Plasma immunoreactive secretin levels were measured in 22 newly diagnosed non-ketotic, maturity-onset diabetics and 10 healthy control subjects during 50 gram oral glucose tolerance test (OGTT). The test was repeated in the diabetic group after 6 months' dietary treatment. At diagnosis the fasting secretin levels in the diabetics were higher than in control subjects but fell within the normal range following dietary treatment. In the diabetics there was a significant positive correlation between the fasting glucose and fasting secretin levels at the time of diagnosis. Suppression of secretin levels occurred during the OGTT, both in diabetic and control subjects.

Serum cholesterol and high-density lipoprotein (HDL) cholesterol concentrations were measured in 192 diabetics (94 with juvenile-onset and 98 with maturity-onset diabetes) and 177 non-diabetic controls. Hb A1C, an index of blood sugar control, was also measured in the diabetics. Serum cholesterol concentrations were similar in all the diabetics and controls, but HDL cholesterol concentrations were lower in patients with maturity-onset diabetes than in those with juvenile-onset diabetes and controls. There was no correlation in diabetics between HDL cholesterol and Hb A1C. We conclude that HDL cholesterol concentrations are abnormally low in patients with maturity-onset diabetes but essentially normal in those with juvenile-onset diabetes. They are not related to diabetic control.

The circadian rhythms of preprandial and postprandial blood glucose, free fatty acids, and insulin levels were measured in groups of normal and of diabetic patients controlled by diet alone or by diet with tolbutamide, chlorpropamide, or metformin. All groups showed a greater insulin release after the first meal of the day than after subsequent meals. Diabetic patients controlled for more than two years on diguanide therapy showed the closest resemblance to non-diabetic control subjects. There was no significant difference between the values for diabetic patients on long-term control with diet alone and those on short- or long-acting sulphonylurea. The circadian rhythms disappeared in obese subjects on total starvation.

In 1956 Stevenson published the results of 119 diabetic pregnancies treated in the Royal Maternity Hospital, Belfast, during the 16-year period 1940 to 1955. As a result of his findings he concluded that some improvement in the foetal mortality should be possible and recommended (1) that there should be the closest co-operation between obstetrician, physi cian, and paediatrician ; (2) strict antenatal care from the beginning of pregnancy ; (3) admission to hospital at the twenty-eighth week, or earlier if stabilization is unsatisfactory ; (4) premature delivery at 35 weeks, or earlier if indicated ; (5) delivery by caesarean section in all primigravidae and all but a very few multigravidae ; and (6) skilled nursing of the neonate in a modern nursery unit. For the past eight years the management of diabetic preg nancies in the Roy? Maternity Hospital, Belfast, has been influenced to some extent by these recommendations. Con sequently it was decided to carry out a further review to see if there had been any improvement in the foetal mortality and whether or not any alterations were necessary in our present management.

Intestinal fat absorption has been studied in fifty-six patients by a simplified Kaihara and Wagner technique using14C labelled triglycerides. The peak radioactivity in the expired air is attained in the majority of patients between five and seven hours after administration of the dose. The highest of these three one hourly readings of14CO2 below 30×10−4 per cent of the14C−T.G. dose per m.mol of exhaled CO2 is consistent with steatorrhoea provided there is no disturbance of lipid or general metabolism. The test is well tolerated and easy to carry out but is not reliable in the presence of obesity and uncontrolled thyrotoxicosis.

Results of glucose tolerance tests in a selected group of 1718 patients studied at the thirty-second week of gestation are related, a) mainly to the maternal weight, and b) in part to the maternal age, foetal maturity at delivery and foetal weight at delivery. No clear preference between the three tests emerges, but the K value (glucose disappearance rate after I.V. glucose) appears to be most sensitive to the variables studied. It is not possible to predict the foetal outcome with information obtained from a glucose tolerance test carried out at the thirty-second week of gestation.

The incidence of secondary failure was 5.6 per cent, in 784 patients treated with sulphonylureas during a period of 7 years. Not all of those who relapsed needed insulin and half were successfully re-established with a combination of diguanide and sulphonylurea. Secondary failure sometimes becoming permanent may develop as the result of metabolic stress. Some of the factors contributing to the incidence of secondary failure are discussed, and it is concluded that poor primary selection and inadequate dietary adherence have contributed to the high incidence reported by some workers previously.

PERSISTENT choreo-athetotic movements are not widely recognized as being associated with portal cirrhosis of the liver although such movements are a feature of many cases of Wilson's disease (hepatolenticular degeneration). The most characteristic neurological abnormality in hepatic cirrhosis is a "flapping" tremor (asterixis). Sherlock 13 has described this and the occurrence of other neuropsychiatric disturbances in her monograph. The flapping tremor and organic mental state are greatly influenced by variations in the blood levels of substances arising from the impaired deamination of protein, and improve or disappear on a diet free from protein and reappear when dietary protein is increased. In their comprehensive communication on the neurological disorder associated with liver disease, Adams and Foley 1 stated that they found no instance of athetosis or chorea though repeated grimacing with peculiar lapses of voluntary movements were commonly present. The mean normal values of right hand volume and skin thickness in both sexes and in each decade (from 20–69 years) in right-handed subjects are presented. These measurements vary with age and sex. Hand volume and skin thickness showed a decrease in normal subjects above 50 years. In acromegaly there is an increase in hand volume and skin thickness, but this does not closely relate to other estimates of activity of the disease.

A retrospective survey of incidents of myocardial infarction and causes of death in 670 maturity-onset female diabetic patients aged forty to seventy-nine years attending a hospital diabetes clinic during 1958-70 is reported. There were changes in treatment of individual patients (determined on clinical criteria) from dietary management alone to oral hypoglycaemic agents or insulin, so the incidence of myocardial infarction has been analysed in terms of patient-years for each form of treatment. There were 1.52 myocardial infarctions per 100 patient-years for dietary treatment alone and 3.06 per 100 patient-years for treatment by diet with oral hypoglycaemic agents.

In hyperthyroidism suppression and non-suppression of early radioiodine neck uptake by exogenous thyroid hormone after a course of antithyroid drugs does not indicate clearly those patients who will eventually relapse or have a remission. Sixty-four hyperthyroid patients have been followed up for two years after an 18 to 24 months9 course of carbimazole. Twenty-eight patients had suppressed at the end of the carbimazole course and 20 of these remained in remission for two years, and 36 were non-suppressed and 21 of these relapsed. Patients with the highest neck uptakes relapsed soonest after stopping treatment. Pronounced rebound occurred in neck uptakes at one month after stopping treatment, both in patients who relapsed and in those who remained in remission. By one year after stopping treatment those who remained in remission had shown a further fall in early neck uptake. In two patients who relapsed after exposure to stressful events no change in 20-minute radioiodine neck uptakes was found in relation to the supposed stress.

An investigation was made into the prevalence of symptoms suggestive of partial hypoparathyroidism in 154 patients submitted to subtotal thyroidectomy between January 1954 and December 1963. The assessment was made by means of a physical examination, a questionnaire and serum calcium estimation. Control calcium studies were performed in 40 hospital patients simultaneously. One hundred and twenty-seven patients were available for study and their parathyroid function was divided into three grades

The long term results (over 20 years) of our first 13 patients treated by adrenalectomy for Cushing'ssyndrome are described. Twelve survived operation and 5 of these died within 5 years, but thereafter the expectation of life of the treated patients was similar to that of the general population. At 20 years 7 patients were alive and 6 were well. If modern therapeutic measures to control adrenocortical function before operation had been available 20 years ago, some of the early deaths might have been prevented.

Analysis of the first six months of intensive dietary management of 57 maturity onset diabetics showed that a large proportion of such patients could be satisfactorily controlled without the need of either oral hypoglycaemic agents or insulin. A dietitian's assessment of the patient's adherence to the prescribed diet allowed groups of good and poor dieters to be selected. Among the poor dieters the plasma insulin and triglyceride levels were significantly increased though plasma glucose levels were not significantly higher. Dietary adherence may thus be an prognostic risk factor in this group of diabetic patients.

Seventeen patients with thyrotoxicosis have been treated with relatively small doses of iodine-131 and their thyrotoxic symptoms have been controlled with propranolol. This use of propranolol, which does not interfere with thyroid-function tests, may allow an earlier and better differentiation of those patients who will respond effectively to a small dose of therapeutic radioactive iodine.

In an initial trial of glibenclamide in the treatment of maturity onset diabetes mellitus 28 patients were treated for up to one year and no toxic effects or side-effects were encountered. The hypoglycaemic potency of this drug is such that 5 mg. of glibenclamide corresponds to about 1,500 mg. of tolbutamide and 375 mg. of chlorpropamide.

Basal plasma levels of insulin, gastrin, secretin, vasoactive intestinal polypeptide and N-terminal and C-terminal glucagon immunoreactivity were measured in 6 fasting hyperprolactinaemic patients following the ingestion of 2.5 mg bromocriptine. No significant changes occurred in the basal plasma levels of these hormones up to 3 hours after bromocriptine ingestion. In a further 10 similar patients studies were carried out on the effect of bromocriptine on the release of these hormones following a standard mixed meal containing 50g carbohydrate, 18g protein and 20g fat with a total energy value of 1.9 megajoules. Plasma hormone levels were measured at 15 minute intervals for 2 hours after ingestion of the meal on 2 occasions—first without bromocriptine and second one hour after 2.5 mg of bromocriptine orally. Following the meal mean plasma glucose levels rose from 5.1±0.2 to 7.6±0.4 mmol/l and from 4.9±0.2 to 7.1±0.4 mmol/l before and after bromocriptine respectively. Mean plasma insulin levels rose from 7.5±0.5 to 84±12 mU/l before bromocriptine but only from 7.0±0.5 to 52.5±12.5 mU/l after bromocriptine. The responses of plasma gastrin, secretin, vasoactive intestinal polypeptide, N-terminal and C-terminal glucagon-like immunoreactivity were not significantly altered by bromocriptine. Seven of the 10 patients were extremely nauseated and 3 vomited just after the study was completed on the day bromocriptine was given.

Summary Two mothers with hypothyroidism gave birth to normal infants. In one patient there was good evidence that maternal hypothyroidism improved during the last trimester due to placental transfer of fetal thyroid hormone. If pregnancy is advanced when the diagnosis of hypothyroidism is made, it would seem that replacement therapy may be withheld without detriment to the fetus.

Summary Phaeochromocytomas were diagnosed in 72 patients in Hammersmith, Belfast and Newcastle between 1955 and 1976. Fourteen were found unexpectedly at autopsy while 58 were diagnosed in life. Associated lesions included neurofibromatosis (7 patients), medullary carcinoma of the thyroid (3), renal artery stenosis (2) and cerebellar haemangioblastoma (1). The clinical features were similar to those reported by others. Hypertension was found in all but one of the patients and was sustained only in 17 per cent, paroxysmal only in 48 per cent and sustained with superadded paroxysms in 33 per cent. Hypertension was significantly greater in the women than in the men. Diagnosis was confirmed chemically in all patients and urinary vanylmandelic acid (VMA) was elevated in all but one. Tumours were localized correctly by various radiographic techniques and by selective venous sampling in over 80 per cent of patients. The tumours were in adrenals in 59 patients (bilateral in 6) and at extra-adrenal, intra-abdominal sites in 13. The tumours were removed surgically from those in whom they were diagnosed in life. Before 1967 3 of 17 patients died after operation (18 per cent). Since then patients have been prepared with alpha blockade and have received infusions of plasma, blood or dextran after removal of the tumours. One of 41 patients so treated has died after operation (2 per cent). Phaeochromocytomas were diagnosed and treated in pregnancy in 4 patients, all of whom survived. Two of the fetuses died. Five tumours diagnosed in life were malignant and 2 recurred (40 per cent). Fifty-three were benign and 2 of these recurred locally (4 per cent). All but 2 patients (who could not be traced) were followed completely from 1 to 20 years after operation and 3 died during this period. Two-thirds of the patients were alive after 10 years. Paroxysmal hypertension was abolished by operation, but sustained hypertension often persisted and required drug therapy. It is concluded that at least one-third of phaeochromocytomas are not diagnosed in life and that a high index of suspicion is required to bring them to light. If they are diagnosed and treated by present methods, the prognosis is good.

Hypothalamic-Pituitary-Adrenal function was studied in 5 longterm survivors (11 to 24 years) who achieved independence of steroid replacement after subtotal adrenalectomy for Cushing's disease. Adrenal function seemed adequate when plasma cortisol values were measured, but the response of the adrenal gland to insulin-induced hypoglycaemia and to tetracosactrin stimulation was subnormal in all 5. It is concluded that the function of the hypothalamic-pituitary-adrenal axis never fully recovers and that adrenal crisis, at times of stress, remains a hazard indefinitely in these patients.

A CASE of malignant insulinoma was treated with streptozotocin intravenously and by coeliac axis infusion. Treatment was repeated over a period of two years, and the total dose was 82.5g. Several clinical remissions of hypoglycaemia were obtained despite the persistence of abnormally elevated plasma insulin levels. At necropsy tumour metastases within the liver were not necrotic. Severe medial degeneration of the lower abdominal aorta was present but the relationship of this change to intra-arterial infusion of the drug could not be established.

Dietary management of maturity-onset diabetes is a basic medical concept. It is difficult to study scientifically because of variable eating habits and the impossibility of rigorous measurement. The non-insulin-requiring diabetic patient is often managed by the family doctor, who may not have access to dietetic assistance, and assessment of the success of simple but careful dietary measures is infrequent. We have taken a special interest in this group of patients, and find that most maturity-onset diabetic patients can be satisfactorily managed on diet only. In a group of 58 such patients there was a significant fall in mean weight and fasting plasma glucose and triglyceride concentrations after six months, which persisted for up to three years. This achievement was chiefly due to those patients who were graded as "good" in their dietary adherence, but less good dieting still achieved weight loss, though plasma glucose and triglyceride concentrations did not fall so low. The clinical dietitian has played a central part in this study.

Two hundred and twenty-three newly-diagnosed symptomatic diabetic patients with onset age 40–69 years enrolled in a prospective study of intensive dietary management of diabetes were observed for a period of six years and the data obtained is analysed. The variables studied were weight and fasting levels of plasma glucose and insulin, and of serum total cholesterol and triglyceride. These tests were monitored throughout the study and in addition the oral glucose tolerance test was analysed at entry to the study, after six months intensive dietary management and again after 72 months. Blood pressure, electrocardiogram and the presence of posterior tibial artery pulsation were recorded at entry to the study and at 36 months and 72 months. Approximately 80 per cent of the patients were managed solely by dietary restriction for the entire six years, but 25 patients received oral hypoglycaemic drugs and 26 required insulin treatment. Weight, and fasting glucose and triglyceride values fell in the first few months of intensive dietary management. Analysis of possible risk factors in survivors and patients dead at six years showed no significant differences, apart from a higher mean age at diagnosis in those who died. During the six years of intensive dietary management the mortality from all causes in these diabetic patients was no greater than that for the general population of Northern Ireland.

Two patients with phaeochromocytomas developed crises in which severe hypertension alternated with profound hypotension. One patient died during an emergency operation. The other was controlled with a long-acting alpha-cholinergic blocking agent and had his tumour removed uneventfully.

Post-irradiation hypothyroidism has developed in about one-third of the patients treated with radioiodine in the past six years. The incidence was higher in patients receiving a higher dose of radioiodine (January 1961–December 1963) as compared to those treated with smaller dose in conjunction with antithyroid drugs (January 1964–December 1966). The period of follow-up of the lower dosage group is short and it is premature to suggest that this altered scheme of therapy will eventually result in a lower incidence of permanent hypothyroidism. The incidence of hypothyroidism was highest in the younger age group (40–49 years) irrespective of the mode of treatment. It is suggested that the thyroid tissue of younger patients is more sensitive to radiation. The relapse rate was higher in the lower dosage group. In both groups, the recurrence rate of hyperthyroidism was higher in patients above 60 years of age. A minor degree of hypocalcaemia was noted in about 20 per cent of patients treated with radioiodine, but none had clinical features of tetany.

A cardiovascular study of a group of 90 newly diagnosed diabetic women aged 35 to 75 years was begun in 1965 and a repeat examination was carried out on the same patients in 1968. A high prevalence of ischaemic heart disease was found in these patients at the time of diagnosis, and this finding had some predictive value as regards prognosis over the three-year period.A comparative study with general medical outpatients and long-established diabetics (greater than 10 years' duration of disease) confirmed the high prevalence of ischaemic heart disease in late-onset mild diabetics controlled by diet or oral drugs. It is suggested that this type of milder diabetic patient contributes in undue proportion to the high prevalence of ischaemic heart disease in diabetes.

The effect of propranolol on radioiodine utilization by the thyroid gland has been studied in 18 patients. All parameters of iodine uptake by the thyroid – initial 5–20 minutes after intravenous administration, 2, 4, 24 and 48 hours after oral administration were unaltered by propranolol administration. In three of four patients studied an increase in isotopically labelled serum PBI was observed after 8 days propranolol therapy.

Propranolol reduced heart-rate in 10 patients with hyperthyroidism by 17.5 beats per minute and in 5 patients with hypothyroidism by 11.4 beats per minute. The heart-rate after propranolol in the two groups was 74.2 and 47.2 beats per minute respectively. The intrinsic heart-rate was greater than expected in 14 of 15 patients with hyperthyroidism and less than expected in 7 patients with hypothyroidism. It was concluded that the differences in heart-rate between patients with hyperthyroidism and hypothyroidism are mainly due to thyroxine acting directly on the heart and not to its potentiation of the action of catecholamines.
