The regional key comparison of absolute gravimeters, EURAMET.M.G-K3 and the simultaneously organized additional comparison, was held in Germany at the Geodetic Observatory Wettzell of the German Federal Agency for Cartography and Geodesy in the spring of 2018. Here we present the list of the participants who actually performed measurements during the comparison, the data submitted by the operators as well as the results of the determination of the gravity as a function of height at the comparison sites. The measurement strategy is briefly discussed and the results of the data harmonization is documented. Finally, the results of the constrained least squares adjustment are presented including the degrees of equivalence of each gravimeter and the key comparison reference values. A blend of mannanoligosaccharide and Saccharomyces cerevisiae mixture was used as symbiotic. Two initial body weight groups (L= Light and H= Heavy chicks) and two feeds (B= basal diet and S= 0,2% symbiotic supplemented diet) were tested in 2x2 factorial experimental design. One hundred and twenty newly hatched chicks obtained from a healthy broiler parent stock aged 40 wk old. Each treatment group had 3 replicates consist of 10 chicks. Trial lasted for 21 days. Daily body weight gain (DBWG), daily feed intake (DFI), feed conversion ratio (FCR) (g feed/g gain) was recorded weekly. The DBWG, DFI, FCR and digestive tract development (heart, liver, gizzard, proventriculus, pancreas, bursa fabricius weight and gut length) were not affected by treatments. Symbiotic supplementation increased villi length. Symbiotic supplementation decreased pathogenic microorganisms (Escherichia Coli, Coliform and Enterobactericaea) in caeca irrespective to the DBWG of chicks. To conclude, symbiotic supplementation can be used to improve villi morphology and to suppress pathogenic microorganisms in caeca.

The aim of the present study is to determine the frequency of the occurrence of supraorbital foramen/notch (SOF/N) in the skulls of the people who lived in the modern era and the late Byzantine era, to determine the symmetry and the asymmetry between the two halves of the skulls by measuring the linear distance to various landmarks, to check the consistency between the location of the SOF/N and the golden ratio by calculating the ratio between linear distances and to evaluate the differences between the skulls from both historical periods. In the study, the frequency of the occurrence of the supraorbital notch in the skulls from the Byzantine era was found to be 26.60 % on the right and 13.30 % on the left while it was 14.30 % on the right and 9.52 % on the left in the skulls belonging to modern humans. In the skulls belonging to the Byzantine era, the average distance between SOF/N and the sagittal axis passing along the lateral orbital wall was found to be 34.81±2.51 mm and 32.99±2.81 mm respectively on the right and the left while it was 33.14±2.19 mm and 33.39±2.06 mm in the skulls belonging to modern era. The average distance between the SOF/N and the sagittal plane passing along the midline of the skull was found to be 24.55±2.79 mm and 21.57±2.44 mm on the right and the left respectively in the skulls belonging to the Byzantine era while it was 0.04±3.30 mm and 20.96±2.37 mm in the skulls belonging to the modern era. The average distance between the SOF/N and sagittal plane passing along the medial orbital wall was found to be 23.78±3.60 mm and 23.81±3.20 mm on the right and the left respectively in the skulls belonging to the Byzantine era while it was found to be 22.23±3.29 mm and 23.97±1.93 mm in the skulls belonging to the modern era. The average value of the distance between the sagittal planes passing along the lateral and medial sides of the orbit and the ratios between the distance from SOF/N to the sagittal plane passing along the lateral side of the orbit was found to be 1.47±0.21 mm and 1.60±0.08 mm respectively in the skulls belonging to the Byzantine era and the modern era. No significant difference was found between this ratio and the golden ratio; the average value of the ratios between the distance from the SOF/N to the sagittal plane passing along the midline of the skull and the distance from the SOF/N to the sagittal plane passing along the lateral side of the orbit was found to be 0.98±0.26 mm and 1.04±0.36 mm respectively in the skulls belonging to the Byzantine era and the modern era. The aim of this study was to examine the morphometric characteristics of the infraorbital foramen of skulls of people living in modern society and in the late Byzantine period, to ascertain the symmetry or asymmetry of the two halves of the skulls by measuring the linear distance between various landmarks, to evaluate at the conformity between the infraorbital foramen and the golden ratio by calculating the ratios between these linear distances, and to set out the differences or similarities between the skulls of these different periods. It was found in the study that the morphometric characteristics of the infraorbital foramen in skulls of the modern period were 47.05 % circular, 41.17 % oval and 11.76 % atypical (semilunar and triangular) on the right, and 70.58 % circular and 29.41 % oval on the left, while those of the Byzantine period were 46.06 % circular and 53.3% oval on the right, and 50% circular and 50 % oval on the left. It was found that the measurements across the infraorbital foramen of the Byzantine skulls averaged 2.93±1.05 mm and 3.15±1.03 mm on the right side and 2.62±0.97 mm and 3.16±0.68 mm on the left vertically and horizontally respectively, and those of the modern period measured 2.32±0.50 mm and 3.00±0.92 mm on the right and 2.48±0.45 mm and 2.76±0.65 mm on the left vertically and horizontally respectively. The ratio between the distances from the outer orbital wall of the IOF to the sagittal plane and the piriform aperture were 1.46±0.25 mm and 1.40±0.21 mm for left and right respectively for the Byzantine skulls, and 1.24±0.24 mm and 1.29±0.42 mm for the modern skulls. In this study, a training set consisting of servo motors is designed to assist development of the students, especially who are in electrical, electronics and mechanical engineering, in industrial field. For this training set; three AC servo motors and one DC servo motor are used and four-axis control mechanism is generated. In order to control servo motors a special driving circuit is developed through which position and speed is controlled. These controls can be managed either via operating panel (touch panel) or via computer depending on user's purpose. In addition, a local network based training software is developed, which involves theoretical information regarding servo motors and allows testing opportunity and observing animations. Thus, a practical, functional and also economical training set allowing students both to learn and to perform many applications that they might be introduced with industrial field is developed. This study was conducted to determine the effects of liquid or dietary supplementation of sourdough on quail performance, composition of caecal microbiota and ileum histomorphology. A total of 120 healthy 14 d old quail (Coturnix cot. Japonica) were divided into three dietary treatments with 4 replicates each including 10 mixed sex quail (5 males and 5 females). Treatments were 1) Control, 2) liquid sourdough (4 ml/l) in drinking water, 3) dried sourdough (5 g/kg) in experimental diet. The experiment lasted 21 days. Live weight gain (LWG), feed intake (FI), and feed conversion ratio (FCR) recorded weekly. Caecum microbiota and ileum histomorphology were determined at the 21st day of the experiment. Results showed that the dietary sourdough supplementation increased LWG and FCR (P<0.05). FI was not affected by any treatments. Dietary sourdough supplementation decreased liver weight (P<0.05). Both dietary and liquid sourdough supplementation increased the colonisation from 5.74 to 7.60 and 6.90 cfu g-1 of yeast in the caecum respectively (P<0.05). Lactic acid bacteria (LAB) count tended to increase without any statistical difference. The colonisation of Enterobactericaea, E. coli and Coliforme bacteria were not affected by any treatments, but Coliformes counts tended to decrease. Dietary sourdough supplementation increased villi length and villi width compared to control and liquid sourdough supplemented group (P<0.05). To conclude, sourdough showed a probiotic effect and enhanced growth performance by affecting caecum microflora and increasing ileum villi length when given with the diet. In this study we discuss a rare case; hydatid cyst disease that is located in the retroperitoneum. Two cases were treated in our department with hydatid cyst disease located in the retroperitoneum; one of the cysts was in the minor pelvis and the other was on the left psoas muscle. Diagnosis was confirmed with blood tests, and radiological examinations. The cyst wall was excised partially and was oversewn. Following the operation both of the patients were given albendasole tablets for 10 weeks with blood count and liver enzyme monitorization. Hydatic cysts located in retroperitoneum were treated successfully. Especially in the endemic areas hydatid cyst should be remembered when evaluating cystic masses in the retroperitoneum. It can be treated successfully with surgery.

23. The importance of intraoperative cholangiography during laparoscopic cholecystectomy

Laparoscopic cholecystectomy (LC) using an electrosurgery energy source was successfully performed in 59 (95%) out of 62 selected patients. The procedures were performed by different surgical teams at Trakya University, Medical Fakulty, in the department of General Surgery and the Karl-Franzens-University School of Medicine, in the department of General Surgery. Cholangiography was routine at Karl Franzens University and selective at Trakya University. Laparoscopic intraoperative cholangiography (IOC) was performed in 48 (81.3%) patients, and open IOC was performed in 3 patients. Two patients had common duct stones; one of which was unsuspected preoperatively. These cases underwent endoscopic retrograde cholangiopancreatography (ERCP) and endoscopic papillotomy (EP). One patient had a choledocal tumor, unsuspected preoperatively. Anatomical anomalies were not identified. Cholangiography could not be performed in one case in which there was no suspected pathology. ERCP was performed on one patient 30 days after being discharged because of acute cholangitis. In this case, residual stones were identified in the choledocus. Four patients underwent open cholecystectomy because of tumor, unidentified cystic duct or common bile duct pathology that could not be visualized on the cholangiogram. Our study suggests that cholangiography performed via the cystic duct before any structures are divided can prevent the most serious complication of laparoscopic cholecystectomy – common duct injury. We recommend that cholangiography be attempted on all patients undergoing LC.

The aim of this experiment was to determine the effects of in ovo injection of dried whey and Enterococcus faecium to fertile Ross 308 chicken eggs on performance, ileal histomorphology, ileum microbiota and edible viscera weights. A 2×2 factorial design was used. Fertile Ross 308 eggs were injected with 4 different solutions: A) control solution (distilled water), B) distilled water + 4% dried whey, C) distilled water + Enterococcus faecium, D) distilled water + 4% dried whey and Enterococcus faecium. Live weight, feed consumption, feed conversation ratio and ileal histomorphology, ileum microbiota, organ weight were recorded at 21 days of age. Live weight gain, feed consumption and feed conversion ratio were not affected by the treatments. At the end of the experiment, lactic acid bacteria (LAB) colonisation in ileum were 3.68, 4.81, 4.04 and 5.92 cfu/g respectively and the highest LAB colonisation was in the combined injection group. At the end of the experiment, villus height was 455, 484, 438 and 542 µm respectively and the highest villus height was found in the combined injection group. Although broiler performance was not different among the groups, a combined injection of Enterococcus faecium and dried whey provided better LAB colonisation and increased villus height in ileum. In conclusion, a combined injection of Enterococcus faecium and dried whey has a symbiotic effect on ileal histomorphology and gut microbiota. However, further studies should be conducted to determine the effects of in ovo injection of different probiotic and/or symbiotic microorganisms and different stress factors on broiler performance. - Abstract
Purpose: To evaluate the results of stent placement for obstructive atherosclerotic aortic disease with or without involvement of the common iliac artery.Materials and Methods: Forty patients had self-expanding stents primarily or after balloon dilatation in the abdominal aorta between January 2005 and May 2011. All patients had trouble walking. Follow-up examinations were performed with clinical visits; these included color Doppler ultrasonography and computed tomographic angiography.Results: Technical, clinical, and hemodynamic success was achieved in all patients. None of the patients underwent reintervention during the follow-up period, which ranged from 3 months to 6 years (median 24 months). Nine complications occurred in six patients. Of the nine complications, four were distal thromboembolisms, which were successfully treated with catheter-directed thrombolysis or anticoagulation therapy.Conclusion: Endovascular treatment of the obstructive aortic disease using self-expanding stents was safe and effective, with high technical success and long-term patency. Thromboembolic complications were high even though direct stenting was considered protective for thromboembolism formation. Particularly for infrarenal aortic stenosis, it can be recommended as the first-line treatment option for patients with obstructive atherosclerotic aortic disease. [ABSTRACT FROM AUTHOR]- Published

34. Relation of serum cortisol to delirium occurring after acute coronary syndromes.

- Abstract
BACKGROUND: Delirium can be associated with cardiac system disorders. Stress plays an important role in the pathogenesis of postoperative delirium. Cortisol is one of the most important stress hormones in humans. We aimed to investigate whether a relation exists between serum cortisol and the degree of delirium after acute coronary syndromes (ACS). METHODS: We enrolled 52 consecutive patients who presented with ACS and were hospitalized in the coronary care unit. Patients were examined daily by a single psychiatrist, and delirium was diagnosed by using the Delirium Rating Scale (DSR). Blood samples were obtained at 6:00 am of the next morning after admission. RESULTS: The mean age was 66 years (SD, ±6 years), and 52% were men. Delirium occurred in 25 patients (48%). The median score on the DRS was 17 for the delirious patients and 5 for the nondelirious. Median cortisol levels were significantly different between the delirium and nondelirium groups (13.9 vs 6.2 [mu]g/dL; P < .01). There were significant correlations between the cortisol levels and the severity of the delirium based on DRS scores as well as between the cortisol levels and the presence of delirium (r = 0.65 and 0.74, respectively; P = .01). In a linear logistic regression model, cortisol predicted the occurrence of delirium ([beta] = .81; P < .01). In receiver operating characteristics analysis, the optimal cutoff value of cortisol to predict delirium was 10.8 [mu]g/dL, with 96% sensitivity and 89% specificity. CONCLUSION: Delirium was common after ACS, and serum cortisol levels correlated with the degree of delirium and the risk of delirium. [ABSTRACT FROM AUTHOR]

- Abstract
Specialty cut flowers are in demand, especially in the domestic market as they can be grown with low production costs without the need for specially equipped greenhouses and offer diversity in terms of form, texture, and colour. These products, which are widely cultivated in the USA, are not well known in Türkiye. One of the main problems of the Turkish cut flower sector is its dependence on foreign inputs and the lack of product diversity. Therefore, specialty cut flower production can be an alternative crop for Türkiye, which has climatic advantages. The cut flower potential of plants such as Zinnia elegans, Tagates erecta, Helianthus annuus, Gomphrena globosa, Centaurea cyanus , and Cleome spinosa that are commonly grown in gardens has been evaluated. After harvesting these flowers grown in an open field in June-October 2020, the stem length (cm), stem thickness (mm), flower length (cm), flower diameter (cm), flower weight (g), and vase life (days) were measured. As a result of the evaluations, considering the phenological findings such as flowering and harvesting period, Zinnia elegans, Tagates erecta, Helianthus annuus, Gomphrena globosa, and Centaurea cyanus were found to be suitable for Eskişehir climatic conditions due to their long flowering periods. On the other hand, if the stem length value, which is one of the most important parameters for cut flowers, is taken as a reference, the minimum stem length value of 30 cm and above is met by Zinnia elegans , Tagates erecta , Helianthus annuus , and Cleome spinosa while the vase life value of 6 days and above is met by Zinnia elegans , Tagates erecta , Helianthus annuus , Gomphrena globosa , and Cleome spinosa . However, Cleome spinosa was not found to be suitable for the region due to its low yield value and short flowering period, while Zinnia elegans, Helianthus annuus, Tagates erecta , and Gomphrena globosa were found to be plants that could be evaluated for the region. In addition, it is believed that the cultivation of specialty cut flowers, with the selection of suitable species, will be an alternative production in regions without climatic advantages., Competing Interests: The authors declare that they have no competing interests. - Abstract
The present work investigated the effects of dietary incorporation of laurel (Laurus nobilis) essential oil on the zootechnical performance and digestive physiology of juvenile Black Sea salmon (Salmo labrax). In this trial, 15 fiberglass tanks (39 × 39 cm square and 33 cm high) were used. Forty-five fish (3.52 ± 0.01 g) were placed randomly per tank. Fish were fed for 90 days with the diet containing 50, 100, 200, or 400 mg kg
-1 laurel (Laurus nobilis) essential oils, respectively. The work was performed in the recirculating aquaculture system (RAS) operating with freshwater. Fish were manually fed 3% level of live weight during the experiment period. Final weight (FW), weight gain (WG), feed conversion rate (FCR), and specific growth rate (SGR) were not affected by laurel essential oil supplementation. Dietary laurel essential oil (50 mg kg-1 ) affected positively the surface area of fish intestinal villus that required for digestion. Both villus height and villus width were affected positively in fish fed with 50 mg laurel essential oil kg-1 . While incorporation with 100 mg laurel essential oil kg-1 increased the total α-amylase enzyme, 50 mg laurel kg-1 increased lipase enzyme. Moreover, 50 mg laurel essential oil kg-1 increased lactic acid bacteria (LAB) count in fish. Besides, 50 mg laurel essential oil kg-1 reduced the number of total coliform and E. coli., (© 2022. Ackground: There are very few studies in the literature focusing on whether dexmedetomidine exerts a protective effect on colistin nephrotoxicity. Our study aims to investigate the nephroprotective effect of dexmedetomidine in an experimental model of nephrotoxicity in rats., Methods: The control group was administered saline (SF) intraperitoneally twice a day. The colistin group received an intraperitoneal (ip) injection of 10 mg/kg of colistin twice a day. The DX10 group received 10 mg/kg of colistin 20 minutes after the intraperitoneal injection of 10 mcg/kg of dexmedetomidine. The DX20 group received 10 mg/kg of colistin 20 minutes after the intraperitoneal injection of 20 mcg/kg of dexmedetomidine. Applications were continued for 7 days, twice a day. All rats were sacrificed on the 8th day after blood and kidney tissue samples were taken. BUN, Creatine, KIM-1 and Endothelin-1 were studied in blood samples., Results: There was a significant difference in the median values of Urea, BUN and Creatine between the groups (p<0.001, p<0.001, p<0.001, respectively). There was a significant difference in the median values of KIM-1 and Endothelin-1 between the groups (p=0.009, p=0.001, respectively). A significant difference was observed between the histopathological scores of the groups (p<0.001)., Conclusion: Dexmedetomidine significantly decreased the elevated levels of BUN, Creatinine, KIM-1, and Endothelin-1 induced by colistin. Dexmedetomidine, at both doses, histopathologically prevented apoptosis and reduced the number of necrotic cells in the kidneys. Dexmedetomidine provides renoprotective effects, therefore it is a valuable sedation agent for clinicians working in intensive care units (Tab. 2, Fig. 4, Ref. 19). The present study was conducted to investigate the effects of raw (PP) and fermented pomegranate pomace (FP) on performance, antioxidant activity, caecal microbiota and ileal morphology in broiler chickens. A total of 175 male broiler chicks were allocated to five treatment groups with five replicates and seven birds per replicate in a completely randomised design. Dietary treatments included a soy-corn based diet (control), diets supplemented with PP at 5 (5PP) and 10 g/kg (10PP), and diets supplemented with FP at 5 (5FP) and 10 g/kg (10FP). Dietary PP and FP did not change the body weight and feed conversion ratio. Moreover, dietary PP and FP did not alter the serum glutathione peroxidase, superoxide dismutase, and catalase levels but decreased malondialdehyde ( p < 0.05) in breast meat. Caecal Clostridium perfringens count was decreased in broiler chickens of groups 10PP, 5FP and 10FP ( p < 0.05). However, PP and FP had detrimental effects on the ileum morphology of broiler chicks. The villus height was decreased in the 10PP, 5FP and 10FP groups compared with the control group ( p < 0.01). Crypt depth was higher in the 5PP and 10FP groups than control and 10PP groups ( p < 0.01). The villus height to crypt depth ratio was also decreased in 5PP, 5FP, and 10FP groups ( p < 0.01). These results suggest that PP and FP have the potential to be used in broiler diets as antioxidant and antimicrobial agents. However, detailed studies should be conducted to investigate the underlying reasons for the detrimental effects on ileal morphology.

- Abstract
Background The extent of pressure-related damage might be related to acceleration rate of the applied pressure (peak dP/dt) in the vascular system. In this study, we sought to determine whether dP/dt applied to the aortic wall (aortic dP/dt) and in turn vascular extracellular matrix degradation can be mitigated via modulation of left ventricular (LV) contractility (LV dP/dt) by pacemaker-mediated desynchronization. Methods and Results First, in 34 patients, changes in aortic dP/dt values in 3 aortic segments in response to pacemaker-mediated stepwise QRS widening leading to gradual desynchronization of the LV contraction by means of steadily changed atrioventricular delay (AVD) with temporary dual-chamber pacing was examined before and after beta-blocker (15 mg IV metoprolol) administration. Second, serum matrix metalloproteinase-9 levels were measured in the 20 patients with permanent pacemaker while they were on sinus rhythm with normal QRS width and 3 weeks after wide QRS rhythm ensured by dual pacing, dual sensing, and dual response to sensing with short AVD. LV dP/dt substantially correlated with dP/dt measured in ascending ( r =0.83), descending ( r =0.89), and abdominal aorta ( r =0.96). QRS width strongly correlated with dP/dt measured in ascending ( r =-0.95), descending ( r =-0.92), and abdominal ( r =-0.96) aortic segments as well. In patients with permanent pacemaker, wide QRS rhythm led to a significant reduction in serum matrix metalloproteinase-9 levels (from 142.5±32.9 pg/mL to 87.5±32.4 pg/mL [ P <0.001]) at the end of 3 weeks follow-up. Conclusions QRS prolongation by short AVD dual pacing, dual sensing, and dual response to sensing results in concomitant decreases in peak dP/dt values in the LV and in all aortic segments with or without background beta-blocker administration, which in turn led to a significant reduction in circulating matrix metalloproteinase-9 levels. Objective: To evaluate the sedative efficacy and effects on early period cognitive function of premedication in endoscopic retrograde cholangiopancreatography (ERCP)., Methods: Forty patients (18-70 years; American Society of Anesthesiology risk category I-III) undergoing elective ERCP were randomized to receive oral premedication with 0.15 mg/kg midazolam or 1 mg/kg tramadol. Cognitive function was determined by mini-mental test (MMT). Target scores for effective sedation were determined as a Bispectral index score of 70-90 and modified Ramsay Sedation Scale score (mRSS) of 2-4., Results: Global MMT score was not significantly different between treatment groups at 60 min post-ERCP. A significant deterioration in the MMT subcategory of recall was determined in with midazolam versus tramadol. Level of sedation (mRSS) was higher in with midazolam compared with tramadol reaching statistical significance at 30 min after drug administration., Conclusions: Although more effective sedation was obtained with midazolam in patients undergoing ERCP, there was a dysfunction in memory recall. 45. Predictors of in-hospital mortality following redo cardiac surgery: Single center experience.

- Abstract
Purpose: Redo cardiac operations represent one of the main challenges in heart surgery. The purpose of the study was to analyze the predictors of in-hospital mortality in patients undergoing reoperative cardiac surgery by a single surgical team., Methods: A total of 1367 patients underwent cardiac surgical procedures and prospectively entered into a computerized database. Patients were divided into 2 groups based on the reoperative cardiac surgery (n = 109) and control group (n = 1258). Uni- and multivariate logistic regression analysis were performed to evaluate the possible predictors of hospital mortality., Results: Mean age was 56 ± 13, and 46% were female in redo group. In-hospital mortality was 4.6 vs. 2.2%, p = 0.11. EuroSCORE (6 vs. 3; p < 0.01), cardiopulmonary bypass time (90 vs. 71 min; p < 0.01), postoperative bleeding (450 vs. 350 ml; p < 0.01), postoperative atrial fibrillation (AF) (29 vs. 16%; p < 0.01), and inotropic support (58 vs. 31%; p = 0.001) were significantly different. These variables were entered into uni- and multivariate regression analysis. Postoperative AF (OR1.76, p = 0.007) and EuroSCORE (OR 1.42, p < 0.01) were significant risk factors predicting hospital mortality., Conclusions: Reoperative cardiac surgery can be performed under similar risks as primary operations. Postoperative AF and EuroSCORE are predictors of in-hospital mortality for redo cases.

Purpose: Popliteal artery aneurysms (PAAs) can be treated successfully by surgical and endovascular methods; however, the best treatment strategy for a ruptured PAA has yet to be established. We assessed the clinical results of using saphenous vein interposition to treat ruptured PAAs in our hospital., Methods: The subjects of this study were seven men (average age 59 years, range 43-71 years), who underwent emergency surgery for a ruptured PAA at our hospital between January 2007 and November 2012. The patients were assessed after 1, 6, and 12 months, postoperatively., Results: All included patients underwent saphenous vein graft interposition via a medial approach. No complications or graft thromboses were encountered in the immediate postoperative period. The patients were discharged after an average of 4 days postoperatively (range 3-5 days). The patients were followed up for an average of 32 months (range 2-60 months). The medium-term graft patency was 100%. No patients suffered early or medium-term limb loss and there was no mortality., Conclusion: Based on our positive results, saphenous vein graft interposition should be considered as the first choice of surgical treatment for a ruptured PAA.

- Abstract
We retrospectively compared the results of conventional coronary artery bypass grafting (CABG) performed on patients who showed no preoperative evidence of serious atherosclerosis of the ascending aorta with the results of the aortic no-touch technique (using coronary artery-saphenous vein composite grafts) on CABG patients who did show such evidence. From 2003 through 2012, 3,152 consecutive patients underwent isolated primary CABG at our hospital. We chose 360 for the current study. The study group (n=120) comprised patients who had undergone operation via the aortic no-touch technique. Propensity-score-matching (1:2) was used to select the control group of 240 patients who had undergone conventional CABG. Early and late survival rates, reintervention-free survival rates, and freedom from cardiac death were compared. Early and late mortality rates were similar in the study and control groups (P=0.19 vs P=0.29, respectively), as were cardiac-related death (2.5% vs 2.1%, respectively; P=0.53) and overall death (8.3% vs 7.9%, respectively; P=0.51). Overall survival rates were 91.7% vs 92.1% and freedom-from-cardiac-death rates were 97.4% vs 97.5% (P=0.71 vs P=0.78, respectively; mean follow-up period, 5.27 ± 2.51 yr). Reintervention-free survival rates were also similar (96.7% vs 98.8%, respectively; P=0.2). As a result of the similar rates of early and late survival, reintervention-free survival, and freedom from cardiac death, we conclude that the aortic no-touch technique with composite grafts might be a reasonable option in patients who have atherosclerotic ascending aorta that cannot be clamped.

- Abstract
Objective: Arteriovenous fistula (AVF) in patients undergoing hemodialysis (HD) may cause coronary left internal mammary artery (IMA) steal. This phenomenon was demonstrated by few prospective studies with limited number of patients and case reports. We aimed to demonstrate with a relatively larger patient population that the AVF may cause ipsilateral IMA steal., Methods: We included 22 prospective patients undergoing HD who had left IMA to left anterior descending artery graft and left upper limb AVF. Right IMA was taken as control. Flows were assessed by using color Doppler ultrasonography., Results: The mean age was 57.8 ± 9 years. Statistically nonsignificant increases in AVF flow and decreases in left IMA flow were observed during HD compared with pre-HD. Moreover, fistula localization did not affect median left IMA flows (for peak systolic velocity [PSV] 43.7 versus 70 cm/s, respectively; p = 0.7, and for end diastolic velocity [EDV] 3.4 versus 6.5 cm/s, respectively; p = 0.7). We have not detected significant difference in left IMA flows during HD (median values of PSV 58.4 versus 68.4 cm/s, respectively; p = 0.1, and EDV 6.4 versus 4.4 cm/s, respectively; p = 0.08). Only three patients experienced dialysis-induced reduction of ipsilateral IMA flow that was accompanied by evidence of hypokinesia of the anterior left ventricular wall without clinical angina., Conclusions: Hemodynamically affected left IMA flow by ipsilateral upper extremity AVF may cause steal phenomenon. - Abstract
Background: A neurologic event (NE) after coronary artery bypass graft (CABG) surgery is a principal complication affecting morbidity and mortality. We investigated the risk factors predicting postoperative NEs and survival in patients with pre-existing NE undergoing CABG., Methods: Between 2001 and 2012, 3137 patients underwent isolated primary CABG. The data were stored in a computerized database and retrospectively evaluated. Patients were divided into 2 groups based on the existence of preoperative NE (study group; n=126) and without NE (n=3011). Uni- and multivariate logistic regression analyses were performed to evaluate the possible predictors of postoperative NEs. Survival was determined using Kaplan-Meier survival analyses of the study group and propensity score-matched control group., Results: The mean age of the 3137 patients was 60±9 years, and 28% (n=885) were female. Postoperative NE was seen in 4 (3.2%) patients with pre-existing NE and in 16 (0.5%) without pre-existing NE. Preoperative NE (odds ratio 4.47; 95% confidence interval 1.32-15.18; P=.01) and age (≥70 years; odds ratio 2.98; 95% confidence interval 1.21-7.33; P=.01) showed strong multivariate associations with postoperative NE. Median CHA2DS2-VASc scores were 4.5 (interquartile range 3-5) and 3 (range 0-4) in patients in the pre-existing and without NE groups, respectively (P=.01). The overall mean follow-up was 4.6±3 years. Overall survival rates (88.1% and 94%, respectively) were different between the 2 groups (P=.02)., Conclusions: Preoperative neurologic events and advanced age are significant risk factors predicting postoperative neurologic events. Meticulous management of the ascending aorta and carotid artery are important in diminishing postoperative neurologic events. - Abstract
Aspirin is one of the preferred therapies in the primary prevention of ischemic stroke in paroxysmal atrial fibrillation (PAF). Mean platelet volume (MPV) is a marker of platelet size and activation. Increased MPV reflects active and large platelets. The present observational study was designed to investigate whether aspirin treatment does affect MPV levels in patients with PAF. The study included 101 patients who were detected to have PAF by 24-hour Holter monitoring and divided into two groups based on aspirin treatment [ASA (+) and ASA (-)]. MPV was measured. Patients with aortic and mitral stenosis, hyperthyroidism, hypothyroidism, malignancy, infection, and pregnancy were excluded from the study. Of the 101 patients, 50 had no antiplatelet therapy and 51 had daily aspirin (100 mg) intake. Mean age of the patients was 66 ± 10 years and 35 (68%) were male in ASA (+) group. There was no difference in median levels of MPV (9.9 vs. 10.2 fl, respectively; p = 0.9) between groups. Both uni- and multivariate logistic regression analyses did not show an association between MPV and ASA use. Our results indicate that MPV as a predictive marker of platelet size and activity is not affected by aspirin use in patients with PAF.
