1. The dark side of the torsion: dark energy from propagating torsion

Abstract

Abstract An extension to the Einstein–Cartan (EC) action is discussed in terms of cosmological solutions. The torsion incorporated in the EC Lagrangian is assumed to be totally anti-symmetric, represented by a time-like axial vector $$S^\mu $$ S μ . The dynamics of torsion is invoked by a novel kinetic term. Here we show that this kinetic term gives rise to dark energy, while the quadratic torsion term, emanating from the EC part, represents a stiff fluid that leads to a bouncing cosmology solution. A constraint on the bouncing solution is calculated using cosmological data from different epochs.

2. Quintessential inflation from Lorentzian slow roll

Abstract

Abstract From the assumption that the slow-roll parameter $$\varepsilon $$ ε has a Lorentzian form as a function of the e-fold number N, a successful model of a quintessential inflation is obtained, as succinctly studied in [1]. The form corresponds to the vacuum energy both in the inflationary and in the dark-energy epochs and satisfies the condition to climb from small values of $$\varepsilon $$ ε to 1 at the end of the inflationary epoch. We find the corresponding scalar quintessential inflationary potential with two flat regions. Moreover, a reheating mechanism is suggested with numerical estimation for the homogeneous evolution of the universe. The suggested mechanism is consistent with the BBN bound.

4. Dynamically generated inflation from non-Riemannian volume forms

Abstract

Abstract We propose a simple modified gravity model without any initial matter fields in terms of several alternative non-Riemannian spacetime volume elements within the metric (second order) formalism. We show how the non-Riemannian volume-elements, when passing to the physical Einstein frame, create a canonical scalar field and produce dynamically a non-trivial inflationary-type potential for the latter with a large flat region and a stable low-lying minimum. We study the evolution of the cosmological solutions from the point of view of theory of dynamical systems. The theory predicts the spectral index $$n_s \approx 0.96$$ ns≈0.96 and the tensor-to-scalar ratio $$r \approx 0.002$$ r≈0.002 for 60 e-folds, which is in accordance with the observational data. In the future Euclid and SPHEREx missions or the BICEP3 experiment are expected to provide experimental evidence to test those predictions.

5. Dynamically generated inflationary two-field potential via non-Riemannian volume forms

Abstract

We consider a simple model of modified gravity interacting with a single scalar field φ with weakly coupled exponential potential within the framework of non-Riemannian spacetime volume-form formalism. The specific form of the action is fixed by the requirement of invariance under global Weyl-scale symmetry. Upon passing to the physical Einstein frame we show how the non-Riemannian volume elements create a second canonical scalar field u and dynamically generate a non-trivial two-scalar-field potential Ueff(u,φ) with two remarkable features: (i) it possesses a large flat region for large u describing a slow-roll inflation; (ii) it has a stable low-lying minimum w.r.t. (u,φ) representing the dark energy density in the “late universe”. We study the corresponding two-field slow-roll inflation and show that the pertinent slow-roll inflationary curve φ=φ(u) in the two-field space (u,φ) has a very small curvature, i.e., φ changes very little during the inflationary evolution of u on the flat region of Ueff(u,φ). Explicit expressions are found for the slow-roll parameters which differ from those in the single-field inflationary counterpart. Numerical solutions for the scalar spectral index and the tensor-to-scalar ratio are derived agreeing with the observational data.

Abstract

The exploration of the universe has recently entered a new era thanks to the multimessenger paradigm, characterized by a continuous increase in the quantity and quality of experimental data that is obtained by the detection of the various cosmic messengers (photons, neutrinos, cosmic rays and gravitational waves) from numerous origins. They give us information about their sources in the universe and the properties of the intergalactic medium. Moreover, multi-messenger astronomy opens up the possibility to search for phenomenological signatures of quantum gravity. On the one hand, the most energetic events allow us to test our physical theories at energy regimes which are not directly accessible in accelerators; on the other hand, tiny effects in the propagation of very high energy particles could be amplified by cosmological distances. After decades of merely theoretical investigations, the possibility of obtaining phenomenological indications of Planck-scale effects is a revolutionary step in the quest for a quantum theory of gravity, but it requires cooperation between different communities of physicists (both theoretical and experimental). Abstract

The two-body problem of $M31$ and the Milky Way (MW) galaxies with a Cosmological Constant background is studied, with emphasis on the possibility that they experienced Past Encounter (PE). PE are possible only for non-zero transverse velocity and their viability is subject to observations of the imprints of such near collisions. By implementing the Timing Argument (TA) for two isolated point bodies, it is shown that if $M{31}$ and the MW have had PE, then the predicted mass of the Local Group (LG) should be twice larger. Since the predicted mass is too large, the MW and $M31$ galaxies should have collided at $\sim 8 Gys$. Abstract

Purpose To evaluate a diffractif trifocal toric intraocular lens (IOL) comparated with a trifocal IOL and a monofocal toric IOL. Methods This retrospective comparative study included patients with visually significant cataract or presbyopia who were scheduled for lensectomy and implantation of IOL. They were included in 3 groups according to the IOL implanted: multifocal group, multifocal toric group and toric group. At 1 month postoperatively, visual acuity (VA) at long and near distance, and refractive outcomes were examined. Results 70 eyes of 45 patients were included. 21 eyes in the multifocal group, 22 in the multifocal toric group, and 27 in the toric group. The mean age of patients was 70.7 years +/− 10.6 (range 52–91 years). Postoperatively, the mean refractive astigmatism decreased to 0.71D, 0.77D and 0.72D respectively in the 3 groups. The distance uncorrected VA was 0.13logMar +/− 0.11 for multifocal group, 0.13logMar +/− 0.16 for multifocal toric group and 0.12logMar +/− 0.11 for toric group. There were no statistically differences between the 3 groups for the uncorrected distance VA, and between multifocal group and multifocal toric group for the uncorrected near VA. Conclusions Diffractif trifocal toric IOL provided useful VA at any distance. The addition of a toric surface to the aspheric diffractive multifocal IOL resulted in a comparable visual performance.

Abstract

This paper describes two cases of toxoplasmic chorioretinitis presenting in two French soldiers who had been receiving oral doxycycline for malaria prophylaxis. This is despite the proven effectiveness of oral doxycycline in treating Toxoplasma gondii, the most common cause of this infection. The lack of effectiveness of oral doxycycline in these two cases most likely reflected that the ocular concentration of 100 mg daily doxycycline is too low to treat or prevent Toxoplasmic retinochoroiditis (TC). Clinicians should therefore be aware that soldiers taking prophylactic oral doxycycline are still at risk of developing ocular TC with potentially sight-threatening consequences if not treated adequately.

Purpose Proliferative vitreoretinopathy (PVR) is the leading cause of failure in retinal detachment (RD)surgery. The migration of retinal pigment epithelial cells and the proliferation of the extracellular matrix has been associated with PVR. The purpose of this prospective study was to compare the cellular contents of the vitreous of patients undergoing RD surgery with patients scheduled for epiretinal membrane surgery (ERM). Methods The vitreous samples of patients with ERM and RD were obtained at the initial phase of surgery. A immunocytochemical labeling was performed in all cases with AE1/AE3, CD68, Vimentin and GFAP antibodies. The cytological identification was performed by a trained pathologist unaware of immunocytochemical labeling. The concentration of vitreous cells were analyzed semi-quantitatively. Results 14 vitreous samples were analysed, 9 patients with RD and 5 undergoing ERM. In the RD group, the cytological findings correlated with the results of immunolabeling. Hyalocytes (labeled with vimentin only)were present in all cases, more abundant in RD cases. Macrophages (labeled with vimentin and CD68) were found in 10/14 cases. Abstract

Purpose Vigabatrin is an active treatment for epilepsia in children particularily for West’s syndrome. In children when visual field testing cannot be performed, regular ERG recording is mandatory to screen for retinal toxicity. Some peripheral retinal changes have been described with ophthalmoscopy. The purpose of this study was to evaluate the occurrence of retinal changes wide field imaging and to compare the results with ERG recordings. Methods Five consecutive patients treated with vigabatrin were followed up with Flicker ERG and wide field imaging using scanning laser ophthalmoscopy (Optomap, Optos). Flicker ERG amplitude was considered as abnormal below 50µV. Results Two patients had abnormal ERG recordings and presented with peripheral pigmentary changes. One patient presented with abnormal flicker ERG and normal retinal imaging. Two patients had normal ERG recordings with no peripheral changes on wide field imaging. Conclusion The follow-up with ERG in children treated with vigabatrin is complicated because the recordings are regularly repeated. A follow-up protocol combining electrophysiological recordings and wide field imaging could simplify screening for retinal toxicity but the remains to be demonstrated with a prospective study.

Abstract

Purpose To report the usefulness of wide field imaging and angiography in patients with Coat’s disease. Methods In 5 consecutive patients, wide field imaging using scanning laser ophthalmoscopy and angiography (Optomap) was performed before treatment. The laser was performed on the basis of the wide field images. When the ophthalmologist considered that the treatment was complete, the imaging was repeated. After reviewing the images, the ophthalmologist decided whether further treatment was required. Results Among all patients, wide field imaging enabled to monitor the localization of laser treatment with respect to the vascular abnormalities and non perfusion areas displayed on angiography. In all cases, additional laser treatment was performed on the basis of image lecture although the treatment had been considered to be complete by the ophthalmologist. Conclusion Wide field imaging and wide field angiography enables to target laser treatment in Coat’s disease.

Abstract

Objectif Le diabete est responsable notamment de complications microvasculaires telle que la retinopathie diabetique. Il a ete note une atteinte endotheliale precoce se manifestant au niveau retinien par une diminution de la reponse vasodilatatrice a une stimulation lumineuse flicker et au niveau cutane par une diminution de la vasodilatation a l'acetylcholine. L'objectif etait de caracteriser l'atteinte microvasculaire initiale retinienne et cutanee chez le patient diabetique. Patients et methodes 19 jeunes patients diabetiques de type 1, normotendus et sans signe de retinopathie diabetique et 17 temoins apparies ont ete inclus. Les vaisseaux retiniens ont ete etudies a l'aide du Retinal Vessel Analyser, realisant une mesure dynamique des diametres vasculaires a l'etat basal, apres une stimulation flicker et apres l'administration sublinguale de nitroglycerine (vasodilatation maximale, endothelium-independante). La microcirculation cutanee a ete etudiee par la technique du laser Doppler flowmetrie couplee a l'administration d'acetylcholine par iontophorese et au rechauffement de la peau (vasodilatation endothelium-independante). Resultats A l'etat basal, Les diametres des arterioles et des veinules retiniennes etaient plus eleves chez les diabetiques que chez les temoins. Apres administration de nitroglycerine, alors qu'on observait une vasodilatation arteriolaire et veinulaire chez les temoins, on ne notait pas de reactivite vasculaire chez les diabetiques. Dans chaque groupe, la stimulation par flicker a induit des vasodilatations arteriolaire et veinulaire; cependant, la reponse au flicker etait diminuee chez les diabetiques. Concernant la microcirculation cutanee, nous avons observe une diminution de la reponse a l'acetylcholine et a la chaleur chez les diabetiques par rapport aux controles. Conclusion Chez les jeunes patients diabetiques de type 1 normotendus, une atteinte morphologique et fonctionnelle de la microcirculation precede les signes cliniques de retinopathie diabetique caracterisant la microangiopathie. On a observe ainsi une vasodilatation retinienne a l'etat basal, et aux niveaux retinien et cutane une dysfonction endotheliale et une diminution de la capacite de vasodilatation maximale.

18. The Clinical Significance of Circulating Lymphocytes Morphology in Diffuse Large B-Cell Lymphoma As Determined by a Novel, Highly Sensitive Microscopy.

Abstract

Chimeric Antigen Receptor T-cell (CAR T) therapy has become the preferable treatment in relapsed/refractory diffuse large B-cell lymphomas (DLBCL) patients. Detection of CAR Ts in peripheral blood smear (PBS) is challenging due to insufficient data regarding their morphology and low sensitivity. The morphological evolution of CAR Ts along their production process, and in patients, was established by Full-Field Morphology (FFM), a novel digital microscopy approach that provides highly sensitive PBS analysis. At day 8 of production, 42.7 ± 10.8% of the CAR T transduced cells exhibited activated morphology compared with 9.3 ± 3.8% in untransduced cells. Moreover, engagement of transduced CAR Ts with target cells resulted in further morphological transformation into activated morphology (83 ± 5.6% of the cells). In patients, the average number of day 5 CAR Ts, and their sustained presence, were significantly higher in patients obtaining complete response. A high number of activated morphology CAR Ts at day 14 was associated with prolonged cytokine release storm. Overall, CAR Ts exhibited heterogeneous morphology, with the activated morphology attributed predominantly to transduced cells following engagement with target cells. Post-transfusion CAR T detection was associated with increased complete responses. FFM CAR T surveillance in PBS may serve as a simple inexpensive method to provide clinically relevant insights into this treatment modality.

19. Remote Digital Microscopy Improves Hematology Laboratory Workflow by Reducing Peripheral Blood Smear Analysis Turnaround Time.

Background: The demand for morphological diagnosis by peripheral blood smear (PBS) analysis with clearly defined turnaround times (TAT), coupled with a shortage of morphologists and increasing cost containment, is driving digitalization to the forefront of laboratory workflow. Labor-intensive manual PBS review affects weekend workflow with limited staff availability. The impact of remote analysis of PBS on the performance of hematology laboratories has not yet been assessed., Objectives: Following implementation of fully remote digital microscopy within our laboratory, we measured its impact on morphology workflow efficiency, TAT, and hours saved per month., Methods: A retrospective study of the effects of remote PBS analysis on the morphology workflow in a tertiary medical center using the Scopio Labs X100 Full-Field PBS system was conducted. 10,704 PBS samples were analyzed pre-and post -implementation, over a 5-month period. Overall PBS workload, and average TAT of PBS samples over weekends and the first two weekdays were collected and evaluated., Results: Remote weekend viewing resulted in a 15.8% reduction in the overall morphology TAT of the laboratory ( p <0.03) over a 5-month period, despite similar overall workload. PBS analysis TAT on Fridays was reduced by 41.4% ( p <0. 006), and by 59.1% on the first weekday ( p <0.02). The additional hours incurred over the weekend were offset against a reduced need for double weekday shifts resulting in approximately 12.76 work hours saved per month. Internet links to clinically relevant cases are provided., Conclusion: The Scopio Labs Full-Field X100 PBS system with remote analysis capacity significantly reduced PBS TAT and improved the morphology workflow of the hematology laboratory. PBSs with significant clinical findings are now available for remote viewing by on-call clinicians located outside the medical center perimeter. 20. Comprehensive Morphological Assessment of Cord Blood: Normal Values and the Prevalence of Morphologically Aberrant Leukocytes.

Abstract

Introduction: Cord blood (CB) is becoming a valuable source for stem cells utilized in a variety of cell therapy applications, as well as for newborn diagnostics. Some parameters of the CB cellular components can be provided by automated analyzers, while others, such as immature or aberrant cells, require blood film morphological assessment. The objectives of the study were to establish normal CB morphology and to determine the prevalence of morphologically aberrant leukocytes in CB., Methods: We performed a comprehensive morphological analysis of 100 CB samples taken from healthy term and appropriate-for-gestational-age neonates born to healthy mothers, preterm neonates, neonates of diabetic mothers, and small-for-gestational-age neonates. Blood counts were assessed, and manual morphological analyses were performed by laboratory specialists., Results: The manual differential count of normal CB samples established the following values: 47.8 ± 10.7% neutrophils, 31.2 ± 9.8% lymphocytes, 10.0 ± 4.0% monocytes, and 3.0 ± 2.5% eosinophils, with no significant sex-related differences. Blasts were observed in 44/100 samples with an average of 0.5 ± 0.7% per sample, and only a minor left shift was observed. There were significant populations of large granular lymphocytes (19.1 ± 10.6% of the total lymphocytes) and morphologically aberrant lymphocytes (12.4 ± 5.4% of the total lymphocytes) in the samples, irrespective of neonatal status. The differentials of preterm CB samples differ significantly from normal term CB samples, including the reverse of neutrophils/lymphocytes ratio, and the lack of basophils., Conclusions: Normal values and unique morphological features in the CB of neonates are described. 21. Evaluation of Scopio Labs X100 Full Field PBS: The first high-resolution full field viewing of peripheral blood specimens combined with artificial intelligence-based morphological analysis.

Abstract

Background: Current digital cell imaging systems perform peripheral blood smear (PBS) analysis in limited regions of the PBS and require the support of manual microscopy without achieving full digital microscopy. We report a multicenter study that validated the Scopio Labs X100 Full Field PBS, a novel digital imaging system that utilizes a full field view approach for cell recognition and classification, in a decision support system mode., Methods: We analyzed 335 normal and 310 abnormal PBS from patients with various clinical conditions and compared the performance of Scopio's Full Field PBS as the test method, with manual PBS analysis as the reference method. Deming regression analysis was utilized for comparisons of WBC and platelet estimates. Measurements of WBC and platelet estimation accuracy along with the agreement on RBC morphology evaluation were performed. Reproducibility and repeatability (R&R) of the system were also evaluated., Results: Scopio's Full Field PBS WBC accuracy was evaluated with an efficiency of 96.29%, sensitivity of 87.86%, and specificity of 97.62%. The agreement between the test and reference method for RBC morphology reached 99.77%, and the accuracy for platelet estimation resulted in an efficiency of 94.89%, sensitivity of 90.00%, and specificity of 96.28%, with successful R&R tests. The system enabled a comprehensive review of full field PBS as shown in representative samples., Conclusions: Scopio's Full Field PBS showed a high degree of correlation of all tested parameters with manual microscopy. The novel full field view of specimens facilitates the long-expected disengagement between the digital application and the manual microscope., (© 2021 The Authors. 22. Low FXIII activity levels in intensive care unit hospitalized COVID-19 patients.

Abstract

Background: COVID-19 infection is associated with a hypercoagulable state. Severe COVID-19 patients present with high plasma fibrinogen levels, continuous deposition of fibrin and the presence of microthrombi in their lungs, accompanied by significant fibrinolysis, resulting in high D-dimer levels. Due to the role of FXIII in fibrin crosslinking and clot stabilization, we analyzed its activity levels and dynamics in COVID-19 patients hospitalized in the intensive care unit (ICU)., Methods: FXIII levels were measured in thirty four COVID-19 patients hospitalized in the ICU and in fourteen non-severe COVID-19 patients. FVIII levels were measured for comparison. Laboratory data and clinical variables were recorded., Results: The average FXIII activity level in 34 ICU hospitalized COVID-19 patients was 69.9±33 %, significantly lower compared to an average of 120±20.9 % FXIII activity in 14 non-severe COVID-19 patients. FXIII activity levels were below the low normal value (< 79 % FXIII activity) in 74 % of the ICU hospitalized COVID-19 patients. In contrast, high FVIII activity was measured among all severe COVID-19 patients. Consecutive measurements, performed in fourteen ICU hospitalized COVID-19 patients, pointed to a significant decrease in FXIII activity from the average of 85.7±28.2 %, (which is in the normal range), to an average of 68.0±20.4 %, below the low normal range, within 6.4±3.4 days of ICU hospitalization. Liver functions did not differentiate between patients with low and normal FXIII activity. No inhibitor to FXIII activity was found in the plasma of severe COVID-19 patients. Levels of FXIII-A antigen correlated with FXIII activity, and were low in severe COVID-19 patients., Conclusions: Low FXIII activity levels were found in COVID-19 patients hospitalized in the ICU, with gradual decline during their hospitalization. A mechanism of consumption may account for the low FXIII activity in these patients., (© 2021. The Author(s).)
