Purpose: The coronavirus disease 2019 (COVID-19) pandemic forced hospitals to adopt tighter restrictions, the most impacting is no access to visitors. Disorder of consciousness (DOC) due to severe acquired brain injury is a condition needing neurorehabilitation and the role of relatives is essential, hence besides physical "disconnection" digital "re-connection" is crucial. We aimed to assess whether digital communication benefits in patients with DOC, considering the sensorial and emotional deprivation due to the COVID-19 emergency lock-down.Methods: For eleven consecutive patients with DOC admitted to our Intensive Neurorehabilitation Care (mean age: 45; females: 9), two observers registered neurobehavioral changes during a video-calls with their relatives. Heart-rate variability was measured before and during the calls. The video-call was performed by using two displays of different sizes: tablet (T-video-call) and large screen (LS-Video-call).Results: The video-calls impacted on the patients' vigilance and in the relationship with relatives. Moreover, positively impacted on their relatives. The current results showed significant greater impact on patients during the LS-video-call than when they are exposed to T-video-call.Conclusions: During the COVID-19 pandemic, besides the physical disconnection to stop the contagion spread, a "digital re-connection" is needed for all and especially for fragile population groups as patients with DOC.

The advent of the SARS-CoV-2 pandemic has resulted in an increase in sedentary behavior, with consequences on cardiopulmonary capacity, especially in the elderly population. Prehabilitation is a strategy usually used before a surgical procedure to improve functional capacity; however, it can be used for non-surgical patients and not in the acute phase of disease. The purpose of this study is to evaluate the effectiveness of a prehabilitation program, using telerehabilitation, in frail elderly patients with chronic heart failure.This is a randomized, controlled, single-blind study. Fifteen patients with chronic heart failure were randomized into three groups: two active groups (telerehabilitation and in-person) and the control group. Patients in the active groups underwent a rehabilitation program divided into two 4-week periods, for 45-60 minutes per day, 2 days per week.In the Study Group, the quality of life significantly improved (EQoL-5D), and between the two groups a statistically significant difference in the motor dimension of SF-36 was identified.The telerehabilitation prehabilitation program for patients with chronic heart failure was confirmed to be effective and not inferior to a prehabilitation program performed in-person, avoiding the worsening of some domains of quality of life and motor performance, and leading to the improvement of others.

Prehabilitation, intended as a multidisciplinary approach where physical training is combined with educational and counselling training, in cardiology could optimizing care, and has been shown to be able to reduce morbidity and mortality in several diseases. The present study aims to assess the effectiveness of a prehabilitation program in elderly patients (over 65) with chronic heart failure and to evaluate functional and quality indices of life.This is randomized, single blind controlled trial. Fourteen older adult patients diagnosed with chronic heart failure were enrolled. Patients were randomly assigned into the study or the control group. Patients in the study group underwent physical training organized into 10 twice-weekly meetings, nutritional and lifestyle counseling.In the Study Group, the quality of life improved significantly (EQoL-5D), and between the two groups there is a statistically significant difference in the motor dimension of SF-36.Because of our preliminary results, prehabilitation program should be included among the management strategies of in elderly patients with chronic heart failure to better manage their disease and to improve their Quality of Life.

Among the tools useful for the management of neuropathic pain, ultrasound presents several advantages, shown by the literature. We assessed the scientific production about neuropathic pain and ultrasound from different points of view: general topics, journal categories, geographical origin and lexical analysis. We searched papers on PubMed using the Medical Subject Headings “neuropathic pain” AND “ultrasound”. We collected data about the journals where the papers were published, the country of the affiliation of the first author. For the lexical analysis, we evaluated the presence of selected words in the papers, and we built a graph representing the connections among words and papers. The papers were focused on the use of ultrasound as a diagnostic tool and guide for the therapy, assessing its application in different diseases such as Morton’s neuroma and piriformis syndrome. The most represented journal category was anesthesia while the most common country the United States of America. The lexical analysis confirmed the importance of ultrasound for diagnosis of specific disease and treatment of pain. The described approaches provide a multiperspective evaluation of the literature and may support the interpretation of the information contained by the papers.

Patients with Parkinson's disease (PD) are at a higher risk of hospitalization and recurrent hospitalizations, with consequent complications. Polypharmacy is associated with several adverse outcomes, including hospitalization, increased length of hospital stay, and mortality. The aim of this study was to evaluate among patients with PD the association between the number of medications and incident hospitalizations.We analysed the data of 165 patients with Parkinson's disease attending a geriatric Day Hospital who were enrolled in a cohort study and followed for a median of two years.Over the follow-up, 46 participants (46%) were hospitalized at least one time; multiple admissions were observed in 12 subjects (7%). The median number of agents was 5 (4-7). In Cox regression, the number of drugs was associated with increased hospitalization rates (HR=1.23; 95% CI=1.06-1.43), also after excluding non-neurological medications (HR=1.18; 95% CI=1.01-1.38). Using Poisson regression, polypharmacy (i.e., use of5 drugs) predicted the number of repeated hospitalizations (IRR=2.62; 95% CI=1.28-5.36; p=.008).Among patients with PD, the number of daily medications is associated with increased risk of hospitalization; an increasing number of drugs is associated with increasing number of hospitalizations.

Computed gait analysis is a valid tool able to assess human walking. The joint angle variation during the cycle consists in a function. The derivation process, producing angular velocity and acceleration, may reveal and quantify the small angle variations. This approach may support physician’s interpretation of the joint movements. We would like to propose a calculation, combining the data from angular velocity and angular acceleration, to obtain a graph showing some features of joint behavior. We compared the results among a healthy subject, a coxarthrosis patient and a stroke patient. The built graph clearly shows the direction changes of the joint movements and the differences between the two sides and quantifies the acceleration/deceleration of a joint. The proposed approach is a mathematical artifice, which amplifies the information contained in the usual angle graphs and possibly opens the doors to more specific rehabilitation approaches.

Abdominal acupuncture (AA) reduces laser-evoked potentials (LEP) amplitude and laser pain perception in healthy subjects (Pazzaglia et al., 2014). The aim of the current study was to investigate the site of AA analgesic effect. We recorded LEPs in 6 healthy volunteers by using 32 EEG scalp electrodes. The experimental protocol included 3 times: (1) baseline, in which LEPs evoked to stimulation of the bilateral dorsal wrist and right foot were recorded before acupuncture; (2) acupuncture, in which LEPs were recorded during AA performed in the abdominal area corresponding to right wrist; (3) rest, in which LEPs were recorded 15 min after the needle removal. Compared to baseline, N2/P2 LEP amplitude evoked by stimulation of both wrists was reduced in the acupuncture and rest times while LEP amplitude evoked by foot stimulation was not modified. Our results suggest that the AA analgesic effect occurs at spinal level.

Bifid median nerve is an anatomic variation that occurs in about 18% of patients with symptoms suggestive of carpal tunnel syndrome and in about 15% of symptom-free subjects. Reversed palmaris longus is a rare anatomic muscular variation. The simultaneous presence of a bifid median nerve and a reversed palmaris longus has been very rarely described, usually during surgical exploration or in cadavers. We present two cases where ultrasound showed the presence of both abnormalities, allowing a correct diagnosis and influencing the treatment plan.

Ulnar neuropathy at elbow (UNE) is often related to postures and work-hobby activities. The use of mobile-phones has drastically increased in the last decades leading to prolonged phone posture (PPP) with flexed elbow. We aimed to assess the effect of PPP in patients with symptoms of UNE and in symptom-free subjects. Patients with pure sensory symptoms of UNE and negative neurophysiological tests (min-UNE) and symptoms-free subjects were enrolled. We evaluated ulnar motor nerve conduction velocity across elbow at baseline and after 6, 9, 12, 15, 18 min of PPP. Thirty-eight symptom-free subjects and thirty-eight patients were enrolled and 121 ulnar nerves were studied. Conduction velocity of ulnar nerve across the elbow significantly changed over PPP time both in control group and, at a greater extent, in patients with min-UNE. Conduction velocity during PPP showed different evolution between the two groups. The changes became significantly different after 6 min of PPP and progressively increased, with the greatest difference at 15 min. In conclusion, PPP causes a modification of nerve function, expressed by slowing of motor nerve conduction velocity, which is greater in patients with min-UNE although it also occurs in symptom-free subjects. Excessive PPP should be avoided in patients with UNE-symptoms.

We studied patients with neuropathy and anti-myelin-associated glycoprotein antibodies nerve ultrasound (US). 28 patients with anti-MAG neuropathy underwent nerve US. Echotexture, nerve cross sectional area (CSA), intra-nerve and inter-nerve CSA variability were assessed. Patients with IgM paraproteinemic neuropathy without anti-MAG antibodies and with chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) associated with IgM paraprotein were studied as controls. US findings were correlated with disease duration and clinical severity (INCAT disability score). 26/28 patients had increased CSA (23 at atleast one nerve outside entrapment sites). Intra-nerve CSA variability was abnormal in 21/28 patients (in 14 for increased nerve CSA outside entrapment sites). Inter-nerve CSA variability was abnormal in 16 patients (in half of whom for CSA increase out of entrapment sites). No correlation was found between US findings and INCAT disability score or disease duration. Patients with IgM paraproteinemic neuropathy without anti-MAG antibodies had similar US abnormalities. Instead 4/5 patients with CIDP and IgM paraprotein presented significant and diffuse nerve CSA enlargement. The US findings in anti-MAG antibody neuropathy, different from those of CIDP, might express the diverse pathogenic mechanisms and contribute to diagnosis in atypical cases.

Immune-mediated neuropathies include clinically heterogeneous disorders such as Guillain–Barre syndrome (GBS) and its variants, chronic inflammatory demyelinating polyradiculoneuropathy (CIDP), multifocal motor neuropathy (MMN), neuropathy with monoclonal gammopathy and others. High-resolution ultrasound (US) is a helpful technique for the evaluation of peripheral nerves. In previous studies on immune-mediated neuropathies, nerve US showed diffuse enlargement of cross sectional area (CSA) of peripheral nerves trunks and cervical roots and morphological alterations at conduction block site. Data are however heterogeneous, correlations with clinical history or disease severity are lacking and there are no longitudinal studies in literature. The objective of the protocol is to prospectively evaluate, through a multicenter longitudinal study, nerve US findings in patients with newly diagnosed immune-mediated neuropathies and evaluate the relationship with clinical and neurophysiological findings and changes over time (natural history) or in response to therapy. The protocol includes clinical assessment, neurophysiological examination, US examination of median, ulnar, radial, fibular, tibial, sural nerves (and brachial plexus in extended protocol) with evaluation of maximal/minimal CSA for each nerve, inter- and intra-nerve variability, US classification depending on echogenicity and fascicles enlargement. US and neurophysiological follow-up timing examinations follow different schedules in acute and chronic immune-mediated disease. We present preliminary results and feasibility of the protocol.

High-resolution ultrasound (US) is a helpful technique for the evaluation of peripheral nerves and literature data show that US may crucially influence diagnosis and clinical care in entrapment in nerve tumours. We report the case of a 48 year-old woman complaining of pain and electric shocks radiated in sciatic course and trigger point in the gluteal region. It was suspected radiculopathy due to disk herniation. MRI showed mild L5–S1 bulging. Due to the trigger, US of sciatic nerve was suggested, nevertheless the trigger was in a region where usually sciatic nerve is not depictable (commonly accepted that is not depictable proximally to plica glutea). The clinical examination and EMG were normal. We performed an ultrasound examination to study sciatic nerve from its distal formation by the union of peroneal and tibial nerves and along its proximal tract. The right sciatic nerve was visualizable along all its tract, even proximally to plica glutea. At the middle-third of gluteus the nerve showed a round-oval shape formation with homogeneous hypoechoic US image. After accurate evaluation the formation seemed on the nerve edge and nerve fibers seemed dislocated laterally, moreover the proximal edge was clearly visualizable. The morphology suggested a probable neurinoma. MRI confirmed dislocation of fascicles and surgical excision was planned. This case report shows the importance of trying ultrasound sciatic nerve examination even proximal to plica glutea.

Our work shows the association of neurophysiology and ultrasound in the assessment of peripheral nerve tumors. In these cases, neurophysiologic examination presents an important role to define the possible lack of function of the nerve. Ultrasonography allows us to visualize the peripheral nerves and underline the morphologic changes in nerve structure. We present a literature revision about ultrasound in peripheral nerve tumor study and a collection of 10 cases where the combined use of clinic, neurophysiology and ultrasound guided the diagnostic process of nerve tumors. All the patients presented a clinic pattern of nerve involvement and everyone was evaluated with neurophysiology which confirmed a mononeuropathy, but without the possibility of a sure diagnosis. The following ultrasound evaluation allowed to visualize a focal lesion, compatible with a nerve tumor. These cases show that neurophysiology and ultrasound together contribute in diagnosis of peripheral nerve tumors. Ultrasonography is not able to certainly define the type of tumor, but it gives suggestions about discrimination between schwannoma and neurofibroma. Nerve ultrasound is useful in the assessment of peripheral nerve tumors.

Ultrasound shows its usefulness in traumatic nerve injures. Methods: We present 5 patients with a history of knee dislocation associated with peroneal nerve damage, evaluated by clinical, neurophysiologic and ultrasonographic (US) examination. All the patients were examined in follow-up. At the first evaluation, the patients presented a severe clinic and neurophysiologic damage of peroneal nerve. US showed an increase of the cross sectional area (CSA) of the involved peroneal nerve, between popliteal fossa and fibular head. Results: Among these, 4 subjects presented a CSA four-seven times larger than normal, while one subject had the damaged nerve CSA only double compared to the other side. The first 4 patients did not present improvement in clinical, neurophysiologic and ultrasonographic follow-up. The latter one showed a general improvement in the later evaluations. Conclusions: This observation let us consider that larger is the CSA of the involved peroneal nerve and worse is the prognosis for the patient. Associating neurophysiology to US evaluation, we can obtain a guide for diagnosis, treatment and rehabilitation. In fact this combined evaluation gives data about the specific details and the changes over the time of the condition. This allow us to perform the best management for every individual case. P606 Ultrasonic evaluation as a method for determining diagnosis underlying clinical symptoms of carpal tunnel syndrome N. Wolfram, U. van Deurs, M. Lauritzen Glostrup Hospital, Department of Clinical Neurophysiology, Glostrup, Denmark Background: High resolution ultrasound (HRUS) was used to differentiate carpal tunnel syndrome (CTS) from other pathologies in the media nerve. Materials and methods: Forty-one patients, referred for diagnosis of CTS, were examined with HRUS supplemental to electrodiagnostic (EDX) evaluation when the medical history gave suspicion of polyneuropathy, cervical root affection on MRI scans, trauma or EDX changes not typical for CTS. Results: Twelve patients showed CTS in both EDX (29%) and HRUS, two patients had a bifid median nerve, one was supplemental investigated for HNPP. Seventeen patients had normal EDX (42%). Eight had normal findings by HRUS, five had CTS in HRUS, whereas four patients showed other pathologies including neurovascular contacts, bifid median nerve, partial nerve compression under the flexor retinaculum, palmaris muscle contacts and arthritis. Twelve patients showed atypical changes in EDX (29%). In this group, eight patients showed CTS in HRUS, four patients showed other pathologies including fibrolipomatous harmatoma, intraneural venous congestion, partial traumatic neuroma and flexor muscle compression under the flexor retinaculum. P607 Intraneural collateral circulation in the median nerve after radiocephalic fistula N. Wolfram, U. van Deurs, M. Lauritzen Glostrup Hospital, Clinical Neurophysiology, Glostrup, Denmark Introduction: Carpaltunnel Syndrome (CTS) is a known complication to chronic renal failure and radiocephalic fistula for hemodialysis, but the cause of median nerve affection is unknown. Case report: A 61 year old male with chronic renal failure, hemodialysis and radiocephalic fistula in the left forearm was referred for CTS, because of pain in the left thumb, progressive in flexion of the wrist and parestesia in finger 1 + 2. Electrodiagnostic (EDX) evaluation showed normal distal motor latency and motor velocity in the forearm, but reduced motor amplitude from the elbow. Normal sensory velocity from finger 2 and palm to wrist, with reduced sensory amplitudes. High resolution ultrasound (HRUS) (Esaote MyLAb Twice, 6-18 MHz) revealed intraneural arterial blood flow, presumably located in the intraneural venous plexus of the left median nerve at the level of the radiocephalic fistula, as well as hypo-eccogenic changes in the nerve fascicles. Discussion: Clinically, CTS may be mimicked by proximal nerve damage followed by intraneural collateral circulation in patients with radiocephalic fistula. Shin2 1Hanyang University, Seoul, Republic of Korea; 2National Rehabilitation Center, Rehabilitation, Seoul, Republic of Korea Question: Tremor is a common movement disorder, as which a varied neurological disorders can be presented and pharmacological treatment was firstly tried. Botulinum toxin type A (BoNT-A) has been adopted as another treatment method, and recently ultrasonography-guided injection has been emphasized for accuracy. We hypothesized ultrasonography could takes another role as an objective indicator for tremor. Methods: We report two case studies in which tremor were the chief complaint. Case 1. A 29-year-old male patient with a postural and rest tremor of right hand consistent with Holmes tremor secondary to left pontine hemorrhage visited. His tremor gradually spread to wrist and elbow with the characteristics of 1.4-2 Hz, irregular flexion-extension oscillation being present at rest. Ultrasonography-guided BoNT-A injections were administered at extensor pollicis longus (40 U) muscles, and flexor digitorum superficialis 2nd, 3rd and 4th digits (20 U, each), which showed plainest contraction in real time ultrasonography. Ultrasonography revealed the frequency of tremor decreased from 1.4-2 Hz (baseline) to 0-0.5 Hz (2 weeks after injection), as calculated by each muscle contraction. Case 2. A 30-year-old female patient with bilateral tremor secondary to hypoxic brain damage visited. Her muscle contraction was 0.2-0.4 Hz at her left extensor carpi radialis, extensor policis longus, flexor carpi ulnaris muscles with the help of ultrasonography. BoNT-A injections were administered at extensor carpi radialis (20 U), extensor policis longus (15 U), and flexor carpi ulnaris (20 U) muscles with ultrasonography. The frequency of muscle contraction decreased to 0.1-0.2 Hz at 2 weeks after injection. Conclusion: Ultrasonography has an advantage of objective evaluation of tremor as well as selecting the target muscles. A patient instead had a CSA of the damaged nerve only double compared to the other side. This subject showed a general improvement in the follow-up. On the basis of these preliminary findings, we may hypothesize that larger is the CSA of the damaged peroneal nerve worse the prognosis. Furthermore, this study suggests that US evaluation can aim the diagnostic process providing data about morphology of the involved nerve, exact site of the injury, conditions of surrounding structures and changes over the time.

At the point where it crosses the inguinal ligament, the lateral femoral cutaneous nerve (LFCN) can be entrapped, causing a clinical syndrome (Meralgia Paresthetica) consisting of numbness and sensory disturbances in the territory of nerve distribution. The causes of MP include conditions associated with obesity, pregnancy and orthopedic procedures. The diagnosis of MP is based on clinical history, physical examination, neurophysiological findings and exclusion of other causes of pain or sensory disturbances. Routine neurophysiological tests are not commonly accepted as sensitive although several methods have been developed. Sonographic examination (US) of the LFCN could be useful as a screening test in the diagnosis. The nerve is accessible to US from a few centimeters before it exits the pelvis down to the third proximal part of the thigh. The main US signs of LCFN neuropathy include detection of a fusiform nerve swelling and nerve flattening under or within the ligament. The true sensitivity are not assessed because of absence of gold standard diagnostic test. We present 8 cases of clinical MP confirmed by US.

Multifocal motor neuropathy (MMN) is an immune-mediated disorder characterized by slowly progressive asymmetric muscle weakness and electrophysiological findings of motor conduction block. The immunological hypothesis for pathogenesis of MMN is supported by the association, in 30–80% of cases, with high title Anti-GM1 antibodies and positive response to intravenous immunoglobulin (IVIg) therapy. Ultrasound (US) is recently gaining a crucial role in the assessment of immune-related neuropathies. We report the clinical, electrophysiological and US follow-up of five cases of patients affected with MMN treated with IVIg. In two patients (male 44 years old, female 37 years old) who started IVIg treatment soon after onset, US demonstrated complete or partial normalization. Conversely, in the other three patients (two males 49 and 70 years old, female 48 years old) who started IVIg respectively four months, twenty years and 1 year after onset, we observed an incomplete clinical, electrophysiological and ultrasound IVIg response.

Immune-mediated neuropathies include Guillain–Barre syndrome (GBS) and its variants, chronic inflammatory demyelinating polyradiculoneuropathy (CIDP), multifocal motor neuropathy (MMN), paraproteinemic neuropathies. High-resolution ultrasound (US) is a helpful technique for the evaluation of peripheral nerves. In previous studies on immune-mediated neuropathies, nerve US showed diffuse enlargement of cross sectional area (CSA) of peripheral nerves trunks and cervical roots and morphological alterations at conduction block site. Data are however heterogeneous, correlations with clinical history or disease severity are lacking and there are no longitudinal studies in literature. The objective is to prospectively evaluate, through a multicenter longitudinal study, US findings in newly diagnosed patients with immune-mediated neuropathies and changes during time and relationship with clinical and neurophysiological findings. The proposal of the protocol is to enroll patients with immune-mediated neuropathies. We will perform clinical assessment, neurophysiological examination; US examination will study median, ulnar, radial, fibular, tibial, sural nerves (and brachial plexus in extended protocol). Maximal/minimal CSA for each nerve, inter- and intra-nerve variability, classification depending on echogenicity and fascicles enlargement will be considered. The protocol and its timing of follow-up evaluations, depending on acute/subacute or chronic disease, will be discussed so that the final version will be based on consensus among expert committees or clinical experience.

Ultrasound (US) can aim clinical and neurophysiologic assessment of the nerve lesions. After a case report we previously reported (Liotta et al., 2010), we present further three cases in which US allowed to depict concomitant nerve lesions far from the trauma sites. The first one is a man with humerus fracture immediately followed by a radial muscles plegia.Three months after the trauma, he presented no clinical/neurophysiologic recovery of fingers and wrist extensor muscles. US showed an enlargement of radial nerve in the fracture site and a contemporary marked enlargement of posterior interosseous nerve close to Frohse’s arcade. The other cases concern two patients with trauma at the level of clavicle and following plexopathy. Clinical/neurophysiologic examination in both cases showed a discrepancy between biceps brachii muscle (complete denervation) and the all other muscles (mild involvement). US showed a marked enlargement of the muscolocutaneous nerve distally to axilla. The nerve damage in a point far from the fracture site is probably due to the nerve traction caused by the trauma. These findings suggest a new mechanism of damage, that must be hypothesized in atypical muscles involvement after trauma, and the role of US in these cases for diagnosis, prognosis and treatment.

A 50-year old man received a cut injury to the left forearm with involvement of median and ulnar nerves and consequent hand motor and sensory deficits. He underwent surgical revision of the wound, but 6 months later the patient did not present recovery. A following neurophysiological examination showed sporadic fibrillation potentials with reduced voluntary recruitment in the first dorsal interosseus muscle and complete denervation in muscles innervated by distal median nerve, indicating an axonal ulnar neuropathy and a possible median nerve axonotmesis or neurotmesis. Ultrasound examination showed: median nerve neurotmesis and amputation neuroma with a gap of about 7–8 cm between proximal and distal stumps; a slight focal increase in cross-sectional area of the ulnar nerve close to the scar. The patient newly underwent surgery. A reconstruction of the left median nerve by a sural nerve graft and neurolysis of the ulnar nerve were performed. In the following days the patient reported an improvement in the cold and hot sensitivity on the ulnar nerve distribution in the left hand. In conclusion ultrasound nerve examination played a key role in decision-making.

- Abstract
The invention of RNA-guided DNA cutting systems has revolutionized biotechnology. More recently, RNA-guided RNA cutting by Cas13d entered the scene as a highly promising alternative to RNA interference to engineer cellular transcriptomes for biotechnological and therapeutic purposes. Unfortunately, "collateral damage" by indiscriminate off-target cutting tampered enthusiasm for these systems. Yet, how collateral activity, or even RNA target reduction depends on Cas13d and guide RNA abundance has remained unclear due to the lack of expression-tuning studies to address this question. Here we use precise expression-tuning gene circuits to show that both nonspecific and specific, on-target RNA reduction depend on Cas13d and guide RNA levels, and that nonspecific RNA cutting from trans cleavage might contribute to on-target RNA reduction. Using RNA-level control techniques, we develop new Multi-Level Optimized Negative-Autoregulated Cas13d and crRNA Hybrid (MONARCH) gene circuits that achieve a high dynamic range with low basal on-target RNA reduction while minimizing collateral activity in human kidney cells and green monkey cells most frequently used in human virology. MONARCH should bring RNA-guided RNA cutting systems to the forefront, as easily applicable, programmable tools for transcriptome engineering in biotechnological and medical applications.

Neuroscientists routinely use reverse inference (RI) to draw conclusions about cognitive processes from neural activation data. However, despite its widespread use, the methodological status of RI is a matter of ongoing controversy, with some critics arguing that it should be rejected wholesale on the grounds that it instantiates a deductively invalid argument form. In response to these critiques, some have proposed to conceive of RI as a form of abduction or inference to the best explanation (IBE). We side with this response but at the same time argue that a defense of RI requires more than identifying it as a form of IBE. In this paper, we give an analysis of what determines the quality of an RI conceived as an IBE and on that basis argue that whether an RI is warranted needs to be decided on a case-by-case basis. 35. Evaluation of Rehabilitation Outcomes in Patients with Chronic Neurological Health Conditions Using a Machine Learning Approach.

- Abstract
Background: Over one billion people worldwide suffer from neurological conditions that cause mobility impairments, often persisting despite rehabilitation. Chronic neurological disease (CND) patients who lack access to continuous rehabilitation face gradual functional decline. The International Classification of Functioning, Disability, and Health (ICF) provides a comprehensive framework for assessing these patients. Objective: This study aims to evaluate the outcomes of a non-hospitalized neuromotor rehabilitation project for CND patients in Italy using the Barthel Index (BI) as the primary outcome measure. The rehabilitation was administered through an Individual Rehabilitation Plan (IRP), tailored by a multidisciplinary team and coordinated by a physiatrist. The IRP involved an initial comprehensive assessment, individualized therapy administered five days a week, and continuous adjustments based on patient progress. The secondary objectives include assessing mental status and sensory and communication functions, and identifying predictive factors for BI improvement using an artificial neural network (ANN). Methods: A retrospective observational study of 128 CND patients undergoing a rehabilitation program between 2018 and 2023 was conducted. Variables included demographic data, clinical assessments (BI, SPMSQ, and SVaMAsc), and ICF codes. Data were analyzed using descriptive statistics, linear regressions, and ANN to identify predictors of BI improvement. Results: Significant improvements in the mean BI score were observed from admission (40.28 ± 29.08) to discharge (42.53 ± 30.02, p < 0.001). Patients with severe mobility issues showed the most difficulty in transfers and walking, as indicated by the ICF E codes. Females, especially older women, experienced more cognitive decline, affecting rehabilitation outcomes. ANN achieved 86.4% accuracy in predicting BI improvement, with key factors including ICF mobility codes and the number of past rehabilitation projects. Conclusions: The ICF mobility codes are strong predictors of BI improvement in CND patients. More rehabilitation sessions and targeted support, especially for elderly women and patients with lower initial BI scores, can enhance outcomes and reduce complications. Continuous rehabilitation is essential for maintaining progress in CND patients.

Background and Aims: Ultrasound nerve cross-sectional area (CSA) of patients affected with axonal neuropathy usually shows normal value. Cerebellar ataxia, neuropathy and vestibular areflexia syndrome (CANVAS) seems to represent an exception, showing smaller CSA, but previous reports did not test for biallelic RFC1 gene repeat expansions., Methods: We compared nerve CSA from CANVAS patients (tested positive for biallelic RFC1 gene repeat expansions) with the CSA from a group of patients with chronic idiopathic axonal polyneuropathy (CIAP) who tested negative for RFC1 gene repeat expansions, hereditary axonal neuropathy (Charcot-Marie-Tooth type 2, CMT2), and Friedreich ataxia (FRDA)., Results: We enrolled 15 CANVAS patients (eight men, mean age 66.3 ± 11.5 years, mean disease duration 9.3 ± 4.1 years), affected with sensory axonal neuronopathy. Controls consisted of 13 CIAP (mean age 68.5 ± 12.8 years, seven men), seven CMT2 (mean age 47.9 ± 18.1 years, four men), 12 FRDA (mean age 33.7 ± 8.8, five men). Nerve ultrasound was performed at median, ulnar, sciatic, sural, and tibial nerves and brachial plexus, bilaterally. The nerve CSA from CANVAS patients was significantly smaller than the one from the other cohorts at several sites with significant and high accuracy at Receiver-operating characteristic (ROC) curve analyses. RFC1 AAGGG pentanucleotide expansion, disease duration, and disability did not correlate with CSA at any site, after Bonferroni correction., Interpretation: Decreased sonographic nerve sizes, in arms and legs, in patients with sensory neuropathy and normal motor conduction studies could point to CANVAS-spectrum disease and help guide appropriate genetic testing., (© 2024 Peripheral Nerve Society.)

Introduction: The role of rehabilitation during and after the COVID-19 pandemia was influenced by the progressive acquisition of knowledge on the pathology with the adaptation of the rehabilitative instruments to the heterogeneous impairments of the patients. The aim of this systematic review is to describe the effects of the rehabilitation tools applied in the last three years in the different phases of the COVID-19 disease., Evidence Acquisition: A literature search of MEDLINE (PubMed), Scopus and Physiotherapy Evidence Database (PEDro) was conducted. 2994 participants were included in this systematic review: 1225 patients for acute-COVID, 1331 patients for post-COVID, 438 patients for long-COVID., Evidence Synthesis: Of the initial 907 hits only 45 randomized controlled trials were included. The Cochrane library assessment tool was used to evaluate the risk of bias. The trials selected studied the effects of respiratory rehabilitation alone or in association with exercises, tele-rehabilitation, endurance training, virtual reality, electrostimulations in different settings and phases of the infection with the evaluation of clinical, quality of life and impairment outcome measures., Conclusions: All the rehabilitations tool were used based on the experts' opinion and on the rules of good clinical practice, during and after the pandemic period. Despite the heterogeneity of the studies, the different outcome measures and the small sample sizes, pulmonary rehabilitation, tele-rehabilitation and low/moderate intensity aerobic and endurance exercises seemed to improve clinical and quality of life outcomes at short (8 weeks) and mean time (2 months) after treatments in all phases of the infection.

Introduction/aims: In a recent study, we showed that nerve ultrasound of the upper limbs could distinguish inherited sensory neuronopathy from inherited axonopathy; surprisingly, no differences were found in the lower limb nerves. In this study, we compared lower limb nerve ultrasound measurements in inherited neuronopathy, inherited axonopathy, and acquired axonopathy., Methods: Tibial and sural nerve ultrasound cross-sectional areas (CSAs) of 34 healthy controls were retrospectively compared with those of three patient groups: 17 with cerebellar ataxia with neuronopathy and vestibular areflexia syndrome (CANVAS), 18 with Charcot-Marie-Tooth type 2 (CMT2), and 18 with acquired length-dependent sensorimotor axonal neuropathy, using ANOVA with post-hoc Tukey honestly significance difference (HSD) (significance level set at p < .05)., Results: The nerve CSAs of CANVAS and CMT2 patients were not significantly different. Both the tibial and the sural nerve CSAs were significantly smaller in CANVAS and CMT2 compared with the acquired axonal neuropathy group. Tibial nerve CSAs of CANVAS and CMT2 were significantly smaller than controls. Tibial and sural nerve CSAs of the acquired axonal neuropathy group were also significantly larger than the controls'., Discussion: Ultrasound of the lower limb nerves distinguished inherited from acquired axonopathy with the nerve size respectively reduced and increased in these two groups. 39. Symptoms patterns and health-related quality of life in a real-life cohort of Long COVID patients: understanding the complexity to optimize the rehabilitation treatment.

Objective: We wanted to identify prevalent symptoms and patterns of Long COVID syndrome, assess the impact on health-related quality of life, and explore factors linked to lower quality of life, including vaccination status and symptom count, in a real-life cohort., Design: observational retrospective study., Results: We assessed 133 patients and 85 completed the evaluations. The most common symptoms reported were motor deficit (95.29%) and fatigue (94.12%), while respiratory symptoms, cognitive deficits and weakness (76.47%) were less frequent. Many patients experienced multiple symptoms, with the majority reporting 5 to 8 symptoms simultaneously, most associated with fatigue. However, the number of symptoms did not correlate with health-related quality of life as measured by the 12-Item Short Form Survey (SF-12) questionnaire. Finally, vaccination status did not significantly affect SF-12 scores or the number of reported symptoms., Conclusion: Our analysis highlighted the presence of diverse multisystemic symptoms in Long COVID patients. Many individuals experienced multiple associated symptoms, negatively affecting their overall quality of life. Neither vaccination status nor the number of symptoms appeared to influence reported quality of life. Directed evolution focuses on optimizing single genetic components for predefined engineering goals by artificial mutagenesis and selection. In contrast, experimental evolution studies the adaptation of entire genomes in serially propagated cell populations, to provide an experimental basis for evolutionary theory. There is a relatively unexplored gap at the middle ground between these two techniques, to evolve in vivo entire synthetic gene circuits with nontrivial dynamic function instead of single parts or whole genomes. We discuss the requirements for such mid-scale evolution, with hypothetical examples for evolving synthetic gene circuits by appropriate selection and targeted shuffling of a seed set of genetic components in vivo. 41. Reliability, Concurrent Validity, and Clinical Performances of the Shorter Version of the Roland Morris Disability Questionnaire in a Sample of Italian People with Non-Specific Low Back Pain.

- Abstract
Background: Evaluating the psychometric and clinical performances of the RM-18, the shorter version of the Roland Morris Disability Questionnaire (RMQ), in Italian people with non-specific low back pain (NSLBP) as a time-saving and clinically useful method of assessing disability., Methods: This cross-sectional study included 74 people (52 females and 22 males, 53.03 ± 15.25 years old) with NSLBP. The RM-18, the RMQ, the Oswestry Disability Index (ODI), and a pain intensity numerical rating scale (NRS) were administered. Psychometric testing included reliability by internal consistency (Cronbach's alpha) and test-retest measurement (Intraclass Correlation Coefficient, ICC2.1), and concurrent validity by comparing the RM-18 with the RMQ and the ODI (Pearson's r correlation). Two separate regression analyses were performed to investigate the different impact of RM-18 and RMQ on NRS., Results: Cronbach's α of RM-18 was 0.92 and ICC (2,1) = 0.96. Strong correlations were found with the RMQ and the ODI (r = 0.98; r = 0.78, respectively). The regression models showed that the RM-18 and the RMQ similarly impacted the NRS ( p < 0.001)., Conclusion: The RM-18 showed satisfactory psychometric testing and similarly impacted the NRS when compared to the RMQ. It can be recommended for clinical and research purposes in Italian people with NSLBP.

- Abstract
Introduction: Over the last few decades, the use of neo/adjuvant therapies has significantly increased the number of breast cancer survivors who experience chemotherapy-induced peripheral neuropathy (CIPN). To date, few, low-efficacy, pharmacological remedies exist to manage this side effect. For this reason, alternative treatments are increasingly being investigated as possible strategies to prevent or promote faster recovery from CIPN. In this review we aimed to provide an overview of the literature evidence regarding all the non-pharmacological and rehabilitative interventions for patients affected by CIPN secondary to breast cancer care., Evidence Acquisition: A comprehensive literature search was conducted on PubMed, Scopus and Web of Science and included a total of 1895 patients (1528 with breast cancer) with a wide range of CIPN (motor, sensory and autonomic neuropathies) and chemotherapy treatments (e.g., Taxanes, Platins, Vinca alkaloids or monoclonal antibody drugs)., Evidence Synthesis: Of the initial 1108 hits, only 25 studies - describing different treatment modalities for peripheral neuropathies - were finally included in the qualitative synthesis. Most studies focused on acupuncture, physiotherapy, cryotherapy, and yoga., Conclusions: There is still controversial evidence on conservative non-pharmacological interventions for the management of CIPN symptoms. We believe however that moderate exercise, as well as all types of stress reducing activities like sport, yoga and mindfulness, should be encouraged in cancer patients for their positive effect on global physical and psychological health. Further studies of higher methodological quality are needed to determine the best conservative approach to CIPN.

43. Predictive Prognostic Factors in Non-Calcific Supraspinatus Tendinopathy Treated with Focused Extracorporeal Shock Wave Therapy: An Artificial Neural Network Approach.

The supraspinatus tendon is one of the most involved tendons in the development of shoulder pain. Extracorporeal shockwave therapy (ESWT) has been recognized as a valid and safe treatment. Sometimes the symptoms cannot be relieved, or a relapse develops, affecting the patient's quality of life. Therefore, a prediction protocol could be a powerful tool aiding our clinical decisions. An artificial neural network was run, in particular a multilayer perceptron model incorporating input information such as the VAS and Constant-Murley score, administered at T0 and at T1 after six months. It showed a model sensitivity of 80.7%, and the area under the ROC curve was 0.701, which demonstrates good discrimination. The aim of our study was to identify predictive factors for minimal clinically successful therapy (MCST), defined as a reduction of ≥40% in VAS score at T1 following ESWT for chronic non-calcific supraspinatus tendinopathy (SNCCT). From the male gender, we expect greater and more frequent clinical success. The more severe the patient's initial condition, the greater the possibility that clinical success will decrease. The Constant and Murley score, Roles and Maudsley score, and VAS are not just evaluation tools to verify an improvement; they are also prognostic factors to be taken into consideration in the assessment of achieving clinical success. Due to the lower clinical improvement observed in older patients and those with worse clinical and functional scales, it would be preferable to also provide these patients with the possibility of combined treatments. The ANN predictive model is reasonable and accurate in studying the influence of prognostic factors and achieving clinical success in patients with chronic non-calcific tendinopathy of the supraspinatus treated with ESWT.

44. Botulinum toxin in the rehabilitation of painful syndromes: multiperspective literature analysis, lexical analysis and systematic review of randomized controlled trials.

- Abstract
Pain represents a common symptom of several diseases and is often associated with a reduction in rehabilitation outcomes and recovery. The effectiveness of pain alleviation by botulinum toxin has been recently demonstrated. We searched in PubMed the papers about this topic published in the last ten years, and we selected clinical trials, guidelines, meta-analyses, reviews, and systematic reviews. We used different approaches: multiperspective presentation, lexical evaluation, and systematic review. The systematic review was only performed for the randomized controlled trials. We predominantly found reviews and trials about the rehabilitation of stroke/brain injury and epicondylitis. The most common outcome measures were pain, function, and spasticity. Among the common words, pain was the most frequent and the terms were grouped into different families, especially concerning the outcomes. Rehabilitation showed a relatively low frequency. Finally, the systematic review showed moderate-low levels of bias which confirms the effectiveness of botulinum toxin for pain treatment. The current literature about botulinum toxin is wide and globally diffuse but with some limitations in study strategies and clearness in the formal presentation. The evidence justifies the use of botulinum toxin in treating pain in different diseases.

Objectives: The aim of this study was to investigate the mechanism of falls in male elite wheelchair basketball (WB) players and to analyse the falls characteristics considering their classification score which ranges from 1.0 to 4.5 and it is related to their functional capacity and particularly to their volume of action., Design: A cross-sectional video analysis was conducted using European para championships 2023official match videos., Setting: Players of the sample team were divided into 2 groups according to their classification point: low-point players (LPP) and high-point players (HPP). Every occurred fall was systematically analysed focusing on falling related characteristics and game circumstances., Participants: Twelve WB players of the Italian national team., Main Outcome: Number of falls and mechanism of falls., Results: The results showed a higher number of falls for HPP and highlighted a statistically significant differences between LPP and HPP for the fall direction, the playing time and the first anatomical site in contact with the floor., Conclusion: Results reported significant differences between LPP and HPP in terms of mechanism of fall. 48. Effects of focal muscle vibration on cervical pain in Parkinson's disease patients: a pilot study.

Musculoskeletal pain is a common symptom of Parkinson's disease (PD) that is not adequately treated with current dopaminergic drugs. This pilot study sought to investigate the effect of focal muscle vibration (fMV) on a group of Parkinson's disease patients suffering from chronic cervical pain. In addition to conventional physiotherapy, twenty-two patients with idiopathic Parkinson's disease (Hoehn and Yahr stages II-III) received three weeks of bilateral focal musclevibration to the trapezius muscles. The Visual Analogue Scale (VAS), the Short-form McGill, and the Present PainIntensity scales were used to assess pain at baseline (T0), after three weeks of treatment (T1), one week after the last treatment session (T2), and three weeks after T2 (T3). Pain intensity decreased significantly from baseline to T1 across all pain scales (p < 0.0001). Furthermore, the beneficial effect of fMV on cervical pain lasted up to one month after treatment. Our findings show that fMV, in combination with conventional physiotherapy, is effective at reducing pain intensity in PD patients, with results visible even after a month of follow-up.

Parkinson's disease (PD) is defined by progressive worsening of gait, posture, and balance, as well as disability in daily life activities, and improvement in chronic musculoskeletal pain, particularly neck pain associated with worsening of balance. The study's goal is to look into the relationship between scoliosis, balance, and cervical pain in Parkinson's disease patients. Cross-sectional, pilot study. The study included 16 Parkinson's patients. Neck cervical pain was measured using the pain visual analogue scale and the short form McGill pain questionnaire, while dynamic balance was assessed using static balance, Tinetti, Berg Balance, and the Short Physical Performance Battery scales (SPPB). Cobb angles are measured on a whole-spine standard X-ray to assess spinal scoliosis.An observational statistical analysis was performed with patients subdivided into two groups: non-scoliosis (NS) and true scoliosis (TS) based on whether they presented a Cobb's angle below or ≥10°. Neck pain was reported by 37% (n=3) of participants in the NS group versus 50% (n=4) in the TS group. Neck pain was more prevalent in patients with a disease duration of less than 48 months (n=6; 75.0% vs n=1; 12.5%; p-value < 0.05). Scoliosis, cervical pain, and postural imbalance are all significant but often overlooked Parkinson's disease complaints.
