Abstract

Introdução: A estimativa da prevalência de fibrilação atrial (FA) sem diagnóstico ou tratamento é crucial para a prevenção do acidente vascular cerebral isquêmico (AVCI) relacionado à FA. Objetivo: Avaliar a prevalência de FA sem diagnóstico ou tratamento na Atenção Primária à Saúde. Métodos: Realizamos um estudo prospectivo transversal não randomizado em Joinville, Brasil, entre indivíduos idosos em Unidades Básicas de Saúde (UBS). Resultados: Foram incluídos mil indivíduos, com idade média de 69 ± 7 anos, 57% dos quais eram mulheres. A prevalência de FA foi de 3%, com 50% dos pacientes com FA não-diagnosticada e 33% dos pacientes diagnosticados previamente sem receber anticoagulação oral apesar de indicação clínica. Usando prevalências padronizadas por idade, estimamos 68 e 27.342 AVCI por ano devido à FA sem diagnóstico ou tratamento em Joinville e no Brasil, respectivamente. Conclusão: A observação de que dois em cada três pacientes com FA estavam sem diagnóstico ou tratamento é muito alarmante. Estratégias de rastreamento para alcançar esta população são urgentemente necessárias.

Abstract

Introduction: Atrial Fibrillation (AF) is not only one of the main causes of stroke, but it is also the most common sustained cardiac arrhythmia. As a result of population aging, an increase in the prevalence of AF and other associated chronic cardiovascular diseases, such as Systemic Arterial Hypertension (SAH) and Diabetes Mellitus (DM), is expected, contributing to a higher incidence of stroke. The magnitude of this fact reflects the reason why stroke is considered the second cause of death in the world, and the first cause of death in Brazil, in addition to being the second cause of cognitive losses. The main causes that favor the occurrence of ischemic stroke determine the situation of risk groups. Such factors can be classified as modifiable, non-modifiable and potential risk group. Objective: The aim of this study was to systematically review the public health literature on the relationship between ischemic stroke and atrial Fibrillation. Methods: Systematized literature review performed by searching the Web of Science database (Clarivates), Science Direct (Scopus), Wiley Online Library (John Wiley and Sons), Taylor and Francis (Taylor & Francis Group) and PubMed (NIH), using the descriptors Relationship between Ischemic Stroke and Atrial Fibrillation and other cardiovascular diseases in primary care, both in English and Portuguese. A total of 3860 articles were found, of which 64 were selected for the review by six evaluators independently. The articles surveyed are from the period 2015 to 2022. Results: In addition to the epidemiological magnitude, AF is important for its clinical consequences, including thromboembolic occurrences, with an increase, on average, of 4 times the probability of a stroke, in addition to being correlated with the high risk of all-cause mortality and other circumstantial determinants, such as heart failure. Conclusion: The analyzed studies demonstrated that there may be an adjusted incidence of ischemic stroke associated with AF and other cardiovascular diseases by stroke age, being age dependent in men. Social factors may contribute to the late discovery of the association between stroke, AF and other cardiovascular diseases.

Abstract

Covid-19 is an infectious disease that causes inflammation in the respiratory system, is contagious and spreads rapidly. The sequelae caused after infection and treatment of Covid-19 are not fully known. The study aims to evaluate the quality of life and respiratory capacity of individuals who have been contaminated by the Covid-19 virus. This study is characterized as descriptive, with a cross-sectional design and a quantitative-qualitative approach. The sample consisted of 54 participants, of both sexes, diagnosed with Covid-19, who were hospitalized or not. Data collection was carried out through a mixed online questionnaire, developed on the Google Docs® platform. The results pointed to a worse quality of life in the domains Limitations due to physical aspects, Pain, Vitality, Limitations due to emotional aspects and a better quality of life for the domains Functional Capacity, General State of Health and Mental Health. And a worse quality of life assessed from the respiratory disease. It is concluded that the quality of life was affected and impaired after being affected by Covid-19 and that Physiotherapy helps to improve the sequelae and resumption of day-to-day activities, generating a good recovery.

9. Prevention of Melasma in Pregnant Women: Aspects of Approach in Aesthetic Treatment

Abstract

In the gestational period, several changes can occur in the woman's body, almost all the body's systems can undergo modifications, among them is the skin. One of the main changes that occur in the skin is melasma or pregnancy stain, which is the most common in the pigmentary changes of pregnancy. Hyperpigmentations are, in general, disorders distinguished by an increase in melanin and other pigments in the skin. The main triggers for the emergence of melasma are: sex hormones solar radiation and external agents, sources of free radicals. Melasma is characterized by irregularly shaped, brownish melanic pigmentation that appears mainly on the face, and may appear in the 1st or 2nd trimester of pregnancy, especially in women who are of childbearing age and also with phototypes IV and V. During pregnancy, the use of photoprotection correctly is essential for skin protection and to prevent the appearance of melasma, it is always recommended to use sunscreen daily, being applied 30 minutes before sun exposure and reapplying whenever necessary, avoiding the maximum sun exposure between the hours of 10 am to 4 pm. After pregnancy, there may be a large increase in hyperpigmentation, so some professionals indicate treatment after weaning, with some whitening substances such as glycolic acid, azelaic acid, and kojic acid, and can be associated with combined therapy, that is, in some cases, associate diamond pelling, which is a device with a sandpaper at the tip where it produces a sanding in the epidermis. , thus generating cell renewal. It is extremely important that pregnant women receive, at the beginning of pregnancy, all the information on prevention of melasma, and care with adequate photoprotection, thus avoiding psychological suffering and changes in their self-esteem.

Abstract

Obesity is a chronic disease described by the large amount of body fat that leads to an increase in Body Mass Index (BMI). It is subdivided into three levels, being grade I with a BMI between 30 and 34.9 kg/m2, grade II between 35 and 39.9 kg/m2 and grade III or morbid obesity with a BMI above 40 kg/m2. Aiming at reversing the morbid condition caused by the disease, the bariatric surgical technique presents itself as the most effective option to contain grade III obesity, as well as the associated co morbidities. The objective of surgical techniques is to improve the quality of life of the obese, reducing both physical and psychosocial problems. As endocrine disease is related to several co morbidities, such as high blood pressure, sleep apnea and even some types of cancer, it is necessary to carry out an investigation into the surgical technique to be applied as well as its effectiveness during and after the surgical procedure.

Abstract

There are several factors that can cause us to have any disease, with those of the cardiorespiratory system is no different. They can arise by genetic inheritance, viroses and bacteria. Prevention in such cases is essential. Directly with the speech, the cardiorespiratory system of the teachers must be worked, be it of rehabilitative or preventive form. The objective of this work was to preempt the activities with respiratory exercises to strengthen and stretch the muscles responsible for breathing. Materials and methods: stretches, exercises, Respiron® and explanatory folders of how to perform the breathing exercises were used in a closed room with a maximum of 5 teachers at a time, so that there was relaxation. Results: The entire group of teachers performed the exercises, and they reported that there was a significant decrease in stress in the classroom, and improved sleep. Conclusion: it was concluded that with the exercises and stretches, the improvement occurred in the cardiorespiratory system, in the quality of life, as well as in the classroom.

Abstract

Diabetes Mellitus is a metabolic disease caused by an excess of glucose in the blood (hyperglycemia), due to the inability of insulin to perform its functions. It is, innumerable times, related to obesity and sedentary lifestyle. Diabetes is a complex metabolic disease that can have devastating effects on multiple organ systems. With this, the objective of the present study was to carry out a systematic review about the effectiveness of the practice of physical activity in the control of glycemia in individuals with Diabetes Mellitus. For this review, searches were performed in the following databases: PubMed, Scielo, LILACS and MEDLINE. The following descriptors were used in Portuguese and Spanish: diabetes mellitus, exercise, type 1 diabetes mellitus and type 2 diabetes mellitus. A total of 252 articles were found with the descriptors mentioned, of which, after the selection process, 13 articles remained for full reading, of which 7 met all the inclusion criteria. Data collection was restricted to clinical research carried out with human beings of both genders in the last 5 years (2015 to 2020). The literature points out as the main form of managing diabetes mellitus, aerobic physical exercise of moderate intensity, which causes positive changes in glycemic control and cardiovascular response.

Catechol-O-methyltransferase (COMT) is an important enzyme participating in inactivation of carcinogenic oestrogen metabolites. In humans there is a single nucleotide polymorphism in COMT gene (COMT va1158met) that has been associated with an increased risk for developing breast cancer. In dogs, there is a single nucleotide polymorphism in COMT gene (G482A), but its relation with mammary carcinogenesis has never been investigated. The aim of this study was to focus on the evaluation of such polymorphism as a risk factor for the development of mammary tumors in bitches and on the analysis of its relationship with some clinicopathologic features (dog's age and weight, number and histologic type of the lesions, lymph node metastasis) of canine mammary neoplasms. A case-control study was conducted analyzing 90 bitches with mammary tumors and 84 bitches without evidence of neoplastic disease. The COMT G482A polymorphism was analyzed by PCR-RFLP. We found a protective effect of the polymorphism in age of onset of mammary tumors, although we could not establish a significant association between COMT genotype and other clinicopathologic parameters nor with mammary tumor risk overall. Animals carrying the variant allele have a threefold likelihood of developing mammary tumors after 9 years of age in comparison with noncarriers. The Kaplan-Meier method revealed significant differences in the waiting time for onset of malignant disease for A allele carrier (12.46 years) and noncarrier (11.13 years) animals. This investigation constitutes the first case-control study designed to assess the relationship between polymorphic genes and mammary tumor risk in dogs. Our results point to the combined effect of COMT genotype with other genetic and/or environmental risk factors as important key factors for mammary tumor etiopathogenesis.

Abstract

Cyclin D1 (CCND1) is a key regulatory protein of the cell cycle. The purpose of our study was to assess the role of CCND1 genetic variants influencing the genetic susceptibility of non-small cell lung cancer (NSCLC). We conducted a study of 1234 individuals, including 892 controls and 342 cases. Individuals carrying two G-alleles have a 2-fold increased risk for the development of NSCLC and the waiting time for onset of NSCLC in these patients was 2 years earlier in comparison with other individuals. Our results may be important in contributing to the knowledge of the mechanisms involved in lung carcinogenesis.

Abstract

The relationship between E-cadherin epithelial expression, as detected by immunohistochemical methods, and other clinico-pathological characteristics of canine malignant mammary tumours was studied in 77 tumours surgically removed from 45 female dogs. The immunohistochemical assessment was based on the estimated percentage of epithelial cells with membranous labelling. Reduction of E-cadherin expression was significantly related to size and ulceration of tumours but not to fixation to skin or underlying tissue; it was also related to lymph node metastasis, necrosis and infiltrative growth. Histological type (but not histological grade) was related to E-cadherin expression, with solid tumours more frequently lacking expression and tubulopapillary tumours showing increased expression as compared with the other types. The significant relationship between E-cadherin and other known factors of poor prognosis suggests that the loss of E-cadherin expression may have prognostic value in canine malignant mammary tumours.
