1. A Comparison of Control Populations in Quebec Using the Short Musculoskeletal Function Assessment

Abstract

The Short Musculoskeletal Function Assessment (SMFA) questionnaire is a health status instrument validated in the United States for use with patients with musculoskeletal impairments. It allows patients to self-rate their level of disability and impairment. This questionnaire has never been validated for Canada's English and French speaking populations. The first objective of this study was to determine the baseline responses of a healthy (i.e., without orthopaedic pathology) population representative of English and French speaking Canadians. The second objective was to compare the results of the Short Musculoskeletal Function Assessment to see if language or gender had any significant effect on the responses. A sample population (n=144) of Quebec Francophone and Anglophone subjects was interviewed using the Short Musculoskeletal Function Assessment questionnaire over the course of five weeks. All subjects were obtained from the orthopaedic clinic of the Montreal General Hospital in Montreal, Quebec. All subjects were self-reported as not being orthopaedic patients themselves in the past or present, and were merely accompanying patients of the clinic. Results were analyzed for differences between four groups using the ANOVA statistical test: Francophone females, Francophone males, Anglophone females, and Anglophone males. No statistically significant differences were detected between the four groups. Results were also analyzed for any differences between three age groups using the ANOVA statistical test, (15-35 years, 36-55 years, and 56 and greater years), with no significant differences detected. The overall Short Musculoskeletal Function Assessment values obtained for the four patient populations were: Francophone males = 5.41; Francophone females = 5.40; Anglophone males = 6.41; and Anglophone females males = 5.33. For the three age groups, the results were: 15 - 35 years = 6.87; 36 - 55 years = 5.22; 56 years and greater = 5.18. These results provide an initial baseline to which future orthopaedic patients can be compared, and suggest that analysis of the Short Musculoskeletal Function Assessment results may be compared across gender and language lines within Canada.

Abstract

Myoepithelial tumors of the soft tissue are a rare tumor displaying myoepithelial elements and lacking obvious ductal differentiation. The rarity of these precludes any evidence-based consensus regarding optimal management. Nevertheless, the current approach to these lesions begins with amputation or complete excision. The efficacy of neoadjuvant or adjuvant radiation therapy or chemotherapy has not been established. Here, we present the first report to the authors' knowledge of neoadjuvant radiation therapy for the treatment of this rare soft tissue neoplasm and review the management and outcomes of published cases of myoepithelial carcinoma. A patient with a soft tissue myoepithelial carcinoma that declined both amputation and chemotherapy was treated with neoadjuvant radiation therapy and wide surgical excision followed by a brachytherapy boost to the resected tumor bed. Neoadjuvant radiation therapy resulted in an excellent response with extensive treatment-related changes consisting predominantly of fibrosis, hyalinization and hemorrhage and only 10% residual viable myoepithelial carcinoma present in the surgical specimen.

Abstract

Purpose To address the factors that surgeons use to decide between 2 options for treatment when the evidence is inconclusive. Methods We tested the null hypothesis that the factors surgeons use do not vary by training, demographics, and practice. A total of 337 surgeons rated the importance of 7 factors when deciding between treatment and following the natural history of the disease and 12 factors when deciding between 2 operative treatments using a 5-point Likert scale between "very important" and "very unimportant." Results According to the percentages of statements rated very important or somewhat important, the most popular factors influencing recommendations when evidence is inconclusive between treatment and following the natural course of the illness were "works in my hands," "familiarity with the treatment," and "what my mentor taught me." The most important factors when evidence shows no difference between 2 surgeries were "fewer complications," "quicker recovery," "burns fewer bridges," "works in my hands" and "familiarity with the procedure." Europeans rated "works in my hands" and "cheapest/most resourceful" of significantly greater importance and "what others are doing," "highest reimbursement," and "shorter procedure" of significantly lower importance than surgeons in the United States. Observers with fewer than 10 years in independent practice rated "what my mentor taught me," "what others are doing" and "highest reimbursement" of significantly lower importance compared to observers with 10 or more years in independent practice. Conclusions Surgeons deciding between 2 treatment options, when the evidence is inconclusive, fall back to factors that relate to their perspective and reflect their culture and circumstances, more so than factors related to the patient's perspective, although this may be different for younger surgeons. Clinical relevance Hand surgeons might benefit from consensus fallback preferences when evidence is inconclusive. It is possible that falling back to personal comfort makes us vulnerable to unhelpful commercial and societal influences.

Abstract

Pediatric patients with cerebral palsy present unique challenges. Any treatment regimen must take into account potential growth, possible sequelae of surgery, and, in some cases, significant behavioral issues. Careful evaluation of motor and sensory function of the extremity and of use patterns is imperative because these findings play a critical role in determining the ultimate success of any intervention. Every patient is addressed independently and treatment individualized. The patient and parents must understand that surgery can address only the function or position of the anatomic area. Surgery will not correct the underlying problem.
