Verruciform xanthoma (VX) is a rare benign lesion of unknown etiology, with a rough or papillary aspect, painless, sessile, well-defined, most lesions do not exceed 2 cm in their largest diameter, the degree of keratinization of the surface influences color, varying white to red, affecting mainly the gingiva and alveolar mucosa, and can also be seen in skin and genital. Herein, we present a report a clinical case of oral verruciform xanthoma in the buccal mucosa associated with the lichen planus lesion, as well as the morphological and immunohistochemical characteristics of the lesion. The clinical diagnostic hypothesis of oral lichen planus of the white reticular lesions on the buccal mucosa and on the tongue was confirmed by histopathology before a subepithelial connective tissue exhibiting intense inflammatory infiltrate in a predominantly lymphocytic band. In contrast, the hypothesis of the verrucous lesion in the left buccal mucosa was leukoplakia, with histopathological evidence showing exophytic and digitiform proliferations with parakeratin plugs between the papillary projections. Subepithelial connective tissue was characterized by macrophages with foamy cytoplasm (xanthoma cells). An immunohistochemical examination was performed, showing positivity for CD68, a macrophage marker, in addition to testing by Schiff's periodic acid (PAS) with diastasis, which was detected the presence of lipids inside these macrophages. The patient is free of recurrences of verruciform xanthoma and is being monitored due to the presence of lesions of oral lichen planus.

Introduction Several treatments have been used to reduce inflammation and to reverse epithelial alterations in actinic cheilitis (AC). Aim A systematic review was conducted to analyze the potential of topical treatments for remission and clinical improvement of AC as well as patient acceptability. Methods A systematic review of clinical trials was conducted following the PICO strategy to answer the following question: Are topical anti-inflammatory and antineoplastic agents effective in the treatment of actinic cheilitis? The quality of the studies was assessed by ROB-2 and the certainty of evidence was rated by GRADE guidelines. Results Eight clinical trials were selected, including four that investigated the use of anti-inflammatory drugs and four use of antineoplastic agents. The use of 3% diclofenac sodium was associated with partial remission of AC, while 5% imiquimod and ingenol mebutate promoted complete remission. Furthermore, 5% fluorouracil was the drug most associated with complications during treatment. Diclofenac sodium (3%) and fludroxycortide showed the best acceptance by the patients, especially in terms of symptom relief and comfort provided. Conclusion The anti-inflammatory and antineoplastic agents analyzed largely provided good clinical outcomes, with evidence of remission of AC lesions, development of few local adverse reactions during treatment, and good patient adherence.

Oropharyngeal cancer is characterized by high morbidity and mortality. Prognostic factors for this cancer are therefore useful to predict overall survival and may provide additional therapeutic targets.To evaluate the 5-year overall survival and prognostic factors for oropharyngeal squamous cell carcinoma.Retrospective cohort (2008-2018) of a cancer referral center. The population of the study was a hospital-based cohort consisting of patients diagnosed with oropharyngeal cancer who underwent surgery and/or adjuvant therapy (radio- and/or chemotherapy).A total of 253 patients with oropharyngeal squamous cell carcinoma were analyzed. The mean age was 59.8 ± 11.9 years and there was a male predominance (81.8%). Smoking and alcohol consumption were found in 88.0% and 84.2% of the sample, respectively. The combination of radiotherapy and chemotherapy was the treatment modality in 42.7% of the sample, followed by surgery combined with radio- and chemotherapy in 15.8%. There were 143 deaths (events), the mean survival was 11.55 ± 9.69 months, and the 5-year overall survival rate was 1.1%. Overall survival was lower for clinical stage III/IV (p 0.001), HPV p16-negative status (p = 0.019), and an interval 4 weeks between diagnosis and the beginning of treatment (p 0.007).Among the prognostic factors analyzed in this cohort, p16-negative status as a poor prognostic indicator and tumor stage III/IV and an interval longer than 4 weeks between diagnosis and the beginning of treatment were significantly associated with lower overall survival.

Salivary gland cystadenoma is a rare benign neoplasm, representing 1.4% of all benign salivary gland tumors. It presents a predilection for the major salivary gland and middle-aged or elderly adults. The present study reports a rare case of cystadenoma in the floor of the mouth a 5-year-old male patient, who was referred to an oral diagnosis center for evaluation of the lesion. After performing an incisional biopsy, microscopic analysis revealed a cystadenoma. The patient’s treatment was excision of the lesion and he has been under follow-up for one year with no signs of recurrence.

Background The objective of this study was to describe the upper and lower lip lesion occurrence in an oral diagnostic service. Material and Methods Retrospective descriptive sectional study was performed. Clinical records were obtained from the archives of an Oral Diagnostic Service referral center between 2006 and 2016. Data such as gender, age, anatomical location, and diagnosis were collected and categorized. The collected data were submitted to a descriptive analysis and Pearson's chi-square test (p ≤ 0.05). Results A total of 587 patient records of lip lesions were analyzed. Most lesions were diagnosed in female (52.1%) and adults (56.9%) patients in the lower lip (76.2%). Among all lip lesions, the reactive/inflammatory lesions (n = 238; 40.5%) and oral potentially malignant disorders (n = 164; 28%) were the most frequent group lesions. Mucocele (n = 147; 25%), actinic cheilitis (n = 136; 23.1%) and vascular lesions (n = 51; 8.7%) were the most frequent lesion in the sample. Actinic cheilitis was significant in relation to gender (p < 0.001), all three most frequent lesions were significant in concerning to age group and anatomical site. Conclusions Mucocele was the most common lower lip lesion in all age groups, followed by actinic cheilitis and vascular lesions, which mainly affected adults and the elderly. O Cirurgião Dentista deve estar capacitado a conhecer os riscos relacionados a utilização dos bisfosfonatos. Este estudo tem como objetivo avaliar o conhecimento e condutas dos Cirurgiões Dentistas acerca da utilização dos bisfosfonatos e seus efeitos adversos. A amostra foi constituída por 60 Cirurgiões Dentista especialistas nas áreas de Cirurgia (n=20), Implantodontia (n=20) e Periodontia (n=20), que responderam a um questionário estruturado e validado por Albuquerque (2014) com 12 perguntas, possuindo questões pessoais, tópicos sobre a anamnese realizada por estes profissionais e sobre o conhecimento sobre os bisfosfonatos, incluindo indicações, efeitos adversos e tratamentos empregados quando necessário. Dos 60 participantes, 88,3% já atenderam pacientes que relatam o uso de Bisfosfonatos, porém apenas 33,3% conhecem as complicações orais do seu uso. Foi concluído que houve um número considerável de Cirurgiões Dentistas cientes da existência dos bisfosfonatos, uma pequena parcela sabe sobre as complicações orais desse medicamente e sua forma de tratamento.

O presente estudo objetivou analisar o impacto da pandemia de coronavírus Sars-CoV-2 nos atendimentos odontológicos à população infantojuvenil na cidade de João Pessoa-PB. Trata-se de um estudo temporal realizado de setembro de 2019 à 2020. Foram utilizadas informações secundárias públicas aos serviços de atenção básica e secundária, caracterizadas pelos indicadores de saúde bucal os quais foram obtidos pelo SIA/SUS, banco de dados do Ministério da Saúde (DATASUS). Nesse estudo foram considerados os atendimentos odontológicos feitos no SUS, no semestre que antecedeu a pandemia de COVID-19. Nesse período, 1.413 procedimentos foram realizados em crianças e adolescentes na grande João Pessoa. Em média são realizados 235,5 ± 85,3 procedimentos mensais. No primeiro semestre da pandemia do novo coronavírus, o número de procedimentos realizados apresentou uma queda de 46,42%. Foram realizados 757 procedimentos odontológicos e no mês de maio, nenhum procedimento no público infantojuvenil foi referenciado. Dessa forma, nós podemos concluir que houve uma redução significativa no número de atendimentos odontológicos, e esse fenômeno pode gerar uma demanda reprimida significativa nesse público. Com isso nós sugerimos estudos para desenvolver políticas de prevenção e promoção de saúde bucal quando os atendimentos coletivos forem normalizados.

A temática relacionada aos cuidados paliativos (CP) deve ser inserida na grade curricular do ensino de graduação, favorecendo desta forma uma promoção dos conhecimentos reais praticados no atual sistema de saúde. Este estudo teve como objetivo avaliar as abordagens pedagógicas relacionadas aos CP utilizados pelos docentes do curso de fisioterapia de uma Instituição de ensino superior. Trata-se de uma pesquisa descritiva, exploratória e quantitativa/qualitativa realizada por meio de um questionário composto de perguntas abertas e fechadas com 13 docentes da instituição. Diante dos resultados, foi observado que a temática de CP não está contemplada na ementa e/ou nos objetivos de muitas disciplinas do curso, no entanto, os docentes inserem esse tema utilizando algumas abordagens metodológicas nas aulas, como aulas expositivas, estudos de casos e visitas técnicas. Na odontologia os fármacos antibacterianos são indicados em infecções pré-estabelecidas, como também para a prevenção de bacteremias diante de procedimentos invasivos em paciente com a condição sistêmica que possibilite a disseminação dessas bactérias na corrente sanguínea. Portanto, o objetivo do presente estudo foi revisar na literatura cientifica, os conceitos atuais relacionados a prescrição de antibacterianos na odontologia. É de suma importância criar medidas para realizar de forma correta a prescrição antibacteriana, a fim de evitar uma prescrição inadequada desses fármacos. O uso indevido desses medicamentos ocasiona prejuízos para saúde sistêmica do paciente, contribuindo para a resistência de bacteriana. O uso racional desses medicamentos, engloba também o conhecimento do profissional da saúde sobre a substância prescrita, devendo o mesmo orientar corretamente o usuário em relação a posologia e os intervalos para o uso do fármaco. A formação do cirurgião-dentista sobre os conhecimentos teóricos e práticos da farmacologia devem ser recorrentemente atualizados, já que a prescrição engloba desde o uso das substâncias até as informações necessárias que devem estar contidas.

A odontologia estética vem despertando o interesse dos pacientes que desejam obter um sorriso harmônico. Uma das técnicas para alcançar a estética dental é o procedimento de clareamento dentário que pode ser associada ao uso de fontes de luz para potencialização do branqueamento. O objetivo do presente estudo é revisar na literatura as evidências científicas do efeito da utilização de fontes luminosas no clareamento dentário. Na busca de simplificar a técnica, diminuir o tempo operatório e acelerar os resultados do clareamento dentário, alguns profissionais passaram a fazer uso de fontes de luz ativadoras. Em relação à sensibilidade dental e a microdureza do substrato, observou-se na literatura que as duas técnicas contribuíram para o aumento da sensibilidade pós-operatória e da rugosidade superficial do esmalte. As evidências científicas apontam que o emprego da luz no processo de clareamento ocasiona processos reversíveis e irreversíveis ao elemento dental, sendo observado também em menor magnitude durante a técnica convencional. Desta forma, o benefício acelerador da fonte luz no clareamento dentário não supera os fatores adversos relacionados ao seu uso.

Juvenile ossifying fibroma (JOF) is an uncommon benign fibro-osseous lesion that affects young individuals and presents an aggressive clinical behavior and high rates of recurrence. This paper aims to report 7 cases diagnosed as JOF in a reference center, correlating them with clinical-pathologic aspects in the literature. We found 7 cases diagnosed as JOF with the age of patients ranging from 11 to 39 years. The mandible was the most common location, and in 3 cases pain symptoms were reported. Regarding the histopathologic findings, the majority of cases had a well-cellular pattern, and the trabecular subtype was the most commonly found. It was observed that in 3 cases that had relapsed, 2 had conservative treatment. In view of this, it is important to know the clinical-pathologic aspects of this lesion to perform a correct diagnosis and appropriate treatment to reduce the high recurrence rate of this lesion.

Objective To develop a Clinical Index for Actinic Cheilitis (CIAC) with the intention of assisting in the identification of clinical characteristics that indicate a greater severity of this condition. Study Design CIAC was developed through the opinion of experts, being applied in 20 patients with QA and indicated for biopsy. The scores obtained after application of the index were associated with the histopathologic grading for oral epithelial dysplasia from the World Health Organization (WHO) and the Binary System using Fisher exact test (P Results There was no statistically significant association between clinical characteristics and degree of dysplasia. The cut-off point with value 11 exhibited better performance for ROC curve and higher percentage for specificity, PPV, and accuracy in both histopathologic grading analyzed. Conclusions The cut-off point 11 can be used to indicate higher degree of severity of the AC, suggesting that out of this score patients are indicated for biopsy. The size of the sample and the need to verify the reproducibility of the index make it necessary to carry out further researches.

A 43-year-old female patient attended the oral maxillofacial and traumatology service presenting volume increase in the right hemiface with painful symptomatology. At the intraoral examination, an exophytic lesion involving the alveolar ridge and palate region was observed, extending to the right nostril. The lesion showed a partially ulcerated surface, with purulent secretion, firm consistency, and normal mucosal color. Computed tomography revealed a mixed appearance of hyperdensity and ill-defined hypodensity in the maxillary region with extension to the zygoma and the right orbit. Because of poor oral hygiene and a history of regression with antibiotic therapy, a clinical diagnosis of actinomycosis was issued, and incisional biopsy of the lesion was subsequently performed. In the microscopic analysis, proliferation of pleomorphic and atypical malignant mesenchymal cells was associated with deposition of osteoid matrix, thus confirming the histopathologic diagnosis of osteosarcoma. The patient was referred to the State Reference Center for appropriate treatment.
