Background Improving survival and extending the longevity of life for all populations requires timely, robust evidence on local mortality levels and trends. The Global Burden of Disease 2015 Study (GBD 2015) provides a comprehensive assessment of all-cause and cause-specific mortality for 249 causes in 195 countries and territories from 1980 to 2015. These results informed an in-depth investigation of observed and expected mortality patterns based on sociodemographic measures. Methods We estimated all-cause mortality by age, sex, geography, and year using an improved analytical approach originally developed for GBD 2013 and GBD 2010. Improvements included refinements to the estimation of child and adult mortality and corresponding uncertainty, parameter selection for under-5 mortality synthesis by spatiotemporal Gaussian process regression, and sibling history data processing. We also expanded the database of vital registration, survey, and census data to 14 294 geography-year datapoints. For GBD 2015, eight causes, including Ebola virus disease, were added to the previous GBD cause list for mortality. We used six modelling approaches to assess cause-specific mortality, with the Cause of Death Ensemble Model (CODEm) generating estimates for most causes. We used a series of novel analyses to systematically quantify the drivers of trends in mortality across geographies. First, we assessed observed and expected levels and trends of cause-specific mortality as they relate to the Socio-demographic Index (SDI), a summary indicator derived from measures of income per capita, educational attainment, and fertility. Second, we examined factors affecting total mortality patterns through a series of counterfactual scenarios, testing the magnitude by which population growth, population age structures, and epidemiological changes contributed to shifts in mortality. Finally, we attributed changes in life expectancy to changes in cause of death. We documented each step of the GBD 2015 estimation processes, as well as data sources, in accordance with Guidelines for Accurate and Transparent Health Estimates Reporting (GATHER). Findings Globally, life expectancy from birth increased from 61.7 years (95% uncertainty interval 61.4-61.9) in 1980 to 71.8 years (71.5-72.2) in 2015. Several countries in sub-Saharan Africa had very large gains in life expectancy from 2005 to 2015, rebounding from an era of exceedingly high loss of life due to HIV/AIDS. At the same time, many geographies saw life expectancy stagnate or decline, particularly for men and in countries with rising mortality from war or interpersonal violence. From 2005 to 2015, male life expectancy in Syria dropped by 11.3 years (3.7-17.4), to 62.6 years (56.5-70.2). Total deaths increased by 4.1% (2.6-5.6) from 2005 to 2015, rising to 55.8 million (54.9 million to 56.6 million) in 2015, but age-standardised death rates fell by 17.0% (15.8-18.1) during this time, underscoring changes in population growth and shifts in global age structures. The result was similar for non-communicable diseases (NCDs), with total deaths from these causes increasing by 14.1% (12.6-16.0) to 39.8 million (39.2 million to 40.5 million) in 2015, whereas age-standardised rates decreased by 13.1% (11.9-14.3). Globally, this mortality pattern emerged for several NCDs, including several types of cancer, ischaemic heart disease, cirrhosis, and Alzheimer's disease and other dementias. By contrast, both total deaths and age-standardised death rates due to communicable, maternal, neonatal, and nutritional conditions significantly declined from 2005 to 2015, gains largely attributable to decreases in mortality rates due to HIV/AIDS (42.1%, 39.1-44.6), malaria (43.1%, 34.7-51.8), neonatal preterm birth complications (29.8%, 24.8-34.9), and maternal disorders (29.1%, 19.3-37.1). Progress was slower for several causes, such as lower respiratory infections and nutritional deficiencies, whereas deaths increased for others, including dengue and drug use disorders. Age-standardised death rates due to injuries significantly declined from 2005 to 2015, yet interpersonal violence and war claimed increasingly more lives in some regions, particularly in the Middle East. In 2015, rotaviral enteritis (rotavirus) was the leading cause of under-5 deaths due to diarrhoea (146 000 deaths, 118 000-183 000) and pneumococcal pneumonia was the leading cause of under-5 deaths due to lower respiratory infections (393 000 deaths, 228 000-532 000), although pathogen-specific mortality varied by region. Globally, the effects of population growth, ageing, and changes in age-standardised death rates substantially differed by cause. Our analyses on the expected associations between cause-specific mortality and SDI show the regular shifts in cause of death composition and population age structure with rising SDI. Country patterns of premature mortality (measured as years of life lost [YLLs]) and how they differ from the level expected on the basis of SDI alone revealed distinct but highly heterogeneous patterns by region and country or territory. Ischaemic heart disease, stroke, and diabetes were among the leading causes of YLLs in most regions, but in many cases, intraregional results sharply diverged for ratios of observed and expected YLLs based on SDI. Communicable, maternal, neonatal, and nutritional diseases caused the most YLLs throughout sub-Saharan Africa, with observed YLLs far exceeding expected YLLs for countries in which malaria or HIV/AIDS remained the leading causes of early death. Interpretation At the global scale, age-specific mortality has steadily improved over the past 35 years; this pattern of general progress continued in the past decade. Progress has been faster in most countries than expected on the basis of development measured by the SDI. Against this background of progress, some countries have seen falls in life expectancy, and age-standardised death rates for some causes are increasing. Summary Objective To evaluate the efficacy of voluntary counselling and testing (VCT) for HIV/AIDS in changing risky sexual behaviour in central Mozambique. Method Longitudinal cohort study of men and women aged at least 18 years from October 2002 to June 2003. We interviewed 622 participants in VCT groups and 598 in non-VCT groups. The interviews occurred before counselling and 4 and 6 months afterwards. Results Reported use of condoms while having sex with a friends/prostitute increased over each time period in the VCT group and between baseline and first visit in the non-VCT group. Both men and women in the VCT group increased their condom use over time, but the women in the non-VCT group did not. Reported always/sometimes use of condoms for both literate and illiterate subjects was higher and rose over time in the VCT group. Conclusion People who undergo voluntary counselling and testing fro HIV/AIDS change their behaviour, presumably as a result of their counselling. Objectif Evaluer l'efficacite de la consultation et du test volontaire pour le VIH/SIDA dans le changement du comportement sexuel a risque dans le centre du Mozambique. Methode Une etude longitudinale de cohorte d'hommes et de femmes ages d'au moins 18 ans, d'octobre a juin 2003. Nous avons interviewe 622 participants au sein de groupes de consultation et test volontaires (CTV) et 598 participants sans CTV. Les interviews ont eu lieu avant la consultation puis a quatre mois et a six mois apres la consultation. Resultats Le nombre de reports d'utilisation de preservatifs lors de rapport sexuel avec un(e) ami(e) ou avec une prostituee augmentait au cours de chacune des periodes dans le groupe avec CTV et entre le debut et la premiere visite dans le groupe sans CTV. Hommes comme femmes dans le groupe avec CTV ont augmente leur utilisation de preservatifs au cours du temps. Mais cela n’etait pas le cas chez les femmes dans le groupe sans CTV. L‘utilisation «toujours ou parfois» de preservatifs etait plus elevee et a augmente au cours du temps dans le groupe avec CTV autant chez les pesonnes lettrees que chez les illettrees. Conclusion Les personnes qui prennent part aux consultations et tests volontaires pour le VIH/SIDA changent leur comportement sexuel, grâce probablement a la consultation. Objetivo Evaluar la eficacia del Aconsejamiento Voluntario y Prueba (AVP) de VIH/SIDA en el cambio del comportamiento sexual de riesgo, en la region central de Mozambique. Metodos Estudio de cohorte longitudinal de hombres y mujeres de al menos 18 anos de edad, entre octubre del 2002 y Junio del 2003. Entrevistamos 622 participantes en el grupo de Aconsejamiento Voluntario y Prueba (AVP) y 598 en el grupo de No-AVP. Las entrevistas ocurrieron antes del aconsejamiento y 4 y 6 meses despues del mismo. Resultados El uso de preservativos en contactos sexuales con amistades/prostitutas aumento en cada periodo de tiempo en el grupo AVP, y entre el valor basal y la primera visita en el grupo No-AVP. Tanto hombres como mujeres en el grupo AVP aumentaron su uso de preservativos en el tiempo, pero no en las mujeres del grupo No-AVP. El reporte de uso de preservativos siempre/algunas veces fue mas alto y aumento en el tiempo en el grupo AVP tanto para sujetos alfabetos como para analfabetos Conclusion Las personas que participaron en el aconsejamiento voluntario y examen de VIH/SIDA cambian su comportamiento, presumiblemente como resultado del aconsejamiento.

The perinatal and maternal outcomes of 65 symphysiotomies and 108 caesarean sections carried out after failed trial of assisted delivery at the Port Moresby General Hospital between 1988 and 1994 were retrospectively analyzed. There were no significant differences in perinatal outcomes between the treatment groups. There were no maternal deaths in either group. Mothers who had symphysiotomy had a longer postoperative stay in hospital but fewer complications requiring further surgery. There are many advantages of symphysiotomy, particularly in developing countries, following a failed trial of assisted delivery, provided the indications for it are strictly met. Obstetricians experienced in the technique are able to apply it at the optimal time, with long-term benefit to their patients, who thereby avoid the risks of pregnancy subsequent to caesarean section.A retrospective analysis of 65 symphysiotomies and 108 cesarean sections performed in 1988-94 after a failed trial of assisted delivery at the Port Moresby General Hospital (Papua New Guinea) revealed no significant differences in perinatal or maternal outcomes. There were no significant differences between groups in terms of duration of first and second-stage labor, Apgar scores, admission to a special care neonatal unit, or perinatal mortality. There were no maternal deaths. Mothers who had symphysiotomy required a longer hospital stay, but had fewer complications necessitating additional surgery (e.g., wound infection) than women delivered by cesarean section. These findings confirm that, with proper technique and selection of cases, symphysiotomy can both eliminate difficult vaginal deliveries and reduce maternal morbidity and mortality. Indications for this procedure include presentation of the vertex, moderate cephalopelvic disproportion, and a live fetus. The main complications are leg and pelvic pain, pelvic instability, and stress incontinence.

13. Postoperative Hypofractionated Whole Breast Radiotherapy in Patients with Early Stage Breast Cancer: Interim Cosmetic and Toxicity Evaluation

Results: Cosmetic results were rated as good/excellent in 92% of the patients with more than 1-year of follow-up (excellent 74%/ good 18%). The LENT-SOMA Erythema G1 was described in 30 pts (46%) and G2 in 18 pts (28%); hyper pigmentation G1 in 20 (31%), G2 in 10 (15%); telangiectasia G1 in 15 (23%); fibrosis G1 in 5 (85%), and G2 in 3 (4%). No local recurrences have been recorded to date. Conclusions: In our experience, 3D-conformal postoperative hypofractionated radiotherapy resulted in good/excellent results in 92% of patients after 15-months mean follow-up, better than others reported with the same hypofractionated schedule (Shelley W et al., IJROBP 2000; Whelan T et al., JNCI 2002). We suggest a selection of patients clinically eligible to WBhRT based on dosimetric parameters of OARs (skin, subcutaneous tissue, residual breast parenchyma) to avoid an increased late worse cosmesis. Additional follow-up is requested to evaluate long-term toxicity and its impact on local control.

Background: Processing of linguistic negation has been associated to inhibitory brain mechanisms. However, no study has tapped this link via multimodal measures in patients with core inhibitory alterations, a critical approach to reveal direct neural correlates and potential disease markers., Methods: Here we examined oscillatory, neuroanatomical, and functional connectivity signatures of a recently reported Go/No-go negation task in healthy controls and behavioral variant frontotemporal dementia (bvFTD) patients, typified by primary and generalized inhibitory disruptions. To test for specificity, we also recruited persons with Alzheimer's disease (AD), a disease involving frequent but nonprimary inhibitory deficits., Results: In controls, negative sentences in the No-go condition distinctly involved frontocentral delta (2-3 Hz) suppression, a canonical inhibitory marker. In bvFTD patients, this modulation was selectively abolished and significantly correlated with the volume and functional connectivity of regions supporting inhibition (e.g. precentral gyrus, caudate nucleus, and cerebellum). Such canonical delta suppression was preserved in the AD group and associated with widespread anatomo-functional patterns across non-inhibitory regions., Discussion: These findings suggest that negation hinges on the integrity and interaction of spatiotemporal inhibitory mechanisms. Moreover, our results reveal potential neurocognitive markers of bvFTD, opening a new agenda at the crossing of cognitive neuroscience and behavioral neurology., (© The Author(s) 2022. - Abstract
The COVID-19 pandemic (and its aftermath) highlights a critical need to communicate health information effectively to the global public. Given that subtle differences in information framing can have meaningful effects on behavior, behavioral science research highlights a pressing question: Is it more effective to frame COVID-19 health messages in terms of potential losses (e.g., "If you do not practice these steps, you can endanger yourself and others") or potential gains (e.g., "If you practice these steps, you can protect yourself and others")? Collecting data in 48 languages from 15,929 participants in 84 countries, we experimentally tested the effects of message framing on COVID-19-related judgments, intentions, and feelings. Loss- (vs. gain-) framed messages increased self-reported anxiety among participants cross-nationally with little-to-no impact on policy attitudes, behavioral intentions, or information seeking relevant to pandemic risks. These results were consistent across 84 countries, three variations of the message framing wording, and 560 data processing and analytic choices. Thus, results provide an empirical answer to a global communication question and highlight the emotional toll of loss-framed messages. - Abstract
The COVID-19 pandemic has increased negative emotions and decreased positive emotions globally. Left unchecked, these emotional changes might have a wide array of adverse impacts. To reduce negative emotions and increase positive emotions, we tested the effectiveness of reappraisal, an emotion-regulation strategy that modifies how one thinks about a situation. Participants from 87 countries and regions (n = 21,644) were randomly assigned to one of two brief reappraisal interventions (reconstrual or repurposing) or one of two control conditions (active or passive). Results revealed that both reappraisal interventions (vesus both control conditions) consistently reduced negative emotions and increased positive emotions across different measures. Reconstrual and repurposing interventions had similar effects. Importantly, planned exploratory analyses indicated that reappraisal interventions did not reduce intentions to practice preventive health behaviours. The findings demonstrate the viability of creating scalable, low-cost interventions for use around the world. - Abstract
Aims and Background: To present the Italian state-of-the-art contribution to radiobiology of external beam radiotherapy, brachytherapy, and radionuclide radiotherapy., Methods and Study Design: A survey of the literature was carried out, using PubMed, by some independent researchers of the Italian group of radiobiology. Each paper was reviewed by researchers of centers not comprising its authors. The survey was limited to papers in English published over the last 20 years, written by Italian investigators or in Italian institutions, excluding review articles., Results: A total of 135 papers have been published in journals with an impact factor, with an increase in the number of published papers over time, for external beam radiotherapy rather than radionuclide radiotherapy. The quantity and quality of the papers researched constitutes a proof of the enduring interest in clinical radiobiology among Italian investigators., Conclusions: The survey could be useful to individuate expert partners for an Italian network on clinical radiobiology, addressing future collaborative investigations.
