Abstract

Method: ProRSeg was trained using 5-fold cross-validation with 110 T2-weighted MRI acquired at 5 treatment fractions from 10 different patients, taking care that same patient scans were not placed in training and testing folds. Segmentation accuracy was measured using Dice similarity coefficient (DSC) and Hausdorff distance at 95th percentile (HD95). Registration consistency was measured using coefficient of variation (CV) in displacement of OARs. Ablation tests and accuracy comparisons against multiple methods were done. Finally, applicability of ProRSeg to segment cone-beam CT (CBCT) scans was evaluated on 80 scans using 5-fold cross-validation. Results: ProRSeg processed 3D volumes (128 $\times$ 192 $\times$ 128) in 3 secs on a NVIDIA Tesla V100 GPU. It's segmentations were significantly more accurate ($p<0.001$) than compared methods, achieving a DSC of 0.94 $\pm$0.02 for liver, 0.88$\pm$0.04 for large bowel, 0.78$\pm$0.03 for small bowel and 0.82$\pm$0.04 for stomach-duodenum from MRI. ProRSeg achieved a DSC of 0.72$\pm$0.01 for small bowel and 0.76$\pm$0.03 for stomach-duodenum from CBCT. ProRSeg registrations resulted in the lowest CV in displacement (stomach-duodenum $CV_{x}$: 0.75\%, $CV_{y}$: 0.73\%, and $CV_{z}$: 0.81\%; small bowel $CV_{x}$: 0.80\%, $CV_{y}$: 0.80\%, and $CV_{z}$: 0.68\%; large bowel $CV_{x}$: 0.71\%, $CV_{y}$ : 0.81\%, and $CV_{z}$: 0.75\%). ProRSeg based dose accumulation accounting for intra-fraction (pre-treatment to post-treatment MRI scan) and inter-fraction motion showed that the organ dose constraints were violated in 4 patients for stomach-duodenum and for 3 patients for small bowel. Abstract

Vision transformers, with their ability to more efficiently model long-range context, have demonstrated impressive accuracy gains in several computer vision and medical image analysis tasks including segmentation. However, such methods need large labeled datasets for training, which is hard to obtain for medical image analysis. Self-supervised learning (SSL) has demonstrated success in medical image segmentation using convolutional networks. In this work, we developed a \underline{s}elf-distillation learning with \underline{m}asked \underline{i}mage modeling method to perform SSL for vision \underline{t}ransformers (SMIT) applied to 3D multi-organ segmentation from CT and MRI. Our contribution is a dense pixel-wise regression within masked patches called masked image prediction, which we combined with masked patch token distillation as pretext task to pre-train vision transformers. We show our approach is more accurate and requires fewer fine tuning datasets than other pretext tasks. Unlike prior medical image methods, which typically used image sets arising from disease sites and imaging modalities corresponding to the target tasks, we used 3,643 CT scans (602,708 images) arising from head and neck, lung, and kidney cancers as well as COVID-19 for pre-training and applied it to abdominal organs segmentation from MRI pancreatic cancer patients as well as publicly available 13 different abdominal organs segmentation from CT. Our method showed clear accuracy improvement (average DSC of 0.875 from MRI and 0.878 from CT) with reduced requirement for fine-tuning datasets over commonly used pretext tasks. Abstract

This dissertation traces the emergence of 'lifestyle' - a self-curated life experience - in California in the period between 1880-1915. Lifestyle emerged at the end of the nineteenth century: following Michel Foucault, this dissertation argues that it constituted a powerful new means for the management and presentation of the self. This dissertation suggests that lifestyle manifested in California in its most evident forms. Each chapter unfolds through an analysis of primary texts and photography. Chapter 1 traces the emergence of lifestyle through self-help literature, consumerism and ideas about the production of the self. Chapter 2 compares narratives of experience in Yosemite Valley through three interrelated works: John Muir's My first Summer in the Sierra (1911), J. M. Hutchings's Scenes of Wonder and Curiosity in California (1868) and Therese Yelverton's Zanita: A Tale of the Yo-Semite (1872). Chapter 3 examines the editor and writer Charles Lummis and his home 'El Alisal'. This chapter traces the role of Lummis, his magazine 'The Land of Sunshine' (1895-1900), the building of his home El Alisal, and his photographic collection, in shaping and defining particular literary, economic and architectural vernaculars of late 19th century Los Angeles. Chapter 4 focuses on the rapid growth of Pasadena and the emergence of lifestyle in relation to the diaries and photographs of early tourists as well as the promotional literature of George Wharton James. The dissertation concludes that California in 1880-1915 was an exemplary site wherein lifestyle materialised and became a defining feature of the cultural landscape. Through this, the dissertation argues that the unique combination of rapid urban development, wealth and geography in Southern California gave rise to the origins of what I term deliberate culture: a model of lifestyle that is performed through a self-constructed duality of narrative and experience.

Abstract

Background Neoadjuvant therapy leads to a clinical complete response in a considerable proportion of patients with locally advanced rectal cancer, allowing for possible nonoperative management. The presence of mucin on magnetic resonance imaging (MRI) after neoadjuvant therapy leads to uncertainty about residual disease and appropriateness of a watch-and-wait strategy in patients with no evidence of disease on proctoscopy (endoscopic clinical complete response). Methods MRI reports for locally advanced rectal cancer patients seen between July 2016 and January 2020 at Memorial Sloan Kettering Cancer Center were queried for presence of mucin in the tumor bed on MRI following neoadjuvant therapy. Clinicodemographic, pathologic, and outcome data were compiled and analyzed. Results Of 71 patients with mucin on posttreatment MRI, 20 had a clinical complete response, and 51 had abnormalities on endoscopy and/or physical exam. One patient with a clinical complete response opted out of watch-and-wait; thus, 19 (27%) patients entered watch-and-wait, and 52 (73%) patients were planned for surgery (non–watch-and-wait). Of the 19 watch-and-wait patients, 15 (79%) have had no local regrowth with median follow-up of 50 months (range = 29-76 months), while 4 (21%) experienced regrowth between 9 and 29 months after neoadjuvant therapy. Of the 52 patients who were planned to have surgery (non–watch-and-wait), 49 underwent resection while 3 developed metastatic disease that precluded curative-intent surgery. Of the 49 patients who underwent surgery, 5 (10%) had a pathologic complete response (including the patient with an endoscopic clinical complete response). Conclusions The presence of mucin after neoadjuvant therapy for locally advanced rectal cancer does not preclude watch-and-wait management in otherwise appropriate candidates who achieve an endoscopic clinical complete response. Abstract

Introduction Immune checkpoint inhibitor (ICI) combinations extend overall survival (OS) while anti-PD-1/L1 monotherapy is non-inferior to sorafenib in treatment-naïve, patients with advanced hepatocellular carcinoma (HCC). Clinicogenomic features are posited to influence patient outcomes. Methods The primary objective of this retrospective study was to define the clinical, pathologic, and genomic factors associated with outcomes to ICI therapy in patients with HCC. Patients with histologically confirmed advanced HCC treated with ICI at Memorial Sloan Kettering Cancer Center from 2012 to 2022 were included. Association between clinical, pathological, and genomic characteristics were assessed with univariable and multivariable Cox regression model for progression-free survival (PFS) and OS. Results Two-hundred and forty-two patients were treated with ICI-based therapy. Patients were predominantly male (82%) with virally mediated HCC (53%) and Child Pugh A score (70%). Median follow-up was 28 months (0.5-78.4). Median PFS for those treated in 1st line, 2nd line and ≥ 3rd line was 4.9 (range: 2.9-6.2), 3.1 (2.3-4.0), and 2.5 (2.1-4.0) months, respectively. Median OS for those treated in 1st line, 2nd line, and ≥ 3rd line was 16 (11-22), 7.5 (6.4-11), and 6.4 (4.6-26) months, respectively. Poor liver function and performance status associated with worse PFS and OS, while viral hepatitis C was associated with favorable outcome. Genetic alterations were not associated with outcomes. Conclusion Clinicopathologic factors were the major determinates of outcomes for patients with advanced HCC treated with ICI. Molecular profiling did not aid in stratification of ICI outcomes. Abstract

Purpose: Develop a true real‐time implementation of MR signature matching (MRSIGMA) for free‐breathing 3D MRI with sub‐200 ms latency on the Elekta Unity 1.5T MR‐Linac. Methods: MRSIGMA was implemented on an external computer with a network connection to the MR‐Linac. Stack‐of‐stars with partial kz sampling was used to accelerate data acquisition and ReconSocket was employed for simultaneous data transmission. Movienet network computed the 4D MRI motion dictionary and correlation analysis was used for signature matching. A programmable 4D MRI phantom was utilized to evaluate MRSIGMA with respect to a ground‐truth translational motion reference. In vivo validation was performed on patients with pancreatic cancer, where 15 patients were employed to train Movienet and 7 patients to test the real‐time implementation of MRSIGMA. Dice coefficients between real‐time MRSIGMA and a retrospectively computed 4D reference were used to evaluate motion tracking performance. Results: Motion dictionary was computed in under 5 s. Signature acquisition and matching presented 173 ms latency on the phantom and 193 ms on patients. MRSIGMA presented a mean error of 1.3–1.6 mm for all phantom experiments, which was below the 2 mm acquisition resolution along the motion direction. The Dice coefficient over time between MRSIGMA and reference contours was 0.88 ± 0.02 (GTV), 0.87 ± 0.02(duodenum‐stomach), and 0.78 ± 0.02(small bowel), demonstrating high motion tracking performance for both tumor and organs at risk. Conclusion: The real‐time implementation of MRSIGMA enabled true real‐time free‐breathing 3D MRI with sub‐200 ms imaging latency on a clinical MR‐Linac system, which can be used for treatment monitoring, adaptive radiotherapy and dose accumulation mapping in tumors affected by respiratory motion. Abstract

BACKGROUND:The majority of patients with pancreatic cancer who undergo curative resection experience rapid disease recurrence. In previous small studies, high expression of the mismatch-repair protein mutL protein homolog 1 (MLH1) in pancreatic cancers was associated with better outcomes. The objective of this study was to validate the association between MLH1 expression and survival in patients who underwent resection of pancreatic cancer and received adjuvant chemoradiation. METHODS:Samples were obtained from the NRG Oncology Radiation Therapy Oncology Group 9704 prospective, randomized trial (clinicaltrials.gov identifier NCT00003216), which compared 2 adjuvant protocols in patients with pancreatic cancer who underwent resection. Tissue microarrays were prepared from formalin-fixed, paraffin-embedded, resected tumor tissues. MLH1 expression was quantified using fluorescence immunohistochemistry and automated quantitative analysis, and expression was dichotomized above and below the median value. RESULTS:Immunohistochemical staining was successfully performed on 117 patients for MLH1 (60 and 57 patients from the 2 arms). The characteristics of the participants who had tissue samples available were similar to those of the trial population as a whole. At the time of analysis, 84% of participants had died, with a median survival of 17 months. Elevated MLH1 expression levels in tumor nuclei were significantly correlated with longer disease-free and overall survival in each arm individually and in both arms combined. Abstract

Stereotactic ablative radiotherapy (SABR) commonly is used for small liver metastases. Modern conformal radiotherapy techniques, including 3-dimensional conformal radiotherapy and intensity-modulated radiation therapy, enable the safe delivery of SABR to small liver volumes. For larger tumors, the safe delivery of SABR can be challenging due to a more limited volume of healthy normal liver parenchyma and the proximity of the tumor to radiosensitive organs, such as the stomach, duodenum, and large intestine. Controlling respiratory motion, the use of image guidance, and increasing the number of radiation fractions sometimes are necessary for the safe delivery of SABR in these situations.
