Les paragangliomes et phéochromocytomes sont des tumeurs rares pouvant entraîner des symptômes tels qu’une hypertension artérielle (HTA) non contrôlée, des céphalées, des sueurs, des palpitations ou une constipation secondaires à une sécrétion excessive de catécholamines. La métyrosine (Demser®) vise à atténuer ces symptômes en limitant cette sécrétion, et bénéficie d’une autorisation temporaire d’utilisation en France. Son efficacité clinico-biologique ainsi que ses effets secondaires ont peu été étudiés.

14. Opening the black box of conservation philanthropy:A co-produced research agenda on private foundations in marine conservation

Abstract

In the ‘new Gilded Age’ of mega-wealth and big philanthropy, academics are not paying enough attention to private foundations. Mirroring upward trends in philanthropy broadly, marine conservation philanthropy has more than doubled in recent years, reaching virtually every globally salient marine conservation issue in all corners of the planet. This paper argues that marine conservation philanthropy warrants a dedicated research agenda because private foundations are prominent, unique, and under-studied actors seeking to shape the future of a “frontier” space. We present a co-produced social science research agenda on marine conservation philanthropy that reflects the priorities of 106 marine conservation donors, practitioners, and stakeholders who participated in a research co-design process in 2018. These “research co-designers” raised 137 unique research questions, which we grouped into five thematic research priorities: outcomes, governance roles, exits, internal foundation governance, and funding landscape. We identify issues of legitimacy, justice, and applied best practice as cross-cutting research priorities that came up throughout the five themes. Participants from the NGO, foundation, and government sectors identified questions within all five themes and three cross-cutting issues, underscoring shared interest in this work from diverse groups. The research we call for herein can inform the practice of conservation philanthropy at a time when foundations are increasingly reckoning with their role as institutions of power in society. This paper is broadly relevant for social and natural scientists, practitioners, donors, and policy-makers interested in better understanding private philanthropy in any environmental context globally.

Le cancer parathyroïdien est une tumeur endocrinienne rare (<0,5 % des hyperparathyroidie) caractérisée par un néoplasme malin dérivé des cellules parathyroïdiennes. Les signes cliniques sont liés à une sécrétion excessive de PTH ou à l’envahissement tumoral avec : hypercalcémie marquée, atteinte rénale, atteinte osseuse, paralysie récurrentielle. La tumeur peut se développer de manière sporadique ou peut être d’origine génétique (mutation germinale CD73).

Les PE sont des polluants environnementaux omniprésents qui, de par leur similitude avec nos hormones, peuvent stimuler ou bloquer nos récepteurs hormonaux et engendrer de nombreuses pathologies [1]. La grossesse et l’enfance sont des périodes particulièrement sensibles aux effets néfastes des PE. Dans ce contexte, notre établissement de santé a reçu un financement par l’ARS afin de former le personnel soignant, sensibiliser les patients et évincer au maximum les PE dans notre maternité.

Abstract

Les myélolipomes surrénaliens sont des tumeurs bénignes composés de graisse et d’éléments myéloïdes et représentent 3,3 à 6,5 % des masses surrénaliennes. Ils sont unilatéraux dans 95 % des cas avec une taille moyenne de 2–4cm [1]. Le diagnostic est scanographique avec la présence de graisse macroscopique. Nous présentons ici le cas d’un patient avec des myélolipomes surrénaliens bilatéraux de 10 et 11cm ayant présenté un saignement actif.

The carbon emissions from a new home do not begin from the day the first tenant moves in – the construction of new housing involves a substantial amount of energy due to the products and materials used in their construction, and it is argued by some that this ‘embodied carbon’ needs to be taken into account in any decision concerning the removal of older housing and replacement with new. Other factors such as the loss of heritage and communities also come into play, along with concerns over the demand for natural resources and the relative volumes of waste associated with each option. Throw into the equation the day-to-day pressures on developers to realise profit quickly, with minimal costs and risks, and it becomes clear that this is going to be a subject with few straightforward answers. Abstract

The search for a cancer is part of the classical investigation of unexplained venous thrombosis. Arterial thrombosis associated with neoplasia is more rare. The authors report two cases in which arterial thrombosis was the final event of their malignant disease. The first case had abacterial thrombotic endocarditis and disseminated intravascular coagulation at the origin of multiple thrombotic complications. The initially unknown cancer was a pancreatic adenocarcinoma. The second case presented with acute occlusion of the iliac artery after ablation of a malignant melanoma. Despite embolectomy with a Fogarty catheter and effective anticoagulation, the thrombosis recurred several times at the same site. The clinical features and the mechanisms of these two cases suggestive of Trousseau's syndrome are discussed.
