A catarata é uma das principais afecções que causam cegueira nos cães. Devido à opacidade do cristalino, ocorre a dificuldade ou mesmo a não visualização de estruturas presentes na câmara vítrea, degenerações e hemorragias. Com isso, a ultrassonografia ocular torna-se uma ferramenta importante para essa avaliação das estruturas oculares. Objetivou-se com esse trabalho realizar a ultrassonografia ocular em cães de diversas raças, idades e graus de desenvolvimento da catarata com o intuito de avaliar a presença da degeneração vítrea (DV), condições do cristalino, localização da opacidade, posicionamento da retina, suas relações e outras alterações que prejudiquem o pós-cirúrgico ou impeçam a realização da facectomia. Um total de 31 animais participaram do estudo, no qual 57 olhos foram examinados. A raça predominante foi a Poodle, com 45,16% (14/31) dos cães avaliados. A idade variou de 2 a 16 anos, 48,4% (15/31) eram machos e 51,6% (16/31) fêmeas. Quanto à DV, 19,29% (11/57) dos bulbos oculares apresentavam grau zero; o mesmo quantitativo foi encontrado nos graus 1 e 2 com 35,8% (20/57) cada e, no grau 3, 10,52% (6/57). Em 12,56% (7/57) havia hialose e em 3,5% (2/57) descolamento parcial da retina. Após a análise dos dados não foi possível observar uma correlação entre a catarata e a DV, bem como o grau de DV e a idade dos animais.

3. Hemorragia subretiniana por rotura de Macroaneurisma arterial retinal: reporte de caso

El macroaneurisma arterial retiniano es una afección poco frecuente en la población y suele observarse en ancianos con diversas comorbilidades metabólicas como diabetes mellitus tipo II e hipertensión arterial, que predisponen a la esclerosis de los vasos retinianos. Puede conducir a la pérdida de visión por edema macular o hemorragia. Presentamos el caso de una paciente de 87 años que acudió a consulta oftalmológica por pérdida de visión. La exploración física reveló opacificación del cristalino y astigmatismo miópico simple en ambos ojos, además, las pruebas complementarias mostraron adelgazamiento corneal, exudados bilaterales y hemorragia subretiniana en el ojo derecho. El diagnóstico fue de una retinopatía diabética proliferativa en conjunto con macroaneurisma arterial retiniano y catarata en el ojo derecho. Como tratamiento se decidió administrar anti-factor de crecimiento endotelial vascular (Bevacizumab) durante cinco meses para evitar la deformación macular y después se realizó una cirugía láser de retina en el ojo derecho.

RESUMO Ao longo da vida, o cristalino produz novas fibras dispostas de forma concêntrica, que aumentam seu diâmetro anteroposterior e peso, tornando seu núcleo mais compacto e endurecido. A catarata hipermadura é uma forma de progressão avançada dessa proliferação de fibras, que pode desencadear uma variedade de complicações. A ruptura espontânea da cápsula anterior do cristalino, evoluindo com deslocamento anterior do núcleo, é uma complicação rara e com poucos casos publicados na literatura. Descrevemos o caso de uma paciente do sexo feminino, 68 anos, que apresentou ruptura espontânea da cápsula anterior do cristalino com deslocamento anterior do núcleo em olho esquerdo sem histórico de trauma ocular. A paciente foi submetida à facoemulsificação do cristalino e ao controle da pressão intraocular, evoluindo com melhora do quadro clínico.

La quimioterapia es el tratamiento más utilizado en los pacientes oncológicos, siendo causante de efectos secundarios en el organismo. A nivel ocular y visual, la evidencia científica es bastante limitada. El objetivo principal del presente estudio es evaluar el estado visual y ocular de pacientes oncológicos tratados con quimioterapia, comparándolos con un grupo control. La muestra fue de 20 pacientes (10 oncológicos y 10 control) a los cuales se les realizaron una anamnesis y pruebas optométricas. No se obtuvieron diferencias significativas entre ambos grupos, excepto en el síntoma de sequedad ocular. En relación con el resto de medidas, el grupo oncológico presentó peores valores, principalmente en la acomodación y morfología del cristalino, tanto en pruebas objetivas como subjetivas. Se podría relacionar con la debilidad muscular secundaria al tratamiento. En conclusión, la quimioterapia parece afectar múltiples parámetros visuales, aunque se requieren futuros estudios que lo demuestren.

Introduction and Objectives: To compare the real time skin dosimeter values at lens between with the lens included in the scan range (orbitomeatal base line [OML]) or without the lens included in the scan range (superior orbitomeatal line [SOML]) at different tube voltages., Materials and Methods: We used three pediatric anthropomorphic phantoms with a 64 detector-row computed tomography (CT) scanner with the OML- or SOML-protocol at different tube voltages during the head CT. A real time skin dosimeter was inserted into the phantom center of the head, and surfaces of the lens. We compared the real time skin dosimeter values at lens between the OML- and SOML-protocol., Results: There were no significant differences in the real time skin dosimeter values for the head in the scan area for each phantom at different tube voltages between the OML- and SOML-protocol (p > 0.05 for all phantom). Compared with the OML protocol, it is possible to reduce the real time skin dosimeter values at lens by approximately 80% by using the SOML protocol (p < 0.05) at all tube voltages. Compared with the OML protocol, it is possible to reduce the real time skin dosimeter values at mammary gland by approximately 20% by using the SOML protocol (p < 0.05) at all tube voltages., Conclusions: During the pediatric head CT examination, SOML protocol was possible to reduce the real time skin dosimeter values at lens by approximately 80% compared with OML protocol at all tube voltages., (Copyright © 2023. Chlorpromazine is a medication widely used in psychiatry for the treatment of psychoses, especially schizophrenia. Since 1964, published articles have been correlating this medication with the appearance of ocular alterations. In this paper, we report the case of a 65-year-old patient with ocular effects due to long-term therapy with chlorpromazine. Biomicroscopy of both eyes presented diffuse granular brown deposits, most prominent at the deep stroma and corneal endothelium level. Also showed anterior subcapsular brown deposits with a stellate pattern in the lens. The total amount exceeds 2.000g (significant for the ocular alterations described) considering the patient's daily dosage of chlorpromazine of 300mg for ten years. After performing complete ophthalmic evaluation and discarding other causes for the ocular deposits, we diagnosed a secondary corneal deposit and cataract due to the use of chlorpromazine. This case reinforces the importance of periodic follow-up with an ophthalmologist for chlorpromazine users to trace ocular changes, heeding the exposure time and its dosage. [ABSTRACT FROM AUTHOR]

Introduction: Clear lens extraction is considered in patients older than 50 years with primary angle closure and intraocular pressure greater than or equal to 30 mm Hg or damage due to glaucoma. The results are unknown in eyes with a moderate elevation of intraocular pressure. Objective: To evaluate the influence of preoperative intraocular pressure in the control of the primary angle closure treated with clear lens extraction. 14. Implantação de Lentes Iris-Claw Retropupilares: Resultados Refractivos e de Segurança

INTRODUCTION: Iris-claw intraocular lenses are one of the surgical alternatives to treat patients without capsular and zonular support after complicated cataract surgery, trauma, or diseases that disrupt the normal anatomy of the eye’s posterior chamber. Retropupillary fixation, through the enclavation of the lens to the posterior iris, constitutes a modification of the original technique, and is associated with fewer long-term complications, particularly regarding the loss of corneal endothelial cells. The purpose of this study was to evaluate the visual and refractive results and to report the associated complications of patients who underwent retropupillary implantation of the Artisan Aphakia IOL® (Ophtec). METHODS: A retrospective, observational, single-center study of consecutive eyes, including 26 patients who underwent surgery in Hospital Pedro Hispano, between 2012 and 2021. The outcome measures consisted of postoperative best-corrected visual acuity, spherical equivalent, intraocular pressure, and complications. RESULTS: The most common indication for surgery was previous complicated cataract surgery (50%), followed by trauma (26.9%) and spontaneous intraocular lens luxation (11.5%). The mean best-corrected visual acuity was 0.52 ± 0.62 logMAR and the mean spherical equivalent was 0.075 ± 1.46 diopters. Postoperative ocular hypertension occurred in 7.7% of the patients. The most common complication was the luxation of one of the haptics of the lens, which happened to 19.2%of patients. Cystoid macular edema was present in 11.5% of the patients. CONCLUSION: Retropupillary iris-claw intraocular lens implantation is a valid alternative for treating aphakia in the absence of capsular support. It is a safe technique, with decent functional outcomes. Most long-term complications are related to the status of the eye before surgery, as it is often performed in complex patients, with important comorbidities, and that should be con- sidered regarding the expected final outcome. Alternatives such as scleral fixation of intraocularlenses may provide a more predictable refractive outcome but are technically harder and depend on the surgeon’s experience. The surgical technique should be individualized according to the characteristics of the patient, and the preference and experience of the surgeon., INTRODUÇÃO: O implante de lentes intra-oculares de fixação à íris é uma das alternativas cirúrgicas para tratar doentes sem suporte capsular e zonular após cirurgia de catarata complicada, trauma, ou doenças que alteram a normal anatomia da câmara posterior do olho. A fixação retropupilar, através do enclavamento da lente à face posterior da íris, constitui uma modificação da técnica original, associada a menor taxa de complicações a longo prazo, sobretudo em relação à perda de células do endotélio corneano. O objectivo deste trabalho foi avaliar os resultados visuais e refractivos, e descrever as complicações associadas com a fixação retropupilar das lentes Artisan de Afaquia® (Ophtec). MÉTODOS: Foi feita uma análise retrospectiva, observacional, de olhos consecutivos, que incluiu 26 doentes submetidos à cirurgia no Hospital Pedro Hispano, entre 2012 e 2021. Foram analisadas a melhor acuidade visual corrigida, equivalente esférico, pressão intraocular e complicações pós-operatórias. RESULTADOS: A indicação cirúrgica mais comum foi a cirurgia de catarata complicada prévia (50%), seguida de trauma (26,9%) e de luxação espontânea da lente intraocular (11,5%). A melhor acuidade visual corrigida média foi de 0,52 ± 0,62 logMAR e o equivalente esférico médio foi de 0,075 ± 1,46 dioptrias. Dos doentes, 7,7% tiveram hipertensão ocular no período pós-operatório. A complicação pós-operatória mais frequente foi a luxação de um dos hápticos da lente, que ocorreu em 19,2% dos doentes, seguida do desenvolvimento de edema macular cistóide, em 11,5% dos casos. CONCLUSÃO: O implante de lentes com fixação à íris em posição retropupilar é uma alternativa válida para tratar a afaquia, na ausência de suporte capsular. Trata-se de uma técnica segura, com resultados funcionais satisfatórios. A maioria das complicações a longo prazo estão relacionadas com o estado funcional do olho antes da cirurgia, uma vez que esta é frequentemente realizada em doentes complexos e com comorbilidades importantes, factores que devem ser considerados para o resultado final. Alternativas como a fixação escleral de lentes intraoculares podem proporcionar um resultado refractivo mais previsível, mas são tecnicamente mais difíceis e dependem da experiência do cirurgião. A escolha da técnica cirúrgica deve ser individualizada de acordo com as características do doente e com a preferência e experiência do cirurgião.

16. Dosis de radiación al cristalino y al cerebro en estudios de tomografía computarizada. La importancia de la justificación de los estudios.

Objective: To know the accumulated doses that can be associated to repetitive computed tomography (CT) scans to show the importance of the justification. Method: A single patient underwent several CT studies, 35 head CTs, 4 chest CT, 2 chest CT angiography (CTA), 2 head CTA and 1 sinuses CT, during 5 months. Due to this, an estimation of the dose to the eye lens and brain was performed to show the importance of the justification of the exams. The dose to the eye lens and brain was estimated using CT-Expo v.2.4, a dedicated CT dosimetry software, where all the CT parameters of each of the scans were considered. Results: The cumulative dose to the eye lens and brain attributed to the 44 CT scans was 1860 mGy and 1580 mGy, respectively. There was not always a clear justification of all the performed CT scans. Conclusions: The cumulative dose to the eye lens was more than 3 times the threshold established by the International Commission on Radiological Protection for detectable opacities. Medical doctors should understand and always justify the examinations to avoid any detriment to the patients and avoid high cumulative doses when not justified. [ABSTRACT FROM AUTHOR]

- Abstract
We build a simple and inexpensive system that simulates the process of human accommodation. We used a ball lens, made of a male latex condom filled with water, and two diode lasers that can change from collimated to divergent beams, adjusting their rotatory supports. The device is suitable for secondary classes in optics. Students have calculated and measured the focal length of the ball lens (fmeasured /fcalculated = 0.94 ±0.06) and have studied how human vision shifts from the relaxed condition to the condition of visualizing nearby objects, by squeezing the lens (simulating the crystalline work). El coloboma de cristalino es una anomalía congénita que se caracteriza por una muesca del ecuador inferior del cristalino con una ausencia correspondiente de las fibras zonulares que causa un defecto en el borde del lente haciéndolo más esférico. Puede aparecer solo o asociado a otras malformaciones oculares como colobomas de iris, coroides o retina. Este defecto congénito puede causar ambliopía especialmente cuando está asociado a una catarata. Se describió el caso de una fémina de 29 años de edad con un coloboma unilateral de cristalino típico, no asociado a otras malformaciones, a la cual se le realizó tratamiento quirúrgico con técnica de facoemulsificación obteniéndose resultados satisfactorios.

Objetivo Comparar a acuidade visual (AV) final dos pacientes submetidos à cirurgia de catarata com e sem complicações intraoperatórias e possíveis fatores que contribuíram para o resultado ao final do pós-operatório. Métodos Análise dos prontuários de 179 pacientes, retrospectivo, longitudinal, operados de fevereiro a julho de 2013 no Instituto Dr. Suel Abujamra, pela técnica de facoemulsificação (FACO), divididos em dois grupos: com e sem complicações intraoperatórias. Os dados obtidos foram analisados pelos métodos teste-t para duas amostras ou Análise de Variância (ANOVA). Os critérios de exclusão foram pacientes com cirurgia ocular prévia, retinopatia diabética proliferativa ou não proliferativa grave, outras retinopatias, escavação do disco óptico igual ou maior que 0,7x0,7 ou uso de mais de dois hipotensores oculares, olho único, catarata secundária à uveíte, traumática ou congênita. Resultados Dos 179 pacientes, 37 (20,7%) tiveram complicações intraoperatórias e 142 (79,3%) não tiveram complicações intraoperatórias. A média da idade dos pacientes foi de 70,33 anos. O olho direito foi o operado em 49,7% dos casos, e o olho esquerdo em 50,3%. O diabetes mellitus estava presente em 29,05% dos casos, sendo 29,73% de pacientes com complicação e 28,87% sem complicações. Do total, 77,65% atingiram uma AV final 20/40 ou melhor, sendo que nos pacientes com complicação a média para esta AV foi de 59,46% e no grupo sem complicações foi de 82,40%. A principal complicação foi ruptura de cápsula posterior. Conclusão Após toda a análise, verificamos que os fatores que implicaram com significância estatística em menor AV final foram as complicações intraoperatórias e a idade dos pacientes.

Chlorpromazine is a medication widely used in psychiatry for the treatment of psychoses, especially schizophrenia. Since 1964, published articles have been correlating this medication with the appearance of ocular alterations. In this paper, we report the case of a 65-year-old patient with ocular effects due to long-term therapy with chlorpromazine. Biomicroscopy of both eyes presented diffuse granular brown deposits, most prominent at the deep stroma and corneal endothelium level. Also showed anterior subcapsular brown deposits with a stellate pattern in the lens. The total amount exceeds 2.000g (significant for the ocular alterations described) considering the patient's daily dosage of chlorpromazine of 300mg for ten years. After performing complete ophthalmic evaluation and discarding other causes for the ocular deposits, we diagnosed a secondary corneal deposit and cataract due to the use of chlorpromazine. This case reinforces the importance of periodic follow-up with an ophthalmologist for chlorpromazine users to trace ocular changes, heeding the exposure time and its dosage. RESUMO A clorpromazina é uma medicação muito empregada na psiquiatria para tratamento de psicoses, especialmente em casos de esquizofrenia. Desde 1964 existem artigos publicados que correlacionam o uso dessa medicação com o aparecimento de alterações oculares. Neste trabalho, relatamos o caso de um paciente de 65 anos com efeitos oculares devido à terapia de longo prazo com clorpromazina. A biomicroscopia de ambos os olhos apresentou depósitos granulares difusos e de cor marrom, mais proeminente ao nível do estroma profundo e do endotélio da córnea, além de depósitos castanhos subcapsulares anteriores centrais em um padrão estrelado no cristalino. Considerando a dose diária de clorpromazina de 300mg por 10 anos usada pelo paciente, a quantidade total ultrapassa 2.000g (dose considerada significativa para as alterações oculares descritas). Após avaliação oftalmológica completa e descartado outras causas desses depósitos oculares, foram diagnosticados depósito corneano e catarata secundários ao uso de clorpromazina. O caso apresentado reforça a importância do acompanhamento oftalmolÓgico periÓdico de usuários de clorpromazina para o rastreio de alteraçÕes oculares, atentando-se ao tempo de exposição à droga e à posologia da mesma.

RESUMEN: El ojo humano es un sistema óptico de una potencia aproximada de 60 dioptrías. Gran parte se debe a la córnea, que tiene una potencia de refracción fija de 43 dioptrías; el resto es debido al cristalino, que se trata de una lente biconvexa con una potencia variable gracias a un proceso denominado acomodación. Otra característica del cristalino es que su índice de refracción no es constante, sino que varía siguiendo un gradiente de índice (GRIN). Esta magnitud alcanza su valor máximo en el centro de la lente debido a la acumulación de proteínas en las fibras del núcleo, y va disminuyendo hasta la corteza. El objetivo de este proyecto es estudiar el efecto del gradiente de índice en la calidad óptica de la imagen y las aberraciones oculares. Para ello se realiza, usando Matlab, una simulación del trazado de rayos y se obtiene la distribución de luz en la retina. Para caracterizar el gradiente de índices del cristalino se usa una aproximación a un modelo de capas, con índices de refracción constante entre superficies. Este modelo se compara con el modelo de ojo completo, que considera dos superficies refractivas para la córnea y dos para el cristalino. ABSTRACT: The human eye is an optical system with an approximate optical power of 60 diop ters. Much of it is due to the cornea, which has a fixed refractive power of 43 diopters; the rest power is due to the lens, which is a biconvex lens with variable power as a result of a process called accommodation. Another characteristic of the lens is that its refractive index is not constant, but rather a continuous gradient index (GRIN). This magnitude reaches its maximun value in the center of the lens due to the accumulation of proteins in the fibers of the nucleus, and decreases to the cortex. The objective of this project is to study the effect of the index gradient on the optical quality of the image and on the ocular aberrations. To do this, a simulation of the ray tracing is performed in Matlab and the distribution of light on the retina is obtained. To characterize the index gradient of the lens, an approximation to a shell model is used, with constant refractive indices between surfaces. 24. Correlación de la densidad del cristalino medida por imágenes de Scheimpflug y parámetros facodinámicos en la optimización de la facoemulsificación

Introducción: Correlacionar el grado de la opacidad cristaliniana según el sistema Lens Opacities Classification System (LOCS III) y la densitometría por sistema de imágenes de Scheimpflug, y los beneficios de la densitometría como método eficaz en la optimización de la cirugía de catarata por facoemulsificación. Método: Se estudiaron 266 ojos con diferentes grados de dureza del cristalino que tenían criterio de cirugía. Se clasificaron por el mismo observador según el sistema de LOCS III, luego se les realizó densidad del cristalino según sistema de imágenes de Scheimpflug, y se tomaron los datos de poder de ultrasonido (PU), tiempo de ultrasonido (TU), tiempo de facoemulsificación efectiva (TFE), y conteo celular endotelial preoperatorio y posoperatorio a los 30 días de la cirugía. Calculándose el porcentaje de pérdida celular endotelial (PPCE). Se calcularon valores de media y desviaciones estándar, y correlación de Pearson, utilizando el paquete estadístico SPSS® versión 19. Resultados: La correlación del sistema LOCS III para opalescencia nuclear (NO) y color nuclear (NC) con la densitometría medida por sistema de imágenes de Scheimpflug fue positiva y alta, sin hallar diferencias significativas para NO y NC. Al correlacionar los parámetros facodinámicos con la densidad cristaliniana la correlación resultó lineal alta entre los mismos, al igual que la correlación entre densitometría y, PPCE y TEF con PPCE. Conclusiones: La medición de la densidad del cristalino a través del sistema de imágenes de Scheimpflug ha demostrado ser superior al sistema de clasificación LOCS III, demostrándose las ventajas de este método en la planificación preoperatoria de los parámetros facodinámicos en la cirugía de cataratas por facoemulsificación, permitiendo además la personalización de esta cirugía.

OBJETIVO:Correlacionar os níveis de straylight, de scattering, de opacidade do cristalino (LOCS III) e das aberrações ópticas intraoculares em pacientes com catarata nuclear. MÉTODOS: Estudo observacional, prospectivo envolvendo 30 olhos de 30 pacientes com diagnóstico de catarata nuclear. A avaliação do straylight (espalhamento de luz) através do Log(s) foi realizada pelo C-Quant (Oculus, Wetzlar, Germany). A densitometria do cristalino (scattering) através do PNS (Pentacam Nucleus Staging) foi realizada pela tomografia de córnea e segmento anterior (Pentacam® - Oculus, Wetzlar, Germany) e a análise do tipo e graduação da opacidade do cristalino foi feito pelo Lens Opacities Classification System (LOCS III). O Ray tracing (iTrace, Vista, Tracy Technologies) foi utilizado para avaliação da aberrometria total e intraocular onde foi calculado com a integração da Topografia de Plácido. Os critérios de exclusão foram: doença corneana, doenças da retina e/ou nervo óptico e cirurgia ocular prévia. O Teste de Kolmogorov-Smirnov foi utilizado para avaliar a distribuição normal e o Teste de Spearman (não paramétrico) utilizado para as correlações entre as variáveis Log(s) x PNS; Log(s) x LOCS III; LOCS III x PNS; RMS IO x PNS; RMS IO x LOCS III e RMS IO x Log(s). Foi considerado como estatisticamente significante p≤ 0,05%. RESULTADOS: A média de idade dos pacientes foi de 72,3 anos (±8,9). As médias do PNS, do LOCS III, do Log(s) e da RMS IO, foram 2,7 (±1,14), 3,7 (±1,15), 1,49 (±0,38) e 1,23 (±0,5), respectivamente. As correlações de Spearman e o seu coeficiente de correlação (rho) foram p=0,270 (rho 0,20) entre Log(s) x PNS, um p=0,717 (rho 0,06) entre Log(s) x LOCS III, um p=0 (rho 0,76) entre LOCS III x PNS, um p=0,41 (rho -0,15) entre RMS IO x PNS, um p=0,39 (rho -0,15) entre RMS IO x LOCS III e um p=0,83 (rho 0.03) entre RMS IO x Log(s). CONCLUSÃO:A perda da qualidade de visão não se restringe somente aos resultados da acuidade visual, logo novos métodos objetivos e qualitativos do desempenho visual têm sido introduzidos. Os estudos do espalhamento de luz na retina, da sensibilidade ao contraste e da análise da óptica intraocular através da aberrometria são novos parâmetros funcionais que associados a métodos objetivos de diagnóstico como o PNS vão nos permitir o entendimento e o conhecimento da qualidade de visão destes pacientes e o seu prognóstico visual.

Se presenta un caso clínico asistido de urgencia en el Hospital Universitario Clínico Quirúrgico Comandante Faustino Pérez Hernández, de la ciudad de Matanzas, en el año 2012, con un cuerpo extraño intraocular a nivel del cristalino en una paciente del sexo femenino, de 43 años de edad, ama de casa, con antecedente de trauma ocular en su hogar. En el examen físico presentaba déficit visual de 0,3 con herida corneal, que afectaba, además, limbo corneo escleral y cristalino con cuerpo extraño incluido. En examen ecográfico se observó imagen ecogénica compatible con cuerpo extraño a nivel del cristalino. Se suturó córnea y se extrajo cuerpo extraño con evolución satisfactoria; no presentando secuelas.We present a clinical case attended at the emergency unit of the University Clinic-Surgical Hospital Comandante Faustino Perez, of Matanzas, in 2012, with a foreign intraocular body at the level of the crystalline lens in a female patient aged 43 years old, housewife, with antecedents of an ocular trauma at home. At the physical examination she had a visual deficit of 0,3, and a corneal injury affecting also the cornea-scleral limbus and the crystalline with an included foreign body. At the echographic examination we found an echogenic image compatible with a foreign body at the level of the crystalline. The cornea was sutured and the foreign body was extracted with a satisfactory evolution; there were not sequels.

Los traumatismos oculares pueden provocar opacidad secundaria del cristalino, así como la pérdida de su posición anatómica o la de la lente intraocular. Estas son afecciones frecuentes con devastadoras consecuencias si no son atendidas correctamente. Con el desarrollo de las nuevas técnicas quirúrgicas, los resultados anatómicos y visuales de estos pacientes operados son cada día más sorprendentes; pero es mandatorio examinar exhaustivamente al paciente, el abordaje en el momento idóneo y la elección de la técnica quirúrgica adecuada.Ocular injuries can develop secondary opacity of the crystalline lens as well as the loss of its anatomical position or intraocular lens dislocation. These disorders can frequently lead to devastating consequences if not properly assisted. Recent surgical techniques have dramatically changed the visual prognosis of these patients, achieving amazing anatomical and functional success. However, thorough examination of the patient and the selection of the right surgical time and surgery technique are the keys to attaining good final visual results.

This is a case report of a 45-year-old anisometropic woman with stage 2 keratoconus submitted to refractive lens exchange employing SN60T5 AcrySof Toric intraocluar lens on her left eye. Phacoemulsification followed by proper lens implantation was accomplished successfully. On postoperative period, favorable refractive and visual outcomes were obtained. The use of toric intraocular lenses may be an option in the refractive treatment of stage 2 keratoconus in selected cases.

OBJETIVO: Avaliar o perfil demográfico dos pacientes atendidos na Campanha da Catarata e correlacionar as características do sistema de referenciamento com o desfecho cirúrgico. MÉTODOS: Estudo prospectivo, observacional, realizado com pacientes atendidos no ambulatório de Oftalmologia do Hospital Santa Clara do Complexo Hospitalar Santa Casa de Porto Alegre referenciados à consulta via Campanha de Catarata entre fevereiro e abril de 2008. Foi utilizado questionário de avaliação dos pacientes atendidos e realizado exame oftalmológico de biomicroscopia e fundoscopia. RESULTADOS: Dos 234 pacientes com questionários completos, a média de espera pela consulta no sistema terciário foi de 2,5 meses, variando de 1 a 24 meses. Do total de pacientes incluídos no estudo, 152 (65%) foram operados. CONCLUSÃO: Através da análise do atendimento dos pacientes referenciados pôde-se perceber, pelo tempo de espera para a consulta no sistema terciário e pelo percentual de pacientes operados, a eficiência e agilidade proporcionada por esse tipo de campanha de saúde pública.OBJECTIVE: Evaluate the demographic profile of patients assisted on Cataract Campaign and correlate the characteristics of the reference system incoming patients with surgery results. METHODS: Prospective study, observational, performed with outpatients assisted on the Ophthalmology Ambulatory of Santa Clara's Hospital (Complexo Hospitalar Santa Casa de Porto Alegre) in Cataract Campaign between february and april 2008. It was utilized a questionnaire to evaluate assisted patients and performed the ophthalmological exam by biomicroscopy and fundoscopy. RESULTS: From 234 patients with complete questionnaire, the average waiting time for the consult in the tertiary system was 2,5 months, varying from 1 to 24 months. From the total of patients included in the study, 152 (65%) were operated. CONCLUSION: Through the analyses of the attending patients who were referred we can realize by the time of waiting for the consult in the tertiary system and by the percentage of patients operated the efficiency and agility provided by this public health campaign.

33. Assessment of the biometric modifications of the eye in LASIK during suction.

Purpose: To study the biometric modifications of the eyeball during suction in Laser assisted in Situ Keratomileusis (LASIK)., Methods: Observational and cross-sectional study. We studied 43 patients who underwent surgery for myopia and myopic astigmatism. Mean age was 38.3 ± 11.5 years, and 19 were female (44.2%). Conventional LASIK surgery with a manual microkeratome was performed. Before and during the suction maneuvre the following parameters were measured using an 11 Mhz biometric probe: aqueous depth (AQD), lens thickness (LT), vitreous cavity length (VCL) and axial length (AXL). Paired t-test was used to compare the biometric measurements before and during suction., Results: The mean spherical equivalent refractive error was -4.5 ± 2.3 diopters. During suction, the AQD did not change significantly (p = 0.231). However, AXL and VCL increased by 0.12 mm and 0.22 mm respectively (p = 0.039 and <0.01) and LT decreased by 0.20 mm (p < 0.01). AXL increased in 42% of the eyes and decreased in 16%, VCL increased in 70% of the eyes and decreased in 9%, and the LT was reduced in 67% of the eyes., Conclusions: Suction maneuvres during LASIK surgery produce changes of little magnitude in the eye globe, mainly a decrease in LT and an increase in VCL and AXL. The term dysfunctional lens syndrome has gained acceptance in the field and encompasses natural changes due to aging of crystalline lens. The evolution of diagnostic devices has been a key factor in better staging, understanding and characterizing of these degenerative changes. Even with these technological advances and the use of subjective classifications, such as the classic Lens Opacities Classification System, an objective staging of early dysfunctional lens syndrome has yet to be established. Ocular wavefront aberrometry and objective scatter index, associated with Scheimpflug backscatter densitometry, have proven instrumental in detecting early dysfunctional lens syndrome. Staging of early dysfunctional lens syndrome has been proposed in the literature, but no classification has been recognized worldwide. The purpose of this literature review is to assess the current state of dysfunctional lens syndrome from a technological perspective and propose a new staging system to assist surgeons in making surgical decisions. RESUMO O termo “síndrome disfuncional do cristalino” tem sido mais aceito na área e engloba mudanças naturais devido ao envelhecimento do cristalino. A evolução dos dispositivos diagnósticos tem sido fator fundamental para melhor estadiamento, compreensão e caracterização dessas alterações. Mesmo com esses avanços tecnológicos e o uso de classificações subjetivas, como o Lens Opacities Classification System , um estadiamento objetivo da síndrome disfuncional do cristalino precoce ainda não foi estabelecido. A aberrometria ocular total e o índice de superfície ocular, associado à densitometria de Scheimpflug, mostraram-se instrumentais na detecção da síndrome disfuncional do cristalino precoce. Embora estadiamentos precoces de síndrome disfuncional do cristalino tenham sido propostos na literatura, nenhum foi reconhecido mundialmente até o momento. O objetivo desta revisão de literatura é avaliar o estado atual da síndrome disfuncional do cristalino a partir de uma perspectiva tecnológica, e propor um novo sistema de estadiamento para auxiliar os cirurgiões na tomada de decisões cirúrgicas.

El cristalino es una estructura transparente y biconvexa ubicada posterior al iris y anterior al cuerpo vítreo. El cristalino está suspendido en su posición por fibras delicadas pero fuertes, llamadas fibras zonulares o zónulas de Zinn) que lo sostienen y adhieren al cuerpo ciliar. Los componentes del cristalino son la cápsula, el epitelio, la corteza y el núcleo (1). La lente sigue creciendo a lo largo de la vida. Al nacer, mide aproximadamente 6,4 mm ecuatorialmente y 3,5 mm anteroposteriormente y pesa aproximadamente 90 mg. En los adultos mide típicamente 9-10 mm en su ecuador y aproximadamente 5 mm anteroposteriormente y pesa aproximadamente 255 mg. Con la edad, aumenta el grosor de la corteza cristalina; adoptando una forma cada vez más curva para tener más poder refractivo. Sin embargo, el índice de refracción disminuye con la edad, probablemente como resultado de la creciente presencia de proteínas insolubles. Por tanto, el ojo puede volverse más hipermétrope o más miope con la edad, dependiendo del equilibrio de estos cambios opuestos (1). La cápsula del cristalino, muy importante en la cirugía de cataratas porque soportará el cristalino intraocular, es una membrana basal elástica y transparente que está formada por colágeno tipo IV y otras proteínas de la matriz, producidas por células epiteliales. La cápsula contiene la lente y es capaz de cambiar su forma durante los cambios de acomodación. La capa exterior de la cápsula del cristalino, llamada laminilla zonular, también sirve como punto de unión para las fibras zonulares. La cápsula del cristalino es más gruesa en las zonas preecuatoriales anterior y posterior y más fina en la parte central de su polo posterior, donde puede medir solo 2-4 μm. La cápsula anterior del cristalino es considerablemente más gruesa que la cápsula posterior al nacer y su grosor aumenta con la edad (1). A medida que el cristalino envejece, aumenta su masa y grosor y, a su vez, disminuye su poder de acomodación. A medida que se forman nuevas capas de fibras corticales de forma concéntrica, el núcleo se comprime y se endurece, en un proceso conocido como esclerosis nuclear. La modificación química y la escisión proteolítica de las proteínas del cristalino dan como resultado la formación de agregados de proteínas de alto peso molecular. Estos agregados pueden ser lo suficientemente grandes como para causar fluctuaciones abruptas en el índice local de refracción de la lente, dispersando la luz y reduciendo la transparencia. La modificación química de las proteínas del cristalino nuclear también aumenta la opacidad, de modo que el cristalino se vuelve cada vez más amarillo o marrón con la edad. Otros cambios relacionados con la edad incluyen la disminución de las concentraciones de glutatión y potasio y el aumento de las concentraciones de sodio y calcio en el citoplasma de la célula del cristalino (2-3). La invención de la técnica de facoemulsificación por Charles Kelman en 1967 marcó el comienzo de la era moderna de la cirugía de cataratas. Aunque inicialmente encontró una fuerte resistencia, la facoemulsificación ganó popularidad en la década de 1990. En este procedimiento, se utiliza una punta accionada por ultrasonidos para emulsionar el núcleo de la lente y eliminar fragmentos con un sistema de aspiración automático. Este cambio de paradigma permitió que la cirugía de cataratas se realizara a través de incisiones corneales relativamente pequeñas, lo que resultó en una menor incidencia de complicaciones relacionadas con la herida y pérdida de vítreo, además de una recuperación visual más rápida. Este avance también coincidió con la invención de los dispositivos viscoquirúrgicos oftálmicos, la evolución del diseño de lentes intraoculares y la transformación de la cirugía de cataratas en un procedimiento ambulatorio (5) Desde 1949, cuando Harold Ridley implantó la primera lente intraocular, el polimetilmetacrilato (PMMA) fue el material de elección para lentes y la "regla empírica" por la que se juzgaron otros materiales. Utilizando un material rígido como el PMMA, el diámetro mínimo de la óptica es de 5 mm y por tanto la herida tenía que tener una dimensión similar. The lens is suspended in position by delicate but strong fibers, called zonular fibers, or zonules of Zinn) that support and adhere it to the ciliary body. The components of the lens are capsule, epithelium, cortex, and nucleus (1). The lens continues to grow throughout life. At birth, it measures approximately 6.4 mm equatorially and 3.5 mm anteroposteriorly and weighs approximately 90 mg. In adults it typically measures 9-10 mm at its equator and approximately 5 mm anteroposteriorly and weighs approximately 255 mg. With age, the thickness of the crystalline cortex increases; adopting an increasingly curved shape to have more refractive power. However, the refractive index decreases with age, probably as a result of the increasing presence of insoluble proteins. Therefore, the eye may become more hyperopic or more myopic with age, depending on the balance of these opposing changes (1). The lens capsule, very important in cataract surgery because it will support the intraocular lens, is an elastic and transparent basement membrane that is made up of type IV collagen and other matrix proteins, produced by epithelial cells. The capsule contains the lens and is capable of changing its shape during accommodative changes. The outer layer of the lens capsule, called the zonular lamella, also serves as the point of attachment for the zonular fibers. The lens capsule is thickest in the anterior and posterior pre-equatorial zones and thinner in the central part of its posterior pole, where it can measure only 2-4 μm. The anterior capsule of the lens is considerably thicker than the posterior capsule at birth, and its thickness increases with age (1). As the lens ages, it increases in mass and thickness, and in turn decreases in accommodative power. As new layers of cortical fibers form concentrically, the nucleus compresses and stiffens, in a process known as nuclear sclerosis. Chemical modification and proteolytic cleavage of lens proteins result in the formation of high molecular weight protein aggregates. These aggregates can be large enough to cause abrupt fluctuations in the local index of refraction of the lens, scattering light and reducing transparency. Chemical modification of nuclear lens proteins also increase opacity, such that the lens becomes increasingly yellow or brown with advancing age. Other age-related changes include decreased glutathione and potassium concentrations and increased sodium and calcium concentrations in the cytoplasm of the lens cell (2-3). The invention of the phacoemulsification technique by Charles Kelman in 1967 ushered in the modern era of cataract surgery. Although initially met with strong resistance, phacoemulsification gained popularity in the 1990s. In this procedure, an ultrasonically actuated tip is used to emulsify the lens nucleus and remove fragments with an automated aspiration system. This paradigm shift allowed cataract surgery to be performed through relatively small corneal incisions, resulting in a lower incidence of wound-related complications and vitreous loss, in addition to faster visual recovery. This advance also coincided with the invention of ophthalmic visco-surgical devices, the evolution of intraocular lens design, and transformed cataract surgery into an outpatient procedure (5) Since 1949, when Harold Ridley implanted the first intraocular lens, polymethylmethacrylate (PMMA) was the lens material of choice and the "rule of thumb" by which other materials were judged. Using a rigid material such as PMMA, the minimum diameter of the optic is 5 mm and therefore the wound had to be of a similar dimension. To preserve the advantages of a small phacoemulsification incision, several materials have been developed that allow the IOL to fold (6 ).

- Abstract
Spontaneous rupture of the anterior lens capsule, evolving with anterior dislocation of the lens nucleus is a rare complication of hypermature cataract. We report a case of a female patient, 79 years old, who presented with spontaneous rupture of the anterior lens capsule with anterior dislocation of the nucleus in the right eye with no history of ocular trauma. The diagnosis was confirmed by the visualization of the tabs of the anterior capsule, especially when the nucleus was removed. We perform phacoemulsification of the nucleus by the risk of decompensating the cornea. It is important to highlight the rarity of the case, the first being reported in Brazil. [ABSTRACT FROM AUTHOR]

41. Reconstruction of the gradient refractive index of the crystalline lens with optimization methods.

This publication represents a summary of the results of the doctoral thesis Reconstruction of the Gradient Refractive Index of the Crystalline Lens presented in Universidad de Valladolid, Spain, on July, 2012. The optical properties of the crystalline lens of the eye depend on the shape of the external surfaces and on its refractive index. In humans and many other species, the refractive index of the lens is not homogeneous and there is a gradient refractive index (GRIN) that peaks around the center of the lens. To study the influence of the GRIN in the optics of the lens, a method to reconstruct the GRIN in in vitro lenses was proposed in this PhD thesis. The method was used to measure the GRIN of lenses of animal models and humans and study its changes with age or its influence on the optical properties of the lens. Also, the effect of the GRIN in the visualization of the posterior surface of the lens in Optical Coherence Tomography images was quantified. [ABSTRACT FROM AUTHOR]

RESUMEN El glaucoma se puede definir como una neuropatía óptica crónica irreversible. Es la segunda causa de ceguera irreversible en Estados Unidos y la más frecuente a nivel mundial. En Cuba es la primera causa. El cristalino puede comportarse en ocasiones como inductora de glaucoma, específicamente si no está en condiciones fisiológicas: glaucomas inducidos por el cristalino, por ejemplo, el facoanafiláctico. Se presenta el caso de un paciente masculino de 89 años de edad operado de catarata senil con luxación a vítreo de restos de cristalino hace 7 años, acude con sintomatología inflamatoria e hipertensiva oculares recientes del ojo operado. Con el diagnóstico de glaucoma facoanafiláctico se pone tratamiento médico y quirúrgico, con lo cual remitió el cuadro, con ligeras secuelas visuales. En esta entidad aparece el antecedente de cirugía de cristalino o herida penetrante y luego un período de latencia, durante el cual se produce la sensibilización a las proteínas cristalinianas. El tratamiento antinflamatorio, antihipertensivo y desaparición del agente causal es el indicado. Diagnosticar oportunamente los factores de riesgos del glaucoma facoanafiláctico, y orientar a los pacientes que los presentan, constituyen los pilares fundamentales para evitar los daños colaterales por esta entidad. ABSTRACT Glaucoma can be defined as an irreversible chronic optic neuropathy. It is the second cause of irreversible blindness in the United States and the most frequent worldwide. In Cuba it is the first cause. The lens can sometimes behave as a glaucoma inducer, specifically if it is not in physiological conditions: glaucoma induced by the lens, for example, the phacoanaphylactic. We present the case of an 89-year-old male patient operated on for a senile cataract with a vitreous dislocation of crystalline rests 7 years ago, who presented with recent eye inflammatory and hypertensive symptoms of the operated eye. With the diagnosis of phacoanaphylactic glaucoma, medical and surgical treatment was applied, with which the problem was solved, with slight visual sequelae. In this entity there is a history of lens surgery or penetrating wound and then a period of latency, during which the sensitization to the crystalline proteins takes place. The anti-inflammatory, antihypertensive treatment and disappearance of the causative agent is indicated. To diagnose opportunely the risk factors of the phacoanaphylactic glaucoma, and to orient the patients who present them, constitute the fundamental pillars to avoid the collateral damages by this entity.

Cellulose acetate electrophoresis of eye-lens proteins and Polyacrylamide flat gel electrophoresis of skeletic muscle proteins of six species of marine catfishes were carried out. Genetic polymorphism only occured at one locus of the electropherograms of eye-lens of Cathorops spixii. Ontogenetic variations in the relative concentration of bands were found in the electropherograms of eye-lens and skeletic muscle proteins. The six species of catfishes can be identified by means of quantitative and qualitative differences in the electropherograms. Coefficients of similarity were determined by the band-counting method and UPGMA dendrograms were constructed to illustrate the interspecific relationships among the species.Eletroforeses de proteínas gerais de cristalinos e de músculo esquelético de seis espécies de bagres marinhos foram realizadas, respectivamente, em membranas de acetato de celulose e em géis de poliacrilamida. Polimorfismo genético ocorreu apenas em um locus de eletroferogramas do cristalino de Cathorops spixii. Variações ontogenéticas nas concentrações relativas das bandas foram observadas nos eletroferogramas do cristalino e do músculo esquelético. As seis espécies de bagres marinhos podem ser identificadas através das diferenças quantitativas e qualitativas nos eletroferogramas. Coeficientes de similaridade foram determinadas pelo método de contagem de bandas e dendrogramas UPGMA foram construídos para ilustrar as relações interespecíficas entre as espécies.

Gold catalysts supported on ZrO2 and ZrO2-SiO2 were prepared by incipient impregnation and deposition-precipitation with urea (DPU) with 1% of nominal gold loading. The solids were characterized by X-ray diffraction analysis (XRD), N2-physisorption and temperature programmed reduction (TPR). Catalytic activities were investigated in cinnamaldehyde hydrogenation. The aim of this study was to report the frequency and characterization of cataracts diagnosed in dogs that attended consultation at the Ocularvet Ophthalmological Unit, in the city of Medellín (Antioquia, Colombia) between August 2019 and August 2020. A sampling was carried out at convenience, with the inclusion of dogs of all ages with a diagnosis of cataract and the exclusion of those with previous treatments or with pathologies that make it difficult to evaluate the lens. The population was demographically characterized (breed, sex, age, previous / concomitant diseases), in addition to information related to cataract (affected eye, cataract location, degree of development). An ophthalmological evaluation was performed on 442 dogs during the study year, of which 36% were diagnosed with cataracts, representing 203 affected eyes. Male dogs (54.1%), Poodle (11.9%), Schnauzer (10.1%) and crossbreds (8.2%) were mainly affected. The frequency of cases between the age groups before 3 years of age was similar and increased after 7 years. Ten of the 159 patients (6.3%) had previous or concomitant diseases at the time of cataract diagnosis, the most frequent being type I diabetes mellitus (2.5%). The nuclear location (23.2%) was the most frequent. Likewise, the degree of incipient development (44.3%) and mature cataract (30.5%) were the most frequent. It is concluded that the degree of incipient development was the most common form of cataract, without a defined distribution by breed, sex or age, El objetivo del presente estudio fue reportar la frecuencia y caracterización de cataratas diagnosticadas en perros que asistieron a consulta a la Unidad Oftalmológica Ocularvet, en la ciudad de Medellín (Antioquia, Colombia) entre agosto de 2019 y agosto de 2020. Se realizó un muestreo a conveniencia, con la inclusión de perros de todas las edades con diagnóstico de catarata y la exclusión de aquellos con tratamientos previos a la misma o con patologías que dificultaran la evaluación del cristalino. Se caracterizó demográficamente la población (raza, sexo, edad, enfermedades previas/concomitantes), además de la información relacionada con la catarata (ojo afectado, ubicación de la catarata, grado de desarrollo). Se realizó una evaluación oftalmológica a 442 perros durante el año de estudio, de los cuales el 36% fue diagnosticado con cataratas, representando 203 ojos afectados. Perros machos (54.1%), de raza Poodle (11.9%), Schnauzer (10.1%) y mestizos (8.2%), fueron los principalmente afectados. La frecuencia de casos entre los grupos etarios antes de los 3 años de edad fue similar y se incrementó a partir de los 7 años. Diez de los 159 pacientes (6.3%) presentaron enfermedades previas o concomitantes al momento del diagnóstico de catarata, siendo la más frecuente la diabetes mellitus tipo I (2.5%). La ubicación nuclear (23.2%) fue la más frecuente. Así mismo, el grado de desarrollo incipiente (44.3%) y la catarata madura (30.5%) fueron las de mayor frecuencia. Se concluye que el grado de desarrollo incipiente fue la forma más común de catarata, sin una distribución definida por raza, sexo o edad.

Os principais processos que determinam a composição química das águas superficiais e subterrâneas da Bacia da Gameleira, no município de Aiuaba/CE foram estudados. A área é preservada pelo IBAMA e faz parte do projeto IBESA (Implantação de Bacias Experimentais no Semi-Árido). Análises hidroquímicas de três açudes e de três cacimbas coletadas durante o período de fevereiro de 2003 a maio de 2004 mostram que: (i) as águas dos açudes são menos mineralizadas, com STD até 274,3 mg/L e as das cacimbas, mais mineralizadas com STD até 794 mg/L; (ii) o risco de sódio é baixo em todas as amostras; (iii) predominam águas bicarbonatadas mistas; águas cloretadas magnesianas foram encontradas em uma cacimba; (iv) o excesso de Cl- em relação ao Na+ na cacimba C2 está associado ao magnésio (v) Os diagramas de Gibbs indicam que as águas têm domínio de interação com as rochas na maioria das amostras.

Crystalline lens is a basic component of the vision organ and one of its diseases, the cataract, is the first cause of blindness in the world. Currently there are safe and effective surgical techniques to solve this pathological condition. It is fundamental that primary care physician provides adequate information to the patients, so that he/she can guide them in the search of the solution to cataract. In this article we reviewed recognized textbooks and using Medline we found some representative articles on this subject.
