Permanent pacemaker implantation (PPMI) reduction and optimal management of newly acquired conduction disturbances after transcatheter aortic valve implantation (TAVI) are crucial. We sought to evaluate the relation between transcatheter heart valve (THV) implantation depth and baseline and newly acquired conduction disturbances on PPMI after TAVI. This study included 1,026 consecutive patients with severe symptomatic aortic stenosis (mean age 79.7 ± 8.4 years; 47.4% female) who underwent TAVI with the newer-generation self-expanding THVs Primary outcomes were early and late PPMI defined as the need for PPMI during the index admission and between discharge and 30 days, respectively. Early and late PPMI was required for 115 (11.2%) and 21 patients (2.0%), respectively. Early PPMI rates decreased from 26.7% in 2015 and 2016 to 5.7% in 2021, and so did the mean THV depth from 4.4 ± 2.4 mm to 1.8 ± 1.6 mm. Receiver operator characteristics curve analyses showed THV depth had significant discriminatory value for early and late PPMI with cutoff values of 3.0 and 2.2 mm, respectively. Rates of early and late PPMI were significantly lower for patients with shallower compared with deeper implantations (5.1% vs 22.6% and 0.4% vs 4.1%, p 0.001 for both, respectively). Furthermore, rates of early PPMI were lower with shallower implantations in patients with new left bundle branch block after TAVI (2.4% vs 15.9%; p 0.001) and those with baseline right bundle branch block (7.5% vs 29.6%; p = 0.017). Lower rates of PPMI with shallower THV implantation were consistently observed, including in patients with baseline and newly acquired conduction disturbances. Our findings might help optimize the management of a temporary pacemaker after TAVI.

Resumen Introducción: El dispositivo MyVal balón-expandible (Meril Life Sciences, India) fue recientemente aprobado en Chile y la Unión Europea para uso comercial. El objetivo del presente estudio fue proveer datos de eficacia y seguridad temprana y de mediano plazo en pacientes portadores de estenosis aórtica (EA) severa de alto riesgo quirúrgico sometidos a reemplazo valvular aórtico percutáneo (RVAP) con dispositivo MyVal. Material y Métodos: Se enrolaron retrospectivamente todos los pacientes con EA severa sintomática de alto riesgo según criterio del ´heart team´ local, sometidos a RVAP usando prótesis MyVal en Hospital San Borja Arriarán. Resultados: La población quedó compuesta por 14 sujetos tratados entre Octubre 2018 y Noviembre 2019. La población tuvo una edad media de 82,5±7,8 años y elevado perfil de riesgo (puntaje STS promedio 11,6±5,1% de mortalidad a 30 días). Se logró éxito de dispositivo y procedimiento en 12 pacientes (86%) con caída sustancial de la gradiente aórtica media, persistente a 6 meses de seguimiento sin insuficiencia aórtica más que leve. Ocurrió falla de dispositivo en 2 pacientes, una debida a falla de entrega y otra por embolización a ventrículo, esta última con resultado de muerte. En términos de complicaciones, ocurrió una muerte precoz atribuida a disección/ruptura de aorta y 2 hemorragias mayores. La tasa de marcapasos ascendió a 3 pacientes, 23% considerando todos quienes recibieron implante. Conclusión: El presente registro cuestiona la seguridad de MyVal en el tratamiento percutáneo de la EA severa de alto riesgo. Sin embargo, una vez logrado un implante exitoso MyVal muestra adecuados parámetros de funcionamiento, persistentes a un plazo mediano de seguimiento.

Myocardial ischemia/reperfusion-related oxidative stress as a result of cardiopulmonary bypass is thought to contribute to the adverse clinical outcomes following surgical aortic valve replacement (SAVR). Although the acute response following this procedure has been well characterized, much less is known about the nature and extent of oxidative stress induced by the transcatheter aortic valve replacement (TAVR) procedure. We therefore sought to examine and directly compare the oxidative stress response in patients undergoing TAVR and SAVR. A total of 60 patients were prospectively enrolled in this exploratory study, 38 patients undergoing TAVR and 22 patients SAVR. Reduced and oxidized glutathione (GSH, GSSG) in red blood cells as well as the ferric-reducing ability of plasma (FRAP) and plasma concentrations of 8-isoprostanes were measured at baseline (S1), during early reperfusion (S2), and 6-8 hours (S3) following aortic valve replacement (AVR). TAVR and SAVR were successful in all patients. Patients undergoing TAVR were older (79.3±9.5 vs. 74.2±4.1 years; P<0.01) and had a higher mean STS risk score (6.6±4.8 vs. 3.2±3.0; P<0.001) than patients undergoing SAVR. At baseline, FRAP and 8-isoprostane plasma concentrations were similar between the two groups, but erythrocytic GSH concentrations were significantly lower in the TAVR group. After AVR, FRAP was markedly higher in the TAVR group, whereas 8-isoprostane concentrations were significantly elevated in the SAVR group. In conclusion, TAVR appears not to cause acute oxidative stress and may even improve the antioxidant capacity in the extracellular compartment.

Background Robot-assisted minimally invasive heart surgery is an effective alternative when compared with classical approaches. It has a low mortality and postoperative complications and its long-term durability is comparable with conventional techniques. Aim: To report short- and long-term results with the use of a robot-assisted transthoracic approach. Patients and Methods: Review of patients undergoing heart surgery between 2015 and 2019 using a robot assisted minimally invasive technique in a single center. We analyzed demographic characteristics, surgical and early ultrasound results. Results: Thirteen procedures were reviewed, nine mitral valve repairs (MVR) in patients aged 61 ± 21 years (seven males) and four atrial septal defect (ASD) closures in patients aged from 24 to 52 years (three men). For MVR, the average extracorporeal circulation and myocardial ischemia times were 120 ± 20.9 and 89 ± 21 minutes, respectively. The median hospitalization was four days. Two cases of MVR had postoperative complications. There was no mortality. All cases showed improvement in their symptoms. Ultrasound findings showed no postoperative mitral insufficiency except in one case. Conclusions: We report very good results in both complex mitral repair and CIA closure, comparable to centers with high standards in minimally invasive robot-assisted heart surgery.

A previously healthy, 39-year-old obese farmer, arrived hypotensive and tachycardic, with fever, myalgia, headache, abdominal pain, diarrhea, and progressive dyspnea. Ten days before symptoms onset, he was in direct contact with mice and working in a contaminated drain. Patient laboratory showed acute kidney injury and thrombocytopenia. Chest X-ray exhibited bilateral diffuse interstitial infiltrates. First-line empirical antibiotics were started and influenza discarded. Patient evolved with severe respiratory failure, associated with hemoptysis, and rapidly severe hemodynamic compromise. Despite neuromuscular blockade and prone positioning, respiratory failure increased. Accordingly, veno-venous ECMO was initiated, with bilateral femoral extraction and jugular return. After ECMO connection, there was no significant improvement in oxygenation, and low pre-membrane saturations and low arterial PaO2 of the membrane showed that we were out of the limits of the rated flow. Thus, a second membrane oxygenator was installed in parallel. Afterward, oxygenation improved, with subsequent perfusion enhancement. Regarding etiology, due to high suspicion index, Leptospira serology was performed, coming back positive and meropenem was maintained. The patient ultimately recovered and experience excellent outcome. The clinical relevance of the case is the scared evidence of leptospirosis-associated severe respiratory failure treated with ECMO. This experience emphasizes the importance of an optimal support, which requires enough membrane surface and flow for an obese, highly hyperdynamic patient, during this reversible disease. A high index of suspicion is needed for an adequate diagnosis of leptospirosis to implement the correct treatment, particularly in the association of respiratory failure, pulmonary hemorrhage, and an epidemiological-related context.

AIM Transcatheter mitral valve implantation (TMVI) is a novel technology for patients with severe mitral valve disease but at high surgical risk. Imaging guidance during the procedure is critical for successful device deployment. Identification of the mitral annular plane (MAP) with fluoroscopy during the procedure is limited by lack of clearly defined landmarks. We hypothesized that a plane defined by left circumflex-right coronary arteries (LCX-RCA) would have a consistent relationship to MAP. METHODS AND RESULTS We studied 25 patients with gated cardiac computed tomography. We identified the MAP and the LCX-RCA plane in mid systole and diastole. The distance between the two planes in prespecified four points (anterior, posterior, medial, and lateral) in the apical 2 and 3-chamber views. Alignment of the planes was described by cranial/caudal angulation for both planes in RAO 30° and LAO 90° (lateral) angulation. En el ámbito de las investigaciones educativas se da una convergencia de estudios de diferentes aspectos de la realidad social. Cuando consideramos los principios filosóficos que fundamentan la ética de las investigaciones educativas se puede afirmar que la orientación de estas es, por una parte, la orientación del saber hacia un fin bueno, y por otra, asegurar un conocimiento científico. En ambos casos, lo que está en juego es la relación del investigador educativo con la población en estudio. En función de aclarar los supuestos que debe conocer el investigador, haremos una exposición de los principios filosóficos sobre los que se fundamentan los principios éticos de la investigación, y luego, caracterizamos la investigación educativa con especial atención en su aplicación para la investigación con niños y adolescentes. Será necesario tener en cuenta el papel de las políticas en públicas para asegurar lo que consideramos el fin específico de esta investigación, a saber, que el investigador, en tanto que responsable del mejoramiento de la vida de las personas, asegure el bienestar de los sujetos de la investigación científica.

Transcatheter valve therapy provides a less invasive alternative for tricuspid regurgitation associated with right heart failure. We have evaluated the feasibility of transcatheter tricuspid valve implantation of the NaviGate valved stent in a long-term swine model. The valved stent was successfully implanted through transjugular and transatrial approaches on the beating heart with excellent hemodynamic and valve performance. No conduction disturbance or coronary obstruction was observed. This technology could provide an alternative treatment for patients who are at high surgical risk with severe tricuspid regurgitation and compromised right ventricular function.

Transcatheter aortic valve replacement remains challenge in patients with ball-cage-type mechanical valve in mitral position. Potential under-expansion of the percutaneous valve and interaction between the mitral ball-cage mechanical valve tilted towards the left ventricular outflow tract and the percutaneous valve adds risk during and after implantation. We report a successful implantation of the novel CoreValve Evolut-R self-expanding in a patient with severe aortic stenosis and a mitral Starr-Edwards mechanical valve implanted 28years ago.

AIMS The aim of this study was to test the feasibility of transcatheter mitral valve implantation of the NaviGate device in acute and chronic preclinical models. METHODS AND RESULTS We evaluated NaviGate valved stent implantation in the mitral position in an acute swine model (n=24, ≤5 days) through three different approaches - transatrial, transapical, and transseptal - and in a chronic swine model (n=12, >10 days) through a transatrial approach. ObjectivesThis study was designed to assess whether the reinforcement of the antioxidant system, through n-3 fatty acids plus antioxidant vitamin supplementation, could reduce the incidence of post-operative atrial fibrillation.BackgroundTherapy to prevent post-operative atrial fibrillation remains suboptimal. Although oxidative stress plays a key role in the pathogenesis of this arrhythmia, antioxidant reinforcement has produced controversial results.MethodsA total of 203 patients scheduled for on-pump cardiac surgery were randomized to placebo or supplementation with n-3 polyunsaturated fatty acids (2 g/day) (eicosapentaenoic acid:docosahexaenoic acid ratio 1:2), vitamin C (1 g/day), and vitamin E (400 IU/day). The primary outcome was the occurrence of post-operative atrial fibrillation. Secondary outcomes were the biomarkers related to oxidative stress and inflammation.ResultsPost-operative atrial fibrillation occurred in 10 of 103 patients (9.7%) in the supplemented group versus 32 of 100 patients (32%) in the placebo group (p < 0.001). Early after surgery, placebo patients presented with increased levels of biomarkers of inflammation and oxidative stress, which were markedly attenuated by antioxidant supplementation. The activity of catalase, superoxide dismutase, and glutathione peroxidase in atrial tissue of the supplemented patients was 24.0%, 17.1%, and 19.7% higher than the respective placebo values (p < 0.05). The atrial tissue of patients who developed atrial fibrillation showed NADPH oxidase p47-phox subunit protein and mRNA expression 38.4% and 35.7% higher, respectively, than patients in sinus rhythm (p < 0.05).ConclusionsThis safe, well-tolerated, and low-cost regimen, consisting of n-3 polyunsaturated fatty acids plus vitamins C and E supplementation, favorably affected post-operative atrial fibrillation, increased antioxidant potential, and attenuated oxidative stress and inflammation. En el presente estudio, los autores desarrollan un análisis riguroso de la evolución de la Industria Chilena de Fondos Mutuos. Se aplican para ello las medidas tradicionales de performance, correspondientes a los autores Sharpe, Treynor y Jensen, poniendo para ello de manifiesto, que su funcionamiento correcto depende de una serie de circunstancias, que no siempre se cumplen en los mercados financieros. En estos casos se aplican medidas alternativas de medición de la performance, las cuales eliminan las incoherencias detectadas. Estas medidas fueron desarrolladas por los profesores Ferruz y Sarto (1997). El presente estudio comprende el periodo 2000- 2005. Se analizan rentabilidades mensuales correspondientes a 196 fondos mutuos de renta variable. Los índices alternativos no hicieron más que ratificar el resultado de los índices originales. Se concluye además que las gestoras con mayor patrimonio administrado no poseen los mejores índices de performance, similar situación ocurre con aquellas gestoras pertenecientes a la industria bancaria.

[ES] Las paradas de transporte público (TP), tanto en su distribución como en su localización, son generalmente analizadas desde un punto de vista macroscópico, utilizando modelos analíticos para observar tanto una línea de buses, en particular, o la red de transporte público, en general. Sin embargo, la importancia y complejidad de lograr una buena operación de las paradas de transporte público y en particular lograr que éstas tengan una buena accesibilidad, requiere de un análisis más detallado. Actualmente no existen desarrollos metodológicos que solucionen este problema, menos aún en el caso de proyectos viales de mejora de la infraestructura urbana que implican una intervención importante de un sector de la ciudad, por lo que en este trabajo se propone una metodología de localización de paradas de transporte público que a través de un modelo matemático de optimización permita ubicar dichos paraderos provisorios a lo largo de las calles propuestas en un Plan de Desvíos de manera tal de optimizar la accesibilidad de los usuarios al transporte público. Esta metodología fue aplicada a un caso real de estudio en la intersección de las avenidas 5 de Abril – Esquina Blanca con Avenida Del Ferrocarril de Santiago, Chile, y los resultados muestran que se puede mantener y, en algunos casos, mejorar el nivel de servicio del transporte público desde el punto de vista de la accesibilidad al mismo. Por lo tanto, se demuestra la importancia y el aporte que esta metodología puede tener para mejorar la planificación de los Planes de Desvío por obras de mejoramiento vial al incorporar la variable accesibilidad al transporte público., Esta investigación fue apoyada y financiada a través del Proyecto N° 061312MT del Departamento de Investigaciones Científicas y Tecnológicas (DICYT) de la Vicerrectoría de Investigación, Desarrollo e Innovación de la Universidad de Santiago de Chile (USACH).

ResumenActualmente la estenosis aórtica (EA) degenerativa es la valvulopatía adquirida más frecuente en la población mayor, y debido al envejecimiento de ésta su número absoluto continua incrementándose. La obstrucción mecánica a la salida de sangre a través de la válvula estenótica determina una serie de cambios morfo-funcionales del corazón que luego de un período de latencia relativamente largo son sobrepasados apareciendo los síntomas. Una vez que un paciente se hace sintomático, se establece un punto de quiebre tanto en su calidad de vida como en su pronóstico. Tradicionalmente estos pacientes son sometidos a cirugía de reemplazo valvular aórtico, considerando que esta terapia mejora el pronóstico y la calidad de vida. Sin embargo, dado que la población portadora de EA es una población mayor con múltiples co-morbilidades, hasta un 30-60% se considera fuera del alcance quirúrgico. Recientemente se desarrollaron dos dispositivos valvulares biológicos para la corrección de la EA, a través de la implantación valvular aórtica transcatéter (TAVI), procedimiento menos invasivo que la cirugía. Se revisaron los antecedentes que apoyan las bases fisiopatologicas, dispositivos, complicaciones y proyecciones de esta novedosa técnica en pacientes mayores, sintomáticos e inoperables. Este procedimiento no solo constituye un gran adelanto técnico y conceptual, sino que ha permitido abordar pacientes antes considerados inoperables. Los resultados han permitido un crecimiento de las TAVI a nivel mundial de aproximadamente de 10.000 válvulas implantadas el 2010 a más de 90.000 hoy. Un crecimiento como éste en la población de más alto riesgo solo demuestra su eficiencia y seguridad. Probablemente estemos asistiendo a un cambio conceptual en el enfrentamiento de esta patología.SummaryCurrently degenerative aortic stenosis is the most common acquired valvular heart disease in the elderly population, and because the aging of this, the absolute patient number continues to increase. Mechanical obstruction to the outflow of blood through the stenotic valve causes a series of morpho-functional changes of the heart after a relatively long latency period are outweighed symptoms appear. Once a patient becomes symptomatic establishing a turning point in terms of quality of life and in its forecast Traditionally, these patients undergo aortic valve replacement surgery, considering that this therapy improves prognosis and improved quality of life. However, given that patient with aortic stenosis is an aging population with multiple co-morbidities, up to 30-60% of them does not undergo surgery. Recently, two heart biological valve devices have been develope for the correction of aortic stenosis through transcatheter aortic valve implantation (TAVI), less invasive procedure than surgery. We reviewed the backgrounds that support the pathophysiological bases, devices, complications and projections of this novel technique in older patients, symptomatic and inoperable. This procedure is not only a technical and conceptual breakthrough, but allowed to treat patients previously considered inoperable. The results have enabled a huge growth in worldwide TAVI approximately 10,000 valves implanted in 2010 to over 90,000 today. Growth like this in the highest risk population only proves its efficiency and safety. We are probably witnessing a conceptual change in dealing with this disease.

Ischaemia reperfusion injury is a pathophysiological event that occurs after cardiac surgery with extracorporeal circulation. This clinical event has been associated with the induction of oxidative and inflammatory damage in atrial tissue. Here, we tested whether combined omega 3 polyunsaturated fatty acids (n-3 PUFA)-antioxidant vitamin protocol therapy reduces oxidative and inflammatory cardiac tissue damage. This trial assigned 95 either-sex patients to supplementation with n-3 PUFA (2 g/day), or matching placebo groups, 7 days before on-pump surgery. Antioxidant vitamins C (1 g/day) and E (400 IU/day) or placebo were added from 2 days before surgery until discharge. Blood and atrial tissue samples were obtained during the intervention. Reduced/oxidized glutathione (GSH/GSSG) ratio, malondialdehyde (MDA) and protein carbonylation were determined in atrial tissue. Leucocyte count and high-sensitivity C-reactive protein (hs-CRP) in blood plus nuclear factor (NF)-κappaB activation in atrial tissue served for inflammation assessment. Lipid peroxidation and protein carbonylation were 27.5 and 24% lower in supplemented patients (p < 0.01). GSH/GSSG ratio was 38.1% higher in supplemented patients compared with placebo (p < 0.01). Leucocyte count and serum hs-CRP levels were markedly lower throughout the protocol in supplemented patients (p < 0.01). Atrial tissue NF-κB DNA activation in supplemented patients was 22.5% lower than that in placebo patients (p < 0.05). The combined n-3 PUFA-antioxidant vitamin protocol therapy here proposed reduced the oxidative stress and inflammation biomarkers, in patients undergoing on-pump cardiac surgery.

Valve replacement surgery prolongs survival and is the technique of choice considering its historical background and long experience worldwide. Recently however, TAVI has positioned itself as the only standard therapy for symptomatic patients who are not candidates for surgery. Aim To report the experience with this new technique comparing our results with those reported in the literature. Material and methods Between May 2010 and December 2011,17 patients aged 81 ± 7.3 years (58.8% men with an Euro SCORE 29 ± 22.4%) underwent a TAVI. Results The median transvalvular gradient was 54 ± 15.7 mmHg. All patients received a CoreValve™. Technical success was 100%, with a post implant gradient of 6.29 ± 3.3 mmHg. Residual aortic regurgitation was observed in 94%, none greater than grade II. There were no complications at the vascular access site. One patient developed cardiac tamponade during the procedure. Permanent pacemaker implantation was required in 35.2%. Hospital mortality rate was 5.8%, a figure that remained unchanged at 30 days of follow-up. Conclusions In high-risk patients with aortic stenosis, TAVI has a high success rate and a low rate of complications. Besides an appropriate patient selection, a trained multidisciplinary team and technical conditions to solve possible complications of the procedure are required.

Although cardiac transplantation is the most effective treatment in patients with end-stage heart failure, its availability is limited. While waiting for transplantation, some patients become refractory to treatment and deteriorate progressively. Secondary multi-organ damage could highly compromise the transplant success and also could contraindicate it. Mechanical ventricular assist devices allow reestablishing normal cardiac output and they have been used as a bridge to recovery and transplantation. We report four patients that underwent mechanical ventricular support using the ABIOMED BVS 5000 system as a bridge for transplantation. Two patients were connected to biventricular assistance; a third patient was connected to a left ventricular support and the fourth to a right ventricular support. Three were successfully transplanted and one died of refractory non-cardiogenic shock. There were no complications related to the support system, such as infection, hemorrhage or stroke. In our experience, the ABIOMED BVS 5000 was an effective strategy as a bridge to heart transplant in patients in cardiogenic shock.

Surgery of the aortic arch is a very complex procedure since it requires protective strategies for the brain, heart and rest of the body.To communicate our experience in the first 23 total or partial replacements of aortic arch.Retrospective search in the database of the Cardiovascular Surgery Unit for patients subjected to partial or total replacement of the aortic arch since 1998.Between 1988 and 2002, 23 patients were operated. Seventeen had aortic dissection (10 acute and 7 chronic), five had an atherosclerotic aneurysm and one had a traumatic lesion. Thirteen patients were subjected to a replacement of the arch plus ascending aorta, six to a replacement of the arch plus descending aorta and four to a replacement of the arch, ascending and descending aorta. Seven patients had previous operation of the thoracic aorta. Arterial perfusion was done via the femoral artery, axillary artery or a combination of both. A hypothermic circulatory arrest was induced in 22; it was associated with cerebral retro perfusion alone in 8 patients, antegrade cerebral perfusion in 5; isolated or associated axillary perfusion was used in five patients. In seven, procedures on the aortic or mitral valve, or coronary artery operations were added. Operative mortality was 26%, 3 of the 8 patients operated as an emergency and 3 of 15 elective operations. There was no mortality among those without dissection and of 7 chronic dissections, one died. All patients were followed for an average of 45 months. Two patients required reinterventions on the aorta and one for colon cancer. There was one late death of unknown cause. Postoperative complications were agitation, bleeding and temporary vocal cord dysfunction.There is a learning curve, where more extensive operations, particularly those done as emergency or for dissections, had an increased operative risk.

Surgical valve repair is a good alternative for correction of incompetent bicuspid aortic valve.To report the early and late surgical, clinical and ecochardiographic results of surgical repair of incompetent bicuspid aortic valves.Retrospective review of medical records of 18 patients aged 19 to 61 years, with incompetent bicuspid aortic valve in whom a valve repair was performed. Four patients had infectious endocarditis and 17 were in functional class I or II. Follow up ranged from 3 to 113 months after surgery.A triangular resection of the prolapsing larger cusp, which included the middle raphe, was performed in 17 cases; in 13 of these, a complementary subcommisural annuloplasty was performed. In the remaining case, with a perforation of the non-coronary cusp, a pericardial patch was implanted; this procedure was also performed in 2 other cases. In 3 cases large vegetations were removed. Postoperative transesophageal echocardiography showed no regurgitation in 11 patients (62%) and mild regurgitation in 7 (38%). There was no operative morbidity or mortality. There were no deaths during the follow-up period. In 3 patients (17%) the aortic valve was replaced with a mechanical prosthesis, 8 to 108 months after the first operation. Reoperation was not needed in 93%+/-6,4% at 1 year and 85%+/-9,5% at 5 years, these patients were all in functional class I at the end of the follow-up period. 60% had no aortic regurgitation, 20% had mild and 20% moderate aortic regurgitation on echocardiographic examination. A significant reduction of the diastolic diameter of the left ventricle was observed, but there were no significant changes in systolic diameter or shortening fraction.Surgical repair of incompetent bicuspid aortic valves has low operative morbidity and mortality and has a low risk of reoperation.

Pulmonary hypertension due to chronic pulmonary thromboembolismis frequently underdiagnosed and has a very poor prognosis if untreated. When the presence of centralpulmonary artery thrombus is confirmed, thromboendarterectomy is the treatment of choice, with verygood results. We report a 28 years old male with two previous episodes of deep venous thrombosis(DVT) who was admitted due to 8 months of progressive shortness of breath and a syncope. Heunderwent a CT pulmonary angiogram and an echocardiogram. Severe pulmonary hypertensionwas confirmed, secondary to a chronic pulmonary thromboembolism with an overlapped acutecomponent. He received systemic thrombolysis with partial thrombus disappearance. Therefore apulmonary thromboendarterectomy was performed and an inferior vena cava filter was placed.The patient was discharged with marked improvement in his functional capacity (Rev Med Chile2005; 133: 565-9).(

Background: Mortality of traumatic aortic lesions is over 80%. A group of those who survive, develop a chronic pseudo aneurism, usually asymptomatic, that is detected during imaging studies. Since conventional surgical treatment of traumatic aortic lesions has a great mortality, endovascular treatment has been used as an alternative treatment in the last decade. Aim: To report our experience with endovascular treatment of traumatic aortic lesions. Patients and methods: Report of seven patients aged 22 to 65 years, with traumatic aortic lesions. Under general anesthesia an endovascular prosthesis was inserted through the femoral artery. Results: No complications were observed in the postoperative period, and after a follow up ranging from 4 to 40 months, no endoleaks or other complications have been detected. Conclusions: Endovascular treatment of traumatic aortic lesions has good immediate and midterm results
