16. A Personal Experience of Performing a Systematic Literature Review

Abstract

This paper describes the personal experience of two novice reviewers in developing and conducting a systematic litera¬ture review (SLR) in the field of medical ultrasound. The review was performed as part of a PhD study by the first author, which required the researcher to critically review all relevant studies in their specific research area. The review was conducted using a formal, scientific process and high¬lighted 20 relevant papers. Among the lessons learnt, undertaking an SLR was a time-consuming process. The search produced 1,987 papers, and screening the search yield was facilitated by using a pre-defined protocol. Extraction of data from each study was standardised and generated a 1-page summary of the key findings and methodology to facilitate comparisons of the included studies. The Critical Appraisal Skills Programme (CASP) tool was used for quality assessment. In the critical appraisal process, it is essential that reviewers have good baseline knowledge in the research subject, a solid under¬standing of the fundamentals of a robust research study including potential bias in research. With the great amount of information that a critical appraisal produces, good information management was key throughout the review to ensure the process was explicit and replicable and so that reviewers could justify their decisions. The methods and processes detailed in this report, as well as its lessons, are relevant to an SLR for any research subject.

Item does not contain fulltext, BACKGROUND: Exercise interventions are often incompletely described in reports of clinical trials, hampering evaluation of results and replication and implementation into practice. OBJECTIVE: The aim of this study was to develop a standardized method for reporting exercise programs in clinical trials: the Consensus on Exercise Reporting Template (CERT). DESIGN AND METHODS: Using the EQUATOR Network's methodological framework, 137 exercise experts were invited to participate in a Delphi consensus study. A list of 41 items was identified from a meta-epidemiologic study of 73 systematic reviews of exercise. For each item, participants indicated agreement on an 11-point rating scale. Consensus for item inclusion was defined a priori as greater than 70% agreement of respondents rating an item 7 or above. Three sequential rounds of anonymous online questionnaires and a Delphi workshop were used. RESULTS: There were 57 (response rate=42%), 54 (response rate=95%), and 49 (response rate=91%) respondents to rounds 1 through 3, respectively, from 11 countries and a range of disciplines. In round 1, 2 items were excluded; 24 items reached consensus for inclusion (8 items accepted in original format), and 16 items were revised in response to participant suggestions. Of 14 items in round 2, 3 were excluded, 11 reached consensus for inclusion (4 items accepted in original format), and 7 were reworded. Sixteen items were included in round 3, and all items reached greater than 70% consensus for inclusion. LIMITATIONS: The views of included Delphi panelists may differ from those of experts who declined participation and may not fully represent the views of all exercise experts. CONCLUSIONS: The CERT, a 16-item checklist developed by an international panel of exercise experts, is designed to improve the reporting of exercise programs in all evaluative study designs and contains 7 categories: materials, provider, delivery, location, dosage, tailoring, and compliance. 43. The effects of resistance training on quality of life in cancer: a systematic literature review and meta-analysis.

Abstract

Purpose: The purpose of this study was to carry out a systematic review and meta-analysis to evaluate the effect of resistance training upon quality of life (QoL) in cancer.Methods: Search Strategy: A wide range of electronic databases were searched from inception to October 2009 using relevant key words. Reference lists of all studies identified for inclusion and relevant reviews were also searched. Relevant journals were hand searched and experts in the field contacted.Selection Criteria: Randomized controlled trials that investigated the specific effect of resistance training on QoL in adult cancer survivors were included.Data Collection and Analysis: Two review authors independently assessed methodological quality and extracted data based upon predefined criteria. A meta-analysis was performed for QoL using a random effects model.Results: Six studies were identified for inclusion. Two studies demonstrated a significantly beneficial effect of resistance training on QoL compared to usual care. Post-test means ± standard deviations were available for all comparisons providing data for 278 participants who received a resistance training intervention and 270 control participants. The results of the meta-analysis demonstrated that, at the end of the intervention period, resistance training was statistically more effective than the control intervention (SMD -0.17, 95% CIs -0.34 to -0.00). Overall, there was heterogeneity between studies in relation to tumor type, stage of cancer treatment, type of cancer treatment, and duration of the intervention.Conclusions: Existing evidence suggests that strength training programs for cancer survivors have marginal benefit. Abstract

Background: Emergency departments (EDs) are one of the main providers of minor injury care in the UK. The use of extended scope physiotherapists (ESPs), independently managing patients from arrival to discharge, has not been widely considered in the UK or internationally. It is possible that ESPs will contribute to the staffing of the twenty-first-century EDs, and their role is gaining momentum in Australia, Canada, and several European counties. Objectives: The aim of the review was to establish the UK and international evidence for clinical effectiveness and cost efficiencies for ESPs independently managing a case load of 'minor injuries' in the ED setting. Methods: A comprehensive review of the published and non-published international literature was undertaken. Results: ESPs are independent professionals with pre-existing skills in the assessment and diagnosis of musculoskeletal injuries. Evidence was only found from UK hospitals regarding the clinical effectiveness and cost efficiencies. ESPs obtain equivalent clinical outcomes compared to the care provided by doctors of all grades, and they are likely to be equivalent in cost, but patients may take longer to return to normal activities when treated by an ESP. ESPs can work to existing ED protocols and achieve significantly higher patient satisfaction than other professional groups, but spend longer with patients. Conclusions: ESPs can provide a high standard of care at an affordable cost, whilst positively influencing patient satisfaction. The use of ESPs working in the ED, carrying out duties traditionally undertaken by doctors, could provide one of the solutions to staffing shortages in emergency care. [ABSTRACT FROM AUTHOR]

45. Non-surgical management of piriformis syndrome: a systematic review.

Abstract

The aim of this review was to identify and evaluate the evidence for the non-surgical management of Piriformis Syndrome. A systematic review of electronic databases was carried out up to February 2006. This was supplemented by hand searching journals and 'snow-balling' from reference lists. Strict inclusion/exclusion criteria were applied. Five independent observers reviewed the methodological quality of the core studies using the published Physiotherapy Evidence Database (PEDro) critique tool. Differences were resolved by consensus. Two studies met the inclusion/exclusion criteria. PEDro scores were 6/10 and 10/10. Both studies demonstrated positive effects for botulinum toxin type A (Botox) injections compared to a placebo or control treatment. One study identified this positive effect in addition to a physiotherapy protocol. Evidence implies that Botox is a potentially successful treatment for Piriformis Syndrome; however, a paucity of high-quality clinical trials limits evidence of its overall efficacy. Further clinical trials of the effectiveness of non-surgical measures in the management of this syndrome are indicated. [ABSTRACT FROM AUTHOR]

46. Fusion of mouse fibroblasts with oleylamine

Abstract

As part of a search for chemical agents able to promote the fusion of mouse A 9 fibroblasts, oleylamine, a positively charged compound, has been investigated for its fusogenic properties. In the pH range 5·5–7·5 and in the presence of dextran, fibroblast polykaryons were produced on treatment of monolayers of cultured cells with oleylamine dispersed directly in a modified Eagle’s medium at concentrations of not less than 0·111 mg/ml. Electron microscopy demonstrated the absence of a dividing plasma membrane between the constituent nuclei of the polykaryons, and showed clustering of other subcellular organelles around their original parent nuclei. Fusion, which was preceded by rounding and swelling of the cells, occurred between cells in contact after 10-15 min. Oleylamine in lipid droplets containing glyceryl mono- and dioleate also caused swelling and fusion but to a lesser extent. Phosphatidylcholine appeared to have an inhibitory effect on oleylamine-induced fusion : lecithin liposomes containing oleylamine were only weakly fusogenic. The fusion process, but not the preceding swelling, was calcium-dependent; fusion was inhibited by low concentrations of lanthanum ions. While oleylamine inhibited cell division in monolayer cultures and prevented adhesion of fibroblasts in suspension to glass coverslips, oleylamine in lipid droplets was less toxic and is thus potentially more useful in this form for interspecific hybridization experiments.

Abstract

Hen erythrocytes that were fixed after treatment with lysolecithin in aqueous solution for 30 s at 37 °C showed evidence of bridge formation between adjacent lysed cells. Generally, the homokaryons that were produced using lysolecithin in this way contained large numbers of nuclei. These giant syncytia had damaged nuclear membranes and unstable plasma membranes; complete disintegration of the syncytia occurred within 1 min of adding lysolecithin to the erythrocytes. In order to localize the action of lysolecithin, the fusing agent was incorporated into microdroplets of lipid. Cell fusion following the addition of lysolecithin in an aqueous glyceride-lecithin emulsion was slower than with lysolecithin in aqueous solution, taking 10–30 min, and it was accompanied by considerably less damage to the plasma and nuclear membranes. The fused erythrocytes, which usually contained only two or three nuclei, lysed slowly during the 45 min following fusion, and lysis could be arrested by cooling the fused cells. The plasma membranes of lysed, multinucleated cells remained intact at 37 °C for at least 90 h. Mouse fibroblast-hen erythrocyte heterokaryons formed with the aid of the emulsion were more stable than those produced with lysolecithin in solution, but the hybrid cells nevertheless had damaged subcellular organelles. Viable clones of hybrid mouse-hamster fibroblast cells were obtained using the emulsion although, possibly owing to reduced viability of the lysole-cithin-treated cells, only at twice the frequency of spontaneously produced hybrids.

48. The use of mechanical insufflation-exsufflation in invasively ventilated critically ill adults : a scoping review

Abstract

Mechanical insufflation-exsufflation (MI-E) is traditionally used in the neuromuscular population. There is growing interest of MI-E use in invasively ventilated critically ill adults. We aimed to map current evidence on MI-E use in invasively ventilated criticall ill adults.Two authors independently searched electronic databases MEDLINE, EMBASE and CINAHL via the OVID platform, PROSPERO, The Cochrane Library, ISI Web of Science and International Clinical Trials Registry Platform between January 1990 and April 2021. Inclusion criteria were (1) adult critically ill invasively ventilated patients, (2) use of MI-E, (3) study design with original data, (4) published from 1990 onwards. Data were extracted by two authors independently using a bespoke extraction form. We used Mixed Methods Assessment Tool to appraise risk of bias. Theoretical Domains Framework was used to interpret qualitative data. Of 3090 citations identified, 28 citations were taken forward for data extraction. Main indications for MI-E use during invasive mechanical ventilation were presence of secretions and mucus plugging (13/28 46%). Perceived\ud 5\ud contraindications related to use of high levels of positive pressure (19/28 68%). Protocolised MI-E settings with a pressure of +/- 40 cmH2O were most commonly used with detail on timing, flow and frequency of prescription infrequently reported. Various outcomes were reintubation rate, wet sputum weight, and pulmonary mechanics. Only 3 studies reported the occurrence of adverse events. From qualitative data, the main barrier to MI-E use in this patient group was lack of knowledge and skills. We conclude that there is little consistency in how MI-E is used and reported and therefore recommendations about best practices are not possible.

Abstract

Much like the corporate sector, the charity and non-profit sector has adopted social media as one of their core engagement and fundraising tools. The online community has great power and has worked well for many brands; however, the extent to which this is benefitting the third sector is debatable. Is liking a non-profit organization on Facebook equal to donating money or running a marathon? A consumer’s online interaction with a charity, e.g. liking or sharing social media content and pages, is sometimes referred to as ‘slacktivism’. To date, studies on ‘slacktivist’ behavior in the social media context are limited to few conceptual papers. This study provides empirical insights into motivations and interactions of social media users towards non-profit and charity-related social media campaigns.
