Obesity, a complex disease that involves energy imbalance and chronic low-grade inflammation, is implicated in the pathogenesis of several chronic non-communicable diseases. As dietary components modulate the human body's inflammatory status, the Dietary Inflammatory Index (DII®), a literature-derived dietary index, was developed in 2009 to characterize the inflammatory potential of a habitual diet. Abundant research has been conducted to investigate the associations between DII and obesity. In this narrative review, we examined the current state of the science regarding the relationships between DII and the inflammatory pathophysiological aspects related to obesity. DII is associated with inflammation in obesity. The most pro-inflammatory diet was directly related to higher levels of pro-inflammatory markers, which included C-reactive protein (CRP), interleukin-6 (IL-6), IL-1β, and tumor necrosis factor-α (TNF-α). Therefore, evidence suggests that the use of the DII may be useful for understanding the relationship between diet and the inflammatory process related to obesity.

Objective Fibroblast growth factor 21 (FGF21) is among the activators that can stimulate thermogenesis in the white adipose tissue and brown adipose tissue. People with obesity have elevated blood levels of FGF21, but also develop resistance to its action, impairing its beneficial role. Inversely, clinical treatments to weight loss has been pointed out as an important therapy for increasing and recovering sensitivity to FGF21. The aim was to analyse the effect of long-term weight loss interdisciplinary intervention on FGF21 and body composition. Subjects and methods Eighty-six post-pubertal obese adolescents (14-19 years-old), were submitted to 20 weeks of weight loss therapy (clinical, nutritional, psychological and physical exercise support). Anthropometric measures, body composition and rest metabolic rate (RMR) by bioelectrical impedance, and serum FGF21 sample by ELISA were evaluated. The adolescents were grouped according to FGF21 individual delta variations after therapy: Higher Increase (HI); lower increase (LI); lower decrease (LD); higher decrease (HD). Results All groups present weight loss. Only in FGF21 ≥ 76,5 pg/mL variation the free-fat-mass and rest metabolic rate were preserved and to others group these variables were significantly reduced. Conclusion High increase in FGF21 can contribute to preservation of FFM and RMR after weight loss therapy, could have important implications for energy balance regulation. Future studies are necessary to continue determining the role of magnitude effects of FGF21 levels in obesity to improve clinical practice, especially in paediatrics population.

Os insultos nutricionais e ambientais no período perinatal produzem alterações endócrinas, funcionais e comportamentais na vida adulta. Assim, é necessário avaliar o impacto da alimentação infantil no aumento do risco de obesidade e outras doenças associadas, inclusive nas gerações subsequentes. O objetivo do presente estudo foi avaliar os efeitos metabólicos e reprodutivos da sobrenutrição pós-natal precoce durante a lactação em ratas Wistar e as consequências na segunda geração. Ratas Wistar prenhes (geração F0) foram distribuídas em dois grupos experimentais: ninhada normal (NL, 9 filhotes, n=11) e ninhada reduzida (SL, 3 filhotes, n=11) durante todo o período de lactação. No 3º dia pós-natal (PN3), as ninhadas SL foram reduzidas para 3 filhotes/mãe. Após o desmame (PN21), as fêmeas F1 (NLF1 e SLF1) foram alimentadas com água e ração à vontade e os parâmetros metabólicos e reprodutivos foram analisados nas fases adulta (nulípara), gestação e lactação. Na prole da geração F1 (geração F2 – GCF2 e GOF2) foram avaliados parâmetros físicos e sensório-motor, metabólicos e reprodutivos. Em comparação com o grupo NLF1, o grupo SLF1 mostrou: aumento de massa significante a partir do PN7, o qual se manteve durante a fase adulta, no final da gestação e da lactação; aumento da ingestão alimentar durante o PN21 a PN70, sem diferença significativa nas fases gestacional e lactacional; aumento nas massas de gorduras nas fases adulta, gestacional e lactacional; aumento nas massas do fígado na fase lactação e do pâncreas em todas as fases; antecipação da instalação da puberdade e menor número de estros; menor desempenho no comportamento sexual; redução no número de corpos lúteos, sem alteração no percentual de pré-implantação, perdas pós-implantação e no número de fetos vivos machos e fêmeas; menor número de filhotes nascidos vivos machos e fêmeas; maior cuidado materno; redução da concentração sérica de estradiol; aumento das concentrações séricas de triglicérides e redução do colesterol HDL na gestação e lactação, sem alteração nas dosagens de colesterol total; aumento da glicemia na fase adulta. Nas análises histopatológicas também houve aumento da área dos adipócitos e da espessura da camada adiposa das glândulas mamárias; redução do número de folículos em crescimento e aumento do número de folículos císticos e atrésicos em ovários; glândulas endometriais com células inflamatórias e em degeneração no útero. A prole GOF2, comparada com a prole GCF2, apresentou maior massa corporal desde o nascimento até o PN21, assim como maior ingestão de leite no PN8. Na avaliação do desenvolvimento físico e sensório-motor, a prole GOF2 apresentou antecipação dos parâmetros sensório-motores (reflexo postural, geotaxia negativa, esquiva ao abismo) em machos e fêmeas. Todavia o reflexo de sobressalto auditivo foi mais tardio em machos e fêmeas, não tendo diferença significativa nos parâmetros de olfato e queda livre. Quanto aos parâmetros físicos, a erupção dos dentes incisivos foi antecipada e a abertura dos olhos, atrasada, sem diferença significativa no desdobramento das orelhas. A instalação da puberdade foi antecipada nos machos e fêmeas. Concluindo, a sobrenutrição pós-natal precoce, atuando como fator de programação metabólica, promoveu obesidade, alterações metabólicas e reprodutivas em ratas da geração F1 e, consequentemente, também promoveu alterações metabólicas, reprodutivas e no desenvolvimento físico e sensório-motor da prole F2. Thus, it is necessary to assess the impact of infant feeding on the increased risk of obesity and other associated diseases, including in subsequent generations. The objective of the present study was to evaluate the metabolic and reproductive effects of early postnatal overnutrition in Wistar during lactation and the consequences in the second generation. Female Wistar rats (F0 generation) were distributed in two experimental groups: normal litter (NL, 9 pups, n=11) and reduced litter (SL, 3 pups, n=11) throughout the lactation period. On the 3rd postnatal day (PN3), litters SL were reduced to 3 pups/dam. After weaning (PN21), F1 females (NLF1 and SLF1) were fed with water and feed at will, and metabolic and reproductive parameters were analyzed in the adult (nulliparous), pregnancy and lactation phases. In the offspring of the F1 generation (F2 generation – GCF2 and GOF2), physical and sensory-motor, metabolic and reproductive parameters. In comparison to the NLF1 group, the SLF1 generation F1 group showed: a significant increase body weight in PN7, which was maintained during the adult phase, at the end of gestation and lactation; increased food intake during PN21 to PN70, without significant difference in the pregnancy and lactation phases; increase in the fat masses in the adult, pregnancy and lactation phases; increase in the masses of the liver in the lactation phase and of the pancreas in all phases; early of the onset of puberty and lower numbers of estrus; lower performance in sexual behavior; reduction in the number of corpus luteum, with no change in pre-implantation percentage, post-implantation losses and number of live male and female fetuses; fewer live births, both male and female; greater maternal care; reduction of serum estradiol concentration; increase in serum triglyceride concentrations and reduction of HDL cholesterol in pregnancy and lactation, with no change in total cholesterol levels; increase in adult glucose. In the histopathological analyzes there was also an increase in the area of adipocytes and the thickness of the adipose layer of the mammary glands; reduction in the number of growing follicles and increase in the number of cystic and atretic follicles in ovaries; endometrial glands with inflammatory cells and degenerating in the uterus. The GOF2 offspring, compared to the GCF2 offspring, presented higher body weight from birth to PN21, as well as higher milk intake in PN8. In the evaluation of the physical and sensory-motor development, the GOF2 offspring presented advance of the sensory-motor parameters (righting reflex, negative geotaxis, cliff avoidance) in males and females. However, the auditory startle reflex was later in males and females, with no significant difference in smell and free-fall righting parameters. Regarding the physical parameters, the eruption of the incisors was anticipated and the opening of the eyes delayed, without significant difference in the unfolding of the ears. The onset of puberty was anticipated in males and females. In conclusion, early postnatal overnutrition, acting as a factor of metabolic programming, promoted obesity, metabolic and reproductive changes in F1 generation rats and, consequently, also promoted metabolic and reproductive changes and in the physical and sensory-motor development of F2 offspring. Perturbações ambientais e nutricionais durante o período perinatal causam disfunções metabólicas na prole através de alterações epigenéticas afetando negativamente a saúde em geral. Portanto, a restrição alimentar possui o potencial de afetar o organismo através dos mesmos mecanismos, e seus efeitos em animais metabolicamente programados para obesidade precisam ser elucidados. O objetivo deste trabalho foi determinar se a restrição alimentar impactaria parâmetros comportamentais e cardiovasculares de animais obesos e não obesos e o envolvimento do sistema renina angiotensina (SRA) e da MAPK / ERK no coração desses animais. Três dias após o nascimento, ratas Wistar foram separadas em grupos small litter (SL, 3 filhotes) ou normal litter (NL, 9 filhotes). Posteriormente uma leva de animais de ambas as proles foram submetidos a restrição alimentar (50%) enquanto outra leva permaneceu com livre acesso a alimento durante o período da puberdade (dos 30 aos 60 dias). Aos 120 dias, foi realizado registro acordado para a avaliação de parâmetros cardiovasculares, testes de campo aberto (CA), labirinto em cruz elevado (LCE) e nado forçado (NF) para avaliação de parâmetros comportamentais. Adicionalmente, o peso corporal e ingestão alimentar foram monitorados durante todo o período experimental. Por fim, os animais foram anestesiados e eutanasiados para coleta de tecidos, para a subsequente realização de western blot e histologia. O estágio supervisionado constitui item obrigatório para a formação superior. Neste contexto, destaca-se a figura do preceptor, um ator importante e indispensável para o desenvolvimento do estágio. A preceptoria é um exercício relativamente novo que deve ser acompanhada de uma formação pedagógica que propicie capacitação aos profissionais para o desenvolvimento de tal função. Estudos que abordam a preceptoria em áreas da saúde são comuns na literatura, porém há poucos estudos que abordam essa temática na área da nutrição, tornando-se relevante a coleta, a sistematização e a análise de temas relativos a esse assunto. O objetivo foi analisar o estágio em Nutrição Clínica do curso de nutrição da Universidade Federal Goiás, em hospitais de ensino no âmbito do preceptor. Pesquisa de natureza exploratória, utilizando-se a abordagem qualitativa. Foi aplicado um questionário aos preceptores do estágio de nutrição clínica, onde foram avaliadas três dimensões: processo ensino-aprendizagem, competências gerais para a formação do estudante e a relação interpessoal da equipe do estágio. Após tabulação dos dados, foram realizados dois grupos focais com os preceptores de cada local. Para a aplicação do questionário e a realização dos grupos focais foi aplicado o termo de consentimento livre e esclarecido. A análise dos dados qualitativos foi realizada, utilizando-se a técnica de análise de conteúdo, segundo Bardin. Nos resultados, observou-se uma falta de clareza na conceituação e na delegação das funções do preceptor e do professor. O relacionamento e o diálogo entre eles foram avaliados de forma negativa, isto porque tal relacionamento foi intimamente ligado à presença do professor no local do estágio, que foi considerada insuficiente por alguns preceptores. A falta de formação em preceptoria foi apontada como fator limitador da atuação do preceptor no estágio. A presença do estagiário no local de trabalho foi considerada um agente de mudanças positivas. Ações que visem uma atuação conjunta entre preceptores e professores são importantes para o desenvolvimento adequado do estágio. A universidade tem o papel de desenvolver educação permanente e capacitação para os profissionais preceptores com objetivo de esclarecer os papéis e a importância de cada ator. The supervised internship is a compulsory item for higher education. In this context, we highlight the figure of the preceptor, an important and indispensable actor for the development of the stage. The preceptory is a relatively new exercise that must be accompanied by a pedagogical training that offers training to professionals for the development of this function. Studies that address the preceptory in health areas such are common in the literature, but few studies that address this issue in the area of ​​nutrition, so it becomes relevant to collect, systematize and analyze topics related to this issue. The objective was to analyze the preceptor in Clinical Nutrition of the nutrition course of the Federal University of Goiás, in teaching hospitals in the ambit of the preceptor. Research of exploratory nature using the qualitative approach. A questionnaire was applied to preceptors of the clinical nutrition in two teaching hospitals where the internship were carried out, where three dimensions were evaluated: teaching-learning process, general competences for student training and interpersonal relationship of the trainees team. After tabulation of the data, two focus groups were performed with the preceptors of each site. For the application of the questionnaire and the realization of the focus groups, the term of free and informed consent was applied. Qualitative data analysis was performed using the Bardin content analysis technique. The results showed a lack of clarity in the conceptualization and delegation of the functions of the preceptor and the teacher. The relationship and the dialogue between them were evaluated in a negative way, because this relation was closely linked to the presence of the teacher in the place of the internship, considered insufficient by some preceptors. The lack of preceptor training was pointed out as a limiting factor in the performance of the preceptor at the stage. The presence of the trainee in the work environment was considered an agent of positive changes. Actions aimed at joint action between preceptors and teachers are important for the proper development of the internship. The university has the role of developing continuing education and training for preceptory professionals in order to clarify the roles and importance of each actor.
