Abstract

Although hydroxychloroquine (HCQ) has long been used to treat autoimmune diseases, its mechanism of action remains poorly understood. In CD4 T-cells, we found that a clinically relevant concentration of HCQ inhibited the mitochondrial antioxidant system triggered by TCR crosslinking, leading to increased mitochondrial superoxide, impaired activation-induced autophagic flux, and reduced proliferation of CD4 T-cells. In antigen-presenting cells, HCQ also reduced constitutive activation of the endo-lysosomal protease legumain and toll-like receptor 9, thereby reducing cytokine production, but it had little apparent impact on constitutive antigen processing and peptide presentation. HCQ's effects did not require endo-lysosomal pH change, nor impaired autophagosome-lysosome fusion. We explored the clinical relevance of these findings in patients with celiac disease-a prototypic CD4 T-cell-mediated disease-and found that HCQ limits ex vivo antigen-specific T cell responses. We report a T-cell-intrinsic immunomodulatory effect from HCQ and suggest potential re-purposing of HCQ for celiac disease.

Abstract

In an external (static of high-frequency) electric field, image forces cause a torque moment to act on semiconducting or metallic nanowires. As a result, a system of nanowires which are suspended in a dielectric liquid acquire characteristics, typical of those of nematic liquid crystals, such as field-induced optical anisotropy. For freestanding nanowires prepared on a substrate, an applied electric field is shown to be capable of causing nanowire bending, which can potentially provide a means of nanoactuation. The bending of such nanowires in a high-frequency electric field is comprised of a superposition of static bending and induced mechanical oscillations, whose amplitude depends on the resonant mechanical vibration characteristics of the nanowires. Abstract

In this work, the reduction of acoustic noise in piping systems through the installation of finite flexible segments is explored. A mathematical model describing the relevant parameters is developed. To verify the model, experimental work is undertaken using rubber hoses as flexible segments and air as the contained fluid. The effect of the segment on propagating acoustical energy is studied in terms of its insertion loss. Both theoretical and experimental results indicate that, for light fluid loading, the flexibility of the segment is only important when significant axial shell resonances exist, or when one or both acoustical termination impedances at the ends of the segment are much larger or smaller than the characteristic impedance of the medium. Further, the model indicates that for heavier fluid loading (i.e., when the compressibility of the flexible section is significant in relation to the bulk compliance of the fluid), in addition to higher associated insertion losses, the finite length resonances become more pronounced, and performance is less sensitive to small variations in the termination impedances. Abstract

A series of 40 patients undergoing primary unilateral total knee arthroplasty were entered into a randomised controlled trial to assess the safety and efficacy of postoperative autologous blood salvage and reinfusion. The mean volume of autologous blood reinfused was 520 ml per patient (51% of the mean total drainage). Homologous blood transfusion was required in only 35% of patients in the study group compared with 95% of patients in the control group (P less than 0.001). The mean volume of homologous blood transfused was 0.9 units per patient in the study group compared with 2.5 units in the control group (P less than 0.001), a saving of 64%.
