3. M/M/$\infty$ queues in semi-Markovian random environment

In this paper we investigate an M/M/$\infty$ queue whose parameters depend on an external random environment that we assume to be a semi-Markovian process with finite state space. For this model we show a recursive formula that allows to compute all the factorial moments for the number of customers in the system in steady state. The used technique is based on the calculation of the raw moments of the measure of a bidimensional random set. Finally the case when the random environment has only two states is deeper analyzed. We obtain an explicit formula to compute the above mentioned factorial moments when at least one of the two states has sojourn time exponentially distributed., Comment: 17 pages, 2 figures

In this paper we analyze of a special kind of M/M/[infinity] queueing system. We assume that the rate of the servers alternates between two speeds according to an ON-OFF process whose ON periods are exponentially distributed and with general OFF periods. We look at the particular case where the OFF service rate is zero while the distribution of the OFF periods is heavy tailed, and for this case we derive the tail of the number of customers in the system. Infections are a major complication of solid organ transplants (SOTs). This study aimed to describe recipients’ characteristics, and the frequency and etiology of infections and transplant outcome in liver and lung SOTs, and to investigate exposures associated to infection and death in liver transplant recipients. The study population included recipients of SOTs performed in Italy during a 1-year period in ten Italian lung transplant units and eight liver transplant units. Data on comorbidities, infections, retransplantation, and death were prospectively collected using a web-based system, with a 6-month follow-up. The cumulative incidence of infection was 31.7% and 47.8% in liver and lung transplants, respectively, with most infections occurring within the first month after transplantation. Gram-negatives, which were primarily multidrug-resistant, were the most frequent cause of infection. Death rates were 0.42 per 1000 recipient-days in liver transplants and 1.41 per 1000 recipient-days in lung transplants. Infection after SOT in adult liver recipients is associated to an increased risk of death (OR = 13.25; p-value < 0.001). Given the frequency of infection caused by multidrug-resistant microorganisms in SOT recipients in Italy and the heavy impact of infections on the transplant outcome, the reinforcement of surveillance and control activities to prevent the transmission of multidrug-resistant microorganisms in SOT recipients represents a priority. The implementation of the study protocol in liver and lung transplant units and the sharing of results have increased the awareness about the threat due to antimicrobial resistance in the country.

We study the sojourn time in a queueing system with a single exponential server, serving a Poisson stream of customers in order of arrival. Service is provided at low or high rate, which can be adapted at exponential inspection times. When the number of customers in the system is above a given threshold, the service rate is upgraded to the high rate, and otherwise, it is downgraded to the low rate. The state dependent changes in the service rate make the analysis of the sojourn time a challenging problem, since the sojourn time now also depends on future arrivals. We determine the Laplace transform of the stationary sojourn time and describe a procedure to compute all moments as well. First we analyze the special case of continuous inspection, where the service rate immediately changes once the threshold is crossed. Then we extend the analysis to random inspection times. This extension requires the development of a new methodological tool, that is "matrix generating functions". The power of this tool is that it can also be used to analyze generalizations to phase-type services and inspection times. An open exponential queueing network with signals and impatient service is considered. Upon completion of service at a node, a positive customer passes to another node with fixed probabilities either as a positive customer or as a signal, or quits the network. Every signal is activated during a random exponentially distributed amount of time. Activated signals with fixed probabilities either move a customer from the node they arrive to another node or kill a positive customer. Each customer can be served in a node at most a random time ("patient" time) distributed exponentially. When the patient service is finished, the customer with fixed probabilities either goes to another node or quits the network. The stationary state probabilities for such a C-network in which positive customers are processed in each node by a single server is derived in product form. The solution for an analogous symmetrical G-network in which service rate of a positive customer at each node depends on the number of positive customers in this node is expressed in product form too.

Objectives A prospective cohort study was conducted in Italy in order to describe the microbiologic aspects of colonization/infection by carbapenemase-producing Enterobacteriaceae (CPE) in donors and recipients of lung and liver transplants and the possible CPE transmission from donors to recipients. Methods Between 15 January 2014 and 14 January 2015, all recipients of solid organ transplants (SOT) at ten lung and eight liver transplantation centres and the corresponding donors were enrolled. Screening cultures to detect CPE were performed in donors, and screening and clinical cultures in recipients with a 28-day microbiologic follow-up after receipt of SOT. Detection of carbapenemase genes by PCR, genotyping by multilocus sequence typing, and pulsed-field gel electrophoresis and whole-genome sequencing were performed. Results Of 588 screened donors, 3.4% were colonized with CPE. Of the liver first transplant recipients (n = 521), 2.5% were colonized before receipt of SOT and 5% acquired CPE during follow-up. CPE colonization was higher in lung first transplant recipients (n = 111, 2.7% before SOT and 14.4% after SOT). CPE infections occurred in 1.9% and 5.3% of liver or lung recipients, respectively. CPE isolates were mostly Klebsiella pneumoniae carbapenemase (KPC)-producing K. pneumoniae belonging to CG258. Three events of donor–recipient CPE transmission, confirmed by whole-genome sequencing and/or pulsed-field gel electrophoresis, occurred in lung recipients: two involving K. pneumoniae sequence type 512 and one Verona integron-encoded metallo-β-lactamase (VIM)-producing Enterobacter aerogenes. Conclusions This study showed a low risk of donor–recipient CPE transmission, indicating that donor CPE colonization does not necessarily represent a contraindication for donation unless colonization regards the organ to be transplanted. Donor and recipient screening remains essential to prevent CPE transmission and cross-infection in transplantation centres.

The single-machine scheduling problem (SMSP) with release dates concerns the optimal allocation of a set of jobs on a single machine that is not able to process more than one job at a time. Each job is ready to be processed at a release date and without interruption. The goal is to minimize the total weighted completion time of the jobs. In this paper the time-indexed formulation is considered and a new lagrangean heuristic is proposed, based on the observation that lagrangean relaxation of job constraints leads to a weighted stable set problem on an interval graph. The relaxed problem is polynomially solvable by a dynamic-programming algorithm. We report computational experience, showing that instances up to 400 jobs and maximum processing time 50 (around 5,000,000 variables) are solved in less than 40 minutes on a personal computer, yielding duality gaps never exceeding 3%. We also test a set of hard instances, built to produce bad performances where we yield duality gaps less than 5%.

- Abstract
The anticipative information refers to some information about future events that may be disclosed in advance. This information may regard, for example, financial assets and their future trends. In our paper, we assume the existence of some anticipative information in a market whose risky asset dynamics evolve according to a Brownian motion and a Poisson process. Using Malliavin calculus and filtration enlargement techniques, we derive the information drift of the mentioned processes and, both in the pure jump case and in the mixed one, we compute the additional expected logarithmic utility. - Abstract
Within the well-known framework of financial portfolio optimization, we analyze the existing relationships between the condition of arbitrage and the utility maximization in presence of insider information. We assume that, since the initial time, the information flow is altered by adding the knowledge of an additional random variable including future information. In this context we study the utility maximization problem under the logarithmic and the Constant Relative Risk Aversion (CRRA) utilities, with and without the restriction of no temporary-bankruptcy. In particular, we show that the value of the insider information may be bounded while the arbitrage condition holds and we prove that the insider information does not always imply arbitrage for the insider by providing an explicit example.

- Abstract
Objective: The World Health Organization, the United Nations Children's Fund, and the International Council for the Control of Iodine Deficiency Disorders recommend a median urinary iodine concentration (UIC) in pregnant women between 150 µg/L and 249 µg/L. In the present study, we evaluated whether in the urban area of Cassino (central Italy), after a national salt iodination program (30 mg/kg) was introduced in 2005, the increased demand of iodine during pregnancy was satisfied., Methods: Between January 2016 and April 2017, 99 pregnant women were enrolled to evaluate UIC in spot urine samples, serum level of thyrotropin, free thyroxine, antithyroglobulin and antithyroperoxidase autoantibodies, and thyroid volume by ultrasonography. Eighty clinically healthy non-pregnant women were evaluated as controls., Results: The median UIC was of 97.7 µg/L and 110.3 µg/L, respectively, in control and pregnant women. A significant increase (P < 0.001) of median thyroid volume was found in pregnant women, relative to control women, being, respectively, 10.4 mL (range 3.68-19.49 mL) and 7.16 mL (range 2.57-14.00 mL). A positive correlation was found between thyroid volume and anthropometric parameters, and an inverse correlation was identified between free thyroxine serum levels and anthropometric parameters., Conclusions: This observational study found that the majority of pregnant women and their fetuses appear not to be protected from the detrimental consequences of iodine deficiency. Therefore, the identification of new strategies to increase the knowledge and awareness of the general population regarding the beneficial effects of iodine supplementation during pregnancy is highly required., (Copyright © 2017 Elsevier Inc.
