Abstract

We present ground-based optical observations of the September 2009 and January 2010 transits of HD 80606b. Based on 3 partial light curves of the September 2009 event, we derive a midtransit time of T_c [HJD] = 2455099.196 +- 0.026, which is about 1 sigma away from the previously predicted time. We observed the January 2010 event from 9 different locations, with most phases of the transit being observed by at least 3 different teams. We determine a midtransit time of T_c [HJD] = 2455210.6502 +- 0.0064, which is within 1.3 sigma of the time derived from a Spitzer observation of the same event., Comment: Accepted to ApJ. Abstract

We describe the ring structure of the cohomology of the Hilbert scheme of points for a smooth surface X. When the canonical class K_X = 0, this was done by Lehn and Sorger, extending earlier work when X = C^2. Their approach does not generalize. Instead we recast this problem as a question of finding integrals of motion for a Hamiltonian which describes ``intersection with the boundary''. To do so, we use the identification of the cohomology of the Hilbert schemes with a Fock space modelled on the lattice H(X). With this identification, Lehn computed the operator of intersection with the boundary. It is essentially the Calogero-Sutherland Hamiltonian. We then solve the problem of finding integrals of motion by using the Dunkl-Cherednik operators to find an explicit commuting family of differential operators; these operators represent cup product on the Hilbert scheme. We provide two characterizations of the Hilbert scheme multiplication operators; the first as an algebra of operators that can be inductively built from functions and the CS Hamiltonian, and the second in terms of the centralizer of the CS Hamiltonian inside an appropriate ring of differential operators.

Effective communication between patients with MS and their neurologist is important for improving clinical outcomes and quality of life. Methods: A closed-ended online market research survey was used to assess the current state of MS care from the perspective of both patients with MS (≥18 years of age) and neurologists who treat MS from Europe and the US and to gain insight into perceptions of treatment expectations/goals, treatment decisions, treatment challenges, communication, and satisfaction with care, based on current clinical practice. Results: A total of 900 neurologists and 982 patients completed the survey, of whom 46% self-identified as having remitting-relapsing MS, 29% secondary progressive MS, and 11% primary progressive MS. Overall, patients felt satisfied with their disease-modifying therapy (DMT); satisfaction related to comfort in speaking with their neurologist and participation in their DMT decision-making process. Patients who self-identified as having relapsing-remitting MS were more likely to be very satisfied with their treatment. Top challenges identified by patients in managing their DMT were cost, side effects/tolerability of treatment, and uncertainty if treatment was working. Half of the patients reported skipping doses, but only 68% told their health care provider that they did so. Conclusion: Several important differences in perception were identified between patients and neurologists concerning treatment selection, satisfaction, expectations, goals, and comfort discussing symptoms, as well as treatment challenges and skipped doses. The study results emphasize that patient/neurologist communication and patient input into the treatment decision-making process likely influence patient satisfaction with treatment. 9. Multidisciplinary HIV adherence intervention: a randomized study

Abstract

Maintaining greater than 95% adherence to antiretroviral medication is necessary in order to have the greatest therapeutic impact on HIV infection. Furthermore, evidence suggests that adherence rates of between 70% and 89% are significantly associated with viral rebound and the development of drug resistance. Adherence rates at and above the 95% level are difficult for patients to achieve and maintain. Our aim was to determine if an adherence intervention could improve adherence among patients attending an ambulatory care clinic at a large public hospital. The intervention was delivered by a multidisciplinary team of health care professionals and consisted of education coupled with the provision of devices designed to assist patient memory and adherence. A crucial component of the intervention consisted of the identification of patient specific barriers to adherence and the development of strategies to circumvent these problems. Adherence was assessed using patient self-report over the past 4, 7, and 28 days and by calculation of the Morisky score. The study was conducted as a randomised controlled trial using the stepped wedge design with a total of 68 subjects randomised to receive the intervention over a 20-week period. Adherence before and after the intervention formed the analysis. There was a significant decrease in the number of missed doses over the past 4 (1.9 to 1.0, p < 0.001), 7 (3.0 to 1.8, p < 0.001) and 28 (7.4 to 4.2, p < 0.001) days and a decrease in the Morisky score, indicating an improvement in medication taking behaviour (1.3 to 0.5 p < 0.001).

Abstract

Introduction: The California Prehospital Antifibrinolytic Therapy (Cal-PAT) study seeks to assess the safety and impact on patient mortality of tranexamic acid (TXA) administration in cases of trauma-induced hemorrhagic shock. The current study further aimed to assess the feasibility of prehospital TXA administration by paramedics within the framework of North American emergency medicine standards and protocols. Methods: This is an ongoing multi-centered, prospective, observational cohort study with a retrospective chart-review comparison. Trauma patients identified in the prehospital setting with signs of hemorrhagic shock by first responders were administered one gram of TXA followed by an optional second one-gram dose upon arrival to the hospital, if the patient still met inclusion criteria. Patients administered TXA make up the prehospital intervention group. Control group patients met the same inclusion criteria as TXA candidates and were matched with the prehospital intervention patients based on mechanism of injury, injury severity score, and age. The primary outcomes were mortality, measured at 24 hours, 48 hours, and 28 days. Secondary outcomes measured included the total blood products transfused and any known adverse events associated with TXA administration. Results: We included 128 patients in the prehospital intervention group and 125 in the control group. Although not statistically significant, the prehospital intervention group trended toward a lower 24-hour mortality rate (3.9% vs 7.2% for intervention and control, respectively, p=0.25), 48-hour mortality rate (6.3% vs 7.2% for intervention and control, respectively, p=0.76), and 28-day mortality rate (6.3% vs 10.4% for intervention and control, respectively, p=0.23). There was no significant difference observed in known adverse events associated with TXA administration in the prehospital intervention group and control group. A reduction in total blood product usage was observed following the administration of TXA (control: 6.95 units; intervention: 4.09 units; p=0.01). Conclusion: Preliminary evidence from the Cal-PAT study suggests that TXA administration may be safe in the prehospital setting with no significant change in adverse events observed and an associated decreased use of blood products in cases of trauma-induced hemorrhagic shock. Given the current sample size, a statistically significant decrease in mortality was not observed. Additionally, this study demonstrates that it may be feasible for paramedics to identify and safely administer TXA in the prehospital setting.

Abstract

The implementation of a Project Management Information System (PMIS) in three public sector agencies is examined using Actor-Network Theory, an infrequently applied approach to project management research. Use of the PMIS focused on practitioner capability, without acting as a mechanism of project control. This is different to how a PMIS is generally portrayed in the literature, suggesting a need to rethink the ways a PMIS can support project management.The research revealed that the PMIS software application, by itself, would have had limited impact, and similarly the practitioner-researchers who developed and implemented the PMIS would, by themselves, have been less effective. Instead, the contribution, effectiveness and capability came from the stable network of association between the practitioner-researchers and the PMIS; an actor-network, which allowed other networks to stabilise and develop, as new users learned how to conceptualise their work through project management concepts, and developed their project management capability. © 2012 Elsevier Ltd and APM IPMA.

25. When Closing the Human-Animal Divide Expands Moral Concern: The Importance of Framing

Abstract

Humans and animals share many similarities. Across three studies, the authors demonstrate that the framing of these similarities has significant consequences for people’s moral concern for others. Comparing animals to humans expands moral concern and reduces speciesism; however, comparing humans to animals does not appear to produce these same effects. The authors find these differences when focusing on natural tendencies to frame human–animal similarities (Study 1) and following experimental induction of framings (Studies 2 and 3). In Study 3, the authors extend their focus from other animals to marginalized human outgroups, demonstrating that human–animal similarity framing also has consequences for the extension of moral concern to other humans. The authors explain these findings by reference to previous work examining the effects of framing on judgments of similarity and self-other comparisons and discuss them in relation to the promotion of animal welfare and the expansion of moral concern.
