8. Clinicopathological analysis of conjunctival lesions: An experience from the eastern part of Türkiye.

Abstract

Aim: The aim of our study is to determine the clinical-pathological features of conjunctival lesions operated in a tertiary-care hospital in the eastern region of our country over a 10-year period. Materials and Methods: Demographic data of the patients and histopathological findings in the conjunctival biopsy specimens that were sent to our pathology laboratory between January 2010 and July 2020 were retrospectively analyzed. Lateralization, localization, distribution rates and frequency of lesions in different histopathological subgroups according to age and gender were evaluated. Results: 286 conjunctival biopsy materials from 272 patients were analyzed. The age of the patients ranged from 1 to 95, and the mean age was 50.39±23.59. One hundred and sixteen (43%) of the patients were female and 156 (57%) were male. Of the lesions, 144 (50.3%) were located in the right eye, and 142 (49.7%) were located in the left eye. In our series, the most common lesion was pterygium with a rate of 33.9%, and the second most common conjunctival lesion was nevi with a rate of 19.2%. Nevi were the most common melanocytic lesion with a rate of 93.2%. Melanocytic lesions are the most common lesions in childhood and adolescence, with a significant decrease in incidence with increasing age. Degenerative lesions were seen in all age groups, they were most common over the age of 45 (n:76, %78). In the 46-65 age groups pterygium was the most common lesion among all groups with a rate of 40.2%. Among epithelial neoplastic lesions, benign lesions, premalignant lesions, and malignant lesions were detected with a rate %2.4 %16.4, and %3.5 respectively. Most of the lesions were located in the bulbar conjunctiva (87.4%). This was followed by tarsal (5.2%), caruncle/fornix (4.9%) and limbus (2.4%) locations, respectively. Conclusion: Melanocytic lesions are more common in the first two decades, and degenerative lesions are commonly seen after the 4th decade. In advanced ages (over 65 years), premalignant and malignant lesions are seen at a very high rate. Pterygium is accompanied by CIN with a rate of 7% and all conjunctival lesions should be directed to the pathology laboratory for histopathological examination. [ABSTRACT FROM AUTHOR]

18. Which dose of bevacizumab is more effective for the treatment of aggressive posterior retinopathy of prematurity: lower or higher dose?

Abstract

Purpose: To compare 0.5 mg and 0.625 mg of bevacizumab for treating aggressive posterior retinopathy of prematurity (AP-ROP). Methods: The medical records of patients with AP-ROP who were administered intravitreal bevacizumab (IVB) as a primary treatment at a university clinic were evaluated retrospectively. Five eyes of three patients (Group 1) who received 0.625 mg/0.025 ml IVB and 10 eyes of another five patients (Group 2) who received 0.5 mg/0.02 ml IVB were evaluated. Laser photocoagulation was used as additional treatment after relapses. Anatomic results and complications were evaluated in both groups. Results: We evaluated 15 eyes of eight patients (four girls and four boys) with a flat demarcation line at posterior zone 2 and plus disease or stage-3 disease in this study. The mean gestational age of the three babies in Group 1 was 26 ± 1 weeks and the mean birth weight was 835.33 ± 48.01 g. The corresponding values were 25.2 ± 1.6 weeks and 724 ± 139.03 g, respectively, for the five babies in Group 2. Retinal vascularization was completed at a mean postmenstrual duration of 53.6 ± 1.5 weeks without additional treatment in the five eyes in Group 1. Laser photocoagulation for relapse was administered to five of the 10 eyes in Group 2. Retinal vascularization was completed at a mean postmenstrual duration of 47.6 ± 1.5 weeks in the remaining five eyes. None of the patients developed complications such as cataract, glaucoma, retinal tear, retinal or vitreous hemorrhage, or retinal detachment. Conclusion: Although lower IVB doses in the treatment of AP-ROP are expected to be safer in terms of local and systemic side effects in premature infants, these patients may require additional treatment with IVB or laser photocoagulation. RESUMO Objetivo: Comparar doses de 0,5 mg e 0,625 mg de bevacizumab no tratamento da retinopatia da prematuridade posterior agressiva (ROP-PA). Métodos: os registros médicos de pacientes com ROP-PA que receberam bevacizumab intravítreo (IVB) como tratamento primário em uma clínica universitária foram avaliados retrospectivamente. Houve 5 olhos de 3 casos (Grupo 1) que receberam 0,625 mg/0,025 ml de IVB e 10 olhos de outros 5 casos (Grupo 2) que receberam 0,5 mg/0,02 ml de IVB. A fotocoagulação com laser foi utilizada como tratamento adicional para casos de recidiva. Os resultados e complicações anatômicas foram avaliados em ambos os grupos. Resultados: Incluímos os 15 olhos de 8 pacientes (4 meninas e 4 meninos) com linha de demarcação plana na zona posterior 2 e doença "plus" (dilatação e tortuosidade vascular) neste estudo. A idade gestacional média dos três bebês no Grupo 1 foi de 26 ± 1 semana e o peso médio ao nascer foi de 835,33 ± 48,01 g, enquanto esses valores foram de 25,2 ± 1,6 semanas e 724 ± 139,03 g, respectivamente, para os cinco bebês do Grupo 2. A vascularização da retina foi completada com uma duração média pós-menstrual de 53,6 ± 1,5 semanas sem tratamento adicional nos cinco olhos no Grupo 1. A fotocoagulação a laser foi administrada devido à recaída em 5 dos 10 olhos do Grupo 2. A vascularização da retina foi completada em média de 47,6 ± 1,5 semanas do período pós-menstrual nos cinco olhos restantes. Nenhum dos casos desenvolveu complicações, como catarata, glaucoma, rasgo da retina, hemorragia retiniana ou vítrea ou descolamento da retina. Lichen planus (LP) is an autoimmune, inflammatory disease of unknown etiology that commonly affects the skin and mucous membranes. Retinal vasculitis is a group of vision-threatening disorders, in which autoimmunity is thought to play a role in pathogenesis. We present the case of a patient who was diagnosed with retinal vasculitis and who was followed up for mucosal LP. LP has not been reported as a cause of retinal vasculitis in the literature. We believe that the retinal vasculitis in this case was related to LP because cellular immunity plays a role in the pathogenesis of both entities.

Vasculite retiniana engloba um grupo de doenças que ameaçam a visão em que a autoimunidade parece desempenhar um papel na sua patogênese. Apresentamos um caso que foi diagnosticado com vasculite da retina e que tinha sido acompanhado com LP de mucosa. LP não consta entre as causas de vasculite retiniana na literatura. Acreditamos que a vasculite da retina no nosso caso, foi relacionado com o LP considerando que a imunidade celular desempenha um papel na patogênese de ambas as entidades.

Abstract

Purpose: To evaluate the complications encountered in the postoperative period as well as anatomic results following the removal of silicone oil tamponade in patients undergoing 23 gauge pars plana vitrectomy (PPV) and silicone oil endotamponade for diabetic tractional retinal detachment. Materials and methods: Fifty-nine eyes of 54 patients who underwent surgery for diabetic tractional RD between January 2015 and April 2016 in our clinic were included in the study. The files of the cases were examined retrospectively. Age, gender, duration of silicone oil endotamponade, best corrected visual acuity (BCVA), intraocular pressure (IOP), redetachment rate and other postoperative complications were evaluated. Results: 35 of the patients were female (64.8%) and 19 were male (35.2%). The mean age was 59.1 ± 10.2 (29-80) years. The preoperative mean BCVA was 0.06 ± 6.6 (0.03-0.4), while the postoperative mean BCVA was 0.10 ± 13.3 (0.03-0.7). The preoperative and postoperative mean IOP were 16.3 ± 6.04 and 14.3 ± 4.1 mmHg, respectively. The mean duration of silicone oil endotamponade was 6.8 ± 4.01 (2-24) months. Cataract surgery was performed in 31 (52.5%) patients as removing silicone oil endotamponade. Postoperative complications were transient severe hypotony in one eye (1.7%), transient hypertony in three eyes (5.08%), vitreous hemorrhage requiring surgery, hyphema and hypopyon in one eye (1.7%) and redetachment in four eyes (6.8%). Conclusion: In patients who underwent pars plana vitrectomy and silicone oil endotamponade due to diabetic tractional retinal detachment, ocular hypotonia or hypertonia and vitreous hemorrhage may rarely develop, besides retinal redetachment, following removal of silicone oil endotamponade. 36. Prematürite Retinopatisi: İki Yıllık İzlem Sonuçlarımız

Amaç: Yenidoğan yoğun bakım ünitemizde takip ettiğimiz ve dış merkezlerden hastanemize refere edilen prematüre bebeklerde prematürite retinopatisi (PR) sıklığını saptamak ve hastalığın klinik gidişatını değerlendirmek. Gereç ve Yöntemler: Ocak 2012-Aralık 2013 tarihleri arasında Samsun Kadın Doğum ve Çocuk Hastalıkları Hastanesi yenidoğan yoğun bakım ünitesinde takibi yapılan ve ayrıca dış merkezlerden hastanemize refere edilen 34 hafta ve altında doğum haftası olan bebekler ile doğum ağırlığı 2500 gr ve daha düşük doğum ağırlıklı olan prematüre bebekler PR gelişimi açısından tarandı. Bebeklerin doğum haftaları ile doğum kiloları ve tek, ikiz ya da üçüz eşi olup olmadıkları kaydedildi. Bulgular: Tarama programına toplam 319 bebek alındı. Bebeklerin doğum haftaları 23-34 hafta (ortalama 30,7±4,2), doğum ağırlıkları ise 540-2480 (ortalama 1517,60±318,2) gr arasında değişmekte idi. Taramaya alınan bebeklerin 133'ünde (%41,6) PR saptandı. PR tespit edilen bebeklerden 70'inde (%52,6) evre 1, 22'sinde (%16,5) evre 2, 33'ünde (%24,8) evre 3 ve 8'inde (%6) agresif posterior prematürite retinopatisi (APPR) saptandı. Hiçbir bebekte evre 4 veya evre 5 PR görülmedi. Yüksek riskli eşik öncesi hastalığı olan 31 olguya ve APPR'si olan 5 olguya diod lazer fotokoagülasyon (LFK) uygulandı. LFK yapılan iki bebeğin birer gözünde retina dekolmanı gelişti. Sonuç: Prematürite retinopatisine bağlı körlükler risk grubundaki bebeklerin zamanında tanı ve tedavisi ile kısmen önlenebilir. Bu konuda ilgili tüm hekimlerin farkındalıklarının artırılması büyük önem taşımaktadır. 37. Göz Kapağında Nadir Bir Psödolenfoma: Jessner in Lenfositik İnfiltrasyonu (JLIS)

Abstract

Jessner's lymphocytic infiltration of the skin is a benign, unusual skin disorder with unknown etiology. It mostly affects middle-aged adults and very rarely occurs in children. It is typically localized on the head, neck, upper back. The eruption resolves spontaneously after months or a few years but can usually recur in several years. Benign hyperplastic lymphoid pathologies, mainly cutaneous lupus erythematosis with quite similar skin lesions, and malignant lymphoma should be considered in differential diagnosis. Treatment of Jessner's lymphocytic infiltration is unsatisfactory and a variety of empirical treatments with limited success has been tried. In this study, we aimed to report a rare case of Jessner's lymphocytic infiltration on and around the eyelid of a thirty-nine-year-old male patient. Key Word: Jessner's Lymphocytic İnfiltration; Eyelid; Pseudolymphoma., Jessner'in lenfositik infiltrasyonu derinin etyolojisi bilinmeyen nadir, benign bir hastalığıdır. Sıklıkla orta yaştaki bireylerde baş, boyun ve gövdenin üst kısmında görülmektedir. Çocuklar nadiren etkilenmektedir. Aylarca ya da yıllarca sebat edebilen lezyonlar, kendiliğinden gerileyebilmekte ve genellikle birkaç yıl içinde nüksetme eğilimindedir. Deri lezyonlarının oldukça benzerlik gösterdiği kutanöz lupus eritematozis başta olmak üzere benign lenfoid hiperplastik patolojiler ve malign lenfoma ayırıcı tanıda düşünülmelidir. Jessner'in lenfositik infiltrasyonunun tedavisi tatmin edici değildir ve tedavide sınırlı yararı olan ampirik yöntemler denenmiştir. Bu yazıda nadir bir ortaya çıkış şekli olarak göz kapağı ve çevresine sınırlı lezyonları olan ve Jessner'in lenfositik infiltrasyonu tanısı alan otuz dokuz yaşındaki erkek hastayı sunduk. 43. Surgical results in cases with subluxated cataract and pseudoexfoliation syndrome

Abstract

AMAÇ: Senil katarakt, psödoeksfoliasyon (PSX) ve sublukse lensi olan hastalarda cerrahi seçenekleri, görme keskinliği değişimi ve komplikasyonları değerlendirmek.GEREÇ-YÖNTEM: Kliniğimizde Ocak 2005 – Mart 2009 tarihleri arasında senil katarakt, psödoeksfoliasyon ve sublukse lensi olan ve cerrahi tedavi yapılan 50 olgunun 50 gözü retrospektif olarak değerlendirildi. Uygulanan cerrahi tekniklerin komplikasyonlar ve görme keskinliği (GK) üzerine etkileri araştırıldı.BULGULAR: Senil kataraktı, psödoeksfoliasyon ve sublukse lensi bulunan 50 olgunun 25'i kadın, 25'i erkek idi. Olguların yaş ortalaması 76.06±10.4 yıl (45-100) olarak saptandı. Bir hastaya intrakapsüler katarakt cerrahisi (İKKE), 24 hastaya ekstrakapsüler katarakt cerrahisi (EKKE), 25 hastaya ise fakoemülsifikasyon cerrahisi (FE) uygulandı. Ekstrakapsüler katarakt cerrahisi yapılan, yeterli kapsüler desteği olmayan 5 göz afak bırakılırken, 18 gözde arka kamara (12 kese içi, 6 sulkus) ve bir gözde ön kamara göz içi mercek (GİM) yerleştirildi. Fakoemülsifikasyon cerrahisi yapılan bir göz afak bırakılırken 21'inde arka kamara (18 kese içi, 3 sulkus), 3'ünde ön kamara GİM implante edildi. Fakoemülsifikasyon yapılan 9 göze kapsül germe halkası (KGH) yerleştirildi. Snellen eşeli ile ölçülen en iyi düzeltilmiş görme keskinliği (EDGK) EKKE cerrahisi uygulanan grupta preoperatif ışık hissi (p+p+) ile 0.2 arasında değişirken, postoperatif ışık hissi (p+p+) ile 0.9 arasında değişmekteydi ve 3 (%12.5) gözde postoperatif EDGK 0.5 ve üzerinde saptandı. Fakoemülsifikasyon cerrahisi uygulanan grupta ise preoperatif EDGK ışık hissi (p+p+) ile 0.4 arasında değişirken, postoperatif 2 mps ile tam arasında değişmekteydi ve 15 (%60) gözde postoperatif EDGK 0.5 ve üzeri olarak tespit edildi. Ekstrakapsüler katarakt cerrahisi yapılan olgulardan 8 (%33.3) gözde arka kapsül rüptürü ve vitreus kaybı görülürken, FE cerrahisi yapılan olgulardan 7 (%28) gözde arka kapsül rüptürü görüldü ve bunların 6'sında (%24) vitreus kaybı da mevcuttu. 45. Retinopathy of Prematurity: Results of a Two-Years Follow-Up.

Abstract

Objective: To determine the frequency of retinopathy of prematurity (ROP) and the clinical course of the disease in premature infants that were referred to our hospital and followed in our premature intensive care unit. Material and Methods: The premature infants with a gestational age of <34 weeks and a birth weight of <2500 gr that were referred to our hospital and hospitalized in Samsun Maternity and Pediatrics Hospital, Neonatal Intensive Care Unit between January 2012 and December 2013 were studied in terms of ROP development. They were recorded as singletons, twins, or triplets with their gestational ages and birth weights. Results: In the study, 319 premature infants were screened and ROP was found in 133 (41,6%) infants. The infants's gestational weeks were between 23-34 (mean:30,7±4,2) while their birth weights were between 540-2480 (mean:1517,60±318,2) g. Of the 133 infants who had ROP, 70 (52,6%) had stage 1 ROP, 22 (16,5%) had stage 2 ROP, 33 (24,8%) had stage 3 ROP and 8 (6%) had aggressive posterior ROP (APROP) while none of the infants had stage 4 or stage 5 ROP. Diode laser photocoagulation (LP) was performed on 31 cases with high risk of prethreshold ROP and on 5 cases with APROP. Retinal detachment developed in two eyes which LP was performed. Conclusion: ROP associated blindness can be partly prevented with timely diagnosis and treatment of babies who are in risk group. 46. Kazara Lens İçi Deksametazon İmplant Enjeksiyonu: İki Olgu Sunumu.

Abstract

An intravitreal dexamethasone implant (Ozurdex®, Allergan, USA) is currently commonly used for the treatment of macular edema due to causes such as retinal vein occlusion (RVO) and also diabetes and uveitis. We present two cases where a dexamethasone implant planned to be placed intravitreally for macular edema due to retinal vein occlusion was instead placed into the lens body by mistake as a rare complication of intravitreal dexamethasone implant injection. Possible causes of the inadvertent injection into the lens body include physician carelessness, patient incompliance, type of local anesthesia, the distance of the injection area to the limbus, hypotonia, weakness of the sclera, damage in the zonules holding the lens due to repeated injections, and a change in lens position. A good knowledge of injection technique, especially during procedures involving implants, is required to avoid this rare complication. [ABSTRACT FROM AUTHOR]

Abstract

After radiological imaging analysis, a mass extending into the orbit was detected in a 10-year-old girl wirh complaints ofswelling and protrusion in the right eye. The mass was excised by the neurosurgery department through transcranial approach and found to be ossifying fibroma in histopathological examination. The case, presenting wirh head position and diplopia as well as trochlear swelling in physical examination in the posroperative firsr week, was diagnosed with canine tooth syndrome developing after surgical trauma. Abstract

Jessner's lymphocytic infiltration of the skin is a benign, unusual skin disorder with unknown etiology. It mostly affects middle-aged adults and very rarely occurs in children. It is typically localized on the head, neck, upper back. The eruption resolves spontaneously after months or a few years but can usually recur in several years. Benign hyperplastic lymphoid pathologies, mainly cutaneous lupus erythematosis with quite similar skin lesions, and malignant lymphoma should be considered in differential diagnosis. Treatment of Jessner's lymphocytic infiltration is unsatisfactory and a variety of empirical treatments with limited success has been tried. In this study, we aimed to report a rare case of Jessner's lymphocytic infiltration on and around the eyelid of a thirty-nine-year-old male patient. [ABSTRACT FROM AUTHOR]
