9. Structure, function and plasticity of convergent synapses in reinnervated muscle

Abstract

The experiments described in this thesis were directed towards clarifying the role of activity in sculpting patterns of innervation at adult reinnervated rat NMJs. Two hypotheses were tested: first, that activity is sufficient for synapse elimination, and second, that activity is necessary for synapse elimination. To test the first hypothesis, 4DL muscles were subjected to partial denervation and allowed to recover for at least 2 weeks from a period of paralysis. Polyneuronally innervated 4DL NMJs were examined to ask whether terminals contributing relatively small fractional occupancies of the endplate (<49% of total endplate area) were disproportionately weak and, in fact, in the process of being eliminated from endplates. Polyneuronally innervated endplates were identified with the vital dyes FM1-43 and RH414. Simultaneous intracellular recording and synaptic imaging were used to correlate input strength with size. To test the second hypothesis, 4DL muscles were subjected to partial denervation and paralysis, but were not allowed to recover from the activity block. Vital staining and immunocytochemistry revealed that competitive synapse elimination had taken place at some endplates, even when evoked and spontaneous activity was abolished throughout the muscles. These results show that activity, though influential, is neither sufficient nor necessary for the competitive elimination of synapses in reinnervated muscle. Thus, polyneuronal innervation may now be viewed as one of a number of stable states of innervation of skeletal muscle fibres. Further, the data suggest that the timing of transmitter-receptor activation is not the only mechanism by which inputs are removed during neuromuscular synapse elimination. Alternative mechanisms that may be operating during the competitive reinnervation of skeletal muscle will be discussed.

11. COVID‐19‐Associated Chemosensory Loss Continues to Decline.

Abstract

Chemosensory losses have long been considered a cardinal symptom of COVID‐19 infection. Recent studies have shown changing symptom profiles with COVID‐19, including decreasing incidence of olfactory losses. We accessed the National COVID Cohort Collaborative database to identify patients with and without smell and taste loss within 2 weeks of COVID‐19 diagnosis. Peak prevalence time intervals for variants were determined from Covariants.org. Using rates of chemosensory loss during the peak time interval for "Untyped" variants as baseline (4/27/2020‐6/18/2020), odds ratios for COVID‐19‐associated smell or taste disturbance fell for each of the Alpha (0.744), Delta (0.637), Omicron K (0.139), Omicron L (0.079), Omicron C (0.061), and Omicron B (0.070) peak intervals. These data suggest that during the recent Omicron waves and potentially moving forward, the presence or absence of smell and taste disturbances may no longer have predictive value in the diagnosis of COVID‐19 infection. Abstract

Spinocerebellar ataxia (SCA) type 7 (SCA7) is caused by a CAG trinucleotide repeat expansion in the ataxin 7 (ATXN7) gene, which results in polyglutamine expansion at the amino terminus of the ATXN7 protein. Although ATXN7 is expressed widely, the best characterized symptoms of SCA7 are remarkably tissue specific, including blindness and degeneration of the brain and spinal cord. While it is well established that ATXN7 functions as a subunit of the Spt Ada Gcn5 acetyltransferase (SAGA) chromatin modifying complex, the mechanisms underlying SCA7 remain elusive. Here, we review the symptoms of SCA7 and examine functions of ATXN7 that may provide further insights into its pathogenesis. We also examine phenotypes associated with polyglutamine expanded ATXN7 that are not considered symptoms of SCA7.

Abstract

The literature regarding clinical olfaction, olfactory loss, and olfactory dysfunction has expanded rapidly over the past two decades, with an exponential rise in the past year. There is substantial variability in the quality of this literature and a need to consolidate and critically review the evidence. It is with that aim that we have gathered experts from around the world to produce this International Consensus on Allergy and Rhinology: Olfaction (ICAR:O). Using previously described methodology, specific topics were developed relating to olfaction. Each topic was assigned a literature review, evidence-based review, or evidence-based review with recommendations format as dictated by available evidence and scope within the ICAR:O document. Following iterative reviews of each topic, the ICAR:O document was integrated and reviewed by all authors for final consensus. The ICAR:O document reviews nearly 100 separate topics within the realm of olfaction, including diagnosis, epidemiology, disease burden, diagnosis, testing, etiology, treatment, and associated pathologies. This critical review of the existing clinical olfaction literature provides much needed insight and clarity into the evaluation, diagnosis, and treatment of patients with olfactory dysfunction, while also clearly delineating gaps in our knowledge and evidence base that we should investigate further.

Abstract

Reissued 03 Jan 2022 to update keywords. The United States Marine Corps continues to develop Expeditionary Advanced Base Operations (EABO) as the future operating model in the Pacific. EABO will employ Marines using widely dispersed, low profile, highly potent, and tightly integrated Navy and Marine Corps teams. This operating model challenges the Marine Corps’ ability to command and control due to inherent resource limitations, long communication ranges, and the enemy’s ability to detect, intercept, and interfere in the electromagnetic spectrum. This study sought to evaluate bursty-signal mesh network (BSMN) technology as a potential solution to these problems. The technology’s suitability was evaluated through a comparison of the character of command and control in EABO (established through qualitative case study analysis of recent EABO exercises) against the characteristics of BSMN technology (established through quantitative modeling analysis). Finally, the viability of acquiring and fielding the technology was evaluated through a quantitative financial analysis. Though the researchers recommend further study, they conclude that BSMN technology’s long-range, stealth, and low-power capabilities are well suited for communication at-and-below the regimental level. Further, researchers conclude that the technology is viable to acquire and field, with a price point far below other long-range communication assets (i.e., satellite communication) currently in use. Abstract

Objective: To study the efficacy of liposomal bupivacaine on postoperative opioid requirement and pain following abdominal wall reconstruction. Summary Background Data: Despite the widespread use of liposomal bupivacaine in transversus abdominis plane block, there is inadequate evidence demonstrating its efficacy in open abdominal wall reconstruction. We hypothesized that liposomal bupivacaine plane block would result in decreased opioid requirements compared with placebo in the first 72 hours after surgery. Methods: This was a single-center double-blind, placebo-controlled prospective study conducted between July 2018 and November 2019. Adult patients (at least 18 yrs of age) undergoing open, elective, ventral hernia repairs with mesh placed in the retromuscular position were enrolled. Patients were randomized to surgeon-performed transversus abdominis plane block with liposomal bupivacaine, simple bupivacaine, or normal saline (placebo). The main outcome was opioid requirements in the first 72 hours after surgery. Secondary outcomes included total inpatient opioid use, pain scores determined using a 100 mm visual analog scale, length of hospital stay, and patientreported quality of life. Results: Of the 164 patients who were included in the analysis, 57 patients received liposomal bupivacaine, 55 patients received simple bupivacaine, and 52 received placebo. There were no differences in the total opioid used in the first 72 hours after surgery as measured by morphine milligram equivalents when liposomal bupivacaine was compared with simple bupivacaine and placebo (325 ± 225 vs 350 ± 284 vs 310 ± 272, respectively, P = 0.725). Similarly, there were no differences in total inpatient opioid use, pain scores, length of stay, and patient-reported quality of life. Conclusions: There are no apparent clinical benefits to using liposomal bupivacaine transversus abdominis plane block when compared with simple bupivacaine and placebo for open abdominal wall reconstruction. Abstract

In his doctoral dissertation On the Principle of Sufficient Reason, Arthur Schopenhauer there outlines a critique of Euclidean geometry on the basis of the changing nature of mathematics, and hence of demonstration, as a result of Kantian idealism. According to Schopenhauer, Euclid treats geometry synthetically, proceeding from the simple to the complex, from the known to the unknown, 'synthesizing' later proofs on the basis of earlier ones. Such a method, although proving the case logically, nevertheless fails to attain the raison d'etre of the entity. In order to obtain this, a separate method is required, which Schopenhauer refers to as 'analysis,' thus echoing a method already in practice among the early Greek geometers, with however some significant differences. In this essay, I here discuss Schopenhauer's criticism of synthesis in Euclid's Elements, and the nature and relevance of his own method of analysis.

34. Decreasing Incidence of Chemosensory Changes by COVID‐19 Variant.

Abstract

Anecdotal clinical observation suggests that rates of chemosensory dysfunction associated with COVID‐19 infection may be decreasing. To investigate, the National COVID Cohort Collaborative database was queried for all patients with and without smell and taste loss within 2 weeks of COVID‐19 diagnosis. Six‐week periods of peak variant prevalence were selected by using CoVariants.org for analysis. Of 3,678,214 patients with COVID‐19 in the database, 616,318 met inclusion criteria during the time intervals of interest, with 3431 having an associated smell or taste disturbance diagnosis. With the initial/untyped variant set as the baseline, the odds ratios for alpha, delta, and omicron (December 27, 2021–February 7, 2022) were 0.50 (95% CI, 0.45‐0.55; P <.0001), 0.44 (95% CI, 0.41‐0.48; P <.0001), and 0.17 (95% CI, 0.15‐0.18; P <.0001), respectively. These data strongly support the clinical observation that patients infected with more recent variants are at a significantly lower risk of developing associated chemosensory loss.
