1. A singular case of massive urethrorrhagia solved by transarterial embolization

Abstract

Abstract Background Urethrorrhagia is frequent in pelvic trauma, rarely due to traumatic injuries of internal pudendal artery branches. Case presentation Our aim is to underline the role of transarterial embolization in selected patients, as in this case in which a young man manifested urethral hemorrhage after high-energy motorcycle crash not associated with injuries of the inferior urinary tract. Multi-detector computed tomography (MDCT) showed pubic symphysis diastasis and perineal hematoma with pseudoaneurysm into the penis bulb. The first approach was conservative with perineal external compression and intravenous injection of tranexamic acid. Afterward, due to the decline of clinical conditions, we decided to perform a selective angiography, confirming the vascular injury of distal branches of both internal pudendal arteries with contrast agent extravasation into urethral bulb; endovascular embolization was performed with detachable micro-coils. The principal results were seen quickly, indeed urethrorrhagia arrested and hemoglobin values normalized. Erectile function was preserved at 6-months follow-up. Conclusions Endovascular embolization proved to be a minimally invasive therapeutic approach, clinically effective, with a low rate of complications and high probability to preserve erectile function.

Abstract

Structural investigations of receptor-ligand interactions on living cells surface by high-resolution Nuclear Magnetic Resonance (NMR) are problematic due to their short lifetime, which often prevents the acquisition of experiments longer than few hours. To overcome these limitations, we developed an on-cell NMR-based approach for exploring the molecular determinants driving the receptor-ligand recognition mechanism under native conditions. Our method relies on the combination of high-resolution structural and dynamics NMR data with Molecular Dynamics simulations and Molecular Docking studies. The key point of our strategy is the use of Non Uniform Sampling (NUS) and T1ρ-NMR techniques to collect atomic-resolution structural and dynamics information on the receptor-ligand interactions with living cells, that can be used as conformational constraints in computational studies. In fact, the application of these two NMR methodologies allows to record spectra with high S/N ratio and resolution within the lifetime of cells. In particular, 2D NUS [1H–1H] trNOESY spectra are used to explore the ligand conformational changes induced by receptor binding; whereas T1ρ-based experiments are applied to characterize the ligand binding epitope by defining two parameters: T1ρ Attenuation factor and T1ρ Binding Effect. This approach has been tested to characterize the molecular determinants regulating the recognition mechanism of αvβ5-integrin by a selective cyclic binder peptide named RGDechi15D. Our data demonstrate that the developed strategy represents an alternative in-cell NMR tool for studying, at atomic resolution, receptor-ligand recognition mechanism on living cells surface. Additionally, our application may be extremely useful for screening of the interaction profiling of drugs with their therapeutic targets in their native cellular environment.

Ocular symptoms and comfort, tear volume and stability, and ocular surface condition were assessed by Ocular Surface Disease Index (OSDI), 5-Item Dry Eye Questionnaire (DEQ5), tear meniscus height (TMH), non-invasive tear break-up time (NIBUT), and evaluation of ocular redness (OR). The assessment was performed before and after 15 days of use of the ATs in the 3 groups (BW-Ws, DD-Ws, and N-Ws).Results: No clear significant difference was noted in symptoms and signs between OPTOyalA and OPTOidro irrespectively of the group of people studied. ATs use for 15 days produced a significant improvement in DEQ5 and OR in DD-Ws (Δ=− 34%, p=0.006; Δ=− 23%, p< 0.001) and in N-Ws (Δ=− 21%, p=0.001; Δ=− 10%, p=0.006) but not in BW-Ws (Δ=− 5%, p=0.072; Δ=− 2%, p=0.257). No significant change was noted for TMH.Conclusion: In young and healthy subjects, the aqueous-supplementing effect of the ATs under consideration is more a rinsing and tear replacem ent effect than an increase in tear volume, and it produces an improvement of the eye redness and ocular symptoms. Abstract

We propose a Markovian model to understand how Italy's public sphere behaves on the green energy transition theme. The paper uses the example of solar photovoltaics as a point of reference. The adoption decision is assumed to be sequentially influenced by the network communication of each person or family and the payback period of the investment. We apply the model for a case study based on the evolution of residential PV systems in Italy over the 2006-2026 period. The baseline configuration of the model is calibrated based on the actual diffusion of residential PV in Italy from 2006 to 2020. The comprehensive analysis leads to a discussion of two interesting societal implications. (1) Cognitive biases such as hyperbolic discounting of individual future utility crucially influence inter-temporal green choices. (2) Individual green choices count for the effect that it has on the chances that other individuals will also make a choice, in turn abating other greenhouse gas emissions., Comment: 43 pages, 28 figures

Abstract

We construct a class of time-symmetric initial data sets for the Einstein vacuum equation modeling elementary configurations of multiple ``almost isolated" systems. Each such initial data set consists of a collection of several localized sources of gravitational radiation, and lies in a family of data sets which is closed under scaling out the distances between the systems by arbitrarily large amounts. This class contains data sets which are not asymptotically flat, but to which nonetheless a finite ADM mass can be ascribed. The construction proceeds by a gluing scheme using the Brill--Lindquist metric as a template. Such initial data are motivated in part by a desire to understand the dynamical interaction of distant systems in the context of general relativity. As a by-product of the construction, we produce complete, scalar-flat initial data with trivial topology and infinitely many minimal spheres, as well as initial data with infinitely many Einstein--Rosen bridges., Comment: 40 pages

Abstract

A timely and long-term programme of kaon decay measurements at a new level of precision is presented, leveraging the capabilities of the CERN Super Proton Synchrotron (SPS). The proposed programme is firmly anchored on the experience built up studying kaon decays at the SPS over the past four decades, and includes rare processes, CP violation, dark sectors, symmetry tests and other tests of the Standard Model. The experimental programme is based on a staged approach involving experiments with charged and neutral kaon beams, as well as operation in beam-dump mode. Abstract

This paper develops two mathematical models to understand subjects' behavior in response to the urgency of a change and inputs from governments e.g., (subsides) in the context of the diffusion of the solar photovoltaic in Italy. The first model is a Markovian model of interacting particle systems. The second one, instead, is a Mean Field Game model. In both cases, we derive the scaling limit deterministic dynamics, and we compare the latter to the Italian solar photovoltaic data. We identify periods where the first model describes the behavior of domestic data well and a period where the second model captures a particular feature of data corresponding to companies. The comprehensive analysis, integrated with a philosophical inquiry focusing on the conceptual vocabulary and correlative implications, leads to the formulation of hypotheses about the efficacy of different forms of governmental subsidies., Comment: 19 pages, 5 figures

Abstract

We previously demonstrated that consumption of U.S.-grown sugar kelp (Saccharina latissima) prohibits inflammation and fibrosis in the liver and white adipose tissue in male diet-induced obesity (DIO) mice. This study investigated whether sugar kelp can elicit similar benefits in female DIO mice. Male and female C57BL/6J mice were fed a low-fat (LF) control, a high-fat/high-sucrose/high-cholesterol (HF) control, or HF containing 6% sugar kelp (HF-Kelp) for 16 weeks. Body weight gain, serum triglyceride and total lipids, and glucose intolerance were decreased in male HF-Kelp compared with HF control. Furthermore, sugar kelp decreased gonadal white adipose tissue (gWAT) weight with smaller adipocytes and total body fat in males. In gWAT, male HF-Kelp exhibited less inflammation and fibrosis than the HF control, confirmed by histological analysis. Moreover, HF-Kelp increased physical activity only in males. Gut microbiome analysis showed sugar kelp elevated observed species and altered beta diversity in both males and females, suggesting changes in the gut microbiome may contribute to the effects of sugar kelp on gWAT inflammation and fibrosis. Therefore, our results demonstrate that sugar kelp consumption prevents obesity-induced inflammation and fibrosis in WAT in male mice, which is attributed, at least in part, to modest increases in metabolic rates, energy expenditure, and physical activity, and also modulating the gut microbiota.

Abstract

Abstract Measurement of the ultra-rare K + → π + ν ν ¯ $$ {K}^{+}\to {\pi}^{+}\nu \overline{\nu} $$ decay at the NA62 experiment at CERN requires high-performance particle identification to distinguish muons from pions. Calorimetric identification currently in use, based on a boosted decision tree algorithm, achieves a muon misidentification probability of 1.2 × 10 −5 for a pion identification efficiency of 75% in the momentum range of 15–40 GeV/c. In this work, calorimetric identification performance is improved by developing an algorithm based on a convolutional neural network classifier augmented by a filter. Muon misidentification probability is reduced by a factor of six with respect to the current value for a fixed pion-identification efficiency of 75%. Alternatively, pion identification efficiency is improved from 72% to 91% for a fixed muon misidentification probability of 10 −5.
