Abstract

Objective: To evaluate strategies that are followed after pediatric renal trauma during the recovery stage, with an emphasis on mobility and involvement in subsequent sporting activities. Renal trauma is the most common urogenital trauma in children. The American Association for the Surgery of Trauma (AAST) scale is most commonly used to stratify the severity of injury. There is no consensus in the existing literature with respect to the recovery stage following renal trauma. Methods: A survey was constructed by the European Association of Urology (EAU) – Young Academic Urologists (YAU) Pediatric Urology Working Group and then made digitally available on SurveyMonkey. The survey consists of 15 questions exploring relevant factors and timing to start again with mobility and activity. Results: In total 153 people responded, of whom 107 completed the entire survey. The presence of pain and severity of trauma were acknowledged as most important factors to commence mobilization, whereas presence of hematuria was identified as an additional factor for sporting activity. Regardless of severity of trauma a minimum of 90% of respondents recommend return to noncontact sports within 12 weeks. For contact sports, a minimum of 33% of respondents advised >12 weeks minimum before starting again. A small number of respondents would never allow sporting activities again. Conclusion: The time to allow sporting activity shows high variation among the respondents, some even restricting sporting activities completely. This survey highlights the need for a standardized protocol based on multicenter follow-up data.

Abstract

Background: The term glass ceiling coined by Loden in 1978 is commonly used to describe difficulties faced by minorities and women when trying to move into senior roles. Objective: To analyse trends and patterns for female representation at the European Association of Urology (EAU) and European Society for Paediatric Urology (ESPU) annual general meetings over the past decade. Design, setting, and participants: We used objective data on female representation in the roles of chairs, moderators, and lecture speakers at the EAU and ESPU meetings from 2012 to 2022. Outcome measurements and statistical analysis: We evaluated gender based representation in paediatric urology sessions at the EAU and ESPU meetings, collecting data on the overall number of sessions, lectures, symposiums, abstract/poster sessions, and courses, and analysed the male/female ratio. Data were derived from printed and digital programmes for the relevant meetings. Results and limitations: During the period from 2012 to 2022, the percentage female representation varied from 0% (2012) to a maximum of 35% (2022) at EUA paediatric urology sessions, and from 13.5% (2014) to a maximum of 32% (2022) at ESPU meetings. Both associations show clear progression towards equality. Conclusions: Female representation at EAU and ESPU meetings has risen over the years, reaching 35% and 32%, respectively, in 2022, which is in line with the number of female members. We hope that this motivates a move towards the equality objectives for 2030. A clear and fundamental societal change is needed, with fair and more consistent institutional policies and framework commitments in the areas of science, medicine, and global health. Gender equality and diversity taskforces are essential to achieve these goals. Patient summary: We analysed the male/female ratio for participants in annual meetings held by the European Association of Urology and the European Society for Paediatric Urology. From a low level in 2012, the ratio increased to over 30% in 2022, in line with the female membership of the societies. Focus on fair and consistent policies is needed to ensure that women are well represented in medicine.

Abstract

Objective: Information on cardiovascular problems related to childhood urinary stone disease is limited. The aim of this study was to assess the ventricular functions and subclinical cardiovascular risk in children with urolithiasis using echocardiographic measurements. Materials and Methods: Children diagnosed with urolithiasis were prospectively enrolled in the study as well as children with no urinary stone disease were confirmed via urinary ultrasonography. Body mass index and blood pressures were noted, as well as basic serum parameters. Carotid intima media thickness (cIMT), epicardial fat tissue (EFT) thickness and periaortic fat tissue (PFT) thickness were measured via transthoracic echocardiography in addition to pulsed and tissue Doppler imaging. Myocardial performance indexes were also calculated and correlation analyses were conducted. Results: A total of 17 patients (10 boys) with a mean age of 8.57±2.62 years were included in this study. There were 17 children (12 boys) in the control group and their mean age was 9.53±1.72 years. There was no statistically significant difference between the two groups in terms of demographic and laboratory variables. Tissue Doppler echocardiography revealed that Tei indexes of the left ventricle, right ventricle and septum were significantly higher in the study group than in the controls (p<0.001 for all). The cIMT (0.041±0.012 vs. 0.025±0.002), EFT (0.432±0.083 vs. 0.325±0.032) and PFT thicknesses (0.138±0.029 vs. 0.113±0.008) of the study group was statistically higher than the control group (p<0.001, p<0.001 and p=0.002, respectively) indicating a higher CVD risk. Conclusion: Children with urolithiasis had not only biventricular early systolic and diastolic dysfunction but also subclinical atherosclerosis at early ages. 27. Maintenance biofeedback therapy for dysfunctional voiding: Does every child need it?

Abstract

Objectives: The aim of the present study is to analyze the long‐term clinical outcomes in children who were treated with biofeedback therapy (BF) for dysfunctional voiding (DV) and to determine the need for maintenance of BF due to clinical relapse. Methods: Files of children with DV who underwent BF between 2013 and 2020 were retrospectively reviewed. Patients with neurological or anatomical problems were excluded. A total of 64 patients (52 girls, 12 boys) with a mean age of 8.89 ± 2.48 years who completed the initial BF sessions were included in the study. The demographic and clinical data, dysfunctional voiding symptom scores (DVSS), and uroflowmetry parameters were recorded before and after the initial BF sessions. Clinical success was regarded as the cessation of electromyography activity as well as concurrent resolution of lower urinary tract symptoms and improvement in uroflowmetry parameters. After the initial BF sessions, children who had initial success showed DV relapse at any time during follow‐up received the maintenance BF sessions. Therefore, the patients with successful initial BF were divided into two groups: the group that needed maintenance BF, and the group which required no maintenance BF. Results: Clinical success was achieved in 48 (75.0%) of 64 children following a median of 6 sessions (range 2–8). At the follow‐up, 10 (20.8%) out of 48 patients showed symptom relapse at a median of 8 months (range 2–24 months). After a median of 3.5 maintenance BF sessions (range 1–6), clinical success was observed in all patients. Both groups showed a significant DVSS decrease after initial BF, however, those who needed maintenance had significantly higher DVSS (6.80 ± 2.53 vs. 3.61 ± 1.12, p < 0.001). At admission, DVSS <17.5 (odds ratio [OR]: 4.31, p = 0.025) and post‐voiding residual volume as a percentage of estimated bladder capacity for age <28.9 (OR: 5.00, p = 0.009) were found as the predictive factors for initial BF success. The need for maintenance BF was 2.56‐fold higher with a DVSS above 5.5 after initial BF sessions. Conclusions: Our results show that despite a clinical success rate of 75% after the initial BF, relapse can be seen within 2 years in approximately 20% of the patients. Nevertheless, maintenance of BF may provide clinical success in all patients. Relatively higher DVSS after initial BF can be used as a predictor of the need for maintenance BF. 28. Can Impaired Distal Ureteral Contraction Detected by Urinary Ultrasonography Indicate Presence of VUR?

Abstract

Objective: The aim of this study was to determine if impaired distal ureteral contraction detected by urinary ultrasonography (US) could predict presence of vesicoureteral reflux (VUR). Materials and Methods: A Retrospective analysis of patients' files between November 2016 and September 2020 was performed. A total of 248 children with demonstrated VUR on voiding cystourethrogram (VCUG) and who underwent urinary US within one week of the VCUG examination were included in the study. The degree of hydronephrosis was classified as per Society for Fetal Urology grading system and grading of VUR was done by the International Reflux Study Group grading system. The impaired distal ureteral contraction was defined as the reduced movement of the distal ureter as well as the deficiency of urine discharge into the bladder. Results: There was a significant correlation between the grade of VUR and the grade of hydronephrosis (p=0.0028). Also, the presence of distal ureteral dilation was related to increasing grades of VUR (p=0.001). The impaired distal ureteral contraction was present only in children with dilating VUR (grades III IV V) while its relationship between VUR grade was significantly high for grades IV and V VUR (p=0.001). Conclusion: In children, the impaired distal ureter contraction as well as the increase of grades of hydronephrosis, and the presence of distal ureteral dilation may aid decision-making in terms of requesting VCUG to detect dilating VUR. Abstract

Öz Amaç: Renal skar, primer veziko-üreteral reflü (VUR) hastalarında konjenital veya kazanılmış şekilde gözlenebilir. Renal displazi ise embriyolojik gelişimdeki kusurlar sonucunda ortaya çıkar ve VUR ile birliktelik gösterebilir. Bu çalışmamızda amaç VUR tanısı ile takip edilen hastalarda skar ve displastik böbrek sıklığını ve bunun İYE ile olan ilişkisini araştırmaktır. Gereç ve Yöntem: Kliniğimizde 2012-2018 tarihleri arasında primer VUR tanısı ile takip edilen hastaların dosyaları retrospektif olarak incelendi. Sekonder VUR tanılı hastalar (Nörojenik mesane, posterior uretral valv vb.) çalışma dışı bırakıldı. Hastaların yaş, cinsiyet, idrar analizi, dimerkaptosüksinik asit (DMSA) sintigrafi ve işeme sistoüretrografi (İSUG) bulguları kaydedildi. Dilate VUR, İSUG’da ≥grade 3 VUR olarak kabul edildi. Renal skar ve displazinin İYE, VUR derecesi, lateralite ve cinsiyet ile ilişkisi olup olmadığı araştırıldı. Bulgular: Çalışmaya, kriterlere uyan toplam 121 hasta dahil edildi. Hastaların 66’ı kız (%55) iken 55’i erkek (%45) idi. Ortanca yaş 6 ay (2-42 ay) olarak bulundu. Çalışmamızda 58 hastada (%48) bilateral VUR saptanırken, 78 hastada (%65) dilate VUR tespit edildi. İYE, toplamda 76 hastada (%62,8) saptandı. Tüm hastaların 33’ünde (%27,3) displastik böbrek gözlenirken, 20’sinde (%16,5) renal skar tespit edildi. ÖzAmaç:Böbrek ve üriner sistemindoğumsal anomalileri (BÜSDA) tanımı, konjenital yapısal anomaliler kullanılanbir terimdir. Prenatal ultrasonografinin yaygın kullanıma girmesiyle BÜSDA,çocuk nefroloji ve üroloji vizitlerinin önemli bir bölümünü oluşturmaktadır. Bu retrospektif çalışmada, BÜSDA tanısı ile takip edilen hastalarınanalizi ve bu hastalardaki renal hasar varlığı araştırılmıştır.Gereç ve Yöntem: Kliniğimizde 2012-2018 yılları arasında BÜSDA tanısı iletakip edilen hastaların dosyaları retrospektif olarak incelendi. Birden fazlaanomalisi olanlar (sendromik olgular vb.) çalışma dışı bırakıldı. Dosyalardan antenataltanı varlığı, yaş, cinsiyet, görüntüleme ve sintigrafi sonuçları kaydedildi.Ultrasonografi ile takip edilen hastalarda gerektiğinde işemesistoüretrografisi (İSUG) ve dimerkaptosüksinik asit (DMSA) sintigrafisikullanıldı.Bulgular: Çalışmaya 572 erkek(%71) ve 234 kız (%29) olmak üzere toplam 806 hasta dahil edildi. Ortanca yaş 2ay ve ortanca takip süresi 16 ay olarak bulundu. Hastaların 503’ünde (%62.4)antenatal tanı mevcuttu ve 29 hastada (%3.6) prematür doğum öyküsü mevcuttu.Üst üriner sistemde en sık görülen anomaliler sırasıyla izole hidronefroz(%58.2), ektopik böbrek (%7.8) ve at nalı böbrek (%6.7) idi. Alt ürinersistemde ise konjenital vezikoüreteral reflü (%10.9) ve megaüreterdi (%1.6).Hastalardan 75 olguda (%9.3) renal skar, 53 olguda ise (%6.6) renal hipodisplazi saptandı.Tartışma: BÜSDA’nın prenatal tanısı renal fonksiyon kaybınaneden olabilecek problemlerin erken dönemde tanınması şanısını artırmaktadır.Buna rağmen hastaların ¼’ünden fazlası uzun dönemde tanı almadan kalmaktadır.BÜSDA’lı çocuklar için, uygun tanı ve tedavi ile renal hasarın ilerlemesiniönlenmesi gereklidir., ObjectiveCongenital anomalies of the kidney and theurinary tract (CAKUT) is a term used for a broad spectrum of congenital structuralanomalies. After implementation of prenatal ultrasonography, CAKUT became animportant part of pediatric nephrology and urology visits. The aim of thisretrospective study is to analyze the outcomes as well as prevelance of renaldamage in this group of patients.Material and MethodsPatients diagnosed with CAKUT between 2012and 2018 were retrospectively reviewed. Patients with multiple anomalies(i.e.syndromes) were excluded. Presence of antenatal diagnosis, patient age,gender, imaging studies and scintigraphy results were noted. Ultrasonographywas used for follow-up imaging, in addition to VCUG and DMSA when needed.ResultsThere were 806 patients in our cohort (572males and 234 females, 71% vs. 29%). Median age was 2 and median follow-up periodwas 16 months. Only 3.6% of the patients were prematurely born where antenataldiagnosis was present in 503 patients (62.4%). Most common anomalies in theupper urinary tract were isolated hydronephrosis (58.2%), ectopic kidney (7.8%)and horseshoe kidney (6.7%), respectively. On the other hand, VUR (10.9%) wasthe most common abnormality of the lower urinary tract. Renal scarring wasobserved in 75 patients (9.3%) while renal hypodysplasia was present in 53patients (6.6%).ConclusionsPrenatal diagnosis of CAKUT has increasedearly diagnosis of problems that may lead to renal function loss. For childrenwith CAKUT, proper diagnosis and follow-up is necessary in order to preventrenal deterioration.

Background/Aims:Calcium stones are the most common stones observed inpediatric population and they are generally a consequence of an underlyingmetabolic problem. Potassium citrate is the standard treatment for pediatriccalcium stone formers. In this study, we aimed to evaluate the effects of oralmagnessium added to potassium citrate treatment for pediatric urolithiasispatients who have hipomagnesuria in addition tohipercalciuria/hiperoxaluria/hipocitraturia.Methods:Retrospective chart review was conducted for pediatricpatients with urolithiasis. Patiens identified with hipomagnesuria in additionto hipercalciuria/hiperoxaluria/hipocitraturia(one or more) were included.Hiperuricosuria, primary hiperoxaluria, sistinuriaand patients with stones ≤3 mm as well as under 2 years ofage were excluded. Outcomes related to urinary stone disease such as stonesize, spontaneous passage and duration of treatment were compared betweenpatients receiving potassium citrate only versus potassium citrate andmagnesium Results:There were 14 patients inpotassium citrate arm (group 1) while there were 15 patients who receivedpotassium citrate and magnesium (group II). There were 9 girls and 5 boys withmean age of 9.3±4.3years and there were 7 girls and 8 boys with a mean age of 7.0±3.9 in group II.No difference wasa observed in terms of gender and age (p˃0.05). There was nostatistically significant difference between grooups with regard to duration oftreatment (group I, 3.2±1.2 months vs. group II, 3.1±1.4 months,p=0.872). Mean stone size before treatment was 5.2±1.6 mm in group I and 5.1±0.9 mm in group II.Mean stone size after treatment was 3.4±1.3 mm in group I while it was 4.6±1.6 mm in group II showingno difference between pre and post treatment. Spontaneous stone passage wasobservedin 2 patiens (14.3%) in group I whereas urinary Stones were spontaneouspassed in 5 patients (33.3%) in group II. However, it did not reach tostatistically significant level between groups. Conclusions: Treatment of hipomagnesuriawith oral magnesium in pediatric patients with hipomagnesuria in addition to hypercalciuria/hyperoxaluria/hypocitraturiadoes not change the outcomes in short term., ÖzetGiriş: Çocukluk çağı taş hastalığında en sıkgörülen taş tipi kalsiyum taşlarıdır ve genellikle altta yatan metabolik biranormalliğe bağlı görülür. Kalsiyum taşlarının tedavisinde potasyum sitrat sıkkullanılan bir tedavi seçeneğidir. Bu çalışmada kalsiyum taşı tanısı ile takipedilen ve hiperkalsiüri/hiperokzalüri/hipositratüriye ek olarak hipomagnezürisiçocuklarda potasyum sitrat tedavisi ile potasyum sitrat ve magnezyumkombinasyonunun etkinliği araştırılmıştır.Metod: Kliniğimizde üriner sistem taşhastalığı tanısı ile takip edilen hasta dosyaları geriye dönük incelendi. Metaboliktaramada hiperkalsiüri/hiperokzalüri/hipositratüri (biri veya birden fazlası)anormalliklerine ek olarak hipomagnezüri saptanan hastalar çalışmaya dahiledildi. Hiperürikozüri, primer hiperokzalüri, sistinüri ile başlangıç taşboyutu ≤3 mm ve 2 yaşın altında olan hastalar çalışma dışı bırakıldı. Tedavidesadece potasyum sitrat kullanan grup ile potasyum sitrata ek olarak magnezyumtedavisi ilave edilen grup arasında tedavi öncesi ve sonrası taş boyutları, taşdüşürme oranları ve tedavi süreleri karşılaştırıldı.Bulgular: Çalışmaya potasyum sitrat kullanan 14hasta (grup I) ile potasyum sitrat ve magnezyum kullanan 15 hasta (grup II)olmak üzere toplam 29 hasta alındı. Grup I’de 9 kız ve 5 erkek, (yaş ortalaması9.3±4.3 yıl), grup II’de ise 7 kız ve 8 erkek (yaş ortalaması 7.0±3.9 yıl)mevcuttu. Her iki grupta yaş ve cinsiyet açısından fark yoktu (p˃0.05). Tedavisüreleri açısından iki grup arasında fark bulunmadı (grup I, 3.2±1.2 ay vs.grup 2, 3.1±1.4 ay, p=0.872). Tedavi öncesi taş boyutları grup I’de ortalama 5.2±1.6 mm, grupII’de 5.1±0.9 mm iken, tedavi sonrası grup I’de 3.4±1.3 mm, grup II’de 4.6±1.6mm idi. Tedavi sonrası taş düşürme grup I’de 2 hastada (%14.3), grup II’de ise5 hastada (%33.3) gözlendi. Tedavi öncesi ve sonrası taş boyutları ile taşdüşürme oranları açısından her iki grupta anlamlı fark saptanmadı.Sonuç: Kalsiyum taşlarında risk faktörlerine eşlik eden hipomagnezürinintedavi edilmesi kısa dönemde çocukluk çağı taş hastalığında etkilibulunmamıştır.

40. The association between the congenital heart diseases and congenital anomalies of the kidney and the urinary tract in nonsyndromic children.

Abstract

Several congenital heart diseases (CHD) are present in children with congenital anomalies of the kidney and the urinary tract (CAKUT) in syndromic or nonsyndromic patterns. The purpose of this study was to identify nonsyndromic children with CAKUT who required echocardiographic evaluation and to find the prevalence of CHD in these patients. Retrospective chart review of nonsyndromic patients with CAKUT between 2012 and 2018 was conducted. Types of congenital anomalies of the kidney and urinary tract and echocardiogram reports of the patients, age, gender, prematurity, prenatal diagnosis of CAKUT, and family history were noted. Among 806 children with CAKUT, 135 patients underwent transthoracic echocardiographic evaluation and CHD was detected in 91 (11.2%). The most common type of CHD was secundum type atrial septal defect that was found in 73 (80.2%) patients. On the other hand, the most frequent types of congenital anomalies of the kidney and the urinary tract were isolated hydronephrosis and primary vesicoureteral reflux (61.4% and 12.8%, respectively). Our results showed that 75.8% of the patients with congenital heart diseases had isolated hydronephrosis. Finally, only 18 (2.2%) cases out of 806 patients that were evaluated via transthoracic echocardiography due to the clinical findings and family history had a CHD that required close (<6 months) follow‐up. Congenital heart diseases that required close follow‐up are rarely observed in nonsyndromic patients with CAKUT. Therefore, we suggest echocardiography should be performed in cases of clinical suspicion and family history rather than routinely in this patient group. [ABSTRACT FROM AUTHOR]

Abstract

Objective: The primary aim of this study is to evaluate if severe obesity has any deleterious effect on the early postoperative course in patients undergoing percutaneous nephrolithotomy (PNL) under spinal anesthesia, and the secondary aim is to determine whether severe obesity changes surgical results. Material and Method: The study included 100 patients who underwent a PNL operation for kidney stones of ≥2cm in our clinic. Accordingly, those with BMI <35 kg/m² formed the first while the severely obese BMI ≥35 kg/m² formed the second group. Age, gender, stone burden, pre/postoperative hemoglobin levels, number and regions of access, duration of surgery, perioperative complications and Visual Analogue Scale score at 24h, PACU admission/ discharge Aldrete scores, and PACU length of stay were recorded. Results: The VAS score was 5.0±1.3 in group 1 while it was 5.3±1.8 in group 2 (p=0.32). The length of the hospital stay was 3.3±1.3 days in patients with BMI <35 kg/m² and 3.0±1.0 in patients with BMI ≥35 kg/m² (p=0.36). A 98% stone-free rate was found in the severely obese group and 96% in the normal BMI group, and there was no significant difference between the groups (p=0.672). There was a statistically significant difference in terms of PACU discharge Aldrete scores between groups (9.9±0.3 vs. 9.6±0.4 in groups 1 and 2, respectively) (p=0.03). Finally, the time interval in PACU was 39.0±7.5 mins in group 1 while it was 58.8±14.0 mins in group 2 (p<0.0001). Conclusions: Severe obesity does not alter surgical outcomes in patients undergoing PNL under spinal anesthesia, and in our opinion, spinal anesthesia in PNL is a safe and effective anesthesia modality for renal stone(s) in obese and morbidly obese patients. Abstract

Objective: Urinary stone disease is a growing entity in both children and adults. It is an important cause of morbidity in endemic countries such as Turkey. The aim of this study was to identify clinical features and metabolic disturbance of pediatric urinary stone disease in Konya province. Material and Methods: Retrospective analysis of patients with diagnosis of urinary stone disease was conducted. Those with complete demographics, imaging and laboratory results were included. Ultrasonography and computed tomography without contrast was used to obtain stone location and size. Metabolic analysis were based on spot urinary excretions in children without potty training where 24-hour urine samples were used to identify urinary oxalate, citrate, cystine, calcium, uric acid and creatinine levels. Results: A total of 107 males and 100 females were included in the study. Male/female ratio was 1.1 whereas median age was 1.6 years (range 2 months - 16.1 years). Most common complaint of admission was urinary tract infection (25.1%) followed by abdominal/side pain (18.4%), discomfort (17.4%) and microscopic/macroscopic hematuria (15.5%). Metabolic disturbances were found in 150 patients. Most common metabolic abnormalities were hypocitraturia (51 patients, 24.6%) and hypercalciuria (40 patients, 19.3%), respectively. Conclusion: Results of our study indicate most common metabolic abnormality in children with urinary stone disease in Konya province is hypocitraturia. Necessary test in order to evaluate metabolic disturbance should be obtained in every pediatric urolithiasis patient, thus the risk of renal morbidity should be decreased. [ABSTRACT FROM AUTHOR]
