Aims: In many countries, real‐time continuous glucose monitoring (rt‐CGM) is not funded, and cost presents a barrier to access. A do‐it‐yourself conversion of intermittently scanned CGM (DIY‐CGM) is a cheaper alternative. This qualitative study aimed to explore user experiences with DIY‐CGM in people aged 16 to 69 years with type 1 diabetes (T1D). Methods: Convenience sampling was used to recruit participants for semi‐structured virtual interviews exploring experiences of DIY‐CGM use. Participants were recruited after completing the intervention arm of a crossover randomised controlled trial that evaluated DIY‐CGM versus intermittently scanned CGM (isCGM). Participants were previously naive to DIY‐CGM and rt‐CGM but not isCGM. The DIY‐CGM intervention consisted of a Bluetooth bridge connected to isCGM, adding rt‐CGM functionality over 8 weeks. Interviews were transcribed, then thematic analysis was performed. Results: Interviews were with 12 people aged 16 to 65 years, with T1D: mean age ± SD 43 ± 14 years; baseline mean HbA1c ± SD 60 mmol/mol ± 9.9 (7.6 ± 0.9%) and time in range 59.8% ± 14.8%. Participants perceived that using DIY‐CGM improved both glycaemic control and aspects of quality of life. Alarm and trend functionality allowed participants to perceive reduced glycaemic variability overnight and following meals. The addition of a smartwatch increased discrete access to glucose information. There was a high degree of trust in DIY‐CGM. Challenges while using DIY‐CGM included signal loss during vigorous exercise, alarm fatigue and short battery life. Aim. To assess children's subsequent device usage and caregiver attitudes to do-it-yourself real-time continuous glucose monitoring (DIY-rtCGM) at least 3 months after completing a randomized controlled trial (RCT). Methods. A brief online questionnaire or telephone call was used to collect the subsequent device usage and caregivers' attitudes from a total of 55 families at least 3 months after their completion of an RCT investigating DIY-rtCGM adapted from their preexisting intermittently scanned glucose sensors plus education on using DIY-rtCGM system. To be eligible for the RCT, children had to be aged 2–13 years, have type 1 diabetes ≥6 months, and be rtCGM naïve. Data collected investigated current CGM use post-RCT and attitudes/user experiences to DIY-rtCGM in the months since RCT study support ended. Results. Overall, responses from 81.8% (45/55) of caregivers were received. Mean age of children was 9.0 ± 2.7 years, and 31 (68.9%) children used insulin pumps. After 3 months, 44.4% (20/45) of responding caregivers reported ongoing DIY-rtCGM use, and of these, only 13 used DIY-rtCGM as the primary glucose monitoring method 100% of time. Of the 25 (55.6%) families who ceased DIY-rtCGM, 40% (10/25) had transitioned to commercial rtCGM. More than half of families (60%, 12/20) who continued DIY-rtCGM use had a very or extremely positive attitude toward the technology and 75% (15/20) of these families planned to continue DIY-rtCGM use. However, signal loss and sensor inaccuracy remained the major reasons among all responders both for decreased DIY-rtCGM wear time and eventual cessation. Burden of use primarily related to technical errors that could not be solved, and alarms, both of which were reported to contribute to discontinuation. Conclusions. This study highlights that, among families voluntarily using DIY-rtCGM at least 3 months following support from a trial, more than half have ceased using DIY-rtCGM, with 40% of those discontinuing switching to commercial rtCGM. Aims: To investigate the experiences of parents caring for young children with type 1 diabetes type 1 diabetes using a do‐it‐yourself continuous glucose monitor (DIYrtCGM) in a supported setting. Methods: Exit interviews were conducted with parents from 11 families at the end of the MiaoMiao study: a randomised cross‐over trial focusing on parental fear of hypoglycaemia. Technical support was provided to participants while using DIYrtCGM during the trial. A convenience sampling approach was used to recruit parents. An in‐depth, semi‐structured interview approach was used. Thematic analysis was used to identify key themes and subthemes. Results: Parents identified that remote monitoring enabled proactive management and that overall alarms/glucose alerts were useful. Some parents reported reductions in anxiety, increased independence for their child, and improvements in the child–parent relationship. However, parents also reported regular signal loss with DIYrtCGM, along with complicated apps and challenges troubleshooting technical problems. Despite this, nine of the 11 families continued to use the system after the end of the trial. Conclusions: Do‐it‐yourself continuous glucose monitoring (CGM) was on balance beneficial for the parents interviewed. However, while access to CGM shifted the burden of care experienced by parents, burden did not significantly reduce for all parents, as the improved glycaemic control that they achieved was accompanied with the responsibility for continually monitoring their child's data. Supported use of do‐it‐yourself CGM may be an achievable, cost‐effective option for parents caring for children with type 1 diabetes in countries without funded access to CGM. [ABSTRACT FROM AUTHOR]

Background: Open‐source automated insulin delivery (AID) is a user‐driven treatment modality used by thousands globally. Healthcare professionals' (HCPs) ability to support users of this technology is limited by a lack of knowledge of these systems. Aims: To describe the challenges experienced by HCPs supporting participants' use of open‐source automated insulin delivery in the Community deRivEd AuTomatEd insulin delivery (CREATE) study. Methods: Data were collected prospectively from the study team's fortnightly meetings and Slack Workspace (Slack Technologies, Ltd. 2018) during the first 4 months of the trial. Key topics were identified from minutes of meetings. Slack conversations were categorised by topic, with the number of posts per conversation, number of sites per conversation and involvement of experts in open‐source AID being recorded. Results: In the first 4 months of the trial, there were 254 conversations in Slack with a mean of 5.2 (±4.25) posts per conversation. The most frequent learning challenge was insulin pump and cannula problems relating to the DANA‐iTM insulin pump, which totalled 24.0% of all conversations. Experts on open‐source AID use were involved in 83.3% of conversations. Conclusions: A significant proportion of challenges related to specific devices, rather than AID. Challenges relating to the functioning of open‐source AID were more likely to involve input from experts in open‐source AID. This is the first report of challenges experienced by a multidisciplinary team in a supported open‐source environment that may inform expectations in routine clinical care. [ABSTRACT FROM AUTHOR]

Dominant analyses of sporting subjectivities suggest the contemporary athletic subject embodies a win-at-all-costs instrumental rationality. Yet, as Carless and Douglas (2012) argue, athletes are able to find less problematic alternatives to this understanding of sport. In this article, I use Foucault's concept of "practices of the self' to undertake a sociological analysis of ethical subjectivities within Ultimate Frisbee. I focus specifically on ascetic, or self-controlling, practices of the self through which players create relationships between their self, Ultimate's moral code and others. I use this case study to argue that ethical subjectivities offer a productive perspective for sociology of sport. [ABSTRACT FROM AUTHOR]

In this article, I examine the relatively youthful sporting retirement of four athletes from popular Western team sports, namely soccer, ice hockey and rugby union. Drawing on studies of sporting transitions and retirement (e.g„ Denison, 1997; Douglas & Carless, 2009; Sparkes, 1998; Sparkes & Smith, 2002; Stambulova, Alfermann, Statler, & Côté, 2009; Wylleman, Alfermann, & Lavallee, 2004) and Foucault’s notions of games of truth and ethical self-creation, I argue that these athletes’ decisions to retire from sport were based on a refusal to accept the subject position proposed to them within their sports and, subsequently, these sporting transitions formed acts of ethical self-creation. My conclusions reveal an as yet under-explored interpretation of sporting retirement in which the process of retirement forms part of an attempt to recreate an ethical self. [ABSTRACT FROM AUTHOR]

Foucault's technologies of the self have been used by sociological scholars of sport for nearly two decades. Yet Markula's seminal articulation of a feminist Foucauldian ethics in 2003 stands as a watershed publication, insofar as the majority of publications following this article have framed much of their analyses in relation to this work. In this article, then, I review sociological studies of sport and exercise that draw on Foucauldian ethics from Markula's article onward, paying careful attention to how Foucault's ethics and Markula's Foucauldian feminism have been deployed. Although I interpret this body of work as productive and insightful, I offer a critical reading of the emphasis on explicit problematizations and, relatedly, develop a methodological critique of researchers' reliance on interviews as a prime form of research method. In this article I offer insights into tensions within the lifestyle sport, Ultimate Frisbee (Ultimate) through an examination of differing forms of dress. Like many other sporting subcultures, sartorial choices, both on and off pitch, are important markers of insider status. Ultimate dress I argue is ‘a mélange of whimsical, ironic and athletic styles’ [Crocket, Hamish. 2015a. “Confessions of the Disc: A Foucauldian Analysis of Ethics within Ultimate Frisbee.” InThe Psychology of Sub-Culture in Sport and Physical Activity: A Critical Approach, edited by Robert J. Schinke and Kerry R. McGannon, 184–195. Hove, UK: Routledge, 184], which contains significant potential for aesthetic exploration amongst participants. Subsequently, much of Ultimate's dress, particularly that worn at themed tournament parties, can be read as a carnivalesque celebration of a subculture that prioritizes fun at both social and competitive levels, via ‘a controlled decontrol of the emotions’ [Featherstone, Mike. 2007.Consumer Culture and Postmodernism. 2nd ed. Yet, Ultimate is far from being a homogenous subculture and close attention to varying forms of dress offers insight into debates over the meaning and direction of the sport. Subsequently, I will examine the role of dress within a central debate amongst Ultimate players; namely, whether Ultimate should strive to maintain values and practices which currently differentiate it from mainstream sports, or work to become more recognizably mainstream. I argue that despite the tendency of advocates on both sides of this debate to construct the issue in oppositional terms, the fluidity of dress, identity and meaning within Ultimate is suggestive of an ongoing hybridity which intermingles elements of subversive, carnivalesque aesthetics with the more predictable and uniform aesthetics of mainstream sport.
