institutional investors, the article reveals the agency of financialised owner-occupiers who, as investor-activists, seek to influence planning processes to secure the profitability of their own housing assets. The article thus reconceptualises resident action as a financial strategy to protect long- and short-term housing investments and, in doing so, charts the urban implications of financialised home ownership and investor subjectivities. As governments encourage circular economy (CE) initiatives, markets for waste recirculation are taking shape. But implementation is in its infancy and material circuits are emergent. Early food waste CEs shaped by commercial players emphasise capital investment, routinised forms of waged labour, processing sites distant from food waste sources, and transactional relationships. Less well understood is the potential for vernacular circularity beyond market-based, transactional frames. This paper reports from a collaborative research exercise with a non-profit community farm in nonmetropolitan Australia, seeking to connect with cafés to access food waste for composting. Cafés are a nexus of production and consumption, ubiquitous in the contemporary multicultural Australian context, and therefore ideal for grassroots CEs. Ten local cafes participated, reviewing existing food waste practices, motivations for circularity, and contextual factors including the regional setting. We found that food waste circularity emerges via divergent pathways related to enterprise type and scale, environmental values of actors, place embeddedness, and local relationships. These pathways reflect the place-based attributes and diverse sustainability values of residents and businesses in the coastal, industrial city of Wollongong, where the study is based. Contrasting distant, transactional circuits, are more-than-transactional food waste pathways, developed by microscale actors shaping vernacular material flows and "hacking" public provision of Food Organic and Garden Organic (FOGO) waste services to mobilise environmental values and community relationships. Overlooked by "big policy" more-than-transactional relationships bind producers, intermediaries and consumers in closer loops and, in so doing, enrich place and facilitate an ethic of care for soil and land. Human encounters with waste can trigger reflections on taken-for-granted assumptions about consumption. Taking this observation as its starting point, this paper explores whether and how the introduction of the kitchen caddy and food waste composting at the municipal scale generates new environmental subjectivities in Australian homes. Using visceral research methods, the paper shows that, other than participation in municipal composting, close encounters with food becoming compost do not trigger more experimental or sustainable relationships with food. Rather, they trigger new configurations of cleanliness, tidiness and storage as participants seek to stabilise their home against the disruptive sight, smell and touch of food becoming compost. In unsettling the boundaries between humans and nonhumans, and cleanliness and chaos, food becoming compost produces visceral disgust among residents who are primed through modern home cultures to maintain cleanliness, control and the prevention of nonhuman intrusions at home. The success of municipal composting thus hinges on households' commitment to 'sustainability work' in placing, storing, cleaning and sealing food to reconfigure the modern home for waste recovery. Reconceptualising municipal composting as a co-production of sensory engagement and household practices, we centre a visceral politics of household sustainability at the heart of municipal composting and resource management. As decarbonisation interventions proliferate within cities, local governments setting ambitious targets are increasingly engaged in complex financial relations. Recognising the necessary cost of renewable and energy efficient infrastructures, and the ever-present constraints on public funds, this paper argues that finance is a critical node through which local governments advance decarbonisation in urban localities. While local decarbonisation strategies have been viewed cautiously for their potential to overburden individuals at the expense of more systematic and organisational change, this paper reveals a more complex picture. Drawing on decarbonisation initiatives in two Melbourne municipalities—Moreland and Darebin—it identifies four ways in which local governments are using public finance to achieve their sustainability objectives. Local governments are brokering bulk product purchases for residents; lending upfront capital for solar PV via local property taxes; purchasing energy efficient products and funding innovative technology pilots; and procuring renewable energy supply through multi-stakeholder power purchase agreements. By targeting lower income households and pooling resources with other organisations, the paper shows that local governments can address socio-economic inequality and facilitate extra-local change towards a low-carbon city. However, these incremental achievements emphasise the need for co-ordination and state engagement to realise decarbonisation at a meaningful scale. [ABSTRACT FROM AUTHOR]

Through a comparative study of political activism in Millers Point, Dawes Point, and The Rocks in Sydney from 1971 to 1974 and from 2014 to 2017, this paper examines the more‐than‐human force of bodies and buildings in redemocratising cities. The paper brings theoretical insights from urban assemblage and Deleuzian scholarship into dialogue with post‐political theory. In doing so, it shifts the focus in urban political geography from a concern with diverse social groups to the political potential in more‐than‐human assemblages. Through a re‐reading of Sydney's Green Bans in Millers Point, Dawes Point, and The Rocks, the paper first shows how the withdrawal of labour and stilled building sites comprise informal pathways of planning rejection that centralise social and ecological concerns in decision‐making in urban development. That first case is then contrasted with the contemporary case of privatisation and displacement at the same sites, showing how aged bodies can energise latent deliberative spaces across diverse (if repressed) institutions in liberal democracies. Given the capacities of materials and bodies to produce planning outcomes, the paper conceptualises planning as a more‐than‐human process enhanced by immersion in the experimental materialism of political movements—and this materialism is inherently geographical. The paper concludes with my reflecting on whether the proximity of workers and residents to the materials of the built environment in social movement activism of the 1970s intensified the reach of the event compared with the uncertain struggle over Millers Point today. Objective: We evaluated the extent to which providing training and technical assistance to early childcare centre (ECC) directors, faculty and staff in the implementation of evidence-based nutrition strategies improved the nutrition contexts, policies and practices of ECC serving racially and ethnically diverse, low-income children in Broward County, Florida, USA. The nutrition strategies targeted snack and beverage policies and practices, consistent with Caring for Our Children National Standards.Design: We used the nutrition observation and document review portions of the Environment and Policy Assessment and Observation (EPAO) instrument to observe ECC as part of a one-group pre-test/post-test evaluation design.Setting: ECC located within areas of high rates of poverty, diabetes, minority representation and unhealthy food index in Broward County, Florida, USA.Subjects: Eighteen ECC enrolled, mean 112·9 (sd 53·4) children aged 2-5 years; 12·3 (sd 7·2) staff members; and 10·2 (sd 4·6) children per staff member at each centre.Results: We found significant improvements in centres' overall nutrition contexts, as measured by total EPAO nutrition scores (P=0·01). ECC made specific significant gains within written nutrition policies (P=0·03) and nutrition training and education (P=0·01).Conclusions: Our findings support training ECC directors, faculty and staff in evidence-based nutrition strategies to improve the nutrition policies and practices of ECC serving racially and ethnically diverse children from low-income families. The intervention resulted in improvements in some nutrition policies and practices, but not others. This special section examines the possibility of meaningful debate and contestation over urban decisions and futures in politically constrained contexts. In doing so, it moves with the post-political times: critically examining the proliferation of deliberative mechanisms; identifying the informal assemblages of diverse actors taking on new roles in urban socio-spatial justice; and illuminating the spaces where informal and formal planning processes meet. Secondhand smoke (SHS) exposure is a public health issue for residents of multi-unit housing (MUH) properties. We evaluated the impact of smoke-free policy implementation on reported SHS exposure among racially, ethnically diverse seniors living in low-income MUH properties. In Spring 2013 and Summer 2014, we surveyed residents (n = 960) at 15 MUH properties in Broward and Miami-Dade Counties, Florida. The percentage of residents reporting SHS exposure within their apartments from elsewhere in or around their building decreased from 31.1 %, before policy implementation, to 23.6 % at follow-up ( p = 0.02). In multivariate analysis of non-smoking residents after policy implementation, residents who reported having one or more comorbidities were two times more likely to report SHS exposure (aOR 2.23, 95 % CI 1.12-4.40). Considering the vulnerability of low-income seniors to SHS exposure, our findings are relevant to residents, property owners/managers, and public health professionals making decisions about smoke-free policies for MUH properties in which seniors reside. This paper explores relationships between community opposition, planning assessments and local political processes. While resident opposition to development proposals is thought to delay housing supply, the nature, extent and pathways of influence have not been quantitatively established. In Victoria the number of third party objections has no direct legal weight, but in practice, development applications involve multiple decision makers. Community expectations that objection numbers “count” may reflect suspicion that refusals are more likely from elected local decision makers. This paper tests for relationships between procedural and political pathways in planning. It uses descriptive statistics and binary logistic regression models based on one year (15 676) of Melbourne residential development assessments. It is found that objection numbers increase significantly with local socio-economic status and that, as applications receive more objections, elected representatives more often intervene. Assessments by elected councilors are significantly more likely to be refused, and have relative odds more than seven times higher of resulting in appeal. The paper argues that local contestation of housing, particularly from better-resourced groups, is highly adaptable to reforms seeking to overcome or rationalise it. Reducing or shifting opportunities for third party opposition may less reduce planning uncertainty, than increase its variation, complexity, and spatial concentration. [ABSTRACT FROM AUTHOR]

After central business districts (CBD) emptied from COVID-19 pandemic lockdowns and widespread working-from-home, culture and creativity feature prominently within recovery strategies, enrolling the arts and events to enliven urban precincts and attract people back into city centres. We draw upon resilience theory and creative city policymaking to critique present formulations of CBD revitalisation, and suggest alternatives. Despite overtures to social inclusion and environmental sustainability, revitalisation strategies mobilise pre-existing "vehicular ideas" that support corporate business interests in and claims on central city space. We articulate concerns around inclusivity, financial and property interests, creativity as consumption rather than production, livelihood concerns, and underwhelming acknowledgement of pandemic disruption. Instead of placing creativity in service of the competitive positioning of the CBD, we ask: what is the very purpose of the post-pandemic CBD, and who is the CBD for? The Human Resource and Services Administration, Bureau of Primary Health Care Health Center program was developed to provide comprehensive, community-based quality primary care services, with an emphasis on meeting the needs of medically underserved populations. Health Centers have been leaders in adopting innovative approaches to improve quality care delivery, including the patient centered medical home (PCMH) model. Engaging patients through patient experience assessment is an important component of PCMH evaluation and a vital activity that can help drive patient-centered quality improvement initiatives. A total of 488 patients from five Health Center PCMHs in south Florida were surveyed in order to improve understanding of patient experience in Health Center PCMHs and to identify quality improvement opportunities. Overall patients reported very positive experience with patient-centeredness including being treated with courtesy and respect (85 % responded 'always') and communication with their provider in a way that was easy to understand (87.7 % responded 'always'). Opportunities for improvement included patient goal setting, referrals for patients with health conditions to workshops or educational programs, contact with the Health Center via phone and appointment availability. After adjusting for patient characteristics, results suggest that some patient experience components may be modified by educational attainment, years of care and race/ethnicity of patients. Findings are useful for informing quality improvement initiatives that, in conjunction with other patient engagement strategies, support Health Centers' ongoing transformation as PCMHs. The Indiana Science Initiative ( ISI) is a systemic effort to reform K-8 science education. The program provides teachers with professional development, reform-oriented science modules, and materials support. To examine the impact of the initiative's professional development, a participant observation study was conducted in the program's pilot year. Five teachers in grades 3-6 were observed and interviewed as they implemented the ISI-provided modules. Analysis of the observation data revealed that the teachers incorporated each of the features of inquiry science instruction. However, they did not consistently teach in a way that was aligned with the intent of the ISI. Examination of interview data provided insight into influences on teachers' use of inquiry with the ISI-provided modules. These data revealed that teachers were aware of the intent of the ISI and attempted to align their instruction. However, teachers were influenced by their perceptions of students' behavior and abilities as well as timing and the appropriate level of teacher control needed to facilitate science instruction. The research suggests that professional development activities should prepare teachers to help learners evaluate explanations against alternatives, connect explanations to scientific knowledge, and provide strategies to address teachers' perceptions of students, timing, and teacher control. Behavioral interventions for children with ASD typically target disability specific behaviors such as social skills and communication. There is a particular need for age-appropriate interventions/programs for school-aged children with ASD to teach age-appropriate behaviors (Dawson & Burner, 2011). Adapted physical activity appears to be an appropriate means to help eliminate this disparity and teach agea-ppropriate motor behaviors (Staples & Reid, 2010; MacDonald, Esposito & Ulrich, 2011; MacDonald, Lord & Ulrich, 2013). The purpose of this paper is to outline the behavior management strategies used in a two-wheeled bicycle riding intervention for children with autism, and provide examples of how these techniques can be transferred to the home environment. Examples are provided as to how parents or other family members can utilize these behavior modification techniques to increase two-wheeled bicycle riding frequency at home for school-aged children with ASD. Community-based physical activity, provided by the community sector, allows for a safe and effective delivery of physical activity for children with disabilities (USDHHS, 2008). To assist community- based health professionals in providing community-based physical activity to children with disabilities, three evidence-based resources will be presented: (1) Global and national resources and certifications (2) Facility and program accessibility measures such as AIMFREE, and (3) Collaboration with academic and clinical sectors by the utilization of the Healthy People 2010 Toolkit. Through the use of these tangible resources, community health professionals can become better prepared in the delivery of community-based physical activity for children with disabilities. Previous studies have gauged support for implementing smoke-free multi-unit housing (MUH) policies in the United States, but none have specifically examined attitudes among racially and ethnically diverse elders living in low-income MUH. We surveyed a convenience sample of elders 62 years of age and older ( n = 807) across 24 low-income housing properties in Broward County, Florida in order to assess residents' smoking behaviors, exposure to second-hand smoke, and support for smoke-free policies. The study sample was ethnically and racially diverse with Hispanics comprising more than 61 % of the population, and 22 % identifying as Black or other races. Although close to 22 % of the sample were former smokers, only 9 % of residents reported being current smokers. The majority of residents surveyed supported no-smoking policies: 75 % support no-smoking policies for individual units; 77 % supported no-smoking policies in common areas; and, 68 % supported no-smoking policies in outdoor areas. Over 29 % of residents surveyed reported being exposed to secondhand smoke entering their units from elsewhere in their building. Residents who reported having a home smoking rule were more than twice as likely to support an indoor policy compared to residents who allowed smoking anywhere in their home (OR = 2.36; 95%CI 1.25-4.43; p ≤ 0.01), and nonsmoking residents were nearly three times as likely to support an indoor policy compared to smokers (OR = 2.89; 95%CI 1.44-5.79; p ≤ 0.01). Support for an indoor policy was not modified by age, gender, ethnicity or race. . This paper assesses the relevance of mortgage-led consumption for the assemblage of home. Drawing on qualitative research completed in the UK, we show how the materials and meanings of owner-occupation are constituted by, and experienced through, the accumulation and deployment of secured debt. This is enabled by a particular financial regime, in which the proceeds of equity borrowing are freed for discretionary expenditure. Homes and their contents thus acquire the status of “debted objects”, and these form an interface between the financial and familial values comprising residential space. By attending to these mortgage-enabled purchases, we expose and evaluate the myriad ways in which equity borrowings animate the assemblage of home, adding value to property, linking domestic space with distant geographies and inspiring the art of dwelling. A new methodology for the facile isolation of biomolecules containing carbonyl groups has been developed. This methodology is based on tethering of 8-aminopyrene-1,3,6-trisulphonic acid (APTS) to a target molecule via amino or imino bonds, followed by treatment with cyclen-thiourea receptor (cyclen 1). This receptor forms an insoluble complex with APTS conjugates, and therefore precipitates these conjugates from the solution. A sucrose/lactose mixture has been used for proof of concept and lactose was successfully isolated using this procedure. Whereas labelling of lactose via reductive amination resulted in the permanent attachment of the dye to the disaccharide, the labelling via an imine bond was reversible. Additional protocol developed for the lactose labelled with APTS dye via imine bond allowed us to recover all components in their unaltered form. Such an additional protocol not only allows isolation of target molecules without their modification, but also allows one to use recovered cyclen 1 and the dye for subsequent isolation cycles. This paper explores the transition of talk to text in the process of performing and recording semi-structured interviews over the telephone. In particular, it considers the ways respondents and the materials enjoined in the process of interviewing exceed their role in the research process. Drawing on my experience of conducting telephone interviews in the UK, I position the interview as an uncertain encounter: one framed by earlier narrations as much as the aims and objectives of the research project; and an orientation towards ‘talk’ as much as the reduction of performance to text. Although there is widespread consensus in human geography that neoliberalism is achieved through the intersection of multiple scales and states, there has been growing emphasis in recent years on the hybrid qualities of neoliberalist-styles of governance. Because hybrid approaches draw attention to the wide range of actors and contradictory agendas underpinning the always uneven expression of neoliberalist projects, they are particularly suited to the identification of residual state capacities in contexts where neoliberalist forms dominate economic and political life. Drawing on two cases of large-scale residential development in New South Wales, namely Warnervale Town Centre on the New South Wales Central Coast and the Australian Defence Industry site at St Marys in Sydney's west, the present paper adopts an hybrid approach in order to identify current state capacities. Introduction: The NSW Integrated Care Strategy involves implementing innovative, locally led models of integrated care across the State to transform the NSW healthcare system. The Agency for Clinical Innovation's Patient Reported Measures (PRMs) Program is an important enabler of this strategy. The PRMs Program aims to enable patients to provide direct, timely feedback in order to drive improvement and integration of health care across NSW. Short description of practice change implemented: The PRMs Program enables the collection and use of Patient Reported Outcome Measures (PROMs) and Patient Reported Experience Measures (PREMs). The program has used a collaborative co-design approach working with a range of consumers and clinicians throughout each phase. The program is currently working with 11 proof of concept sites (geographical areas) from rural, regional and metropolitan NSW and has facilitated the development of strong partnerships between the acute, community and primary health care services. The program has significantly built capability and capacity of the health care system to transform how care is delivered and how people think about patient outcomes using a range of resources. Aim and theory of change: A formative evaluation of the program was completed to assess the short-term and intermediate outcomes and identify the barriers and enablers of embedding PRMs into routine clinical practice. Targeted population and stakeholders: The targeted population for the program was people living with complex and long term conditions. The PRMs Program has worked with a variety of consumers, clinicians and managers from the proof of concept sites across a range of different settings. Timeline: The Integrated care strategy commenced in 2014 and will run over six years. The PRMs Program commenced late 2014 initially with four proof of concept sites and, due to heavy demand for support and broadening of the program, the program has expanded to 11 proof of concept areas which comprises 81 sites. Highlights (innovation, Impact and outcomes): The PRMs Program formative evaluation identified that having a genuine co-design approach (working from the start with consumers and clinicians) of the program was identified as a key strength of the program as was the co mmitment to travelling out to visit the sites to localise implementation and the education and training was well received. There was early evidence that care planning changed following completion of PROMs and that PRM data has been used to monitor quality and refine service delivery. There was also early evidence that care was more patient-centred, more responsible to patient's needs, clinical practices improved, service delivery more effective and added to reduction in unplanned hospital admissions. Conclusions (comprising key findings): PRMs Program demonstrates how the use of co-design and technology can be used to improve the integration of care and change the way care is delivered to ensure that we really hear what matters most to patients. OBJECTIVES: Understanding how the COVID-19 pandemic affected cardiovascular disease (CVD) risk monitoring in primary care may inform new approaches for addressing modifiable CVD risks. This study examined how pandemic-driven changes in primary care delivery affected CVD risk management processes. STUDY DESIGN: This retrospective study used electronic health record data from patients at 70 primary care community clinics with scheduled appointments from September 1, 2018, to September 30, 2021. METHODS: Analyses examined associations between appointment type and select care process measures: appointment completion rates, time to appointment, and up-to-date documentation for blood pressure (BP) and hemoglobin A1c (HbA1c). RESULTS: Of 1,179,542 eligible scheduled primary care appointments, completion rates were higher for virtual care (VC) vs in-person appointments (10.7 percentage points [PP]; 95% CI, 10.5-11.0; P < .001). Time to appointment was shorter for VC vs in-person appointments (--3.9 days; 95% CI, --4.1 to --3.7; P < .001). BP documentation was higher for appointments completed pre-- vs post pandemic onset (16.2 PP; 95% CI, 16.0-16.5; P < .001) and for appointments completed in person vs VC (54.9 PP; 95% CI, 54.6-55.2; P < .001). HbA1c documentation was higher for completed appointments after pandemic onset vs before (5.9 PP; 95% CI, 5.1-6.7; P < .001) and for completed VC appointments vs in-person appointments (3.9 PP; 95% CI, 3.0-4.7; P < .001). CONCLUSIONS: After pandemic onset, appointment completion rates were higher, time to appointment was shorter, HbA1c documentation increased, and BP documentation decreased. This special section builds on Planning the Post‐Political City—Part 1 to examine if and how planning is showing signs of a post‐democratic turn taking place in Australian cities. In Part 1, we presented a collection of papers examining Australia as a post‐political landscape, exploring the new ways in which Australian publics are resisting dominant neoliberal practices and logics of growth and, in doing so, are intervening in decision‐making practices to assert new forms of power and participation. In Part 2, we show how participatory practices continue to evolve. We use this brief editorial to ask a foundational question: have those implicated in the governance and management of Australian cities embarked on a post‐democratic path? As they are presented with new exclusionary and managerial governance systems, the public's participation suggests at the very least that post‐political and post‐democratic conditions are neither immutable nor inevitable. However, more democratic forms of governance rely on a rich array of activist types and approaches requiring greater institutional support in order to challenge Australia's post‐political condition. Introduction: Current guidelines continue to lead to under- and over-triage of injured patients in the pre-hospital setting. End-tidal carbon dioxide (ETCO2) has been correlated with mortality and hemorrhagic shock in trauma patients. This study examines the correlation between ETCO2 and in-hospital outcomes among non-intubated patients in the pre-hospital setting.Methods: We retrospectively studied a cohort of non-intubated adult trauma patients with initial pre-hospital side-stream capnography-obtained ETCO2 presenting via ground transport from a single North Carolina EMS agency to a level one trauma center from January 2018 to December 2018. Using the Liu method, the optimal threshold for low ETCO2 was ≤ 28.5 mmHg.Results: Initial pre-hospital ETCO2 was recorded for 324 (22.0%) of 1473 patients with EMS data. Patients with low ETCO2 (N = 98, 30.3% of cohort) were older (median 58y vs 45y), but mechanisms of injury and scene vital signs were similar (p>0.05) between low and normal/high ETCO2 cohorts. Median injury severity score (ISS) did not differ significantly between the low and normal/high ETCO2 groups (5 vs 8, p=0.48). Compared to normal/high ETCO2, low ETCO2 correlated with increased unadjusted odds of mortality (OR 5.06), in-hospital complications (OR 2.06), and blood transfusion requirement (OR 3.05), p<0.05. Low ETCO2 was associated with 7.25 odds of mortality (95% CI 2.19,23.97, p=0.001) and 3.94 odds of blood transfusion (95% CI 1.32-11.78) after adjusting for age, ISS, and scene GCS. All but one of the massive transfusion patients (N = 8/9) had a low pre-hospital ETCO2.Conclusions: Low initial pre-hospital ETCO2 associates with poor clinical outcomes despite similar ISS and mechanisms of injury. ETCO2 is a potentially useful pre-hospital point-of-care tool to aid triage of trauma patients as it may identify hemorrhaging patients and predict mortality. [ABSTRACT FROM AUTHOR]

Most debate on home ownership and risk has focused on the management of mortgage debt. But there are other risks for home buyers in settings where housing dominates people's wealth portfolios: where the investment dimensions of property are at a premium; and where housing wealth is, de facto, an asset base for welfare. This article draws from qualitative research with 150 UK mortgage holders to assess the character, extent and possible mitigation of this wider risk regime. The analysis first explores the value home buyers attach to the financial returns on housing. Next we document the extent to which home equity is earmarked and used as a financial buffer. Finally, reflecting on the merits and limitations of this tactic, we conclude by asking whether - in the interests of housing and social policy, as well as with a view to managing the economy - there is any need, scope or appetite for more actively sharing the financial risks and investment gains of housing systems anchored on owner-occupation.
