Background: Community water fluoridation began in the 1945 as a public health measure to prevent and control caries and was implemented in Israel in 1981. Community water fluoridation reduced caries significantly, but in 2014, the Ministry of Health decided to stop Community water fluoridation in Israel. The aim of our study was to examine the effect of fluoridation cessation on the dental health of children aged 3-5, treated in "Assuta Tel Aviv" dental clinics, under general anesthesia or deep sedation., Methods: The computerized Maccabi-Dent database provided data for this retrospective study. Records from the years 2014-2019 including treatment codes for procedures relevant to the study, the number of stainless-steel crowns and restorations of all types were examined. Kruskal-Wallis test was performed to compare the results from before and after fluoridation cessation., Results: A statistically significant increase in the mean number of treatments in the years after fluoridation cessation (P < 0.05) was found. There was approximately a two-fold increase in the number of all treatments for all ages., Conclusion: The results of the study emphasize the advantages of water fluoridation and are further proof of the need to restore community water fluoridation in Israel., (© 2024. Introduction: Hall's technique preformed metal crown (HTPMC) has been used widely by pediatric dentists in developed countries as a new approach for managing decayed primary molars without local anesthesia, caries removal, and tooth preparation. Currently, inadequate information is available regarding the implementation of this technique (HTPMC) in Malaysia. This study is aimed at evaluating the implementation of HTPMC by Malaysia's pediatric dentists and identify the co-occurrence frequencies of the HTPMC implementation with the respondents' demographic profile., Materials and Methods: A cross-sectional questionnaire-based research was conducted among 65 pediatric dentists in Malaysia. Online questionnaires were distributed to the pediatric dentists employed at public hospitals (MOH) and universities in Malaysia., Result: It was found that over half of the respondents (65.6%) employed HTPMC. The analysis of the co-occurrence network frequency revealed that a high frequency of female pediatric dentists who were within the age group of 31-40 years old had fulfilled their postgraduation overseas and was employed in the university mainly applied HTPMC., Conclusion: The application of HTPMC among respondent pediatric dentists in Malaysia was high. The aim of this retrospective study was to evaluate implant survival, marginal bone loss and peri-implant complications in 326 short and ultra-short implants. Implants were placed in the maxillary and mandibular posterior regions of 140 patients with (PP) and without (NPP) a history of periodontal disease. Clinical and radiographic examinations were performed at 3-year recall appointments. The 8.0, 6.0 and 5.0 mm-length implants placed in PP and NPP were respectively 43.75% and 38.46%, 35.10% and 34.19%, 21.15% and 27.35%; 325 implants (one early failure) were rehabilitated with single crowns in 139 patients. Overall implant survival after 3 years of follow-up was 97.55%, 98.08% and 96.61% for PP and NPP ( p = 0.46). Crestal bone level variations were not statistically different among PP and NPP; 15.41% of implants presented signs of mucositis, 14.71% and 16.67% in PP and NPP ( p = 0.64). Setting the threshold for bone loss at 2 mm after 36 months, peri-implantitis prevalence was 2.2%, 1.96% and 2.63% in PP and NPP ( p = 0.7). Overall implant success was 82.39%, 83.33% and 80.7% for PP and NPP ( p = 0.55). Short-term outcomes suggest that short and ultra-short locking-taper implants can successfully be restored with single crowns in the posterior jaws both in PP and NPP.

Objective: To determine the frequency of complications in crowns cemented over a 5-year period in a tertiary care hospital and also to report the survival of these crowns., Methods: The retrospective cross-sectional study was conducted at the Aga Khan University Hospital, Karachi, from March 2017 to March 2018 and comprised porcelain fused-to-metal crowns placed on the anterior or posterior teeth that were fabricated at the institutional laboratory and were placed in institutional dental clinics in the preceding 5 years. Porcelain-fused to-metal The crowns were clinically and radio-graphically evaluated in a duration of two months for presence/absence of complications. Factors associated with the failure of the crowns, their survival and the impact of covariates, like gender, flossing and bruxism, on the survival time were determined. Data was analysed using SPSS 22., Results: There were 150 crowns related to 107 patients who had a mean age of 45.0±11.4 years. The most common complication observed was open proximal contacts 9(6%) followed by de-cementation 8(5.3%), porcelain chipping 9(6%) and abutment fracture 2(1.3%). The 5-year survival rate of the crowns was 127(84.7%). The mean follow-up time was 57.2±1.0 months. Overall, year-wise survival of PFM crowns for one year till five years was 147 (98%), 144 (96%), 135 (90%), 130 (86%) and 119 (79%) respectively., Conclusions: The porcelain-fused-to-metal crowns had a high 5-year survival rate. Open proximal contacts represented the most common complication.

Aim: To study patient- and tooth-specific characteristics of teeth indicated for root canal treatment, in the public dental service of the county of Västra Götaland, Sweden., Methodology: During a designated 8-week period, general dental practitioners working at 20 different public dental clinics consecutively registered indications for undertaking root canal treatment. The patients' subjective level of pain was also registered (visual analogue scale) at the very first appointment. The following information was retrieved from computerized dental records and radiographs: gender, age, number of remaining teeth, tooth group, previous restoration, number of restored surfaces, dental caries and tooth substance loss. Gender and age were compared using both descriptive and analytical statistics., Results: The material comprised 243 teeth in 243 patients: 128 (52.7%) women and 115 (47.3%) men, mean age 48.3 years. Molar teeth predominated (47.7%). Most of the teeth (83.5%) had previously been restored and exhibited significant loss of tooth substance, more than a third of the crown (71.3%). Dental caries was present in 127 teeth (62.9%). Dental trauma was implicated in only seven cases (2.9%). Initial treatment was frequently undertaken at an emergency visit, for relief of symptoms (64.9%). The most commonly registered indication was pulpal necrosis with apical periodontitis (38.1%), followed by pulpitis (37.7%). Retreatment of a root filled tooth was reported in 18 teeth (7.4%)., Conclusions: In the general Public Dental Service of Sweden, root canal treatment is most frequently undertaken in molars. Aim: To assess whether the timing of pulp disease after tooth restoration was associated with type of restorative dental material used, extent of the restoration or tooth type., Methodology: A comprehensive search and analysis of data using the Titanium Oral Health Management software program at The Oral Health Centre of Western Australia were performed to correlate procedural codes for teeth that had restorations placed and subsequently developed pulp disease requiring endodontic treatment or extraction from 1st January 2009 to 31st December 2013. Manual analysis of paper and/or electronic patient record cards was also performed. Data collected included restoration type, restored tooth surfaces, tooth type and the dates of restoration and subsequent endodontic intervention or extraction., Results: Of 330 teeth that met the inclusion criteria, 84 (26%) had composite resin restorations, 80 (24%) had amalgams, 119 (36%) had glass-ionomer cement (GICs), and 47 (14%) had crowns. The average time between restoration and further intervention was 330 days with a range from 3 to 1775 days (approximately 5 years). Teeth restored with crowns or five-surface restorations were significantly more likely to require earlier intervention than other restorations. Premolar and anterior teeth were also more likely to require earlier intervention., Conclusions: Teeth that developed pulp disease requiring further intervention that were restored with crowns and five-surface GIC developed the disease sooner than teeth that were restored with amalgam or composite. Aim: It is the aim of this paper to present data on the survival of restorations in canine teeth by analysis of the time to re-intervention on the restorations and time to extraction of the restored tooth, and to discuss the factors which may influence this., Methods: A data set was established, consisting of General Dental Services (GDS) patients, this being obtained from all records for adults (aged 18 or over at date of acceptance) in the GDS of England and Wales between 1990 and 2006. The data consist of items obtained from the payment claims submitted by GDS dentists to the Dental Practice Board (DPB) in Eastbourne, Sussex, UK. This study examined the recorded intervals between placing a restoration in a canine tooth and re-intervention on the tooth, and the time to extraction of the restored tooth., Results: Data for more than three million different patients and more than 25 million courses of treatment were included in the analysis. Included were all records for adults (aged 18 or over at date of acceptance). Overall, 1,232,052 restorations involving canine teeth were included in the analysis. With regard to time to re-intervention, 33% of restorations had survived at 15 years, and with regard to time to extraction of the restored tooth, cumulative survival was 78% at 15 years. Veneers and crowns performed best in terms of time to re-intervention, but crowns performed worst when time to extraction was analysed., Conclusions: Overall, crowns and porcelain veneers placed on canine teeth perform best to re-intervention after 15 years, but crowns perform worst when the time to extraction of the restored tooth is examined and veneers perform best, with a 20 percentage point difference in cumulative survival at fifteen years between crowns and the best performing restoration in the under-40 year age group, this effect being accentuated in the over-40 year age group. Other factors influencing restoration longevity in canine teeth include: patient age, dentist age and the patient's annual need for dental treatment.

Purpose: The purpose of this study was to evaluate the clinical success of a new primary zirconia molar crown compared with stainless steel crowns (SSCs). Methods: This randomized, controlled clinical trial was designed as a split-mouth study. Fifty three- to seven-year-old children were recruited to provide a total of 50 pairs of teeth requiring primary molar crowns, with each participant receiving a SSC and zirconia crown. Restorations were evaluated at six-month, 12-month, and 24-month recall appointments examining the following criteria: gingival health; estimate of extent the crown was high in occlusion; surface roughness; staining on crown surface; wear of opposing arch tooth; color match; anatomic form; marginal integrity; marginal discoloration; proximal contact area; secondary caries at crown margin; and parent/guardian satisfaction with crown appearance. Results: The 24-month follow-up included 39 patients (78 percent). Seventy crowns (70 percent) were evaluated; of the 36 zirconia crowns and 34 SSCs, there were no failures at the 24-month evaluation. The only significant difference in the parameters evaluated was in parental satisfaction with the zirconia crown preference (P<0.05). Conclusion: Current 24-month results indicate that zirconia primary molar crowns perform similarly to an established stainless steel crown for restoration of primary molar teeth.

Objectives: This retrospective longitudinal study aimed to assess the longevity of single unit crowns placed by several dentists and to investigate risk factors associated with crown failures., Methods: From patient files, longevity of 3404 full crown restorations placed in 1557 patients by 8 Dutch dentists between 1996 and 2011 were analyzed. Annual failure rates (AFRs) were calculated and variables associated with failure (success and survival of crowns) were assessed by multivariate Cox-regressions analysis with shared frailty for patients., Results: Most of crowns were PFM (63.8%) placed in molars (58.1%) and non endodontically treated teeth (65.4%). The observation time of restorations varied from 3 weeks to 11 years with a mean of 7 years, resulting in a mean AFR at 11 years of 2.1% and 0.7% for success and survival of crowns, respectively. Among dentists a relevant variation for type of interventions was observed with AFR varying between 1.2% and 3.5%. The most significant risk factor for failure of crowns was the presence of an endodontic treatment, resulting in Hazard ratios of 1.31 for success [95%CI 1.07-1.61] and 1.89 [95%CI 1.35-2.65] for survival of crowns. Tooth type, tooth position (jaw) and gender showed also a significant influence on success of crowns. For survival, increase in patients' age results in a higher risk for failure., Conclusions: Overall, crowns placed by a selected group of dentist showed a good to acceptable success and survival rates, mainly dependent from the practice. Objectives: The aim of this 5-year randomized controlled trial was to compare the longevity and clinical behavior of single posterior crowns made with pressable ceramic on zirconia and on metal frameworks, and if failures occur, to delineate the contributing factors., Methods: 72 patients, who needed the covering of at least a molar and/or premolar, were included in the study. All teeth were endodontically treated, with absence of periapical lesion or active periodontitis. Ninety single crowns were made with zirconia or metal framework and covered with pressable veneering ceramics. Two independent examiners assessed the survival of restorations at 6 months, 1-4 and 5 years after restoration placement including periapical radiographs, intraoral photographs, and USPHS modified criteria. The statistical analyses were performed with the Kaplan-Meier method., Results: One core fracture occurred in Zircad/Zirpress crowns and one metal ceramic crown was lost for root fracture. Chipping fracture of the veneering ceramic was detected in 2 metal-ceramic crowns and in 3 zirconia-based crowns. The Estimate Cumulative Survival (ECS) and the Estimate Cumulative Success (ECSs) with standard deviation (SE) were respectively 97,73±2,19 and 92,64±4,14 for zirconia-based crowns whereas 97,44±2,39 and 91,11±4,27 for porcelain fused to metal crowns., Conclusions: The present randomized controlled trial shows that the survival of zirconia-based and metal-based single crowns is similar over a follow-up period of 5 years. No significant differences in esthetic, functional and biological outcomes were demonstrated between the two groups. The main failure mode was the chipping fracture of the veneering ceramic in both materials. The aim of this study was to carry out a representative survey on the implementation of and experience with repairs of single-tooth restorations among dentists in the Canton of Zurich, Switzerland. An anonymous questionnaire was sent to all 1,411 dentists registered in the Canton of Zurich; 38.9% of the delivered questionnaires were returned and 35.3% could be evaluated. The statistical analysis comprised Kendall’s rank correlation coefficient (tau), Wilcoxon signed-rank tests, and Kruskal-Wallis tests. The level of significance was set at p≤0.05. Repair restorations are frequently made (composite: 98.5%, ceramic: 88.9%, crowns: 86.5%, metal: 54.6%, amalgam: 51.5%). Main indications for repairs were the partial loss of an existing restoration or of the adjacent dental hard substance, while restoration failures due to secondary caries were repaired to a lesser extent. The decision to repair is largely dependent on the size of the defect (90%), the size of the original restoration (63%), and the material of the failed restoration (84%). Repair restorations are most frequently made with composite following adequate conditioning of the repair surface. A majority of the dentists rate the lifespan of repair restorations as reduced in comparison with newly made restorations. In summary, repairs of defective single-tooth restorations are frequently performed by dentists in the Canton of Zurich, Switzerland, and constitute a well-established treatment procedure.

This cross-sectional survey investigated the reasons for placement and replacement of single crowns, the type of materials selected for initial placement and evaluated their longevity. Information was collected over 19 months period using a questionnaire focusing on the principal reasons for the placement and replacement of crowns, selected material type and the age of the crowns at the time of replacement. A total of 842 single crowns were evaluated in 476 patients. Of the 842 crowns, 472 (56%) were initial placements and 370 (44%) replacements. The main reason for placement of first single crowns was related to endodontic problems (26.5%). Metal-ceramic was indicated significantly more frequent (88.9%; p⟨0.01) than other materials for the placement and replacement of single crowns. Objectives: To prospectively evaluate the clinical long-term outcome of tooth-supported crowns (SCs), implant-supported crowns (ISCs), and fixed dental prostheses (FDPs) made of a lithium-disilicate glass-ceramic framework material (IPS Empress 2)., Materials and Methods: Between 1997 and 1999, a total of 184 restorations (106 SCs, 32 ISCs, 33 FDPs, and 13 diverse restorations) were placed in 73 patients. Kaplan-Meier estimation was applied for survival and chipping-free rates. Inter-group comparison of both rates was realized by a log rank test and a 2×2 contingency table. Also, SCs and FDPs were compared regarding adhesive vs. conventional cementation, and anterior vs. posterior positioning, for impact on survival., Results: Due to 14 dropouts (34 restorations) and reasonable exclusion of 19 other restorations, the final dataset included: i) 87 SCs [37 patients, mean observation time 11.4 (±3.8)years]; ii) 17 ISCs [12 patients, mean observation time 13.3 (±2.3)years; and iii) 27 FDPs [19 patients, mean observation time 8.9 (±5.4)years]. The 10-year survival rate/chipping-free rate for SCs were 86.1%/83.4%, for ISCs 93.8%/94.1%, and for FDPs were 51.9%/90.8%. Both ISCs and SCs had a significantly higher survival than FDPs (ISCs vs. FDPs: both tests p=0.001; SCs vs. FDPs: p=0.001 and p=0.005). Differences in the chipping-free rates did not reach significance. Also, neither the cementation mode nor positioning of the restoration had an impact on survival., Conclusions: SCs had a slightly lower outcome than can generally be expected from single crowns. In contrast, ICSs had a favorable outcome and the FDPs predominantly failed., Clinical Significance: The practitioner's choice of dental materials is based (at best) on long-term experience. Introduction: The aim was to compare the periapical status of root-filled teeth restored with resin composite, laboratory-fabricated crowns, or amalgam in a Swedish adult population., Methods: The subjects consisted of 440 individuals from a randomly selected sample of 1000 adult residents of a Swedish county. The type, material, and quality of the restorations were recorded for all root-filled teeth by using clinical examination and intraoral clinical photographs. Periapical status, root-filling quality, and marginal bone loss were evaluated on panoramic radiographs. The association between periapical status and type, material, and quality of the restorations was analyzed by using the χ(2) test and logistic regression., Results: No difference in the frequency of apical periodontitis (AP) between teeth restored with resin composite, laboratory-fabricated crowns, or amalgam (29.7%, 26.2%, and 43.1%, respectively) of adequate quality was found. No association between AP and resin composite restorations was disclosed; however, there was an association between AP and inadequate root-filling quality and marginal bone loss >1/3 of the root length., Conclusions: The results did not indicate any association between AP and resin composite restorations. Statement of Problem: Fundamental crown preparation principles are taught and are highly regarded in dental education. Whether tooth preparations made by dental students match these principles is unclear., Purpose: The purpose of this retrospective study was to report the total occlusal convergence (TOC) and margin widths of crown preparations clinically prepared by New Zealand predoctoral dental students between 2013 and mid-2015., Material and Methods: A total of 371 stereolithography files of tooth preparations for metal ceramic crowns prepared by predoctoral dental students were extracted from the Technical Services Laboratory database at the University of Otago. The files were put through the preparation measuring software Preppr, with outputs being TOC angles in faciolingual and mesiodistal cross sections and margin widths in facial, lingual, distal, and mesial aspects. Means, standard deviations, 95% confidence intervals, and distributions using box and whisker graphs were calculated and presented., Results: The majority of TOC angles fell within an acceptable range of 10 to 20 degrees; however, the angles ranged from undercuts (<0 degrees) to >60 degrees. The majority of margin widths were between 0.5 and 1 mm, while the maximum was approximately 2 mm and the minimum was 0 mm., Conclusions: Predoctoral dental students in New Zealand are able to produce literature-recommended TOC angles and margin widths for metal ceramic crowns; however, further attention and training are needed for excessive tooth preparations, mainly in the form of large TOC angles., (Copyright © 2016 Editorial Council for the Journal of Prosthetic Dentistry. Peri-implantitis is an inflammatory disease affecting soft and hard tissues surrounding dental implants. As the global number of individuals that undergo restorative therapy through dental implants increases, peri-implantitis is considered as a major and growing problem in dentistry. A randomly selected sample of 588 patients who all had received implant-supported therapy 9 y earlier was clinically and radiographically examined. Prevalence of peri-implantitis was assessed and risk indicators were identified by multilevel regression analysis. Forty-five percent of all patients presented with peri-implantitis (bleeding on probing/suppuration and bone loss >0.5 mm). Moderate/severe peri-implantitis (bleeding on probing/suppuration and bone loss >2 mm) was diagnosed in 14.5%. Patients with periodontitis and with ≥4 implants, as well as implants of certain brands and prosthetic therapy delivered by general practitioners, exhibited higher odds ratios for moderate/severe peri-implantitis. Similarly, higher odds ratios were identified for implants installed in the mandible and with crown restoration margins positioned ≤1.5 mm from the crestal bone at baseline. Traditionally, faculty assessment of preclinical crown preparations occurs by visualizing preparation features. However, contemporary CAD/CAM tools have the ability to more precisely evaluate preparation features, which is beneficial for psychomotor development. Taper is one of the most challenging features to objectively assess. The aim of this study was twofold: first, to validate the software's ability to distinguish differences in taper, and second, to compare traditional faculty assessment with digital assessment of taper. In the study, 30 all-metal crown preparations were created on typodont teeth with varying degrees of axial reduction and placed into three groups based on amount of taper (minimum, moderate, or excessive). Each tooth was scanned with the D4D scanner, and the taper was analyzed using E4D Compare. A second experiment used 50 crown preparations of tooth #19, which were done as a formative exercise. A comparison faculty assessment with CAD/CAM assessments of taper was performed. The results showed that when the taper was varied, E4D Compare was able to distinguish those differences; the Tukey post-hoc test revealed a significant difference (p=0.001). The qualitative analysis comparing faculty grading to CAD/CAM grading demonstrated a trend for CAD/CAM to be more precise. These results suggest that E4D Compare is an effective means of quantitatively measuring the amount of total occlusal convergence or taper on a crown preparation and that digital assessment may be more precise than faculty visual grading.

A search of the medical and dental records at the Matthew J. Ryan Veterinary Hospital of the University of Pennsylvania was conducted to identify dogs that received full or partial prosthodontic crowns over a 13-year period (2000-2012). Forty-one dogs with a total of 68 prosthodontic crowns were identified. Further criteria for acceptance into the study included presence of complete medical records containing adequate information pertaining to the procedures performed, and current follow-up either by telephone, electronic mail, or via in-person examination. Treatment was considered to be successful when the prosthodontic crown was in place and there was no further structural injury to the tooth upon making contact with the client or at the time of death of the dog confirmed by the client. The mean number of days the prosthodontic crowns remained in place without further injury to the tooth was 1,598 (range, 161-4, 464 days [median, 1,414 days], standard deviation 1,093 days). Bond failure between the cement and the tooth or the cement and the prosthodontic crown occurred in 3 cases (4.4%). Fracture of the prosthodontically treated tooth occurred in an additional 7 cases (10.3%). Treatment was classified as successful in 58 cases (85.3%). The results suggest that prosthodontic crown therapy is a successful, practical, and durable treatment option for protection of previously injured teeth in pet and working dogs.

Background: Single implants and implant-supported single crowns (ISSCs) have become popular treatment modalities for single tooth replacement. Studies have identified high implant survival rates, but also many complications. The aim of this five-year retrospective study was to assess the survival rates, complication types and occurrences for single implants and ISSCs at the Melbourne Dental School (MDS) in Victoria, Australia., Methods: A search of the Royal Dental Hospital of Melbourne (RDHM) database was conducted for data on all implant treatment and reported complications during the period between 1 January 2005 and 31 December 2009. Complications were categorized into surgical, biological and restorative types., Results: A total of 622 implant fixtures and 444 ISSCs were inserted into 406 patients. Seventeen implants failed during the mean follow-up time of 2.18 years, yielding a 2.7% failure rate and an estimated one- and five-year survival rate of 98.8% and 93.9%, respectively. The cumulative surgical, biological and restorative complication incidences were 11.9%, 17.6% and 14.1%, respectively., Conclusions: This study confirmed that single tooth replacement using implant therapy within a teaching environment had a high survival rate. However, complications frequently occurred. This article only provides a descriptive analysis. Purpose: This study compares the FEA-calculated stresses generated within the supporting periodontal structure of a mandibular second molar restored with a full ceramic crown and with a porcelain-fused-to-metal (PFM) crown, each resisting occlusal forces acting at different inclinations., Materials and Methods: Three-dimensional finite element models representing the crown of an unrestored mandibular second molar and two relevant restoration designs were constructed. Two designs represented the molar restored with a full ceramic crown and with a PFM crown, each cemented with the same resin cement. Occlusion was assumed at three contact areas, which equally shared a 100 N force. The analysis was carried out for forces located in the bucco-axial-lingual plane at five inclinations, 0, 22.5°, 45°, 67.5°, and 90°, measured from the axial direction of the tooth. The magnitudes and sites of the maximum equivalent stress (MES) generated within the supporting periodontium of each analyzed model were collected., Results: Generally, there were no significant differences in the site and magnitude of MES in the regions of the supporting structure for the analyzed models. The MES was located at the tooth periodontal ligament (PDL) bifurcation area and distal root apex, crestal bone at the junction between cortical and cancellous bone, and the distal wall of the mesial root socket of cancellous bone. The highest stresses corresponded to a horizontal load, followed by the axial load in the PDL and cortical zones. The results show opposite observations for the cancellous bone. The lowest stresses were generated under a load inclination between 22.5° and 45°., Conclusion: Considering the stresses generated within the supporting structures, the present work validates, by calculation, the proposed clinical use of either a full ceramic crown or a PFM crown as a restoration for mandibular second molars., (© 2014 by the American College of Prosthodontists.)

Aims: To assess patients with acute odontogenic maxillofacial infections (AOMIs), regarding their functional dentition and dental treatment needs, and identify factors explaining these outcomes., Methods: During a 1-year period, 160 patients with AOMIs were treated at the specialised dental care centre of Vilnius University. Both oral status and specific dental treatment needs were evaluated for each patient. For the restorative dental treatment need, we examined if patients needed fillings, crowns or bridges. Periodontal dental treatment needs were based only on the most severe cases, and patients were allocated either to a group for which periodontal treatment was recommended or to a group that did not need periodontal treatment. Based on these clinical assessments, four ratios for specific dental treatment needs (restorations, extractions, endodontic treatment and periodontal treatment) and two summative ratios (total dental treatment needs and presence of a functional dentition) were calculated. The questionnaire included variables from various domains., Results: Patients with AOMIs retained one-third of their functional dentition and the mean ± standard deviation of their total dental treatment needs was 46.0 ± 29.7%, of which 32.4 ± 17.1% related to the need for restorations. Higher dental treatment needs were associated with a low level of education, low income, irregular oral self-care, systemic diseases and self-treatment of acute dental conditions before seeking professional help., Conclusions: Patients with AOMIs retained one-third of their functional dentition, and almost half of their dentition were in need of dental treatment., (© 2015 FDI World Dental Federation.)

Cleft lip and palate incidence is high in northern Finland. This study aimed to investigate the proportion of children in need of restorative dental treatment among cleft lip and palate patients in northern Finland, as well as their need for dental treatment under general anesthesia. The records of 183 cleft lip and palate patients, treated in Oulu University Hospital from 1997 to 2013, were reviewed. Data on dental caries were analyzed in association with cleft type, considering also the presence of syndromes. The frequency of dental general anesthetic (DGA) use, and of treatments, were also analyzed. Dental treatment need was most frequently observed, in this rather limited study population, in patients with the most severe deformities, namely bilateral cleft lip and palate, of whom 60% had caries. Among the study population, 11.5% (n = 21) had a syndrome. Of those, 57.1% had dental caries at the age of 3 or 6 yr, and only four could be treated without a DGA. Dental treatment under general anesthesia was performed in 14.8% of cleft patients without a syndrome, but in 38.1% of those with a syndrome. General anaesthesia is required for the provision of dental care more often in cleft (17.5%) than in non-cleft (0.2%) patients, and especially for those with a syndrome., (© 2015 Eur J Oral Sci.)

Statement of Problem: Determining the retention and resistance of a tooth preparation for a complete crown has only existed in theory, and these theories have never been measured on tooth preparations performed in vivo., Purpose: The purpose of this study was to measure the theoretical retention and resistance of clinically produced complete crown preparations by using an objective measuring method., Material and Methods: Stone dies from 236 complete crown preparations were collected from dental laboratories. The dies were scanned and analyzed with the coordinate geometry method. Cross-sectional images were captured, and the surface area was measured with a cone frustum and right truncated pyramid formula. Two different theories of resistance form, the "on" or "off" theory (limiting taper) and the linear model (resistance length), were calculated for premolar and molar preparations., Results: The mean surface areas ranged from 33.97 mm(2) to 105.44 mm(2) for the cone frustum formula and 41.75 mm(2) to 117.50 mm(2) for the right truncated pyramid formula. The facial side of maxillary premolars exhibited the highest percentage of resistance form with the limiting taper, at 58%, and the mesial side of the mandibular molars exhibited the lowest percentage of resistance form, at 6%., Conclusions: The objective method used in this study provides a way for retention and resistance theories to be tested and for further clinical implications to be investigated., (Copyright © 2015 Editorial Council for the Journal of Prosthetic Dentistry. Statement of Problem: Scientific data about the long-term survival of teeth treated with cast post and cores are scarce. Retrospective studies often use different target events for their analyses. A comparison is therefore complicated. For associated tooth-, jaw-, and patient-related factors little evidence exists as to their effect on survival., Purpose: The purpose of this study was to extend the knowledge on the survival of teeth treated with cast post and cores for observation periods of more than 10 years. A decrease or increase in survival times according to the presence or absence of associated parameters needs to be evaluated., Material and Methods: A retrospective evaluation was conducted of all cast post and cores inserted in 1 university clinic between January 1992 and June 2011. A Kaplan-Meier survival analysis was carried out by using extraction as the target event. The survival curves for different tooth types, the presence or absence of adjacent teeth, and the prosthetic restoration of the respective jaws were compared by using the log-rank test (α=.05). A Cox regression model was calculated for multivariate analyses., Results: A total of 717 cast post and cores for 343 patients were recorded. The mean survival time was 13.5 years. A statistically significant decrease in survival times was found for canines (11.9 years) and premolars (13.4 years) versus molars (14.1 years), no adjacent teeth (10.6 years) versus at least 1 adjacent tooth (13.8 years), and the restoration with removable dental prostheses (12.5 years) versus fixed dental prostheses and single crowns (13.9 years). The largest reduction in survival time was found for teeth being used as an abutment for a double crown-retained removable partial dental prosthesis (telescopic denture) (9.8 years). Tooth type and adjacent tooth status remained as significant variables within the multivariate Cox regression model., Conclusions: Cast post and cores have an acceptable long-term survival time. Purpose: The purpose of this retrospective clinical study was to evaluate the clinical outcomes of fixed implant-supported dental prostheses (FISDPs) and removable ISDPs (RISDPs) retained by telescopic attachments, the factors influencing survival, and the type and number of maintenance treatments required during the observation period., Materials and Methods: This retrospective clinical study is based on patients who were provided with ISDPs between 2004 and 2010. Patient sex, type of prosthesis, location, opposing dentition, and the effect of continuous follow-up on the probability of a favorable outcome, as well as the number of maintenance treatments, were analyzed. A statistical analysis was performed using the Kaplan-Meier method., Results: A sample of 233 patients with 157 FISDPs and 76 RISDPs supported by a total of 567 implants was randomized and included in the analysis. The mean observation period was 15.9 ± 15.4 months (maximum, 66.0 months). During the observation period, 3.9% of prostheses (7/157 FISDPs and 2/76 RISDPs) ceased to function and 2.3% of implants (13 implants) were lost. The mean survival time was 59.7 ± 2.3 months, with a survival probability of 90.2% after 3 years. Prostheses in the mandible showed significantly higher survival rates than those in the maxilla. Maintenance treatments had to be performed at an earlier stage for patients with RISDPs than for patients with FISDPs. In patients with a conventional removable prosthesis in the opposing arch, the time until maintenance was needed was significantly shorter., Conclusion: Within the limitations of a retrospective study, it can be concluded that the FISDPs and RISDPs show equally good survival rates after 3 years in function. RISDPs showed a greater and earlier need for maintenance treatments during the first years in function.

Purpose: To assess and compare the outcomes and economic complication burden of three-unit tooth-supported fixed dental prostheses (TFDPs) and implant-supported single crowns (ISCs) provided to a sequential cohort in a specialist prosthodontic practice over a 15-year period., Materials and Methods: Sequential patients requiring replacement of a single missing tooth between 1996 and 2010 with a metal-ceramic three-unit TFDP (n = 145 patients, n = 174 prostheses) or ISC (n = 174 patients, n = 220 prostheses) were included. Prostheses subjectively judged at insertion to have an unfavorable 10-year prognosis (17 TFDPs, 0 ISCs) were removed from statistical analyses. The estimated cumulative survival (ECS) was calculated with the life table actuarial method and standard errors calculated with the Greenwood formula. Differences in outcomes between all prostheses and those replacing only anterior or only posterior teeth were assessed with the log rank test. Complication incidence, severity, and economic burden, measured in time/cost accounting units (TAUs), were tallied and compared descriptively., Results: The 15-year ECS did not differ for 112 TFDPs (92.75% ± 3.28%) and 81 ISCs (95.95% ± 2.92%) replacing posterior teeth. However, the 15-year ECS was significantly greater for 139 ISCs (93.33% ± 6.44%) than for 45 TFDPs (82.82% ± 6.50%) replacing anterior teeth. The economic burden of nonterminal complications for both prostheses was low (mean = 0.3 and 0.2 TAUs per prosthesis for TFDPs and ISCs, respectively). This equated to 3 TAUs/100 years in clinical service and 4 TAUs/100 years in clinical service for the TFDPs and ISCs, respectively., Conclusion: The survival of three-unit TFDPs and ISCs over 15 years was not statistically different when replacing posterior teeth, but ISCs survived significantly better when replacing anterior teeth. The complication rates of the TFDPs and ISCs were similar, but the economic burden for the TFDPs was greater.

Statement of Problem: An implemented objective measuring system for measuring clinical tooth preparations does not exist., Purpose: The purpose of this study was to compare clinically achieved tooth preparations for ceramic crowns by general dentists with the recommended values in the literature with an objective measuring method., Material and Methods: Two hundred thirty-six stone dies prepared for anterior and posterior complete ceramic crown restorations (IPS e.max Press; Ivoclar Vivadent) were collected from dental laboratories. The dies were scanned and analyzed using the coordinate geometry method. Cross-sectioned images were captured, and the average total occlusal convergence angle, margin width, and abutment height for each preparation was measured and presented with associated 95% confidence intervals., Results: The average total occlusal convergence angles for each tooth type was above the recommended values reported in the literature. The average margin widths (0.40 to 0.83 mm) were below the minimum recommended values (1 to 1.5 mm). The tallest preparations were maxillary canines (5.25 mm), while the shortest preparations were mandibular molars (1.87 mm)., Conclusions: Complete crown preparations produced in general practice do not achieve the recommended values found in the literature. Statement of Problem: A valid system for assessing and classifying functional occlusion has not been established. The prevalence of anterior protected articulation is not known., Purpose: The purpose of this study was to quantify the prevalence of various functional occlusal contact patterns, including anterior protected articulation, among dental students., Material and Methods: Occlusal contacts were examined during lateral excursions from the maximal intercuspal position to the canine-to-canine position in 100 young adults. A combination of 3 common clinical methods was used: a visual examination, articulating paper, and feedback from the participants., Results: Data from 3 classification systems were analyzed: (1) Occlusal contacts on the working side only – canine protected articulation was present in 25% of laterotrusions; anterior protected articulation was present in 18.5% of laterotrusions. Group function was present in 56% of laterotrusions. Other schemes were present in 0.5% of laterotrusions. (2) Contacts on both the working and the nonworking side. (3) Contacts on the working and nonworking side during both right and left laterotrusion. Nonworking side contacts were present in 33% of the participants. Nonworking side interference was present in 1 participant., Conclusions: The prevalence of anterior protected articulation found in this study was high enough to allow anterior protected articulation to be considered one of the fundamental working side occlusal contact patterns. Statement of Problem: Little peer-reviewed information is available regarding the accuracy and reproducibility of digitally fabricated casts compared to conventional nondigital methods., Purpose: The purpose of this in vitro study was to compare the accuracy and reproducibility of a digital impression and cast fabrication with a conventional impression and cast fabrication., Material and Methods: Conventional impressions were made via a 1-step single viscosity technique with vinyl siloxanether material of a typodont master model, and conventional casts were cast from dental stone. Digital impressions were obtained with a digital scanner, and digital stereolithographic models were printed. The typodont and fabricated casts were digitized with a structured light scanner and saved in surface tessellation language (STL) format. All STL records were superimposed via a best-fit method. The digital impression and cast fabrication method was compared with the conventional impression and cast fabrication method for discrepancy, accuracy, and reproducibility. The Levene test was used to determine equality of variances, and a 1-way ANOVA was conducted to assess the overall statistical significance of differences among the groups (n=5, α=.05)., Results: No significant statistical difference was found between the digital cast and conventional casts in the internal area or finish line area (P>.05). In addition, there was no statistically significant difference between these 2 techniques for a fixed dental prosthesis or single crown (P>.05). However, statistically significant differences were observed for overall areas of the casts in terms of accuracy (P<.01) and reproducibility (P<.001). Digital impression and cast fabrication were less accurate and reproducible than conventional impression and cast fabrication methods., Conclusions: No statistically significant difference was found between the digital cast and conventional cast groups in the internal and finish line areas. Objective: To investigate time trends in dental service provision., Methods: A random sample of Australian dentists was surveyed by mailed questionnaires in 1983-1984, 1993-1994, 2003-2004, and 2009-2010 (response rates 67-76%). The service rate per visit was collected from a log of services., Results: The rate of service provision per visit [rate ratio (RR)] increased from 1983-1984 to 2009-2010 for the service areas of diagnostic (RR=1.8; 1.6-1.9), preventive (RR=1.9; 1.6-2.1), endodontic (RR=2.1; 1.7-2.6), and crown and bridge (RR=2.9; 2.3-3.8), whereas prosthodontic services decreased (RR=0.7; 0.6-0.9)., Conclusions: The profile of services provided by dentists changed over the study period to include less emphasis on replacement of teeth and more on diagnosis, prevention, and retention of natural dentitions., (© 2014 FDI World Dental Federation.)

Background: To investigate the periodontal disease status in a multi-center cross-sectional study in Germany. Associations of dental, socio-economic, blood and biomedical variables with periodontal outcome parameters were evaluated., Methods: From 4 different centers N = 311 persons were included, drawn randomly from the registration offices. Maximal pocket depth (PD) was used as primary indicator for periodontitis. It was classified as: no/mild ≤3 mm, moderate 4-5 mm, severe ≥6 mm. Associations between socioeconomic (household income, education), lifestyle, and biomedical factors and PD or bleeding on probing (BOP) per site ("Yes"/"No") was analyzed with logistic regression analysis., Results: Mean age of subjects was 46.4 (range 20-77) years. A significantly higher risk of deeper pockets for smokers (OR = 2.4, current vs. never smoker) or persons with higher BMI (OR = 1.6, BMI increase by 5) was found. Severity of periodontitis was significantly associated with caries lesions (p = 0.01), bridges (p < .0001), crowns (p < .0001), leukocytes (p = 0.04), HbA1c (p < .0001) and MCV (p = 0.04). PD was positively correlated with BOP. No significant associations with BOP were found in regression analysis., Conclusions: Earlier findings for BMI and smoking with severity of PD were confirmed. Dental variables might be influenced by potential confounding factors e.g. dental hygiene. For blood parameters interactions with unknown systemic diseases may exist.

Introduction: The purpose of this study was 2-fold: (1) to investigate the critical time-lapse of endodontic intervention subsequent to various restorations and tooth surfaces and (2) to assess and compare the risk factors associated with the restorations, tooth surfaces, and endodontic treatment., Methods: A comprehensive computerized analysis of all dental school patients at the Case Western Reserve University School of Dental Medicine who received restorations from 2008-2013 was obtained. Inclusion and exclusion criteria were applied. The sample size was limited to teeth with endodontic treatment completed by the endodontic postgraduate dental clinic. Data collected included restoration type, restored tooth surfaces, tooth type, and the dates of restoration and subsequent endodontic treatment. A 2-sample paired t test (95% confidence interval, P < .05) and pair-wise comparison with Bonferroni corrections were implemented., Results: The mean time between restoration placement and resultant endodontic intervention was 270 days, with a mean difference of 247-294 days (P < .0001). Further analysis revealed composite resin was 1.91 times more likely than amalgam and 5.69 times more likely than crowns to cause resultant endodontic intervention. Teeth with 2 or more restorative surfaces required endodontic intervention (P < .001)., Conclusions: Of the patients who required endodontic treatment after restoration placement, the critical time-lapse was 9 months. Composite restorations and teeth with 2 or more restorative surfaces were significantly associated with endodontic treatment. From the results of the current study, we recommended that all dental practitioners should perform a thorough endodontic evaluation and diagnosis before, during, and after all restorative procedures., (Copyright © 2014 American Association of Endodontists. Statement of Problem: Milling is a central and important aspect of computer-aided design and computer-aided manufacturing (CAD/CAM) technology. High milling accuracy reduces the time needed to adapt the workpiece and provides restorations with better longevity and esthetic appeal. The influence of different milling processes on the accuracy of milled restorations has not yet been reviewed., Purpose: The purpose of this study was to investigate the influence of different milling processes on the accuracy of ceramic restorations., Material and Methods: Four groups of partial crowns were milled (each n = 17): Three groups in a 4-axial milling unit: (1) 1-step mode and Step Bur 12S (12S), (2) 1-step mode and Step Bur 12 (1Step), (3) 2-step mode and Step Bur 12 (2Step), and (4) one group in a 5-axial milling unit (5axis). The milled occlusal and inner surfaces were scanned and superimposed over the digital data sets of calculated restorations with specialized difference analysis software. The trueness of each restoration and each group was measured. One-way ANOVA with a post hoc Tukey test was used to compare the data (α = .05)., Results: The highest trueness for the inner surface was achieved in group 5axis (trueness, 41 ± 15 μm, P<.05). The 4-axial milling unit exhibited trueness at settings ranging from 61 μm (2Step) to 96 μm (12S). For the occlusal surface, the highest trueness was achieved with group 5axis (trueness, 42 ± 10 μm). The 4-axial milling unit exhibited trueness at settings ranging from 55 μm (1Step) to 76 μm (12S)., Conclusions: Restorations milled with a 5-axial milling unit have a higher trueness than those milled with a 4-axial milling unit. A rotary cutting instrument with a smaller diameter results in a more accurate milling process. Purpose: The purpose of this paper was to determine if number and cost of dental treatments in high caries-risk children differs in children with early dental intervention compared to children with later intervention., Methods: Billing data from children age zero to seven years old, whose first dental visit was between January 1, 2004 and December 31, 2004, were collected from 20 corporate treatment centers serving children from lower socioeconomic status backgrounds. Data included age at first visit, dental treatment codes, and associated costs for eight years after the first dental visit. Treatment included restorations, crowns, pulpotomies, and extractions. First visit age was categorized into early starters (younger than four years old) and late starters (four years of age or older). Linear regression with cluster adjustment for clinic determined a difference in costs and dental treatments by early and late starters., Results: Of 42,532 subjects, 17,040 (40 percent) were early starters and 25,492 (60 percent) were late starters. There were 3.58 more dental procedures performed on late starters, over eight years of follow-up, than on early starters (P<.001). Late starters spent $360 more over eight years of follow-up than early starters (P<.001)., Conclusion: In this study, number of procedures performed were fewer and cost of treatment less for children seen earlier versus later.

Objectives: The longevity of dental restorations in patients with amelogenesis imperfecta (AI) is limited. The aim of this paper is to compare oral health and longevity of dental restorations in a group of young patients with AI compared to a control group., Methods: Patients included were 82 patients with AI, 40 boys and 42 girls, 6 to 25 years old (mean age 14.5±4.3 years) and a control group matched in age, gender and residential area. All patients received an examination recording dental caries, gingivitis, previous therapy, replaced restorations, tooth sensitivity, and number of dental visits. Patient dental records, extending from 6 to 10 years before the study, provided data on previous care., Results: Annual mean number of dental visits in the AI group was 2.9±1.7 compared to 1.9±1.2 in the control group (p<0.001). DMFS was 8.1±15.6 in the AI group compared to 1.0±2.0 in the control group (p<0.001). The longevity of dental restorations was significantly lower in the patients with AI, with 24.7±35.1% of the AI group requiring replacement of fillings during the observation period compared to 9.23±23.7% in the control group (p=0.001). Patients with hypomineralized/hypomaturized AI have restorations of shorter longevity than those with hypoplastic AI (p<0.01). Porcelain crowns had significantly longer survival than composite resin materials in the AI group (p<0.001). Clinical Significance This study shows the need for long-lasting restorative solutions for patients with AI. Introduction: Experimental studies show that dental pulp cells respond unfavorably to contact with resin composite restorative material. Hypothetically, in a random population, the frequency of apical periodontitis should be higher for teeth restored with resin composite than with amalgam. Therefore, the aim was to compare the periapical status of non-root-filled teeth restored with resin composite, amalgam, or laboratory-fabricated crowns in an adult Swedish population., Methods: The subjects comprised 440 individuals from a randomly selected sample of 1,000 adult residents of a Swedish county. The type, material, and quality of the restorations were recorded for all non-root-filled teeth by clinical examination and intraoral clinical photographs. Periapical status was evaluated on panoramic radiographs. The association between periapical status and type, material, and quality of the restorations was analyzed using the chi-square test and logistic regression analysis., Results: There was no significant difference in the frequency of apical periodontitis (AP) between teeth restored with resin composite or amalgam (1.3% and 1.1%, respectively). The frequency of AP for teeth restored with laboratory-fabricated crowns was significantly higher (6.3%). Regression analysis showed no association between AP and resin composite restorations but a significant association with laboratory-fabricated crowns., Conclusions: The results indicate that the risk of damage to the pulp-dentin complex from exposure to resin composite material and dentin bonding agents shown in experimental studies is not reflected in the clinical setting. Purpose: To investigate the relationship between antenatal/intrapartum factors and Medicaid use., Methods: Three databases were used: (1) birth records; (2) Medicaid files; and (3) Medicaid dental claims., Results: Children of Caucasian mothers were 34 percent more likely to have more than one restorative claim versus children of African American mothers (odds ratio [OR] equals 1.34, 95 percent confidence interval [95% CI] equals 1.10 to 1.65, P<.005). Children born with low birth weight were 37 percent more likely to have emergency claims (OR equals 1.37, 95% CI equals 1.02 to 1.83, P=.03). The adjusted analysis found that Caucasian mothers had higher odds ratio of having a dental claim than African American mothers (P<.001): 33 percent for a restorative claim and 56 percent for an emergency claim. When race was analyzed, the odds of a restorative claim among African American mothers were 2.5 times higher in children delivered by C-section versus those vaginally delivered (OR equals 2.52, 95% CI equals 1.02-6.2, P<.001)., Conclusions: This study found: an association between children of Caucasian mothers and the likelihood of experiencing claims; and a relationship between children born with low birth weight and C-section and the likelihood of use of Medicaid services.

The objective of this study was to evaluate the potential of a survival prediction method for the assessment of ceramic dental restorations. For this purpose, fast-fracture and fatigue reliabilities for 2 bilayer (metal ceramic alloy core veneered with fluorapatite leucite glass-ceramic, d.Sign/d.Sign-67, by Ivoclar; glass-infiltrated alumina core veneered with feldspathic porcelain, VM7/In-Ceram Alumina, by Vita) and 3 monolithic (leucite-reinforced glass-ceramic, Empress, and ProCAD, by Ivoclar; lithium-disilicate glass-ceramic, Empress 2, by Ivoclar) single posterior crown restorations were predicted, and fatigue predictions were compared with the long-term clinical data presented in the literature. Both perfectly bonded and completely debonded cases were analyzed for evaluation of the influence of the adhesive/restoration bonding quality on estimations. Material constants and stress distributions required for predictions were calculated from biaxial tests and finite element analysis, respectively. Based on the predictions, In-Ceram Alumina presents the best fast-fracture resistance, and ProCAD presents a comparable resistance for perfect bonding; however, ProCAD shows a significant reduction of resistance in case of complete debonding. Nevertheless, it is still better than Empress and comparable with Empress 2. In-Ceram Alumina and d.Sign have the highest long-term reliability, with almost 100% survivability even after 10 years. When compared with clinical failure rates reported in the literature, predictions show a promising match with clinical data, and this indicates the soundness of the settings used in the proposed predictions., (© International & American Associations for Dental Research.)

Unlabelled: Minimal Intervention Dentistry (MID) is an effective treatment approach with increasing acceptance among dental professionals., Objective: This study aimed to evaluate the MID impact on Dentistry by analyzing procedures performed on patients treated at a Pediatric Dentistry Graduate Program clinic which implemented MID., Study Design: The number of procedures including sealants, modified atraumatic restorative treatment (mART), resin crowns, direct pulp capping, pulpotomy, pulpectomy, and deciduous/ permanent extractions from 333 pediatric patients treated between the years 2001 to 2003 and 2008 to 2010 in Distrito Federal, Brazil were analyzed. Statistical analysis involved chi-square and G Williams tests., Results: 783 procedures were analyzed and demonstrated that there was a significant reduction of sealant placement in the last triennium when compared to the first one (p<0.0001). Moreover, there was a significant increase in the amount of mART (p<0.0001). This increase in mART procedures resulted in a significant reduction in procedures with pulp involvement: direct pulp capping (p=0.0014), pulpotomy (p=0.0014) and pulpectomy (p=0.0002)., Conclusions: Based on the results, MID represented a positive impact on the intervention on caries lesions in patients, mainly reflected by the significant reduction in the number of direct pulp capping, pulpotomy and pulpectomy.

Statement of Problem: Total occlusal convergence of crown preparation is an important didactic and clinical concept in dental education., Purpose: The purpose of this study was to compare the discrepancy between the total occlusal convergence of dental students' typodont crown preparations and the ideal range (4 to 10 degrees) in 3 different regions of the mouth and in 4 different planes of the teeth., Material and Methods: The dental students of the Class of 2012 at Harvard School of Dental Medicine were asked to prepare typodont teeth for crowns on 3 different teeth, the maxillary left central incisor (ceramic), mandibular left first molar (complete metal), and mandibular left first premolar (metal ceramic), during their third year preclinical summative examination and the Comprehensive Clinical Examination in their fourth year. Eighteen students prepared 3 teeth in their third and fourth years, whereas 19 students participated only in their fourth year, for a total of 55 sets of 3 teeth. By using custom fit die bases to reproduce the position, a novel procedure of measuring each tooth was accomplished in 4 different planes: the faciolingual, mesiodistal, mesiofacial-distolingual, and mesiolingual-distofacial. The total occlusal convergence of each image was measured with a computer screen protractor. The gingival 2 mm of the axial wall was used to determine the taper of each wall. Linear mixed model analysis was used to estimate and compare the total occlusal convergences of different teeth and planes (α=.05). Bonferroni corrections were used to adjust for post hoc multiple comparisons., Results: The mean total occlusal convergence varied by tooth and plane (2-way interaction; P<.001). For the first molar, dental students excessively tapered in all 4 planes; the model-predicted 99% CIs for the total occlusal convergence were as follows: faciolingual (12.7, 19.4), mesiodistal (14.0, 19.3), mesiofacial-distolingual (13,4, 19.4), and mesiolingual-distofacial (13.7, 19.1). For the central incisor, 99% CIs for the total occlusal convergence were (15.9, 24.4) for the faciolingual measurement, providing strong evidence of excessive tapering, and (4.1, 8.0) for the mesiodistal measurement, which was within the ideal total occlusal convergence range. The mesiofacial-distolingual and mesiolingual-distofacial planes in the central incisor and all 4 planes in the first premolar had mean total occlusal convergences that exceeded 10 degrees; however, excessive tapering could not be statistically established, because their CIs included values within the ideal range., Conclusions: The present study found significant evidence of excessive tapering in a study comparing the total occlusal convergence values of crown preparations with those of the ideal preparation for 3 different teeth in 4 different planes. Partial secondary adentia together with caries and parodont diseases is among the most widespread diseases of dentofacial system, correlated, in addition, to the number of lost teeth, with the age. Result in speech and chewing functions' disorders, change of face shape, esthetic and psychosocial inferiority, pathologies of digestive and other systems, formation of the chronic infection areas, changes of reactivity etc,. Improvement of the methods of prevention and treatment of the above diseases is one of the most significant problems in the dentistry. Effectiveness of dental assistance organization and planning is based on the epidemiological studies. In Georgia, epidemiological studies were conducted in the populations of various age groups in previous years though, according to our data, no studies of dentition defects and deformations were conducted in the recent decades. Goal of our research was study of prevalence and severity of dentition defects and deformations, regarding sizes of existing defects, their localization, causes and types of deformation in different age groups, for development of specialized dentistry assistance plans and prevention programs. We have studied 147 patients of ages between 15 and 40 years. We have developed special questionnaire. Researches showed that in the studied age group (15-40), 62% (96 patients), i.e. more than half of studied 147 patients had secondary adentia. Most of them had the defects in the buccal teeth area. 112 of studied 147 patients required orthopedic treatment but only 18 (16%) of them have visited the clinic for this purpose while 94 (83.9%) of them were unaware about need of prothetic assistance. Regarding significance of the mentioned problem, the obtained results show the need of timely orthopedic intervention for the purpose of prevention of further complications. As a result, a specialized dental assistance plan and prevention measures' program has been developed to prevent further complications.

Objective: Nearly all state Medicaid programs reimburse nondental primary care providers (PCPs) for providing preventive oral health services to young children; yet, little is known about how treatment outcomes compare with children visiting dentists. This study compared the association between the provider of preventive services (PCP, dentist, or both) with Medicaid-enrolled children before their third birthday and subsequent dental caries-related treatment (CRT) and CRT payment., Methods: We conducted a retrospective study of young children enrolled in North Carolina Medicaid during 2000 to 2006. The annual number of CRT and CRT payments per child between the ages of 3 and 5 yr were estimated with a zero-inflated negative binomial regression and a hurdle model, respectively. Models were adjusted for relevant child- and county-level characteristics and used propensity score weighting to address observed confounding., Results: We examined 41,453 children with > 1 preventive oral health visit from a PCP, dentist, or both before their third birthday. Unadjusted annual mean CRT and payments were lowest among children who had only PCP visits (CRT = 0.87, payment = $172) and higher among children with only dentist visits (CRT = 1.48, payment = $234) and both PCP and dentist visits (CRT = 1.52, payment = $273). Adjusted results indicated that children who had dentist visits (with or without PCP visits) had significantly more CRT and higher CRT payments per year during the ages of 3 and 4 yr than children who had only PCP visits. However, these differences attenuated each year after age 3 yr., Conclusions: Because of children's increased opportunity to receive multiple visits in medical offices during well-child visits, preventive oral health services provided by PCPs may lead to a greater reduction in CRT than dentist visits alone. Objectives: The aims of this pilot study were to investigate the prevalence of xerostomia and hyposalivation and their impact on quality of life in a cohort of elderly patients including dental status and the character of potential prosthetic restorations as independent variables., Methods: Patients aged 60 years or older without any objective or subjective need for prosthodontic treatment taking part in a regular recall programme were included in the trial. Quality of life was assessed using the German version of the GOHAI; prevalence and severity of xerostomia was investigated using the shortened version of the Xerostomia Inventory (XI). Stimulated salivary flow rate was determined using a sialometric approach. Dental status and the character of prosthetic restorations (no/fixed restorations and removable but tooth-supported dentures vs. gum-supported dentures) were assessed in a clinical examination by experienced dentists specialized in prosthodontic treatment., Results: A total of 68 patients were included in the trial; a prevalence of xerostomia of 16% and a prevalence of hyposalivation of 31% were identified. The quality of life in the study cohort decreased significantly as a function of xerostomia severity but not salivary flow; moreover, a significant impact of the number of teeth/implants in the upper jaw and the presence of gum-supported dentures in both jaws on GOHAI scores could be identified., Conclusions: Within the limitations of a pilot study, the results support the assumption that the quality of life in elderly patients is particularly related to their subjective perception of xerostomia. A decline in salivary flow, the dental status and the character of prosthetic restorations appear to play a subordinate role for the quality of life in elderly patients., Clinical Significance: The quality of life in elderly patients may be severely diminished due to an increased subjective perception of dry mouth. Objective: To provide a snapshot of the New Zealand dental technology industry and influencing factors., Background: Developing an understanding of the commercial dental laboratory environment in New Zealand can provide insight into the entire dental industry., Methods: A web-based survey was the primary method for data collection, with separate questionnaires used for dental laboratory owners and dental technician employees., Results: The mean net income for dental laboratory owners in New Zealand was similar to that of the United Kingdom, at $40.50 per hour. Clinical dental technicians are the highest paid employees, with a mean of $33.49 per hour. The mean technical charge for complete dentures was $632.59; including clinical services, it was $1907.00. The mean charge for a porcelain-fused-to-metal (PFM) crown was $290.27. Dental laboratory owners expressed fear about the possibility of losing dental clients to overseas laboratories due to the availability and cheap charge of offshore work. Only 25.4% of dental laboratories surveyed had computer-aided design (CAD) facilities, and even fewer (7.9%) had computer-aided manufacturing (CAM) systems., Conclusion: Clinical dental technology appears to be prospering. The dental technology industry appears to be adapting and remains viable, despite facing many challenges.

Purpose: This retrospective study investigated the frequency and time history of chipping and facing failures, recurrent caries (RC), periodontitis (PE), and loss of retention (LR) of porcelain-fused-to-metal (PFM) single crowns., Materials and Methods: A total of 997 PFM single crowns had been inserted according to a standardized treatment protocol from January 1984 to May 2009. The frequency and time history of chipping and facing failures were evaluated, as were possible risk factors from historical clinical data. Risk factors were bruxism, the type of antagonist, and the location of crowns (mandible, maxilla, anterior, posterior). The survival times of crowns were estimated using Kaplan-Meier (KM) analysis., Results: The median follow-up time calculated with the inverse KM method was 4.33 years. Anterior and posterior PFM crowns showed 5-year survival rates (time to crown replacement) of 96.4% and 97.5% and 10-year survival rates of 92.3% and 95.9%, respectively. Chipping was found in 17 (1.7%) of the 997 PFM crowns. According to the KM method, the 5- and 10-year free-of-event-rates for chipping of anterior crowns were both 98.9%, and the rates for posterior crowns were 98.2% for 5 years and 97.3% for 10 years. Thirteen patients showed RC (1.3%) and 144 (14.4%) PE. The 5-year free-of-event-rate for RC was 98.7% and the 10-year free-of-event rate was 97.2%. For PE, the 5-year free-of-event-rate was 85.8% and the 10-year free-of-event rate was 72.2%. The 5- and 10-year free of- event-rates for LR were 92.2% each for anterior teeth and 97.1% each for posterior teeth., Conclusions: Patients with PFM crowns may expect long-term survival for their restoration. Clinical complications are rare. Chipping of the veneer or loss of retention may occur during the first few years. While chipping of the veneer may occur during the first few years, the frequency of caries or periodontitis increases with the length of oral service and with age.
