Gamma-ray emission in the MeV-GeV range from explosive cosmic events is of invaluable relevance to understanding physical processes related to the formation of neutron stars and black holes. Here we report on the detection by the AGILE satellite in the MeV-GeV energy range of the remarkable long-duration gamma-ray burst GRB 221009A. The AGILE onboard detectors have good exposure to GRB 221009A during its initial crucial phases. Hard X-ray/MeV emission in the prompt phase lasted hundreds of seconds, with the brightest radiation being emitted between 200 and 300 seconds after the initial trigger. Very intense GeV gamma-ray emission is detected by AGILE in the prompt and early afterglow phase up to 10,000 seconds. Time-resolved spectral analysis shows time-variable MeV-peaked emission simultaneous with intense power-law GeV radiation that persists in the afterglow phase. The coexistence during the prompt phase of very intense MeV emission together with highly nonthermal and hardening GeV radiation is a remarkable feature of GRB 221009A. During the prompt phase, the event shows spectrally different MeV and GeV emissions that are most likely generated by physical mechanisms occurring in different locations. AGILE observations provide crucial flux and spectral gamma-ray information regarding the early phases of GRB 221009A during which emission in the TeV range was reported., Comment: Accepted for publication in ApJL on September 19, 2023. Due to be launched in late 2021, the Imaging X-Ray Polarimetry Explorer (IXPE) is a NASA Small Explorer mission designed to perform polarization measurements in the 2-8 keV band, complemented with imaging, spectroscopy and timing capabilities. At the heart of the focal plane is a set of three polarization-sensitive Gas Pixel Detectors (GPD), each based on a custom ASIC acting as a charge-collecting anode. In this paper we shall review the design, manufacturing, and test of the IXPE focal-plane detectors, with particular emphasis on the connection between the science drivers, the performance metrics and the operational aspects. We shall present a thorough characterization of the GPDs in terms of effective noise, trigger efficiency, dead time, uniformity of response, and spectral and polarimetric performance. In addition, we shall discuss in detail a number of instrumental effects that are relevant for high-level science analysis -- particularly as far as the response to unpolarized radiation and the stability in time are concerned., Comment: To be published in Astroparticle Physics

X-ray spectra of nonmagnetic cataclysmic variables (nmCVs) in the ~ 0.3$-$15 keV energy band have been described either by one or several optically thin thermal plasma components, or by cooling flow models. We tested if the spectral continuum in nmCVs could be successfully described by Comptonization of soft photons off hot electrons presented in a cloud surrounding the source [the transition layer, (TL)]. We used publicly XMM-Newton Epic-pn, Chandra HETG/ACIS and LETG/HRC, and RXTE PCA and HEXTE observations of four Dwarf Novae (U~Gem, SS~Cyg, VW~Hyi and SS~Aur) observed in the quiescence and outburst states. In total, we analyzed 18 observations, including a simultaneous 0.4$-$150 keV Chandra/RXTE spectrum of SS~Cyg in quiescence. We fitted the spectral continuum with up to two thermal Comptonization components (compTT or compTB models in XSPEC), using only one thermal plasma temperature and one optical depth. In this framework the two seed photon components are coming presumably from the innermost and outer parts of the TL (or innermost part of the disk). We obtained that the thermal Comptonization can successfully describe the spectral continuum of these nmCV in the ~ 0.4$-$150 keV energy band. Moreover, we present the first principal radiative transfer model which explains the quasi-constancy of the spectral photon index observed around 1.8, which strongly supports the Comptonization framework in nmCVs., Comment: 34 pages, 8 figures

Fast radio bursts (FRBs) are short (millisecond) radio pulses originating from enigmatic sources at extragalactic distances so far lacking a detection in other energy bands. Magnetized neutron stars (magnetars) have been considered as the sources powering the FRBs, but the connection is controversial because of differing energetics and the lack of radio and X-ray detections with similar characteristics in the two classes. We report here the detection by the AGILE satellite on April 28, 2020 of an X-ray burst in coincidence with the very bright radio burst from the Galactic magnetar SGR 1935+2154. The burst detected by AGILE in the hard X-ray band (18-60 keV) lasts about 0.5 seconds, it is spectrally cutoff above 80 keV, and implies an isotropically emitted energy ~ $10^{40}$ erg. This event is remarkable in many ways: it shows for the first time that a magnetar can produce X-ray bursts in coincidence with FRB-like radio bursts; it also suggests that FRBs associated with magnetars may emit X-ray bursts of both magnetospheric and radio-pulse types that may be discovered in nearby sources. Guided by this detection, we discuss SGR 1935+2154 in the context of FRBs, and especially focus on the class of repeating-FRBs. Based on energetics, magnetars with fields B ~ $10^{15}$ G may power the majority of repeating-FRBs. Nearby repeating-FRBs offer a unique occasion to consolidate the FRB-magnetar connection, and we present new data on the X-ray monitoring of nearby FRBs. In Mediterranean aquaculture little research has examined the interaction between rearing density and dietary composition on main key performance indicators, physiological processes and gut bacterial community. A study was undertaken, therefore to assess growth response, digestive enzyme activity, humoral immunity on skin mucus, plasma biochemistry and gut microbiota of gilthead sea bream (Sparus aurata, L. 1758) reared at high (HD) and low (LD) final stocking densities and fed high (FM30FO15, 30 % fishmeal FM, 15 % fish oil, FO) and low (FM10FO3; 10 % FM and 3 % FO) FM and FO levels. Isonitrogenous and isolipidic extruded diets were fed to triplicate fish groups (initial weight: 96.2 g) to overfeeding over 98 days. The densities tested had no major effects on overall growth and feed efficiency of sea bream reared at high or low FM and FO dietary level. Results of digestive enzyme activity indicated a comparable digestive efficiency among rearing densities and within each dietary treatment. Plasma parameters related to nutritional and physiological conditions were not affected by rearing densities under both nutritional conditions a similar observation was also achieved through the study of lysozyme, protease, antiprotease and total protein determination in skin mucus, For the first time on this species, the effect of rearing density on gut bacterial community was studied. Different response in relation to dietary treatment under HD and LD were detected. Low FM-FO diet maintained steady the biodiversity of the gut bacterial community between LD and HD conditions while fish fed high FM-FO level showed a reduced biodiversity at HD. According to the results, it seems feasible to rear gilthead sea bream at the on-growing phase at a density up to 36-44 kg m3 with low or high FM-FO diet without negatively affecting growth, feed efficiency, welfare condition and gut bacterial community.

With the advent of the nanosat/cubesat revolution, new opportunities have appeared to develop and launch small ($\sim$\ts 1000 cm$^3$), low-cost ($\sim$\ts US\$ 1M) experiments in space in very short timeframes ($\sim$ 2\ts years). In the field of high-energy astrophysics, in particular, it is a considerable challenge to design instruments with compelling science and competitive capabilities that can fit in very small satellite buses such as a cubesat platform, and operate them with very limited resources. Here we describe a hard X-ray (30--200\ts keV) experiment, LECX ("Localizador de Explos\~oes C\'osmicas de Raios X" -- Locator of X-Ray Cosmic Explosions), that is capable of detecting and localizing within a few degrees events like Gamma-Ray Bursts and other explosive phenomena in a 2U-cubesat platform, at a rate of $\sim${\bf 5 events year$^{-1}$.} In the current gravitational wave era of astronomy, a constellation or swarm of small spacecraft carrying instruments such as LECX can be a very cost-effective way to search for electromagnetic counterparts of gravitational wave events produced by the coalescence of compact objects., Comment: submitted to MNRAS

- Abstract
Whether Clonal Hematopoiesis (CH) represents a risk factor for severity of the COVID-19 disease remains a controversial issue. We report the first high- sensitivity analysis of CH in COVID-19 patients (threshold of detection at 0.5% vs 1 or 2% in previous studies). We analyzed 24 patients admitted to ICU for COVID-19 (COV-ICU) and 19 controls, including healthy subjects and asymptomatic SARS-CoV2-positive individuals. Despite the significantly higher numbers of CH mutations identified (80% mutations with <2% variant allele frequency, VAF), we did not find significant differences between COV-ICU patients and controls in the prevalence of CH or in the numbers, VAF or functional categories of the mutated genes, suggesting that CH is not overrepresented in patients with COVID-19. However, when considering potential drivers CH mutations (CH-PD), COV-ICU patients showed higher clonal complexity, in terms of both mutation numbers and VAF, and enrichment of variants reported in myeloid neoplasms. However, we did not score an impact of increased CH-PD on patient survival or clinical parameters associated with inflammation. These data suggest that COVID-19 influence the clonal composition of the peripheral blood and call for further investigations addressing the potential long-term clinical impact of CH on people experiencing severe COVID-19. We acknowledge that it will indispensable to perform further studies on larger patient cohorts in order to validate and generalize our conclusions. Moreover, we performed CH analysis at a single time point. It will be necessary to consider longitudinal approaches with long periods of follow-up in order to assess if the COVID-19 disease could have an impact on the evolution of CH and long-term consequences in patients that experienced severe COVID-19.

- Abstract
BACKGROUND: Lombardy was the epicenter of the first coronavirus disease 2019 (COVID-19) outbreak in western countries. The outbreak began in February 2020 and rapidly disseminated throughout the region. ICU beds were vastly insufficient and clinical knowledge of disease was poor at that time. Unfortunately, data on long-term mortality, morbidity, and Quality of Life are scarce and controversial. The aim of this study was to evaluate 1-year survival, Quality of Life, and functional recovery in patients with COVID-19 admitted to Intensive Care Units. METHODS: All COVID-19 patients invasively ventilated and successfully discharged from 3 important academic hospitals in Lombardy were evaluated. Evaluations were performed by qualified medical staff and monitoring over time was performed by telephone call. Functional, cognitive, and psychological outcomes were explored using validated questionnaires. Selected patients were offered a follow-up chest computed tomography (CT) scan. RESULTS: Four hundred twenty-seven patients were invasively ventilated and 268 (63%) were successfully discharged. Out of these 268 patients, 266 (99%) were alive at one year with no patient loss during follow-up. Very severe or severe dyspnea was reported by 7% of patients, while most patients (84%) did not experience dyspnea at rest. A small proportion of patients (17%) reported severe anxiety/depression. Good Quality of Life was reported by 64% of survivors. In patients complaining of dyspnea on exertion, fibrotic-like changes were observed at chest CT scans in 32/37 (86%) and 7/11 (63%) patients who underwent CT at 3 months and 1 year, respectively. CONCLUSIONS: COVID-19 patients discharged from the hospital after invasive ventilation had excellent one-year survival and good overall recovery and Quality of Life.

This molecule is administrated orally and is currently approved for the treatment of rheumatoid arthritis, atopic dermatitis, and psoriatic arthritis. Upadacitinib has been approved by the United States Food and Drug Administration for the induction and maintenance therapy of moderate-to-severe ulcerative colitis (UC) and is under investigation by the European Medicines Agency. Data from two induction and two maintenance Phase III randomized controlled trials (RCTs) proved the efficacy of upadacitinib in achieving clinical and endoscopic remission in patients with moderate-to-severe UC, regardless of previous inadequate response to other biologic therapies. The most frequently reported adverse events in the induction trials were acne, creatine phosphokinase increase, nasopharyngitis, headache, and anemia, while in the maintenance studies nasopharyngitis, elevation of creatine phosphokinase, UC exacerbation, upper respiratory tract infection, arthralgia, and anemia were reported. A limited proportion of upadacitinib-treated patients experienced adverse events of special interest, like herpes zoster infections or thromboembolic events, indicating a reliable safety profile. The LIGO-Virgo Collaboration (LVC) detected, on 2017 August 17, an exceptional gravitational-wave (GW) event temporally consistent within $\sim\,1.7 \, \rm s$ with the GRB 1708117A observed by Fermi-GBM and INTEGRAL. The event turns out to be compatible with a neutron star-neutron star (NS-NS) coalescence that subsequently produced a radio/optical/X-ray transient detected at later times. We report the main results of the observations by the AGILE satellite of the GW170817 localization region (LR) and its electromagnetic (e.m.) counterpart. At the LVC detection time $T_0$, the GW170817 LR was occulted by the Earth. The AGILE instrument collected useful data before and after the GW-GRB event because in its spinning observation mode it can scan a given source many times per hour. The earliest exposure of the GW170817 LR by the gamma-ray imaging detector (GRID) started about 935 s after $T_0$. No significant X-ray or gamma-ray emission was detected from the LR that was repeatedly exposed over timescales of minutes, hours, and days before and after GW170817, also considering Mini-calorimeter and Super-AGILE data. Our measurements are among the earliest ones obtained by space satellites on GW170817 and provide useful constraints on the precursor and delayed emission properties of the NS-NS coalescence event. We can exclude with high confidence the existence of an X-ray/gamma-ray emitting magnetar-like object with a large magnetic field of $10^{15} \, \rm G$. The LIGO/Virgo Collaboration (LVC) detected on 2017 January 4, a significant gravitational-wave (GW) event (now named GW170104). We report in this Letter the main results obtained from the analysis of hard X-ray and gamma-ray data of the AGILE mission that repeatedly observed the GW170104 localization region (LR). At the LVC detection time $T_0$ AGILE observed about 36% of the LR. The gamma-ray imaging detector did not reveal any significant emission in the energy range 50 MeV--30 GeV. Furthermore, no significant gamma-ray transients were detected in the LR that was repeatedly exposed over timescales of minutes, hours, and days. We also searched for transient emission using data near $T_0$ of the omnidirectional detector MCAL operating in the energy band 0.4--100 MeV. A refined analysis of MCAL data shows the existence of a weak event (that we call "E2") with a signal-to-noise ratio of $4.4\,\sigma$ lasting about 32 ms and occurring $0.46\,\pm\,0.05 \,\rm s$ before $T_0$. A study of the MCAL background and of the false-alarm rate of E2 leads to the determinination of a post-trial significance between $2.4\,\sigma$ and $2.7\,\sigma$ for a temporal coincidence with GW170104. We note that E2 has characteristics similar to those detected from the weak precursor of the short GRB 090510. The candidate event E2 is worth consideration for simultaneous detection by other satellites. If associated with GW170104, it shows emission in the MeV band of a short burst preceding the final coalescence by 0.46 sec and involving $\sim 10^{-7}$ of the total rest mass energy of the system., Comment: Submitted to Astrophysical Journal Letters on 2017 May 31; revised 2017 July 19; accepted 2017 July 20

At the end of March 2015 the onboard software configuration of the AGILE satellite was modified in order to disable the veto signal of the anticoincidence shield for the minicalorimeter instrument. The motivation for such a change was the understanding that the dead time induced by the anticoincidence prevented the detection of a large fraction of Terrestrial Gamma-Ray Flashes (TGFs). The configuration change was highly successful resulting in an increase of one order of magnitude in TGF detection rate. As expected, the largest fraction of the new events has short duration ($< 100 \mathrm {\mu s}$), and part of them has simultaneous association with lightning sferics detected by the World Wide Lightning Location Network (WWLLN). The new configuration provides the largest TGF detection rate surface density (TGFs/$\mathrm{km^2}$/year) to date, opening prospects for improved correlation studies with lightning and atmospheric parameters on short spatial and temporal scales along the equatorial region.

Finding robust brain substrates of mood disorders is an important target for research. The degree to which major depression (MDD) and bipolar disorder (BD) are associated with common and/or distinct patterns of volumetric changes is nevertheless unclear. Furthermore, the extant literature is heterogeneous with respect to the nature of these changes. We report a meta-analysis of voxel-based morphometry (VBM) studies in MDD and BD. We identified studies published up to January 2015 that compared grey matter in MDD (50 data sets including 4101 individuals) and BD (36 data sets including 2407 individuals) using whole-brain VBM. We used statistical maps from the studies included where available and reported peak coordinates otherwise. Group comparisons and conjunction analyses identified regions in which the disorders showed common and distinct patterns of volumetric alteration. Both disorders were associated with lower grey-matter volume relative to healthy individuals in a number of areas. Conjunction analysis showed smaller volumes in both disorders in clusters in the dorsomedial and ventromedial prefrontal cortex, including the anterior cingulate cortex and bilateral insula. Group comparisons indicated that findings of smaller grey-matter volumes relative to controls in the right dorsolateral prefrontal cortex and left hippocampus, along with cerebellar, temporal and parietal regions were more substantial in major depression. These results suggest that MDD and BD are characterised by both common and distinct patterns of grey-matter volume changes. This combination of differences and similarities has the potential to inform the development of diagnostic biomarkers for these conditions.

In this work we present the results of imaging simulations performed with the help of the GEANT4 package for the protoMIRAX hard X-ray balloon experiment. The instrumental background was simulated taking into account the various radiation components and their angular dependence, as well as a detailed mass model of the experiment. We modeled the meridian transits of the Crab Nebula and the Galatic Centre region during balloon flights in Brazil ($\sim -23^{\circ}$ of latitude and an altitude of $\sim 40 \thinspace$ km) and introduced the correspondent spectra as inputs to the imaging simulations. We present images of the Crab and of three sources in the Galactic Centre region: 1E 1740.7-2942, GRS 1758-258 and GX 1+4. The results show that the protoMIRAX experiment is capable of making spectral and timing observations of bright hard X-ray sources as well as important imaging demonstrations that will contribute to the design of the MIRAX satellite mission., Comment: 9 figures

Hard X-ray spectra of black hole binaries in the low/hard state are well modeled by thermal Comptonization of soft seed photons by a corona-type region with $kT$\thinspace$\sim 50${\thinspace}keV and optical depth around 1. Previous spectral studies of 1E{\thinspace}1740.7$-$2942, including both the soft and the hard X-ray bands, were always limited by gaps in the spectra or by a combination of observations with imaging and non-imaging instruments. In this study, we have used three rare nearly-simultaneous observations of 1E{\thinspace}1740.7$-$1942 by both XMM-Newton and INTEGRAL satellites to combine spectra from four different imaging instruments with no data gaps, and we successfully applied the Comptonization scenario to explain the broadband X-ray spectra of this source in the low/hard state. For two of the three observations, our analysis also shows that, models including Compton reflection can adequately fit the data, in agreement with previous reports. We show that the observations can also be modeled by a more detailed Comptonization scheme. Furthermore, we find the presence of an iron K-edge absorption feature in one occasion, which confirms what had been previously observed by Suzaku. Our broadband analysis of this limited sample shows a rich spectral variability in 1E{\thinspace}1740.7$-$2942 at the low/hard state, and we address the possible causes of these variations. More simultaneous soft/hard X-ray observations of this system and other black-hole binaries would be very helpful in constraining the Comptonization scenario and shedding more light on the physics of these systems., Comment: 6 pages, two figures, accepted for publication in A&A

Caffeine penetration kinetics and Lucifer yellow (LY) permeability modifications induced by these products were compared to those induced by a negative control series (saline solution, code NC) and positive control series (white Vaseline, code V) under neutral and acidic pH conditions.Results: Under neutral and acidic pH conditions, compared to the negative control, all the products tested reduced (> 80% and 85– 90%, respectively) the caffeine passage, and no significant difference was observed among the products tested. Under neutral and acidic conditions, the LY permeabilities registered with device A and device C were not different from that registered with the negative control, while an LY flux% increase was calculated after 2 hrs of treatment (21.1%) with device B under acidic conditions.Conclusion: These results confirm the ability of the products tested to interact with the oesophageal epithelium in order to adhere and create a stable protective film for at least 2 hours after their homogeneous distribution on the epithelium surface. - Abstract
Carbonate biomineralization in marine organisms normally occurs as three distinct types, known as: “controlled”, “induced” and “influenced”. Among invertebrates, reef-building polychaetes of the family Cirratulidae appear to provide the first example of “dual-phased” biomineralization as they simultaneously combine “controlled” and “induced” types in the formation processes of their aggregated tubes. Recently, Kočí et al. (2020) described the general morphology and internal framework of some bioconstructions of a Miocene cirratulid species and stressed the need of further comparative studies to better understand the nature of the calcareous matrix, tube ultrastructure and biomineralization processes. The main purpose of this work is to study the mineral structure of some Miocene (Diplochaetetes) and Recent (Dodecaceria) cirratulid bioconstructions from southern Peru through optical microscopy, epifluorescence, SEM-EDS analyses and morphometry. Results have also been statistically processed in order to highlight the possible relationships between size and chemical composition of the skeletal component with the morphology of the bioconstructions themselves, which in turn hints to environmental parameters. Three linear correlations, considering as variables morphological parameters (i.e., tube diameters) and the weight concentration of the main elements (Ca and Sr), were tested. Both fossils and recent specimens show similar correlations among composition and morphometric parameters, suggesting that the growth/evolution of these bioconstructions is presumably unaffected by environmental chemo-physical conditions. The primary ultrastructure of the tube walls points to controlled biomineralization, while the “intertube” areas of the bioconstruction, filled by a calcareous matrix frequently agglutinating extraneous sand grains, would be the product of an induced biomineralization. - Abstract
Enhanced perioperative care protocols become the standard of care in elective surgery with a significant improvement in patients’ outcome. The key element of the enhanced perioperative care protocol is the multimodal and interdisciplinary approach targeted to the patient, focused on a holistic approach to reduce surgical stress and improve perioperative recovery. Enhanced perioperative care in emergency general surgery is still a debated topic with little evidence available. The present position paper illustrates the existing evidence about perioperative care in emergency surgery patients with a focus on each perioperative intervention in the preoperative, intraoperative and postoperative phase. For each item was proposed and approved a statement by the WSES collaborative group.

- Abstract
Background & Aims: Split liver transplant(ation) (SLT) is still considered a challenging procedure that is by no means widely accepted. We aimed to present data on 25-year trends in SLT in Italy, and to investigate if, and to what extent, outcomes have improved nationwide during this time. Methods: The study included all consecutive SLTs performed from May 1993 to December 2019, divided into three consecutive periods: 1993–2005, 2006–2014, and 2015–2019, which match changes in national allocation policies. Primary outcomes were patient and graft survival, and the relative impact of each study period. Results: SLT accounted for 8.9% of all liver transplants performed in Italy. A total of 1,715 in situ split liver grafts were included in the analysis: 868 left lateral segments (LLSs) and 847 extended right grafts (ERGs). A significant improvement in patient and graft survival (p <0.001) was observed with ERGs over the three periods. Predictors of graft survival were cold ischaemia time (CIT) <6 h (p = 0.009), UNOS status 2b (p <0.001), UNOS status 3 (p = 0.009), and transplant centre volumes: 25–50 cases vs. <25 cases (p = 0.003). Patient survival was significantly higher with LLS grafts in period 2 vs. period 1 (p = 0.008). No significant improvement in graft survival was seen over the three periods, where predictors of graft survival were CIT <6 h (p = 0.007), CIT <6 h vs. ≥10 h (p = 0.019), UNOS status 2b (p = 0.038), and UNOS status 3 (p = 0.009). Retransplantation was a risk factor in split liver graft recipients, with significantly worse graft and patient survival for both types of graft (p <0.001). Conclusions: Our analysis showed Italian SLT outcomes to have improved over the last 25 years. These results could help to dispel reservations regarding the use of this procedure. - Abstract
Laparotomy incisions provide easy and rapid access to the peritoneal cavity in case of emergency surgery. Incisional hernia (IH) is a late manifestation of the failure of abdominal wall closure and represents frequent complication of any abdominal incision: IHs can cause pain and discomfort to the patients but also clinical serious sequelae like bowel obstruction, incarceration, strangulation, and necessity of reoperation. Previous guidelines and indications in the literature consider elective settings and evidence about laparotomy closure in emergency settings is lacking. This paper aims to present the World Society of Emergency Surgery (WSES) project called ECLAPTE (Effective Closure of LAParoTomy in Emergency): the final manuscript includes guidelines on the closure of emergency laparotomy.

- Abstract
Many countries are facing an aging population. As people live longer, surgeons face the prospect of operating on increasingly older patients. Traditional teaching is that with older age, these patients face an increased risk of mortality and morbidity, even to a level deemed too prohibitive for surgery. However, this is not always true. An active 90-year-old patient can be much fitter than an overweight, sedentary 65-year-old patient with comorbidities. Recent literature shows that frailty—an age-related cumulative decline in multiple physiological systems, is therefore a better predictor of mortality and morbidity than chronological age alone. Despite recognition of frailty as an important tool in identifying vulnerable surgical patients, many surgeons still shun objective tools. The aim of this position paper was to perform a review of the existing literature and to provide recommendations on emergency laparotomy and in frail patients. This position paper was reviewed by an international expert panel composed of 37 experts who were asked to critically revise the manuscript and position statements. The position paper was conducted according to the WSES methodology. We shall present the derived statements upon which a consensus was reached, specifying the quality of the supporting evidence and suggesting future research directions.

- Abstract
Sigmoid volvulus is a common surgical emergency, especially in elderly patients. Patients can present with a wide range of clinical states: from asymptomatic, to frank peritonitis secondary to colonic perforation. These patients generally need urgent treatment, be it endoscopic decompression of the colon or an upfront colectomy. The World Society of Emergency Surgery united a worldwide group of international experts to review the current evidence and propose a consensus guidelines on the management of sigmoid volvulus.

- Abstract
Background and objectives: Depression is highly prevalent in older adults, especially in those with dementia. Trazodone, an antidepressant, has shown to be effective in older patients with moderate anxiolytic and hypnotic activity; and a common off-label use is rising for managing behavioral and psychological symptoms of dementia (BPSD). The aim of the study is to comparatively assess the clinical profiles of older patients treated with trazodone or other antidepressants. Methods: This cross-sectional study involved adults aged ≥ 60 years at risk of or affected with COVID-19 enrolled in the GeroCovid Observational study from acute wards, geriatric and dementia-specific outpatient clinics, as well as long-term care facilities (LTCF). Participants were grouped according to the use of trazodone, other antidepressants, or no antidepressant use. Results: Of the 3396 study participants (mean age 80.6 ± 9.1 years; 57.1% females), 10.8% used trazodone and 8.5% others antidepressants. Individuals treated with trazodone were older, more functionally dependent, and had a higher prevalence of dementia and BPSD than those using other antidepressants or no antidepressant use. Logistic regression analyses found that the presence of BPSD was associated with trazodone use (odds ratio (OR) 28.4, 95% confidence interval (CI) 18-44.7 for the outcome trazodone vs no antidepressants use, among participants without depression; OR 2.17, 95% CI 1.05-4.49 for the outcome trazodone vs no antidepressants use, among participants with depression). A cluster analysis of trazodone use identified three clusters: cluster 1 included mainly women, living at home with assistance, multimorbidity, dementia, BPSD, and depression; cluster 2 included mainly institutionalized women, with disabilities, depression, and dementia; cluster 3 included mostly men, often living at home unassisted, with better mobility performance, fewer chronic diseases, dementia, BPSD, and depression. - Abstract
Background: Artificial intelligence (AI) is gaining traction in medicine and surgery. AI-based applications can offer tools to examine high-volume data to inform predictive analytics that supports complex decision-making processes. Time-sensitive trauma and emergency contexts are often challenging. The study aims to investigate trauma and emergency surgeons’ knowledge and perception of using AI-based tools in clinical decision-making processes. Methods: An online survey grounded on literature regarding AI-enabled surgical decision-making aids was created by a multidisciplinary committee and endorsed by the World Society of Emergency Surgery (WSES). The survey was advertised to 917 WSES members through the society’s website and Twitter profile. Results: 650 surgeons from 71 countries in five continents participated in the survey. Results depict the presence of technology enthusiasts and skeptics and surgeons’ preference toward more classical decision-making aids like clinical guidelines, traditional training, and the support of their multidisciplinary colleagues. A lack of knowledge about several AI-related aspects emerges and is associated with mistrust. Discussion: The trauma and emergency surgical community is divided into those who firmly believe in the potential of AI and those who do not understand or trust AI-enabled surgical decision-making aids. Academic societies and surgical training programs should promote a foundational, working knowledge of clinical AI.

- Abstract
Background: Shared decision-making (SDM) between clinicians and patients is one of the pillars of the modern patient-centric philosophy of care. This study aims to explore SDM in the discipline of trauma and emergency surgery, investigating its interpretation as well as the barriers and facilitators for its implementation among surgeons. Methods: Grounding on the literature on the topics of the understanding, barriers, and facilitators of SDM in trauma and emergency surgery, a survey was created by a multidisciplinary committee and endorsed by the World Society of Emergency Surgery (WSES). The survey was sent to all 917 WSES members, advertised through the society’s website, and shared on the society’s Twitter profile. Results: A total of 650 trauma and emergency surgeons from 71 countries in five continents participated in the initiative. Less than half of the surgeons understood SDM, and 30% still saw the value in exclusively engaging multidisciplinary provider teams without involving the patient. Several barriers to effectively partnering with the patient in the decision-making process were identified, such as the lack of time and the need to concentrate on making medical teams work smoothly. Discussion: Our investigation underlines how only a minority of trauma and emergency surgeons understand SDM, and perhaps, the value of SDM is not fully accepted in trauma and emergency situations. The inclusion of SDM practices in clinical guidelines may represent the most feasible and advocated solutions.

We have analyzed a long-term database for Sco X-1 obtained with the telescope IBIS onboard the INTEGRAL satellite in order to study the hard X-ray behavior of Sco X-1 from 20 up to 200 keV. Besides the data used for producing of the INTEGRAL catalog of sources, this is the longest (412 ks) database of IBIS on Sco X-1 up to date. The production of hard X-ray tails in low-mass X-ray binaries is still a matter of debate. Since most of the fits to the high-energy part of the spectra are done with powerlaw models, the physical mechanism for the hard X-ray tail production is unclear. The purpose of this study is to better constrain those possible mechanisms. Our main result shows a strong correlation between the fluxes in the thermal and nonthermal part of Sco X-1 spectra. We thus suggest that Comptonization of lower energy photons is the mechanism for producing hard X-ray tails in Sco X-1., Comment: 4 pages, 3 figures, 2 tables; officially accepted for publication (as a Letter) by A&A in 2013 January 22

Background: Delirium is thought to be common across various settings of care; however, still little research has been conducted in rehabilitation. Aim: We investigated the prevalence of delirium, its features and motor subtypes in older patients admitted to rehabilitation facilities during the three editions of the “Delirium Day project”. Methods: We conducted a cross-sectional study in which 1237 older patients (age ≥ 65 years old) admitted to 50 Italian rehabilitation wards during the three editions of the “Delirium Day project” (2015 to 2017) were included. Delirium was evaluated through the 4AT and its motor subtype with the Delirium Motor Subtype Scale. Results: Delirium was detected in 226 patients (18%), and the most recurrent motor subtype was mixed (37%), followed by hypoactive (26%), hyperactive (21%) and non-motor one (16%). In a multivariate Poisson regression model with robust variance, factors associated with delirium were: disability in basic (PR 1.48, 95%CI: 1.17–1.9, p value 0.001) and instrumental activities of daily living (PR 1.58, 95%CI: 1.08–2.32, p value 0.018), dementia (PR 2.10, 95%CI: 1.62–2.73, p value < 0.0001), typical antipsychotics (PR 1.47, 95%CI: 1.10–1.95, p value 0.008), antidepressants other than selective serotonin reuptake inhibitors (PR 1.3, 95%CI: 1.02–1.66, p value 0.035), and physical restraints (PR 2.37, 95%CI: 1.68–3.36, p value < 0.0001). Conclusion: This multicenter study reports that 2 out 10 patients admitted to rehabilitations had delirium on the index day. Mixed delirium was the most prevalent subtype. Delirium was associated with unmodifiable (dementia, disability) and modifiable (physical restraints, medications) factors. Identification of these factors should prompt specific interventions aimed to prevent or mitigate delirium.
